OBJECTIVE: To investigate the factors affecting surgical success rates and duration of operation in retrograde intrarenal surgery (RIRS) without fluoroscopy in children. The aim of the study was to demonstrate the efficacy of RIRS without fluoroscopy on the treatment of renal stones in children. MATERIALS AND METHODS: All RIRS procedures were performed on pediatric patients at our clinic from August 2013 to January 2017. We studied 52 pediatric patients who had one stone in one kidney and under- went one session. We mapped the kidney collecting system anatomically, and stone localization was defined according to this mapping. Size and localization of the stone, placement of preoperative J stent, use of ureteral access sheath (UAS), and surgical success rates were recorded. The effects of these factors on surgical success rates and the duration of the operations were analyzed. RESULTS: Each patient underwent RIRS once. Of these 52 pediatric patients, 23 (44%) were between 0-5 years of age children (Group 1), 13 (25%) were between 6-11 years of age children (Group 2), and 16 (31%) were between 11-17 years of age children (Group 3). The surgical success rates for each group were 65%, 77%, and 81%, respectively (73% overall). The surgical success rates were found to be affected only by stone size (p < 0.001). The durations of the operations were found to be affected by stone size, stone localization, passive dilatation of ureter, and the application of an UAS (p < 0.05). CONCLUSION: RIRS is a safe and effective method for the treatment of intrarenal stones in pediatric patients. High success rates can be achieved using kidney mapping without the use of fluoroscopy


OBJETIVO: Investigar los factores que afectan el éxito quirúrgico y la duración de la cirugía retrógrada intrarenal sin fluoroscopia en niños El objetivo de este estudio fue demostrar la eficacia de la cirugía retrógrada intrarenal sin fluorosocopia en el tratamiento de litiasis en niños. MÉTODOS: Todos los procedimientos de cirugía retrógrada intrarenal fueron realizados en pacientes pediátricos en nuestra clínica entre agosto 2013 y enero 2017. Estudiamos 52 casos pediátricos con 1 litiasis en 1 riñón y recibieron una sesión. Marcamos el sistema colector renal y la litiasis se definió según ese marcaje. El tamaño y la localización de la piedra, colocación preoperatoria del doble J, la vaina de acceso y la tasa de éxito quirúrgico fueron reportadas. Los efectos de estos factores en el éxito quirúrgico y la duración de las cirugías fueron analizados. RESULTADOS: Cada paciente recibió cirugía retrógrada una vez. De éstos 52 pacientes pediátricos, 23 (44%) tenían entre 0 y 5 años (grupo 1), 12 (25%) entre 6 y 11 años (grupo 2), 16 (31%) entre 11 y 17 años (Grupo 3). El éxito quirúrgico en cada grupo fue de 65%, 77% y 81% respectivamente (73% en general). El éxito quirúrgico se vió afectado sólo por el tamaño de la litiasis (p < 0,001). La duración de las cirugías se vió afectada por el tamaño de la litiasis, localización de la litiasis, dilatación pasiva del uréter, y uso de la vaina de acceso (p < 0,05). CONCLUSIÓN: La cirugía intrarenal retrógrada es segura y efectiva en el tratamiento de litiasis intrarenales en el paciente pediátrico. Altas tasas de éxito se consiguen con el mapeo renal sin fluoroscopia

Introduction: Food and dietetic components have received considerable attention as auxiliary feeding resources on controlling of chronic non-transmissible diseases, among them diabetes. This study evaluated the effect of supplementary diet with multimixture based on linseed, sesame, oats and sunflower seeds on the hepatic function and antioxidant activity of diabetic rats. Methods: Male rats were distributed on groups of seven animals: diabetic control (DC), diabetics subject to multimixture diet (DM), diabetic with insulin (DI), and normal control (NC). The treatment was started on the 5th day after diabetes induction with 40 mg/kg i.v. streptozotocin on tampon citrate and kept during 50 days. Results: The ethereal extract of the supplemented diet showed a higher content of phenolics (p < 0.05) compared to other extracts of the supplemented diet and the ethereal extract of the standard diet. There was no difference in antioxidant in vitro activity of the utilized diets. Concerning to transaminases, signifi cant bigger ALT levels (p < 0.05) were present on diabetic groups compared to NC. The relative liver weight on diabetic groups was significantly higher (p < 0.001) compared to NC group. Non-proteic sulfhydryl group levels were significantly higher (p < 0.05) on DM and I groups when compared to DM and NC. Only the treatment with insulin resulted in an improvement of antioxidant activity concerning to hepatic catalase. The supplementation with multimixture did not improve the metabolic control of diabetes. Conclusion: The multimixture treatment showed an isolated improvement on antioxidant activity in the hepatic tissue, evidenced by the increasing on non-proteic sulfhydryl group levels (AU)


Introducción: alimentos y componentes de la dieta han recibido considerable atención como recursos auxiliares en el control de las enfermedades crónicas, como la diabetes. Este estudio evaluó el efecto de la dieta suplementada con base de linaza multimezcla, sésamo, avena y semillas de girasol en la función hepática y la actividad antioxidante de ratas diabéticas. Métodos: las ratas macho fueron divididas en grupos de siete animales: control de la diabetes (CD), diabéticos sometidos a dieta multimezcla (DM), diabéticos con insulina (DI), y control normal (CN). El tratamiento se inició en el quinto día después de la inducción de la diabetes con estreptozotocina 40 mg/kg i.v. en tampón citrato y se mantuvo durante 50 días. Resultados: la dieta extracto etéreo complementado tenía el contenido fenólico más alto (p < 0,05) que los otros extractos de la suplementación de la dieta y el extracto de éter de la comida estándar. No hubo diferencia en la actividad antioxidante in vitro de la alimentación usada. A medida que las transaminasas, los niveles de ALT significativamente mayor (p < 0,05) estuvieron presentes en el grupo de diabéticos en comparación con CN, el peso relativo del hígado en el grupo de diabéticos fue significativamente mayor (p < 0,001) en comparación con el grupo CN. Los grupos sulfhidrilo de los niveles de proteína no fueron significativamente mayores (p < 0,05) en los grupos F e I comparados con CN y CD. El tratamiento con insulina resultó en una mejora de la actividad antioxidante con respecto a la catalasa hepática. Conclusión: la suplementación con multimezcla no mejora el control metabólico de la diabetes. El tratamiento con multimezcla aislado mostró una mejora en la actividad antioxidante en el tejido hepático, como se evidencia por el aumento de los niveles de grupos sulfhidrilo no proteicos (AU)

Hearts from subjects with different ages have different Ca2+ signaling. Release of Ca2+ from intracellular stores in response to an action potential initiates cardiac contraction. Both depolarization-stimulated and spontaneous Ca2+ releases, Ca2+ transients and Ca2+ sparks, demonstrate the main events of excitation–contraction coupling (ECC). Global increase in free Ca2+concentration ([Ca2+] i ) consists of summation of Ca2+ release events in cardiomyocytes. Since the Ca2+ flux induced by Ca2+ sparks reports a summation of ryanodine-sensitive Ca2+ release channels (RyR2s)’s behavior in a spark cluster, evaluation of the properties of Ca2+ sparks and Ca2+ transients may provide insight into the role of RyR2s on altered heart function between 3-month-old (young adult) and 6-month-old (mature adult) rats. Basal [Ca2+] i and Ca2+ sparks frequency were significantly higher in mature adult rats compared to those of young adults. Moreover, amplitudes of Ca2+ sparks and Ca2+ transients were significantly smaller in mature adults than those of young adults with longer time courses. A smaller L-type Ca2+ current density and decreased SR Ca2+ load was observed in mature adult rats. In addition, RyR2s were markedly hyperphosphorylated, and phosphorylation levels of PKA and CaMKII were higher in heart from mature adults compared to those of young adults, whereas their SERCA protein levels were similar. Our data demonstrate that hearts from rats with different ages have different Ca2+ signaling including hyperphosphorylation of RyR2s and higher basal [Ca2+] i together with increased oxidized protein-thiols in mature adult rats compared to those of young adults, which play important roles in ECC. Finally, we report that ECC efficiency changes with age during maturation, partially related with an increased cellular oxidation level leading to reduced free protein-thiols in cardiomyocytes (AU)

Durante el último congreso Digestive Disease Week (DDW) se presentaron numerosas comunicaciones en relación con la enfermedad de Crohn (EC). Varios estudios han explorado nuevas vías patogénicas. La inhibición de los linfocitos T producida por el abatacept no se mostró útil. Otros fármacos como la naltrexona (antagonista opiáceo) o el traficet- EN (antagonista de una molécula específica del tráfico de leucocitos) sí que se mostraron eficaces. Otra estrategia, modificar fármacos existentes como la mercaptpurina (DR6- MP), para disminuir su toxicidad podría ser útil en un futuro en pacientes con EC. En la prevención de la recurrencia se presentaron varios estudios: el VSL-3 no pudo demostrar su utilidad, pero tanto adalimumab como infliximab parece que son muy eficaces en esta circunstancia. Un objetivo terapéutico, la curación mucosa, fue motivo de varias comunicaciones. Este objetivo es más fácilmente alcanzable cuando se trata a los pacientes de forma continua con biológicos y tiene valor pronóstico de buena evolución. En los pacientes tratados con infliximab los valores valle se correlacionan con la curación mucosa. En caso de fracaso de un biológico se puede sustituir por infliximab, adalimumab, certolizumab o natalizumab. En caso de remisión con infliximab, el cambio por adalimumab no parece adecuado(AU)


Numerous communications were presented on Crohn's disease (CD) in Digestive Disease Week 2010. Several studies explored new pathogenic pathways. T lymphocyte inhibition by Abatacept was not demonstrated to be useful. Other drugs such as naltrexone (an opiate antagonist) and Traficet-EN (which targets the small-intestinal-specific chemokine receptor CCR9) have been shown to be effective. Another strategy, modifying already existing drugs such as mercaptopurine (DR6-MP), to reduce their toxicity could prove useful in future in patients with CD. Several studies on preventing recurrence were presented: VSL-3 failed to demonstrate efficacy but both adalimumab and infliximab seem to be highly effective in achieving this goal. One therapeutic objective —mucosal healing— was the subject of several communications. This aim is easier to achieve when patients are treated continuously with biological agents and has prognostic value in predicting favorable outcome. In infliximab-treated patients, trough levels correlate with mucosal healing. Patients unresponsive to biological agents can be switched to infliximab, adalimumab, certolizumab or natalizumab. In patients achieving remission with infliximab, substituting adalimumab does not seem appropriate(AU)

El mal aliento, mal olor de boca o halitosis, son términos que se utilizan para describir un olor ofensivo que emana de la cavidad oral, independientemente de que las sustancias de olor desagradable provengan de fuentes orales o no orales. La mayoría de las halitosis se originan en el interior de la boca. Se han atribuido principalmente a compuestos volátiles sulfurados (CVSs) tales como sulfídrico, metil mercaptano y dimetil sulfuro. La causa primaria es la existencia de bacterias gramnegativas, que son similares a las que causan las enfermedades periodontales. Estas bacterias producen CVSs a partir del metabolismo de distintas células epiteliales, leucocitos etc, localizadas en la saliva y en la placa dental principalmente. La superficie lingual está cubierta de una gran cantidad de células epiteliales descamadas y bacterias, que pueden, a traves de su actividad proteolítica y capacidad de putrefación, producir CVSs. El objetivo de este artículo de revisión es: 1°) analizar la importancia de la halitosis oral en el contexto actual, 2°) estudiar los datos sobre su etiología y 3°) valorar la evidencia de la asociación entre las enfermedades periodontales y la halitosis de origen oral (AU)


Bad breath, oral malodour and halitosis they are terms used describe unpleasant odour out from the oral cavity, independently from the source where the odorous can be oral no oral. The majority of bad breath originates within the mouth. They have been attributed mainly attributed to volatile sulfur compounds (VSC) such as hydrogen sulfide, methyl mercaptan and dimethyl sulfide. The primary cause are bacteria gram-negative bacteria that are similar to the causing periodontal diseases. These bacterias produce the VSC by metabolizing epithelial cells, leukocytes, etc., mainly located in saliva, dental plaque. Tongue surface is composed of desquamated epithelial cells and bacterias, suggestig that it has the proteolytic and putrefactive capacity to produce VSC. The aim of this review article: (1) to analyze the evidence the importance of the bad breath in the current context, (2) to study the data your etiology and (3) to value the association between the periodontal diseases and the halitosis of oral origin (AU)

La halitosis es un problema frecuente en la población. Puede ser el reflejo de una enfermedad local o de una disfunción orgánica que, por mecanismos diversos, da lugar a la aparición de sustancias odoríferas en el aire espirado. En algunos casos, el aliento puede contener marcadores clínicamente útiles que conducen al diagnóstico de una enfermedad no detectada previamente. Los compuestos volátiles sulfurados especialmente el metil mercaptano y el dimetil sulfuro son los principales agentes Implicados en el mal aliento La realización de una historia clínica minuciosa es el punto de arranque para hacer un diagnóstico ccrrecto. Se plantean en este trabajo de revisión los criterios diagnósticos así como las diferentes medidas terapéuticas para el control y manejo de la halitosis El método ha sido analizar de forma critica los estudios publicados en la bibliografía médica, utilizando el Medline (AU)
