Background: Oral squamous cell carcinoma (OSCC) is the most prevalent malignant head and neck tumor, excluding the nonmelanoma skin cancer. Despite recent advances in the diagnosis and treatment, the disease's mortality rate is nonetheless high. The presence of isolated neoplastic cells or small clusters of up to four cells at the tumor's invasive front, named tumor budding, is associated with a worse prognosis in OSCC. Angiogenesis has also been recognized as a determining factor in the progression of malignancies and in the development of metastases. Several studies have investigated the assessment of microvascular density (MVD) as a potential prognostic factor in OSCC. This study aimed to evaluate, in OSCC, differences in MVD between tumors with high-intensity tumor budding and tumors with low-intensity or no tumor budding. In samples with high-intensity tumor budding, differences in MVD between the budding area and the area outside the budding were also evaluated. Moreover, the study assessed differences in MVD concerning clinicopathological characteristics such as sex, age, tobacco smoking, tumor location and tumor size. Material and methods: One hundred and fifty [150] samples of OSCC were subjected to immunohistochemistry to assess the intensity of tumor budding (by immunostaining for multi-cytokeratin) and MVD (by immunostaining for CD34 and CD105, independently). The data were treated using descriptive and analytical statistics. (AU)

BackgroundTumor-associated macrophages (TAM) are known to facilitate colorectal cancer (CRC) growth. High macrophage infiltration in thymidine phosphorylase (TYMP) expressing CRC may correspond to poor prognosis. The prognostic impact of the expression CD163, a receptor associated with TAM, and TYMP in stroma, respectively, tumor tissue is not yet established. The aim of this study was to identify the potential associations between TYMP and CD163 expression levels and relapse-free survival (RFS) of patients with stage II CRC, and if microdissection is of importance.MethodsStage II CRC patients, radically resected with relapse (n = 104), were matched to patients with a 5-year relapse-free follow-up (n = 206). Gene expression of TYMP and CD163 was analyzed in snap-frozen tumor tissues and in microdissected formalin-fixed tumor tissues separated into tumor epithelium and stroma.ResultsTYMP expression was high in poorly differentiated tumors, right-sided CRC, and tumors with high microsatellite instability CD163-expressing macrophages near tumor epithelial cells had high expression in poorly differentiated and T4 tumors. High TYMP expression in tumor epithelial cells was in the multivariate analyses associated with shorter relapse-free survival (hazard ratio 1.66; 95% confidence interval: 1.09–2.56; p < 0.05).ConclusionsTYMP expression in tumor epithelial cells was associated with RFS and emphasizes the need for tissue microdissection. Additional studies are needed to establish whether TYMP and CD163 could add clinically relevant information to identify high-risk stage II patients that could benefit from adjuvant chemotherapy. Biofilm development is characterized by distinct stages of initial attachment, microcolony formation and maturation (sessile cells), and final detachment (dispersal of new, planktonic cells). In this work we examined the influence of polyhydroxyalkanoate (PHA) accumulation on bacterial surface properties and biofilm formation on polystyrene in detached vs. planktonic cells of an environmental strain isolated from microbial mats, Halomonas venusta MAT28. This strain was cultured either in an artificial biofilm in which the cells were immobilized on alginate beads (sessile) or as free-swimming (planktonic) cells. For the two modes of growth, conditions allowing or preventing PHA accumulation were established. Cells detached from alginate beads and their planktonic counterparts were used to study cell surface properties and cellular adhesion on polystyrene. Detached cells showed a slightly higher affinity than planktonic cells for chloroform (Lewis-acid) and a greater hydrophobicity (affinity for hexadecane and hexane). Those surface characteristics of the detached cells may explain their better adhesion on polystyrene compared to planktonic cells. Adhesion to polystyrene was not significantly different between H. venusta cells that had accumulated PHA vs. those that did not. These observations suggest that the surface properties of detached cells clearly differ from those of planktonic cells and that for at least the first 48 h after detachment from alginate beads H. venusta retained the capacity of sessile cells to adhere to polystyrene and to form a biofilm (AU)

Mas-related G protein-coupled receptor D (MrgD) is expressed almost exclusively in nociceptive primary sensory neurons and the neurons located in stratum granulosum of skin. More and more evidence suggest that MrgD plays an important role in pain sensation and/or transduction. Recent studies have demonstrated that the receptor is also involved in itch sensation in both mouse and human. In the present study, we identified a robust inward current in MrgD-expressing Xenopusoocytes by using â-alanine, a putative ligand of MrgD. The currents were sensitive to inhibitor of Ca2+-activated chloride channels (CaCCs) and intracellular Ca2+ chelator, suggesting they were produced by endogenous CaCCs. Furthermore, it was demonstrated that upon the application of phospholipase C (PLC) inhibitor, or antisense oligonucleotides of inositol trisphosphate receptor (IP3R), the â-alanine-induced currents were dramatically depressed. However, protein kinase C inhibitor did not display any visible effect on CaCC currents. In summary, our data suggest that the activation of MrgD promotes the open of endogenous CaCCs via Gq-PLC-IP3-Ca2+ pathway. The current findings reveal the functional coupling between MrgD and CaCCs in Xenopus oocytes and also provide a facile model to assay the activity of MrgD

Introducción. Los niveles de paracetamol se miden habitualmente en los laboratorios de urgencias ante la evidencia o sospecha de una intoxicación para predecir la posible hepatotoxicidad aguda por este fármaco. Sin embargo varios estudios han reportado un falso incremento en la concentración de paracetamol medida por métodos enzimáticos en plasmas ictéricos, circunstancia que ocurre frecuentemente en casos de fracaso hepático. Material y métodos. Para evaluar la influencia de la bilirrubina en la medición de paracetamol en el analizador Dimension(R) EXL(TM) (Siemens Healthcare Diagnostics Inc.(R)) se preparó una solución primaria de interferente a una concentración de 800 mg/dl (13.680 μmol/l) que se añadió a una solución de control comercial con altos niveles de paracetamol hasta obtener una mezcla con una concentración de bilirrubina de 40 mg/dl (684 μmol/l) realizando la medición de paracetamol en esta mezcla y en una de referencia. Comprobada la existencia de interferencia se procedió a cuantificarla diluyendo la solución primaria hasta obtener mezclas con 20; 10; 5; 2,5; 1,25 y 0,625 mg/dl de bilirrubina (342; 171; 85,5; 42,75; 21,37 y 19,69 μmol/l). Se obtuvo el interferograma representando la desviación de la medida frente a la mezcla de referencia frente a las distintas concentraciones de interferente. Resultados. Las distribuciones fueron gaussianas, por lo que se pudo aplicar la t de Student para el cálculo de las significaciones estadísticas. Se demostró la existencia de una interferencia significativa negativa a partir de 5 mg/dl (85,5 μmol/l) de bilirrubina, lo que muestra que las mediciones de paracetamol deben interpretarse con cautela en casos de hiperbilirrubinemia (AU)


Background. Blood levels of acetaminophen (paracetamol) are measured routinely in large acetaminophen overdoses or when poisoning is suspected, in order to predict the likelihood of hepatotoxicity. However some cases of spurious acetaminophen levels have been reported in jaundiced patients. Material and methods. The interference of bilirubin in acetaminophen measurements was studied on the Dimension(R) EXL(T)M by preparing a primary mixture of bilirubin at 800 mg/dL, which was spiked to a quality control solution with high acetaminophen concentration in order to get a bilirrubin concentration of 40 mg/dl on the mixture. Acetaminophen concentration was measured in the mixture with bilirubin and in reference solution without bilirubin. Interference was assessed and quantified by diluting the bilirubin solution to achieve final concentrations of 20, 10, 5, 2.5, 1.25 and 0.625 mg/dl (342, 171, 85.5, 42.75, 21.37 and 19.69 μmol/l). A graph of the interference was obtained showing the bias of acetaminophen concentrations with reference solution vs. increasing bilirubin concentrations. Results. Acetaminophen concentrations in both solutions followed Gaussian distributions. The t test showed significant negative interference at bilirubin concentrations higher to 5 mg/dl (85.5 μmol/l), which means that blood acetaminophen concentrations must be interpreted with caution in cases of hyperbilirubinemia (AU)

El proceso farmacodinámico se define como el estudio de los efectos bioquímicos y fisiológicos así como los mecanismos de acción de los fármacos en el organismo. Los objetivos en el análisis del proceso farmacodinámico pretenden clarificar la interacción físico-química entre el fármaco y la célula diana y, de esta forma, caracterizar la secuencia completa de acciones farmacológicas derivadas del uso de los fármacos. Los efectos farmacológicos son la consecuencia natural de la interacción del fármaco con los componentes macromoleculares del organismo. A partir de dicha interacción se inician los cambios bioquímicos y fisiológicos que caracterizan la respuesta de un fármaco. Este concepto, que en la actualidad resulta obvio, tuvo su origen en el trabajo de Ehrlich y Langley al final del siglo XIX y comienzos del XX. El concepto de receptor fue elaborado para destacar ese componente macromolecular orgánico sobre el que se presumía que interaccionaba la sustancia química. La afirmación de que el receptor farmacológico puede ser cualquier componente funcional del organismo, tiene diversas connotaciones. Una es que un fármaco puede modificar la velocidad de casi cualquier proceso fisiológico; y, por otro lado, un fármaco no crea o inventa efectos nuevos en el organismo, sino que se limita a regular determinadas funciones preexistentes en las células. En este capítulo se describen también las características de los fármacos agonistas y tipos de bloqueo, a nivel de receptores, producidos por los fármacos antagonistas (AU)


Pharmacodynamics can be defined as the study of the biochemical and physiological effects of drugs and their mechanisms of action. The objectives of the analysis of drug action are to delineate the chemical or physical interactions between drug and target cell and to characterize the full sequence and scope of actions of each drug. The effects of most drugs result from their interaction with macromolecular components of the organism. These interactions alter the function of the pertinent component and thereby initiate the biochemical and physiological changes that are characteristic of the response to the drug. This concept, now obvious, had its origin in the experimental work of Ehrlich and Langley during the late nineteenth and early twentieth centuries. The term receptor was coined to denote the component of the organism with which the chemical agent was presumed to interact. The statement that the receptor for a drug can be any functional macromolecular component of the organism has several fundamental corollaries. One is that a drug potentially is capable of altering the rate at which any bodily function proceeds. Another is that drugs do not create effect, but instead modulate functions. Objetivo. Se presenta una revisión actualizada de losprincipales aspectos farmacológicos, junto con los efectos clínicosy conductuales, asociados al ácido gamma-hidroxibutírico (GHB).Desarrollo. Numerosos estudios farmacológicos, neuroquímicos yelectrofisiológicos indican claramente que el GHB endógeno desempeñaun papel como neurotransmisor y/o neuromodulador en el sistemanervioso central (SNC). Así, el GHB exhibe mecanismos específicosde síntesis, liberación y recaptación, así como lugares deunión específicos que sugieren la existencia de un sistema GHBérgicocentral. Esta sustancia, popularmente conocida como ‘éxtasislíquido’ es, además, una droga con potencial de abuso, cuya administraciónprolongada puede producir dependencia y un síndromede abstinencia tras el cese de su consumo. Sus principales accionesconductuales incluyen sedación/sueño, inducción de crisis de ausencia,catalepsia o reducción de la agresión, entre otras. Algunosde estos efectos parecen relacionarse con una interacción descritaentre el sistema GHBérgico y los receptores dopaminérgicos y gabérgicosen el SNC. Desde el punto de vista clínico, su uso ha aprobadoen algunos países para el tratamiento del síndrome narcoléptico,y se ha planteado también su posible utilidad en el tratamientode la dependencia del alcohol y los opiáceos. Finalmente, estudiosrecientes con animales de experimentación sugieren la existencia deun posible efecto neurotóxico tras su administración prolongada endosis con potencial de abuso. Conclusiones. El GHB es un compuestoextraordinariamente interesante. Funciona como un neurotransmisor/neuromodulador en el SNC. Es, además, una droga recreativacon potencial de abuso, y puede utilizarse para el tratamientode diversas patologías, especialmente de la narcolepsia. Sinembargo, el posible desarrollo de neurotoxicidad tras su administraciónprolongada limita considerablemente su utilidad en contextosclínicos


Aims. The article offers an updated review of the main pharmacological aspects of gamma-hydroxybutyric acid(GHB), as well as its clinical and behavioural effects. Development. A number of pharmacological, neurochemical andelectrophysiological studies have clearly shown that endogenous GHB plays a role as a neurotransmitter and/or neuromodulatorin the central nervous system (CNS). GHB displays specific synthesis, release and reuptake mechanisms, as well asparticular binding sites that suggest the existence of a central GHBergic system. This substance, popularly known as ‘liquidecstasy’, is also a potentially abusable drug; if administered for prolonged periods of time it can lead to dependence andwithdrawal symptoms after the patient stops taking it. Its chief behavioural actions include sedation/sleepiness, induction ofabsence seizures, catalepsy or reduced aggression, among others. Some of these effects appear to be related to an interactionthat has been reported to exist between the GHBergic system and the dopaminergic and GABAergic receptors in the CNS.From the clinical point of view, its use has been approved in some countries to treat the narcoleptic syndrome, and it has alsobeen considered for possible use in the treatment of alcohol or opiate abuse. Finally, recent studies conducted with laboratoryanimals suggest the existence of a possible neurotoxic effect following prolonged administration in abusable dosages.Conclusions. GHB is an extraordinarily interesting compound. It acts as a neurotransmitter/neuromodulator in the CNS. It isalso an abusable recreational drug and may also be used to treat a number of different pathological conditions, the mostimportant of which is narcolepsy. The possible development of neurotoxicity following prolonged administration, however,imposes considerable limitations on its usefulness in clinical contexts

Introducción. La siderosis superficial del sistema nervioso central (SNC) es una enfermedad rara, caracterizada por sordera, ataxia y disfunción piramidal. Se debe al depósito de hemosiderina en las membranas subpiales del cerebro, la médula espinal y los nervios craneales. La mayoría de los casos son secundarios a un sangrado crónico o recurrente en el espacio subaracnoideo. Se requiere la realización de una resonancia magnética (RM) para alcanzar el diagnóstico. Casos clínicos. Presentamos dos pacientes con un cuadro crónico de ataxia cerebelosa y pérdida auditiva progresivas. En ambos casos, la RM mostraba señales hipointensas en T2 en el cerebro, el cerebelo y la médula espinal, diagnósticas de siderosis superficial del SNC. En uno de los pacientes se demostró xantocromía en el análisis del líquido cefalorraquídeo. Los potenciales evocados auditivos revelaron, en ambos casos, una pérdida auditiva retrococlear. Se realizaron estudios vasculares exhaustivos sin que se pudiera demostrar la presencia de fuentes de sangrado. Uno de los pacientes, que recibía tratamiento con anticoagulantes orales, se benefició de la reducción del INR. Conclusión. Debido a su rareza, la sospecha clínica es esencial para el diagnóstico de esta enfermedad (AU)


Introduction. Superficial siderosis of the central nervous system (CNS) is a rare disease characterized by deafness, ataxia and pyramidal dysfunction. It is due to hemosiderin deposition in the subpial membranes of the brain, spinal cord and cranial nerves. Most cases are secondary to chronic or recurrent bleeding into the subarachnoid space. Diagnosis is permitted by magnetic resonance imaging (MRI). Case reports. We report two patients with a chronic, slowly progressive cerebellar ataxia and hearing loss. MRI showed T2 hypointense signals in the brain, cerebellum and spinal cord diagnostic of superficial siderosis of the CNS. Xanthochromia was present in one patient. Evoked potentials showed retrochoclear hearing loss. Extensive vascular studies were negative for bleeding sources. One patient, treated with oral anticoagulants, benefited from reduction of the International Normalized Ratio. Conclusion. Due to its rarity, clinical suspicion is essential for diagnosis of superficial siderosis of the CNS (AU)

Introducción. El término CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy) se refiere a una arteriopatía cerebral hereditaria autosómica dominante que se caracteriza por migraña, ictus recurrentes y deterioro cognitivo progresivo. Presentamos una nueva familia con CADASIL y destacamos la importancia del estudio genético en su diagnóstico. Caso clínico. Mujer de 62 años con ictus lacunares de repetición, demencia subcortical y antecedentes familiares de demencia e ictus. Los estudios de neuroimagen que se realizaron a la paciente y los hermanos mostraron signos de leucoencefalopatía e infartos lacunares. El estudio ultraestructural de la biopsia de la piel de la paciente, que incluyó cinco vasos dérmicos, no mostró depósitos electrodensos. El estudio genético demostró la presencia de la mutación C475T en el exón 4 del NOTCH3. Conclusiones. Debemos sospechar la posibilidad del CADASIL en pacientes con clínica de enfermedad cerebrascular o demencia que presenten alteraciones características en la resonancia magnética cerebral, especialmente cuando exista una historia familiar compatible. El procedimiento diagnóstico de elección debe ser el estudio genético (AU)


Introduction. The term CADASIL (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy) refers to an autosomal dominant hereditary arteriopathy of the brain that is characterised by headache, recurring strokes and progressive cognitive deterioration. We report the case of another family with CADASIL and emphasise the importance of a genetic study in its diagnosis. Case report. A 62-year-old female patient with repeating lacunar strokes, subcortical dementia and a family history of dementia and strokes. Neuroimaging studies conducted on the patient and her siblings showed signs of leukoencephalopathy and lacunar infarctions. The ultrastructural study of the biopsy performed on a sample of the patient’s skin, which included five dermal vessels, did not show any electron-dense deposits. The genetic study revealed the presence of mutation C475T in exon 4 of NOTCH3. Conclusions. The possible presence of CADASIL must be suspected in patients with symptoms of cerebrovascular disease or dementia who present characteristic alterations in the magnetic resonance brain scan, especially when there is a compatible family history. The first choice diagnostic procedure must be a genetic study (AU)

Las glándulas paratiroides tienen una gran sensibilidad a pequeños cambios en el calcio iónico extracelular. El receptor sensor de calcio (CaR) es un receptor acoplado a proteínas G que es capaz de responder a incrementos en el calcio extracelular activando una serie de señales intracelulares (fosfolipasas C, A2 y D) y cuyo resultado final es la inhibición de la secreción de PTH. Además del calcio, existen otros agonistas y moduladores del CaR como el Mg, espermina, péptidos amilodes β, ciertos aminoácidos, especialmente los aminoácidos aromáticos y la fuerza iónica. En la uremia la sensibilidad de las glándulas paratiroides al calcio está alterada, necesitando concentraciones más altas del mismo para suprimir la PTH. En el hiperparatiroidismo secundario la expresión del CaR se encuentra reducida. En glándulas hiperplásicas se ha observado una correlación negativa entre la proliferación celular y la expresión del CaR. A pesar de ser un receptor de calcio, la expresión del CaR no parece estar regulada por el calcio y existe una cierta discrepancia sobre si el calcitriol es capaz de regular su expresión. Por otro lado, el fósforo provoca una hiperplasia de la glándula paratiroides incrementando la proliferación celular y un descenso en la expresión del CaR (AU)


The parathyroid glands have a great sensitivity to small changes in the extracellular ionic calcium. The calcium-sensing receptor (CaR) is a G protein-coupled receptor that responds to extracellular ionic calcium changes activating several intracellular signaling systems (phospholipases C, A2 and D) finally inhibiting the PTH secretion. In addition to calcium, there are some other agonists and modulators such as the Mg2+, spermine, amyloid β-peptides, a variety of aminoacids, especially aromatic aminoacids and ionic strength. In the uraemia, the sensitivity of the parathyroid glands to calcium is altered and higher values of calcium are necessary to suppress the PTH. In the secondary hyperparathyroidism the CaR expression is reduced. It has been found a negative correlation between cellular proliferation and the expression of the CaR in hyperplasic glands. RESUMO

Aunque en la actualidad la etiología de la enfermedad inflamatoria intestinal (IBD) sigue siendo desconocida, se ha propuesto la existencia de una respuesta inflamatoria anormal contra la microflora entérica en un huésped susceptible genéticamente como una de ellas (1). Esta hipótesis del desorden genético se apoya en el agrupamiento familiar que existe en la enfermedad y los estudios en gemelos, especialmente en la enfermedad de Crohn (CD) (2). La búsqueda de genes de susceptibilidad para la IBD ha hecho que se identificaran muchos locus para la enfermedad de Crohn y colitis ulcerosa, en concreto un locus de susceptibilidad para la enfermedad de Crohn en la región pericentromérica del cromosoma 16 (IBD1) por Hugot y cols. en 1996 y otros grupos en diferentes países (3-8).Recientemente Hugot y cols. y Ogura y cols., utilizando diferentes procedimientos y basándose en la existencia de este gen de susceptibilidad que se superpone con el IBD1 en el cromosoma 16q12 identificaron el primer gen de susceptibilidad para la enfermedad de Crohn, el NOD2 ( 3 , 9 ) .La inserción de mutaciones en el gen NOD2 es lo que confiere el incremento de la susceptibilidad a la enfermedad de Crohn pero no a la colitis ulcerosa (10). Así, dada la localización genómica y el papel de las proteínas del Nod en el reconocimiento de los lipopolisacáridos bacterianos (LPS) se ha propuesto al NOD2 como un gen de susceptibilidad para la enfermedad de Crohn y además se le atribuye un papel en el sistema inmune innato (3,9,11). En el número de noviembre de 2001 de esta revista ya tuvimos la oportunidad de discutir la importancia de la inmunidad innata en la regulación de la respuesta inmune y también revisamos el papel desempeñado por el NOD2 o CARD15, en el cambio del enfoque de la investigación dentro de la enfermedad de Crohn hacia el estudio de la inmunidad innata en lugar del sistema inmune adquirido (12). Este importante descubrimiento, el primer gen de susceptibilidad para la enfermedad de Crohn, ha mostrado la existencia de un modelo molecular para entender un mecanismo patogénico en la enfermedad de Crohn (3) (AU)

Numerosos estudios realizados en las dos últimas décadas han demostrado la existencia en el organismo animal de un sistema cannabinoide endógeno, constituido por unos ligandos, los endocannabinoides. Se han descrito dos tipos de receptores para cannabinoides: los denominados CB1, localizados preferentemente en cerebro y los CB2 que están localizados en el sistema inmune. Los dos endocannabinoides de los que más datos se dispone son la araquidoniletanolamida o anandamida y el 2-araquidonilglicerol, habiéndose postulado su posible actuación como neurotransmisores o neuromoduladores. La distribución cerebral de los endocannabinoides y de los receptores CB1 ha permitido conocer las funciones fisiológicas en las que está involucrado este sistema. Participa, a través de modular la actividad de los neurotransmisores, en la regulación del comportamiento motor y de la secreción de hormonas adenohipofisarias, interacciona con la dopamina y con el GABA, mientras que, en el caso de la memoria y el aprendizaje, lo hace con el GABA y el glutamato. La dopamina y los péptidos opioides podrían estar implicados en la participación de los endocannabinoides en el sistema de recompensa y en el control de la nocicepción. Por otro lado, la síntesis de anandamida, en condiciones de isquemia, podría jugar un papel protector en las regiones cerebrales afectadas. Se ha visto que este compuesto inhibe la captación mitocondrial de calcio y la liberación de glutamato, efectos ámbos que contribuyen a la citotoxicidad cerebral (AU)


Numerous studies carried out in the last two decades have shown the existence in the animal organism of an endogenous cannabinoide system comprising certain ligands, the endocannabinoides. Two types of receptors for cannabinoides have been described, those known as CB1, located predominantly in the brain, and the CB2 located in the immune system. The two endocannabinoides on which there is more available data are the arachidonylethanolamide or anandamide and the 2-arachidonylglycerol, their possible action as neurotransmitters or neuromodulators having been postulated. The cerebral distribution of the endocannabinoides and the CB1 receptors has led to knowledge of the physiological functions that involve this system. It participates by modulating the action of the neurotransmitters, in regulating motor behaviour and the secretion of adenohypophysary hormones, interacts with dopamine and the GABA and, in the case of memory and learning, it interacts with the GABA and the glutamate. The dopamine and the opioid peptides may be implicated in the participation of the endocannabinoides in the drug reward system and in the control of nociception. At the same time, the synthesis of anandamine in ischemia may play a protective role in the cerebral regions affected. It has been seen that this compound inhibits the mitochondrial uptake of calcium and the liberation of glutamate, both effects that contribute to cerebral cytoxicity (AU)
