INTRODUCTION: The coronavirus disease 2019 (COVID-19) is a viral disease that affects several human organs and sys¬tems. Preventive or prophylactic treatments are specifically useful in emerging infectious diseases such as COVID-19 because they reduce the need for hospitalization and public health spending. Although the SARS-CoV-2 preventive effect of several therapeutic agents (e.g., hydroxychloroquine/chloroquine, remdesivir, lopinavir, and ritonavir) has been extensively evaluated, none of them have demonstrated significant clinical efficacy. METHOD: We aim to address and discuss the recently published studies on the chemoprophylactic potential of quer¬cetin against SARS-CoV-2. A literature search was carried out on different databases, such as PubMed/MEDLINE, Scielo, Scopus, Web of Science, Cochrane Library, and Clinical Trials.gov. Studies that report the effect of quercetin against SARS-CoV-2 or other types of coronaviruses were included and critically evaluated. RESULTS: Studies have shown that quercetin, an FDA-approved flavonoid used as an antioxidant and anti-inflamma¬tory agent, inhibits the entry of coronavirus (SARS-CoV) into the host cell. Moreover, an in silico study showed that quercetin is a potent inhibitor of the SARS-CoV-2 main protease (Mpro), suggesting that this flavonoid is also active against COVID-19. CONCLUSIONS: Because quercetin might prevent and lessen the duration of SARS-CoV-2 infections, it is plausible to assume that the prophylactic use of this flavonoid produces several clinical benefits. However, this preliminary evidence needs to be confirmed by in vitro assays and, posteriorly, in randomized clinical trials
INTRODUCCIÓN: La enfermedad del coronavirus 2019 (COVID-19) es una enfermedad viral que afecta a varios órganosy sistemas. Los tratamientos preventivos o profilácticos son especialmente útiles en enfermedades infecciosas emergentes como COVID-19 porque reducen la necesidad de hospitalización y el gasto en salud pública. Aunque el efecto preventivo del SARS-CoV-2 de varios agentes terapéuticos (e.g., hidroxicloroquina/cloroquina, remdesivir,lopinavir y ritonavir) se ha evaluado ampliamente, ninguno de ellos ha demostrado una gran eficacia clínica. MÉTODO: Por lo tanto, aquí nuestro objetivo es abordar y discutir los estudios publicados recientemente sobre el potencial quimioprofilático de la quercetina contra el SARS-CoV-2. METODOLOGÍA: Se realizó una búsqueda de la literaturaen bases como PubMed/MEDLINE, Scielo, Scorpus, Web of Science, Cochrane Library y Clinical Trials.gov. Se incluyeron y evaluaron críticamente estudios que abordan la quercetina contra el SARS-CoV-2 u otros tipos decoronavirus. RESULTADOS: Algunos estudios han demostrado que la quercetina, un flavonoide aprobado por la FDA que se utiliza como agente antioxidante y antiinflamatorio, inhibe la entrada del coronavirus (SARS-CoV) en la célula huésped.Además, un estudio in silico mostró que la quercetina es un potente inhibidor de la proteasa principal del SARSCoV-2 (Mpro), lo que sugiere que este flavonoide también es activo contra COVID-19. CONCLUSIONES: Debido a que la quercetina podría prevenir y disminuir la duración de las infecciones por SARSCoV-2, es plausible suponer que el uso profiláctico de este flavonoide produce varios beneficios clínicos. OBJETIVOS: Determinar la prevalencia de interacciones potenciales en pacientes COVID-19 en tratamiento con lopinavir/ritonavir (LPV/r). El objetivo secundario fue elaborar recomendaciones e identificar los factores de riesgo asociados a presentar interacciones potenciales con LPV/r. SUJETOS Y MÉTODOS: Estudio transversal y multicéntrico con la participación de 2 hospitales. Se incluyeron pacientes COVID-19 mayores de 18 años, con ingreso hospitalario y en tratamiento con LPV/r. Se realizó un cribado de las interacciones potenciales relacionadas con LPV/r y la medicación domiciliaria y hospitalaria. Se utilizó como base de datos de consulta Lexicomp® (Uptodate), HIV-drug interacctions y COVID-drug interacctions. RESULTADOS: Se incluyeron 361 pacientes con una media de edad de 62,77 ± 14,64 años, donde el 59,6% (n = 215) fueron hombres. El 62,3% (n=225) tuvieron una o más interacciones potenciales y el 26, 87% (n = 97) 2 o más. Las variables independientes asociadas a presentar ≥ 1 interacciones potenciales fueron la edad (> 65) (OR 1,95; IC 95% 1,06-3,59; P = 0,033), el ingreso en UCI (OR 9,22; IC 95% 1,98-42,93; P = 0,005), la enfermedad previa respiratoria (OR 2,90; IC 95% 1,15-7,36; P = 0,024), psiquiátrica (OR 4,14; IC 95% 1,36-12,61; P = 0,013), la dislipemia (OR 3,21; IC 95% 1,63-6,35; P = 0,001) y el número de fármacos prescrito (OR 4,33; IC 95% 2,40-7,81; P = 0,000). CONCLUSIÓN: La prevalencia de interacciones potenciales en paciente COVID-19 en tratamiento con LPV/r es elevada, comportándose como factores de riesgo asociados la edad (> 65), el ingreso en UCI, la enfermedad previa respiratoria, psiquiátrica y la dislipemia y el número de fármacos prescritos
OBJECTIVES: To determine the prevalence of potential interactions in COVID-19 patients receiving lopinavir/ritonavir (LPV/r). The secondary objective was to develop recommendations and identify the risk factors associated with presenting potential interactions with LPV/r. SUBJECTS AND METHODS: Cross-sectional and multicenter study with the participation of 2 hospitals. COVID-19 patients over 18 years of age, admitted to hospital and under treatment with LPV/r were included. A screening of potential interactions related to LPV/r and home and hospital medication was carried out. Lexicomp® (Uptodate), HIV-drug interactions and COVID-drug interactions were used as the query database. RESULTS: 361 patients with a mean age of 62.77 ± 14.64 years were included, where 59.6% (n = 215) were men. 62.3% (n = 225) had 1 or more potential interactions and 26, 87% (n = 97) 2 or more. Las proteasas de serina son enzimas ampliamente distribuidas en la naturaleza, responsables de múltiples e importantes procesos biológicos. Durante las infecciones bacterianas los patógenos secretan y usan sus proteasas de serina como factores de virulencia para combatir contra el huésped, a través de diversos efectos como la desorganización de tejidos, la proteólisis de efectores inmunológicos o la inactivación de componentes relevantes para la fisiología del huésped; sin embargo, desde hace algunos años se ha observado que las proteasas de serina podían modular procesos fisiológicos por un mecanismo altamente específico, a través de la activación de los receptores activados por proteasas. Serine proteases are enzymes widely distributed in nature, and are responsible for multiple and important biological processes. During bacterial infection, pathogens secrete and use their serine proteases as virulent factors to combat against the host, through diverse mechanisms, such as tissue disruption, proteolysis of immunological effectors or inactivation of relevant components for the host physiology. However, some years ago it was observed that serine proteases could modulate physiological processes by a highly specific mechanism, through the activation of protease activated receptors (PARs). In this paper, we review recent knowledge about bacterial serine proteases and their relevance in the pathophysiology of infection. Antecedentes: El marcado descenso en los niveles de C-LDL producidos por los inhibidores de la proproteína convertasa plasmática subtilisina kexina tipo 9 (iPCSK9) podría asociarse con un mayor riesgo de cataratas. Métodos: Realizamos un metaanálisis que incluyó ensayos clínicos aleatorizados y controlados con iPCSK9, solos o combinados con otros fármacos hipolipidemiantes, que reportaron nuevos casos de cataratas, buscando en PubMed/Medline, bases de datos de EMBASE y Cochrane Clinical Trials. Se utilizó un modelo de efectos fijos y se realizó una metarregresión evaluando la relación entre el C-LDL intratratamiento y el riesgo de desarrollar cataratas. Resultados: Se tomaron en cuenta 5 estudios elegibles con iPCSK9 que incluyeron 83.492 pacientes para el análisis, refiriendo 531 nuevos casos de cataratas en el grupo con iPCSK9 frente a 532 en el grupo placebo. La terapia con iPCSK9 no se asoció con un mayor riesgo de presentar cataratas (OR: 0,96; IC 95%: 0,85-1,08; p = 0,86, I2: 0%]. Asimismo, no se encontró una asociación significativa entre la diferencia de C-LDL intratratamiento entre las ramas de los estudios y el riesgo de cataratas. Conclusión. En nuestro análisis, la utilización de iPCSK9 no se asoció con un mayor riesgo de cataratas
Background: The marked decrease in LDL-C levels produced by the inhibitors of the plasma proprotein convertase subtilisin/kexin type 9 (iPCSK9) could be associated with an increased risk of cataracts. Methods: A meta-analysis was performed that included randomised clinical trials controlled with iPCSK9, alone, or in combination with other lipid-lowering drugs, which reported new cases of cataracts, by searching PubMed/Medline, databases of EMBASE and Cochrane Clinical Trials. A fixed-effect model was used, and a meta-regression was carried out evaluating the relationship between intra-treatment LDL-C and the risk of developing cataracts. Results: Five eligible studies of iPCSK9 including 83,492 patients were taken into account for the analysis, and 531 new cases of cataracts in iPCSK9 group vs. 532 in placebo group were diagnosed. The iPCSK9 therapy was not associated with an increased risk of cataracts [OR: 0.96, 95% CI: 0.85-1.08; P = .86, I2: 0%]. Likewise, no significant association was found between on-treatment LDL-C levels, differences between study arms, and new cases of cataracts. Conclusion: In this analysis, the use of iPCSK9 was not associated with an increased risk of cataracts

La infección por el VIH exige una alta adherencia al tratamiento a largo plazo al tratarse este de un tratamiento de por vida. Darunavir/cobicistat/emtricitabina/tenofovir alafenamida (Symtuza(R)) es la primera tri-terapia con un inhibidor de la proteasa (IP) en un comprimido. Combina la potencia y una alta barrera genética del mejor IP, darunavir, con la seguridad renal y ósea de tenofovir alafenamida. Estudios en fase III han demostrado que esta coformulación no es inferior en alcanzar la supresión virológica y en el mantenimiento de la eficacia en pacientes inmunosuprimidos, incluso con antecedentes de fracaso previo. Because lifelong therapy is needed in HIV infection, high long-term adherence is necessary. Darunavir/co-bicistat/emtricitabina/tenofovir alafenamide (Symtuza(R)) is the first triple therapy combining a protease inhibitor (PI) in a single tablet regimen. This drug combines the potency and high genetic barrier of the most effective PI, darunavir, with the renal and bone safety of tenofovir alafenamide. Phase 3 studies have demonstrated its non-inferiority in achieving virologic suppression and maintaining efficacy in virological-ly suppressed patients, even in those with previous failure. Although long-term data are needed, Symtuza(R) has become a preferred option in most patients. The drug is especially useful in patients with suspected poor adherence, those requiring rapid treatment initiation because of late presentation or opportunistic infection, patients with limitations for other alternatives, and those with a history of previous failure or resistance to other classes of drugs. Objetivo: El aumento de la esperanza de vida conduce a un nuevo modelo de paciente VIH positivo, con enfermedades crónicas y, en ocasiones, polimedicado. Pretendemos con este estudio conocer la complejidad de los tratamientos e identificar potenciales interacciones entre antirretrovirales y medicación domiciliaria de nuestros pacientes, con objeto de tenerlas identificadas y poder prevenirlas. Método: Estudio descriptivo, retrospectivo, en una cohorte de pacientes con tratamiento antirretroviral mayores de 50 años en un hospital de tercer grado. Resultados: Se incluyeron 242 pacientes, de los que 148 (61%) recibían algún otro tratamiento. Detectamos 243 potenciales interacciones: 197 consideradas moderadas y 46 graves; afectando a 110 pacientes. De las graves, 35 (76%) se relacionaron con inhibidores de proteasa potenciados. La principal consecuencia fue un aumento de las concentraciones plasmáticas del tratamiento domiciliario (48%). Las estatinas (24%) fueron el grupo especialmente implicado en las interacciones graves, seguidas de los corticoides inhalados (15%). Conclusiones: Prácticamente la mitad de los pacientes estaban polimedicados, observándose un elevado número de potenciales interacciones moderadas o graves. El farmacéutico de hospital debe jugar un papel crucial en su detección, manejo y comunicación precoz
Objective: An increased life expectancy leads to a new model of HIV patient with chronic diseases and occasionally polymedicated. With this study, we intend to understand treatment complexity and to identify any potential interactions between antiretroviral drugs and home medication in our patients, in order to identify and prevent them. Method: A retrospective, descriptive study carried out in a cohort of > 50-year-old patients on antiretroviral treatment in a tertiary hospital. Results: We included 242 patients; 148 (61%) of them were receiving some concomitant treatment. We detected 243 potential interactions: 197 considered moderate and 46 severe, in 110 patients. Of the severe interactions, 35 (76%) were related to boosted protease inhibitors. The main consequence of these interactions was an increase in the plasma concentrations of the home medication (48%). Statins (24%) were the group most involved in severe interactions, followed by inhaled corticosteroids (15%). Conclusions: Practically half of patients were polymedicated, and a high number of potential moderate or severe interactions were observed. La infección por el virus de la hepatitis C constituye un problema importante de salud pública. La introducción de los antivirales de acción directa, cuya eficacia es superior al 90% en todos los grupos de pacientes, incluidos los cirróticos, ha supuesto un cambio muy relevante respecto al tratamiento con las terapias clásicas basadas en interferón. Además, la tolerancia de estos antivirales es significativamente mejor, y la duración del tratamiento más corta. Hepatitis C virus infection is a significant public health problem. The introduction of direct-acting antiviral agents, whose efficacy is greater than 90% in all patient groups (including those with cirrhosis), has represented a highly relevant change compared with classical interferon-based therapies. Tolerance for these antiviral agents is significantly better, and the treatment duration is shorter. Objetivo: Describir la eficacia en práctica clínica de abacavir, lamivudina y atazanavir sin potenciar (ABC/3TC+ATV) en pacientes pretratados. Pacientes y métodos: Se realizó un estudio observacional retrospectivo para describir las características clínicas y la evolución de los pacientes que, por prescripción facultativa, habían recibido tratamiento con ABC/3TC+ATV desde noviembre de 2004 hasta el 15 de junio de 2015. Resultados: Se incluyeron 236 pacientes. La mediana de edad (IQR) fue de 45años (42-50) y el 69% eran varones. Los principales motivos para su indicación fueron toxicidad en 130 pacientes (56%), simplificación en 60 (20%) y fracaso virológico (FV) en 29 (14%). El tratamiento previo contenía un inhibidor de la proteasa (IP) en 115 pacientes (48,7%), 3 inhibidores de la transcriptasa inversa análogos de nucleósido (ITIAN) en 56 (28%) y 2ITIAN y un inhibidor de la transcriptasa inversa no análogo de nucleósido (ITINAN) en 19 (8,1%). Tras una mediana de 2,2años (IQR0,8-5,3), 66 (28%) pacientes continuaban con la misma pauta, se retiró en 170 (72%), en 30 de ellos por FV (12,7%) y en 22 (9,3%) por pérdidas de seguimiento. Conclusión: En pacientes seleccionados, ABC/3TC+ATV es una alternativa de simplificación eficaz y bien tolerada, usada principalmente para minimizar la toxicidad (AU)
Objective: To describe the experience using the combination abacavir, lamivudine plus non-boosted atazanavir (ABC/3TC+ATV) in a group of pretreated patients. Patients and methods: We performed a retrospective observational study to describe baseline characteristics and the evolution of patients who had received or were treating with ABC/3TC+ATV, from November 2004 and June 15th 2015, in the clinical setting. Results: Overall, 236 patients were included in the study. Median age (IQR) was 45 (42-50) years and 69% were male. The main reasons for using this combination were previous toxicity in 130 patients (56%), simplification in 60 (20%) and virologic failure in 29 (14%). Previous treatment was based in boosted protease inhibitor in 115 patients (48.7%), 3 analogs in 56 (28%) and non-analogous based in 19 (8.1%). Median treatment length was 2.2 years (IQR0.8-5.3). A total of 66 (28%) patients continue receiving ABC/3TC+ATV (median time 5.7, IQR2.2-8.3), treatment was changed in 170 patients (72%) (median time 1.6 years, IQR0.7-3.6), and 22 (9.3%) patients were lost. Virological failure was assessed in 30 patients. Conclusion: In selected patients, ABC/3TC+ATV is a durable and attractive therapeutic alternative (AU)

Las plantas han desarrollado un sistema de resistencia a insectos y plagas mediante la síntesis de compuestos como los inhibidores de las alfa-amilasas y/o las proteasas digestivas, proteínas que actúan también contra las enzimas digestivas de mamíferos. Diversos estudios han demostrado su implicación en la activación del sistema inmune y posiblemente, en la sintomatología de patologías como la Sensibilidad al Gluten no Celíaca, siendo esta reacción proporcional al contenido en el cereal. El objetivo de este trabajo fue realizar una revisión de las diferentes estrategias para la extracción, purificación, detección y cuantificación de estas proteínas en el trigo y en otros cereales de la dieta actual. Esta revisión incluyó los métodos de análisis con espectrometría de masas del periodo 2000 - 2017, identificándose 114 artículos relevantes de los que se seleccionaron 26. Actualmente no existe un método estandarizado de control que permita determinar estas proteínas de forma clara, concisa y fiable
Plants have developed a system of resistance to insects and pests through the synthesis of certain compounds such as alpha-amylase inhibitors and/or digestive proteases, proteins that can also act against digestive enzymes of mammals. Studies have shown their involvement in the activation of the immune system and possibly in the symptomatology of non-coeliac Gluten Sensitivity, being the reaction proportional to cereal content. The objective of this study was to review the different strategies used for extraction, purification, detection and quantification of these inhibitors in wheat and other cereals of the current diet. A review of the literature published between 2000-2017 on methods of analysis by mass spectrometry was carried out. 114 relevant articles were identified from which 26 were selected for review. Proteolytic activity is fundamental to survival, so it is not surprising that all living organisms have proteases, especially seine protease. This enzyme in its numerous isoforms and homologues, constitutes the quintessential offence and defence factors, in the form of surface proteins, secreted molecules, gut digestive enzymes, venom in specialised glands or plant latex, among other manifestations. Occurring as trypsin, chymotrypsin, elastase, collagenase, thrombin, subtilisin etc., it mediates a diverse array of functions, including pathological roles as inflammatory, coagulatory to haemorrhagic. This review emphasizes that despite the superficial differences in mechanisms, most health issues, be they infectious, allergic, metabolic, or neural have a common conduit. This enzyme, in its various glycosylated forms leads to signal misinterpretations, wreaking havoc. However, organisms are endowed with serine protease inhibitors which might restrain this ubiquitous yet deleterious enzyme. Hence, serine proteases-driven pathogenesis and antagonising role of inhibitors is the focal point of this critical review (AU)

Objective: Comorbidities associated with the ageing of the HIV+ population may require chronic treatment. Our aim is to determine the degree of polypharmacy and the number of potential drug-drug interactions, as well as the relationship between both variables in a HIV-infected population over the age of 65. Methods: Descriptive transversal study targeting HIV+ patients aged ≥65, attended in a Spanish hospital in 2014. The prevalence of polypharmacy (≥5 drugs) and potential drug-drug interactions were assessed, and also risk factors associated with such. Results: 265 subjects aged ≥65 years were identified, 197 of whom were on antiretroviral treatment and had data about their electronic prescription. 93% were polymedicated. The patients whose antiretroviral treatment included a non-nucleoside reverse transcriptase inhibitor (NNRTI) demonstrated a fourfold probability of being polymedicated. 65% of the patients showed at least one potential drug-drug interaction and 6.6% a severe potential drug-drug interaction. The risk of interaction was significantly associated with the number of prescribed drugs (incidence rate ratio per prescribed drug, CI 95%: 1.18 (1.14;1.22; p<0.0001) and with the use of protease inhibitors (PI) (incidence rate ratio, CI 95%: 1.65 (1.28;2.11; p=0.0001)). Conclusion: Polypharmacy has a high prevalence and is more common in patients treated with NNRTI. The number of potential drug-drug interactions increase with the number of prescribed drugs and is higher in those patients on PI (AU)
Objetivo: Las comorbilidades asociadas al envejecimiento de la población VIH+ pueden requerir tratamientos crónicos. Nuestro objetivo es determinar el grado de polifarmacia y el número de interacciones farmacológicas potenciales, así como la relación entre ambas variables en un grupo de población VIH+ mayor de 65 años. Métodos: Estudio descriptivo transversal en pacientes VIH+≥65 años atendidos en un hospital español en 2014. Se evaluó la prevalencia de polimedicación (≥5 fármacos) y se analizaron las interacciones farmacológicas potenciales y los factores de riesgo asociados a ellas. Resultados: Se identificaron 265 sujetos ≥65 años, de los cuales 197 recibían tratamiento antirretroviral y tenían datos en la receta electrónica. El 93% estaban polimedicados. Los pacientes cuyo tratamiento antirretroviral incluía un inhibidor de la transcriptasa inversa no nucleósido (ITINN) presentaban una probabilidad cuatro veces mayor de estar polimedicados. El 65% de los pacientes presentó al menos una interacción potencial y el 6,6% una interacción potencial grave. El riesgo de interacciones se asoció significativamente al número de fármacos prescritos (razón de tasas de incidencia por fármaco prescrito con IC 95%: 1,18 (1,14;1,22; p<0.0001)) y a los inhibidores de la proteasa (IP) (razón de tasas de incidencia IC 95%: 1,65 (1,28;2,11; p=0,0001)). Conclusión: La prevalencia de la polifarmacia es muy alta y más frecuente en los pacientes tratados con ITINN. El número de interacciones farmacológicas potenciales aumenta con el número de fármacos prescritos y es mayor en los pacientes tratados con IP (AU)

Ischemia is induced when blood flow to an organ is interrupted, and re-establishing blood flow is essential to prevent ongoing hypoxic injury, although it paradoxically imparts further injury. Nafamostat mesilate (NM), a synthetic serine protease inhibitor, has been used in patients undergoing hemodialysis who are at a high risk of bleeding. To determine the protective effect of NM on ischemia-reperfusion injury (IRI) in a mouse renal IRI model, NM was administered as a pre- and post-treatment or during ischemia reperfusion and compared to a control group. Mice were bilaterally nephrectomized and subjected to 40 min of renal pedicle occlusion followed by 24 h reperfusion. NM (240 μg/kg) significantly improved kidney function and lowered serum creatinine and blood urea nitrogen levels. Consistently, NM inhibited collagen formation in kidney tissues. NM treatment attenuated the effects of ischemia/reperfusion on kidney tissues and significantly inhibited activation of Toll-like receptor 4, nuclear factor kappa-light-chain-enhancer of activated B cells-phospho-65 (NF- kB-p65), phospho-inhibitor of NF-κβa, and inducible nitric oxide synthase (iNOS). NM treatment also decreased expression of Bcl-2, Caspase-3 and Bax in kidney tissues, which has been linked with induction of apoptosis in kidney tissues. Our studies suggest that NM may be a novel therapeutic agent to prevent and treat kidney IRI, in which iNOS and/or NF-κβ are upregulated. En el momento histórico en el que nos encontramos en el ámbito del tratamiento de la hepatitis C, en que disponemos de fármacos excepcionalmente eficaces y con muy escasos efectos secundarios, se nos plantea el problema del retratamiento en los pacientes que fracasan al mismo. Los factores que influyen en el fracaso son muy diversos, si bien en esta revisión nos vamos a centrar en las resistencias antivirales. Las sustituciones asociadas a resistencias pueden ser tanto basales como inducidas por el tratamiento; estas últimas parece que son las más importantes clínicamente y las que deben ser estudiadas ante la falta de respuesta virológica. En este artículo ofrecemos los últimos datos de ensayos clínicos y estudios en práctica clínica real, y en base a la evidencia actual se ofrecen unas recomendaciones de manejo y retratamiento de estos pacientes (AU)
We are currently living in an unprecedented era of hepatitis C treatment with the availability of highly effective drugs yielding minimal side effects. The problem we currently face is the retreatment of patients refractory to these drugs. Although several factors can influence treatment failure, this review focuses on antiviral resistance. Resistance-associated substitutions may be identified at baseline or be treatment-emergent. The latter seem to be more clinically relevant and must be studied in the event of treatment failure (no virological response). In this article, we present the latest data from clinical trials and studies in real-life clinical practice. Finally, based on this current evidence, we propose some recommendations for the management and retreatment of these patients (AU)

La infección por el virus de la inmunodeficiencia humana (VIH) puede dar lugar a alteraciones neuropsiquiátricas tales como déficits cognitivos, alteraciones comportamentales o sintomatología psiquiátrica como manía o psicosis secundaria. La evolución y curso pronóstico de los individuos con VIH que presentan comorbilidad psiquiátrica dependerá en gran medida de que se ofrezca un tratamiento adecuado que incluya, por una parte, tratamiento del factor etiológico (VIH) y, por otra, tratamiento de los síntomas psíquicos en cuestión. A partir de la presentación del caso clínico de una paciente con encefalitis en el contexto de una infección por VIH no conocida, que debuta con sintomatología psicótica en forma de trastorno por ideas delirantes de tipo somático, ofrecemos una revisión acerca del manejo de la psicosis en pacientes VIH. Dicha revisión se centra en la epidemiología, etiopatogenia y presentación clínica de la psicosis asociada al VIH así como en el manejo farmacológico recomendado (antirretroviral y antipsicótico) y su particular respuesta al mismo. Ofrecemos al mismo tiempo amplia información acerca de las principales interacciones entre los fármacos antipsicóticos y antirretrovirales que otorgarán al clínico un manejo más adecuado de dichos pacientes
Human immunodeficiency virus (HIV) infection can cause neuropsychiatric disorders such as cognitive impairment, behavioural difficulties or psychiatric symptoms -for instance, mania and psychosis. HIV patients with psychiatric comorbidities need an appropriate treatment which tackles the HIV infection as much as the particular mental symptoms. Here we present the case of a patient suffering from delusions, which turned out to be caused by encephalitis secondary to a previously unknown HIV infection. A review of psychosis in HIV-infected patients is also presented. This review is focused on the epidemiology, etiopathogenesis and clinical presentation of HIV-induced psychosis, as well as the recommended pharmacological treatment (antiretroviral therapy and antipsychotic medication) and the expected treatment response. We also present wide information concerning pharmacological interactions between antiretroviral and antipsychotic medications that we hope will help the clinician to better manage this complex condition

Introducción. La monoterapia con inhibidores de la proteasa potenciados con ritonavir (IP/r): darunavir (DRV/r) o lopinavir (LPV/r), sólo está contemplada en las principales guías de tratamiento en pacientes pretratados para evitar toxicidad asociada a inhibidores de transcriptasa inversa análogos de nucleósidos/nucleótidos (ITIAN), reducir costes y simplificar el tratamiento antirretroviral (TAR). Para iniciar una monoterapia basada en IP/r según las guías GESIDA del año 2016, es necesario que el paciente cumpla los siguientes criterios: ausencia de hepatitis crónica B, carga viral plasmática (CVP) (<50 copias/ mL) durante al menos 6 meses y ausencia de mutaciones en el gen de la proteasa o fracasos virológicos (FV) previos a IP/r. Actualmente no hay estudios que evalúen la eficacia y seguridad de una monoterapia con darunavir/cobicistat (DRV/COBI). Material y Métodos. Se trata de un estudio prospectivo en el que se incluyeron pacientes VIH pretratados con DRV/r en monoterapia que cambiaron a una monoterapia con DRV/ COBI. El objetivo de nuestro estudio es describir la efectividad y seguridad de la monoterapia con DRV/COBI. Resultados. Se estudiaron 78 pacientes. Los pacientes tuvieron una mediana de 31,29 (6-74,82) meses de monoterapia con DRV/r previo al cambio a DRV/COBI en monoterapia. Nueve de los 78 pacientes desarrollaron 'blips' (CVP: 50-200 copias/ml) y cuatro pacientes tuvieron CVP≥ 200 copias/mL. Un 83,3% (65/78) se mantuvieron con CVP indetectable. En cuanto a la seguridad, no hubo diferencias importantes en el perfil lipídico, función hepática (transaminasas) y función renal entre DRV/r y DRV/COBI en monoterapia. Conclusiones. DRV/COBI en monoterapia, parece ser efectivo y seguro (perfil lipídico, hepático y renal). Sin embargo, deberían diseñarse estudios específicos que comparasen DRV/r vs. DRV/COBI en monoterapia para comprobar estos resultados (AU)
Introduction. Ritonavir-boosted protease inhibitor (IP/r) monotherapy: darunavir/ritonavir (DRV/r) or lopinavir/ritonavir (LPV/r) monotherapy is only provided in the major treatment guidelines in pretreated patients to prevent toxicity associated with nucleoside/nucleotide reverse transcriptase inhibitor (NRTI), reduce costs and simplify antiretroviral treatment. To start IP/r monotherapy, according to GESIDA guidelines 2016, patients need to meet the following criteria: absence of chronic hepatitis B, plasma viral load <50 copies/ mL for at least 6 months and absence of protease inhibitors mutations or previous virologic failures to IP/r. Currently, there are no studies that evaluate the efficacy and safety of darunavir/cobicistat (DRV/COBI) monotherapy. Methods. This prospective study analyzed pretreated HIV patients with DRV/r monotherapy that were switched to DRV/ COBI monotherapy. The aim of the study is to describe the effectiveness and safety of the DRV/COBI monotherapy. Results. Seventy-eight patients were evaluated. Patients had a median of 31.29 months of DRV/r monotherapy before DRV/COBI monotherapy. Nine of the 78 patients developed 'blips' (plasma viral load: 50-200 copies/ml) and four patients had plasma viral load ≥200 copies/mL. An 83.3% (65/78) of the patients remained with undetectable plasma viral load. As for safety, there were no significant differences in lipid profile, liver function (transaminases) and renal function between DRV/r and DRV/COBI monotherapy. Conclusions. DRV/COBI monotherapy seems to be effective and safe (lipid profile, liver and kidney function). However, it would be necessary to design specific studies comparing DRV/r vs DRV/COBI monotherapy to confirm these results (AU)

Rezolsta® (darunavir/cobicistat) es el primer inhibidor de la proteasa potenciado en combinación fija con un nuevo potenciador farmacológico. Darunavir es actualmente el inhibidor de la proteasa de uso preferente con un perfil de eficacia y seguridad bien conocido. Cobicistat es un inhibidor del citocromo P450 3A sin actividad antirretroviral que se usa como potenciador farmacológico y que puede ser una alternativa al ritonavir. Los parámetros farmacológicos de darunavir con cobicistat son similares a los que se producen cuando el potenciador es ritonavir. Los ensayos clínicos de darunavir/cobicistat han demostrado gran eficacia y buena tolerabilidad. El cobicistat inhibe de forma más específica el citocromo P450 3A sin efecto inductor, por lo que presenta un menor perfil de interacciones farmacológicas que el ritonavir. El cobicistat está también coformulado a dosis fijas con otros fármacos: elvitegravir, tenofovir diproxil fumarato y emtricitabina (Stribild®), elvitegravir, tenofovir alafenamina y emtricitabina (Genvoya®) y con atazanavir (Evotaz®). Rezolsta® (darunavir/cobicistat) is the first fixed-dose combination tablet with a new boosting agent. Darunavir is currently the preferred protease inhibitor and has a well-known safety and efficacy profile. Cobicistat is a P450 3A cytochrome inhibitor without antiretroviral activity that is used as a boosting agent and can be an alternative to ritonavir. The pharmacological values of darunavir with cobicistat are similar to those produced when the booster is ritonavir. Clinical trials of darunavir/cobicistat have shown high efficacy and good tolerability of the combination. Cobicistat produces more specific inhibition of cytochrome P450 3A induction effect, and consequently has a lower drug interaction profile than ritonavir. Cobicistat is also coformulated in fixed-dose combinations with other drugs: elvitegravir, tenofovir diproxil fumarate and emtricitabine (Stribild®), elvitegravir, tenofovir alafenamine and emtricitabine (Genvoya®) and with atazanavir (Evotaz®). Fundamento y objetivo: Determinar la prevalencia y los tipos de interacciones farmacológicas clínicamente significativas (IFCS) en los regímenes farmacológicos de los pacientes infectados por VIH que están recibiendo un tratamiento antirretroviral, y desarrollar una herramienta diagnóstica que pueda predecir la posibilidad de una IFCS. Material y métodos: Diseño: revisión retrospectiva de bases de datos. Centro: Hospital Universitario Severo Ochoa, Unidad de Infecciosas. Participantes: 142 participantes seguidos por uno de los autores fueron seleccionados desde enero de 1985 hasta diciembre de 2014. Recogida de datos: recogimos en la última visita disponible de los participantes información relacionada con la infección VIH, las comorbilidades, los datos demográficos y los fármacos que estuvieran recibiendo, tanto los antirretrovirales como aquellos no relacionados con la infección VIH. Se definió la existencia de IFCS por la información de la ficha técnica y/o la base de datos de interacciones de fármacos antirretrovirales de la Universidad de Liverpool (http://www.hiv-druginteractions.org). Mediante el modelo de análisis multivariable de regresión logística y con la estimación de la curva de rendimiento diagnóstico obtenida identificamos una herramienta que nos pudiera predecir la existencia de interacciones farmacológicas. Resultados: De 142 pacientes, 39 (29,11%) tenían algún tipo de IFCS y en 11,2% se detectaron 2 o mas interacciones. Solo en un paciente la combinación de los fármacos estaba contraindicada (este paciente estaba recibiendo darunavir/r y quetiapina). En el análisis multivariable, los factores predictores de IFCS fueron el que estuvieran recibiendo un régimen basado en IP o ITINAN y la presencia de 3 o más fármacos no antirretrovirales (AUC 0,886, IC 95% 0,828-0,944; p = 0,0001). El riesgo fue 18,55 veces en aquellos que recibían ITINAN y 27,95 veces en los que recibían IP con respecto a aquellos que tomaban raltegravir. Conclusiones: Las interacciones farmacológicas, incluyendo aquellas definidas como clínicamente significativas, son frecuentes en pacientes infectados por VIH tratados con antirretrovirales, y este riesgo es mayor en los regímenes basados en IP. Prescribiendo regímenes basados en raltegravir, especialmente en pacientes que reciben al menos 3 fármacos no relacionados con el VIH, se podrían evitar interacciones (AU)
Background and objective: To determine the prevalence and types of clinically significant drug-drug interactions (CSDI) in the drug regimens of HIV-infected patients receiving antiretroviral treatment. Material and methods: Design: retrospective review of database. Centre: Hospital Universitario Severo Ochoa, Infectious Unit.Participants: one hundred and forty-two participants followed by one of the authors were selected from January 1985 to December 2014. Data collection: from their outpatient medical records we reviewed information from the last available visit of the participants, in relation to HIV infection, comorbidities, demographics and the drugs that they were receiving; both antiretroviral drugs and drugs not related to HIV infection. We defined CSDI from the information sheet and/or database on antiretroviral drug interactions of the University of Liverpool (http://www.hiv-druginteractions.org) and we developed a diagnostic tool to predict the possibility of CSDI. By multivariate logistic regression analysis and by estimating the diagnostic performance curve obtained, we identified a quick tool to predict the existence of drug interactions. Results: Of 142 patients, 39 (29.11%) had some type of CSDI and in 11.2% 2 or more interactions were detected. In only one patient the combination of drugs was contraindicated (this patient was receiving darunavir/r and quetiapine). In multivariate analyses, predictors of CSDI were regimen type (PI or NNRTI) and the use of 3 or more non-antiretroviral drugs (AUC 0.886, 95% CI 0.828 to 0.944; P = .0001). The risk was 18.55 times in those receiving NNRTI and 27,95 times in those receiving IP compared to those taking raltegravir. Conclusions: Drug interactions, including those defined as clinically significant, are common in HIV-infected patients treated with antiretroviral drugs, and the risk is greater in IP-based regimens. Raltegravir-based prescribing, especially in patients who receive at least 3 non-HIV drugs could avoid interactions (AU)
