Objetivo: Estudio prospectivo de 4 años (2015-2018) de casos de ASCUS y LSIL en la citología cervicovaginal y su control citológico evolutivo en 3 meses. Diseño: Estudio de: edad, paridad, abortos, nuligestas, menopáusicas, métodos contraceptivos, sexualidad y disfunciones sexuales, antecedentes patológicos; citología y virus del papiloma humano, comparando la evolución en ambos grupos. Resultados: La serie son 146 casos de ASCUS y 82 casos de LSIL. Hubo diferencias significativas en ASCUS, mayor en edad (p<0,02), paridad (p<0,001) y abortos (p<0,02). Hubo diferencia significativa en LSIL, mayor en nuligestas (p<0,001). Hubo diferencia significativa, p<0,02, en uso de condón, mayor en LSIL. En ambos grupos, >50% de los casos, no utilizaban ningún método anticonceptivo. No hubo diferencias significativas en la sexualidad ni en las disfunciones sexuales, ni en los antecedentes, entre ambos grupos. ASCUS se asoció a infecciones en el 15,05% y LSIL se asoció a infecciones en el 9,74%, sin diferencias. Hubo diferencia significativa en virus del papiloma humano negativo, p<0,01, mayor en ASCUS, 43,15% versus 20,73% en LSIL. Hubo diferencia significativa en otros virus de alto riesgo positivos, p<0,01, mayor en LSIL. En el control citológico a 3 meses hubo diferencia significativa en ASCUS+vaginosis bacteriana, p<0,02, mayor en LSIL. También en el grupo LSIL hubo más LSIL en el control a 3 meses en el 26,82% (p<0,01). En el seguimiento a un año no hubo diferencias significativas entre ambos grupos. Hubo citología negativa respectivamente en el 64,86% y en el 47,82%. Conclusión: La repetición de la citología a 3 meses tiene valor para descartar los falsos positivos y detectar las infecciones asociadas para poder tratarlas, lo que redundaría en anticipar la normalidad citológica

There were significant differences in negative HPV (P<.001) in ASCUS, 43,15% versus 20,73% in LSIL. There were significant differences in High Risk other viruses positive (P<.001) in LSIL. In the cytologic control at 3 monts, there were significant differences in ASCUS+BV (bacterial vaginosis) (P<.02) in LSIL. Also in LSIL group there were more LSIL in the cytologic control at 3 months, 26,82% (P<.01). In the follow-up at 1 year, there were no significant differences. The negative cytology was in 64,86% and 47,82%, respectively. Conclusion: The reiterative cytology in 3 months are value for detecting the associated infections, and treated; to run over and advance the cytologic normality

Entre las distintas complicaciones pleuropulmonares de la artritis reumatoide, la pleuresía reumatoidea es la más comúnmente observada, sucediendo hasta en el 5% de los pacientes afectos de artritis reumatoide. La mayoría de estos, corresponden a un subtipo compuesto fundamentalmente por varones de edad media con altos títulos de factor reumatoide, nódulos reumatoideos y HLA-B8 y Dw3. Si bien el derrame pleural reumatoideo se presenta generalmente como un exudado inespecífico, se han descrito características de laboratorio, que aunque infrecuentes, nos pueden ser de gran utilidad, en el diagnóstico del mismo


Among the various pleuropulmonary complications of rheumatoid arthritis, rheumatoid pleurisy is the most commonly observed1, occurring in up to 5% of patients with rheumatoid arthritis. The majority of these correspond to a subtype composed mainly of middle-aged men with high rheumatoid factor titres, rheumatoid nodules, and presence of HLA-B8 and Dw3. Although rheumatoid pleural effusion generally presents as a non-specific exudate, laboratory characteristics have been described that, although infrequent, can be very useful in its diagnosis

Objetivo: Conocer la prevalencia de citologías de categoría Bethesda III, su tasa de malignidad y analizar si existen diferencias en la segunda citología, tasa de malignidad, tipo de carcinoma y estadio TNM entre los grupos atipia citológica (AC) y atipia arquitectónica (AA). Pacientes y método: Estudio retrospectivo de 973 citologías. Las de categoría Bethesda III se agruparon en AC cuando se apreció atipia nuclear pero no diagnóstica o sospechosa de malignidad, y en AA cuando se trató de extensiones con poca celularidad, pero con un patrón predominantemente microfolicular y con coloide mínimo o ausente. Se correlacionaron los resultados citológicos y anatomopatológicos. Resultados: Hallamos 87 (8,9%) citologías de categoría Bethesda III (34 AC, 53 AA). Se realizó una segunda citología en 23 casos (16 AC, 7 AA) con resultado benigno en el 68,7% de los AC y 71,4% de los AA. Fueron intervenidos quirúrgicamente 64 casos (23 AC, 41 AA) y 15 (23,4%) resultaron malignos: 39,1% AC vs 14,6% AA (p=0,029). Hubo un falso negativo en la segunda citología. El carcinoma papilar variante folicular fue el más frecuente (60%). No hubo diferencias entre AC y AA respecto al tipo de neoplasia ni estadio TNM. Conclusiones: Nuestra prevalencia de citologías de categoría Bethesda III está dentro de lo recomendado. La tasa de malignidad es significativamente superior en el grupo AC, pero sin diferencias en el resultado de la segunda citología, tipo de carcinoma hallado ni estadio TNM. La división de la categoría Bethesda III es útil para proporcionar una mejor estratificación del riesgo de malignidad


Objective: To ascertain the prevalence of Bethesda category III cytologies and their malignancy rate, and to analyze differences in the second cytology, malignancy rate, type of carcinoma, and TNM stage between the cytological atypia (CA) and architectural atypia (AA) groups. Patients and method: A retrospective study of 973 biopsies. Bethesda category III cytologies were classified as CA when nuclear atypia was seen but they were not diagnostic or suspicious of malignancy, and as AA when smears had few cells but had a predominantly microfollicular pattern and minimal or absent colloid. The cytological and pathological results were correlated. Results: There were 87 (8.9%) Bethesda category III cytologies (34 CC, 53 AA). Second cytologies were performed in 23 patients (16 with CA, 7 with AA), and a benign result was found in 68.7% of CA and 71.4% of the AA group. Sixty-four patients (23 CA, 41 AA) underwent surgery and 15 of these (23.4%) had a malignant disease: 39.1% CA vs 14.6% AA (P=.029). There was a false negative result in the CA group. The follicular variant of papillary thyroid carcinoma was the most common malignancy (60%). There were no differences in type of carcinoma or TNM stage between CA and AA patients. Conclusions: The reported prevalence of Bethesda category III cytologies was as expected. The malignancy rate was significantly higher in the CA group, but there were no differences in the result of the second cytology, type of carcinoma found, or TNM stage. The division of Bethesda category III cytologies is useful to provide a better stratification of the risk of malignancy

Introducción: El objetivo fue estudiar la relación entre la determinación cuantitativa de ARNm (hTERT) en pacientes con tumor vesical, antecedentes de tumor vesical y en sujetos sin antecedentes de esta neoplasia. Material y métodos: Se trata de un estudio prospectivo, aleatorizado y controlado con 91 sujetos incluidos. El valor de ARNm-hTERTN se determinó en 63 pacientes con antecedentes o sospecha de tumor vesical y en 28 controles. Se enviaron muestras de orina para evaluar el nivel de ARNm (hTERT), el estudio citológico y el resultado de NMP22. Resultados: Se observaron diferencias en los niveles medios de hTERTN en cada uno de los grupos: presencia de tumor 21,33 + /- 40,66, antecedente del tumor 2,16 +/ - 2,67, controles 0,9+/- 1, 75 (p < 0,001). En pacientes con tumor, no hubo diferencias en los niveles medios de hTERTN entre los diferentes grados y estadios, aunque hubo una tendencia: tumor de bajo grado 9,04 +/- 16,95, grado alto 28,95+/- 48,36 (p = 0,069), estadio Ta 10,33 +/- 19,39, T1 17,88 +/- 27,14, T2 54,8 +/- 74,05 (p = 0,056). Además, la sensibilidad de hTERTN fue superior a la de otras pruebas (76%), aunque la especificidad y los valores predictivos positivos y negativos fueron mejores para la citología (94%, 88,4% y 72,3% respectivamente) y NMP22 (88%, 80,6% y 73,3% respectivamente). Conclusiones: Los niveles de mRNA de hTERTN en la orina fueron más altos en pacientes con tumores vesicales en comparación con pacientes con antecedentes de tumor de vejiga y con cistoscopia negativa, así como en el grupo de control. Esta determinación mostró un mayor rendimiento diagnóstico en comparación con la detección de NMP22 y citología urinaria


Introduction: To study the relationship between quantitative mRNA determination (hTERT) in patients with bladder tumor, history of bladder tumor, and in subjects without a history of this neoplasia. Material and methods: A prospective randomized controlled study with 91 subjects included. The value of mRNA-hTERTN was determined in 63 patients with a history or suspicion of bladder tumor and in 28 controls. Urine samples were sent for evaluation of the mRNA level (hTERT), the cytological study and the NMP22 result. Results: Differences were observed in mean hTERTN levels in each of the groups: tumor presence 21.33+/- 40.66, tumor history 2.16 +/- 2.67, controls 0.9 +/- 1, 75 (p < 0.001). In patients with tumor, there was no difference in mean hTERTN levels between the different grades and stages, although there was a tendency: low grade tumor 9.04 +/- 16.95, high grade 28.95 +/- 48.36 (p = .069), stage Ta 10.33+/- 19.39, T1 17.88 +/- 27.14, T2 54.8 +/- 74.05 (p = .056). In addition, the sensitivity of hTERTN was superior to that of other test (76%), although specificity and positive and negative predictive values were better for cytology (94%, 88.4% and 72.3% respectively) and NMP22 (88%, 80.6% and 73.3% respectively). Conclusions: hTERTN mRNA levels in urine were higher in patients with bladder tumors compared to patients with a history of bladder tumor and with negative cystoscopy, as well as in the control group. This determination showed a higher diagnostic yield compared with the detection of NMP22 and urinary cytology

El análisis citológico tiene un papel fundamental en el estudio de los nódulos tiroideos. Sin embargo, hasta un 30% de estos muestran citologías indeterminadas (Bethesda III o IV). En estos casos, se realizan cirugías diagnósticas que únicamente demuestran malignidad en un 15-35% de los pacientes. Se precisa una herramienta de mayor precisión para determinar la benignidad o malignidad del nódulo tiroideo con citología indeterminada sin precisar cirugías diagnósticas, evitando así posibles complicaciones y/o costes innecesarios. El uso complementario de paneles moleculares junto con la citología ha sido, de momento, la única herramienta que parece ayudar en este difícil escenario. Se realiza una revisión de la bibliografía sobre el estudio molecular complementario de los nódulos tiroideos para tratar de resumir las características intrínsecas de cada uno de los test disponibles, su coste-efectividad, y determinar sus indicaciones y su aplicabilidad en la práctica clínica habitual


Even though cytology remains the gold standard to assess the nature of thyroid nodules, up to 30% of the results are indeterminate (Bethesda III and IV). In these cases, current guidelines recommend performing diagnostic surgery, which proves malignancy in only 15-30% of cases. A more precise method is needed to avoid unnecessary surgeries, surgical complications and costs in the process of diagnosing indeterminate nodules. Complementary use of molecular profiling tests seems to help in this complex scenario. We present a review of the current literature on the usefulness of molecular profiling of thyroid nodules so as to define its indications, costs and usability for clinical practice

Introduction: the incidence of cystic pancreatic lesions (CPL) in the asymptomatic population is increasing. Achieving a preoperative diagnosis of CPL still remains a challenge. Objectives: to evaluate the diagnostic accuracy of the cytological diagnosis of CPL from samples obtained by cytology brush versus standard endoscopic ultrasound fine needle aspiration (EUS-FNA). Methods: a multicenter, randomized, open-label trial was performed of EUS-cytology brush (EUS-EB) versus EUS-FNA for the cytological diagnosis of CPL. Patients that underwent EUS-FNA with a CPL > 15 mm were included and randomized into two groups: group I, EUS-EB; group II, EUS-FNA. The final diagnosis was based on the histological evaluation of surgical specimens and clinical parameters, imaging and a five year follow-up in non-operated patients. The main outcome was the diagnostic accuracy of both methods. Secondary outcomes were the diagnostic adequacy of specimens and the rate of adverse events. Data were compared using the Chi-squared test. An intention to treat (ITT) and per-protocol (PP) analysis were performed. Results: sixty-five patients were included in the study, 31 in group I and 34 in group II. Three patients initially randomized to group I were changed to group II as it was impossible to obtain a sample using the brush. The mean size of the CPL was 28.2 mm (range 16-60 mm). The diagnostic accuracy of EUS-EB was not superior to EUS-FNA, neither in the ITT nor the PP analysis (44.8% vs 41.1%, p = 0.77 and 38.4% vs 45.9%, p = 0.55). Conclusions: EUS-EB does not improve the diagnostic accuracy of CPL in comparison with EUS-FNA

Background and aim: The human papilloma virus is the leading cause of anal squamous cell carcinoma. Cytological screening may reduce the associated morbidity and mortality. The aim of the study was to estimate the agreement between anal cytological examination, histopathology and anoscopic visual impression. Methods: A prospective study of patients who underwent anal dysplasia screening between 2011 and 2015, in a proctology clinic of a tertiary referral center. Results: During the study period, 141 patients (91% men, 87% with HIV infection) underwent 175 anal cytology tests. Of these, 33% were negative for intraepithelial lesions or malignancy (NILM), 22% were atypical squamous cells of uncertain significance (ASCUS), 33% were low-grade squamous intraepithelial lesion (LSIL) and 12% were high-grade squamous intraepithelial lesion (HSIL). With regard to anoscopic visual impression, 46% of patients had no lesions and excision/biopsy of the identified lesions was performed in the remaining patients. The weighted kappa-agreement between abnormal cytological results and anoscopic visual impression was moderate (k = 0.48). The weighted kappa-agreement between simultaneous anal cytological examinations and anal histopathologic findings was low (kappa = 0.20). With regard to the histological examination of cases with HSIL or superficially invasive squamous cell carcinoma, 64% of patients had dysplasia of a lower grade according to the cytological analysis (6 ASCUS, 18 LSIL and 4 NILM). Conclusion: There was a poor correlation between anal cytology, histopathology and anoscopic visual impression and a high number of histological studies of HGD that were of a lower dysplastic degree according to the cytological examination. Therefore, anal cytology screening should not be used as the sole method of anal dysplasia screening (AU)

Con el objetivo evaluar el desempeño y la calidad de la etapa postanalítica del cribado citológico en el Hospital Nacional Docente Madre Niño «San Bartolomé» se diseñó una investigación de tipo analítico-correlacional con muestreo prospectivo y análisis transversal en todos los frotices referidos de redes y microrredes de salud durante 2016. Se evaluaron 8 parámetros (rotulación de láminas, marcaje de láminas, reporte de resultados, emisión de resultados, archivo de láminas, almacenamiento, sistema informático y errores postanalíticos) (α-Crombach=0,823) en base a la percepción de todos los técnicos, citotecnólogos y citopatólogos del hospital, y a 21 estudiantes de la especialidad de Citología de la Universidad Nacional Federico Villarreal. El procesamiento citológico reportó un rendimiento global adecuado (29 puntos) siendo 18 y 34 el puntaje mínimo y máximo, respectivamente. Esta valoración difiere significativamente en los ítems marcaje de lámina (p=0,722) y archivo de láminas (p=0,503). Se evidenció una diferencia global por grupo de evaluación (p<0,027). Se estableció una correlación directa entre el tipo de evaluador y el total de errores postanalíticos (rho=0,534; p<0,005). Estos procesos deben de tener un cumplimiento de requisitos interrelacionados de manera coordinada y dinámica que permita mantener la identidad de la unidad: muestra-solicitud-usuaria. La evaluación del desempeño y la calidad de la etapa postanalítica para la detección del cáncer de cuello uterino mostro un desempeño «adecuado» que difiere de evaluaciones de otras instituciones. Esta nueva aproximación señala la importancia de los procesos postanalíticos relacionados con rescreening, las necesidades operativas y las metas organizacionales (AU)


In order to evaluate the performance and quality of the post-analysis stage of cervical screening in the "Madre Niño" St Bartholomew's National Teaching Hospital, a correlational analytical study was designed, with prospective sampling and transversal analysis of all the smears referred from the health networks during 2016. Eight criteria were assessed (labelling of slides, marking of slides, reporting of results, sending of results, filing of slides, storage of slides, computer programme and post-analytic errors) (α-Crombach»=»0.823) based on the perception of all the technicians, cytotechincians and cytopathologists in the hospital as well as 21 cytology students at the Federico Villareal National University. Cytology processing reported adequate global efficiency, with an average of 29 points and a range of 18 to 34. This assessment differs significantly in the marking of the slides (P=.722) and filing of slides (P=.503). A global difference was seen per evaluation group (P<.027). A direct correlation was established between the type of assessor and the total number of post-analytical errors (rho=0.534; P<.005). These processes should fulfil certain requirements that are inter-related in a coordinated and dynamic way, in order to maintain the identity of the unit: sample-request-user. The assessment of the performance and quality of the post-analytic phase in the detection of cervical cancer revealed an "adequate" performance which differs from the assessments in other centres. This new approximation points out the importance of the post-analytic processes related to rescreening, the operative needs and the organizational goals (AU)

Objetivo. Analizar la frecuencia del control mamográfico, del control mamográfico a través del programa poblacional y del control citológico según comunidad autónoma y clase social en mujeres españolas de 25 a 69 años. Métodos. Estudio observacional. La fuente de información fue la Encuesta Nacional de Salud (ENS) correspondiente a los años 2003, 2006 y 2011. La población de estudio fueron las mujeres de 25 a 69 años (N=7.844 ENS-2003, N=12.845 ENS-2006 y N=7.695 ENS-2011). Las variables dependientes: control mamográfico periódico, control mamográfico periódico vía programa poblacional y control citológico periódico. Variables independientes: edad, clase social (manual/no manual), comunidad autónoma, años de inicio del programa y ENS. Se calculó la razón de prevalencia con su intervalo de confianza al 95% (IC 95%). Resultados. En el año 2006, el 49,2% de las mujeres de clase no manual realizaron un control mamográfico periódico vía programa poblacional frente al 61,5% de clase manual (razón de prevalencia ENS-2006: 0,81; IC 95% 0,75-0,88). En la ENS-2011, el 53,6% de las mujeres de clase social no manual realizaron un control mamográfico periódico vía programa poblacional frente al 68,0% de las de clase manual. Para la ENS-2003, las mujeres de clase social no manual realizaron un 90,2% de controles citológicos periódicos respecto al 86,7% de las clases manuales (razón de prevalencia ENS-2003: 1,04; IC 95% 1,01-1,06). Conclusiones. No se observan diferencias por clase social en la realización de controles mamográficos periódicos. Los programas poblacionales de cáncer de mama compensan las diferencias de clase en la práctica del control mamográfico. En el control citológico son las mujeres de clase social no manual las que realizan una mayor proporción (AU)


Objective. To analyse the frequencies of mammographic screening, mammographic follow-up through a population-based screening programme, and cytological screening by autonomous community and social class in Spanish women aged 25 to 69 years. Methods. Observational study. The information source was the National Health Surveys (NHS) for 2003, 2006 and 2011. The study population consisted of women aged 25 to 69 years (N=7,844 for NHS-2003, N=12,845 for NHS-2006 and N=7,695 for NHS-2011). The dependent variables were periodic mammographic screening, periodic mammographic screening through a population-based programme, and periodic cervical screening. The independent variables were age, social class (manual/non-manual), autonomous community, the number of years since the start of the programme, and year of the NHS. Prevalence ratios with their 95% confidence intervals (95% CI) were calculated. Results. In 2006, participation in periodic population-based mammographic screening was 49.2% among women from a non-manual social class versus 61.5% among those from a manual social class (prevalence ratios NHS-2006: 0.81; 95% CI 0.75-0.88). In NHS-2011, participation was 53.6% among women from a non-manual social class versus 68.0% among those from a manual social class. For NHS-2003, participation in periodic cytological screening was 90.2% among women from a non-manual social class versus 86.7% among those from a manual social class (prevalence ratios NHS-2003: 1.04; 95% CI 1.01-1.06). Conclusions. No differences were observed by social class in periodic mammographic screening. Population-based breast cancer screening programmes compensate for differences in social class in mammographic follow-up. Cytological screening was more frequent among women from a non-manual social class (AU)

Objectives. To evaluate the independent usefulness of pleural fluid smear and cell block (CB) preparations for the diagnosis of malignant effusions. Patients and methods. A total of 632 cytological smears and 554 CBs from 414 consecutive patients with malignant effusions were retrospectively evaluated. Results. The diagnostic yield of a first specimen was 44% regardless of whether a smear or CB cytologic examination was performed. The use of subsequent separated specimens increased the identification of malignancy to 56%. Overall, 11% of samples found to be negative by cytologic smears showed malignant cells on CBs, whereas 15% of negative CBs were reported as positive on smear slides. Pleural fluid specimens with low red and/or white blood cell counts more frequently resulted in the generation of suboptimal CB preparations. Conclusions. If CBs and smears are prepared and examined, the percentage of positive diagnoses will be greater than if only one method is used (AU)


Objetivos. Evaluar la utilidad independiente de frotis y bloques celulares (BC) del líquido pleural para diagnosticar derrames malignos. Pacientes y métodos. Se evaluaron retrospectivamente un total de 632 frotis citológicos y 554 BC de 414 pacientes consecutivos con derrame pleural maligno. Resultados. La sensibilidad diagnóstica de una primera muestra fue del 44%, tanto en frotis como en BC. El análisis de muestras separadas ulteriores aumentó al 56% la identificación de derrames malignos. Globalmente, el 11% de muestras negativas mediante frotis mostraron células malignas en los BC, mientras que el 15% de BC negativos resultaron positivos en el estudio del frotis. Los líquidos pleurales con recuentos bajos de hematíes o leucocitos produjeron con mayor frecuencia BC insuficientes para diagnóstico. Conclusiones. Si se evalúan frotis y BC, el porcentaje de resultados positivos es superior que si se emplean estas técnicas de forma aislada (AU)

Antecedentes y objetivos: los hombres VIH que tienen sexo con hombres (VIH-HSH) presentan la mayor incidencia de cáncer anal (CA). El estudio tiene como objetivo conocer la prevalencia de la infección anal por virus del papiloma humano de alto riesgo (VPH-AR) y de los hallazgos citológicos e histológicos, así como la implementación de un programa de cribado en el contexto de una ciudad media española, Vigo (España). Método: análisis prospectivo de una cohorte de 264 hombres (VIH-HSH). A los pacientes se les realizó una citología anal y genotipado de VPH-AR para el estudio de la prevalencia de alteraciones citológicas y de la infección VPH-AR. A 209 se les realizó una anoscopia de alta resolución (AAR). Los resultados se relacionaron con variables epidemiológicas, clínicas y analíticas. Resultados: de los 209 pacientes seleccionados, la prevalencia de VPH-AR, alteraciones citológicas e histológicas anales fue de 85,6%, 47,5% y 39,3% respectivamente. La sensibilidad y especificidad para la citología ≥ ASCUS (atipia de células escamosas de significado indeterminado) respecto a las alteraciones histológicas fue de 61% y 85% (OR: 8,7; IC95%: 4,4-17,2), respectivamente. La concordancia observada entre la citología HSIL (lesión escamosa intraepitelial de alto grado) e histología HSIL (AIN-2/3, neoplasia intraepitelial tipo 2 y 3) fue del 64%,(OR: 11,4; IC95%: 3,5-36,7). Un paciente con citología HSIL presentó cáncer escamoso anal prevalente. Conclusiones: la AAR resultó factible y con resultados similares a los de grupos relevantes. Gran prevalencia de la infección anal por VPH-AR y de alteraciones citológicas e histológicas (AU)


Background: Men who have sex with men (MSM) infected with human immunodeficiency virus (HIV) have the highest risk of developing anal cancer (AC). The objective of this study was to describe our screening implementation program in this population, and report the prevalence of human papillomavirus (HPV) anal infection, and cytological and histological findings in a Spanish medium-size community (Vigo, Spain). Method: Prospective cohort analysis of 240 HIV-infected MSM. Cellular anal sample and high risk HPV (HR-HPV)-tests were performed to study cytological changes and HPV genotyping. High resolution anoscopy (HRA) was performed in 209 patients. Results were analyzed with respect to epidemiological, clinical and analytical factors. Results: Of 209 patients selected for HRA, the prevalence of HR-HPV anal infection, cytological and histological alterations was 85.6%, 47.5%, and 39.8%, respectively. Sensitivity and specificity for ≥ ASCUS (atypia of squamous cells of undetermined significance) cytology in relation to histological alterations were 61% and 85%, (OR: 8.7; IC 95%: 4.4-17.2), respectively. Observed concordance between high-grade squamous intraepithelial lesion (HSIL) cytology and HSIL anal intraepithelial neoplasia types 2 and 3 (AIN-2/3) histology was 64% (OR: 11.4; IC 95%: 3.6-36.7). One patient with HSIL cytology presented a prevalent anal squamous carcinoma. Conclusions: HRA was feasible with similar results to relevant groups. There was a high prevalence of anal HR-HPV infection, and cytological and histological alterations (AU)

RESUMO

Endoscopic ultrasound guided fine needle aspiration (EUSFNA) has become an essential tool in the management of multiple diseases. Its accuracy is related to different aspects of the technique, one of the most important being the experience and interaction of the endosonographer and pathologist. Certain studies over the past years have highlighted the importance of having rapid on-site evaluation (ROSE) of samples obtained at the time of EUS-FNA. We have reviewed the role of ROSE, performed by the same endosonographer, a cytotechnologist and an expert cytopathologist. The available data suggest that ROSE (either by the endosonographer, the cytotechnologist, or the cytopathologist) improves sample adequacy and diagnostic yield, with the best option to have ROSE performed by an expert cytopathologist. However, if non-ROSE accuracy is already very high, any improvement is harder to achieve (AU)

INTRODUCCIÓN: La neoplasia intraepitelial anal de alto grado (NIAAG) está en aumento en determinados grupos de riesgo y en su etiopatogenia están implicados algunos genotipos del virus del papiloma humano (VPH). El cribado de la NIAAG contempla el uso sistemático de la citología anal y más recientemente el genotipado de VPH. Nuestro objetivo fue determinar la sensibilidad y especificidad de ambas herramientas diagnósticas en la identificación de NIAAG. MATERIAL Y MÉTODO: Estudio de correlación entre los hallazgos citológicos y microbiológicos con respecto a la biopsia anal de una cohorte de pacientes con conductas de riesgo de desarrollar neoplasia intraepitelial anal atendidos en la consulta de infecciones de transmisión sexual del área de Dermatología del Hospital Costa del Sol desde enero de 2008 a diciembre de 2014. RESULTADOS: De los 151 pacientes sometidos al cribado, se seleccionaron aquellos pacientes con las tres pruebas de cribado realizadas (citología anal, genotipado y biopsia guiada por anoscopia), 92 en total, de los que el 62% presentaban infección por VIH. La sensibilidad y especificidad para identificar NIAAG fue 52,8 y 85,7% para la citología anal (k: 0,328), y 78 y 62,8% de la presencia de dos o más genotipos oncogénicos VPH (k: 0,417). La detección de VPH oncogénicos permitió clasificar correctamente 23 casos de NIAAG confirmados por biopsia guiada por anoscopia e infradiagnosticados con la citología anal, 14 de ellos con al menos 3 genotipos de riesgo. CONCLUSIÓN: La citología anal ha mostrado una sensibilidad insuficiente para la detección de NIAAG. El genotipado del VPH, aunque como única herramienta de cribado daría lugar a un sobrediagnóstico, es una herramienta que puede complementar el procedimiento de cribado, especialmente con el objetivo de identificar los casos de NIAAG


INTRODUCTION: The incidence of high-grade anal intraepithelial neoplasia (HGAIN) -with an aetiological based on high-risk types of human papillomavirus- is increasing in some high-risk groups. Screening for HGAIN includes routine anal cytology and, more recently, HPV genotyping. The main objective of this study was to determine the sensitivity and specificity of anal cytology and HPV genotyping for the detection of HGAIN. MATERIALS AND METHODS: This is a study to determine the correlation of cytological and microbiological findings with anal biopsy findings in a cohort of patients at high risk of developing AIN referred to the department of sexually transmitted infections of the Hospital Costa del Sol, Spain, between January 2008 and December 2014. RESULTS: Of the 151 patients subjected to screening, a total of 92 patients, all of them with the result of three screening test (anal cytology, genotyping and biopsy) were included in the study. Just under two-thirds (62%) of them were HIV-positive. The sensitivity and specificity of anal cytology to detect HGAIN were 52.8 and 85.7%, respectively (k: 0.328), and 78 and 62.8% to detect two or more HPV oncogenic genotypes (k: 0.417). The detection of oncogenic HPV genotypes allowed the identification of 23 new cases of HGAIN that had been underdiagnosed with anal cytology, with 14 cases containing at least three high-risk genotypes. CONCLUSION: Anal cytology did not show enough sensitivity in HGAIN screening. HPV genotyping has shown to be a useful tool to detect HGAIN cases, although it could lead to an over-diagnosis as a solitary screening procedure

RESUMO

We report a case of a 70-year old woman with a high-grade myxofibrosarcoma which presented as a subcutaneous tumor in the posterior right thigh. Fine-needle aspiration revealed isolated and three-dimensional groups of spindle cells with high-grade atypia and a myxoid background. The cytoarchitecture of the tumor showed non-distinctive, highly cellular spindle cell morphology, whilst other areas had a very prominent myxoid matrix with elongated, curvilinear blood vessels. A few scattered mucin-containing pseudolipoblasts were also observed. The differential diagnosis with other myxoid lesions is discussed; a correct diagnosis is important as the metastatic risk is lower than most types of adult high-grade pleomorphic sarcoma (AU)


Describimos el caso de una mujer de 70 años con un mixofibrosarcoma de alto grado que se presentó como un tumor subcutáneo en la región posterior del muslo. El estudio de punción-aspiración demostró células aisladas y grupos tridimensionales de células fusiformes, con atipia de alto grado y fondo mixoide, con vasos sanguíneos elongados, curvilíneos. Las imágenes citoarquitecturales de este tumor mostraron una morfología fusiforme no distintiva altamente celular, y otras áreas tienen matriz mixoide muy prominente. Además se observaron aislados pseudolipoblastos que contenían mucina. Discutimos brevemente el diagnóstico diferencial con otras lesiones mixoides. La importancia de establecer un diagnóstico correcto tiene implicaciones pronósticas, ya que el riesgo metastásico es menor que en otros tipos de sarcomas pleomórficos de alto grado del adulto (AU)

Numerosas técnicas de imagen permiten visualizar y obtener material de órganos de difícil acceso de forma mínimamente invasiva. Es preciso un estudio citológico adecuado, porque el calibre habitual de las agujas no permite obtener material histológico. Por ello, ligado al uso de la ecoendoscopia se debe hablar de punción aspiración con aguja fina (PAAF). La PAAF es una técnica económica cuando se realiza sobre lesiones palpables, pero con control por técnicas de imagen su rentabilidad es dudosa por la variabilidad entre centros y el aumento del coste. El objetivo de este estudio es determinar si el control por el patólogo del material obtenido en PAAF con ecoendoscopia mejora la precisión diagnóstica (en relación con la biopsia), cuál es la mejor estrategia para esta prueba, quién debe hacerla y si resulta económicamente rentable. Dada la escasez de datos a nivel nacional sobre este tema, hemos optado por una revisión bibliográfica de la literatura publicada (AU)


New imaging techniques, allowing the visualization and minimally invasive sampling of organs that are difficult to access, are being used more and more frequently. A suitable, accompanying cytological study is required, as in the majority of cases the caliber of the needle used is not sufficient to obtain a histological sample. Therefore, endoscopic ultrasound is generally associated with fine-needle aspiration (FNA). The performance of FNA on palpable nodules is a cost-effective procedure. However, the cost-effectiveness of using FNA with endoscopic ultrasound guidance is controversial, due to interobserver variability and increase in costs. The object of this study is to determinate if rapid-on site evaluation by a pathologist during endoscopic ultrasound-guided FNA improves diagnostic accuracy (correlated with biopsy), what is the best methodology, who should do it and the cost-effectiveness of the procedure. Because of the lack of Spanish data, we reviewed the pertinent international literature (AU)
