RESUMO

El estado inmunitario del organismo es determinante en la evolución de la infección producida por el virus SARS-CoV-2. La inmunosupresión constituye uno de los factores de riesgo que incrementa la reactivación del virus en el organismo, sumado al propio estado de salud del hospedador y factores virológicos como la carga viral y el genotipo del virus (Ye et al. 2020). El Grupo de Investigación Long COVID ACTS sugiere la importante actividad de los anticuerpos en el control de la enfermedad, pues una actividad inmunitaria debilitada y ausencia de respuesta humoral parece aumentar la persistencia del virus en el organismo, con la consecuente subsistencia de la sintomatología dando lugar a una situación de COVID persistente o long COVID, término que se define como “la persistencia o desarrollo de síntomas más allá de las 4 semanas desde el inicio de la enfermedad” (Naeije & Caravita, 2021). El COVID persistente surge con mayor frecuencia en pacientes de edad avanzada, con una o más comorbilidades y un estado inmunitario comprometido. Presentamos un caso de un varón de 72 años diagnosticado de leucemia linfocítica crónica en tratamiento quimioterápico, que dio positivo en la prueba de Reacción en Cadena de la Polimerasa (PCR) con un umbral de ciclos (CT) menor a 20 durante más de 90 días y una sintomatología severa. El caso fue valorado por un equipo multidisciplinar. Se planteó la utilidad de tratamientos como el uso de ciclos repetidos de remdesivir seguido de tratamiento mantenido con emtricitabina/tenofovir disoproxilo. (AU)


The body’s immune system status is decisive in the evolution of the infection caused by the SARS-CoV-2 virus. Immunosuppression along with host health status and virologic factors such as viral load and variant genotype are risk factors that increase the possibility of viral reactivation (Ye et al. 2020). The group Long COVID ACTS suggests the important activity of antibodies to control the disease. A weakened immune system and lack of humoral response appear to increase the persistence of the virus in the body and the consequent persistence of symptoms leading to a persistent or long COVID situation, which is defined as “the persistence or development of symptoms beyond 4 weeks from the onset of the disease” (Naeije & Caravita, 2021). Persistent COVID is more frequent in elderly patients, with comorbidities and immunocompromised status. We present a case of a 72-year-old man diagnosed with chronic lymphocytic leukemia under chemotherapy treatment. COVID-19 polymerase chain reaction (PCR) testing was positive with cycle threshold (CT) values <20 for more than 90 days and severe symptoms. The case was evaluated by a multidisciplinary team. Repeated courses of remdesivir and maintenance treatment with emtricitabine/tenofovir disoproxil fumarate were applied. (AU)

RESUMO

Human intestinal spirochetosis was described by Harland and Lee in 1971, after observing colonization of the apical membrane of the intestinal mucosa by spirochetes. The clinical importance of these findings is not clear, since it is unknown whether the presence of these microorganisms is pathogenic or commensal. The clinical presentation is variable. It can be asymptomatic or manifest with abdominal pain, changes in intestinal rhythm and rectal bleeding. The prevalence of intestinal spirochaetosis is notably higher in developing countries than in developed countries, with the most likely route of transmission being fecal-oral, although sexual transmission has also been suggested as it is more prevalent in homosexual men. We present the case of a 42-year-old man, in treatment for 3 years with Tenofovir, with an HIV-positive partner, who went to the hospital for persistent diarrhea associated with eating a hamburger. BACKGROUND: Tenofovir disoproxil fumarate (TDF) is recommended for the prevention of perinatal transmission of the hepatitis B virus (HBV). This study aimed to systematically assess the efficacy and safety of TDF in pregnant women with chronic HBV and their infants. MATERIAL AND METHODS: Database searches were performed to identify studies blocking the mother-to-child transmission of the hepatitis B virus with tenofovir. The search included pregnant women with chronic HBV infection administered with TDF compared to the no treatment controls, and data from individual studies were pooled using RevMan v5.3 for meta-analysis. RESULTS: Seven studies with a total of 911 patients met the inclusion criteria: 433 patients in the TDF group and 478 patients in the non-TDF group. The HBV mother-to-child transmission rate in the tenofovir group was effectively reduced compared to the control group (RR: 0.18, 95% CI: 0.08-0.40). HBV-DNA positivity was also significantly low in infants from TDF group (RR: 0.17, 95% CI: 0.10-0.30) and the TDF treatment resulted in significantly higher anti-HBs production (RR: 1.11, 95% CI: 1.04-1.18). Similarly, maternal HBV-DNA was suppression was significantly high in the TDF group (RR: 34.16, 95% CI: 16.40-71.13). Women treated with TDF and their infants did not result in serious adverse events that are statistically different as compared to the women who did not receive any treatment. CONCLUSION: Treatment of HBV infected pregnant women with TDF can effectively and safely prevent the perinatal transmission of chronic hepatitis B


ANTECEDENTES: El fumarato de disoproxilo de tenofovir (FDT) se recomienda para la prevención de la transmisión perinatal del virus de la hepatitis B (VHB). El objetivo de este estudio fue evaluar de manera sistemática la eficacia y la seguridad del FDT en las mujeres embarazadas con VHB crónico y en sus hijos lactantes. MATERIAL Y MÉTODOS: Se realizaron búsquedas en las bases de datos para identificar los estudios sobre el bloqueo de la transmisión del virus de la hepatitis B de madre a hijo con el tenofovir. La búsqueda incluyó a mujeres embarazadas con infección crónica por el VHB que recibieron FDT en comparación con controles sin tratamiento, y los datos de los estudios individuales se agruparon mediante el uso de RevMan v5.3 para el metaanálisis. RESULTADOS: Los criterios de inclusión se cumplieron en siete estudios con un total de 911 pacientes: 433 pacientes en el grupo de FDT y 478 pacientes en el grupo sin FDT. La tasa de transmisión de madre a hijo del VHB en el grupo de tenofovir se redujo efectivamente en comparación con el grupo de control (RR: 0,18, IC del 95 %: 0,08-0,40). Los resultados positivos del ADN del VHB también fueron significativamente bajos en los infantes del grupo de FDT (RR: 0,17, IC del 95 %: 0,10-0,30) y el tratamiento con FDT dio como resultado una producción significativamente mayor de anti-HB (RR: 1,11, IC del 95 %: 1,04-1,18). Del mismo modo, la supresión del ADN del VHB materno fue significativamente alta en el grupo del FDT (RR: 34,16, IC del 95 %: 16,40-71,13). El tratamiento de las mujeres con FDT y sus hijos lactantes no provocó acontecimientos adversos graves en ellas ni en sus hijos que fueran estadísticamente diferentes de los de las mujeres que no recibieron ningún tratamiento. CONCLUSIÓN: El tratamiento con FDT de las mujeres embarazadas infectadas por el VHB puede prevenir de manera eficaz y segura la transmisión perinatal de la hepatitis B crónica

OBJETIVO: Conocer y relacionar las tendencias de búsqueda de información sobre Emtricitabina/Tenofovir y las prácticas sexuales de riesgo (chemsex). MÉTODO: Estudio ecológico de los volúmenes de búsqueda relativo (VBR) obtenidos de la consulta directa a la herramienta «Google Trends», utilizando los Temas de búsqueda «Emtricitabina/Tenofovir» y «Chemsex», y «Truvada» como Término de Búsqueda. El periodo de búsqueda fue del 1 de enero de 2004 al 31 de diciembre de 2019. Fecha de consulta 16 de enero de 2020. RESULTADOS: El VBR = 100, máximo, se alcanzó en julio de 2012 para «Emtricitabina/Tenofovir», siendo la relación entre los términos de 100/97/10. La asociación entre «Emtricitabina/Tenofovir» y «Truvada» fue muy alta (R = 0,99; p < 0,001). Y las medias de los VBR para «Emtricitabina/Tenofovir» y «Chemsex» de 25,68 y 14,41 respectivamente. La evolución temporal del término «Emtricitabina/Tenofovir» fue lineal (R2 = 0,62; p < 0,001), mientras que para «Chemsex» se obtuvo creciente con ajuste exponencial (R2 = 0,54; p < 0,001). La correlación existente entre los VBR de los dos términos analizados mostró asociación directa significativa (R = 0,69; p < 0,001). CONCLUSIONES: Los hitos observados en el VBR coincidieron con momentos relacionados con eventos destacados. Se probó un crecimiento, a lo largo del tiempo, en la búsqueda de información tanto sobre los Temas Emtricitabina/Tenofovir como sobre "chemsex". Igualmente, se probó la relación entre ambos Temas. Estas conclusiones podrían estar relacionadas con el aumento de las prácticas sexuales de riesgo


OBJECTIVE: To know and relate the trends of searches for information about «Emtricitabina/Tenofovir» and risky sexual behaviour (chemsex). METHOD: Ecological study of relative search volumes (RSV) obtained from the direct search for the tool «Google Trends», using the Searching Issues «Emtricitabina/Tenofovir» and «Chemsex», and «Truvada» like Searching Term. The searching period was from 1 january 2004 to 31 december 2019. Consultation date 16 january 2020. RESULTS: RSV = 100, maximum, was reached in july 2012 by «Emtricitabina/Tenofovir», being the relationship between terms: 100/97/10. The association between «Emtricitabina/Tenofovir» and «Truvada» was rather high (R = 0.99; p < 0,001). And the averages of RSV of «Emtricitabina/Tenofovir» and «Chemsex» were 25.68 and 14.41 respectively. The temporal progress of the term «Emtricitabina/Tenofovir» was linear (R2 = 0.62; p < 0.001), meanwhile for «Chemsex» was increased with exponential adjustment (R2 = 0.54; p < 0.001). The existent correlation between the RSV of the two analyzed terms showed significant direct association (R = 0.69; p < 0.001). CONCLUSIONS: The observed milestones in the RSV coincide with moments associated with relevant events. A growth was proved along the time, in the search for information for the Themes Emtricitabina/tenofovir as well as for "chemsex". Likewise, the relation between both Themes was proved. These conclusions could be connected with the increase of the risky sexual behaviour

OBJETIVO: Relacionar las tendencias de búsqueda de información sobre Emtricitabina/Tenofovir y la profilaxis de pre-exposición para el virus de la inmunodeficiencia humana (VIH) en España. MÉTODO: Estudio ecológico de los volúmenes de búsqueda relativo (VBR) obtenidos de la consulta directa a la herramienta «Google Trends», utilizando los Temas de búsqueda «Emtricitabina/Tenofovir» y «Profilaxis de pre-exposición para el VIH», y «Truvada» como Término de Búsqueda. El periodo de búsqueda del 1 enero 2004 al 31 diciembre 2019. Fecha de consulta 23 enero 2020. RESULTADOS: El VBR = 100, máximo, se alcanzó en octubre de 2019 para «Profilaxis de pre-exposición para el VIH», siendo la relación entre los términos de 100/17/15. La asociación entre «Emtricitabina/Tenofovir» y «Truvada» fue muy alta (R = 0,99; p < 0,001). Y las medias de los VBR para «Emtricitabina/Tenofovir» y «Profilaxis de pre-exposición para el VIH» de 12,20 y 27,20 respectivamente. Las tendencias del VBR para «Emtricitabina/Tenofovir» fue levemente creciente con ajuste lineal (R2 = 0,05; p < 0,02), para «Profilaxis de pre-exposición para el VIH» fue moderadamente creciente con ajuste exponencial (R2 = 0,20; p < 0,001). La correlación existente entre los datos del VBR de los dos temas analizados no mostró asociación entre ellos (R = 0,04; p = 0,595). CONCLUSIONES: Se probó un crecimiento, a lo largo del tiempo, en la búsqueda de información tanto sobre los Temas Emtricitabina/Tenofovir como sobre Profilaxis de pre-exposición para el VIH, si bien no se pudo probar la existencia de asociación entre ambos Temas. El aumento de la búsqueda de esta información podría estar relacionado con el aumento del interés por la prevención de las prácticas sexuales de riesgo en relación con el VIH

RESULTS: The VBR = 100, máximum, it was achieved in October 2019 for «Pre-exposure prophylaxis for HIV», being the relationship between the terms of 100/17/15. The association between «Emtricitabine/Tenofovir» and «Truvada» was very high (R = 0,99; p < 0,001). And the mean of the VBR for «Emtricitabine/Tenofovir» and «Pre-exposure prophylaxis for HIV» were 12.20 and 27.20 respectively. The VBR tendencies for «Emtricitabine / Tenofovir» was slightly increasing with linear adjustment (R2 = 0.05; p <0.02), for «Pre-exposure prophylaxis for HIV» it was moderately increasing with exponential adjustment (R2 = 0.20; p <0.001). The correlation between the VBR data of the two topics analyzed didn't show an association between them (R = 0.04; p = 0.595). CONCLUSIONS: Growth was found over time in the search of information on both the Emtricitabine/Tenofovir and HIV Pre-Exposure Prophylaxis Topics, although the existence of an association between both Topics could not be proved. The increased search for this information could be related to the increased interest in the prevention of risk sexual practices related to HIV

OBJETIVOS: Evaluar la seguridad y efectividad en vida real del intercambio en el tratamiento antirretroviral de tenofovir disoproxilo (TDF) a tenofovir alafenamida (TAF) en pacientes VIH. MATERIAL Y MÉTODOS: Estudio retrospectivo observacional durante 2017 en el que se incluyeron todos los pacientes del departamento de salud en tratamiento con TAF. Se evaluó la población obtenida y además se dividió en dos subgrupos no comparativos según el intercambio realizado: subgrupo 1, de elvitegravir/cobicistat/TDF/emtricitabina (EVG/c/TDF/FTC) a EVG/c/TAF/FTC; y subgrupo 2, de TDF/FTC a TAF/FTC. Para la seguridad se midió antes y después del intercambio: creatinina (Cr), urea y velocidad de filtrado glomerular (VFG). Para la efectividad carga viral (CV) y linfocitos CD4. La CV se tomó como variable binaria interpretándose como pacientes respondedores (<20 copias/mL) o no. Las variables numéricas se expresaron en medianas y las cualitativas en porcentaje e intervalo intercuartílico (IQR). Las pruebas utilizadas fueron W de Wilcoxon y chi cuadrado respectivamente. RESULTADOS: Se obtuvo una muestra de 55 pacientes en los que hubo una variación antes y después del cambio de Cr=0,9 mg/dL (IQR=0,8-1) a Cr=0,8 mg/dL (IQR=0,7-0,9) (p < 0,001), de urea=29 mg/dL (IQR = 24-36) a urea = 28 mg/dL (25-34,5) (p = 0,06) y de VFG = 92,8 mL/min (79,3-102,1) a VFG = 103,6 (IQR=95,9-111,1) (p < 0,001). En cuanto a efectividad hubo un incremento desde el 51% al 59,9% (p = 0,821) y para CD4 de 668 CD4/mcL (IQR = 496-824) a 684 CD4/mcL (IQR = 611-807) (p = 0,084). Los resultados por subgrupos fueron similares. CONCLUSIONES: Este estudio muestra en una misma población en vida real, que tras el cambio en la sal de tenofovir se mantiene un buen perfil de seguridad con mejora de parámetros analíticos de la función renal sin perjuicio en la efectividad


OBJECTIVE: To assess safety and effectiveness in real world of the switch in the antiretroviral treatment (ART) of tenofovir disoproxil (TDF) to tenofovir alafenamide (TAF) in HIV patients. MATERIAL AND METHODS: Observational retrospective study during 2017 in which all the patients of the health department under treatment with TAF were included. The population obtained was evaluated and also divided into two non-comparative subgroups according to the switch performed: subgroup 1, from elvitegravir/cobicistat/TDF/emtricitabine (EVG/c/TDF/FTC) to EVG/c/TAF/FTC; and subgroup 2, from TDF/FTC to TAF/FTC. To evaluate safety, before and after the switch, was measured: creatinine (Cr), urea and glomerular filtration rate (GFR). To evaluate effectiveness was measured viral load (VL) and CD4 lymphocytes (CD4). VL was interpreted as binary variable: responder patients (<20 copies/mL) or non-responder patients. Numerical variables were expressed as medians (Md) and qualitative variables as percentage and interquartile interval (IQR). The statistical tests used were W of Wilcoxon and chi square respectively. RESULTS: A sample of 55 patients was obtained. There was a variation, before and after the switch, of Cr=0.9 mg/dL (IQR=0.8-1) to Cr = 0.8 mg/dL (IQR=0.7-0.9) (p < 0.001), from urea=29 mg/dL (IQR = 24-36) to urea=28 mg/dL (IQR = 25-34.5) (p = 0.06) and from GFR = 92.8 mL/min (IQR = 79.3-102.1) to GRT=103.6 (IQR = 95.9-111.1) (p < 0.001). In terms of effectiveness there was an increase from 51% to 59.9% (p = 0.821) responding patients and for CD4 from 668 CD4/mcL (IQR = 446-824) to 684 CD4/mcL (IQR = 611-807) (p = 0.058). The results by subgroups were similar. CONCLUSION: This study showed in real world after the change of tenofovir salt within the same population that a good safety profile is manteined with an improvement of analytical parameters of renal function without affecting its effectiveness

Symtuza(R) es el primer y único tratamiento del VIH-1 que combina 2 análogos de nucleós(t)idos (emtricita-bina y tenofovir alafenamida) junto con un inhibidor de la proteasa potenciado (darunavir/cobicistat) en un solo comprimido una vez al día. Esta combinación es activa frente a una gran variedad de cepas del VIH y, a su vez, evita la toxicidad renal y ósea asociada con el uso de tenofovir disoproxil fumarato, y aúna eficacia, conveniencia, tolerabilidad y elevada barrera genética. Los estudios farmacocinéticos de sus componentes demuestran un perfil favorable que permite su uso en una gran variedad de pacientes y situaciones clínicas. Aunque, como en toda combinación potenciada, es necesario considerar posibles interacciones con medicación concomitante, el potenciador cobicistat inhibe más selectivamente el citocromo P-450 y no posee ningún efecto inductor, por lo que su perfil de interacciones es más predecible que el de ritonavir. Symtuza(R) is the first and only treatment for HIV-1 that combines 2 nucleos(t)ide analogues (emtricitabine and tenofovir alafenamide) together with a boosted protease inhibitor (darunavir/cobicistat) in a once-daily single tablet regimen (STR). This combination is active against a wide variety of HIV strains and, in turn, avoids bone and renal toxicity associated with the use of tenofovir disoproxil fumarate, combining efficacy, convenience, tolerability and high genetic barrier. Pharmacokinetic studies of its components show a favourable profile, allowing its use in a wide variety of patients and clinical situations. Although, as in any boosted combination, possible interactions with concomitant medication should be borne in mind, cobici-stat inhibits cytochrome P-450 more selectively and has no inducing effect, so it has a more predictable interaction profile than ritonavir. La evolución del tratamiento antirretroviral (TAR) en los últimos años ha permitido que se disponga de opciones seguras, eficaces a corto y a largo plazo, y también sencillas hasta el punto de que la mayoría de las combinaciones se administran una vez al día, se concentran en uno o 2 comprimidos, y apenas existen fracasos terapéuticos. Pese a todo, continúan existiendo motivos para el cambio de un TAR eficaz en pacientes inmunodeprimidos, cambio que siempre se debe realizar sin perder la eficacia virológica. Con el aumento de la supervivencia en los últimos años y la necesidad de tratamiento de por vida, es importante disponer de pautas simples y seguras para mejorar la calidad de vida de los pacientes con VIH. El ensayo clínico EMERALD demuestra la no inferioridad del cambio a Symtuza(R), combinación de darunavir, cobicistat, em-tricitabina y tenofovir alafenamida en un solo comprimido, en pacientes con supresión viral mantenida. Because of the development of antiretroviral therapy (ART) in the last few years, we now have safe, effective short- and long-term options and also simple regimens, with most combinations being administered once daily in one or 2 tablets and almost no treatment failures. Nevertheless, there are still reasons to switch an effective ART in patients who have been suppressed, a switch that must always be made without losing virological efficacy. With longer survival and the need for lifelong treatment, it is important to have simple and safe treatment options to improve the quality of life in patients with HIV. The EMERALD clinical trial demonstrates the non-inferiority of the switch to Symtuza(R), a combination of darunavir, cobicistat, emtricitabine and tenofovir alafenamide in a single tablet, in patients with sustained viral suppression. La infección por el VIH exige una alta adherencia al tratamiento a largo plazo al tratarse este de un tratamiento de por vida. Darunavir/cobicistat/emtricitabina/tenofovir alafenamida (Symtuza(R)) es la primera tri-terapia con un inhibidor de la proteasa (IP) en un comprimido. Combina la potencia y una alta barrera genética del mejor IP, darunavir, con la seguridad renal y ósea de tenofovir alafenamida. Estudios en fase III han demostrado que esta coformulación no es inferior en alcanzar la supresión virológica y en el mantenimiento de la eficacia en pacientes inmunosuprimidos, incluso con antecedentes de fracaso previo. Aunque son precisos datos a largo plazo, Symtuza(R) se convierte en una opción preferente de uso en la mayoría de los pacientes y, en especial, en pacientes en quienes se sospecha baja adherencia, que necesitan un inicio rápido del tratamiento antirretroviral por presentación tardía o infección oportunista, que tienen limitaciones respecto a otras coformulaciones y en aquellos con antecedentes de fracaso previo o resistencias a otras familias. Because lifelong therapy is needed in HIV infection, high long-term adherence is necessary. Darunavir/co-bicistat/emtricitabina/tenofovir alafenamide (Symtuza(R)) is the first triple therapy combining a protease inhibitor (PI) in a single tablet regimen. This drug combines the potency and high genetic barrier of the most effective PI, darunavir, with the renal and bone safety of tenofovir alafenamide. Phase 3 studies have demonstrated its non-inferiority in achieving virologic suppression and maintaining efficacy in virological-ly suppressed patients, even in those with previous failure. Although long-term data are needed, Symtuza(R) has become a preferred option in most patients. The drug is especially useful in patients with suspected poor adherence, those requiring rapid treatment initiation because of late presentation or opportunistic infection, patients with limitations for other alternatives, and those with a history of previous failure or resistance to other classes of drugs. Introducción: se analizó la efectividad de una estrategia de cribado de la infección por el virus de la hepatitis B (VHB) con consiguiente profilaxis en pacientes con neoplasia hematológica para evitar la reactivación del VHB. Material y métodos: se utilizó un árbol de decisión para comparar costes y eficacia (reactivaciones evitadas), en 18 meses, de una estrategia con cribado previo a quimioterapia con R-CHOP (rituximab, ciclofosfamida, doxorrubicina, vincristina y prednisona) versus una estrategia sin cribado. Los pacientes HBsAg+ (antígeno de superficie de la hepatitis B) y/o antiHBc+ (anticuerpos del núcleo de la hepatitis B) y ADN-VHB+ recibieron profilaxis antiviral con tenofovir disoproxil desde el inicio de la quimioterapia hasta un año después de su finalización. Los pacientes sin cribado recibieron tenofovir en caso de reactivación. Las probabilidades del modelo se obtuvieron de la literatura. El coste total (Euros, 2015) incluyó: profilaxis antiviral, R-CHOP, pruebas de cribado (HBsAg, antiHBc y ADN-VHB) y función hepática. El coste farmacológico se calculó con el precio de venta al laboratorio aplicando la deducción obligatoria. Resultados: en una cohorte hipotética de 1.000 pacientes, el cribado evitó 7,36 reactivaciones frente a la estrategia sin cribado (14,9 versus 22,3). El coste total/paciente (incluyendo 8.282 Euros de coste de R-CHOP) fue de 8.584 Euros para la estrategia con cribado y 8.449 Euros para la estrategia sin cribado. La relación coste-efectividad incremental del cribado versus la estrategia sin cribado fue de 18.376 Euros/reactivación evitada. Conclusión: el cribado de la infección por el VHB permite implementar una profilaxis antiviral en pacientes con alto riesgo de reactivación, disminuyendo la frecuencia de dichas reactivaciones en pacientes con neoplasias hematológicas en tratamiento quimioterapia, con un coste incremental aceptable (AU)


Introduction: The effectiveness of a screening strategy for the detection of a hepatitis B virus (HBV) infection followed by prophylaxis in order to prevent HBV reactivation was assessed in patients with hematologic neoplasms. Material and methods: A decision tree was developed to compare the cost and effectiveness (prevented reactivations) over an 18 month period of a screening strategy prior to chemotherapy with R-CHOP (rituximab, cyclophosphamide, doxorubicin, vincristine and prednisone) with a non-screening approach. HBsAg+ (hepatitis B surface antigen) and/or anti-HBc+ (antibodies to hepatitis B core antigen) and HBV-DNA+ patients received oral antiviral prophylaxis with tenofovir disoproxil (245 mg once daily) from chemotherapy baseline until one year after chemotherapy completion. Non-screened patients received tenofovir in case of a reactivation. Model probabilities were obtained from the literature. The total cost (Euros, 2015) included: antiviral prophylaxis, R-CHOP, screening tests (HBsAg, anti-HBc, HBV-DNA) and liver function tests. Drug therapy costs were estimated using ex-factory prices with mandatory deductions. The incremental cost-effectiveness ratio (ICER) was calculated in order to assess the cost-effectiveness of this intervention in terms of cost per reactivation averted versus no screening. Results: In a hypothetical cohort of 1,000 patients, screening prevented 7.36 reactivations when compared to the non-screening approach (14.9 versus 22.3). Total cost/patient (including Euros 8,282 for R-CHOP) was Euros 8,584 for the screening strategy and Euros 8,449 for the non-screening approach. The ICER for screening versus non-screening was Euros18,376/prevented reactivation. Conclusion: HBV screening followed by oral antiviral prophylaxis yielded more health benefits than non-screening, reducing HBV reactivation in patients with hematologic neoplasms on chemotherapy (AU)

Administration of antiretroviral drugs to individuals exposed to, but not infected by, HIV has been shown to reduce the risk of transmission. The efficacy of pre-exposure prophylaxis (PrEP) makes it obligatory to include it in an integral program of prevention of HIV transmission, together with other measures, such as use of the condom, training, counseling, and appropriate treatment of infected individuals. In this document, the AIDS Study Group (GeSIDA) of the Spanish Society of Infectious Diseases and Clinical Microbiology (Sociedad Española de Enfermedades Infecciosas y Microbiología Clínica [SEIMC]) provides its views on this important subject. The available evidence on the usefulness of PrEP in the prevention of transmission of HIV is presented, and the components that should make up a PrEP program and whose development and implementation are feasible in Spain are set out (AU)


Se ha demostrado que la administración de fármacos antirretrovirales a personas expuestas y no infectadas por el VIH puede reducir el riesgo de transmisión. La eficacia de la profilaxis pre-exposición obliga a considerar su inclusión en un programa integral de prevención de la transmisión del VIH, junto con otras medidas como el uso del preservativo, la formación y el consejo asistido y el tratamiento adecuado de las personas infectadas. En este documento, el Grupo de Estudio de SIDA (GeSIDA) de la SEIMC aporta su visión sobre este importante tema. Se presenta la evidencia disponible acerca de la utilidad de la PrEP en la prevención de la transmisión del VIH y se enumeran los elementos que deberían integrar un programa de PrEP, cuyo desarrollo y puesta en marcha sea factible y viable en nuestro medio (AU)

Objective: To compare the effectiveness and renal safety of treatment with tenofovir versus entecavir in patients with chronic hepatitis-B. Methods: Retrospective study in hepatitis-B patients who initiated treatment with tenofovir or entecavir since January 1998 until 2013. The primary effectiveness variable was defined as viral DNA < 20 UI/ml (HBV-DNA) and the variable for renal safety was variations in glomerular filtration rate (eGFR) after 48 weeks of treatment. Results: The analysis was conducted in 64 patients (1:1), with similar characteristics except for the prevalence of naive patients (p=0.036), comorbidities (p=0.077) and nephrotoxic drugs (p=0.088) in the entecavi arm, while the tenofovir arm presented a prevalence of patients with HBV-DNA < 20 UI/ml (p=0.032) and HBeAg-positive (p=0.050). Statistical univariate analysis and adjustment for confounding variables was conducted through the Propensity Score (PS). The outcomes for the primary effectiveness variable showed tenofovir superiority after PS adjustment, with an ORadj=6.7 (95% CI:1.2-35.3; p=0.028). Three patients on tenofovir experienced seroconversion (p=0.148). The outcomes for the primary safety variable (eGFR < 60 ml/min/1.73m2 ) showed no difference between both arms after adjustment, achieving an ORadj=0.6 (95% CI:0.1-2.8; p=0.521). The tenofovir arm registered two cases of treatment interruption due to renal toxicity, with subsequent recovery, including one Fanconi Syndrome. Conclusions: In our study, there are significant differences between both treatments regarding effectiveness, with tenofovir demonstrating superiority. In terms of renal safety, we have not found any significant differences, but two cases of treatment interruption due to renal toxicity with tenofovir lead us to the conclusion that treatment decision in patients with renal function alteration should include an individualized assessment of each case (AU)


Objetivo: Comparar la efectividad y seguridad renal del tratamiento con tenofovir frente al entecavir en pacientes con hepatitis B crónica. Métodos: Estudio retrospectivo en pacientes con hepatitis B que iniciaron tratamiento con tenofovir o entecavir entre enero 1998-2013. La variable principal de la efectividad fue definida como DNA viral < 20 UI/ml (HBV-DNA) y la de la seguridad renal como variaciones en el filtrado glomerular (eGFR) tras 48 semanas de tratamiento. Resultados: Se analizaron un total de 64 pacientes (1:1), con características semejantes excepto por el predominio de pacientes sin tratamiento previo (p=0,036), comorbilidades (p=0,077) y fármacos nefrotóxicos (p=0,088) en el grupo-entecavir, y de pacientes con HBV-DNA < 20 UI/ml (p=0,032) y HBeAg-positivo (p=0,050) en el grupo-tenofovir. Se realizaron análisis estadísticos univariantes y se ajustaron las variables confusoras mediante Propensity score (PS). Los resultados para la variable principal de efectividad (HBV-DNA < 20 UI/ml) denotan una superioridad del tenofovir tras el ajuste por PS con una ORadj= 6,7 (IC95%: 1,2-35,3; p=0,028). Tres pacientes con tenofovir sufrieron seroconversión (p=0,148). Los resultados para la variable principal de seguridad (eGFR < 60ml/min/1.73m2 ) no mostraron diferencias entre ambas ramas tras el ajuste, obteniendo una ORadj= 0,6 (IC95%: 0,1-2,8; p=0,521). El grupo-tenofovir registró dos casos de suspensión por toxicidad renal, con posterior recuperación, entre ellos un síndrome de Fanconi. Conclusiones: En nuestro estudio existen diferencias significativas entre ambos tratamientos respecto a su efectividad, mostrándose el tenofovir superior. En cuanto a la seguridad renal, no hemos encontrado diferencias significativas, pero dos casos de suspensión de tratamiento por toxicidad renal con tenofovir nos llevan a concluir que la decisión de tratamiento en los pacientes con alteraciones en la función renal debería incluir un análisis individualizado de cada caso (AU)

La hepatitis B crónica es una importante causa de morbilidad y mortalidad en todo el mundo. Aproximadamente un tercio de la población mundial tiene evidencia serológica de infección pasada o presente por el virus de la hepatitis B (VHB) y 350-400 millones de personas son portadores crónicos del antígeno de superficie del VHB. El objetivo del tratamiento es prevenir la aparición de fibrosis hepática y el riesgo de desarrollar cirrosis o hepatocarcinoma mediante la supresión mantenida de la replicación viral. Actualmente existen dos estrategias para el tratamiento de la hepatitis B crónica: el interferón pegilado y el tratamiento a largo plazo con análogos de nucleósidos/nucleótidos. El interferón pegilado tiene la ventaja de ser un tratamiento de duración limitada y está especialmente indicado en aquellos pacientes con hepatitis crónica con HBeAg (antígeno e de la hepatitis B) positivo, pero su perfil de eventos adversos desfavorable y la vía de administración parenteral hacen que sea menos utilizado que los análogos de nucleósidos/nucleótidos. Tenofovir y entecavir han demostrado ser potentes inhibidores del VHB con una alta barrera genética a la resistencia y pocos efectos adversos, por lo que se consideran los fármacos orales de primera línea


Chronic hepatitis B is a major cause of morbidity and mortality worldwide. Approximately one third of the world's population has serological evidence of past or present infection by hepatitis B virus (HBV) and 350- 400 million people are chronic HBV surface antigen carriers. The aim of therapy is to prevent the onset of liver fibrosis and development of cirrhosis or hepatocarcinoma by sustained suppression of viral replication. Currently there are 2 strategies for the treatment of chronic hepatitis B: the pegylated interferon and longterm treatment with nucleoside/nucleotide analogues. Pegylated interferon has the advantage of being a treatment of limited duration, and is particularly suitable for patients with chronic hepatitis with positive HBeAg (hepatitis B e antigen), but the unfavorable adverse event profile and route of parenteral administration makes it less used than nucleoside/nucleotide analogues. Tenofovir and entecavir have shown to be potent inhibitors of HBV with a high genetic barrier to resistance and few adverse effects, so are considered as the first line therapy

Rezolsta® (darunavir/cobicistat) es el primer inhibidor de la proteasa potenciado en combinación fija con un nuevo potenciador farmacológico. Darunavir es actualmente el inhibidor de la proteasa de uso preferente con un perfil de eficacia y seguridad bien conocido. Cobicistat es un inhibidor del citocromo P450 3A sin actividad antirretroviral que se usa como potenciador farmacológico y que puede ser una alternativa al ritonavir. Los parámetros farmacológicos de darunavir con cobicistat son similares a los que se producen cuando el potenciador es ritonavir. Los ensayos clínicos de darunavir/cobicistat han demostrado gran eficacia y buena tolerabilidad. El cobicistat inhibe de forma más específica el citocromo P450 3A sin efecto inductor, por lo que presenta un menor perfil de interacciones farmacológicas que el ritonavir. El cobicistat está también coformulado a dosis fijas con otros fármacos: elvitegravir, tenofovir diproxil fumarato y emtricitabina (Stribild®), elvitegravir, tenofovir alafenamina y emtricitabina (Genvoya®) y con atazanavir (Evotaz®). En este capítulo se revisan los aspectos farmacológicos de la combinación darunavir/cobicistat (Rezolsta®)


Rezolsta® (darunavir/cobicistat) is the first fixed-dose combination tablet with a new boosting agent. Darunavir is currently the preferred protease inhibitor and has a well-known safety and efficacy profile. Cobicistat is a P450 3A cytochrome inhibitor without antiretroviral activity that is used as a boosting agent and can be an alternative to ritonavir. The pharmacological values of darunavir with cobicistat are similar to those produced when the booster is ritonavir. Clinical trials of darunavir/cobicistat have shown high efficacy and good tolerability of the combination. Cobicistat produces more specific inhibition of cytochrome P450 3A induction effect, and consequently has a lower drug interaction profile than ritonavir. Cobicistat is also coformulated in fixed-dose combinations with other drugs: elvitegravir, tenofovir diproxil fumarate and emtricitabine (Stribild®), elvitegravir, tenofovir alafenamine and emtricitabine (Genvoya®) and with atazanavir (Evotaz®). The present article reviews the pharmacological features of the combination darunavir/cobicistat (Rezolsta®)

Rilpivirina es un potente inhibidor no nucleósido de la transcriptasa inversa que ha mostrado gran eficacia en el tratamiento de la infección por el virus de la inmunodeficiencia humana 1 (VIH-1) en pacientes naïve. Rilpivirina es un fármaco activo, tanto frente a cepas salvajes de VIH-1 como frente a una extensa variedad de cepas virales resistentes a los inhibidores no nucleósidos de la transcriptasa inversa de primera generación. Posee un perfil farmacocinético muy favorable, aunque al ser su absorción dependiente del pH gástrico debe ser administrada con comida para asegurar su correcta absorción. Su metabolismo está mediado a través del citocromo P450 (CYP) 3A, por lo que deben considerarse las potenciales interacciones cuando se administre conjuntamente con inductores o inhibidores de esta vía enzimática. Aunque a dosis más altas puede comportarse como inductor enzimático, no es esperable que rilpivirina a la dosis de 25mg al día pueda alterar las concentraciones de otros fármacos metabolizados por esta vía. Su prolongada vida media permite su administración por vía oral 1 vez al día


Rilpivirine is a potent nonnucleoside reverse transcriptase inhibitor (NNRTI) with high efficacy in the treatment of HIV infection in treatment-naïve patients. This drug is active against both wild-type HIV-1 and a wide variety of first-generation NNRTI. Rilpivirine has a highly favorable pharmacokinetics profile, but, because its absorption depends on gastric pH, it should be administered with food to ensure correct absorption. Rilpivirine is metabolized by cytochrome P450 (CYP) 3A and consequently potential interactions should be considered when it is administered with P450 (CYP) 3A inducers or inhibitors. Although higher doses can behave as enzyme inducers, at a dose of 25mg/day, rilpivirine is unlikely to alter the concentrations of other drugs metabolized through this pathway. Because of its prolonged half-life, rilpivirine can be administered orally once daily
