Introducción: La obturación del sistema de conductos presenta un papel clave en el éxito del tratamiento de endodoncia. En un intento de mejorar las propiedades de los cementos selladores, recientemente se han introducido en el mercado los cementos selladores a base de silicatos. Por ello, al llevar a cabo la obturación del sistema de conductos, es de utilidad conocer las propiedades físicas que los diferentes cementos selladores presentan. Objetivo: El objetivo del presente trabajo fue la revisión bibliográfi ca de las propiedades físicas de utilidad clínica que presentan los nuevos cementos a base de silicatos, y compararlas con las propiedades físicas de los cementos convencionales a base de resina epóxica. Material y método: Tras establecer la pregunta de investigación adaptada, se llevó a cabo una revisión de la literatura en dos bases de datos (Medline vía Pubmed y Wiley Library vía Biblioteca Chrocane) combinando términos MeSH (Medical Subject Headings) y términos libres. Además, se llevó a cabo una búsqueda electrónica manual. Las propiedades físicas de utilidad clínica seleccionadas fueron la decoloración, capacidad de sellado, radiopacidad, tiempo de fraguado y solubilidad. Resultados: Se obtuvieron 224 estudios potenciales. Finalmente, aplicando los criterios de inclusión y exclusión, se incluyeron 22 estudios en la revisión. Los distintos estudios compararon diferentes propiedades físicas de los cementos a base de silicatos, comparándolos con los cementos a base de resina. (AU)


Introduction: In the endodontic treatment success, the fi lling of the root canal system plays a key role. To improve properties of the sealers used in these treatments, new silicate-based sealers have recently been introduced into the market. Therefore, when performing the endodontic treatment, it is useful to know the physical properties of the different sealers. Objectives: The aim of the present study was to review the literature and compare the clinically useful physical properties of the new silicate-based cements with the physical properties of conventional epoxy resin-based cements. Methods: After establishing the adapted research question, a literature review was carried out in two databases (Medline via Pubmed and Wiley Library via Chrocane Library) combining MeSH (Medical Subject Headings) and free terms. In addition, a manual electronic search was performed. The clinically useful physical properties selected were discolouration, sealability, radiopacity, setting time and solubility. Results: A total of 224 potential studies were obtained and 20 were selected for full text reading. Additionally, 4 studies were selected through electronic handsearching. After exclusion of 2 studies, 22 studies were fi nally included in the review, which analyzed the following physical properties: 2 tooth discolouration; 4 sealing ability; 11 radiopacity; 9 setting time; and 12 solubility. 9 of the selected articles evaluated several properties. Background: Periapical surgery focuses on the treatment of teeth with persistent periapical lesions when ortho-grade root canal treatment fails. Although MTA® is the gold standard material for retrograde filling, Biodentine®- a tricalcium silicate-based cement - has been proposed in order to resolve several of its limitations. A systematicreview has been carried out to compare the physicochemical properties of Biodentine® versus MTA® as root-endfilling material in periapical surgery.Material and Methods: An electronic search was conducted by two independent examiners during March 2020 inthe Cochrane, PubMed-MEDLINE and Scopus databases. In addition, a manual search was made in specializedjournals. Comparative human or in vitro studies that evaluated bond strength, the presence of marginal gap andsealing ability were included. No restriction on publication date was applied. Animal studies, clinical cases, casesseries and expert opinions were excluded.Results: After analyzing 147 initially selected studies, 13 publications were included. Regarding bond strength,the studies seemed to evidence better performance of Biodentine® in both acidic and blood contaminatedenvironments. In relation to the presence of marginal gap and sealing ability, the studies yielded contradictory results. According to some authors, the sealing ability of Biodentine® is greater than that of MTA® during the first 24hours, though both materials prove equal after one week. Other authors recorded no significant differences.Conclusions: Considering the limitations and heterogeneity of the studies included, there is not sufficient evidence toconfirm the clinical superiority of Biodentine® as a root-end filling material in periapical surgery.(AU)

Las perforaciones radiculares son complicaciones no deseadas que pueden ocurrir durante el tratamiento de conductos. Los principales factores que afectan el pronóstico de un diente que presenta una perforación son la localización y el tamaño de esta, el tiempo transcurrido hasta su reparación, el grado de contaminación previa, y la presencia de una radiolucidez adyacente. La aparición de materiales biocompatibles como el agregado de trióxido mineral (MTA) incrementó de forma sustancial el pronóstico de estos dientes alcanzando tasas de éxito de alrededor del 90%. El objetivo de este artículo es describir el manejo ortógrado de dos casos con perforaciones radiculares laterales en los que se utilizaron materiales y técnicas distintas para su sellado. En ambos casos, previamente al sellado ortógrado de las perforaciones, se realizó una desinfección abundante con hipoclorito de sodio (NaOCl) al 2,5%. En el primer caso, se realizó la obturación del tercio apical del conducto usando una técnica de condensación vertical con gutapercha caliente, y posteriormente se obturó el resto del conducto, incluida la perforación, mediante MTA blanco (Angelus, Londrina, PR, Brasil). En el segundo caso, que además presentaba la dificultad añadida de un instrumento fracturado, se utilizó Biodentine (Septodont, Saint-Maur-des-Fosses, Francia) en toda la longitud del conducto radicular donde se encontraba la perforación. La ausencia de signos y síntomas clínicos y la curación radiográfica en progreso revelan unos resultados satisfactorios en las revaluaciones postoperatorias a los 6 y 18 meses


Root perforations are undesired complications that can occur during root canal treatments. The prognosis of teeth with root perforations could be affected by several factors described in the literature, such as location, size, immediacy in the repair, previous microbial contamination and the presence of a radiolucency adjacent to perforation. However, the introduction of mineral trioxide aggregate (MTA) resulted in an improved prognosis and higher success rates. The purpose of this case report was to describe two nonsurgical endodontic retreatments with lateral root perforations using two different materials and techniques for filling. A disinfection with 2,5% sodium hypochlorite was carried out in both root canals with root perforation. In the first case, the apical third of the canal was filled with vertically compacted warm gutta-percha, while the rest of the canal was sealed with white MTA (Angelus, Londrina, PR, Brazil) for proper sealing of the perforation. In the second case, in addition to presenting the added difficulty of a separated instrument, the complete root canal filling with perforation was performed using Biodentine (Septodont, Saint-Maur-des-Fosses, France). The absence of clinical signs and symptoms and the radiographic healing in progress revealed satisfactory results after 6 and 18 months of follow-up

INTRODUCCIÓN: El primer molar superior se caracteriza por tener 3 raíces y cuatro conductos radiculares. La presencia de cuatro raíces es un hallazgo inusual, pocas veces reportado en la literatura. CASO CLÍNICO: Acude a la consulta un paciente caucásico con pulpititis irreversible del 2.6. Se decide iniciar el trata-miento de conductos, pero dada la diferente morfología radicular visible radiográficamente, se realiza una CBCT previa. En el corte axial, se aprecian 4 raíces, con ápices totalmente independientes. Además, la raíz mesiovestibular presenta un único conducto y forma redondeada. Con la ayuda del microscopio se realiza una apertura inusual y se localizan los cuatro conductos. Finalizadas la instrumentación, irrigación y obturación, se recubre el acceso cameral con composite, para la posterior realización de una incrustación, capaz de sellar la caries cervical y restaurar los puntos de contacto. En el control 2 años posterior, el paciente está totalmente asintomático y la restauración demuestra su funcionalidad y estética. CONCLUSIÓN: Las anomalías anatómicas pueden presentar-se en cualquier diente. Encontrar un primer molar superior con más de tres raíces es un hallazgo muy extraño, pero gracias a la ayuda de la radiología tridimensional, se pudo visualizar y tratar de forma adecuada. Ante cualquier imagen radiográfica donde no se pueda apreciar con claridad la anatomía dental a tratar, debemos realizar varias proyecciones radiográficas o una CBCT para poder conocer la morfología real a la que nos enfrentamos


INTRODUCTION: The upper first molar is characterized by having three roots and four root canals. The presence of four roots is an unusual finding, rarely reported in the literature. CLINICA CASE: A caucasian patient with irreversible pulpititis of 2.6 comes to the dental clinic. It was decided to start the root canal treatment, but due to the different root morphology visible radiographically, a previous CBCT was performed. In the axial section, 4 roots are seen, with completely independent apexes. Furthermore, the mesiovestibular root has a single canal and a rounded shape. With the help of the microscope, an unusual opening is made and the four canals are located. After the instrumentation, irrigation and obturation are completed, the cameral access is covered with composite, for the subsequent inlay, capable of sealing cervical caries and restoring the contact points. In the control 2 years later, the patient is completely asymptomatic and the restoration demonstrates its functionality and aesthetics. CONCLUSIONS: Anatomic anomalies occur in any tooth. Finding an upper first molar with more than three roots is a very strange finding, but thanks to the help of three-dimensional radiology, it was able to be properly visualized and treated. Before any radiographic image where the dental anatomy to be treated cannot be clearly seen, we must carry out several radiographic projections or a CBCT to be able to know the real morphology that we are facing

Se trata de un retratamiento de conductos en el diente 1.1. El diente presenta percusión y palpación positiva y movilidad grado II. Ante la falta de información en la radiografía convencional se realiza una TCHC donde se observa tres perforaciones y perdida de la cortical vestibular y apical en el diente 1.1. Se llevó a cabo un tratamiento combinado, primero se realizó el retratamiento ortógrado del conducto con gutapercha termoplástica, segundo una apicectomía y sellado de las perforaciones con MTA de manera quirúrgica. A los 12 meses se comprueba mediante TCHC que se ha producido curación ósea y el diente esta asintomático, pero observamos una discoloracion a nivel de la perforación coronal. A los 5 años esta zona presenta una cavitación y se decide realizar un sellado de la perforación con resina compuesta debido a su localización supracrestal. Un año después la paciente no presenta signos clínicos ni radiológicos de patología


This is a root canals retreatment in tooth 1.1. The tooth presents positive percussion and palpation and grade II mobility. Due to the lack of information from the conventional radiography, we performed a CBCT, three root perforations and loss of the vestibular cortical bone and apical bone were observed in tooth 1.1. A combined treatment was carried out, first the orthograde retreatment of the root canal with thermoplastic obturation technique, second and apicoectomy and sealing of the perforations with MTA by surgery. At 12 months, it is observed in a new TCHC that bone healing has occurred and the tooth is asymptomatic, but we observed a discoloration at the level of the coronal perforation. At 5 years this area presents cavitation and it was decided to seal the perforation with composite resin due to its supracrestal location. A year later, the patient had no clinical or radiological signs of pathology

BACKGROUND: Hemostasis is of critical importance in endodontic surgery. Studies on bleeding control in maxillary molars are scarce. The present study compares the efficacy of two hemostatic techniques in controlling bleeding in endodontic surgery. MATERIAL AND METHODS: A randomized two-arm pilot study involving 30 patients with peri-radicular lesions in maxillary molars (first and second molars) was carried out including the following hemostatic agents: polytetrafluoroethylene (PTFE) strips as an adjunct to epinephrine impregnated gauze (test group; n = 15) and aluminum chloride (Expasyl(TM)) (control; n = 15). Bleeding control was independently assessed by the surgeon and by two blinded observers before and after application of the hemostatic agent, and was classified as either adequate (complete bleeding control) or inadequate (incomplete bleeding control). RESULTS: Bleeding control was similar in both groups. Simple binary logistic regression analysis failed to identify variables affecting bleeding control. Only the height of the keratinized mucosal band (≥ 2 mm) suggested a decreased risk of inadequate bleeding control of up to 89% (OR = 0.11; p = 0.06). CONCLUSIONS: No difference in the efficacy of bleeding control was observed between PTFE strips as an adjunct to epinephrine impregnated gauze and aluminum chloride in maxillary molars

BACKGROUND: Several studies published in the last two decades have found an association between the prevalence of apical periodontitis (AP) or root canal treatment (RCT) and cardiovascular diseases (CVDs). However, the demonstration of association does not prove by itself the existence of a cause-effect relationship. Two diseases can appear as statistically related without any of them directly affecting the values of the other, resulting in a non-causal relationship. The aim of this narrative review is to summarize the current state of knowledge regarding the association between AP and CVDs, analysing it according to the Hill's causality criteria. MATERIALS AND METHODS: Epidemiological studies carried out on the association between CVDs and AP or RCT published I n English until 8 December 2019 were identified. For ty-four articles were selected and its results were analysed. RESULTS: Numerous cross-sectional epidemiological studies have found significant relationship between CVDs and AP. The odds ratio values range 1.6 - 5.4. However, other studies have not found significant association. Respect to RCT, some studies found correlation, but others found no association or even found that RCT is a protective factor against CVDs. CONCLUSIONS: The results are inconsistent and a causal relationship between CVDS and endodontic disease cannot be stablished. The risk factors common to both diseases can act as confounding factors, biasing the results. To reach definitive conclusions about the type of association (causal or non-causal) between both diseases, longitudinal epidemiological studies must be carried out to establish the temporal relationship and the dose-response gradient

Las encuestas realizadas en España ponen de manifiesto que los dentistas prescriben con frecuencia antibióticos en casos donde no están indicados, tanto a nivel terapéutico (pulpitis irreversible sintomática, pulpa necrótica con absceso apical agudo, periodontitis apical asintomática, absceso apical localizado sin síntomas generales en pacientes sanos), como profilácticamente. Esto puede contribuir al desarrollo de cepas bacterianas resistentes. La Asociación Española de Endodoncia (AEDE) propone estas recomendaciones, fruto del consenso de un comité de expertos y basadas en la evidencia científica, para proporcionar a los dentistas, y a otros trabajadores de la salud, criterios científicos sobre el uso de antibióticos en el tratamiento de las infecciones endodónticas, en la profilaxis antibiótica en endodoncia, y sobre el uso de antibióticos tópicos en endodoncia. Cuando la inflamación es localizada y discreta y no existe compromiso sistémico, el objetivo principal del tratamiento endodóntico es lograr el drenaje, no siendo necesarios los antibióticos. El tratamiento antibiótico sólo está indicado en los siguientes casos: 1) Pacientes inmunodeprimidos; 2) Absceso apical agudo con sintomatología sistémica (fiebre, adenopatías...); 3) Absceso api-cal agudo que progresa y se expande, como es el caso de las celulitis cérvico-faciales; 4) Reimplante de dientes avulsionados. El antibiótico de elección es la amoxicilina, con / sin ácido clavulánico, y la clindamicina en pacientes alérgicos a las penicilinas. La mejoría clínica debe ser la guía para la duración del tratamiento (3-5 días), y cuando exista evidencia de que los signos y síntomas han remitido, el tratamiento antibiótico debe interrumpirse. Respecto a la profilaxis antibiótica en endodoncia, debe considerarse en los siguientes casos: 1) Pacientes inmunodeprimidos; 2) Pacientes con riesgo de desarrollar endocarditis bacteriana; 3) Pacientes portadores de prótesis articulares; 4) Pacientes oncológicos sometidos a radioterapia; 5) Pacientes en tratamiento con bisfosfonatos por vía intravenosa. En estos casos la pauta será amoxicilina por vía oral, 2 gr en adultos o 50 mg / kg en niños, administrada 1 hora antes de la intervención

Desde la introducción del término revascularización en 1971, muchos han sido los protocolos descritos para el tratamiento de un diente inmaduro con necrosis pulpar y periodontitis apical. La complejidad que supone la realización de técnicas de apicoformación en dientes con raíces cortas, paredes frágiles, ápices no formados y divergentes, permiten que el tratamiento de endodoncia regenerativa esté indicado. El caso que presentamos a continuación se trata de un paciente de corta edad que acude a la consulta por presentar dolor en el cuarto cuadrante. Tras la anamnesis y pruebas complementarias, llegamos al diagnóstico de necrosis pulpar con periodontitis apical sintomática del diente 44. En la radiografía podemos apreciar la presencia de una raíz corta, con paredes frágiles y ápice no formado. Tras hablar con los padres del paciente decidimos optar por el tratamiento de endodoncia regenerativa. Discutiremos a lo largo del caso la técnica empleada durante la realización de este caso clínico, centrándonos en la desinfección del interior del sistema de conductos, y la posterior creación de una matriz de andamiaje necesaria para poder colocar nuestro material biocerámico. En el apartado de discusión acercaremos al clínico las distintas opciones presentes en la literatura para afrontar la desinfección, medicación intraconducto y material de sellado, pilares fundamentales para el éxito del tratamiento de endodoncia regenerativa. En el seguimiento del caso, el paciente se presenta totalmente asintomático, con un desarrollo radicular completo y una formación de las paredes que devuelven la integridad al diente inmaduro


Since the introduction of the term revascularization in 1971, there have been many specific protocols for the treatment of an immature tooth with pulp necrosis and apical periodontitis. The complexity of performing apexification techniques on teeth with short roots, fragile walls, and non-formed and divergent apices, allows regenerative endodontic treatment to be indicated. The case presented below is about a young patient who comes to the office for presenting pain in the fourth quadrant. After the anamnesis and complementary tests, we arrived at the diagnosis of pulp necrosis with symptomatic apical periodontitis of tooth 44. On radiography, we can detect the presence of a short root, with fragile walls and an unformed apex. After talking with the patient's parents, we decided to opt for regenerative endodontic treatment. We will discuss throughout the case the technique used during the realization of this clinical case, focusing on the disinfection of the interior of the duct system, and the subsequent creation of a scaffolding matrix necessary to be able to place our bioceramic material. In the discussion section we will approach the clinician the different options present in the literature to deal with disinfection, intra-conduction medication and sealing material, fundamental pillars for the success of regenerative endodontic treatment. In the follow-up of the case, the patient appears totally asymptomatic, with a complete root development and a formation of the walls that restore integrity to the immature tooth

BACKGROUND: Titanium-prepared platelet rich fibrin (T-PRF) is an autologous hemo-component with a high con-centration of platelets that also incorporates leukocytes, and growth factors into the dense fibrin matrix and can be used as a healing biomaterial. This study assesses the adjunctive use of T-PRF in intrabony defects (IBDs) with open flap debridement (OFD) in comparison with guided tissue regeneration (GTR) as a gold standard and OFD alone as a control. MATERIAL AND METHODS: A total of 45 patients (15 per group) were randomized as either T-PRF (test group), GTR (test group), or OFD alone (control group) sites. Probing depth (PD), clinical attachment level (CAL), and IBD were recorded. The radiographic depth of IBD was also measured. Primary outcomes assessed were changes in PD, CAL, and radiographic IBD that were assessed at the beginning and nine months later. RESULTS: The PRF and GTR group showed significant improvement in clinical parameters compared with the OFD alone (control group) at nine months. While there were no significant differences in PD and CAL between test groups (T-PRF and GTR groups), the significant difference was found in radiographic IBD depth. CONCLUSION: T-PRF may give similar successful results as GTR in the treatment of IBDs with endo-perio lesions

Background: The main cause of endodontic failure is the persistence of microorganisms that cause an intraradicular or extratradicular infection and that become resistant to disinfection measures. The objective of this review is to identify the microbiota associated with endodontic failure, as well as the reasons why these microorganisms are capable of surviving basic disinfection measures. Material and Methods: Systematic search of scientific articles in the databases PubMed with the following key-words "Endodontic Infections", "Endodontic Microbiology, "Endodontic Failure", "Enterococcus Faecalis", "Endodontics Retreatment" was carried out. Case reports and articles with publication date prior to 2000 were not included in this review. Results: Most authors highlight E. faecalis as the main microorganism associated with endodontic failure, nevertheless there are recent studies that isolate, to a greater extent, other bacteria such as Fusobacterium nucleatum and Propioni bacterium. Discussion: These microorganisms have in common the following proprieties, which make them able to escape the disinfection measures: the ability to form a biofilm, to locate in areas unreachable to root canal instrumentation techniques, synergism, the ability to express survival genes and activate alternative metabolic pathways

La endodoncia requiere de una técnica y sistemática depurada, en todas sus partes: remoción y desinfección total del proceso carioso; detección del número y entrada conductual; determinación de longitud de trabajo y preparación biomecánica óptima y necesaria y su posterior obturación efectiva con sellado hermético. En la constante búsqueda de mayor calidad en el tratamiento endodóntico, el microscopio operatorio ha demostrado comportarse como un perfecto aliado del dentista con práctica de endodoncia, pasando de ser un instrumento complementario o auxiliar a convertirse en el coprotagonista del éxito de un tratamiento de conductos. Gracias a proporcionar al endodoncista mejores condiciones de trabajo, una magnificación visual de hasta 40 ×, un aumento en la iluminación del campo operatorio, además de permitir el registro informático de casos clínicos. El objetivo de este trabajo es, mediante la revisión de diferentes artículos, poder resaltar los beneficios que presenta el uso de técnicas de magnificación en la práctica de la endodoncia, además de comparar las diferentes técnicas existentes hoy en día, y señalar las ventajas del microscopio operatorio frente a las lupas convencionales


Endodontics requires a clean technique and systematic, in all its parts: removal and total disinfection of the carious process; Number detection and behavioral input; Determination of working length and optimal and necessary biomechanical preparation and its subsequent effective sealing with hermetic seal. In the constant search for better quality in endodontic treatment, the operating microscope has been shown to be a perfect ally of the dentist with endodontic practice, going from being a complementary or auxiliary instrument to becoming the co-protagonist of the success of a root canal treatment. Thanks to providing the endodontist with better working conditions, a visual magnification of up to 40 ×, an increase in the illumination of the operative field, besides allowing the computer registry of clinical cases. The objective of this work is, through the review of different articles, to highlight the benefits of the use of magnification techniques in the practice of endodontics, in addition to comparing the different techniques existing today, and to point out the advantages of the microscope Operative compared to conventional magnifiers

Objetivo. El propósito de este trabajo es revisar el estado de la anestesia local odontológica en los pacientes con pulpitis aguda en los dientes posteriores mandibulares, para conocer los métodos más eficaces para lograr su anestesia antes del tratamiento endodóntico. Material y método. Se ha realizado una búsqueda bibliográfica de los ensayos clínicos sobre la eficacia de bloqueos mandibulares en pulpitis agudas de molares y premolares inferiores publicados desde el año 1987 hasta 2015. Resultados. La administración de un cartucho de 1,8 mL de la solución estándar de Lidocaína 2% con Adrenalina 1:100.000 (L- 100) o 1:80.000 (L-80), empleando la técnica de bloqueo mandibular, solo es efectiva en el 30% de los casos. En el caso de administrar dos cartuchos anestésicos, la eficacia aumenta al 45%. Si se administra un cartucho de 1,8 mL de Articaína 4% con adrenalina 1:100.000 (A-100) la eficacia es del 40% y, en el caso de administrar dos cartuchos, la eficacia aumenta hasta un 70%. Si se prescribe un antiinflamatorio no esteroideo (AINE) al paciente una hora antes de la intervención y se administra un cartucho de 1,8 mL de solución estándar L-80 o L-100, el porcentaje de casos con éxito alcanza el 55%. En el caso de prescribir paracetamol una hora antes, la eficacia se reduce al 50%. Si tras haber realizado el bloqueo mandibular con L-80 o L-100 se hace un refuerzo vestibular con otros 1,8 mL de A-100, la eficacia es del 65% y si se realiza un bloqueo mandibular de 1,8 mL de A-100 y refuerzo vestibular con otros 1,8 mL de A-100 la eficacia es del 85%. Conclusiones. Se recomienda la toma de AINES una hora antes de la intervención, efectuar un bloqueo mandibular con 3,6 mL deA-100 y refuerzo por vestibular con 1,8 mL deA-100 (AU)


Aim. The purpose of this paper is to review the state of the dental local anesthesia in patients with acute irreversible pulpitis in mandibular posterior teeth, to find out the most effective methods to achieve anesthesia prior to the endodontic treatment. Materials and methods. A literature review was conducted searching for clinical trials on the efficacy of the mandibular block technique in lower molars and premolars with acute irreversible pulpitis, published from 1987 to 2015. Results. The administration of one cartridge of 1,8 mL standard solution of Lidocaine 2% with Adrenalin 1:100.000 (L-100) or 1:80.000 (L-80) with the mandibular block technique is only effective in 30% of the cases. If administering two cartridges, the effectiveness increases to 45%. One cartridge of 1,8 mL Articaine 4% with Adrenalin 1:100.000 (A-100) is effective in 40% of the cases, increasing to 70% when adding a second cartridge. If prescribing a nonsteroidal anti-inflammatory drug (NSAID) one hour before the treatment and injecting one cartridge of 1,8 mL standard solution L-80 or L-100, the percentage of successfully anesthetized cases reaches 55%. In case of prescribing acetaminophen one hour before treatment, the efficacy is reduced to 50%. If following the mandibular block technique with L-80 or L-100, a buccal reinforcement is made with 1,8 mL A-100, the effectiveness is 65%. If such blockage is made with 1,8 mL of A-100 and the buccal reinforcement with 1,8 mL A-100, the effectiveness is 85%. Conclusion. The recommendations to achieve the highest effectiveness of anesthesia are taking NSAIDs one hour before treatment and performing a mandibular block technique with 3,6 mL of A-100 and buccal reinforcement with 1,8 mL of A- 100 (AU)

Objetivo: Comparar el porcentaje de área de gutapercha, cemento y huecos en conductos curvos obturados con sistemas con vástago y evaluar la influencia de la activación del cemento durante su colocación. Metodología: 120 conductos curvos de molares mandibulares fueron preparados a un diámetro apical ProFile 30, 0.04. Se obturaron con los sistemas GuttaCore, GuttaFusion o condensación lateral y AHplus (n = 40). Se subdividieron los grupos (n = 20) y se colocó el cemento con activación sónica o no. El cemento se mezcló con Rodamina B para permitir el análisis mediante microscopía confocal. Se calculó el área de gutapercha, cemento y huecos a 3, 6 y 9 mm del ápice. Se realizó el análisis estadístico utilizando ANOVA de dos vías. Para la comparación por pares se realizó el test de Sidak. Resultados: Cuando se comparan los sistemas de gutapercha a 3 mm, GF mostró un mejor comportamiento. CL obtuvo los peores resultados. No se encontró influencia entre los sistemas de obturación y la activación del cemento. Cuando se comparó el porcentaje de huecos entre todos los grupos, los perores resultados fueron para CL a 6 y 9 mm. Conclusiones: Bajo las condiciones de este estudio, el área de gutepercha, cemento y huecos depende del sistema de obturación utilizado. El uso de activación del cemento no influye en la calidad de la obturación


Aim: To compare the percentage of gutta-percha, sealer and voids in curved root canals filled with different gutta-percha carrier-based systems and to assess the influence of activation of the cement during its placement. Methodology: 120 curved canals of mandibular molars were prepared to an apical size ProFile 30, 0.04. They were filled with either GuttaCore, GuttaFusion or lateral condensation and AHplus (n=40). Groups were subdivided (n=20) and sonic activation or not during the placement of the cement was carried out. Cements were labelled with Rhodamine-B dye to allow analysis under confocal microscopy. Percentage of gutta-percha, sealer and voids area were calculated at 3, 6 and 9 mm from the apex. Statistical analysis was performed using two-way ANOVA test. To compare between the obturation systems Sidak correction for multiple comparisons was applied. Results: When comparing the gutta-percha systems at the 3 mm level, GF revealed a better performance. At 6 and 9mm, LC obtained the worst results. An influence between the obturation systems and the activation of the cement was not found. When comparing the percentage of voids among all the groups, worst results were found for LC at 6 and 9 mm. Conclusions: Under the conditions of this study, gutta-percha, sealer area and voids were dependent on the obturation system. The use of activation of the cement do not influence the quality of root filling

La efectividad antimicrobiana de la terapia fotodinámica (TFD), como complemento a la preparación biomecánica, depende del íntimo contacto entre un agente fotosensible, una fuente de energía (LED) y los microorganismos presentes en el conducto. Objetivo. Evaluar si la eliminación del barrillo dentinario influye en la penetración del agente fotosensibilizante dentro de los túbulos dentinarios en la TFD. Metodología. 20 premolares con conductos tipo I fueron decoronados a 12 mm del ápicy preparados biomecánicamente con MTwo y NaOCL al 5,25%. La muestra se dividió aleatoriamente en dos grupos (ni = 10): I, se irrigó con EDTA 17% seguido de suero fisiológico; II, se irrigó sólo con suero fisiológico. Tras la aplicación de la TFD, los especímenes fueron seccionados, obteniéndose discos correspondientes a los tercios coronal, medio y apical. Las muestras fueron visualizadas con un microscopio óptico de reflexión y el software Canvas X16. Se evaluaron el porcentaje de área teñida (PAT) y la penetración intratubular media (PIM). Análisis estadístico: Tests U de Mann-Whitney y Wilcoxon con un nivel de significación de p<0,05. Resultados. Los resultados de PAT y PIM fueron mayores en el grupo I (11,1 ± 5,1/0,45 ± 0,18) que en el II (4,93 ± 4,1/0,26 ± 0,16). No hubo diferencias significativas de PIM en los tercios coronales de I vs. II. Ambos grupos presentaron diferencias de PAT y PIM entre sus tercios. Conclusión. La remoción del barrillo dentinario previa a la aplicación de la TFD mejora significativamente la penetración del agente fotosensible en los tres tercios de la raíz


Antimicrobial efectiveness of the photodynamic therapy (PDT), in addition to chemicomechanical preparation, depends on the contact between a photosensitizer, a light source (LED) and root canal microorganisms. Aim. To evaluate if the removal of smear layer influences the penetration of the photosensitizer into dentinal tubules in the PDT. Methodology. 20 premolars with Type I canals were sectioned at 12 mm from apex and conformed with MTwo and 5.25% NaOCl. Were randomized divided in two groups (ni = 10): group I was irrigated with 17% EDTA and 0.9% NaCl; group II was irrigated with 0.9% NaOCl. For the PDT 0.01% of TBO (Fotosan® ,CMS) as photosensitizer was applied into the canal and activated for 30 sec with a LED red light source (Foto-san630®, CMS). After this, all specimens were sectioned in order to obtain dentin discs of coronal, medium and apical third. Samples were evaluated with an optical microscope at 20x and the software Canvas X16, measuring the percentage of stained area (PAT) and Intratubular media penetration (PIM). Statistical analysis: Test U de Mann-Whitney y Wilcoxon with a significance level of p < 0.05. Results. The results of PAT and PIM were higher in Group I (11,1 ± 5,1/0,45 ± 0,18) than Group II (4,93 ± 4,1/0,26 ± 0,16). No significant differences of PIM were founded between coronal thirds of I vs II. Both groups presented differences of PAT and PIM between their thirds. Conclusion. The removal of smear layer before the TFD application improves the penetration of the photosensitizer in the three root thirds

Objetivo. Comparar la modificación de conductos simulados con doble curvatura después de su preparación con 4 sistemas de lima única. Material y métodos. Se usaron 40 bloques de resina con conductos simulados de doble curvatura y divididos en 4 grupos (ni=10). Los bloques fueron preparados hasta una lima 25 usando los sistemas Reciproc (R25), Wave One Gold (Primary), One Shape y F360 (25). Se obtuvieron imágenes pre- y post-instrumentación y se superpusieron para su análisis. Se dividió la doble curvatura en 11 niveles. Se calculó la distancia desde la pared no instrumentada hasta la instrumentada. Para comparar el ensanchamiento del conducto en cada nivel se utilizó el test ANOVA, las comparaciones dos a dos se hicieron mediante el test de Tukey. El nivel de significación se estableció en p<0,05. Resultados. Todos los sistemas de lima única estudiados modifican de manera importante la curvatura apical. Wave One Gold ensanchó más en la pared interna de la curvatura apical, manteniendo un ensanchamiento más proporcional en la curvatura coronal; mientras que F360 removió más material desviando el conducto hacia la parte interna en ambas curvaturas, siendo estas diferencias estadísticamente significativas. Conclusiones. La morfología del conducto, tras instrumentación, se mantiene más fiel a la forma inicial en los sistemas Wave One Gold y One Shape


Objective. To compare the modification of simulated double curvature root canals after preparation with four single file systems. Material and methods. Forty resin blocks with simulated double curvature canals were divided into four groups (n=10), respectively instrumented to file size 25 using the Reciproc (R25), Wave One Gold (Primary), One Shape and F360 systems (25). Pre- and post-instrumentation images were obtained and overlapped for analysis. The double curvature was divided into 11 levels. The distance from the non-instrumented to the instrumented wall was calculated. Comparison of canal widening at each level was made using analysis of variance, with the Tukey test for two-by-two comparisons. Statistical significance was considered for p<0.05. Results. All the studied single file systems produced important modifications of the apical curvature. Wave One Gold resulted in greater internal wall widening in the apical curvature, with more proportional widening in the coronal curvature. In comparison, the F360 system removed more material, deviating the canal towards the internal portion in both curvatures - the differences being statistically significant. Conclusions. Canal morphology after instrumentation was seen to more closely reproduce the initial morphology when using the Wave One Gold and One Shape systems

Paciente mujer, de 19 años de edad, se presenta de urgencia a consecuencia de un traumatismo dental. El paciente muestra un sangrado en la encía sulcular del incisivo central superior derecho. La inspección del paciente, muestra un extrusion oclusal y lingual de la corona. Ausencia de movilidad dental. La radiografía periapical muestra una fractura horizontal del incisivo central superior derecho. El test de sensibilidad pulpar provoca una respuesta positiva tanto en este diente como en los incisivos superiores. El tratamiento inicial de reposición se ve dificultado por la impactación del fragmento coronal radicular sobre la cortical externa alveolar. Se realiza un colgajo de espesor total para el acceso a la tabla cortical vestibular, donde se aprecia una fisura cortical de la tabla externa; se realiza una osteotomía a nivel de la línea de fractura al objeto de liberar la impactación radicular y permitir la reposición de ambos fragmentos radiculares del incisivo central superior derecho. Se establece una ferulización mediante un material de resina compuesta, entre las coronas clínicas del incisivo central y el lateral superior derecho, que se mantiene durante cuatro semanas. Una vez retirada la férula interproximal, la vitalidad del fragmento radicular coronal permanece con sensibilidad positiva a la prueba de estimulación con dióxido de carbono. Por ello, la decisión terapéutica consiste en el mantenimiento pulpar. El diente se mantiene en un programa de revisión. Presentamos el seguimiento de este incisivo central superior derecho, a dos años, 21 años y 30 años, del traumatismo y su reposición. En todas estas revisiones, la repuesta obtenida mediante los test de sensibilidad pulpar son positivas. La interpretación radiográfica periapical en 2-D, así como las observaciones de las imágenes CBCT, muestran el modelo de reparación entre ambos fragmentos radiculares con aparentemente mediante tejido conectivo, la ausencia de patología periapical del fragmento apical, la obliteración parcial pulpar de ambos fragmentos en la revisión de los 30 años, el trayecto irregular y cervical de la línea de fractura y a presencia de la tabla cortical externa vestibular


A 19-year-old female patient came in for emergency treatment as a result of dental trauma. The patient displayed sulcular bleeding in the gum of the upper right central incisor. Inspection of the patient revealed an occlusal and lingual displacement of the crown and an absence of dental mobility. The periapical radiograph showed a horizontal fracture of the upper right central incisor. The pulp sensitivity test caused a positive response in this tooth as well as in the upper incisors. The initial repositioning treatment was difficult due to the impaction of the coronal root fragment on the external alveolar cortical bone. A full-thickness flap was made in order to access the vestibular cortical plate, where a cortical fissure of the outer table could be seen; an osteotomy was performed at the level of the fracture line to release the root impaction and allow repositioning of both root fragments of the upper right central incisor. A splint was established using a composite resin material between the clinical crown of the central incisor and the upper right side, which was maintained for four weeks. Once the interproximal splint had been removed, the vitality of the coronal root fragment showed positive sensitivity with regard to the carbon dioxide stimulation test. For that reason, the therapeutic decision consisted of pulp maintenance. A review program was established for the tooth. We present the follow up of this upper right central incisor, at 2 years, 21 years and 30 years from the trauma and repositioning. In all these revisions, there was a positive response to the pulp sensitivity test. The interpretation of he 2D periapical x-ray, as well as the observations of CBCT images, show the repair between the two root fragments by means of connective tissue, the absence of periapical pathology of the apical fragment, the partial pulp obliteration of both fragments in the 30-year review, the irregular and cervical trajectory of the fracture line and the presence of the external vestibular cortical tablet

Objetivo: Determinar la prevalencia de periodontitis apical (PA), tratamiento endodóncico (TE) y estado de salud oral en individuos con diferentes condiciones sistémicas y hábitos de tabaquismo. Métodos: se evaluaron las radiografías panorámicas y procesos clínicos de la primera consulta de 199 pacientes en la Facultad de Medicina Dentaria de la Universidad de Porto. Se registraron el número y presencia de PA y TE, el estado de salud oral (número de dientes ausentes, caries y raíces) y la presencia de diabetes mellitus (DM), enfermedades cardiovasculares (ECV), alergias y hábitos de tabaquismo en cada individuo. Los dientes fueron evaluados de acuerdo con el Índice Periapical de Ørstavik. Los datos fueron analizados por medio de estadística inferencial, considerando un nivel de significancia de 5%. Resultados: La prevalencia de PA y TE por individuo fue de 44,2% (IC 95%, 37,3%-51,4%) y 47,2% (IC 95%, 40,2%-54,4%), respectivamente. Se verificó una asociación entre el número de PA (≥3) e individuos fumadores (p = 0,025). Se detectó que el número de dientes ausentes aumentaba con la edad (r = 0,723; p < 0,005) y era mayor en individuos diabéticos (p = 0,000). Un mayor número de dientes perdidos, caries y raíces fue detectado en los individuos con AP (p = 0,02, p = 0,000, p = 0,000, respectivamente) y con ECV (p = 0,000, p = 0,001, p = 0,040, respectivamente). Conclusiones: La PA y el TE eran altamente prevalentes en la populación estudiada, independientemente de la condición sistémica o hábitos de tabaquismo del paciente. Se verificó que una mayor prevalencia del número de dientes ausentes, caries y raíces estaba asociada a ciertas condiciones sistémicas, lo que sugiere una mayor susceptibilidad de estos individuos a desarrollar lesión endodóncica primaria


Objective: To determine the prevalence of apical periodontitis (AP), endodontic treatment (ET), and oral status in patients with different systemic conditions and smoking habits. Methods: Panoramic radiographs and clinical processes of 199 patients from their first visit to the clinic of the Faculty of Dentistry of the University of Porto were assessed. The number and presence of AP and ET, the oral status (number of missing teeth, caries, and roots), and the presence of diabetes mellitus (DM), cardiovascular diseases (CVD), allergies, and smoking habits were registered. Teeth were assessed according to Ørstavik’s periapical index. Data were analyzed using inferential statistics, based on a significance level of 5%. Results: The prevalence of AP and ET per patient was 44.2% (95% CI, 37.3%-51.4%) and 47.2% (95% CI, 40.2%-54.4%), respectively. An AP ≥ 3 was associated with smokers (p = 0.025). The number of missing teeth increased with age (r = 0.723; p = 0.005) and was higher in patients with DM (p = 0.000). A higher number of missing teeth, caries, and roots was detected in patients with AP (p = 0.002, p = 0.000, p = 0.000, respectively) and patients with CVD (p = 0,000, p = 0,001, p = 0.040, respectively). Conclusions: AP and ET were highly prevalent in the studied population, regardless of the patient’s systemic condition and smoking habit. A higher prevalence of missing teeth, caries, and roots was associated with certain systemic conditions, thus suggesting a higher susceptibility of these patients to develop primary endodontic disease
