RESUMO

Background: Sesame is a traditional oilseed comprising essential amino acids. However, the presence of allergens in sesame is a significant problem in its consumption; thus, this study attempted to reduce these allergens in sesame oilseeds.Objective: The present study aimed to evaluate the effect of cold plasma processing on structural changes in proteins, and thereby the alteration of allergenicity in sesame milk. Method: Sesame milk (300 mL) was processed using atmospheric pressure plasma bubbling unit (dielectric barrier discharge, power: 200 V, and airflow rate: 16.6 mL/min) at different exposure times (10, 20, and 30 min).Results: The efficiency of plasma-bubbling unit as measured by electron paramagnetic resonance in terms of producing reactive hydroxyl (OH) radicals proved that generation of reactive species increased with exposure time. Further, the plasma-processed sesame milk subjected to sodium dodecyl sulfate–polyacrylamide gel electrophoresis and differential scanning calorimetery analysis revealed that plasma bubbling increased the oxidation of proteins with respect to bubbling time. The structural analysis by Fourier transform infrared spectroscopy and circular dichroism revealed that the secondary structure of proteins was altered after plasma application. This change in the protein structure helped in changing the immunoglobulin E (IgE)-binding epitopes of the protein, which in turn reduced the allergen-binding capacity by 23% at 20-min plasma bubbling as determined by the sandwich-type enzyme-linked immunosorbent assay. However, 30-min plasma bubbling intended to increase allergenicity, possibly because of increase in IgE binding due to the generation of neo epitopes.Conclusion: These changes proved that plasma bubbling is a promising technology in oxidizing protein structure, and thereby reducing the allergenicity of sesame milk... Background: Allergen products for subcutaneous immunotherapy (SCIT) contain intact allergen extracts or chemically modified allergoids. Chemical modification was introduced to reduce allergenicity while retaining immunogenicity and thereby enable safer and more efficient allergy immunotherapy. Methods: Experimental allergoids were produced from intact allergen extract for birch, grass, and house dust mite (HDM) to evaluate the effects of chemical modification. Preparations were compared with commercial allergoids and analyzed using SDS-PAGE/immunoblotting, IgE-inhibition assays, and crossed immunoelectrophoresis (CIE). Dermatophagoides pteronyssinus (Der p) vaccines were also tested for protease activity and immunizing capacity in a mouse model. Results: The composition of IgE-binding epitopes in allergoids differed from that of intact allergen vaccines. Birch and grass allergoids produced smears of protein aggregates on SDS-PAGE, whereas intact allergen preparations showed distinct protein bands as expected. Der p allergoid vaccines, however, showed a distinct protein band corresponding to major allergen Der p 1 in both SDS-PAGE and CIE analysis, and commercial Der p allergoid vaccines showed Der p 1–related cysteine protease activity. Conclusion: Allergoids and intact allergen preparations differ with respect to the composition of IgE-binding epitopes. However, chemical cross-linking does not affect every allergen molecule to the same degree. Der p 1, for example, remains largely unmodified. Furthermore, the investigational HDM allergoid vaccines showed reduced and delayed immune responses when used for immunization of mice (AU)


Antecedentes: Los productos de alérgenos para inmunoterapia subcutánea (SCIT) contienen extractos de alérgenos intactos o alergoides modificados químicamente. En este trabajo se ha hecho una modificación química para reducir la alergenicidad a la vez que se conservaba la inmunogenicidad, y por lo tanto, permitir una inmunoterapia más segura y eficiente. Métodos: Se produjeron alergoides experimentales a partir de extracto de alérgeno intacto para abedul, hierba y ácaros del polvo doméstico (HDM) y se evaluaron los efectos de la modificación química realizada. Las preparaciones se compararon con alergoides comerciales y se analizaron mediante SDS-PAGE/inmunotransferencia, ensayos de inhibición de IgE e inmunoelectroforesis cruzada (CIE). Las vacunas de Dermatophagoides pteronyssinus (Der p) también se probaron para determinar la actividad de la proteasa y la capacidad de inmunización en un modelo de ratón. Resultados: La composición de los epítopos de unión a IgE en los alergoides difería de las vacunas de alérgenos intactas. Los alergoides de hierba y abedul produjeron manchas de agregados de proteínas en el SDS-PAGE, mientras que las preparaciones de alérgenos intactos mostraron distintas bandas de proteínas como se esperaba. INTRODUCTION AND OBJECTIVES: Allergen-specific immunotherapy (ASIT) is the only allergic disease-modifying therapy available for children and adults, and recombinant allergens are an interesting approach to improve allergy diagnosis and ASIT. Tyrophagus putrescentiae is a common storage mite that produces potent allergens. The aim of this study was to express and characterize recombinant group 4 allergen protein of T. putrescentiae (Tyr p 4), and to further investigate allergenicity and potential epitopes of Tyr p 4. MATERIALS AND METHODS: The cDNA encoding Tyr p 4 was generated by RT-PCR and subcloned into pET-28a(+) plasmid. The plasmid was then transformed into E. coli cells for expression. After purification by nickel affinity chromatography and identification by SDS-PAGE, recombinant Tyr p 4 protein was used for a skin prick test and an ELISA to determine the allergic response. RESULTS: Study participants' allergic response rate to Tyr p 4 protein was 13.3% (16/120). Eight B-cell epitopes and three T-cell epitopes of Tyr p 4 were predicted. CONCLUSIONS: Similar to group 4 allergens of other species of mite, allergenicity of Tyr p 4 is weak. The expression, characterization and epitope prediction of recombinant Tyr p 4 protein provide a foundation for further study of this allergen in the diagnosis and ASIT of storage mite allergy

INTRODUCCIÓN: La transfusión isogrupo ABO D es la práctica transfusional habitual, sin embargo, cuando no se dispone de sangre D negativo puede ser preciso transfundir concentrados de hematíes D positivo a pacientes D negativo. Estos pacientes pueden desarrollar aloanticuerpos anti-D en los siguientes 3 meses a la exposición. MATERIAL Y MÉTODO: En los últimos 18 años, hemos experimentado situaciones de escasez de sangre D negativo, en las que hemos aplicado un protocolo clínico asistencial, seleccionando s los pacientes según el riesgo de aloinmunización y de requerimientos transfusionales crónicos. Se realizó una recogida prospectiva de estos pacientes, analizando principalmente la aloinmunización y la mortalidad. RESULTADOS: Tras las aplicación del protocolo se han transfundido unidades D positivo al 3% de los pacientes D negativo, con una tasa de aloinmunización conocida del 12,3%, siendo mayor en los pacientes más jóvenes y en aquellos que han recibido mayor número de unidades. No se detectaron complicaciones secundarias a la inmunización y la mortalidad en este grupo fue menor. CONCLUSIÓN: La transfusión de hematíes D positivo a pacientes D negativo no solo es una práctica segura para pacientes seleccionados, sino que además permite optimizar el uso de un producto en situaciones de escasez


BACKGROUND: To transfuse packed red blood cells isogroup ABO D is a usual transfusion practice. However, when there is not enough D negative blood available, we can transfuse positive red blood cells to negative patients. Immunocompetent D negative individuals may develop serologically detectable anti-D antibodies within 3 months after exposure to D positive red blood cells. MATERIAL AND METHOD: Over the last 18 years, we have experienced situations of D negative blood cell scarcity. In these situations, we have applied a clinical assistance protocol, selecting patients with lower risk of alloimmunization and chronic transfusion requirements. We have retrospectively evaluated this policy for the use of D positive red blood cells in D negative patients, focussing on alloinmunization and mortality. RESULTS: Applying the protocol, 3% of D negative patients were transfused with D positive units, with an alloimmunization rate of 12.3%. The rate of alloimmunization was higher in the younger age group and in those transfused with more units. No haemolytic reactions were reported. Mortality in the alloimmunized group was lower. CONCLUSION: The use of D positive red blood cells in selected D negative patients does not induce adverse reactions, is a safe practice and allows saving of a product that is sometimes limited

OBJETIVO: Evaluar la asociación del epítopo compartido, el tabaquismo y la interacción entre ambos sobre la presencia de autoanticuerpos (antipéptidos cíclicos citrulinados [anti-PCC] y factor reumatoide) en pacientes con artritis reumatoide en nuestra área geográfica. MÉTODOS: Estudio descriptivo y transversal realizado en una cohorte de 106 pacientes diagnosticados de artritis reumatoide. Análisis estadístico univariante y multivariante mediante regresión logística ordinal. Se calcularon odds ratios (OR) con un intervalo de confianza del 95% [IC95%] y se considero significativo un valor de p < 0,05. RESULTADOS: En el análisis univariante, el epítopo compartido (OR=2,68; IC95% 1,11-6,46), el hábito tabáquico (OR=2,79; IC95% 1,12-6,97) y un índice de tabaquismo en paquetes-año alto (>20 paquetes/año) (OR=8,93; IC95% 1,95-40,82) se asociaron con la presencia de anti-PCC positivos. Para el factor reumatoide, la asociación solo fue significativa con el hábito tabáquico (OR=3,89; IC95% 1,06-14,28) y el índice de tabaquismo (OR=8,33; IC95% 1,05-66,22). Mediante análisis de regresión logística ordinal se identificó asociación con títulos elevados de anti-PCC (>200 U/mL) en mestizos latinoamericanos, ser homocigoto para el epítopo compartido, tener factor reumatoide positivo y ser gran fumador. CONCLUSIONES: El ser mestizo latinoamericano, tener epítopo compartido, factor reumatoide y un índice de tabaquismo>20 paquetes/año son factores de riesgo independientes para el desarrollo de artritis reumatoide con anti-PCC positivos a títulos elevados (>200U/mL). En los pacientes portadores del epítopo compartido, la intensidad del consumo de tabaco se asocia más fuertemente que el hábito de fumar con un riesgo incrementado de anti-PCC positivos, observándose una interacción entre ambos factores


OBJECTIVES: To evaluate the association of shared epitope, smoking and their interaction on the presence of autoantibodies (anti-cyclic citrullinated peptide [CCP] antibodies and rheumatoid factor) in patients with rheumatoid arthritis in our geographical area. METHODS: A descriptive and cross-sectional study was carried out in a cohort of 106 patients diagnosed with RA. Odds ratios (OR) for antibody development were calculated for shared epitope, tobacco exposure and smoking dose. Statistical analysis was performed with univariate and multivariate statistics using ordinal logistic regression. Odds ratios were calculated with 95% confidence interval (95% CI) and a value of P<.05 was considered significant. RESULTS: In univariate analysis, shared epitope (OR=2.68; 95% CI: 1.11-6.46), tobacco exposure (OR=2.79; 95% CI: 1.12-6.97) and heavy smoker (>20 packs/year) (OR=8.93; 95% CI: 1.95-40.82) were associated with the presence of anti-CCP antibodies. For rheumatoid factor, the association was only significant for tobacco exposure (OR=3.89; 95% CI: 1.06-14.28) and smoking dose (OR=8.33; 95% CI: 1.05-66.22). By ordinal logistic regression analysis, an association with high titers of anti-CCP (>200U/mL) was identified with South American mestizos, patients with homozygous shared epitope, positive FR and heavy smokers. CONCLUSIONS: Being a South American mestizo, having a shared epitope, rheumatoid factor positivity and a smoking dose>20 packs/year are independent risk factors for the development of rheumatoid arthritis with a high titer of anti-CCP (>200U/mL). In shared epitope-positive rheumatoid arthritis patients, the intensity of smoking is more strongly associated than tobacco exposure with an increased risk of positive anti-CCP

Objective: To investigate the prevalence of and factors associated with the presence of alfa-gal-specific IgE in a risk group of foresters and forest workers from La Rioja, Spain and in a control group. Methods: The study population comprised 169 workers and 100 individuals who did not recall having had tick bites. A questionnaire including demographic data and number of tick bites per year was completed by a physician. alfa-Gal sIgE was assessed using ImmunoCAP with serum samples that had been taken in 2010. In 2015, second serum specimens were taken from all but 1 of the workers, who had positive specific IgE to alfa-gal in 2010. These new samples were tested for IgE to the alfa-gal epitope and to mammalian meat. Results: The prevalence of positive sIgE to alfa-gal was 15% in the risk population and 4% in the control population. alfa-Gal sIgE positivity was associated with the number of tick bites per year and with seniority. Thirteen out of 21 patients sensitized to alfa-gal in 2010 showed positive specific IgE to alfa-gal in serum samples from 2015. Eleven had specific IgE to mammalian meat, but none reported symptoms of meat allergy. Conclusion: The prevalence of alfa-gal sIgE antibodies in this risk population was higher than in the control group and was associated with the number of tick bites per year and with seniority. None of the workers sensitized to mammalian meat developed meat allergy, possibly owing to the low levels of sIgE to alfa-gal


Objetivo: Estudiar la prevalencia de IgE específica a alfa-gal y los factores asociados en el grupo de riesgo de agentes forestales y retenes de incendios de La Rioja respecto a un grupo control. Métodos: Se incluyeron 169 trabajadores y 100 personas que referían no haber sufrido picaduras de garrapata. Cumplimentaron un cuestionario dirigido en el que se recogieron datos demográficos y número de picaduras de garrapata al año. Se realizó determinación de IgE específica a alfa-gal mediante la ténica InmunoCAP utilizando muestras serológicas extraídas en 2010. En 2015 se llevó a cabo nueva extracción serológica a todos los trabajadores sensibilizados a α-gal en muestra de 2010 excepto a uno. Se realizó nueva determinación de IgE específica a alfa-gal y a carne de mamíferos. Resultados: La prevalencia de IgE específica a alfa-gal en la población de riesgo fue del 15% vs. 4% en el grupo control. La positividad de la IgE específica a alfa-gal se asoció con el número de picaduras al año y con la antigüedad el puesto de trabajo. Trece de los pacientes sensibilizados en 2010 seguían sensibilizados en 2015. Once de ellos mostraban IgE específica frente a carnes sin manifestar síntomas de alergia. Conclusión: La prevalencia de IgE específica a alfa-gal en población de riesgo española fue superior a la del grupo control y se asoció al número de picaduras-año y a la antigüedad en el puesto de trabajo. Ninguno de los trabajadores sensibilizados a carnes, desarrolló alergia, probablemente por los niveles discretos de IgE específica a alfa-gal

Introduction: mites allergic asthma is caused by exposure to home dust mite (HDM). Der f 3 is believed to be one of the major allergens in mites allergic asthma. The work was to identify the immune characteristics of Der f 3 epitope-based vaccine containing T cell and B cell epitopes. Methods: T cell lines were generated from peripheral blood mononuclear cells of Der f 3 allergic patients. Three T cell epitopes and five B cell epitopes of Der f 3, which we identified previously, were selected to design a polypeptide (named Der f3-peptides). DNA constructions encoding these Der f 3-peptides were expressed in Escherichia coli. The T cell lines were stimulated with the peptides and tested for proliferative capacity and cytokine production. Results: plasmid pET28a (+)-Der f 3-peptides was constructed and expressed in E. coli BL21, and the Der f3-peptides protein was purified and confirmed by Western blotting. The Der f 3-peptides were recognized by the T cell clones from allergic patients. SI value of Der f 3 group and Der f 3-peptides group were both higher than that of PBS group (P< 0.05). Conclusions: our results demonstrate that several major T cell epitopes and B cell epitopes of Der f 3 can be valuable for designing the peptide-based immunotherapeutics for the mites allergic asthma (AU)


Introducción: el asma alérgica está causada por la exposición a los ácaros del polvo casero (HDM). Der f 3 se cree que es uno de los principales alérgenos en los ácaros del asma alérgica. El trabajo consistió en identificar las características inmunológicas de la vacuna basada en epítopo-Der f 3 que contienen las células T y las células B. Métodos: se generaron líneas de células T a partir de células mononucleares de sangre periférica de pacientes alérgicos a Der f 3. Tres epítopos de células T y cinco epítopos de células B de Der f 3, que hemos identificado previamente, fueron seleccionados para diseñar un polipéptido (denominados péptidos Der f 3). Construcciones de DNA que codifican estos péptidos Der f 3 se expresaron en Escherichia coli. Las líneas de células T se estimularon con los péptidos y se utilizaron en el ensayo por su capacidad proliferativa y la producción de citoquinas. Resultados: el plásmido pET28a (+) - Der f 3-péptidos se construyó y se expresaron en E. coli BL21, y la proteína de Der f 3-péptidos se purificó y se confirmaron mediante transferencia de Western. Los Der f 3-péptidos fueron reconocidos por los clones de células T procedentes de pacientes alérgicos. Valor SI de Der f 3 grupo y f grupo 3-péptidos Der eran tanto mayor que la del grupo de PBS (P <0,05). La capacidad de unión a IgE a Der f 3-péptidos (41, 25 ± 5, 67) μg/ml se redujo drásticamente en comparación con el de Der f 3 (83,60 ± 10,92) μg/ml (P <0,05). Conclusiones: nuestros resultados demuestran que varios de los principales epítopos de células T y de células B de Der f 3 pueden ser valiosos para el diseño de agentes inmunoterapéuticos basados en péptidos para los ácaros del asma alérgica (AU)

Recientes e importantes avances en los campos de la inmunología, genómica, genómica funcional, inmunogenética, inmunogenómica, bioinformática, microbiología, ingeniería genética, biología de sistemas, bioquímica sintética, proteómica, metabolómica y nanotecnología, entre otros, facilitan nuevos enfoques en el desarrollo de vacunas. La mejor identificación de epítopos ideales, la potenciación de la respuesta inmunitaria gracias a los nuevos adyuvantes o la búsqueda de nuevas vías de administración son buenos ejemplos de avances que ya son una realidad y que favorecerán el desarrollo de más vacunas, su uso en grupos poblaciones indicados o el abaratamiento de su producción. Actualmente hay en desarrollo más de 130 nuevas vacunas frente a las enfermedades infecciosas más deseadas (malaria y VIH), las que más dificultades están planteando (CMV o VRS), graves infecciones reemergentes (dengue o ebola), enfermedades parasitarias cada vez más presentes en nuestro medio (enfermedad de Chagas o leishmaniasis), o emergentes infecciones bacterianas nosocomiales (Clostridium difficile o Staphylococcus aureus)


Recent and important advances in the fields of immunology, genomics, functional genomics, immunogenetics, immunogenomics, bioinformatics, microbiology, genetic engineering, systems biology, synthetic biochemistry, proteomics, metabolomics and nanotechnology, among others, have led to new approaches in the development of vaccines. The better identification of ideal epitopes, the strengthening of the immune response due to new adjuvants, and the search of new routes of vaccine administration, are good examples of advances that are already a reality and that will favour the development of more vaccines, their use in indicated population groups, or its production at a lower cost. There are currently more than 130 vaccines are under development against the more wished (malaria or HIV), difficult to get (CMV or RSV), severe re-emerging (Dengue or Ebola), increasing importance (Chagas disease or Leishmania), and nosocomial emerging (Clostridium difficile or Staphylococcus aureus) infectious diseases

Several Leishmania antigens have been characterized and assayed, as recombinant products, in vaccination trials using different experimental models of leishmaniasis. Vaccination strategies have been developed to induce adequate cellular responses against a great number of antigenic proteins. Spanish groups, working alone or, in many cases, in collaborative research projects have contributed to gather and get an important level of information about families of antigenic protein properties and procedures for immune intervention. Varios antígenos de Leishmania han sido caracterizados y probados, como productos recombinantes, en ensayos de vacunación empleando diferentes modelos experimentales de leishmaniosis con objeto de inducir respuestas celulares adecuadas, ademas, de protección contra la infección. Diferentes grupos de investigación españoles, trabajando solos o, en muchos casos, en proyectos de investigación cooperativos, han contribuido a reunir un importante nivel de conocimiento sobre diferentes familias de proteínas antigénicas y procedimientos de intervención inmunitaria. Algunos de estos temas se discuten en la presente revisión

Introducción: El Streptococcus pyogenes es la causa bacteriana más importante de faringoamigdalitis aguda. Se analizó la validez de una prueba antigénica rápida para su diagnóstico, en consultas de atención primaria; asimismo se identificó el perfil clínico con mejor rendimiento diagnóstico, se cuantificó el uso innecesario de antibióticos cuando se utilizaba la prueba o solamente diagnóstico clínico y se determinó la sensibilidad microbiana a penicilina, eritromicina y clindamicina. Pacientes y métodos: Estudio descriptivo transversal a niños/as entre 2-14 años con amigdalitis y/o faringitis aguda atendidos en 5 consultas, desde enero de 2008 hasta mayo de 2010. Tras el diagnóstico clínico se realizó frotis faringoamigdalar con 2 hisopos: con el primero se realizó una prueba antigénica rápida y con el segundo cultivo y sensibilidad antibiótica, siendo su análisis ciego al resultado de la prueba. Se ha previsto una muestra de 546 personas y muestreo consecutivo. Resultados: Se incluyó a 192 pacientes. La prevalencia de Streptococcus pyogenes fue del 38,7%(IC del 95%: 31,4-45,7). La odinofagia y el exantema escarlatiniforme fueron más probables con cultivo positivo. En el 100% hubo sensibilidad a la penicilina, en el 97,3% a eritromicina y en el 86,3% a clindamicina. La especificidad de la prueba fue del 91,5% y el valor predictivo negativo del 91,5%. El 49,2% de quienes recibirían antibióticos por sospecha clínica serían tratados innecesariamente, disminuyendo al menos un 29,5% con la prueba. Conclusiones: La prueba permitió un uso más adecuado de antibióticos. Parece recomendable su uso en pediatría de atención primaria, sin necesidad de confirmación con cultivo cuando resulte negativa, en los centros con difícil acceso al laboratorio(AU)


Introduction: Streptococcus pyogenes is the most frequent bacterial cause of acute tonsillopharyngitis. The validity of the rapid antigen test was analysed for its diagnosis in a Paediatric Primary Care setting. The clinical profile with better diagnostic yield was also identified. The unnecessary use of antibiotics was quantified when the rapid antigen test or only the clinical diagnosis was used. The sensitivity of the assay to penicillin, erythromycin and clindamycin was also determined. Patients and methods: Cross-sectional study was conducted on children between 2 to 14 years with acute tonsillitis and/or pharyngitis seen in five Primary Care Centres, from January 2008to May 2010. After a clinical diagnosis, two swabs were taken for pharyngotonsillar smears: the first was used for a rapid antigen test, and the second one for a culture and a study of antibiotic sensitivity, with its analysis being blind to the rapid test result. A total sample of546 consecutive was envisaged and with consecutive sampling. Results: A total 192 patients were included. The prevalence of Streptococcus pyogenes was38.7% (95% CI: 31.4-45.7). Odynophagia and scarlatiniform rash were most likely with positive cultures, the Streptococcus pyogenes was sensitive to penicillin in 100%, to erythromycin in97.3% and to clindamycin in 86.3%. The specificity of the rapid antigen test was 91.5% and with a Negative Predictive Value of 91.5%. About half (49.2%) of those who would have receive antibiotics for clinical suspicion would have been treated unnecessarily, with this decreasing to at least in 29.5% when using the rapid antigen test. Conclusions: The rapid antigen test can lead to a better use of antibiotics. Its use in Paediatric Primary Health Care could be useful when, as when the result is negative, there would be no need to confirm by a culture, in those Health Centres with difficult access to laboratory(AU)

Objetivo. El siguiente trabajo se desarrollo con el objetivo de determinar los perfiles antigénicos celulares que sean reconocidos por el suero de personas vacunados con vax-SPIRAL® de cepas obtenidas de los departamentos de León y Chinandega.Métodos. Se analizaron cepas aisladas de hemocultivos tomadas de casos con sospecha clínica de leptospirosis, determinándose los perfiles antigénicos mediante electroforesis de células enteras y western blotting usando como anticuerpos, sueros provenientes de personas vacunadas con vax-SPIRAL®.Resultados y conclusiones. Se evidenció una gran homología antigénica entre todas las cepas estudiadas. Los resultados del presente estudio sugieren que en los departamentos de León y Chinandega circulan cepas de L. interrogans con un fuerte reconocimiento antigénico frente a sueros provenientes de personas vacunadas con vax-SPIRAL®(AU)


Objective.The following work was development with the objective of determining the profiles cellular antigenics that are recognized by the serum of people vaccinated with vax-SPIRAL® of obtained strains of the León and Chinandega departments. Methods. Blood cultures samples were analyzed taken in sharp phase to cases with suspicion leptospirosis clinic, being determined the profiles antigenics by means of electrophoresis of whole cells and western blotting using as antibodies, serums coming from people vaccinated with vax-SPIRAL®. Results and conclusions. A great antigenic homology was evidenced among all the studied strains. The present study results suggest that in León and Chinandega departments circulate L. interrogans strains with a strong antigenic recognition front of serums of people vaccinated with vax-SPIRAL®(AU)

Las células T autoreactivas CD8+ se están empezando a reconocer como contribuidores importantes en distintos modelos de autoimmunidad; el papel que estas células juegan en enfermedades autoimmunes humanas tan sólo se está empezando a comprender. Estas células pueden actuar bien como ejecutoras de daño tisular, bien comoreguladoras y, por lo tanto, ejercer un efecto anti-patogénico. Recientes avances en este campo incluyen una mejor comprensión de los mecanismos que controlan su activación, de sus mecanismos de acción, de su regulación negativa, y de sus especificidades antigénicas. Una contribución de especial importancia es el desarrollo de métodos analíticos que permiten la detección de estas especificidades antigénicasen pacientes humanos e individuos con riesgo genético a padecer enfermedades autoimmunes. Colectivamente, estos avances ofrecen esperanzas de cara al desarrollo de nuevas modalidades terapéuticas capaces de atacar selectivamente el compartimento celular CD8+ en los procesos autoimmunes


Autoreactive CD8+ T cells are emerging as important players in several animal models of autoimmunity; their roles in the human autoimmune diseases are only beginning to be understood. Autoreactive CD8+ T cells can act as pathogenic effector cells and mediatetissue damage, while other autoreactive CD8+ T cells may have regulatory properties and serve to protect self against autoimmune attacks. Recent advances made in the field include a better understanding of their activation, mechanism of action, negative regulation, and antigenic specificities. Importantly, advances have been made in the development of strategies that detect these antigenic specificities in human patients and at risk individuals. Together, these findings foster hopefor the development of novel antigen-specific therapeutic strategies that target the CD8+ T cell compartment of autoimmune processes

RESUMO

El avance de la epidemia de sida ha convertido la obtención de una vacuna eficaz frente al virus de la inmunodeficiencia humana (VIH) como un objetivo científico prioritario. En el momento actual no disponemos de una vacuna preventiva frente a la infección por el VIH y en ningún modelo animal se ha conseguido la protección frente a la infección. En esta revisión se analizan las dificultades existentes en el desarrollo de una vacuna contra el sida, en especial los mecanismos de escape viral a la respuesta inmunitaria y se describen los prototipos de vacunas preventivas y terapéuticas en desarrollo y los resultados obtenidos. Por otra parte se sitúa esta investigación en el contexto sanitario, económico y social de la pandemia de sida y se analizan las polémicas actualmente planteadas en el desarrollo de ensayos clínicos con los diferentes tipos de vacunas


The uncontrolled progression of the aids epidemic has made the development of an efficacious human immunodeficiency virus (HIV) vaccine a major objective of scientific research. No effective preventive vaccine against HIV is currently available and sterilizing immunity has not yet been achieved in animal models. This review analyses the major challenges in developing an aids vaccine, in particular the mechanisms involved in viral escape from the immune response, and summarizes the results obtained with the different prototypes of therapeutic and preventive vaccines. Finally, social, economic and healthcare aspects of research into HIV vaccines and current controversies regarding the development of clinical trials are discussed

La diabetes tipo 1 (DT1) es una enfermedad autoinmunitariacausada por la destrucción de las células beta pancreáticas productorasde insulina. La patogénesis de DT1 es compleja, interviniendofactores genéticos, inmunológicos y ambientales. Lasinfecciones víricas serían uno de los factores ambientales que, juntamentecon la susceptibilidad genética, podrían estar implicadosen la patogénesis de la enfermedad. En la presente revisión, resumimoslas hipótesis de la contribución de los virus en las enfermedadesautoinmunes en general, centrándonos en la DT1: Mimetismomolecular, aumento del procesamiento y presentación antigénicade autoantígenos durante la infección o diseminación deepítopos (epitope spreading), efectos colaterales, reconocimientode epitopos crípticos por parte de linfocitos T autoreactivos, activaciónde linfocitos T con dos receptores (dual TCRs), reactivaciónde células T de memoria por mecanismos antígeno inespecíficos,anticuerpos anti-idiotípicos y superantígenos. Resultadosde estudios epidemiológicos, serológicos y experimentales sugierenla asociación entre varios virus y el desarrollo de DT1. Losdatos más convincentes son las variaciones estacionales de la incidenciade la enfermedad, el aumento de la frecuencia de diabetesen pacientes con rubeola congénita o infección por enterovirusy la detección de secuencias de DNA de citomegalovirus enlos linfocitos de pacientes con DT1. Sin embargo, los efectos causalesdirectos son muy dificiles de demostrar. El objetivo de lapresente revisión es resumir los datos sobre el papel de los virusen el desarrollo de la DT1


Type 1 diabetes (T1D) is an autoimmune disease that resultsfrom the destruction of insulin producing pancreatic islet betacells. The pathogenesis of T1D is complex and results from a combinationof genetic, immunologic and environmental factors. Virusesseem to play a role among the many environmental factorsthat, together with the genetic susceptibility, have been implicatedin the pathogenesis of T1D. Here we review the various hypothesesof the contribution of viruses to autoimmune diseases ingeneral, focusing on T1D: Molecular mimicry, increased processingand presentation of autoantigens during infection or epitopespreading, direct bystander effects, recognition of cryptic epitopesby autoreactive T cells, activation of T cells with dual TCRs,reactivation of memory T cells by nonantigen-specific mechanisms,anti-idiotypic antibodies and superantigens. Epidemiological,serological and experimental studies suggest the associationof several viruses to the development of autoimmune diabetes.The most convincing findings are the seasonal variationsin the incidence of the disease, the increased frequency of T1D inpatients with congenital rubella syndrome or enterovirus infectionand the detection of CMV DNA sequences in lymphocytesfrom T1D patients. However, direct causative effects are difficultto verify. The aim of the present review is to summarize the findingsregarding the role of viruses in the development of T1D

RESUMO

La infección por el virus de la inmunodeficiencia humana (VIH) origina una destrucción progresiva e irreversible del sistema inmunitario. Aunque este proceso está directamente relacionado con la infección de los linfocitos CD4, la disminución de esta población celular a lo largo de la infección por el VIH no se debe únicamente al efecto citopático provocado por la replicación viral. La inmunopatogenia del sida es un proceso extraordinariamente complejo en el que se encuentran implicados mecanismos patogénicos muy diferentes, algunos de los cuales no son todavía completamente comprendidos. En este artículo se analizan los mecanismos mediante los cuales el VIH se adapta y destruye sus células diana, la respuesta del sistema inmunitario frente a la infección por el VIH y, por último, los mecanismos de escape que permiten al VIH eludir esta respuesta inmunitaria (AU)

RESUMO

En los últimos años se han realizado grandes progresos en el conocimiento de la patogenia de la enfermedad celíaca. Estudios recientes han permitido identificar secuencias de péptidos de la gliadina que actúan como epítopos para las células T intestinales gluten-específicas. La desamidación de la gliadina por la transglutaminasa tisular parece influir en la respuesta de estas células T intestinales.El descubrimiento hasta el momento de unos quince epítopos dominantes permite pensar que en un futuro se podrá utilizar inmunoterapia antígenoespecífica en la enfermedad celíaca (AU)
