RESUMO

This document is the result of the deliberations of the Committee on Emerging Pathogens and COVID-19 of the Il lustrious Official College of Physicians of Madrid (ICOMEM) regarding the current situation of tuberculosis, particularly in Spain. We have reviewed aspects such as the evolution of its incidence, the populations currently most exposed and the health care circuits for the care of these patients in Spain. We have also discussed latent tuberculosis, the reality of extrapul monary disease in the XXI century and the means available in daily practice for the diagnosis of both latent and active forms. The contribution of molecular biology, which has changed the perspective of this disease, was another topic of discussion. The paper tries to put into perspective both the classical drugs and their resistance figures and the availability and indications of the new ones. In addition, the reality of direct observa tion in the administration of antituberculosis drugs has been discussed. All this revolution is making it possible to shorten the treatment time for tuberculosis, a subject that has also been reviewed. If everything is done well, the risk of relapse of tuberculosis is small but it exists. On the other hand, many special situations have been discussed in this paper, such as tuberculosis in pediatric age and tuberculosis as a cause for concern in surgery and intensive care. The status of the BCG vaccine and its present indications as well as the future of new vaccines to achieve the old dream of eradicating this disease have been discussed. Finally, the ethical and medicolegal impli cations of this disease are not a minor issue and our situation in this regard has been reviewed (AU)


El presente documento es el resultado de las deliberacio nes del Comité sobre Patógenos Emergentes y COVID-19 del Ilustre Colegio Oficial de Médicos de Madrid (ICOMEM) en re lación a la situación actual de la tuberculosis, particularmente en España. Hemos revisado aspectos tales como la evolución de su incidencia, las poblaciones actualmente más expuestas y los circuitos sanitarios para la atención a estos pacientes en España. Se ha discutido también la tuberculosis latente, la rea lidad de la enfermedad extrapulmonar en el siglo XXI y los me dios de que en la práctica diaria se dispone para el diagnóstico tanto de las formas latentes como de las activas. La aportación de la biología molecular que ha cambiado la perspectiva de es ta enfermedad ha constituido otro de los temas de debate. El documento trata de poner en perspectiva tanto los fármacos clásicos y sus cifras de resistencia como la disponibilidad e in dicaciones de los nuevos. Junto a esto, se ha discutido la rea lidad de la observación directa en la administración de fárma cos antituberculosos. Toda esta revolución está posibilitando el acortamiento del tiempo de tratamiento de la tuberculosis tema que ha sido igualmente revisado. Si todo se hace bien, el riesgo de recaída de la tuberculosis es pequeño pero existen te. Por otra parte, muchas situaciones especiales han merecido discusión en este documento como por ejemplo la tuberculosis en edad pediátrica y la tuberculosis como causa de preocupa ción en cirugía y cuidados intensivos. Se ha discutido tanto la situación de la vacuna BCG y sus indicaciones presentes, co mo el futuro de nuevas vacunas que permitan alcanzar el viejo sueño de erradicar esta enfermedad. Finalmente, las implica ciones éticas y medicolegales que esta enfermedad plantea no son un tema menor y se ha revisado nuestra situación en este particular (AU)

Objectives: Extensively drug-resistant tuberculosis (XDR-TB) has raised a great threat to human health globally, especially in developing countries. The objective of the present study is to collate and contrast the proportions of treatment outcome in the previously published XDR-TB articles. Material and methods: By considering inclusion criteria and search engines, a total of 22 articles were enrolled. Results. Our findings revealed that the overall favorable treatment outcome was 24.04%. From the cohort of enrolled studies 19.76% (397) and 43.35% (871) patients were cured and died respectively. In 90.9% of enrolled articles, the investigators performed drug-susceptibility testing at the baseline. The overall treatment outcome was improved by the use of new drugs (linezolid, bedaquiline, ciprofloxacin, clofazimine) in the treatment regimen of XDR-TB showing linezolid and bedaquiline better results i.e. 59.44 and 78.88%, respectively. Moreover, use of antiretroviral treatment in XDR-TB patients with HIV infection have not shown any significant difference in the treatment outcome. Conclusions: XDR-TB treatment success can be achieved by implying standardized definitions, upgraded diagnostic procedures, and novel drugs. (AU)


Objetivos: La tuberculosis extremadamente resistente (XDR-TB) ha planteado una gran amenaza para la salud humana a nivel mundial, especialmente en los países en desarrollo. El objetivo del estudio es recopilar y contrastar las proporciones del resultado del tratamiento en los artículos de XDR-TB publicados.Material y métodos: Teniendo en cuenta los criterios de inclusión y los motores de búsqueda un total de 22 artículos fueron incluidos. Resultados. Nuestros hallazgos revelaron que el resultado total del tratamiento favorable fue del 24,04%. De la cohorte de estudios inscritos, el 19,76% (397) y el 43,35% (871) de los pacientes se curaron y murieron, respectivamente. En el 90,9% de los artículos, los investigadores realizaron pruebas de sensibilidad. El resultado total del tratamiento mejoró mediante el uso de nuevos medicamentos (linezolid, bedaquilina, ciprofloxacino, clofazimina) en el régimen de tratamiento de XDR-TB, mostrando linezolid y bedaquilina los mejores resultados, 59,44 y 78,88%, respectivamente. Además, el uso del tratamiento antirretroviral en pacientes con XDR-TB y con infección por VIH no mostró ninguna diferencia significativa en el resultado del tratamiento. Conclusiones: El éxito del tratamiento de la XDR-TB se puede lograr implicando definiciones estandarizadas, procedimientos de diagnóstico mejorados y nuevos medicamentos. (AU)

La tuberculosis con resistencia a fármacos, sobre todo la que conlleva resistencia a rifampicina (TB-RR), se ha convertido en uno de los principales obstáculos para alcanzar el sueño de erradicar esta enfermedad. Y es que, para intentar curar la TB es necesario asociar tres o cuatro fármacos diferentes y, lamentablemente, son pocos los disponibles que se puedan considerar auténticamente eficaces. Pero, afortunadamente, el notable incremento que ha habido en los últimos años de la TB-RR en el mundo, ha motivado que se hayan invertido recursos en el desarrollo de nuevos fármacos para la TB, o que otros antimicrobianos investigados para otras enfermedades se hayan probado con éxito en la TB. Esto ha hecho que el manejo clínico de estos pacientes haya cambiado notablemente en los últimos tres a cuatro años, y resulte ahora más sencillo diseñar esquemas terapéuticos y conseguir mayores tasas de éxito. Todos estos cambios se actualizan en esta revisión. (AU)


Drug-resistant tuberculosis, especially those with resistance to rifampicin (RR-TB), has become one of the main obstacles to achieving the dream of eradicating tuberculosis. Furthermore, it is necessary to combine three or four different drugs in the attempt to cure TB, however, unfortunately, there are few available that can be considered genuinely effective. Fortunately, the notable worldwide increase in RR-TB in recent years has led to the investment of resources in the development of new drugs for TB, and other drugs investigated for other diseases have been successfully tested on TB. This has resulted in a clear change in the clinical management of these patients over the last 3-4 years, and it is now easier to design therapeutic regimens and achieve higher success rates. All these changes are updated in this review. (AU)

Introduction: Tuberculosis infection caused by Mycobacterium tuberculosis is one of the most common infectious diseases, especially in countries such as Iran. The course of treatment and the number of drugs used vary depending on the severity of the disease and the parts of the body involved. The resistant tuberculosis to treatment has increased in recent years. Thus, this study was conducted to investigate the frequency distribution of response to treatment of patients with tuberculosis in Sirjan, Iran. Methods: This descriptive cross-sectional study investigated all patients with tuberculosis in Sirjan city who had referred to health centers during the years 2011-2019. The data collection tool was a pre-prepared checklist that included information on age, sex, sputum smear results, sputum culture results, diabetes, patients' nationality, drug side effects, and response to treatment. Finally, data was entered into SPSS version 22, and analyzed. Results: In this study, the overall response rate was 83% and the mortality rate was 10%. Between the frequency distribution of response to treatment in terms of gender, age, sputum smear results, sputum culture results, patients' nationality and diabetes was not statistically significant difference. Also, no statistically significant difference was found between the frequency distribution of pulmonary TB treatment response in terms of drug allergy, drug hepatitis and other drug side effects. Conclusion: According to results, can be concluded that none of the variables: age, sex, smear and culture result, and history of diabetes have no an effect on response to treatment and mortality of tuberculosis (AU)

Objective: This study aims to analyze the risk factors (knowledge, role of health workers, drug Consumption Supervisors activity, side effects of drugs, duration of taking medication, and feeling healthy) for non-adherence to TB patients taking medication at the Puskesmas. Method: The research design used in this study was a case control where the population was all patients who visited the working region of the Polonia Public Health Center. The research subjects were 138 samples consisting of 68 cases and 68 controls. The data were analyzed using a tabulation and statistical system using the SPSS program with the Odds ratio test and logistic regression. Result: It was found that there was an influence on the role of health workers OR = 8,933 (95% CI: 1,085–73.525), side effects of drugs with OR = 6,873 (95% CI; 2,435–19,398), felt healthy OR = 4,643 (95% CI; 1,247–17,287), and knowledge OR = 2,700 (95% CI; 1,083–6,731) It is recommended to be healthy and monitor the occurrence of drug side effects in patients during treatment. INTRODUCTION: The impact of medical interventions for reducing ototoxicity requires focus in contexts where safe non-ototoxic alternative drugs are not yet available. OBJECTIVES: The goal was to investigate the impact of medical intervention strategies for reducing the ototoxic effects of long-term use of aminoglycosides for the treatment of Multi-Drug Resistant Tuberculosis (MDR-TB) in adults at a rural hospital in the Eastern Cape, South Africa. METHODS: A retrospective record review with a control group design was used, with audiological and medical record reviews of all participants' files over a six-month period. A total of 86 participants (intervention group n = 32 and control group n = 54) were included. Descriptive and inferential statistics were used to analyze the data. RESULTS: Results revealed progressive hearing loss in both groups, with worsening of thresholds at each subsequent assessment session. The progression of the hearing loss, however, was much slower in the intervention group, with degree of hearing loss being more severe in the control group in the final session of assessment (36.78-71.74 dB), when compared to the intervention group (33-44.39 dB). Furthermore, the hearing loss in the intervention group remained in the high frequencies while that in the control group progressed to involve the lower frequencies on the audiogram. The most common medical intervention strategies employed in the current study were to reduce the dosage of the ototoxic drug (61%), with the daily dosage reduced to 800 mg in 39% of participants, while the administration of the drug was reduced from daily to thrice weekly in 22% of participants. CONCLUSIONS: The results reveal better hearing sensitivity over the treatment period in the intervention group when compared to the control group; thus suggesting that early medical strategies implemented had a significant preventive impact. Current findings are relevant to the audiological, medical and pharmaceutical fields, particularly within the South African context where resource constraints are a consideration in all treatment measures


INTRODUCCIÓN: El impacto de las intervenciones médicas para reducir la ototoxicidad requiere concentrarse en los contextos en los que no se dispone de fármacos alternativos no ototóxicos seguros. OBJETIVOS: El objetivo fue investigar el impacto de las intervenciones médicas para reducir los efectos ototóxicos del uso prolongado de aminoglucósidos para el tratamiento de la tuberculosis multirresistente a los fármacos (MDR-TB) en adultos, en un hospital rural en Cabo Oriental, Sudáfrica. MÉTODOS: Se realizó una revisión retrospectiva de historias con grupo control de las revisiones audiológicas y médicas de todos los participantes durante un periodo de 6 meses, incluyéndose a un total de 86 participantes (grupo intervención n = 32 y grupo control n = 54). Se utilizaron estadísticas descriptivas e inferenciales para analizar los datos. RESULTADOS: Los resultados revelaron hipoacusia progresiva en ambos grupos, con empeoramiento de los umbrales en cada examen sucesivo. Sin embargo, la progresión de la hipoacusia fue menor en el grupo intervención, siendo más severo el grado de pérdida auditiva en el grupo control en el examen final de evaluación (36,78-71,74 dB), en comparación con el grupo intervención (33-44,39 dB). Además, la hipoacusia se mantuvo en frecuencias altas en el grupo intervención, mientras que en el grupo control experimentó una progresión, incluyendo las frecuencias bajas en el audiograma. Las intervenciones médicas más comunes utilizadas en el presente estudio fueron la reducción de la dosis de fármacos ototóxicos (61%), reduciéndose la dosis diaria a 800 mg en el 39% de los participantes, mientras que la administración del fármaco se redujo de una a tres veces por semana en el 22% de los participantes. CONCLUSIONES: Los resultados revelan mejor sensibilidad auditiva a lo largo del periodo de tratamiento en el grupo intervención, en comparación con el grupo control, lo cual sugiere que las estrategias médicas tempranas implementadas tuvieron un impacto preventivo significativo. Los hallazgos actuales son relevantes para los campos audiológico, médico y farmacéutico, particularmente en el contexto de Sudáfrica, donde las limitaciones de recursos son dignas de consideración en todas las medidas de tratamiento

Se presenta el caso de una mujer joven con antecedente de tuberculosis pleural y pulmonar previamente tratada, que acudió a nuestro hospital por dolor torácico y nódulo pleural único visualizado en radiografía simple y TC torácicas. Se realizó punción con aguja fina guiada por ecografía torácica de la lesión pleural con la que se obtuvo material de tipo inflamatorio para estudio con cultivo positivo para Mycobacterium tuberculosis complex. El diagnóstico fue de reacción paradójica al tratamiento antituberculoso y fue tratada durante 6 meses más con resolución de la lesión pleural y el dolor torácico


We present the case of a young woman with a history of previously treated pleural and pulmonary tuberculosis referred to our hospital for chest pain and a single pleural nodule seen on plain chest films and chest CT. Cultures of inflammatory-type material obtained by US-guided fine-needle biopsy of the pleural lesion were positive for Mycobacterium tuberculosis complex. The diagnosis was a paradoxical reaction to antituberculosis treatment; after 6 more months of treatment, the pleural lesion and chest pain disappeared

INTRODUCCIÓN: La tuberculosis (TB) continúa siendo un problema de salud pública grave. En la edad pediátrica existe gran dificultad para obtener un diagnóstico de certeza. La principal forma de presentación es la pulmonar, con mayor riesgo de formas graves y extrapulmonares en menores de 2 años. El objetivo del estudio es describir la TB en población pediátrica en nuestro medio en los últimos años. PACIENTES Y MÉTODOS: Estudio retrospectivo de TB diagnosticadas en 2 hospitales de la Comunidad de Madrid durante 26 años (1991-2017). Se analizan variables epidemiológicas, clínicas, pruebas complementarias y tratamientos recibidos. RESULTADOS: Se incluyeron 170 niños. Las 2 franjas de edad más afectadas son la lactancia y la adolescencia. Población inmigrante: 42,9% (Sudamérica y Marruecos). Los principales motivos de consulta fueron contacto con TB (20,6%) y fiebre (15,3%). Al diagnóstico, el 61,8% refería ambiente epidémico. El 30,6% estaba asintomático. El Mantoux fue positivo en el 92,2% de los pacientes y el IGRA fue positivo en el 70,6% de casos realizados. Se halló enfermedad pulmonar en el 91,8% vs. afectación extrapulmonar en el 8,2% de los niños. El cultivo fue positivo en el 36,9% de muestras de jugo gástrico y se aislaron 3 cepas resistentes a isoniacida. Todos los pacientes seguidos curaron sin secuela, salvo uno que falleció. CONCLUSIONES: La afectación pulmonar continúa siendo la forma de presentación más frecuente de TB en pediatría. El diagnóstico de presunción (clínica compatible con Mantoux positivo o radiografía patológica) es el más habitual. Es importante realizar estudio de contactos como medida de control de enfermedad tuberculosa


INTRODUCTION: Tuberculosis (TB) continues to be a serious public health problem and establishing a definitive diagnosis among children is extremely challenging. Pulmonary tuberculosis is the most prevalent form, with children under the age of 2 years being at greatest risk of severe and disseminated forms. The aim of this study was to describe TB among children in Spain in recent years. PATIENTS AND METHODS: A retrospective study of paediatric patients diagnosed with TB in two hospitals of the Community of Madrid over a 26-year period (1991-2017). Epidemiological and clinical variables, additional tests and treatments received were analysed. RESULTS: 170 children were included. The two most-affected age groups were infancy and adolescence. 42.9% of patients were immigrant children (South-America or Morocco). The main reasons for consultation were TB contact (20.6%) and fever (15.3%). At diagnosis, 61.8% of cases referred an epidemic environment, and 30.6% were asymptomatic. The Mantoux tuberculin skin test was positive in 92.2% of patients and IGRA was positive in 70.6%. Pulmonary TB was identified in 91.8% of children vs. 8.2% with extrapulmonary forms. Gastric juice culture was positive in 36.9% of cases and three strains resistant to isoniazid were isolated. All patients were cured without complications except one who died. CONCLUSIONS: Pulmonary TB continues to be the most prevalent form among children and presumptive diagnosis (symptoms consistent with positive Mantoux test or suggestive X-ray) is the most common form of diagnosis. It is important to conduct a contact investigation in order to control the spread of TB

La evidencia acumulada en los 3 últimos años sobre el manejo clínico de la tuberculosis (TB) con resistencia a fármacos ha sido tan importante que hace necesario actualizar la normativa que SEPAR publicó en 2017, sobre todo en lo referente a nuevos métodos diagnósticos, a la clasificación racional de los fármacos con actividad frente a Mycobacterium tuberculosis y a los esquemas básicos a recomendar en los pacientes con TB con resistencia a isoniacida, con resistencia a rifampicina o con multifarmacorresistencia. En el diagnóstico de la enfermedad recomendamos la utilización de métodos moleculares rápidos que son útiles además para la detección precoz de mutaciones asociadas a resistencias a fármacos. Para el tratamiento de los enfermos con TB con multifarmacorresistencia se hace necesario dar prioridad a esquemas orales acortados que incluyan bedaquilina, una fluoroquinolona (levofloxacino o moxifloxacino) y linezolid en lugar de los esquemas cortos previamente recomendados con aminoglucósidos y otros muchos fármacos de menor eficacia y más tóxicos. La recomendación de la normativa es que el diseño de los tratamientos en estos pacientes, tanto el inicial como si se precisan cambios, sea consultado siempre con expertos en el manejo de TB con resistencia a fármacos y que se realicen por personal con experiencia en TB y en unidades que garanticen la supervisión de los tratamientos y el abordaje de sus reacciones adversas


New evidence and knowledge about the clinical management of drug-resistant tuberculosis (TB) in the last 3 years, makes it necessary to update the recent guideline published by SEPAR in 2017, mainly in relation to new diagnostic methods, drug classification, and regimens of treatment recommended to treat patients with isoniazid-resistance TB, rifampicin resistance TB and multidrug-resistant TB. With respect to tuberculosis diagnosis, we recommend the use of rapid molecular assays that also help to detect mutations associated with resistance. In relation to the treatment of multidrug-resistant TB we prioritize effective all-oral shorter treatment regimens including bedaquiline, a fluoroquinolone (levofloxacin or moxifloxacin), bedaquiline and linezolid, instead of the previously recommended short-course treatment with aminoglycosides and other less effective and more toxic drugs. The design of these regimens (initial schedule and changes in the regimen if necessary) should be made in accordance with drug-resistant TB experts; the treatment should be the responsibility of personnel with experience in the treatment of TB and in TB units guaranteeing the follow-up of the treatment and the management of drugs adverse effects
