Objetivos: Recuperar la capacidad de deambulación con prótesis en los amputados de extremidad inferior es fundamental para mejorar su independencia funcional. El objetivo de este trabajo ha sido conocer qué factores intervienen en conseguir la protetización de pacientes amputados vasculares. Material y método: Estudio longitudinal observacional de los pacientes con amputación mayor de extremidad inferior de etiología vascular realizada desde el 1 de abril de 2017 hasta el 1 de abril de 2020. Se compararon las siguientes variables entre los pacientes que fueron protetizados y los que no: edad, sexo, índice de masa corporal, comorbilidades (escala de Charlson), independencia en las actividades de la vida diaria básicas (índice de Barthel) y capacidad de marcha (FAC) previas y al año de la amputación. La utilización de la prótesis al año se midió con el instrumento de Houghton. Resultados: Fueron amputados 80 pacientes con una edad media de 70,5 años, el 78,8% eran varones. El nivel de amputación fue supracondíleo en 42 pacientes y en 38 infracondíleo; se protetizaron 35. Las variables relacionadas con la posibilidad de protetización fueron la menor edad (p=0,020), presentar menos comorbilidad (p=0,00), la amputación infracondílea (p=0,024) y tener una mayor independencia funcional y de marcha previas a la amputación (p=0,00). Al año habían fallecido 22 pacientes, solo uno de los que fueron protetizados. Conclusión: Aunque no existen unas recomendaciones claras para determinar qué amputado ha de ser protetizado, en nuestros pacientes, tener menos comorbilidades, una buena situación funcional previa, menor edad y un nivel de amputación infracondíleo se relacionaron con un mayor éxito en conseguir dicho objetivo.(AU)
Objective: Restoring the ambulation ability with prostheses in lower limb amputeesis essential to improve their functional independence. The aim of this study was to determine the factors involved in achieving prosthesis fitting in vascular amputees. Material and method: Observational longitudinal study of patients with major lower limb amputation of vascular etiology performed from April 1st 2017 to April 1st 2020. The following variables were compared between the group of patients who were prosthetized and those who were not: age, gender, body-mass index, comorbidity (Charlson index), independence in the activities of daily living (Barthel index) and ambulation ability (FAC test) before and a year after the amputation. The prosthetic use after a year was measured with the Houghton scale. Results: A total of 80 patients were amputated with a mean age of 70.5 years old, 78.8% were male. The amputation level was supracondilealin 42 patients and infracondilealin 38 patients. The number of prosthetized patients was 35. The variables related to the possibility of prosthesis fitting were: younger age (P=0.020), less comorbidity (P=0.000), infracondileal amputation (P=0.024) and greater functional independence and ambulation ability prior to amputation (P=0.000). After a year 22 patients had died, only one of those who had been prosthetized. Conclusions: Although there are no clear recommendations to determine which amputees should be prosthetic fitting, in our patients the presence of fewer comorbidities and a good previous functional situation, younger age and infracondileal amputation, are related to greater success in achieving this objective.(AU)

Antecedentes y objetivos: Los dispositivos auditivos osteointegrados utilizan la transmisión del sonido por vía ósea para producir una mejoría auditiva. Los mecanismos y factores que intervienen en esta transmisión han sido ampliamente estudiados, sin embargo, existen otros aspectos que no conocemos, por ejemplo, la influencia que tiene la potencia de salida del procesador. El objetivo principal de este estudio fue conocer si existe alguna relación entre la potencia que emiten estos dispositivos y la mejoría auditiva que producen. Materiales y métodos: Hemos realizado un estudio en 44 pacientes portadores de un Baha® 5 percutáneo. De cada paciente se obtuvieron los umbrales de vía aérea y ósea en audiometría tonal liminar, en audiometría en campo libre, y en audiometría verbal en silencio y con ruido de fondo, tanto previa como posteriormente a la implantación. También se recogieron los umbrales de conducción ósea directa a través del procesador y los valores de ganancia en la potencia de salida del procesador. (AU)
Background and objective: Osseointegrated auditory devices are hearing gadgets that use the bone conduction of sound to produce hearing improvement. The mechanisms and factors that contribute to this sound transmission have been widely studied, however, there are other aspects that remain unknown, for instance, the influence of the processor power output. The aim of this study was to know if there is any relationship between the power output created by the devices and the hearing improvement that they achieve. Materials and methods: Forty-four patients were implanted with a percutaneous Baha® 5 model. Hearing thresholds in pure tone audiometry, free-field audiometry, and speech recognition (in quiet and in noise) were measured pre and postoperatively in each patient. The direct bone conduction thresholds and the power output values from the processors were also obtained. (AU)

OBJETIVO: Los implantes óseos son utilizados cada vez con mayor frecuencia en la práctica clínica y, entre los materiales, el Ti o sus aleaciones son los de mejor rendimiento por sus propiedades fisicoquímicas. Aleaciones como TiNbTa han demostrado mejorar las características biomecánicas del Ti puro comercial (c.p.), sin embargo, su capacidad osteointegradora necesita ser evaluada. El objetivo del presente estudio fue valorar la citotoxicidad y la capacidad de adhesión, proliferación y diferenciación de células osteoblásticas en cultivo, influida por discos de material TiNbTa frente a Ti c.p. MATERIALES Y MÉTODOS: Analizamos a los 4 y 7 días del cultivo la línea celular MC3T3, la viabilidad celular (AlamarBlue Cell Viability Reagent. Invitrogen, España), así como la proliferación y diferenciación celular (actividad de fosfatasa alcalina (ALP) y microscopía electrónica de barrido (Fijación para SEM). Se realizó la prueba t de Student para determinar diferencias estadísticamente significativas entre los dos grupos de discos de estudio. RESULTADOS: Los resultados obtenidos demuestran muy buena viabilidad celular durante el periodo de estudio, sin diferencias significativas para ambos materiales. Así mismo, detectamos una caída en los niveles de ALP que fue significativa para ambos componentes entre los días 4 y 7 del estudio (p < 0,05). Las imágenes de microscopía electrónica revelaron buena capacidad de adhesión al material, así como diferenciación celular frente a ambos tipos de discos. CONCLUSIONES: La aleación de TiNbTa como material para implantes óseos cuenta con una buena capacidad osteointegradora, además de resolver problemas de biomecánica que presenta el titanio puro como componente
OBJETIVE: Bone implants are increasingly used in clinical practice and, among the materials, Ti or its alloys are offer the best performance given their physicochemical properties. Alloys such as TiNbTa have been shown to improve the biomechanical characteristics of commercial pure Ti (c.p.), however, its osseointegration capacity needs to be evaluated. The objective of the present study was to assess the cytotoxicity and the adhesion, proliferation and differentiation capacity of osteoblastic cells in culture, influenced by discs of TiNbTa material versus Ti c.p. MATERIAL AND METHODS: At 4 and 7 days after culture, we analyzed the MC3T3 cell line, cell viability (AlamarBlue Cell Viability Reagent. Invitrogen, Spain), as well as cell proliferation and differentiation (alkaline phosphatase activity (ALP) and scanning electron microscopy (Fixation for SEM) Student's t test was performed to determine statistically significant differences between the two groups of study discs. RESULTS: The results obtained show very good cell viability during the study period, with no significant differences for both materials. Likewise, we detected a drop in ALP levels that was significant for both components between days 4 and 7 of the study (p < 0.05). Electron microscopy images revealed good adhesion capacity to the material, as well as cell differentiation against both types of discs. CONCLUSIONS: The TiNbTa alloy as a material for bone implants offers good osseointegrative capacity, in addition to solving biomechanical problems that pure titanium presents as a component

BACKGROUND: Modification of endosteal implants through surface treatments have been investigated to improve osseointegration. Boronization has demonstrated favorable mechanical properties, but limited studies have assessed translational, in vivo outcomes. This study investigated the effect of implant surface boronization on bone healing. MATERIAL AND METHODS: Two implant surface roughness profiles (acid etched, machined) in CP titanium (type II) alloy implants were boronized by solid-state diffusion until 10-15 µm boron coating was achieved. The surface-treated implants were placed bilaterally into 5 adult sheep ilia for three and six weeks. Four implant groups were tested: boronized machined (BM), boronized acid-etched (BAA), control machined (CM), and control acid-etched (CAA). Osseointegration was quantified by calculating bone to implant contact (BIC) and bone area fraction occupancy (BAFO). RESULTS: Both implant types treated with boronization had BIC values not statistically different from machined control implants at t=3 weeks, and significantly less than acid-etched control (p < 0.02). BAFO values were not statistically different for all 3-week groups except machined control (significantly less at p < 0.02). BAFO had a significant downward trend from 3 to 6 weeks in both boronized implant types (p < 0.03) while both control implant types had significant increases in BIC and BAFO from 3 to 6 weeks. CONCLUSIONS: Non-decalcified histology depicted intramembranous-like healing/remodeling in bone for controls, but an absence of this dynamic process in bone for boronized implants. These findings are inconsistent with in vitro work describing bone regenerative properties of elemental Boron and suggests that effects of boron on in vivobone healing warrant further investigation

Los dispositivos auditivos osteointegrados (DAO) Power y SuperPower presentan indicaciones para umbrales en vía ósea de 55 y 65dB, respectivamente. Realizamos un estudio observacional prospectivo de una serie de 6 casos con hipoacusia mixta en los que se realizó implantación del DAO Ponto(TM) SuperPower. Se realizaron previo a la implantación y 6 meses tras la adaptación valoraciones tonales y verbales sin y con ruido de fondo (HINTS). Todos los participantes mostraron mejoría en los resultados tonales y verbales, variando según el grado de hipoacusia contralateral. Cabe destacar los resultados verbales con ruido de fondo donde la mayoría de los pacientes obtuvieron una relación señal ruido entre 2 y 4 dB. Los resultados en los cuestionarios APHAB y GBI mostraron una reducción de la percepción del problema y una mejoría en la calidad de vida, respectivamente. Los resultados presentados reflejan la posibilidad de tratamiento de hipoacusias mixtas con el dispositivo DAO Ponto(TM) SuperPower
Osseointegrated hearing devices Power and SuperPower present indications for bone thresholds of 55 and 65 dB respectively. We conducted a prospective observational study of a series of six cases with mixed hearing loss for whom implantation of the DAO Ponto(TM) Super-Power was performed. Tonal and verbal evaluations without and with background noise (HINTS) were performed prior to implantation and six months after adaptation. All the participants showed improvement in tonal and verbal results, varying according to the degree of contralateral hearing loss. The verbal results with background noise were noteworthy, where most of the patients obtained a signal-to-noise ratio between 2 and 4 dB. The results in the APHAB and GBI questionnaires showed a reduction in the perception of the problem and an improvement in quality of life respectively. The results presented reflect the possibility of treatment of mixed hearing loss with the Ponto(TM) SuperPower device

OBJETIVO: Determinar la influencia de la vitamina D en la osteointegración de los implantes dentales. MATERIAL Y MÉTODOS: Se realiza una revisión de la bibliografía en las bases de datos online Pubmed, Cochrane y Google Académico sin restricción de fecha o idioma. RESULTADOS: Se seleccionaron un total de 25 artículos, tratándose en su mayoría de estudios de investigación en animales o estudios "in vitro". Tan sólo 4 de los artículos seleccionados son de investigación en humanos. Discusión: La vitamina D es una molécula liposoluble derivada del colesterol, es de gran importancia en el metabolismo óseo favoreciendo su mineralización. Los estudios al respecto, parecen indicar que los niveles de vitamina D suponen una importante influencia sobre el proceso de osteointegración, actuando en el metabolismo óseo y en la respuesta inmune periimplantaria. CONCLUSIONES: Estudios experimentales en animales han demostrado que existe una relación directa entre el déficit de vitamina D y las alteraciones en la osteointegración de los implantes en fases tempranas. En humanos también se asocia el déficit de dicha vitamina con el fracaso temprano de los implantes dentales, sin embargo, esta afirmación no ha podido ser aún demostrada. Por otro lado, se ha relacionado el uso de suplementos vitamínicos en personas con déficit de vitamina D con la supervivencia de los implantes dentales en este tipo de pacientes, así como también se ha observado que los niveles de vitamina D están relacionados con la aparición y avance de enfermedades periimplantarias
OBJECTIVE: To determine the infuence of vitamin D on the osseo integration of dental implants. MATERIAL AND METHODS: A comprehensive search of the literature databases (Pubmed, Cochrane and Google Scholar), along with cross-referencing published peer-reviewed articles, was conducted. The search include full text without restriction of date or language. RESULTS: A total of 25 articles were selected from the reviewed articles, most of them being animal research studies or "in vitro" studies. Only 4 of the selected articles are from human research. Discusión: Vitamin D is a fat-soluble molecule derived from cholesterol, is of great importance in bone metabolism, favoring bone mineralization. Studies in this regard seem to indicate that vitamin D levels are an important infuence on the osseointegration process, infuencing bone metabolism and peri-implant immune response. CONCLUSIONS: Experimental studies in animals have shown that there is a direct relationship between vitamin D defciency and alterations in the osseointegration of implants in early stages. In humans, the defcit of this vitamin is also associated with the early failure of dental implants, however this statement has not yet been proven. On the other hand, the use of vitamin supplements in people with vitamin D defciency has been related to the survival of dental implants in this type of patients, as well as it has been observed that vitamin D levels are related to the appearance and progress of peri-implant diseases
