Introducción y objetivos El momento óptimo para un cateterismo en el síndrome coronario agudo sin elevación del segmento ST (SCASEST) y la necesidad de pretratamiento son motivo de controversia. El objetivo principal del registro IMPACT-TIMING-GO es conocer el porcentaje de pacientes examinados con una coronariografía precoz (0-24 h) y que no recibieron doble antiagregación plaquetaria antes del cateterismo (estrategia sin pretratamiento) en España. Métodos Estudio observacional, prospectivo y multicéntrico, que incluyó a pacientes consecutivos con diagnóstico de SCASEST sometidos a cateterismo en los que se evidenció enfermedad coronaria ateroesclerótica causal. Resultados Entre abril y mayo de 2022 se incluyó a 1.021 pacientes (media de edad, 67±12 años; el 23,6% mujeres). El 86,8% de los pacientes cumplían criterios de alto riesgo (elevación de troponina, cambios electrocardiográficos o puntuación GRACE>140); sin embargo, únicamente el 37,8% se sometió a una estrategia invasiva precoz, y el 30,3% no recibió pretratamiento. Globalmente, solo el 13,6% de los pacientes se sometieron a una estrategia invasiva precoz sin un segundo antiagregante plaquetario, y la estrategia diferida con pretratamiento fue la más utilizada (46%). Durante el ingreso, 9 pacientes (0,9%) fallecieron y 34 (3,3%) presentaron una hemorragia grave. Conclusiones En España, solo el 13,6% de los pacientes con SCASEST sometidos a cateterismo reciben una estrategia invasiva precoz sin pretratamiento. La incidencia de eventos cardiovasculares y hemorragias graves en el ingreso es baja. (AU)


Introduction and objectives The optimal timing of coronary angiography in patients admitted with non–ST-segment elevation acute coronary syndrome (NSTEACS) as well as the need for pretreatment are controversial. The main objective of the IMPACT-TIMING-GO registry was to assess the proportion of patients undergoing an early invasive strategy (0-24hours) without dual antiplatelet therapy (no pretreatment strategy) in Spain. Methods This observational, prospective, and multicenter study included consecutive patients with NSTEACS who underwent coronary angiography that identified a culprit lesion. Results Between April and May 2022, we included 1021 patients diagnosed with NSTEACS, with a mean age of 67±12 years (23.6% women). A total of 87% of the patients were deemed at high risk (elevated troponin; electrocardiogram changes; GRACE score>140) but only 37.8% underwent an early invasive strategy, and 30.3% did not receive pretreatment. Overall, 13.6% of the patients underwent an early invasive strategy without pretreatment, while the most frequent strategy was a deferred angiography under antiplatelet pretreatment (46%). During admission, 9 patients (0.9%) died, while major bleeding occurred in 34 (3.3%). Conclusions In Spain, only 13.6% of patients with NSTEACS undergoing coronary angiography received an early invasive strategy without pretreatment. The incidence of cardiovascular and severe bleeding events during admission was low. (AU)

RESUMO

El hiperaldosteronismo primario (HAP) es una causa importante de hipertensión arterial (HTA) secundaria. El estudio del mismo precisa de una alta sospecha clínica, además de un estudio hormonal que confirme la hipersecreción hormonal. Es importante iniciar el tratamiento adecuado una vez se confirma el diagnóstico, y para ello es preciso demostrar si la hipersecreción hormonal es unilateral (pacientes que podrían ser tributarios a tratamiento quirúrgico) o bilateral (pacientes que son tributarios a tratamiento únicamente farmacológico). En el Hospital del Mar desde el año 2016 existe un equipo de trabajo multidisciplinar en el que participan nefrólogos, endocrinólogos, radiólogos y cirujanos para evaluar los casos con sospecha de hiperaldosteronismo y consensuar el mejor abordaje diagnóstico-terapéutico de estos pacientes, incluyendo la necesidad de cateterismo venoso adrenal, que es una técnica que en los últimos años se ha erigido como gold standard para el estudio del HAP. En el presente estudio recogemos la experiencia de nuestro centro en la realización de cateterismo venoso adrenal y en la utilidad de este para el manejo de tales pacientes (AU)


Primary hyperaldosteronism (PAH) is an important cause of secondary hypertension (HTN). The study of the same requires a high clinical suspicion in addition to a hormonal study that confirms hormonal hypersecretion. It is important to start the appropriate treatment once the diagnosis is confirmed, and for this is necessary to demonstrate whether the hormonal hypersecretion is unilateral (patients who could be candidates for surgical treatment) or bilateral (patients who are candidates for pharmacological treatment only). At the Hospital del Mar since 2016 there has been a multidisciplinary work team in which Nephrologists, Endocrinologists, Radiologists and Surgeons participate to evaluate cases with suspected hyperaldosteronism and agree on the best diagnostic-therapeutic approach for these patients, including the need for adrenal vein sampling, which is a technique that in recent years has become the gold standard for the study of PAH. In the present study we collect the experience of our center in performing adrenal vein catheterization and its usefulness for the management of these patients (AU)

Introduction and objectives Catheter-directed therapy (CDT) for acute pulmonary embolism (PE) is an emerging therapy that combines heterogeneous techniques. The aim of the study was to provide a nationwide contemporary snapshot of clinical practice and CDT-related outcomes. Methods This Investigator-initiated multicenter registry aimed to include consecutive patients with intermediate-high risk (IHR) or high-risk (HR), acute PE eligible for CDT. The primary outcome of the study was in-hospital all-cause death. Results A total of 253 patients were included, of whom 93 (36.8%) had HR-PE, and 160 (63.2%) had IHR-PE with a mean age of 62.3±15.1 years. Local thrombolysis was performed in 70.8% and aspiration thrombectomy in 51.8%, with 23.3% of patients receiving both. However, aspiration thrombectomy was favored in the HR-PE cohort (80.6% vs 35%; P<.001). Only 51 patients (20.2%) underwent CDT with specific PE devices. The success rate for CDT was 90.9% (98.1% of IHR-PE patients vs 78.5% of HR-PE patients, P<.001). In-hospital mortality was 15.5%, and was highly concentrated in the HR-PE patients (37.6%) and significantly lower in IHR-PE patients (2.5%), P<.001. Long-term (24-month) mortality was 40.2% in HR-PE patients vs 8.2% in IHR-PE patients (P<.001). Conclusions Despite the high success rate for CDT, in-hospital mortality in HR-PE is still high (37.6%) compared with very low IHR-PE mortality (2.5%) (AU)


Introducción y objetivos El emergente tratamiento por catéter (TPC) de la embolia pulmonar (EP) aguda combina técnicas heterogéneas. El objetivo del estudio es describir la práctica clínica contemporánea y los resultados relacionados con la TPC en un registro de ámbito nacional. Métodos Registro multicéntrico iniciado por investigador, destinado a incluir a pacientes con EP aguda consecutivos y en riesgo intermedio-alto (IAR) o alto riesgo (AR) elegibles para TPC. El resultado primario del estudio fue la muerte por cualquier causa en el hospital. Resultados Se incluyó en total a 253 pacientes, 93 (36,8%) con EP-AR y 160 (63,2%) con EP-IAR, de una media de edad de 62,3±15,1. Se realizó trombolisis local al 70,8% de los pacientes, trombectomía aspirativa al 51,8% y ambas al 23,3%. Sin embargo, la trombectomía por aspiración fue más frecuente en la cohorte de EP-AR (el 80,6 frente al 35%; p<0,001). Solo 51 pacientes (20,2%) se sometieron a TPC con dispositivos específicos de EP. El éxito de la TPC fue del 90,9% (el 98,1% de los casos de EP-IAR frente al 78,5% de los de EP-AR; p<0,001). La mortalidad hospitalaria fue del 15,5%, muy concentrada en los casos de EP-AR (37,6%) y significativamente menor entre los de EP-IAR (2,5%; p<0,001). La mortalidad a largo plazo (24 meses) fue del 40,2 en EP-AR frente al 8,2% en EP-IAR (p<0,001). Conclusiones A pesar del elevado éxito de la TPC, la mortalidad hospitalaria por EP-AR sigue siendo alta (37,6%) respeto a la muy baja mortalidad de la EP-IAR (2,5%) (AU)

El dolor tras la realización de incisiones por toracotomía, especialmente cuando implican extirpaciones de costillas múltiples, puede ser devastador. Reportamos aquí un manejo del dolor perioperatorio mediante bloqueo en el plano del músculo erector de la columna con inserción de catéter en el caso de un paciente de 16 años con sarcoma de Ewing, que requirió cirugía torácica mayor para resección del tumor con extirpación de la séptima, octava, novena y 10.ª costillas, y de parte del diafragma con reconstrucción mediante malla. El bloqueo en el plano del músculo erector de la columna ecoguiado es un bloqueo simple que puede aportar un control estático y dinámico del dolor, excelente en la cirugía torácica mayor en adolescentes.(AU)


Pain following thoracotomy incisions, particularly involving multiple rib resections, can be devastating. We report our experience using erector spinae plane block with catheter placement for perioperative pain management in a 16 year old patient with Ewing sarcoma. The patient required major thoracic surgery involving resection of the 7th, 8th, 9th and 10th ribs and part of the diaphragm to remove the tumour, followed by mesh reconstruction. Ultrasound guided erector spinae plane block is a simple technique that can provide excellent static and dynamic pain control following major thoracic surgery in adolescents.(AU)

Biliary metal stent implantation is an effective treatment for malignant obstructive jaundice. But it's well known that stents put in for a long time can become occluded and cause jaundice and cholangitis. At this time, endoscopic intervention is usually required to replace the stent or re-insert the stent. Re-cannulation for metal stent occlusion is challenging because the guide wire may pass through the side holes of the uncovered metal stents, resulting in prolonged surgical time and exposure to radiation. Here we present a small tip that may help endoscopists complete the re-cannulation of an uncovered metal stent in a very short time.(AU)

Introducción: En las unidades neonatales se utiliza el catéter venoso umbilical como acceso vascular de elección en la atención inicial del recién nacido debido a su rápido y fácil acceso. Presenta una tasa de fracaso del 25% al 50%, ya que hasta en un 40% de los casos puede encontrarse en el sistema portal conllevando complicaciones. Esto justifica la búsqueda de métodos para disminuir la tasa de catéteres mal posicionados. Se describen diferentes técnicas para mejorar la tasa de éxito a la hora de canalizar la vena umbilical, como son la técnica del doble catéter, la posición del recién nacido para canalizar en decúbito lateral derecho, la compresión hepática y la canalización de forma ecoguiada. El objetivo principal de este estudio es evaluar el impacto de las nuevas técnicas en la tasa de éxito de canalización venosa umbilical central. Material y métodos: Estudio cuasiexperimental pre- y postintervención en una UCIN nivel B durante enero-junio de 2022 (preintervención) y julio-diciembre de 2022 (postintervención). Resultados: Previo a la introducción de estas nuevas técnicas de canalización nuestra tasa de fracaso en la canalización, siempre llevada a cabo a ciegas, era de un 52% y con la introducción de estas medidas ha disminuido la tasa de fracaso a un 27% de forma global. Conclusiones: Tras la introducción de los nuevos métodos de canalización y recanalización, nuestra tasa de éxito en la canalización de la vena umbilical ha aumentado, y creemos necesario implantarlo en unidades con tasas de fracaso similares a la nuestra. Introduction: In neonatal units, umbilical vessel catheterization is the preferred method to gain vascular access in the initial management of the newborn because it is quick and easy. The failure rate ranges from 25% to 50%, as the catheter can be found in the portal system in up to 40% of cases, leading to complications. This failure rate warrants the investigation of different methods to reduce the frequency of catheter malposition. We describe different techniques to improve the success rate in umbilical vein catheterization, such as the double catheter technique, positioning the newborn in right lateral decubitus for insertion, liver compression, and ultrasound-guided catheter insertion. The primary objective of the study was to assess the impact of new techniques on the success rate of central umbilical venous catheterization. Material and methods: Pre- and post-intervention quasi-experimental study in a level B NICU conducted in January to June 2022 (pre-intervention) and July to December 2022 (post-intervention). Results: Prior to the introduction of these new catheterization techniques, the failure rate of blind umbilical catheter insertion was 52%. Since the introduction of these measures, the overall failure rate has decreased to 27%. Conclusions: After the introduction of the new catheterization and recanalization methods, our success rate in umbilical vein catheterization has increased, and we believe it is necessary to implement them in units with similar failure rates to ours. Introduction Pulmonary embolism (PE) response teams (PERT) for the management of high-risk PE (HR-PE) and intermediate-high risk PE (IHR-PE) are encouraged in PE guidelines. We aimed to assess the impact of a PERT initiative on mortality in these groups of patients, compared with standard care. Methods We conducted a prospective, single-center registry, including consecutive patients with HR-PE and IHR-PE with PERT activation from February-2018 to December-2020 (PERT group, n=78 patients) and compared it with an historic cohort of patients admitted to our hospital in a previous 2-year period (2014–2016), managed with standard of care (SC-group, n=108 patients). Results Patients in the PERT group were younger and less comorbid. The risk profile at admission and the percentage of HR-PE was similar in both cohorts (13% in SC-group and 14% in PERT-group, p=0.82). Reperfusion therapy was more frequently indicated in PERT-group (24.4% vs 10.2%, p=0.01), with no differences in fibrinolysis treatment, while catheter-directed therapy (CDT) was more frequent in PERT group (16.7% vs 1.9%, p<0.001). Reperfusion and CDT were associated with lower in-hospital mortality (2.9% vs 15.1%, p=0.001 for reperfusion and 1.5% vs 16.5%, p=0.001 for CDT). The primary outcome, 12-month mortality, was lower in the PERT-group (9% vs 22.2%, p=0.02), There were no differences in 30-day readmissions. In multivariate analysis PERT activation was associated with lower mortality at 12 months (HR 0.25, 95% confidence interval 0.09–0.7, p=0.008). Conclusion A PERT initiative in patients with HR-PE and IHR-PE was associated with a significant reduction in 12-month mortality compared with standard of care, and also with an increase in the use of reperfusion, especially catheter-directed therapies (AU)


Introducción Las guías de manejo de embolia pulmonar (EP) recomiendan organizar equipos de respuesta a la embolia pulmonar (PERT) para el manejo de la EP de riesgo intermedio-alto (EP-IAR) y de alto riesgo (EP-AR). Nuestro objetivo fue evaluar el impacto de una iniciativa PERT sobre la mortalidad en estos pacientes, en comparación con la atención estándar. Métodos Realizamos un registro prospectivo unicéntrico, incluyendo pacientes consecutivos con EP-IAR y EP-AR con activación del PERT desde febrero de 2018 hasta diciembre de 2020 (grupo PERT, n=78 pacientes) y lo comparamos con una cohorte histórica de pacientes ingresados en nuestro hospital en un período previo de 2 años (2014-2016), manejados con atención estándar (grupo SC, n=108 pacientes). Resultados Los pacientes del grupo PERT eran más jóvenes y con menos comorbilidades. El perfil de riesgo al ingreso y el porcentaje de EP-AR fue similar en ambas cohortes (13% en el grupo SC y 14% en el grupo PERT, p=0,82). La terapia de reperfusión fue más frecuentemente indicada en el grupo PERT (24,4% vs. 10,2%, p=0,01), sin diferencias en el uso de fibrinólisis, mientras que la terapia dirigida por catéter (CDT) fue más frecuente en el grupo PERT (16,7% vs. 1,9%, p<0,001). La reperfusión y la CDT se asociaron con una menor mortalidad hospitalaria (2,9% vs. 15,1%, p=0,001 para reperfusión y 1,5% vs. 16,5%, p=0,001 para CDT). El objetivo primario, la mortalidad a los 12 meses, fue menor en el grupo PERT (9% frente al 22,2%, p=0,02). No hubo diferencias en los reingresos a los 30 días. En el análisis multivariado la activación de PERT se asoció con una menor mortalidad a los 12 meses (hazard ratio 0,25, intervalo de confianza del 95%: 0,09-0,7, p=0,008)(AU)

Los CVC (Catéteres Venosos Centrales) son de uso frecuente en las unidades de cuidados intensivos y reanimación para el manejo del paciente en una situación crítica. Estos catéteres aportan información valiosa para el diagnóstico, valoración del paciente y el tratamiento del mismo en situaciones graves de inestabilidad hemodinámica, principalmente. Es de vital importancia, el consenso hospitalario del uso de estos catéteres, ya que proporcionan una herramienta muy útil y con cada vez menos riesgos potenciales asociados a su uso.Los CVC tienen como objetivo, las mediciones hemodinámicas, aportes parenterales al paciente, cubrir necesidades de urgencia, extracciones sanguíneas, administración de fármacos vasoactivos, etc. Por sus características, son los más aptos para los pacientes de UCIs y REAs.Existen numerosos estudios que indican que en un catéter venoso central de tres/cinco luces, las extracciones de muestras para analítica, se deben llevar a cabo extrayendo una mínima cantidad de sangre de la luz proximal, sin lavado previo ni posterior1, con el fin de evitar la manipulación excesiva del catéter, y las anemias iatrogénicas debidas a la cantidad de sangre que se desecha. Sin embargo, no existe consenso, ni estudios de relevancia e investigación que indiquen cuál es la distribución adecuada de las perfusiones en un catéter venoso central de varias luces, cuando se administran drogas vasoactivas, tipo noradrenalina, dopamina, dobutamina, etc., ni tampoco cual es la repercusión hemodinámica si se realiza la extracción por otra rama del catéter venoso central. Los estándares de práctica de la terapia de infusión proporcionan recomendaciones basadas en la evidencia que publica la Infusión Nurses Society2 cada 5 años. Sí hay, por el contrario, estudios que demuestran cuál es la forma más adecuada del cambio de perfusiones cuando se terminan, con el fin de evitar las alteraciones hemodinámicas del paciente. (AU)


CVCs (Central Venous Catheters) are frequently used in intensive care and resuscitation units for the management of patients in a critical situation. These catheters provide valuable information for the diagnosis, assessment of the patient and the treatment of the same in serious situations of hemodynamic instability, mainly. It is of vital importance, the hospital consensus of the use of these catheters, since they provide a very useful tool and with fewer and fewer potential risks associated with their use.The CVCs have as their objective, hemodynamic measurements, parenteral contributions to the patient, cover emergency needs, blood extractions, administration of vasoactive drugs, etc. Due to their characteristics, they are the most suitable for patients with ICUs and OERs.There are numerous studies that indicate that in a three/five-light central venous catheter, the extractions of samples for analysis should be carried out by extracting a minimum amount of blood from the proximal lumen, without prior or subsequent washing1, in order to avoid excessive manipulation of the catheter, and iatrogenic anemias due to the amount of blood that is discarded. However, there is no consensus, nor studies of relevance and research that indicate what is the appropriate distribution of infusions in a central venous catheter of several lights, when vasoactive drugs, noradrenaline type, dopamine, dobutamine, etc. are administered, nor what is the hemodynamic repercussion if the extraction is performed by another branch of the central venous catheter. The infusion therapy standards of practice provide evidence-based recommendations published by the Nurses Society2 Infusion every 5 years. On the contrary, there are studies that demonstrate the most appropriate form of change in infusions when they are finished, in order to avoid hemodynamic alterations in the patient. (AU)

Objetivo: El objetivo fundamental es evaluar la facilidad de uso y comodidad de Actreen® Mini Set para el cateterismo vesical intermitente femenino, siguiendo el número de cateterismos prescritos por el especialista. Adicionalmente, se analiza si la utilización de un set compacto puede facilitar el seguimiento del tratamiento y si este tipo de dispositivo es un producto considerado seguro, de fácil manejo y adecuado, en relación a la edad o el nivel de destreza de las usuarias. Método: Evaluación sobre el manejo del set compacto, teniendo en cuenta la experiencia de uso de mujeres que deben realizar el sondaje vesical intermitente (SVI) como parte del tratamiento en la disfunción vesical. Para ello, se ha contado con la colaboración de profesionales de enfermería urológica de diferentes centros hospitalarios españoles. Se utilizó un cuestionario de elaboración propia. Resultados: El análisis de los resultados muestra un elevado nivel de satisfacción de las usuarias, tanto con las características del producto como con la facilidad de uso para seguir el tratamiento, así como la sensación de que es un producto seguro y adecuado. Objective: The main objective is to evaluate the ease of use and comfort of the Actreen® Mini Set for female intermittent bladder catheterization, following the number of catheterizations prescribed by the specialist. Additionally, it is analyzed whether the use of a compact set can facilitate the follow-up of the treatment and whether this type of device is considered a safe, easy to use and appropriate product, in relation to the age or skill level of the users. Method: Method: Evaluation of the management of the compact set, taking intoaccount the experience of use by women who have to perform intermittentbladder catheterization as part of the treatment of bladder dysfunction. Forthis purpose, urological nursing professionals from different Spanish hospitals collaborated in the study. An own designed questionnaire was used. Results: Analysis of the results shows a high level of satisfaction among users, both with the characteristics of the product and the ease of use in following the treatment, as well as the feeling that it is a safe and appropriate product. Introducción: actualmente los aneurismas de la arteria ilíaca interna se tratan en su mayoría por vía endovascular. En pacientes con intervenciones previas puede ser especialmente difícil tratarlos dada la ausencia de accesos arteriales “clásicos”. Caso clínico: varón de 80 años con crecimiento del saco aneurismático de la arteria ilíaca interna derecha en el que era imposible un acceso percutáneo por vía anterógrada ilíaca y por vía retrógrada femoral por intervenciones previas al que se le realizó un tratamiento mediante punción percutánea de la arteria glútea superior mediante embolización transcatéter con coils. Discusión: en ausencia de accesos clásicos para tratamiento endovascular de un aneurisma de ilíaca interna, la punción transglútea es una técnica sencilla y reproducible que ofrece una alternativa en estos escenarios.(AU)


Introduction: currently, internal iliac artery aneurysms are generally treated with endovascular techniques, in patients with previous interventions it can be particularly challenging to treat these lesions due to lack of a patent “classical” access site. Case report: we report a case of an 80-year-old man with a growing right internal iliac aneurysm, in which a percutaneous antegrade access through the iliac artery or a retrograde access to the femoral artery were impossible due to previous interventions, that was managed with trans catheter coil embolization with a percutaneous puncture to the superior gluteal artery. Discussion: when classical endovascular approaches to the intern iliac artery are absent, the transgluteal puncture is a simple and replicable technique, offering an alternative in challenging scenarios.(AU)

Background: the COVID-19 pandemic has impacted on several aspects of health care services worldwide. The aim of the study was to determine its influence on the case volume, success rate and complication rate of endoscopic retrograde cholangiopancreatography (ERCP).Method: all patients who underwent ERCP one-year before and after applying COVID-19 safety measures at the Qena University Hospital were included. Data were collected from the patients' records, analyzed and compared.Results: a total of 250 patients underwent ERCP between April 1st, 2019 and March 31st, 2021, and the mean age of participants was 52 ± 18 years. There was a 5 % increase in case volume after applying COVID-19 safety measures (128 vs 122) and the total procedure time was significantly shorter (42 vs 46 minutes, p = 0.04). There was no significant difference in the overall success rate and complication rate. Procedure success significantly correlated with cannulation attempts and total procedure time in both groups, and serum bilirubin and cannulation time in the pre-COVID-19 patients and alkaline phosphatase (ALP) in post-COVID patients. ERCP-related complications significantly correlated with cannulation attempts in both groups, and ALP, international normalized ratio (INR), cannulation time and total procedure time in pre-COVID-19 patients, and platelet count and amylase in post-COVID patients. Two patients were confirmed COVID-19 cases at the time of ERCP; therapeutic targets were achieved in both with a smooth post-ERCP recovery. Three out of nine ERCP team members caught a mild to moderate COVID-19 infection and recovered after receiving proper management.Conclusion: our result show that there was no negative impact of using COVID-19 safety measures and precautions on the case-volume, indications, overall outcome or complication rate of ERCP. Introducción: el tratamiento de aneurismas complejos mediante FEVAR incluye entre sus objetivos un tiempoquirúrgico reducido para poder alcanzar el éxito técnico y clínico. Sin embargo, la canulación y el implante de losstents puente en múltiples arterias viscerales pueden suponer un factor limitante. Para evitar un tiempo de escopia y una dosis de radiación prolongados existen algunas maniobras que pueden ayudar a optimizar el tiempo de cateterización. Material y métodos: se realiza una revisión de los últimos casos tratados mediante endoprótesis fenestradas custom made de Zenith Cook® durante el año 2021 en un servicio de angiología, cirugía vascular y endovascular. El objetivo es mostrar las técnicas que sirven para optimizar el tratamiento de aneurismas complejos y que el cirujano puede emplear con el material habitual. Para ello se muestran varios fragmentos de vídeos de estos procedimientos grabados con el sistema OneView. Resultados: el primer paso clave consiste en la liberación del dispositivo fenestrado. La endoprótesis de Cook® presenta una o varias ligaduras de reducción que la mantienen fruncida hasta garantizar una correcta orientación y un correcto posicionamiento. Asimismo, ofrece la opción de canular las arterias viscerales entre la pared arterial y el dispositivo. Con una planifi cación adecuada y un abordaje sistematizado, el uso de guías coaxiales, catéteres de punta simple o reversa e introductores es esencial. El techo de la endoprótesis permite el avance de guías y de introductores con el soporte sufi ciente para su canulación. Los sistemas precargados permiten la canulación desde el miembro superior o el inferior. En este último caso, se utiliza una guía buddy de 0,014" que ofrece soporte al introductor, ya que lo acerca más aún a la fenestración, de tal modo que potencia el momento de torsión (torque) y el empuje del catéter.


Introduction: the treatment of complex aneurysms using FEVAR includes among its objectives a reduced surgicaltime in order to achieve technical and clinical success. However, cannulation and implantation of bridging stentsin multiple visceral arteries can be a limiting factor. To avoid a protracted scope time and radiation dose, there aresome maneuvers that can help optimize catheterization time. Material and methods: a review of the last cases treated with custom made Zenith Cook® fenestrated endoprostheses during the year 2021 is performed in an angiology, vascular and endovascular surgery service. The objective is to show video clips recorded with the OneView system of these techniques that the surgeon can use with the usual material to optimize the treatment of complex aneurysms.Results: the first key step is the release of the fenestrated device. The Cook® endoprosthesis has one or morereduction ligatures that keep it puckered until it guarantees correct orientation and positioning, as well as theoption of cannulating the visceral arteries between the arterial wall and the device.With proper planning and a systematic approach, the use of coaxial guides with single or reverse tip cathetersand introducers are essential. The roof of the endoprosthesis allows the advancement of guides and introducerswith sufficient support for their cannulation. The preloaded systems allow cannulation from the upper or lowerlimb. In the latter case, a 0.014” buddy guide is used to provide support for the introducer, bringing it even closerto fenestration in such a way as to enhance the torque and thrust of the catheter.
