RESUMO
Antecedentes: Los bloqueos continuos de los nervios periféricos son de uso común para el manejo del dolor. Sin embargo, la incidencia de desplazamiento o migración del catéter es dudosa, pudiendo ser infraestimada y no denunciarse. Nuestro objetivo fue evaluar el posicionamiento del extremo del catéter supraescapular antes y después de la manipulación rutinaria y simulada mediante fisioterapia de hombro en un modelo cadavérico anatómico.MétodoSe colocaron 8 catéteres para bloqueo continuo y ecoguiado del nervio supraescapular en cadáveres frescos criopreservados. La tomografía computarizada (TC) confirmó la localización de la punta del catéter tras la inyección de 1ml de medio de contraste. Realizamos una serie de movimientos estandarizados de hombro durante una sesión simulada de fisioterapia de hombro en cadáveres. Tras ello, administramos 1ml de azul de metileno a través de los catéteres, y seguidamente realizamos disecciones para identificar con precisión la localización de las puntas del catéter y compararlas con su localización previa a la «fisioterapia».ResultadosLa imagen de la TC confirmó la localización en todos los casos de los extremos del catéter en la muesca supraescapular. Sin embargo, tras la fisioterapia, se encontró que 2 catéteres (25%) habían migrado y, en especial, uno fue localizado en el músculo supraespinoso y el otro en el músculo trapecio.ConclusiónNuestros hallazgos sugieren que el desplazamiento del catéter puede producirse en cerca del 25% de los casos tras la manipulación mediante fisioterapia simulada. Sin embargo, es necesaria más investigación para determinar la incidencia real de los desplazamientos del catéter en la práctica clínica. (AU)
Background: Continuous peripheral nerve blocks are commonly used for pain management. However, the incidence of catheter dislodgement or migration is unclear, and may be underestimated and underreported. Our objective was to assess suprascapular catheter tip positioning before and after routine simulated shoulder physiotherapy manipulation in an anatomical cadaver model.MethodEight ultrasound-guided continuous suprascapular nerve block catheters were placed in cryopreserved fresh cadavers. Computed tomography (CT) confirmed the location of the catheter tip after injection of 1ml of contrast medium. We performed a series of standardized shoulder movements during a simulated shoulder physiotherapy session in cadavers. Following this, we administered 1ml of methylene blue through the catheters, and then performed anatomical dissections to accurately identify the location of the catheter tips and compare them to their placement prior to the physiotherapyResultCT imaging confirmed the location of the catheter tips at the suprascapular notch in all cases. However, following physiotherapy, 2 catheters (25%) were found to have migrated - specifically, 1 was located in the supraspinatus muscle, and the other was located in the trapezius muscle.ConclusionOur findings suggest that catheter dislodgement may occur in approximately 25% of cases following simulated physiotherapy manipulation. However, further research is needed to determine the read incidence of catheter dislodgement in clinical practice. (AU)

La cavitación intracoroidea es un hallazgo identificado con OCT descrito inicialmente en pacientes miopes, pero también aparece en pacientes no miopes. Puede presentarse tanto en el área peripapilar como en el polo posterior. El coloboma macular es un defecto del desarrollo embrionario del polo posterior, y en la OCT estructural es imprescindible la ausencia del epitelio pigmentario de la retina y de los vasos coroideos para su diagnóstico. Este caso presenta la cavitación intracoroidea circunscribiendo el coloboma macular, en ausencia de membrana intercalar. La imagen en face permite valorar la relación entre ambas estructuras, así como la magnitud de las mismas. (AU)
Intrachoroidal cavitation is a finding identified with OCT initially described in myopic patients, it also appears in non-myopic patients. It can occur in both the peripapillary area and the posterior pole. Macular coloboma is a defect of embryonic development of the posterior pole, in structural OCT the absence of the retinal pigment epithelium and choroidal vessels is essential. In this case, intrachoroidal cavitation circumscribes the macular coloboma, in the absence of an intercalary membrane. The face image allows us to assess the relationship between the two structures as well as their magnitude. (AU)

Propósito: Evaluar las dimensiones del músculo ciliar (MC) y del grosor escleral anterior (AST) in vivo en miopes altos mediante tomografía de coherencia óptica de fuente de barrido (SS-OCT) y comparar con sujetos emétropes e hipermétropes.MétodosEstudio transversal en el que se incluyeron 34 miopes altos (≥−6dioptrías [D]), 90 emétropes (−1 a +1D) y 38 hipermétropes (≥+3,5D). Se midieron el grosor del MC (CMT) y el AST en los cuadrantes temporal y nasal a 1, 2 y 3mm del espolón escleral utilizando la SS-OCT. Además, se evaluó la longitud del MC (CML).ResultadosLas dimensiones tanto del CML como del CMT en cualquiera de sus puntos de medida fueron mayores en miopes altos y en emétropes que en hipermétropes, tanto en el cuadrante nasal como en el temporal (p<0,001). Sin embargo, no existieron diferencias entre miopes magnos y emétropes para ninguno de los parámetros (p≥0,076), salvo para el CMT a 3mm en temporal (p<0,001). No existieron diferencias en el AST entre miopes altos, emétropes e hipermétropes, en ninguno de los puntos de medida ni cuadrantes estudiados (p>0,05).ConclusionesLa SS-OCT permite medir el MC in vivo, no observándose diferencias en sus dimensiones entre miopes altos y emétropes, pero sí que fueron menores en hipermétropes. En la medida de la esclera anterior no se observaron diferencias entre los tres grupos analizados según la refracción. (AU)
Purpose: To assess ciliary muscle (CM) and anterior scleral thickness (AST) dimensions in vivo in high myopia using swept-source optical coherence tomography (SS-OCT) and to compare with emmetropic and hyperopic subjects.MethodsCross-sectional study that included 34 high myopic patients (≥−6 diopters [D]), 90 emmetropes (−1 to +1D) and 38 hyperopic patients (≥+3.5D). CM thickness (CMT) and AST were measured in the temporal and nasal quadrants at 1, 2, and 3mm from the scleral spur using SS-OCT. In addition, the length of the CM (CML) was evaluated.ResultsThe dimensions of the CML and the CMT at any of their measurement points were greater in high myopes and emmetropes than in hyperopes, both in the nasal and temporal quadrants (P<.001). However, there were no differences between high myopes and emmetropes for any of the parameters (P≥.076) except for the CMT at 3mm in the temporal quadrant (P<.001). There were no differences in the AST between high myopes, emmetropes and hyperopes, in any of the measurement points or quadrants studied (P>.05).ConclusionsThe SS-OCT allows to measure the CM in vivo, not observing differences in its dimensions between high myopes and emmetropes, but they were smaller in hyperopes. In the measurement of the anterior sclera, no differences were observed between the three groups analyzed according to refraction. (AU)

Las enfermedades difusas del tejido conectivo con frecuencia desarrollan enfermedad pulmonar intersticial, lo que conlleva peor pronóstico y acorta la supervivencia. La tomografía axial computarizada de alta resolución es la prueba diagnóstica de elección, ya que esta es muy competitiva con la histopatología; sin embargo, el costo y la radiación pueden limitar su empleo, particularmente como escrutinio. El ultrasonido pulmonar, estudio rápido, de acceso fácil, reproducible y de menor costo, resulta muy atractivo para determinar la existencia de enfermedad pulmonar intersticial. Adicionalmente, se requiere de poca experiencia para determinar las alteraciones correlacionables con estos padecimientos pulmonares. Las líneas B y las irregularidades pleurales conforman el denominado síndrome intersticial ultrasonográfico, aunque debemos tener en mente que no es específico y estamos obligados a considerar anormalidades hemodinámicas, cardiovasculares e infecciosas. En esta revisión, exponemos la alta prevalencia de esta enfermedad pulmonar en los principales padecimientos reumatológicos, con énfasis en la utilidad del ultrasonido pulmonar, su facilidad de realización y alto desempeño diagnóstico. Patients with diffuse connective tissue diseases frequently develop interstitial lung disease, which carries a worse prognosis and shortens survival. High-resolution computed tomography is the first-choice test, and is competitive with histopathology, however, the cost and radiation may limit its use, particularly for screening. Lung ultrasound is a rapid, accessible, reproducible, and inexpensive study that is useful for diagnosis of interstitial lung disease. Furthermore, extensive training is not required to identify the alterations associated with these lung diseases. B lines and pleural irregularities compose the ultrasonographic interstitial syndrome, although it must be kept in mind that it is not specific, and it is necessary to rule out haemodynamic, cardiovascular, and infectious abnormalities. This review highlights the elevated prevalence of this lung condition in the main rheumatological diseases, with emphasis on the usefulness of pulmonary ultrasound. (AU)

La endometriosis torácica afecta principalmente a mujeres jóvenes y es originada por la migración de tejido endometrial a la cavidad torácica. Se suele presentar como neumotórax, hemoptisis, hemotórax o nódulos pulmonares. El tratamiento hormonal es el enfoque de primera línea. La cirugía actúa como herramienta diagnóstico-terapéutica y sin embargo el diagnóstico definitivo es histológico. Presentamos el caso de una mujer con diagnóstico de endometriosis pélvica en tratamiento médico a quien se le detecta hemotórax, siendo tratada con cirugía que permitió visualizar implantes pulmonares y diafragmáticos. Thoracic endometriosis mainly affects young women and is caused by the migration of endometrial tissue into the thoracic cavity. It usually presents as pneumothorax, hemoptysis, hemothorax, or pulmonary nodules. Hormone treatment is the first line approach. Surgery acts as a diagnostic-therapeutic tool and yet the definitive diagnosis is histological. We present the case of a woman with a diagnosis of pelvic endometriosis undergoing medical treatment who was detected with a hemothorax, being treated with surgery that allowed the visualization of pulmonary and diaphragmatic implants. La tomografía computarizada permite una representación más detallada de las estructuras óseas que la resonancia magnética, lo que determina que sea la prueba de elección para la planificación de la cirugía otológica.El objetivo de este trabajo es el de elaborar una lista de verificación o checklist que permita al otorrinolaringólogo estudiar y valorar de forma sistemática y organizada las principales estructuras de referencia, variantes anatómicas y cambios posquirúrgicos más frecuentes antes de una cirugía segura.Esta revisión ha sido promovida por la Sociedad Española de Otorrinolaringología y redactada en un formato de lista de verificación dividida en las diferentes regiones del hueso temporal y base de cráneo lateral. Computed tomography scan of the temporal bone is a fundamental imaging modality for both the diagnosis and treatment of a wide range of pathologies affecting this complex structure. Temporal bone computed tomography scan provides a more detailed depiction of bone structures, compared with magnetic resonance imaging and, for this reason computed tomography scan is the imaging modality of choice in the planning of otological surgery.The aim of this article is to present a checklist to allow the otolaryngologist to assess systematically and in an organized manner the main anatomical landmarks, anatomical variants, as well as the most common postoperative surgical changes, which can be identified before any safe otological surgery.This revision was promoted by the Spanish Society of Otolaryngology and elaborated in a checklist template divided into the different areas of the temporal bone and the lateral skull base. Background and Objectives: Spectral-domain optical coherence tomography (SD-OCT) findings in substance use disorders have been investigated in recent years. In this study, we compared the retinal nerve fiber layer (RNFL), the ganglion cell layer (GCL), the inner plexiform layer (IPL), and the choroid thickness (CT) of OUD and control groups before and after buprenorphine/naloxone maintenance treatment (BN-MT).MethodsThe OUD group consisted of 46 male subjects and the control group consisted of 49 male subjects. Patients with chronic opioid use and opioid positivity in their urine during the initial SD-OCT application were included in the study. At the end of the fourth week of BN-MT, SD-OCT was repeated and BN positivity was detected in the urine of the patients at this time.ResultsThere was a significant difference between OUD and control groups in terms of nasal superior and CT values of both eyes (p<0.05) before BN use. The values of RNFL sectors and CT of both eyes before and after BN-MT differed significantly (p<0.05); CT increased and RNFL sectors decreased. After BN-MT, psychometric scales differed significantly in favor of the patients (p<0.05). The SD-OCT values of the OUD group after BN-MT were compared with the control group: the right IPL (p=0.003), the left IPL (p=0.023), the right N (p=0.001) and the left N (p<0.001) values were significantly lower in the OUD group.ConclusionThis is the first study to show the SD-OCT findings of patients with OUD before and after BN-MT. The findings of this study may indicate possible effects of chronic opioid use in patients and/or possible effects of exogenous opioid or BN present in the body during SD-OCT applications. However, based on our findings, it is not possible to distinguish between the two possible outcomes. The fact that the use of BN acting through opioid receptors has different effects from exogenous opioids may be due to different receptor profiles. (AU)

Objective: The organizing pneumonia (OP) pattern is the second most common finding in anti-synthase antibody syndrome (ASS), and nonspecific interstitial pneumonia (NSIP) is the most common finding. This study analysed the OP score changes by semiquantitative and quantitative analysis methods and the correlation between the high-resolution computed tomography (HRCT) indexes and the pulmonary function test parameters (PFTs) in ASS patients.MethodsData from ASS-OP patients admitted to the respiratory department of Ping Jin Hospital from October 2014 to June 2020 were retrospectively reviewed and analysed.ResultsFourteen ASS-OP patients were recruited for this study. (1) In method-1, the consolidation (CO) score and the mean lung attenuation (MA) of poorly aerated and fibrosis lung fields (MAfibrosis) (r=0.56, P=0.04), the ground-glass opacity (GGO) score and the MA of non-aerated lung fields (MAnonaerated) (r=−0.64, P=0.01), and the CO plus the GGO (CG) score and the MAnonaerated (r=−0.59, P=0.03) of the lung fields had liner correlations. In method-2, the GGO score to the MAnonaerated (r=−0.58, P=0.03), and the CG (r=−0.68, P=0.01) score to the MAnonaerated had liner correlations. The FVC% (r=0.68, P=0.01) and FEV1% (r=0.64, P=0.01) to the MAfibrosis had good linear correlations. (2) Compared to the values before treatment, the CO pattern score, volume and weight percentages of the extracted whole lung volume with attenuation values of the nonaerated area (Vnonated%, Wnonaerated%), the volume of poorly aerated and fibrosis lung tissue (Vfibrosis%, Wfibrosis%), the weight percentages of normal aerated lung (Wnormal%), and the MAfibrosis exhibited significant differences during the 36 month follow-up period. Objetivo: El patrón de neumonía organizada (PNO) es el segundo hallazgo más común en el síndrome de anticuerpos antisintasa (SAA), y la neumonía intersticial inespecífica es el hallazgo más común. Este estudio analizó los cambios en la puntuación de PNO mediante métodos de análisis semicuantitativos y cuantitativos, y la correlación entre los índices de la tomografía computarizada de alta resolución y los parámetros de las pruebas de función pulmonar en pacientes con SAA.MétodosSe revisaron y analizaron retrospectivamente los datos de los pacientes SAA-PNO ingresados en el departamento respiratorio del Hospital Ping Jin desde octubre de 2014 hasta junio de 2020.ResultadosSe reclutaron 14 pacientes SAA-PNO para este estudio. (1) En el método 1, la puntuación de consolidación (PC) y la atenuación pulmonar media (APM) de los campos pulmonares mal aireados y fibrosis (APMfibrosis) (r=0,56; p=0,04), la puntuación de la opacidad del vidrio molido (OVM) y la PC de los campos pulmonares no aireados (APMnoaireado) (r=−0,64; p =0,01), y la PC más la puntuación OVM (PV) y el APMnoaireado de los campos pulmonares tenían correlaciones de revestimiento. En el método 2, la puntuación de OVM para el APMnoaireado (r=−0,58; p =0,03) y la puntuación de PV (r=−0,68; p=0,01) para el APMnoaireado tenían correlaciones de revestimiento. El FVC% (r=0,68; p =0,01) y el FEV1% (r=0,64; p =0,01) a la APMfibrosis tuvieron buenas correlaciones lineales. (2) En comparación con los valores anteriores al tratamiento, la puntuación del patrón de PC, los porcentajes de volumen y peso del volumen pulmonar total extraído con los valores de atenuación del área no aireada (Vnoaireado%, Pnoaireado%), el volumen de tejido pulmonar poco aireado y fibroso (Vfibrosis%, Pfibrosis%), los porcentajes de peso del pulmón aireado normal (Pnormal%) y la APMfibrosis mostraron diferencias significativas durante el período de seguimiento de 3 a 6 meses. Antecedentes: El ensayo clínico SLICE no demostró un beneficio clínico de la búsqueda activa de la tromboembolia de pulmón (TEP) en pacientes que requirieron ingreso por agudización de su EPOC. El objetivo de este subanálisis fue determinar la frecuencia y el significado pronóstico del hallazgo de un diagnóstico alternativo a la TEP en la tomografía computarizada de tórax (TC) realizada a pacientes hospitalizados por EPOC agudizada. Métodos: Analizamos los pacientes del grupo intervención de SLICE que recibieron una TC con resultado negativo para TEP. Evaluamos mediante el uso de regresión logística multivariable la asociación entre un diagnóstico alternativo y el evento primario compuesto de reingreso por EPOC y la mortalidad por cualquier causa en los primeros 90 días después de la aleatorización. Resultados: Este análisis incluyó a 175 pacientes del grupo intervención que recibieron una TC con un resultado negativo para TEP. De ellos, 84 (48,0%) fueron diagnosticados de infección bronquial aguda, 13 (7,4%) de cáncer de pulmón, 10 (5,7%) de insuficiencia cardiaca, 8 (4,6%) de neumonía, 18 (10,3%) de otros diagnósticos, y en 42 (24,0%) pacientes la TC fue normal. En el análisis multivariable, un diagnóstico alternativo no se asoció de manera significativa con el evento primario (odds ratio: 0,64; IC 95%: 0,30-1,38; P=0,26). Conclusiones: Entre los pacientes que requieren ingreso por agudización de EPOC, la TC ofrece un diagnóstico alternativo a la TEP en el 76% de las ocasiones. El tratamiento específico de estas patologías no se asocia a un mejor pronóstico durante el seguimiento. (AU)

The objective of this subanalysis was to determine the frequency and prognostic significance of findings on computed tomography pulmonary angiogram (CTPA) supporting an alternative diagnosis to PE. Methods: We analyzed all patients randomized to the intervention in the SLICE trial who received a CTPA that did not show PE. We used multivariable logistic regression to assess the independent association between findings supporting an alternative diagnosis to PE and a composite of readmission for COPD or death within 90 days after randomization. Results: Among the 746 patients who were randomized, this subanalysis included 175 patients in the intervention group who received a CTPA that did not show PE. Eighty-four (48.0%) patients had acute bronchial infection, 13 (7.4%) had lung cancer, 10 (5.7%) had congestive heart failure, 8 (4.6%), 18 (10.3%) had other diagnoses, and 42 (24.0%) had a normal CTPA. In multivariable analysis, findings supporting an alternative diagnosis to PE were not significantly associated with the primary outcome (odds ratio: 0.64; 95% confidence interval: 0.30-1.38; P=0.26). Conclusions: Among patients hospitalized for an exacerbation of COPD, CTPA identified an alternative diagnosis in 76% of the patients. However, specific management of these patients was not associated with improved outcomes within 90 days after randomization. (AU)

Se presentan los casos de dos pacientes con panuveítis que, tras ser vitrectomizados, presentaron en la tomografía de coherencia óptica (OCT), depósitos prerretinianos inusuales en forma de estalagmitas hiperreflectivas, multifocales y de patrón perivascular.Caso 1: Varón de 51 años con panuveítis en el ojo derecho (OD). Tras una vitrectomía diagnóstico-terapéutica, la OCT evidenció depósitos prerretinianos tipo estalagmitas, sin una etiología clara y persistencia de los depósitos.Caso 2: Mujer de 76 años con panuveítis del OD. Tras una cirugía combinada de catarata y pelado de membrana epirretiniana, la OCT reveló depósitos prerretinianos tipo estalagmitas. La analítica reportó títulos IgG elevados para toxoplasma. Se prescribió claritromicina y corticoide con desaparición progresiva de los depósitos prerretinianos.Los depósitos prerretinianos tipo estalagmitas son un signo tomográfico poco común de los procesos uveíticos posteriores, parecen correlacionarse con la actividad inflamatoria, aunque sin un claro mecanismo y sin una etiología en particular.
Two patients with panuveitis are presented in whom, after undergoing a vitrectomy, the optical coherence tomography (OCT) revealed some unusual pre-retinal hyper-reflective deposits with the shape of stalagmites, distributed in a multifocal and perivascular pattern.Case 1: 51-year-old male with panuveitis in the right eye. After diagnostic-therapeutic vitrectomy, OCT showed pre-retinal stalagmite-like deposits. There was no clear aetiology, and the deposits persisted over the time.Case 2: 76-year-old woman with panuveitis in the right eye. After a surgery for cataract-vitrectomy and epiretinal membrane peeling, OCT revealed pre-retinal stalagmite-like deposits. The analysis reported elevated Toxoplasma IgG titres. Clarithromycin and corticosteroid were prescribed, resulting in the disappearance of pre-retinal deposits.The stalagmite-like deposits are not a very common tomographic sign of a posterior uveitis process, but they seem to correlate with inflammatory activity. There does not seem to be a clear mechanism or a particular aetiology.
