Objectives To examine the relation of corticotropin-releasing hormone (CRH) family peptides with inflammatory processes and oncogenesis, emphasizing in vulvar inflammatory, premalignant and malignant lesions, as well as to investigate the possibility of lesion cells immunoescaping, utilizing FAS/FAS-L complex. Methods Immunohistochemical expression of CRH, urocortin (UCN), FasL and their receptors CRHR1, CRHR2 and Fas was studied in vulvar tissue sections obtained from patients with histologically confirmed diagnosis of lichen, vulvar intraepithelial neoplasia (VIN) and vulvar squamous cell carcinoma (VSCC). The patient cohort was selected from a tertiary teaching Hospital in Greece, between 2005 and 2015. For each of the disease categories, immunohistochemical staining was evaluated and the results were statistically compared. Results A progressive increase of the cytoplasmic immunohistochemical expression of CRH and UCN, from precancerous lesions to VSCC was observed. A similar increase was detected for Fas and FasL expression. Nuclear localization of UCN was demonstrated in both premalignant and VSCC lesions, with staining being significantly intensified in carcinomas, particularly in the less differentiated tumor areas or in the areas at invasive tumor front. Conclusions Stress response system and CRH family peptides seem to have a role in inflammation maintenance and progression of vulvar premalignant lesions to malignancy. It seems that stress peptides may locally modulate the stroma through Fas/FasL upregulation, possibly contributing to vulvar cancer development (AU)

INTRODUCCIÓN: El síndrome de West (SW) es una encefalopatía epiléptica dependiente de la edad con pronóstico variable según la etiología subyacente, no siempre identificada. OBJETIVOS: Definir el perfil del SW criptogénico en nuestro medio, subgrupo menos estudiado de forma aislada. Estudiar su evolución, respuesta a los distintos tratamientos y establecer factores pronósticos. PACIENTES Y MÉTODOS: Revisión de historias clínicas de 16 pacientes diagnosticados de SW criptogénico durante el período 2000-2015. El tiempo de seguimiento medio fue 6,6 años y mínimo de 2 años. RESULTADOS: 11 de 16 fueron varones, la edad media de inicio fue de 6 meses y 6/16 presentaban antecedente familiar de epilepsia idiopática. El tratamiento de primera línea con vigabatrina tuvo respuesta electroclínica en 5/16 pacientes, respondiendo los casos restantes a hormona adrenocorticotropa (ACTH). El 44% de los pacientes evolucionaron a otras epilepsias, sin diferencia entre los tratados con vigabatrina o ACTH. Se obtuvo un mayor número de efectos adversos con la ACTH, no se evidenció afectación retiniana con la vigabatrina. Durante el seguimiento se llegó a la causa etiológica en 2/16. El sexo femenino, el comienzo tardío y el control precoz de la hipsarritmia resultaron factores de buen pronóstico. CONCLUSIONES: El pronóstico global del SW criptogénico resultó más grave de los esperado. Aunque la incidencia de síndrome de Lennox-Gastaut fue baja, la epilepsia focal resultó la evolución más frecuente apareciendo en los 2 primeros años del diagnóstico. La respuesta inicial a vigabatrina fue menor a la esperada, pero el resultado a largo plazo resultó superponible a la ACTH


INTRODUCTION: West syndrome (WS) is an age-dependent epileptic encephalopathy in which the prognosis varies according to the, not always identified, underlying origin. OBJECTIVES: To define the profile of cryptogenic (a least studied isolated sub-group) WS, in Spain. To study its outcome, response to different treatments, and to establish prognostic factors. PATIENTS AND METHODS: The study included a review of the medical records of 16 patients diagnosed with cryptogenic WS during the period, 2000-2015. The mean follow-up time was 6.6 years, with a minimum of 2 years. RESULTS: The large majority (11/16) were male. The mean age at onset was 6 months, and 6/16 had a family history of idiopathic epilepsy. The first line treatment with vigabatrin had an electrical-clinical response in 5/16 patients, with the remaining cases responding to adrenocorticotropic hormone (ACTH). Almost half (44%) of the patients progressed to other types of epilepsy, with no difference between those treated with vigabatrin or ACTH. A greater number of adverse effects were obtained with ACTH, with no retinal involvement being observed with vigabatrin. The aetiological cause was found in 2/16. Being female, late onset, and early control of the hypsarrhythmia, were factors of a good prognosis. CONCLUSIONS: The overall prognosis of cryptogenic WS was more serious than expected. Although the incidence of Lennox-Gastaut syndrome was low, the progression to focal epilepsy was the most common, with it appearing within the first 2 years of the diagnosis. The initial response to vigabatrin was lower than expected, but the long-term result was comparable to ACTH

Introducción. La refractariedad es una característica común del tratamiento de los síndromes de Ohtahara, Aicardi, West, Dravet y epilepsia maligna del lactante con crisis migrantes. Objetivo. Actualizar el manejo terapéutico y analizar si la etiología determina de alguna manera el tratamiento. Desarrollo. Las crisis convulsivas en el primer año de vida pueden deberse a una etiología potencialmente tratable, por lo que es imperativo una completa evaluación para instaurar de manera precoz el tratamiento adecuado y el sintomático no específico para el control de las crisis, que evite o minimice el efecto deletéreo de éstas. Es obligado hasta descartar metabolopatía y ensayar pauta de vitaminas y cofactores, además de antiepilépticos. En los síndromes de Ohtahara y Aicardi, la primera línea es fenobarbital y fenitoína, y en segunda línea, los más habituales son midazolam, levetiracetam, lidocaína y valproato. En el síndrome de West, la primera línea la constituye la hormona adrenocorticotropa y la vigabatrina para el caso de esclerosis tuberosa; si no hay respuesta, considerar otros fármacos, dieta cetogénica y cirugía. Para el síndrome de Dravet, los principales son valproato con clobazam y estiripentol, y de segunda línea, considerar otros fármacos y dieta cetogénica. En la epilepsia con crisis migrantes, los más eficaces son bromuros, estiripentol, clonacepam y levetiracetam. Conclusiones. Actualmente existe poco consenso en el abordaje terapéutico para establecer indicaciones taxativas. La etiología influye en el tratamiento, tanto en el caso de disponer de tratamiento curativo (metabolopatías) como en el abordaje sintomático con antiepilépticos u otros tratamientos (dieta cetogénica o cirugía) (AU)


Introduction. Resistance to treatments is a common feature of Ohtahara, Aicardi, West and Dravet syndromes, as well as malignant migrating epilepsy in infancy. Aims. To update the therapeutic management and to analyse whether the aetiology somehow determines the treatment. Development. Convulsive seizures in the first year of life may be due to a potentially treatable aetiology, which makes it essential to carry out a complete evaluation so as to be able to begin, as early as possible, the most suitable and the non-specific symptomatic treatments to control the seizures, which prevents or minimises their deleterious effects. Metabolic disease must be ruled out and it is also essential to try a therapeutic regimen of vitamins and cofactors, as well as antiepileptic drugs. In Ohtahara and Aicardi syndromes, the first-order treatment is phenobarbital and phenytoin, and the most commonly used second-order drugs are midazolam, levetiracetam, lidocaine and valproate. In West’s syndrome, the first-order treatment consists of adrenocorticotropic hormone and vigabatrine for the case of tuberous sclerosis; if there is no response, other pharmaceuticals, a ketogenic diet and surgery must be considered. For Dravet’s syndrome, the main treatment consists in valproate with clobazam and stiripentol, and as the second order, other drugs and a ketogenic diet should be considered. In epilepsy with migrating seizures, the most effective treatment is with bromides, stiripentol, clonazepam and levetiracetam. Conclusions. Today there is little consensus on the therapeutic approach to be able to establish specific indications. The aetiology has an influence on the treatment, both in cases in which a curative treatment exists (metabolic diseases) and in the symptomatic management with antiepileptic drugs or other treatments (ketogenic diet or surgery) (AU)

In response to stress, the hypothalamus releases cortiticotropin releasing hormone (CRH) that travels to the anterior pituitary, where it stimulates the release of adrenocorticotropic hormone (ACTH). ACTH travels to the adrenal cortex, where it stimulates the release of cortisol and other steroids that liberate energy stores to cope with the stress. During pregnancy, the placenta synthesises CRH and releases it into the bloodstream at increasing levels to reach concentrations 1,000 to 10, 000 times of that found in the non-pregnant individual. Urocortins, which are CRH analogues are also secreted by the placenta. Desensitisation of the maternal pituitary to CRH and resetting after birth may be a factor in post-partum depression. Recently, CRH has been found to modulate glucose transporter (GLUT) proteins in placental tissue, and therefore there may be a link between CRH levels and foetal growth. Evidence suggests CRH is involved in the timing of birth by modulating signalling systems that control the contractile properties of the myometrium. In the placenta, cortisol stimulates CRH synthesis via activation of nuclear factor kappa B (NF-êB), a component in a cellular messenger system that may also be triggered by stressors such as hypoxia and infection, indicating that intrauterine stress could bring forward childbirth and cause low birth weight infants. Such infants could suffer health issues into their adult life as a result of foetal programming. Future treatment of these problems with CRH antagonists is an exciting possibility (AU)

We start with this article the publication of a series of cases in pediatric endocrinology that are a frequent consultation in primary care. Although most of them have to be referred to specialist care, it is interesting to know the final diagnosis and the treatment. This first case is a 6 year old girl with pubarche. Objetivo. Evaluar la estabilidad de las concentraciones de cortisol y 25-hidroxivitamina D (25OH-vit D) en suero y de corticotropina (ACTH) en plasma en función del tiempo de almacenamiento en las condiciones habituales de trabajo para estas muestras y comparar dos protocolos diferentes de centrifugación. Material y método. Se obtuvieron muestras de 25 voluntarios sanos (5 hombres y 20 mujeres) para el estudio de cortisol y 25OH-vit D. Para el estudio de ACTH se utilizaron muestras de 15 de estos voluntarios sanos. Las muestras se procesaron en el analizador Liaison® DiaSorin en tiempos comprendidos entre 90 minutos y 7 días desde la toma de la muestra hasta la obtención del resultado. Resultados. El cortisol y 25OH-vit D se mantuvieron estables a lo largo de todo el estudio en las condiciones especificadas. La ACTH se mantuvo estable hasta 8 horas a 4°C. En ninguna de las tres magnitudes se observaron diferencias significativas entre los resultados obtenidos en muestras centrifugadas tras su obtención o inmediatamente antes de su procesamiento hasta 6 horas después de la obtención de la muestra. Conclusiones. Las condiciones habituales de manejo de las muestras, 8 horas a temperatura ambiente con o sin centrifugación previa y posteriormente almacenadas a 4°C durante una semana, no comprometen la estabilidad en el caso de cortisol y 25OH-vit D. En el caso de ACTH es preciso congelar las muestras tras su obtención si no van a ser procesadas en un tiempo inferior a 8 horas (AU)


Objective. To asses the stability of cortisol, and 25 (OH)-Vitamin D concentrations in serum, and adrenocorticotrophic hormone (ACTH) in plasma samples, according to the storage time under normal working conditions for these samples, and to compare two different centrifugation protocols. Material and method. The samples were obtained from 25 healthy volunteers (5 men and 20 women) for the study of cortisol and 25 OH-vit D. For the ACTH only 15 of the 25 samples were used. The samples were processed in the Liaison® DiaSorin analyser at times between 90minutes and 7 days from the time the sample was taken until the result was obtained. Results. Cortisol and 25 OH-Vit D remained stable during the time of the study under the specified conditions. ACTH remained stable only until the 8th hour at 4° C. There were no significant differences in relation to the time of centrifugation. Conclusions. The regular management conditions of the samples, 8hours at room temperature with or without previous centrifugation and later storage at 4° C for a week does not jeopardize the stability of cortisol and 25 OH-vit D. In the case of ACTH the samples have to be frozen after they are obtained if they are not going to be processed in less than 8 hours (AU)

Una función endocrina normal es esencial para la salud cardiovascular. Los trastornos del sistema endocrino, consistentes en hiperfunción o hipofunción hormonales, tienen múltiples efectos en el sistema cardiovascular. En esta revisión se comentan la epidemiología, el diagnóstico y el tratamiento de los trastornos de las glándulas hipófisis, tiroides, paratiroides y suprarrenales en lo relativo a sus repercusiones de disfunción endocrina en el sistema cardiovascular. Se revisan también los efectos beneficiosos cardiovasculares que aporta el restablecimiento de una función endocrina normal(AU)


Normal endocrine function is essential for cardiovascular health. Disorders of the endocrine system, consisting of hormone hyperfunction and hypofunction, have multiple effects on the cardiovascular system. In this review, we discuss the epidemiology, diagnosis, and management of disorders of the pituitary, thyroid, parathyroid, and adrenal glands, with respect to the impact of endocrine dysfunction on the cardiovascular system. We also review the cardiovascular benefits of restoring normal endocrine function(AU)

La leptina es una proteína clave en la regulación de la ingestiónde alimentos, junto con determinadas zonas hipotalámicas,conocidas desde hace mucho tiempo por estar relacionadascon el hambre y la saciedad. RESUMO

Objetivo. Revisar la terapéutica farmacológica antidepresiva presente y futura basada en los avances de la biotecnología. Desarrollo. La visión simplista y errónea de un único sistema neurotransmisor alterado en la depresión y, por tanto, un único tratamiento, ha cambiado. Las técnicas de la biología molecular y la genética han permitido conocer otras posibles alteraciones químicas en el cerebro, más allá de la única participación de los sistemas moduladores monoaminérgicos, la hipótesis clásica. Se describen las evidencias acerca de las relaciones entre la depresión y el efecto terapéutico de los antidepresivos clásicos con: a) El sistema peptidérgico de la hormona liberadora de la corticotropina, el cortisol y el estado funcional de sus receptores; b) Los sistemas de señalización intracelular como el AMPc sobre factores de transcripción como CREB y las neurotrofinas; c) El sistema inmune y las citocinas; d) La transmisión glutamatérgica, y e) El sistema neuropeptidérgico de la sustancia P, los esteroides neuroactivos y la glía. Esto ha permitido la emergencia de otras hipótesis bioquímicas sobre la depresión y la posibilidad de nuevos tratamientos. Conclusiones. Aunque hoy se sigue sin conocer con certeza la causa exacta ni los procesos que determinan las enfermedades mentales como la depresión, ni cómo se produce la mejoría con los antidepresivos disponibles, hay una gran esperanza de que, con la ayuda de la biotecnología, se avanzará hacia un mejor conocimiento de las interrelaciones entre los sistemas nervioso, inmune y endocrino, con sus cascadas intracelulares y sus consecuencias finales en la expresión genética y la función proteica en la depresión; ello permitirá desarrollar fármacos más efectivos, rápidos y selectivos que, en el futuro, con la ayuda de la psicogenómica, podrán incluso diseñarse para cada paciente (AU)


Aims. The objective of this study is to survey present and future antidepressant drug therapy based on the progress made in the field of biotechnology. Development. The simplistic and mistaken view that one single system of neurotransmission is altered in depression and that there is, therefore, just one single treatment has changed. Molecular biology and Genetics have enabled us to determine other possible chemical alterations in the brain, beyond the sole participation of the monoaminergic modulation systems, which is the classical hypothesis. In this paper we describe the evidence for the relations between depression and the therapeutic effect the classical antidepressants have on: 1. The peptidergic system of the corticotropin-releasing hormone, cortisol and the functional state of its receptors; 2. Intracellular signalling systems such as cAMP on transcription factors like CREB and neurotrophins; 3. The immune system and cytosines; 4. Glutamate transmission; and 5. The neuropeptidergic system of substance P, neuroactive steroids and the neuroglia. This has allowed other biochemical hypotheses about depression and the possibility of new treatments to be put forward. Conclusions. We are still not certain about the exact cause or the processes that determine mental illnesses such as depression or how improvements are achieved with the antidepressants we currently have available. Nevertheless, biotechnology is expected to be a great help in advancing towards a better understanding of the interrelations between the nervous, immune and endocrine systems, with their intracellular cascades and final outcomes in genetic expression and protein function, in depression. This will enable more efficient, more selective and faster-acting drugs to be developed and, in the future and with the help of psychogenomics, even make it possible to produce tailor-made medication for each patient (AU)

Objetivo: El proceso de dependencia al alcohol implica modificaciones funcionales en la neurotransmisión cerebral alterando de esta manera el equilibrio homeostático entre neurotransmisores inhibidores y excitadores en diferentes circuitos cerebrales relacionados con el refuerzo inducido por sustancias. Material y métodos: Se revisa la neurobiología de la dependencia al alcohol. Resultados: Un gran número de resultados experimentales sugieren que el alcohol ejerce efectos muy variados sobre los principales sistemas de neurotransmisión dependiendo del tiempo y el patrón de ingesta de la bebida. Así, el consumo de alcohol a corto plazo potencia la neurotransmisión inhibitoria (por inhibición de la transmisión glutamatérgica, y activación de neuronas GABAérgicas), mientras que a largo plazo se produce tolerancia, lo que tiende a desplazar estas alteraciones neuroquímicas hacia valores normales. Por el contrario, si el consumo de alcohol es discontinuo o se reduce bruscamente, se produce activación de la neurotransmisión glutamatérgica e inhibición de la neurotransmisión GABAérgica, entre otras muchas alteraciones neuroquímicas, lo que desencadena síndrome de abstinencia y consiguiente comportamiento de búsqueda y deseo irrefrenable de consumir alcohol. Conclusiones: Los efectos del alcohol están regulados por distintos sistemas de neurotransmisión y neuromodulación, entre los que se puede destacar los sistemas de la dopamina (DA), noradrenalina (NA), factor liberador de corticotropina (CRF), neuropéptido Y (NPY), sistema opioide y el sistema cannabinoide endógeno, de forma que no se pueden explicar muchos de los efectos farmacológicos y comportamentales del alcohol sin entender sus acciones sobre estos sistemas (AU)
