Introducción: La dermatosis perforante adquirida (DPA) es un trastorno frecuente en pacientes en hemodiálisis, y el efecto en la calidad de vida está poco descrito. Investigamos la prevalencia de DPA en pacientes en hemodiálisis, medimos y comparamos la calidad de vida asociada a DPA. Métodos: Desarrollamos un estudio prospectivo, observacional y descriptivo. Invitamos a pacientes mayores de 18 años en hemodiálisis. Se obtuvieron datos de su expediente electrónico, y se realizó exploración dermatológica. Se aplicó el Índice de Calidad de Vida en Dermatología (DLQI). Se hizo un análisis descriptivo de las variables demográficas, de las características clínicas y de los hallazgos de dermatoscopia, así como la comparación de los puntajes del DLQI. Resultados: La muestra fue de 46 pacientes, con una prevalencia de DPA del 11%. Los pacientes con DPA eran más delgados y jóvenes en comparación con los pacientes sin DPA. El tiempo en hemodiálisis fue mayor en los pacientes con DPA en comparación a los pacientes sin DPA, con una mediana de 90 versus 32 meses (p=0,015). La afección en calidad de vida fue mayor en los pacientes con DPA en comparación a los pacientes sin DPA, con un algún efecto en todos los pacientes con DPA y un 33% en los pacientes sin DPA (p=0,001). Los pacientes con DPA tuvieron con más frecuencia prurito en comparación con los pacientes sin DPA (p=0,007). Conclusiones: La edad, el tiempo en hemodiálisis y el índice de masa corporal se asocian con la presencia de DPA. Los pacientes con DPA tuvieron una prevalencia más alta de prurito y mayor afección en la calidad de vida en dermatología en comparación con los pacientes sin DPA. (AU)


Introduction: Acquired perforating dermatosis (APD) is a frequent disorder in hemodialysis patients and the effect on the quality of life is poorly described. We investigated the prevalence of APD in hemodialysis patients, measured and compared APD-associated quality of life. Methods: We developed a prospective, observational, and descriptive study. We invited patients over the age of 18 in hemodialysis. Data was obtained from their electronic file and a dermatological examination was performed. The Dermatology Life Quality Index (DLQI) was applied. Descriptive analysis of demographic variables, clinical features, and dermoscopy findings, as well as comparison of DLQI scores, was made. Results: The sample consisted of 46 patients, with a prevalence of APD of 11%. Patients with APD were leaner and younger compared to patients without APD. The time on hemodialysis was longer in patients with APD compared to those without APD, with a median of 90 versus 32 months (P=.015). The impact on quality of life was greater in patients with APD compared to those without APD, with some effect in all patients with APD and 33% in patients without APD (P=.001). Patients with APD had more frequent pruritus compared to those without APD (P=.007). Conclusions: Age, time on hemodialysis and BMI are associated with the presence of APD. Patients with APD had a higher prevalence of pruritus and a greater impact on quality of life in dermatology compared to patients without APD. (AU)

Background/Aim The prevalence of cognitive impairment (CI) is high in hemodialysis patients. In this study, the relationship between CI and serum carnitine, plasma omega-3, omega-6 and omega-3/omega-6 fatty acid ratio was evaluated in hemodialysis patients. Materials and methods Sixty two patients [male: 40 (64.5%), mean age 51±13 years] were included in this cross-sectional study. Serum total and free-carnitine levels were determined by ELISA. Plasma omega-3 [eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA)] and omega-6 [arachidonic acid (AA), dihomo gamma linoleic acid (DGLA)] levels were measured using LC-ESI-MS/MS. According to the Montreal Cognitive Assessment (MoCA) scores, ≤24 points were considered as CI. MoCA score ≤24 and >24 were determined as Group 1 and Group 2, respectively. Results Group 1 had significantly higher AA+DGLA/EPA+DHA ratios and lower free-carnitine, DHA and EPA+DHA levels compared to Group 2 (P=0.008, P=0.040, P=0.032, P=0.032, respectively). Group 1 had a statistically lower education level (P<0.05). Negative correlation was found between MoCA scores and AA+DGLA/EPA+DHA ratios (rs=−0.284, P=0.026). Free-carnitine levels were positively correlated with EPA and EPA+DHA levels (rs=0.278, P=0.030 and rs=0.271, P=0.034, respectively), and negative correlated with AA+DGLA/EPA+DHA ratios (rs=−0.414, P=0.001). In multivariate logistic regression analysis, MoCA scores was associated with AA+DGLA/EPA+DHA ratio (P=0.009) and education level (P<0.001). Conclusion It was determined that high AA+DGLA/EPA+DHA ratio and low education level could be independent risk factors of the CI (AU)


Contexto/objetivo La prevalencia de deterioro cognitivo (CI) es alta en pacientes en hemodiálisis. En este estudio se evaluó la relación entre el CI y las proporciones de carnitina sérica, ácidos grasos omega-3, omega-6 y omega-3/omega-6 en plasma en pacientes en hemodiálisis. Materiales y métodos En este estudio transversal se incluyeron 62 pacientes (hombres: 40 [64,5%], edad media 51±13años). Los niveles séricos de carnitina total y libre se determinaron mediante ELISA. Los niveles plasmáticos de omega-3 (ácido eicosapentaenoico [EPA], ácido docosahexaenoico [DHA]) y omega-6 (ácido araquidónico [AA], ácido dihomo gamma linoleico [DGLA]) se midieron utilizando LC-ESI-MS/MS. Según las puntuaciones de la Evaluación Cognitiva de Montreal (MoCA), ≤24 indican CI. Las puntuaciones MoCA ≤24 y >24 se determinaron como grupo 1 y grupo 2, respectivamente. Resultados El grupo 1 tenía proporciones de AA + DGLA/EPA + DHA significativamente más altas y niveles más bajos de carnitina libre, DHA y EPA +DHA en comparación con el grupo2 (p=0,008, p=0,040, p=0,032 y p=0,032, respectivamente). El grupo1 tenía un nivel educativo estadísticamente más bajo (p<0,05). Se descubrió una correlación negativa entre las puntuaciones de MoCA y las proporciones AA + DGLA/EPA + DHA (rs=−0,284, p=0,026). Los niveles de carnitina libre se correlacionaron positivamente con los niveles de EPA y EPA + DHA (rs=0,278, p=0,030, y rs=0,271, p=0,034, respectivamente), y negativamente con las proporciones AA +DGLA/EPA +DHA (rs=−0,414, p=0,001). Objetivo: Los pacientes con enfermedad renal crónica (ERC) en hemodiálisis (HD) suelen presentar déficits cognitivos. Sin embargo, existen pocos estudios que hayan examinado el funcionamiento neuropsicológico de aquellos que reciben diálisis peritoneal (DP). Método: Se evaluaron las funciones ejecutivas, la velocidad de procesamiento y la memoria verbal en 27 pacientes en DP, 42 en HD y 42 participantes sanos (PS). La presión sanguínea sistólica y el tiempo total en terapia renal sustitutiva (TRS) se controlaron estadísticamente. Las asociaciones entre el rendimiento y los factores clínicos se analizaron mediante correlaciones y regresión múltiple. Resultados: El grupo DP presentó mejor ejecución respecto al HD en fluidez verbal, memoria de trabajo, flexibilidad cognitiva, planificación y toma de decisiones. El grupo DP mostró peor ejecución que el grupo PS en inhibición y memoria verbal. Las puntuaciones en las funciones ejecutivas se asociaron positivamente con los meses totales en DP, en TRS, en HD, la albúmina, el colesterol total y el fósforo, y de forma negativa con la ferritina. Conclusión: El funcionamiento ejecutivo global fue mejor en los pacientes en DP que en aquellos en HD. Los resultados muestran el efecto positivo de la DP sobre las funciones ejecutivas, lo que debe tenerse en cuenta a la hora de la elección de la TRS. Las asociaciones observadas entre los factores bioquímicos y el rendimiento muestran la importancia de mantener un adecuado estado nutricional en estos pacientes.(AU)


Background: Patients with chronic kidney disease on hemodialysis (HD) often have cognitive deficits. However, there are few studies that have examined the neuropsychological impairments of patients receiving peritoneal dialysis (PD). Methods: Executive functions, processing speed and verbal memory were assessed in 27 PD patients, 42 HD patients, and 42 healthy participants (HP). Systolic blood pressure and total time on renal replacement therapy (RRT) were controlled statistically. Associations between performance and clinical factors were analyzed using correlations and multiple regression. Results: The DP group showed better performance compared to the HD group in verbal fluency, working memory, cognitive flexibility, planning and decision making. The DP group showed worse execution than the HP group in verbal inhibition and memory. Executive function scores were positively associated with total months on PD, total months on RRT, total months on HD, albumin, total cholesterol, and phosphorus, and negatively with ferritin. Conclusion: Global executive functioning was more optimal in PD patients than in HD patients. The results show the positive effect of PD on executive functions, which must be taken into account when choosing the TRS. The associations observed between biochemical factors and performance show the importance of maintaining an adequate nutritional status in these patients.(AU)

Objective: dietary advice provided through a nutritional intervention program (NIP) is recommended by renal clinic guidelines to prevent or treat malnutrition, that could improve quality of life (QoL) and survival in hemodialysis (HD) patients. This study set out to evaluate the effect of a personalized NIP on the nutritional status and its impact on QoL and mortality in dialyzed patients. Material and methods: this was a 12-month intervention study with regular follow-up in which nutritional parameters were measured at baseline and after 6 and 12 months. QoL was assessed by the Kidney Disease Quality of Life version 1.2 (KDQOL-SF) at baseline and at the end of the study. All dialyzed patients received individualized consultations with a trained dietitian. The content of the nutritional education program included a personalized meal plan and educational materials addressing nutrition to manage fluids, electrolytes, and vitamin D. Results: a total of 75 patients were included. After the NIP, visceral proteins, phosphorous, potassium and vitamin D levels had improved significantly (p < 0.001). The percentage of well-nourished patients increased by 30 % (p < 0.001). At the end of the study, the well-nourished patients had significantly improved scores on the general summary areas of the KDQOL-SF, reduced worry concerning fluid and dietary restrictions (p < 0.001), and the survival rate was 12 months longer (p < 0.01). Conclusion: the results of this study suggest that personalized NIP contributed to improved nutritional status, QoL and survival in HD patients. (AU)


Objetivo: el asesoramiento dietético proporcionado a través de un programa de intervención nutricional (PIN) es recomendado por las guías clínicas renales para prevenir o tratar la desnutrición, puediendo mejorar la calidad de vida (CV) y la supervivencia en pacientes en hemodiálisis (HD). El objetivo de este estudio fue evaluar el efecto de un PNI personalizado sobre el estado nutricional y su impacto en la calidad de vida y la mortalidad en pacientes dializados. Material y métodos: estudio de intervención de 12 meses de duración, con seguimiento periódico de los pacientes en el que se midieron los parámetros nutricionales al inicio, a los 6 y 12 meses. La CV fue evaluada por el cuestionario Kidney Disease Quality of Life versión 1.2 (KDQOL-SF) al inicio y al final del estudio. Todos los pacientes dializados recibieron consultas individualizadas con un dietista. El contenido del programa de educación nutricional incluyó un plan de alimentación personalizado y materiales educativos sobre nutrición para el manejo de fluidos, electrolitos y vitamina D. Resultados: se incluyeron un total de 75 pacientes. Después del PIN, los niveles de proteínas viscerales, fósforo, potasio y vitamina D habían mejorado significativamente (p < 0,001). El porcentaje de pacientes bien nutridos aumentó un 30 % (p < 0,001). Al final del estudio, los pacientes bien nutridos mejoraron significativamente las puntuaciones en las áreas de resumen general del KDQOL-SF, redujeron la preocupación por las restricciones dietéticas y de líquidos (p < 0,001) y la tasa de supervivencia fue de 12 meses superior (p < 0,01). Conclusión: los resultados de este estudio sugieren que el PIN personalizado contribuyó a mejorar el estado nutricional, la calidad de vida y la supervivencia en pacientes en HD. (AU)

Antecedentes: Los pacientes en hemodiálisis padecen, debido a su diagnóstico y tratamiento, mayores niveles de ansiedad y depresión que la población general. El objetivo de esta investigación es conocer el impacto emocional generado por la COVID-19 en pacientes hemodializados. Método: Estudio transversal analítico en una muestra total de 88 pacientes mayores de 18 años que reciben hemodiálisis entre abril de 2021 y marzo de 2022. Se recogen variables sociodemográficas y clínicas incluyendo aquellas relacionadas con COVID-19. A todos los pacientes se les administran las escalas HADS y Kessler-10. Resultados: La puntuación media de la escala HADS-A fue de 5,3 (±3,732), de la HADS-D de 5,45 (±4,052) y de la Kessler-10 de 20,11(±7,499). Se observaron diferencias significativas entre aislados y no aislados en las escalas clínicas HADS-A (p=,002) y HADS-D (p=,005). Igualmente, aquellos pacientes con antecedentes psicopatológicos fueron significativamente más jóvenes (p=,014) y con una mayor puntuación en las escalas HADS-D (p=,016) y Kessler-10 (p=,035) que aquellos sin antecedentes. Conclusiones: No hubo asociación entre infección por COVID-19 y la clínica ansioso-depresiva en pacientes hemodializados. El aislamiento, en estos pacientes, no se asocia a la aparición de clínica psicopatológica, mientras que el hecho de haber presentado antecedentes psicopatológicos previos se asocia a su desarrollo.(AU)


Background: Hemodialysis patients suffer, due to their diagnosis and treatment, higher levels of anxiety and depression than the general population. The aim of this research is to know the emotional impact generated by COVID-19 in hemodialysis patients. Method: Analytical cross-sectional study in a total sample of 88 patients over 18 years of age receiving hemodialysis between April 2021 and March 2022. Sociodemographic and clinical variables including those related to COVID-19 are collected. All patients were administered the HADS and Kessler-10 scales. Results: The mean score of the HADS-A scale was 5.3(±3.732) of the HADS-D 5.45 (±4.052) and of the Kessler-10 20.11(±7.499). Significant differences were observed between isolated and non-isolated patients in the clinical scales HADS-A (p=.002) and HADS-D (p=.005). Likewise, those patients with a psychopathological history were significantly younger (p=.014) and scored higher on the HADS-D (p=.016) and Kessler-10 (p=.035) scales than those without a history. Conclusions: There was no association between COVID-19 infection and anxious-depressive symptoms in hemodialyzed patients. Isolation, in these patients, was not associated with the appearance of psychopathological symptoms, while the fact of having a previous history of psychopathology was associated with their development.(AU)

Introducción:Los pacientes en diálisis presentan peores resul-tados en la calidad de vida respecto a la población general. Entre otras variables, ésta peor calidad de vida se ha relacionado con la técnica de diálisis, aunque sin evidencia suficiente.Objetivo: Conocer la influencia de la técnica de diálisis en la calidad de vida relacionada con la salud y analizar las variables más influyentes en cada una de ellas.Metodología: Se realizó una revisión sistemática basada en la declaración PRISMA. Se analizaron 10 artículos extraídos de las bases de datos PubMed, Scopus y WOS. Se incluyeron artículos originales, tanto en inglés como en español, que compararan la calidad de vida relacionada con la salud entre ambas modalida-des de diálisis. Resultados: Se incluyeron 10 artículos (6 estudios observacio-nales y 4 revisiones sistemáticas. Emergieron como principales variables: la comparación de la calidad de vida relacionada con la salud entre diálisis peritoneal y hemodiálisis, y aquellas va-riables más influyentes en la percepción de la calidad de vida (edad, sexo femenino, comorbilidad, estudios, soledad, estado civil, desempleo, dolor, funcionamiento físico y apoyo social).Conclusiones: Los resultados muestran una mejor percepción de la calidad de vida de los pacientes en diálisis peritoneal en diversas esferas, en parte porque son pacientes más jóvenes, con mayor autonomía, presentan menos patologías asociadas y tienen más apoyo social. Sin embargo, no todos los estudios son coincidentes con estos resultados o no encuentran diferencias entre una técnica u otra respecto a la percepción de la calidad de vida relacionada con la salud (AU)


Introduction: Dialysis patients exhibit the poorer quality of life outcomes compared to the general population. Among several factors, such reduced quality of life has been associated with the dialysis technique, although without sufficient evidence.Objective: To understand the influence of dialysis technique on health-related quality of life and analyze the most influential variables in each modality.Methodology: A systematic review based on the PRISMA statement was conducted. Ten articles were analyzed from the PubMed, Scopus, and Web of Science databases. Original articles in both English and Spanish that compared health-related quality of life between both dialysis modalities were included.Results: Ten articles were included (6 observational studies and 4 systematic reviews). Key variables emerged, including the comparison of health-related quality of life between peritoneal dialysis and hemodialysis, and those variables most influential in the perception of quality of life (age, female gender, comorbidity, education, loneliness, marital status, unemployment, pain, physical functioning, and social support).Conclusions: The results demonstrate a better perception of the quality of life among peritoneal dialysis patients in various aspects, partly because they are younger, more autonomous, have fewer associated pathologies, and receive more social support. However, not all studies are consistent with these results or find differences between one technique and another regarding the perception of health-related quality of life (AU)

Objetivo:Analizar la percepción de los pacientes sobre la humanización de los cuidados de enfermería en una unidad de hemodiálisis.Material y Método: Estudio descriptivo transversal, realizado en 2023 en la unidad de hemodiálisis del Hospital Universitario de Jaén. Se utilizó el cuestionario PCHE 3ªversión (32 ítems, escala Likert 1-4), obteniéndose una puntuación sobre percepción global del cuidado humanizado y 3 puntuaciones correspondientes a las dimensiones: “Cualidades del hacer de enfermería”, “Apertura a la comunicación para proporcionar educación para la salud a la persona” y “Priorizar el sujeto de cuidado”. También se recogieron las variables sexo, edad y tiempo en hemodiálisis. Se realizó un análisis descriptivo, y se comparó la puntuación global y de las 3 dimensiones con la variable sexo (U-Mann-Whitney) y con las otras variables (Rho-Spearman). Resultados: Se analizaron 38 cuestionarios, 57,9% hombres, edad media: 65,2±15,28años, mediana tiempo en hemodiálisis: 42 (P25:8-P75:96) meses. Alpha de Cronbach del cuestionario: 0,919. Un 73,7% calificó como “siempre” la percepción global del cuidado humanizado, y un 5,3% “nunca”. Analizando las respuestas “siempre” y “nunca” en cada dimensión, encontramos: “Comunicación” (63,2% vs 5,3%), “Priorizar al paciente” (63,2% vs 5,3%) y “Cualidades del hacer” (84,2% vs 5,3%). Los pacientes con >1 año en hemodiálisis presentaron peor puntuación total PCHE (p=0,03), también encontramos correlación entre “tiempo en diálisis” y la dimensión “Cualidades del hacer” (Rho-Spearman:-0,346; p=0,039).Conclusiones: La percepción por parte de los pacientes en hemodiálisis sobre el cuidado humanizado de enfermería ha sido buena, identificándose áreas de mejora en la comunicación y priorización del paciente (AU)


Objective: To analyze patients’ perception of the humaniza-tion of care in a hemodialysis unit.Material and Methods: A descriptive cross-sectional study was conducted in 2023 in the hemodialysis unit of the Uni-versity Hospital of Jaén.The PCHE questionnaire 3rd version was used (32 items, Li-kert scale 1-4), obtaining a score on the overall perception of humanized care and three scores corresponding to the dimensions: “Qualities of nursing care,” “Openness to com-munication for providing health education to the individual, and “Prioritizing the subject of care”. Sex, age, and time on hemodialysis were also collected as variables. Descriptive analysis was performed, and the overall score and the scores of the three dimensions were compared with the sex variable (U-Mann-Whitney) and other variables (Rho-Spearman).Results: Thirty-eight questionnaires were analyzed, 57.9% male, mean age: 65.2±15.28 years, median time on hemo-dialysis: 42 (P25:8-P75:96) months. Cronbach’s alpha for the questionnaire was 0.919. 73.7% rated the overall perception of humanized care as “always,” and 5.3% as “never.” Percen-tages for “always” and “never” responses in each dimension were: “Communication” (63.2% vs. 5.3%), “Prioritizing the patient” (63.2% vs. 5.3%), and “Qualities of care” (84.2% vs. 5.3%). Patients with >1 year on hemodialysis had a lower to-tal PCHE score (p=0.03), and we also found a correlation be-tween “time on dialysis” and the “Qualities of care” dimension (Rho-Spearman: -0.346; p=0.039).Conclusions: The perception of humanized nursing care by hemodialysis patients has been high, identifying areas for im-provement in communication and patient prioritization (AU)

Introducción:El estreñimiento es un problema frecuente en pacientes en hemodiálisis, asociado, entre otros aspectos a comorbilidad, polimedicación y restricciones hídricas y dieté-ticas, pudiendo llegar a afectar la calidad de vida. Objetivos: Analizar la relación del estreñimiento en la calidad de vida de pacientes en hemodiálisis, determinar la prevalen-cia del estreñimiento objetivo y subjetivo, y analizar las varia-bles más influyentes. Material y Método: Estudio observacional descriptivo trans-versal en una unidad de hemodiálisis hospitalaria. Se emplea-ron los Criterios Diagnósticos Roma IV (2016) y escala visual Bristol para evaluar el estreñimiento. Para evaluar la calidad de vida, el cuestionario CVE-20. Resultados: Se estudiaron 38 pacientes (58% hombres) con una edad media de 68,69±12,76 años, tiempo en hemodiá-lisis de 44±37,19 meses y el 31,57% eran sedentarios. La ingesta hídrica diaria fue de 1004,05±460,13 ml y diuresis 658,11±696,47 ml. El índice de Charlson fue 8,15±2,29 pun-tos. El 45% presentaron estreñimiento subjetivo y 42% obje-tivo. Los pacientes con estreñimiento fueron: 56% mujeres, con edad media de 67,81±14,88 años y tiempo en hemodiá-lisis de 40,06±32,97 meses; 12% tenían pautados opioides, 44% quelantes del fósforo, 56% hierro intravenoso y 25% laxantes. La calidad de vida fue inferior en los pacientes es-treñidos (53,06 vs 69,62 puntos); existiendo diferencias signi-ficativas en todas las dimensiones del cuestionario.Conclusiones: En los pacientes en hemodiálisis existe una importante prevalencia de estreñimiento, siendo más pre-valente en mujeres. Este estreñimiento está asociado a peor percepción de la calidad de vida (AU)


Introducción: Constipation is a frequent issue in hemodialy-sis patients, associated, among other factors, with comorbidi-ty, polypharmacy, and restrictions in fluid and dietary intake, potentially affecting the quality of life.Objectives: To analyze the relationship between constipation and the quality of life of hemodialysis patients, determine the prevalence of objective and subjective constipation, and analyze the most influential variables.Material and Method: Cross-sectional descriptive obser-vational study in a hospital hemodialysis unit. The Rome IV Diagnostic Criteria (2016) and the Bristol Visual Scale were used to assess constipation. Quality of life was evaluated using the CVE-20 questionnaire.Results: A total of 38 patients were studied (58% males) with a mean age of 68.69±12.76 years, a hemodialysis du-ration of 44±37.19 months, and 31.57% were sedentary. aily fluid intake was 1004.05±460.13 ml, and diuresis was 658.11±696.47 ml. Quality of life was lower in constipated patients (53.06 vs. 69.62 points), with significant differences in all dimensions of the questionnaire.Conclusions: Hemodialysis patients have a significant preva-lence of constipation, more prevalent in females. This consti-pation is associated with a poorer perception of quality of life (AU)

Introducción:Determinar el grado de conocimientos que tie-nen los pacientes en hemodiálisis sobre su enfermedad y trata-miento, valorar su capacidad para hacerse el autocuidado y su relación con la calidad de vida relacionada con la salud.Material y Método: Se realizó un estudio descriptivo y trans-versal en dos unidades de diálisis. Se estudiaron 31 pacien-tes en hemodiálisis con edad media de 67,2±14 años (71% hombres). Se estudió nivel de conocimientos sobre la enfermedad y tra-tamiento, capacidad de autocuidado y calidad de vida (lámi-nas COOP/WONCA). Se recogieron, además, variables socio-demográficas, clínicas, comorbilidad y grado de dependencia.Resultados: El 9,7% de los pacientes eran laboralmente acti-vos. El 16,1% no tenía formación, 38,7% básica, 32,3% secun-daria y 12,9% universitaria. El 61% tenía un grado de conoci-mientos alto sobre su enfermedad y tratamiento, el 32% medio y el 7% bajo.El 52 % de los pacientes mostraron alta capacidad para el au-tocuidado y el 48% media. El 3,2% de los pacientes presentaba dependencia severa, moderada el 22,6%, leve el 6,5%, siendo autónomos el 67,7%.La puntuación media en el cuestionario de calidad de vida fue de 24,06±6,5 puntos.Encontramos relación significativa entre el grado de conocimien-tos con edad (0,384, p<0,05) y nivel formativo (0,464, p<0,01).Conclusiones: Los pacientes presentan un alto nivel de cono-cimiento sobre su enfermedad y tratamiento, alto grado de autonomía y capacitación para el autocuidado, siendo los más jóvenes y con mayor nivel formativo, los que muestran mejores resultados, sin que estas variables tengan relación con la cali-dad de vida (AU)


Introduction:To determine the level of knowledge that hemodialysis patients have about their disease and treatment, assess their ability for self-care, and examine its relationship with health-related quality of life.Material and Method:A descriptive, cross-sectional study was conducted in two dialysis units. Thirty-one hemodialysis patients were studied, with a mean age of 67.2±14 years (71% male). The study assessed the level of knowledge about the disease and treatment, self-care ability, and quality of life (using COOP/WONCA charts). Additionally, sociodemographic, clinical, comorbidity, and dependence variables were collected.Results: 9.7% of the patients were employed. 16.1% had no formal education, 38.7% had basic education, 32.3% had secondary education, and 12.9% had a university education. 61% had a high level of knowledge about their disease and treatment, 32% had a moderate level, and 7% had a low level.52% of the patients demonstrated a high capacity for self-care, while 48% showed a moderate capacity. 3.2% of the patients exhibited severe dependence, 22.6% moderate dependence, 6.5% mild dependence, and 67.7% were autonomous. The average score on the quality-of-life questionnaire was 24.06±6.5 points. A significant relationship was found between the level of knowledge and age (0.384; p<0.05) and educational level (0.464; p<0.01).Conclusions: Patients exhibit a high level of knowledge about their disease and treatment, along with a high degree of autonomy and self-care capabilities. Younger patients and those with higher education levels show better results, although these variables do not appear to be directly related to quality of life (AU)

Background: Significant proportion of haemodialysis (HD) patients fails to comply with their prescribed medication regimens, leading to poor health outcomes and survival rates. This highlights the need for interventions that improve medication adherence thereby leading to improved patient outcomes. Objectives: To evaluate the impact of motivational interviewing, a skillful clinical method to improve medication adherence among HD patients. Methods: A pre-post study was conducted prospectively among 63 HD patients at multicentre dialysis units. The recruited patients received three sessions of MI which helps in addressing patient beliefs around medication and overcome barriers for nonadherence. General medication adherence scale (GMAS) was used to evaluate the medication adherence among enrolled patients. An independent t-test was used to analyze the impact of MI in improving adherence among HD patients. In this study, the level of significance was 0.05. Results: A total of 63 patients (27 males and 36 females; mean age 48.5±13.9 years) were included in the study. While the mean of dialysis duration and number of medications was 7.7±6.0 years and 8.1±2.2 respectively. Paired t-test showed a significant increase in medication adherence score from a baseline for nonadherence domain of patient’s attitude and nonadherence domain of additional illness and pill overload at the end of the study (p<0.05). However, the non-adherence domain of financial constraints, was not statistically significant, p=0.507. Motivational interviewing was effective in increasing intrinsic positive attitude by resolving their ambivalence thus improving the adherence towards medication. Conclusion: This study emphasized that MI conducted by pharmacist effectively improves HD patients' medication adherence. Healthcare professionals ought to employ this strategy in order to assist HD patients in better taking their medications (AU)

La hemodiafiltración con reinfusión endógena del ultrafiltrado (HFR) es una técnica de diálisis caracterizada por un cartucho de resina con poder adsorbente que combina los mecanismos difusión, convección y adsorción en un solo esquema terapéutico. Después de cerca de 20 años de experiencia clínica con HFR, el presente artículo revisa la evidencia acumulada con esta técnica, planteando si la adición de la adsorción, como tercer mecanismo depurativo, debería ser el siguiente paso en el tratamiento de los pacientes en hemodiálisis. La HFR, a pesar de producir una extensa eliminación de toxinas urémicas, ha demostrado reducir la pérdida de nutrientes y componentes fisiológicos durante la sesión de diálisis frente a la hemodiafiltración on-line, mitigando el estado inflamatorio y el estrés oxidativo en esta población. Además de su facilidad de uso, la técnica también es altamente biocompatible y puede utilizarse en situaciones de un acceso vascular comprometido. En base a estas observaciones, la HFR parece ser una técnica especialmente útil para pacientes con elevada comorbilidad, incluyendo aquellos con fragilidad, desnutrición o enfermedad cardiovascular. En esta revisión, como panel de consenso de nefrólogos con experiencia clínica en HFR, examinamos la literatura existente y resumimos nuestros puntos de vista sobre cómo usar esta técnica, qué perfil de paciente puede ser más adecuado para la HFR, y cómo prescribir y monitorizar de manera práctica esta modalidad de diálisis (AU)


Hemodiafiltration with endogenous reinfusion of the ultrafiltrate (HFR) is a dialysis technique characterized by a resin cartridge with adsorptive properties that combines the mechanisms of diffusion, convection, and adsorption in a single therapeutic regimen. After nearly 20 years of clinical experience with HFR, this article reviews the accumulated evidence with this technique, considering whether adsorption reduction, as a third purification mechanism, should be the next step in the treatment of hemodialysis patients. HFR, beyond producing an extensive removal of uremic toxins, has demonstrated to reduce the loss of nutrients and other physiological components during the dialysis session as compared to online hemodiafiltration, ameliorating the inflammatory state and oxidative stress in this population. In addition to its ease of use, the technique is also highly biocompatible and can be used in patients with a compromised vascular access. Based on these observations, HFR appears to be an especially useful therapy for high-comorbidity patients, including those with frailty, malnutrition, or cardiovascular disease. In this review, we, as a consensus panel of nephrologists experienced with HFR, survey existing literature and summarize our views on when to use this technique, which patients may be best suited for HFR, and how to effectively prescribe and monitor this modality of dialysis in daily clinical practice (AU)

Objetivo Evaluar el valor de la prestación de servicios de diálisis concertada frente a la hospitalaria para el tratamiento de la enfermedad renal crónica en España mediante la metodología de análisis de decisión multicriterio. Método Se utilizó el marco de evaluación EVIDEM (Evidence and Value: Impact on Decision Making) para el cálculo del valor estimado de ambos modelos de prestación de la diálisis (concertada vs. hospitalaria) mediante un taller virtual en el que participaron diferentes perfiles: directivos y gestores, profesionales y responsables de unidades y representantes de pacientes y familiares. Las puntuaciones se combinaron mediante un modelo lineal aditivo, que combinó la ponderación del modelo con la puntuación individual de los criterios, y cada valor se transformó a una escala entre el 0 y 1. Resultados La estimación del valor para la diálisis concertada fue de 0,29 (DE: ±0,2) y de 0,39 (DE: ±0,2) para la diálisis hospitalaria. Todos los perfiles otorgaron un mayor valor a la hemodiálisis hospitalaria. El mayor valor para la diálisis hospitalaria fue de los pacientes (0,44); el menor valor medio fue de los directivos (0,36) y el rango para la diálisis concertada estuvo entre los pacientes (0,31) y los cargos intermedios (0,27). Conclusiones La hemodiálisis hospitalaria obtuvo un mayor valor que la diálisis concertada. En general, los panelistas afirmaron que resulta un ejercicio útil e interesante y que, en cierta medida, aporta seguridad en la toma de decisiones, ya que permite ordenar, racionalizar y considerar, de manera explícita y transparente, los diferentes criterios involucrados (AU)


Objective To evaluate the value of the provision of contracted versus hospital dialysis services for the treatment of chronic kidney disease in Spain using the multicriteria decision analysis methodology. Method The EVIDEM (Evidence and Value: Impact on Decision Making) evaluation framework was used to calculate the estimated value of both dialysis delivery models (arranged vs. hospital) through a virtual workshop in which different profiles participated: directors and managers, professionals and heads of units and representatives of patients and relatives. The scores were combined using an additive lineal model, which combined the weight of the model with the individual score of the criteria, and each value was transformed to a scale between 0 and 1. Results The estimated value for arranged dialysis was 0.29 (DS: ±0.2) and 0.39 (DS: ±0.2) for hospital dialysis. All profiles gave a higher value to hospital hemodialysis compared to contracted hemodialysis. The highest value for hospital dialysis was for patients (0.44), with the lowest mean value for directors (0.36) and the range for arranged dialysis being between patients (0.31) and intermediate positions (0.27). Conclusions Hospital hemodialysis obtained a higher value than concerted dialysis. In general, the panelists affirmed that it is a useful and interesting exercise and that, to a certain extent, it provides security in decision-making, since it allows ordering, rationalizing and considering, in an explicit and transparent manner, the different criteria involved (AU)
