RESUMO

Introducción. En la reflexoterapia de acupuntura clásica se postula habitualmente que por debajo de un nivel de estimulación, determinado por la aparición de una peculiar sensación denominada De-Qi, no se obtiene efecto terapéutico apreciable. Puesto que las acciones de control de las aferencias sobre el sistema nervioso central no funcionan de este modo, sino que se ajustan, con peculiaridades, a un esquema clásico de proporcionalidad entre el estímulo y la respuesta, nos proponemos valorar en qué medida estímulos de acupuntura de intensidad creciente pueden modificar también de modo creciente parámetros neurofisiológicos, y nos centraremos en la cuantificación metamérica, mediante el estudio de la onda F, y en el telencéfalo, mediante el potencial evocado somatosensorial. Pacientes y métodos. Se estudian 21 voluntarios sanos, de quienes se recogen los valores correspondientes a los parámetros indicados en situación basal, tras punturar un punto extraacupuntural y tras punturar el punto clásico 4GI, simplemente hasta obtener la sensación De-Qi, y tras detectar esta sensación, se añade electroestimulación. Resultados. Se observa que existe una clara progresión en la modificación, tanto de los valores de F como del potencial somestésico, con estímulos crecientes. La inflexión básica se da, ciertamente, tras obtener el De-Qi, pero al sobreestimular a partir de ese momento, se producen mayores variaciones, lo que es especialmente patente en el caso de la latencia del potencial somestésico. La punción de un punto extracanalar, por otra parte, garantiza que la mera punción de la piel no ejerce ninguno de los efectos mencionados. Discusión. A pesar de los postulados clásicos, se observa que, a mayor intensidad del estímulo de acupuntura, mayores son las modificaciones de los parámetros neurofisiológicos estudiados. El control de la aferencia somestésica por acupuntura es algo que ya se demostró y cuantificó con anterioridad; ahora se observa, además, la relación directa entre la intensidad de tal efecto y la del estímulo empleado para producirlo, lo que se aleja claramente de las concepciones energéticas clásicas. Conclusiones. El efecto de la estimulación de acupuntura se relaciona estrechamente con la intensidad del estímulo aplicado; la sensación De-Qi no es necesaria para obtener modificaciones neurofuncionales, y las estimulaciones que sobrepasan a ésta causan mayores efectos (AU)


Introduction. In the reflexotherapy of classical acupuncture it is usually claimed that below a certain level of stimulation, which is determined by the appearance of a peculiar sensation known as De-Qi, no appreciable therapeutic effect is obtained. The control exerted by the afferences over the central nervous system, however, do not work in this way, but instead adjust themselves, albeit with certain peculiarities, to a classical pattern of proportionality between stimulus and response. Our aim is, therefore, to evaluate the extent to which increasingly intense acupuncture stimuli can modify neurophysiological parameters in a way that also grows. We will focus on the quantification on a metametic level, through the study of the F-wave, and in the telencephalon, through somatosensory evoked potentials. Patients and methods. A total number of 21 healthy volunteers were studied and we collected the values corresponding to the parameters indicated in the basal situation, when needling an extra-acupunctural point and following needling of the classical point IG4 until the sensation of De-Qi was felt, and electrostimulation was added after detecting the abovementioned sensation. Results. We observed a clear progression in the modifications of both the F values and the somesthetic potential, with growing stimuli. Basic inflection does indeed occur after obtaining De-Qi, but on overstimulating from then onwards greater variations are produced. This is especially obvious in the case of the latency of the somesthetic potential. Needling a point that is not on a meridian guarantees that merely piercing the skin does not exert any of the effects mentioned above. Discussion. In spite of the classical postulates, it can be observed that the more intense the acupuncture stimulus is, the greater the modifications to the neurophysiological parameters being studied are. The control of the somesthetic afference by acupuncture is something that has already been demonstrated and quantified. What is now seen, in addition, is the direct relation between the intensity of that effect and that of the stimulus used to produce it, which is quite different from the classical conceptions of energy. Conclusions. The effect of acupuncture stimulation is closely related to the intensity of the stimulus applied; the sensation of De-Qi is not necessary to obtain neurofunctional modifications and stimulations that go beyond it cause greater effects (AU)

Introducción. Actualmente, en el tratamiento del deterioro cognitivo asociado a la enfermedad de Alzheimer (EA) se utilizan los inhibidores de la acetilcolinesterasa (iACh). Los efectos secundariosde estos fármacos se asocian al aumento de acetilcolina, lo que limita su efectividad, y se debe ajustar al paciente hasta la dosis máxima tolerada . Pacientes y métodos. Se realiza un estudio comparativoretrospectivo de la tolerancia y de los acontecimientos adversos (AA) de dos iACh en un grupo de pacientes con EA probable de gravedad mínima y leve durante un período de 6 meses. Resultados. La muestra la formaban 175 pacientes, de los cuales 134 iniciaron tratamiento con donepecilo en 5-10 mg/día y 41 con rivastigmina en 6-12 mg/día. El 20 por ciento de los pacientes presentaron AA y el 8 por ciento abandonó el tratamiento. Los trastornos gastrointestinales (TGI) fueron el principal AA observado (57,1 por ciento). El 6 por ciento de los pacientes tratados con donepecilo abandonaron el tratamiento a causa de los AA frente al 14,6 por ciento de los pacientes tratados con rivastigmina. Los pacientes tratados con rivastigmina presentaron una mayor incidencia de TGI y el riesgo relativo de presentar TGI fue 4,4 veces superior que en los pacientestratados con donepecilo. Conclusiones. Los TGI asociados al tratamiento con iACh son el principal motivo de abandono del tratamiento y se producen con mayor frecuencia en pacientes tratados con rivastigmina (AU)


Introduction. At present acetylcholinesterase inhibitors (AChEI) are used in the treatment of the cognitive deterioration associated with Alzheimer’s disease (AD). The side effects of these drugs are linked with the increase in acetylcholine, which limits their effectiveness, and must be adjusted to the patient close to the maximum tolerated dose. Patients and methods. We conducted a comparative retrospective study of the tolerance and the adverse events (AE) of two AChEI in a group of patients with very slight and mild probable AD over a 6-month period. Results. The sample was made up of 175 patients, of which 134 began therapy with 5-10 mg/day of donepezil and 41 with 6-12 mg/day of rivastigmine. 20% of the patients presented AE and 8% abandoned the treatment. Gastrointestinal disorders (GID) were the main AE observed (57.1%). Only 6% of the patients treated with donepezil abandoned the therapy because of the AE as opposed to 14.6% of the patients treated with rivastigmine. Patients treated with rivastigmine displayed a higher incidence of GID and the relative risk of presenting GID was 4.4 times higher than in the patients treated with donepezil. Conclusions. The GID associated to therapy with AChEI are the main reason for abandoning treatment and occur more frequently in patients treated with rivastigmine (AU)
