Introducción y objetivos: La haptoglobina es una proteína implicada en la protección frente al daño oxidativo producido por el hierro de la hemoglobina. Esta proteína es polimórfica, con 3 isomorfas prevalentes en la población. Los portadores de la isoforma Hp2-2 tienen una menor capacidad antioxidante, y en la población con diabetes, un mayor riesgo de enfermedad vascular subclínica y de complicaciones cardiovasculares. Nuestro objetivo fue evaluar si dicha isomorfa se asocia con un mayor riesgo de arteriosclerosis carotídea en sujetos con y sin diabetes, libres de enfermedad cardiovascular. Pacientes y métodos: Estudio realizado en una población de entre 45 y 74años de edad seleccionada aleatoriamente del área noroeste de Madrid. Los participantes fueron caracterizados en cuanto a su estatus glucémico mediante una sobrecarga oral de glucosa y la determinación de la concentración de Hb1Ac. A todos ellos se les determinó el fenotipo de la haptoglobina mediante un ensayo inmunoenzimático y la presencia de arteriosclerosis carotídea mediante ecografía. Resultados: De los 1.256 participantes incluidos en el presente análisis (edad media 61,6 ± 6 años, 41,8% varones), la distribución de las isoformas de la haptoglobina fue la siguiente: Hp1-1: 13,3%, Hp1-2: 48,5% y Hp2-2: 38,2%. En comparación con los sujetos Hp1-1 y Hp1-2, aquellos con el fenotipo Hp2-2 tuvieron una mayor prevalencia de dislipemia (53,3% vs 43%, p < 0,0001) e hipertensión arterial (39,2% vs 32,2%, p = 0,012), y recibieron con más frecuencia tratamiento con estatinas (31,5% vs 21,6%, p < 0,0001) y con antihipertensivos (38,4% vs 30,8%, p = 0,006). Los portadores de la isoforma Hp2-2 tuvieron una mayor prevalencia de placas carotídeas (OR: 1,35; IC 95%: 1,07-1,69; p = 0,011), sin diferencias en dicha prevalencia en función del estatus glucémico. No existieron diferencias en el grosor íntima-media entre los diferentes fenotipos. La relación del fenotipo Hp2-2 con la presencia de placas en carótida fue independiente de la edad, del sexo, de la presencia de factores de riesgo (dislipemia, hipertensión y diabetes), de la concentración de colesterol LDL, proteína C reactiva y ácido úrico, de la presión arterial y del tratamiento con estatinas y antihipertensivos (OR: 1,31; IC 95%: 1,01-1,70; p = 0,044). Conclusión: Los sujetos con el fenotipo Hp2-2 de la haptoglobina tienen una mayor prevalencia de arteriosclerosis carotídea, que es independiente de la presencia de otros factores de riesgo cardiovascular y de su estatus glucémico


Introduction and objectives: Haptoglobin is a protein involved in the protection against oxidative damage caused by iron in haemoglobin. This protein is polymorphic, with 3 isomorphs prevalent in the population. The carriers of the Hp2-2 isoform have a lower antioxidant capacity and, in the population with diabetes, an increased risk of subclinical vascular disease and cardiovascular complications. The objective of this study was to evaluate whether this isomorphy is associated with an increased risk of carotid arteriosclerosis in subjects with and without diabetes, and free of cardiovascular disease. Patients and methods: A study was conducted in a population between 45 and 74years of age, randomly selected from the northwest area of Madrid. The participants were characterised in terms of their glycaemic status by oral glucose overload and the determination of the concentration of Hb1Ac. The haptoglobin phenotypes in all of them were determined by means of an immunoenzymatic assay, and the presence of carotid arteriosclerosis by ultrasound. Results: Of the 1,256 participants included in the present analysis (mean age 61.6 ± 6 years, 41.8% males), the distribution of the isoforms of haptoglobin was as follows: Hp1-1: 13.3%, Hp1-2: 48.5%, and Hp2-2: 38.2%. In comparison with subjects Hp1-1 and Hp1-2, those with the Hp2-2 phenotype had a higher prevalence of dyslipidaemia (53.3% vs 43%; P < .0001) and arterial hypertension (39.2% vs. 32.2%, P = .012), and they more frequently received treatment with statins (31.5% vs 21.6%, P < .0001), and with antihypertensive agents (38.4% vs 30.8%, P = .006). The carriers of the Hp2-2 isoform had a higher prevalence of carotid plaques (OR: 1.35, 95%CI: 1.07-1.69, P = .011), with no differences in that prevalence as regards the glycaemic status. There were no differences in the intima-media thickness between the different phenotypes. The relationship of the Hp2-2 phenotype with the presence of plaques in the carotid was independent of age, gender, presence of risk factors (dyslipidaemia, hypertension and diabetes), the concentration of LDL-cholesterol, C-reactive protein and uric acid, blood pressure, and treatment with statins, and hypertensive drugs (OR: 1.31, 95% CI 1.01-1.70, P = .044). Conclusion: Subjects with the Hp2-2 phenotype of haptoglobin have a higher prevalence of carotid arteriosclerosis, which is independent of the presence of other cardiovascular risk factors and their glycaemic status

RESUMO

TP73 is a member of the TP53 family whose expression has been observed altered in most human cancers and associated with the prognosis. TP73 translates into a complex number of isoforms with both oncogenic and tumour-suppressor functions and presents a complex cross-talk with other members of the family (TP53 and TP63). In this revision, we focus on the evidence that may support TP73 variants as prognostic markers in cancer. Nowadays, most publications in this topic highlight the association between overexpression of the oncogenic variants and failure to respond to chemotherapy and/or shorter survival. In addition, we comment on the putative possibilities that the detection through a liquid biopsy of TP73 variants may provide, and finally, the significance of determining the value of the combined alteration of the TP53 family members in the clinical setting

La interleucina 17 (IL-17) es una citocina proinflamatoria que participa en la defensa frente a infecciones por hongos y bacterias. Sobre la base de los resultados de ensayos clínicos utilizando anticuerpos bloqueadores de una de sus isoformas, la IL-17A, esta citocina se ha demostrado una diana terapéutica muy eficaz para el control de enfermedades como la psoriasis, la artropatía psoriásica o la espondilitis anquilosante. Esta revisión se centra en las evidencias que estudian la presencia de estas citocinas y de las células que las producen en los tejidos diana de las espondiloartropatías: raquis y articulaciones periféricas, así como en sangre periférica


Interleukin-17 (IL-17) is a proinflammatory cytokine that participates in defense against infections by fungi and bacteria. On the basis of the results of clinical trials involving antibodies that block one of its isoforms, IL-17A, this cytokine has been found to be a highly effective therapeutic target for the management of diseases like psoriasis, psoriatic arthropathy or ankylosing spondylitis. This review focuses on the evidence demonstrating the presence of this cytokine and of the cells that produce it in the target cells of spondyloarthropathies: spine and peripheral joints, as well as peripheral blood

En este trabajo se han obtenido nuevas líneas de células de músculo liso vascular (VSMCs) que nos han permitido demostrar que la IRA e IRA/IGF-1R podrían conferirles una ventaja proliferativa y migratoria en respuesta a insulina, IGF-2 o TNF-α. Estos resultados podrían ser relevantes ya que en las fases iniciales del proceso aterogénico nosotros hemos demostrado que hay un aumento significativo de la expresión de la IRA e IGF-1R así como una mayor presencia de receptores híbridos en la aorta de dos modelos experimentales de aterosclerosis temprana. Y finalmente, como el tratamiento con un anticuerpo anti-TNF-α previno las alteraciones vasculares


In this work, we have obtained new lines of vascular smooth muscle cells (VSMCs) to demonstrate that IRA and IRA/IGF-1R might confer a proliferative and migratory advantage in response to insulin, IGF-2 or TNF-α. These results might be relevant due to in the early stages of atherosclerotic process; we have demonstrated that there is a significant increase of IRA and IGF-1R expression as well as higher formation of hybrid receptors in the aorta from two models of early atherosclerosis. Finally, anti-TNF-α treatment prevented vascular alterations

TREK-2 (TWIK-related K+ channel-2), a member of two-pore domain potassium (K2P) channel family, tunes cellular excitability via conducting leak or background currents. In TREK-2, the isoforms generated by alternative translation initiation (ATI) mechanism exhibit large divergence in unitary conductance, but similar in selectivity to K+. Up to now, the structural basis for this similarity in ion selectivity is unknown. Here, we report that externally applied Ba2+ inhibits the currents of TREK-2 in a concentration- and time-dependent manner. The blocking effect is blunted by elevated extracellular K+ or mutation of S4 K+ binding site, which suggests that the inhibitory mechanism of Ba2+ is due to its competitive docking properties within the selectivity filter (SF). Next, we demonstrate that all the ATI isoforms exhibit analogous behaviors upon the application of Ba2+ and alteration of extracellular pH (pHo), which acts on the outer position of the SF. These results strongly support the notion that all the ATI isoforms of TREK-2 possess resembled SF conformation in S4 site and the position defined by pHo, which implicates that neither the role of N-terminus (Nt) nor the unitary conductance is associated with SF conformation. Our findings might help to understand the detail gating mechanism of TREK-2 and K2P channels

S-resistin is a non-secretable resistin spliced variant, which is expressed mainly in the white adipose tissue from Wistar rats. Previous results confirmed that 3T3-L1 cells expressing s-resistin (3T3-L1-s-res) showed an inflammatory response and exhibited a decrease in glucose transport, both basal and insulin-stimulated. Here we present evidences demonstrating for the first time that s-resistin, like resistin, blocks insulin signalling pathway by inhibiting insulin receptor, insulin receptor substrate 1, protein kinase B/Akt and the mammalian target of rapamycin phosphorylation, and increasing the suppressor of cytokine signalling 3 levels being the later probably due to augmented of leptin expression. Thus, our data suggest that s-resistin could act by a still unknown intracrine pathway as an intracellular sensor, regulating the adipocyte insulin sensitivity

La incorporación del antígeno prostático específico (PSA) a la clínica revolucionó el diagnóstico y modificó la epidemiología del cáncer de próstata (CaP). Aunque le faltan muchas de las características de un marcador tumoral ideal, es el marcador más usado para el diagnóstico y seguimiento de cualquier tipo cáncer. Representa la mejor herramienta clínica de las que disponemos en la actualidad para el cribado y estadificación del CaP. Por contra, la mayor limitación que presenta el PSA es su falta de especificidad tumoral. El empleo de los derivados y de las isoformas moleculares del PSA trata de solventar, al menos en parte, sus limitaciones. De hecho, la utilización del cociente del PSA libre (%fPSA) y de la densidad del PSA (PSAD) aumenta de forma significativa la especificidad del test en el diagnóstico y, el uso de los derivados que evalúan la cinética temporal del PSA (velocidad del PSA (PSAV) y tiempo de duplicación del PSA (PSADT)) representan herramientas de gran utilidad para estimar el pronóstico durante el tratamiento y seguimiento de la enfermedad. El mayor avance que se ha producido en los últimos años ha venido del análisis de la isoforma precursora (-2)pPSA y del índice phi. Ambos marcadores han demostrado mejorar los resultados de sensibilidad y especificidad obtenidos hasta ahora, ocasionando una disminución de biopsias innecesarias. Es probable que con el desarrollo de nuevos marcadores para el CaP que está habiendo, en pocos años se modifique el papel que el PSA tiene en el diagnóstico y en la estadificación de la enfermedad


Incorporation of prostatic specific antigen (PSA) to clinical practice was a revolution in the diagnosis and modified the epidemiology of prostate cancer (PCa). Although it lacks of many characteristics of an ideal tumor marker, it is the marker most used for diagnosis and follow up of any kind of cancer. It represents the best clinical tool we have available today for screening and staging of PCa. On the contrary, its greatest limitation is the lack of tumor specificity. The use of PSA by-products and molecular isoforms tries to solve, at least partially, its limitations. Indeed, the use of FreePSA ratio (%fPSA) ad PSA density (PSAD) increase significantly the specificity of the diagnostic test and, the use of derivatives that evaluate time kinetics of PSA (PSA velocity (PSAV) and PSA doubling time (PSADT)) represents a very useful tool for prognosis estimation during treatment and follow up of the disease. The greatest advance over the last years comes from the analysis of the predecessor isoform (-2) pPSA and the phi Index. Both markers have demonstrated to improve the sensitivity and specificity results obtained to date, resulting in a decrease of unnecessary biopsies. Probably, with the ongoing development of new markers for PCa , the role of PSA on disease diagnosis and staging would be modified in a few years

PURPOSE: Luminal subtype breast cancer is defined as oestrogen receptor (ER)- and/or progesterone receptor (PR)- positive breast cancer. We detected the expression of ER-alpha, ER-Β1 and ER-Β2 in the tissue samples of invasive luminal subtype breast cancer patients, evaluated the correlations between these ER statuses and prognosis, and tried to clarify whether the status of ER-alpha isoforms provides clinically useful information further to what is already provided by the traditional ER-alpha/PR assay. METHODS: The expression of ER-alpha, ER-Β1 and ER-Β2 in the paraffin-embedded sections of 162 invasive luminal subtype breast cancer patients was detected with an immunohistochemical staining method. With mid-long-term follow-up, the features of ER-alpha, ER-Β1 and ER-Β2 status and the correlations between clinical characteristics and the prognosis were analysed. RESULTS: ER-Β1-positive status was correlated with PR (rs=0.217, p<0.01). The median follow-up time was 92 months (range, 4-98 months). Univariate analysis suggested that ER-Β1 status was significantly correlated to diseasefree survival (DFS) time (log rank=3.98, p=0.046), especially in patients with positive lymph nodes (log rank=6.20, p=0.013). In patients with smaller tumour size (=20 mm), negative ER-Β2 status was significantly correlated to overall survival time (log rank=3.87, p=0.049). CONCLUSIONS: In invasive luminal subtype breast cancers, ER-Β1 is correlated with good prognosis and could be regarded as one of the factors for evaluating DFS time, especially in lymph node-positive patients. There may be some interactions between ER-Β1 and PR. In clinical practice, besides routine detection of ER-alpha and PR in invasive luminal subtype breast cancers, immunohistochemical staining of ER-Β1 and ER-Β2 should be considered in order to achieve more useful information. OBJECTIVE: Pax3 and Pax7 are closely related genes that are involved in commitment of cells to a myogenic lineage during skeletal muscle development and regeneration. Several Pax3 and Pax7 transcripts are expressed from the genes, generating different isoforms with potentially distinct DNA binding and transactivation properties. The aim of this study was to investigate the implication of Pax3 and Pax7 C-terminal isoforms during myogenic differentiation and tumorigenesis, since fusions involving these genes are commonly associated with alveolar rhabdomyosarcoma (ARMS). METHODS: Uncommitted (mouse mesenchymal stem cells, MSCs) and committed (C2C12) myogenic precursor cells were stably transfected with PAX3/FKHR and PAXC7/ FKHR fusion genes. We analysed gene and protein expression comparing the newly generated cells with the parental cells, to determine the functional importance of Pax3 and Pax7 C-terminal isoforms. RESULTS: We found that the transcript Pax3c was expressed at low levels in undifferentiated C2C12 and MSCs cells, but its expression levels increased considerably at later stages of differentiation. However, expression levels of Pax3d transcript increased only slightly after differentiation. Pax7 transcripts, present before differentiation in committed C2C12 cells, but absent in uncommitted MSCs, increased noticeably in MSCs after differentiation. We also found that the presence of PAX/FKHR fusions prevented both C2C12 and MSC cells from terminal myogenic differentiation and increased the expression of discrete endogenous Pax3/7 transcripts, in particular Pax3d and Pax7B. CONCLUSIONS: Our results suggest that both Pax3 and Pax7 transcripts are required for commitment of cells to the myogenic lineage, with each transcript having a distinct role. More specifically, the Pax3c isoform may be required for terminal myogenic differentiation whereas the Pax3d isoform may be involved in undifferentiated cell maintenance and/or proliferation (AU)

Type 2 diabetes results from a combination of insulin resistanceand impaired insulin secretion. To directly address the effects ofhepatic insulin resistance in adult animals, we developed aninducible liver-specific IR knockout mouse (iLIRKO). Using thisapproach, we were able to induce variable IR deletion in a tissuespecificmanner (liver mosaicism). These mice demonstrateprogressive hepatic and extra hepatic insulin resistance, without liverdysfunction. Initially there is hyperinsulinemia and increased betacellmass in parallel to IR deletion by the liver. Our results withiLIRKO demonstrate a cause and effect relationship betweenprogressive insulin resistance and the fold-increase of plasma insulinlevels and beta-cell mass. Ultimately the beta cells undergo a failurein the insulin secretion that leads to uncontrolled diabetes. In thiscontext, iLIRKO mice induced IGF-1 in parallel to IR (IR) deletionin the liver. This resulted in an increase of circulating IGF-1.ÓSCAR ESCRIBANO Y COLS. AN. R. ACAD. NAC. FARM.24Concurrently, there was a huge increase of IR-A in the hyperplasticbeta cells. More importantly, as assessed in mouse beta-cell lines,IR-A, but not IR-B, confers a proliferative capability to beta-cellsin response to insulin or IGF-1 that may account for beta-cellhyperplasia induced by liver insulin resistance in iLIRKO mice. Thus,our results in iLIRKO mice suggest a liver-pancreatic endocrine axis,IGF-1 being a liver factor that might contribute together with insulinto compensatory pancreatic islet hyperplasia through IR-A(AU)


La diabetes tipo 2 es el resultado de una combinación de resistenciaa insulina y un defecto en la secreción de la misma. Paradeterminar el papel de la resistencia a la insulina hepática en animalesadultos, nuestro grupo generó un ratón para el receptor deinsulina en hígado de forma inducible (iLIRKO). Utilizando estatecnología fuimos capaces de obtener ratones con distinto grado dedeleción del receptor de insulina hepático. Estos ratones mostraronuna resistencia progresiva a la insulina inicialmente hepática peroque se extendió a tejidos extrahepáticos. Además, no se observóningún tipo de patología hepática. Inicialmente se observó un incrementoen los niveles de insulina circulantes y un aumento de lamasa de célula beta pancreática que fue proporcional al grado dedeleción del receptor de insulina en hígado. Finalmente, tiene lugarun fallo en la secreción de insulina por parte de la célula beta pancreática.En este contexto, el hígado de los animales iLIRKO escapaz de sintetizar IGF-1 de forma proporcional al grado de delecióndel receptor de insulina en hígado. Además, tuvo lugar un importanteincremento en la expresión de la isoforma A del receptor de insulina(IR-A) en las células beta. Más importante, como demostramosen células beta inmortalizadas, IR-A pero no IR-B confiere una grancapacidad proliferativa a las células beta en respuesta tanto a insulinacomo IGF-1, lo que puede explicar, al menos en parte, losmecanismos involucrados en la hiperplasia de la célula beta pancreáticadesarrollada por los ratones iLIRKO(AU)

Objetivo: Analizar si existe alguna asociación entre la resistencia a aspirina (RA) y la presencia de polimorfismos genéticos de un único nucleótido (SNPs) en el gen de la COX-1, así como su relación con modificaciones en la expresión de proteínas plasmáticas en pacientes con enfermedad isquémica estable y tratamiento continuado de aspirina. Materiales y métodos: Analizamos el proteoma plasmático de 19 pacientes sensibles, 19 resistentes. RA se definió mediante el sistema PFA-100. Se realizó electroforesis bidimensional (IPG 17cm, pH(4-7), geles SDS-PAGE 10%) y tinción con plata. Se analizaron cambios en tres SNPs (A-842G, C22T y C50T) en 50 pacientes sensibles, 33 resistentes y 83 controles mediante PCR a tiempo real. Resultados: La expresión de cuatro isoformas de alfa1-antitripsina estaba aumentada en los pacientes resistentes. No encontramos diferencias en la expresión de ceruloplasmina, precursor de haptoglobina, apolipoproteína AI y precursor de albúmina entre ambos tipos de pacientes. Ningún paciente presentó cambios en el SNP A-842G. La frecuencia de cambio en C22T y C50T fue relativamente baja con respecto a la población total. Conclusiones: No encontramos asociación entre la presencia de polimorfismos en el gen de la COX-1 y la peor respuesta a la aspirina. Los cambios en observados alfa1-antitripsina podrían estar relacionados con un diferente estado inflamatorio entre ambos tipos de pacientes (AU)


Aim: To evaluate the existence of a possible association between Aspirin resistance (AR), COX-1 single-nucleotide polymorphisms (SNPs) and the modifications in the plasma proteome of clinically stable coronary patients. Materials and methods: AR was defined according to the PFA-100 assay. AR-sensitive and AR-resistant patients had been taken aspirin for the last 9 months. The proteomic study (19 AR-sensitive, 19 AR-resistant) was performed using IPG strips (17cm, pH 4-7), SDS-PAGE gels (10%) and silver staining. We study three SNPs (A- 842G, C22T y C50T) in 50 AR-sensitive patients, 33 AR-resistant and 83 controls using a real-time PCR. Results: The expression of four alpha1-antitripsin isoforms was increased in the aspirin-resistant patients. No differences were found in the expression of ceruloplasmin, haptoglobin-precursor, apolipoprotein-AI and albumin- precursor between both groups of patients. The A-842G SNP was undetectable in all subjects. The remaining two SNPs (C22T y C50T) showed a low frequency with respect the global population. Conclusions: The low SNPs frequencies were unlikely to explain the difference in aspirin responsiveness between both groups of patients. The changes in alpha1-antitripsin could be linked with a different inflammatory state in these patients (AU)

FOXP2 es el primer gen ligado a una variante hereditaria del trastorno específico del lenguaje y parececodificar un represor transcripcional que interviene en la regulación del desarrollo y del funcionamiento de determinados circuitos corticotalamoestriatales. Desarrollo. En los últimos tres años se han producido significativos avances en la determinación de las propiedades estructurales y funcionales del gen, que conciernen fundamentalmente al análisis preciso delos motivos estructurales más importantes de la proteína que codifica y de los parámetros más relevantes que determinan la interacción de ésta con el ADN; al esclarecimiento de las propiedades funcionales y del comportamiento in vivo de las principales isoformas de la misma; a la determinación exacta del patrón de expresión de nuevos ortólogos del gen; y a la identificación de diferentes genes diana para el factor FOXP2. Conclusiones. Estas nuevas evidencias sugieren una gran versatilidad in vivo por parte de la proteína FOXP2 a la hora de unirse al ADN; la funcionalidad biológica de sus diversas isoformas; la implicación del gen FOXP2 en el desarrollo y/o el funcionamiento de los circuitos corticotalamoestriatales asociados a la planificación motora, el comportamiento secuencial y el aprendizaje procedimental, tanto en la fase adulta como durante el desarrollo embrionario; una significativa conservación en términos evolutivos del mecanismo regulador en el que interviene el gen; y la idoneidad de los modelos de procesamiento lingüístico que consideran que el lenguaje es, en buena medida, el resultado de una interacción entre determinadas estructuras corticales y subcorticales (AU)


FOXP2 is the first gene linked to a hereditary variant of specific language impairment and seems tocode for a transcriptional repressor that intervenes in the regulation of the development and the functioning of certain thalamic-cortical-striatal circuits. Development. In the last three years, significant progress has been made in the determination of the structural and functional properties of the gene. These advances essentially have to do with the precise analysis of the most important structural motifs of the protein that it codes for and the main parameters that determine its interaction with DNA. They also concern the determination of the functional and behavioural properties in vivo of the main isoforms of the FOXP2 protein, the exact determination of the pattern of expression of new orthologues of the gene, and the identification of the different target genes for factor FOXP2. Conclusions. This new evidence suggests that protein FOXP2 protein has a high degree of versatility in vivo when it comes to binding to DNA; that its different isoforms are biologically functional; and that the FOXP2 gene is functional during embryonic development and during the adult phase. It also suggests that it is involved in the development and/or functioning of the thalamic-cortical-striatal circuits associated to motor planning, sequential behaviour and procedural learning (a significant saving in developmental terms of the regulatory mechanism in which the gene is involved), as well as the accuracy of the models of linguistic processing that consider language to be, to a large extent, the result of an interaction between certain cortical and subcortical structures (AU)

OBJETIVOS: Valorar el papel actual del PSA como método de diagnóstico en el cáncer de próstatay analizar los avances con nuevos marcadores relacionadoscon dicho tumor.MÉTODOS: Revisamos en la literatura el valor del PSA como marcador para definir la presencia de cáncer de próstata, así como sus fórmulas moleculares. Las factores relacionados con sus modificaciones, modelos predictivoso las diferentes discrepancias en la utilidad sobre el nivel de corte para definir riesgo o como marcador en si mismo. Analizamos los posibles nuevos marcadores o líneas más interesantes de trabajo en el desarrollo de nuevos test. Utilizamos como vía de trabajo fundamental para la búsqueda bibliografica el Medline.RESULTADOS: Los datos disponibles confirman que el PSA mantiene un nivel alto de sensibilidad aunque la especificidad es baja especialmente en rango de PSA≤10 ngr/ml, se puede ver aumentada con sus diferentesisoformas moleculares, ratios o modelos predictivos.Si bien es verdad que a pesar de dichos estudios persiste la dificultad para aumentar la especificidad y por lo tanto obviar biopsias. En la actualidad se disponede nuevos marcadores, algunos comercializados y otros en vía de desarrollo como las nuevas isoformas o con biología molecular, que parecen mejorar la especificidaddel PSA.CONCLUSIONES: El PSA sigue siendo el marcador patrón para el diagnóstico del cáncer de próstata. Es importante mejorar la especificidad para lo que necesitamosnuevos modelos predictivos o nuevas isoformas que puedan ayudarnos a seleccionar mejor los pacientescandidatos a biopsia. Existen en este momento diferenteslíneas de investigación prometedoras con nuevos marcadores, si bien aún no existe sustituto ideal para el PSA que sigue siendo el patron estándar


OBJECTIVES: To evaluate the current role of PSA as a diagnostic method for prostate cancer, as well as to analyze possible new markers.METHODS: We perform a bibliographic review for PSA, and its molecular forms, as a marker to define the presence of prostate cancer. We review the factorsrelated to PSA modifications, predictive models, or the current controversies about the usefulness of its cutpoint to define the risk of prostate cancer or the marker itself. We analyze possible new markers and the most interesting work lines in the development of new markers. We used MEDLINE for the bibliographic search.RESULTS: Available data confirm that PSA has a high sensitivity; although specificity is low, mainly in the ≤ 10ng/ml range, it may be increased with the use of variousmolecular isoforms, ratios or predictive models.Nevertheless, it is true that despite such studies it isdifficult to increase specificity, so biopsies are reduced. Currently we have new markers, some of them already marketed, others in development, which seem to improve the specificity of PSA (isoforms, use of molecular biology).CONCLUSIONS: PSA is still the standard marker for the diagnosis of prostate cancer. It is important to improve the specificity; therefore we need new predictive models or new isoforms that help us to do a better selection of candidates for biopsy. There are various promising research lines with new markers, but there is not ideal substitute for PSA yet

Introducción. La repercusión económica y social de la encefalopatía espongiforme bovina (EEB) y su relación con la nueva variante de la enfermedad de Creutzfeldt-Jakob (nvCJD) hace interesante un estudio de la expresión y localización de la proteína priónica celular (PrPC) en el sistema nervioso central de la vaca, con especial atención al tronco del encéfalo, especialmente alterado en la EEB. Según varios autores la presencia PrPC es esencial para el establecimiento y replicación del prión (PrPSc), presente en la EEB. Material y métodos. Se utilizaron muestras de encéfalos de vacas de raza autóctona pirenaica. Se estudió la expresión de PrP mediante técnicas de wester blot, así como la localización de PrPC en el sistema nervioso central, con especial atención al tronco del encéfalo. Resultados. La expresión y cuantificación de PrP fue mayor en zonas rostrales que en caudales. La inmunohistoquímica fue concordante con el western. La localización de PrPC era importante en corteza cerebral, hipocampo, etc. Se destacó inmunopositividad notable en el núcleo motor dorsal del nervio vago y en el del nervio hipogloso. El asta anterior de la médula cervical era más inmunopositiva que el asta posterior. Conclusión. Existe una mayor abundancia de PrPC en zonas rostrales que en caudales del sistema nervioso central de la vaca. Debido a la presencia de PrPC en neuronas motoras, y de acuerdo con hallazgos en otras especies de mamíferos, la transmisión de la PrPSc se haría retrógradamente desde su vía de acceso y en el interior del sistema nervioso central (AU)


Introduction: The appearance of bovine spongiform encephalopathy (BSE) and its involvement in the generation of the new variant of Creutzfeldt-Jakob Disease (nvCJD) had important implications from a social and economical point of view. As the presence of PrPC is necessary to establish prion diseases and for the replication of PrPSC in BSE, the study of the expression and localization of PrPC in the central nervous system of cow, mainly the brainstem, is interesting itself. Material and methods: Expression of PrP by western blot technique and its precise localization by immunohistochemistry were analyzed in the central nervous system of the autochthonous pyrenees breed, a group of cows with a high incidence for BSE. Results: PrP was more abundantly expressed in rostral areas rather than caudal regions of the cow brain. Immunohistochemistry was congruent with western blotting studies. Localization of PrPC was particularly abundant in the cerebral cortex, hippocampus, etc. Noteworthy, immunopositivity was present in the dorsal motor nucleus of the vagus and the hypoglossal nucleus. The anterior horn of the spinal cord yielded higher labelling for PrP than the posterior horn. Conclusion: PrPC is more abundantly expressed in rostral areas of the central nervous system. According to the findings observed in other mammal species, presence of PrPC in motoneurons of cow indicates that PrPSc might be transported from its site of entry to the central nervous system following a retrograde direction (AU)

Se ha postulado que en las enfermedades neurodegenerativas,las alteraciones en los fenómenos de neuroplasticidady adaptación tienen una función muy importante,como desencadenante o motor del curso patogénico delas enfermedades. Para aclarar este extremo se ha llevadoa cabo un estudio morfohistoquímico comparativo en lacorteza cerebral prefrontal y en la corteza cerebelosa(neocerebelo) de cerebros de enfermos de Alzheimer yCreutzfeldt-Jakob (EA y ECJ). Se han analizado las variacionesen marcadores supuestamente relacionados confenómenos de neuroplasticidad sináptica (Drebrina, SNAP-25), factor de activación nuclear (NF kappa Beta -NFkB) eisoforma neuronal de la óxido nítrico sintasa (nNOS) juntoa marcardores de neuropatología (acumulación de proteínasbeta –amiloide en EA y PrPsc en ECJ-, reacción microgliale inducción de enzimas pro-inflamatorias iNOS yciclo-oxigenasa 2 (COX-2). Los resultados han mostradograndes variaciones de los marcadores entre ambos procesospatológicos, entre las cortezas cerebral y cerebelosa,entre diferentes áreas de esas regiones y entre diferentesestirpes neuronales y gliales. El significado de algunosde los marcadores (NFkB, nNOS, proteínas sinápticas) puede ser variable (plástico o involutivo) según los casos(enfermedad, región, factores). La neuroplasticidad disminuyede manera diversa según las áreas cerebrales y susrelaciones con las manifestaciones neuropatológicas tambiénson variables, aunque son manifiestos fenómenos deneuroplasticidad/adaptación en muchas áreas y neuronas.Se concluye que la activación de los supuestos marcadoresde neuroplasticidad en fases avanzadas de la enfermedad,con una idea terapéutica, puede activar fenómenosinvolutivos en algunas regiones o neuronas del cerebro


Changes in neuroplasticity and neuronal adaptativemechanisms have been postulated as origin and/or mainpathophysiological factor in neurodegenerative diseases.To analyze these theories, a comparative morphohistochemicalstudy on the cerebral (prefrontal) and cerebellar(neocerebellar) cortex from Alzheimer´s (AD) and Creutzfeldt-Jakob (CJD) post-mortem brains has been carriedout. Variations in putative markers of neuroplasticity andneuronal adaptation (synaptic proteins such as drebrinand SNAP-25; nuclear factor NF kappa Beta –NFkB-; neuronalisoform of oxide nitric synthase -nNOS) have beenstudied in close association with neuropathological markers(beta-protein deposition – amyloid in AD and PrPsc inCJD-; microglial activation, induction of iNOS and cyclooxygenase2 –COX-2). Results have shown sharp variationsin these markers when compared AD and CJD; cerebraland cerebellar cortex; different areas of these anatomicalregions; and different sets of neurons and glial cells.The meaining of somme of these markers (NFkB; nNOS;synaptic proteins) could be variable (plastic/adaptative orinvolutive), depending on different factors (disease, anatomicalregion, general or local factors, etc.). Neuroplasticity is evident in several brain regions or neurons, but this neuronalfeature decreases in different form depending alsoon the disease and the anatomical region. Their relationshipsto the neuropathological findings were also variable.In conclusion, the activation of these putative markers ofneuroplasticity, considering as therapeutical targets, inadvanced steps of the diseases, could activate neuronalinvolutive phenomena in several regions or neurons

Objetivo: Analizar la relación entre el éxito de la inducción del parto y la presencia, en la secreción cervical, de la forma altamente fosforilada de la proteína transportadora del factor de crecimiento similar a la insulina tipo I (IGFBP-1). Sujetos y métodos: Se estudió a 102 pacientes ingresadas para inducción del parto entre las semanas 37 y 41 con gestaciones únicas y presentación cefálica. Se realizó una historia obstétrica, examen físico, ecografía transvaginal y determinación de la isoforma altamente fosforilada de la IGFBP-1 (Actim ® PARTUS). El éxito de la inducción se definió como el parto vaginal dentro de las 24 h siguientes al inicio del procedimiento. Se realizó un análisis de regresión logística para identificar las variables que se asociaron de manera independiente con el éxito de la inducción. Resultados: El éxito en la inducción se produjo en 56 (65,1%) casos. El test Actim ® PARTUS resultó positivo en el 67,4% de los casos, y mostró una sensibilidad del 76,8% y una especificidad del 50%. El análisis de regresión logística mostró que la única variable que se asoció de modo independiente con el éxito de la inducción fue la paridad. El resto de covariables analizadas, incluyendo el test Actim ® PARTUS, no mostraron una relación independiente con el éxito de la inducción. Conclusiones: La presencia en la secreción cervical de la forma altamente fosforilada de la IGFBP-1 no se relaciona de modo independiente con el éxito de la inducción


Objective: To evaluate the relationship between successful induction of labor and the presence of the highly phosphorylated isoform of type-I insulin-like growth factor binding protein-1 (IGFBP-1) in the cervical fluid. Subjects and methods: A total of 102 women with singleton vertex pregnancies undergoing induction of labor between 37 and 41 completed weeks of gestation were studied. Obstetric history, physical examination, transvaginal ultrasound and determination of the highly phosphorylated isoform of IGFBP-1 (Actim TM PARTUS) were performed. Successful labor induction was defined as vaginal delivery within 24 hours of the procedure. Logistic regression modeling was performed to identify which variables were independently associated with successful induction. Results: Labor induction was successful in 56 patients (65.1%). An Actim TM PARTUS positive result was observed in 67.4% of the patients, with a sensitivity of 76.8% and a specificity of 50%. On logistic regression, the only variable independently associated with the outcome of labor induction was parity. The other covariates, including the Actim TM PARTUS test, did not show a significant association. Conclusions: The presence of the highly phosphorylated isoform of IGFBP-1 in the cervical fluid is not an independent factor associated with the success of labor induction

Objetivos. En esta revisión se pretende describir los conocimientos actuales de los receptores neuronales nicotínicos de la acetilcolina (nAChR) y su implicación en la patogénesis de diversos trastornos neuropsiquiátricos, así como las propiedades de los recién sintetizados nAChR de subtipo selectivo. Se revisan también los posibles blancos terapéuticos cerebrales para los nAChR. Desarrollo. Existen múltiples tipos de estos receptores, cada uno con diversas estructuras y unas propiedades farmacológicas y funcionales individuales. En contraste con los nAChR musculares, las funciones fisiológicas de los nAChR aún no han sido bien definidas. Además, la mayor parte de los datos que definen los posibles blancos terapéuticos que implican los nAChR provienen de estudios sobre los efectos de la nicotina en varios modelos preclínicos y, aunque en menor grado, clínicos. Las investigaciones preclínicas siguen centrándose en la nicotina y en los últimos años se han sintetizado diversos agonistas nuevos de los nAChR que podrían tener un valor terapéutico potencial en diversas patologías neurológicas y psiquiátricas. Conclusiones. La estructura de los nAChR y la elevada diversidad molecular en las subunidades facilitan la posibilidad de que exista un gran número de subtipos de nAChR, que, en base a los precedentes farmacológicos, pueden desempeñar diversas funciones discretas en el cerebro y por lo tanto representar blancos nuevos para las sustancias terapéuticas. Una tendencia esperanzadora en la terapéutica es la síntesis de nuevos agonistas con un elevado grado de selectividad de subtipos de nAChR, los cuales no parecen tener los efectos adversos de la nicotina y sí que exhiben una actividad beneficiosa clara en los trastornos neuropsiquiátricos (AU)


Objectives. This review will be attempt to describe the current knowledge on the neuronal nicotinic acetylcholine receptors (nAChRs) and their involvement in the pathogenesis of a number of neuropsychiatric disorders and the properties of recently synthesized subtype-selective nAChRs. The potential brain therapeutic targets for nAChRs are reviewed. Development. The nAChRs belong to a family of ligand-gated channels which are widely distributed in brain. Multiple subtypes of these receptors exist, each with diverse structures, individual pharmacological and functional properties. In contrast to the muscular nAChRs, the physiological functions of nAChRs are not well defined to date. In addition, the majority of evidence defining potential therapeutic targets involving nAChRs has resulted from studies on the effects of nicotine in a variety of preclinical and, to a lesser extent, clinical models. The preclinical research continues to focus on nicotine and a number of novel nAChRs agonists have been synthesized in the last few years that may have therapeutic potential in a number of neurological and psychiatric conditions. Conclusions. The structure of nAChRs and the considerable molecular diversity in subunits offers the possibility of a large number of nAChRs subtypes, which, based on pharmacological precedent, may serve a variety of discrete functions within the brain and thus represent novel targets for therapeutic agents. A promising trend for therapy is the synthesis of new agonists with high nAChRs subtype selectivity, which do not exhibit nicotine’s side effects and do show clear beneficial actions in the neuropsychiatric disorders (AU)

RESUMO

- Propósito: el CD44v6 es una isoforma variante del CD44 involucrada en la diseminación metastática de ciertos tumores. Nosotros hemos querido analizar su expresión en carcinomas escamosos de pulmón y correlacionarla con otros parámetros clínico-biológicos de utilidad clínica.- Material y método: el CD44v6 fue determinado mediante un EIA (Bender Diagnostics. Austria), cuyo dintel de positividad fue establecido en 5 ng/mg prot. En las membranas celulares determinamos, asimismo, las concentraciones del receptor del factor de crecimiento epidérmico (EGFR) y proteína erbB2. También se ha tenido presente el estadio clínico, grado histológico (GH), ploidía y fase de síntesis celular (FS).- Resultados: en los carcinomas escamosos de pulmón, la positividad para el CD44v6 fue apreciada en 76 de los 95 casos (80 por ciento). Los tumores CD44v6 positivos presentaron mayores concentraciones de EGFR (p:0,007), proteína erbB2 (p:0,056) y valores globales de FS (p:0,011). Asimismo, cursaron más frecuentemente con FS + (>15,4 por ciento) (p:0,041).- Conclusiones: los resultados anteriores nos inducen a las siguientes consideraciones: 1) la expresión de CD44v6 se observa en un alto porcentaje de los carcinomas escamosos de pulmón; 2) Su asociación con altas concentraciones de EGFR, proteína erbB2 y una proliferación celular elevada sugieren que pueda reflejar un peor comportamiento y evolución de estos tumores (AU)
