RESUMO

Desde la organogénesis y hasta estadios adultos, las células madre mesenquimales participan activamente dando origen y manteniendo la homeostasis del organismo. En la cavidad oral han sido aisladas desde variadas estructuras del órgano dental tales como el ligamento periodontal, pulpa dental, tejido gingival, folículo dental y papila apical significando una prometedora fuente de células madre mesenquimales las que pueden ser caracterizadas de acuerdo a los criterios mínimos establecidos por 'The International Society for Cellular Therapy' que son: a) La adherencia al plástico; b) La expresión de marcadores CD73, CD90, CD105 y la carencia de CD34,CD45, CD14, CD11, CD79, CD19 y HLA-DR (clase II); c) Capacidad multipotencial de diferenciación hacia linajeosteogénico, condrogénico y adipogénico. El objetivo de esta revisión consiste en realizar un levantamiento de la situación actual de este tema efectuando una revisión comprensiva de la literatura en los campos de; identificación a través de marcadores de superficie, aislamiento por medio de mecanismos de digestión enzimática o explante, almacenamiento atendiendo a la necesidad de suprimir el uso de suero fetal bovino como medio de cultivo en un esfuerzo por avanzar hacia aplicaciones terapéuticas, banca o criopreservación destacando nuevas experiencia en este campo como lo es la criopreservación de piezas dentales completas gracias a la tecnología láser Nd:YAG. Y, finalmente, las aplicaciones clínicas que promete este grupo de células a través de la medicina regenerativa y la ingeniería tisular tanto en el campo de la odontología como la medicina general (AU)


Since organogenesis and even adult stages, mesenchymal stem cells actively participate starting and maintaining body homeostasis. In the oral cavity they have been isolated from various structures dental organ such as the periodontal ligament, dental pulp, gingiva, dental follicle and apical papilla meaning a promising source of mesenchymal stem cells which can be characterized according to the minimum criteria set by The International Society for Cellular Therapy that are: a) the adherence to plastic, b) expression of markers CD73, CD90, CD105 and the absence of CD34, CD45, CD14, CD11, CD79, CD19 and HLA-DR (class II ) c) multipotent differentiation capacity to osteogenic lineage, chondrogenic and adipogenic. The objective of this review is to conduct a survey of the current status of this issue by a comprehensive review of the literature in the fields; identification through surface markers, isolation through mechanisms of enzymatic or explant, storage digestion with the need to eliminate the use of fetal bovine serum as the culture medium in an effort to move towards therapeutic applications, highlighting new banking and cryopreservation experience in this field such as the cryopreservation of whole teeth thanks to the NdYAG laser technology. And finally promising clinical applications this group of cells through regenerative medicine and tissue engineering both in the field of dentistry and general medicine (AU)

Introducción: El aparato genitourinario puede resultar afectado por traumatismos, enfermedades congénitas, infecciones o patología tumoral, conllevando la pérdida de alguno de sus órganos o de sus funciones. Dentro de la patología tumoral, el cáncer de pene es una de las patologías que, con una frecuencia considerable, ocasiona dichas alteraciones. El tratamiento convencional es quirúrgico, implicando la amputación parcial o total, y puede generar consecuencias importantes, como impotencia sexual o incontinencia urinaria. Objetivo: Realizar una revisión bibliográfica sobre los últimos avances y opciones terapéuticas en relación con la ingeniería tisular en la reconstrucción de estructuras peneanas. Método: Realizamos una revisión bibliográfica en PubMed introduciendo las palabras clave «tissue engineering AND penis» y seleccionando las publicaciones con abstract en dicho buscador científico. Resultados: Tras realizar un análisis detallado de las publicaciones seleccionadas se pone de manifiesto que las terapias regenerativas aplicadas en la reparación de estructuras peneanas (cuerpos cavernosos, uretra, túnica albugínea) se basan en la utilización de diferentes materiales sintéticos o biológicos acelulares o con cultivo celular. Conclusiones: El avance en la construcción de tejidos mediante ingeniería tisular supone una revolución en la medicina actual, si bien son necesarios más ensayos clínicos y una mejora en las condiciones de uso de estas nuevas terapias para que puedan ser aplicadas de forma estandarizada en la clínica habitual (AU)


Introduction: the genitourinary tract may be affected by trauma, congenital diseases, infections or tumour pathology, producing a loss of any of their organs or functions. In tumour pathology, penile cancer is one of the pathologies that, with considerable frequency, causes these alterations. Conventional treatment is surgical, involving partial or total amputation, and can generate significant consequences, such as impotence or urinary incontinence. Objective: our objective was to review the literature on recent advances and therapeutic options in relation to tissue engineering in the reconstruction of penile structures. Method: a literature review of PubMed, was carried out, entering the keywords "tissue engineering AND penis", selecting those publications with abstracts. Results: a detailed analysis of the selected publications revealed that regenerative therapies applied in the repair of penile structures (corpora cavernosa, urethra, tunica albuginea) are principally based on the use of different acellular synthetic or biological materials or with cell culture. Conclusions: progress in the construction of tissue-engineered tissues represents a revolution in medicine today. However, more clinical trials are needed to improve the conditions of use of these new therapies, so they can be applied in a standardized way in clinical practice

Las estenosis de uretra son uno de los problemas urológicos más comunes; no obstante, las limitaciones naturales de la cicatrización de heridas y las exigencias fisiológicas de las estructuras anatómicas se combinan para hacer que las estenosis de uretra sean también uno de los problemas urológicos más difíciles de tratar. La adecuada cicatrización de heridas exige unos bordes bien aproximados debido a la prolongada inflamación y granulación necesarias para cerrar heridas grandes y profundas, que tiene como resultado una excesiva producción de colágeno, fibrosis y la formación de una cicatriz o, en la uretra, una estenosis. Los biomateriales han sido utilizados con éxito para aproximar la matriz extracelular de diferentes tipos de tejidos y pueden definir un espacio tridimensional adecuado para la formación de nuevos tejidos con una estructura y función adecuadas. Los biomateriales pueden ser clasificados de una forma amplia como polímeros sintéticos o matrices tisulares, cada uno con sus ventajas y limitaciones. Estudios recientes utilizando biomateriales naturales sembrados con células en la reparación uretral han dado algunos resultados prometedores. Sin embargo, los avances en la utilización de fuentes alternativas de células para sembrar en matrices y los materiales sintéticos sembrados de células albergan la posibilidad de resultados incluso mejores en el futuro


Urethral strictures are one of the most common urological problems, yet the natural limitations of wound healing and the physiologic demands on the anatomic structures combine to also make urethral strictures one of the most challenging urological problems to manage. Proper wound healing demands well approximated edges because prolonged inflammation and granulation, required to close large, deep wounds, will result in excess collagen production, fibrosis, and the formation of a scar or, in the urethra, a stricture. Biomaterials have successfully been used to approximate the ECM of several different tissue types and can define a three dimensional space suitable for the formation of new tissues with both appropriate structure and appropriate function. Biomaterials can be broadly categorized as either synthetic polymers or tissue matrices, each with their advantages and limitations. Recent studies utilizing cell seeded natural biomaterials in urethral repair has yielded some promising results. However, advancements in the use of alternative sources of cells for matrix seeding and cell-seeded synthetic materials hold the possibility of even better results in the future

Objetivos. Numerosas patologías que afectan a la vejiga, de origen congénito (extrofia) o adquirido (traumatismos, tumores), requieren la reconstrucción de la pared vesical utilizando intestino delgado, sigma o estómago, los cuales no están exentos de complicaciones. Por ese motivo, en el presente trabajo pretendemos desarrollar un nuevo modelo de pared vesical humana mediante ingeniería tisular que pudiese tener una utilidad clínica. Material y métodos. En primer lugar, se procedió a generar cultivos primarios de células epiteliales y estromales de la mucosa vesical a partir de pequeñas biopsias de la pared vesical humana, utilizando para ello técnicas de digestión enzimática mediante tripsina-EDTA y colagenasa. Posteriormente, se generó un sustituto tridimensional de la mucosa vesical utilizando como soporte biomateriales de fibrina-agarosa. El análisis de las muestras se realizó a los 14 días mediante examen histológico de muestras teñidas con hematoxilina-eosina. Resultados. La aplicación de los métodos de digestión enzimática permitió generar eficientemente cultivos primarios de células epiteliales y estromales de la mucosa vesical humana, comprobándose que la tasa de proliferación de las células estromales era superior a la de las células epiteliales. Una vez generados los sustitutos de la pared vesical, se comprobó el adecuado nivel de biocompatibilidad del biomaterial y las células estromales y epiteliales. La estructura histológica de los sustitutos de pared vesical presentaba una gran analogía con la mucosa vesical humana nativa. Conclusiones. El tejido vesical generado por ingeniería tisular muestra importantes similitudes estructurales e histológicas con el tejido vesical nativo. Estos resultados sugieren que los tejidos generados mediante ingeniería tisular podrían tener utilidad terapéutica en el futuro (AU)


Introduction. Certain urological congenital conditions, such as bladder exstrophy and acquired conditions such as trauma and tumors may require the use of different tissues like small bowel, sigmoid colon or stomach for bladder reconstruction. However, these tissues are often associated to important complications. The aim of this study is to develop a novel substitute of the human bladder wall by tissue engineering. Material and methods. We first generated primary cell cultures of epithelial and stromal bladder mucosa cells from small tissue biopsies of human bladder by using enzymatic methods based on trypsin-EDTA and collagenase I. Then, a three-dimensional substitute of the bladder mucosa was generated using fibrin-agarose biomaterials. The analysis of the tissue substitutes was carried out at day 14th of development by histological examination of samples stained with hematoxylin-eosin. Results. The use of enzymatic digestion methods allowed us to efficiently generate primary cell cultures of the human bladder epithelial and stromal cells. The proliferation rate was higher in stromal cells as compared to epithelial cells. Once the bladder mucosa substitutes were generated, a good biocompatibility of the stromal and epithelial cells into the biomaterial was found. The histological structure of the bladder wall substitutes was analogue to that of the native human bladder mucosa. Conclusions. The bladder mucosa substitute generated by tissue engineering showed structural and histological similarities with the native human bladder tissues and open the door to the future therapeutic use of these bioengineered tissues (AU)

In the last decade, tissue engineering is a field that has been suffering an enormous expansion in the regenerative medicine and dentistry. The use of cells as mesenchymal dental stem cells of easy access for dentist and oral surgeon, immunosuppressive properties, high proliferation and capacity to differentiate into odontoblasts, cementoblasts, osteoblasts and other cells implicated in the teeth, suppose a good perspective of future in the clinical dentistry. However, is necessary advance in the known of growth factors and signalling molecules implicated in tooth development and regeneration of different structures of teeth. Furthermore, these cells need a fabulous scaffold that facility their integration, differentiation, matrix synthesis and promote multiple specific interactions between cells. In this review, we give a brief description of tooth development and anatomy, definition and classification of stem cells, with special attention of mesenchymal stem cells, commonly used in the cellular therapy for their trasdifferentiation ability, non ethical problems and acceptable results in preliminary clinical trials. In terms of tissue engineering, we provide an overview of different types of mesenchymal stem cells that have been isolated from teeth, including dental pulp stem cells (DPSCs), stem cells from human exfoliated deciduous teeth (SHEDs), periodontal ligament stem cells (PDLSCs), dental follicle progenitor stem cells (DFPCs), and stem cells from apical papilla (SCAPs), growth factors implicated in regeneration teeth and types of scaffolds for dental tissue regeneration (AU)

La reconstrucción de grandes defectos de mucosa oral a menudo es desafiante, por la escasez de mucosa oral sana para reemplazar tejidos dañados. De esta forma, las técnicas de ingeniería tisular pueden suponer una fuente de tejidos autólogos disponible para trasplantar a estos pacientes. En este trabajo hemos desarrollado un nuevo modelo de mucosa oral artificial generada mediante ingeniería tisular usando un soporte de fibrina-agarosa. Para ello, se han generado cultivos primarios de fibroblastos de la mucosa oral humana y queratinocitos a partir de pequeñas biopsias de mucosa oral normal y aplicándoles tratamiento senzimáticos. Después, se ha determinado la viabilidad de las células cultivadas mediante microanálisis por rayos-X, demostrando que la mayoría de las células de los cultivos primarios estaban vivas y tenían elevados K/Na ratios. Una vez que la viabilidad celular fue determinada, se usaron los fibroblastos y queratinocitos cultivados para desarrollar un constructo de mucosa oral sobre una matriz extracelular de (..) (AU)


Reconstruction of large oral mucosa defects is often challenging, since the shortage of healthy oral mucosa to replace the excised tissues. This way, tissue ingineering techniques may provide a source of autologoustissues available for transplant in these patients. In this work, wehave developed a new model for artificial oral mucosa generated by tissue engineering using a fibrin-agarosa scaffold. For that purpose, we have generated primary cultures of human oral mucosa fibroblasts and keratinocytes from small biopsies of normal mucosa oral using enzymatic treatments. Then, we have determined the viability of cultured cells by electron probe quantitative X-ray microanalysis, and we have demonstrated that most of the cells in the primary cultures were alive and hd high K/Na ratios. Once cell viability was determined, we used cultured fibroblasts and keratinocytes to develop an artificial oral mucosaconstruct by using a fibrin-agarosa extracellular matrix and a sequential culture technique using porous culture inserts. RESUMO

La aplicación de criterios tumorales en el tratamiento quirúrgico secuencial de las osteítis crónicas y seudoartrosis sépticas en forma de resecciones segmentarias origina amplios defectos óseos. Una vez controlado el proceso séptico, en un entorno estable y vascularizado, se procede a la reconstrucción en dichas lesiones mediante transporte óseo basado en la osteogénesis a distracción. A pesar de los buenos resultados obtenidos, los periodos de fijación externa y de curación son muy largos, 2,4 meses por centímetro reconstruido. Estos prolongados tratamientos motivan que haya un elevado número de complicaciones. Los avances en ingeniería tisular y regeneración ósea contribuyen con nuevas técnicas y opciones terapéuticas. La estimulación de la formación de hueso nuevo en la zona de distracción o en el punto de atraque del fragmento transportado puede minimizar las complicaciones asociadas a la larga duración del procedimiento. La utilización de un osteoinductor como BMP-7 origina en la práctica un aumento de la formación ósea y el nuevo hueso puede tener incrementadas sus propiedades biomecánicas (AU)


The application of oncologic criteria in the sequential surgical treatment of chronic osteomyelitis foci and septic pseudarthrosis, through segmental resections, produces extensive bone defects. Once the septic process is under control, in a stable and well vascularized atmosphere, we perform reconstruction of intercalary defects through bone transportation based on distraction osteogenesis. Despite the good results, the healing index and the time on ex-fi x are very long, about 10 weeks per centimeter rebuilt. These prolonged treatments lead to a large number of complications. Advances in tissue engineering and bone regeneration have led to new techniques and therapeutic options. Stimulation of new bone formation in the distraction area or in the “docking-site” can minimize the complications associated with the length of time required by the procedure. The use of an osteoinducer, such as BMP-7, increases new bone formation and may enhance its biomechanical properties (AU)

Se ha demostrado que el colágeno es un nuevo biomaterial utilizado para la administración de fármacos, la fabricación de apósitos o como sustrato para ingeniería tisular cuya biocompatibilidad y propiedades biodegradables son únicas. El colágeno bovino y porcino tipo I constituyen una fuente fácilmente disponible de material de soporte para diversas aplicaciones biomédicas. Sin embargo, estas fuentes conllevan cierto riesgo potencial de enfermedades infecciosas como la encefalopatía espongiforme bovina o la encefalopatía espongiforme transmisible. Por esta razón, existe una demanda de colágeno tipo I procedente de otras fuentes. En el presente estudio, se utilizan animales acuáticos y, en concreto, especies de tiburón en las que el colágeno tipo I es una proteína principal de la piel y la estructura tiene similitud con la de las especies mamíferas. Se ha intentado utilizar colágeno de piel de tiburón como matriz de soporte con extracto de aloe para mejorar la estabilidad. Estas matrices de soporte se caracterizaron por varias propiedades fisicoquímicas y por la evaluación de biocompatibilidad para facilitar el crecimiento de fibroblastos dérmicos humanos in vitro. La incorporación de extracto de aloe influyó enormemente en la morfología y las propiedades fisicoquímicas de la matriz de soporte. Se observó in vitro que los fibroblastos conservaban la orientación organizada en forma de huso al cultivarse sobre la matriz de soporte de colágeno. Así, la matriz de soporte de colágeno desarrollada con una proporción de 10:1 de colágeno de piel de tiburón y extracto de aloe, respectivamente, sirvió como material biocompatible con una resistencia a la tracción apreciable. La investigación anterior sugiere que la matriz de soporte de colágeno de piel de tiburón desarrollada puede ser una alternativa efectiva al colágeno de mamífero en el campo de la ingeniería tisular y para diversas aplicaciones en la curación de heridas (AU)


Collagen has proven to be a novel biomaterial used for drug delivery, wound cover dressings or as a substrate for tissue engineering with unique biocompatibility and biodegradable properties. Bovine and porcine Type I collagen provide a readily available source of scaffold material for various biomedical applications. However these sources have some potential risk of infectious diseases such as bovine spongiform encephalopathy or transmissible spongiform encephalopathy. Hence there is demand for an alternative Type I collagen from various other sources. The present study utilizes the aquatic animals particularly the shark species in which collagen Type I is a major protein in the skin and the structure has similarity to that of mammalian species. An attempt was made to use shark skin collagen as scaffold with the extract of aloe to improve the stability. These scaffolds were characterized for various physicochemical properties and biocompatibility assessment to support the growth of human dermal fibroblasts in vitro. The incorporation of aloe extract highly influenced the morphology and physicochemical properties of the scaffold. It was observed in vitro that the fibroblasts retained the spindle shape, organized orientation when cultured over collagen scaffold. Thus the developed collagen scaffold at 10: 1 ratio of shark skin collagen and aloe extract respectively served as a biocompatible material with appreciable tensile strength. The above investigation suggests that the developed shark skin collagen scaffold could be an effective alternative for the mammalian collagen for tissue engineering and various wound healing applications (AU)

Objetivo. Valorar la efectividad de un equivalente cutáneo diseñado en un banco de tejidos, en el tratamiento de úlceras vasculares crónicas. Pacientes y métodos. Entre septiembre de 1999 y diciembre de 2001 se incluyó a 25 pacientes con úlceras vasculares de evolución tórpida (>4 meses) en quienes, tras ingreso hospitalario y tratamiento estándar, se objetivó un estancamiento en su cicatrización. Se constataron nueve úlceras venosas, dos arteriales, siete hipertensivas y siete mixtas (componente flebostático y arterial). Descartada la infección clínica y bacteriológica de la úlcera, los injertos se colocaron una vez a la semana hasta la cicatrización o suspensión del tratamiento por no respuesta. Se realizaron controles fotográficos digitales semanales para medir el área de las lesiones. A partir del segundo injerto, el tratamiento se realizó ambulatoriamente. El seguimiento medio fue de 18 meses (intervalo: 6-30 meses). Se valoró la ausencia de rechazo, la tasa de cicatrización, el tiempo de reducción del área al 50 y 75%, y la recidiva ulcerosa. Resultados. No se detectaron signos de rechazo. Tasa global de cicatrización del 80% (20/25); en hipertensivas y mixtas, tasa del 100% (14/14). Tiempo medio de cierre: 5,3 semanas (intervalo: 3-12 semanas). Número medio de injertos utilizados: 5,8 (intervalo: 3-13 injertos). La recidiva ulcerosa fue del 25% (cuatro venosas y una hipertensiva). Aims. To assess the effectiveness of a skin equivalent designed in a tissue bank in the treatment of chronic vascular ulcers. Patients and methods. Between September 1999 and December 2001, 25 patients with vascular ulcers that progressed in a torpid fashion (>4 months) were admitted to hospital and administered standard therapy, following which sluggish healing processes were observed. Nine venous, two arterial, seven hypertensive and seven mixed ulcers (the latter with a phlebostatic and arterial component) were found. After ruling out the possibility of the ulcer being affected by clinical and bacteriological infections, the grafts were placed once a week until healing had been achieved or the treatment was suspended due to lack of response. Weekly digital photographic monitoring was carried out to measure the extension of the lesions. From the second graft onwards, patients were treated in the outpatients department. Average follow-up time was 18 months (interval: 6-30 months). Values measured included the absence of rejection, healing rate, the time required to reduce the extension to 50 and 75%, and recurrence of ulcers. Results. No signs of rejection were detected. Overall healing rate was 80% (20/25); in hypertensive and mixed cases, the rate was 100% (14/14). Average closing time: 5.3 weeks (interval: 3-12 weeks). Average number of grafts used: 5.8 (interval: 3-13 grafts). Recurrence of ulcers was 25% (four venous and one hypertensive). Of the five ulcers that did not heal (four venous and one arterial), none of them got worse: there was no response in three cases and two of them showed a reduction of the extension that reached 85%. Conclusions. Skin equivalents could be a good alternative in the treatment of chronic vascular ulcers (AU)
