El proceso editorial de las revistas científicas es complejo pero esencial para la diseminación del conocimiento científico. La calidad del proceso reside en los autores, editores y revisores, quienes deben tener la responsabilidad, experiencia y el conocimiento necesario para garantizar la calidad de los artículos publicados. Uno de los retos más importantes que enfrentan las revistas científicas en la actualidad es la revisión por pares de los manuscritos. Los editores son responsables de coordinar y supervisar todo el proceso editorial, desde la recepción del manuscrito hasta la publicación final, y asegurar que los artículos cumplan con los estándares éticos y de integridad científica. Además, los editores tienen la responsabilidad de seleccionar correctamente a los revisores. Sin embargo, esta última tarea se está volviendo complicada debido al rechazo por parte de revisores expertos a participar en el proceso editorial. Los motivos son diversos, pero destacan la falta de reconocimiento del trabajo de revisor y el desgaste de los revisores más solicitados. Algunas de las medidas que se podrían tomar para paliar el problema se relacionan con la profesionalización de la revisión por pares. (AU)


The editorial process of scientific journals is complex but essential for the dissemination of scientific knowledge. The quality of the process depends on the authors, editors and reviewers, who must have the necessary experience and knowledge to ensure the quality of the published articles. One of the most significant challenges scientific journals face today is the peer review of manuscripts. Editors are responsible for coordinating and overseeing the entire editorial process, from manuscript submission to final publication, and ensuring that articles meet ethical and scientific integrity standards. Editors are also in charge of selecting appropriate reviewers. However, the latter is becoming difficult due to the increasing refusal of expert reviewers to participate in the editorial process. The reasons for it are diverse, but the lack of recognition for review work and reviewer fatigue in the most sought-after reviewers are among the most important. Some of the measures that could be taken to alleviate the problem concern the possibility of professionalizing peer review. (AU)

Objective: To analyse the outcome of scientific abstracts submitted to the Argentine Congress of Rheumatology (ACOR) in 2000, 2005, 2010, and 2015. Methods: Every abstract submitted to the ACOR was analysed. The number of these manuscripts published was determined through Google Scholar and PubMed searches. The impact of the scientific journals was established through the SCImago Journal (SJR) indicator. Results: Considering the 727 abstracts evaluated, 10.2% of the articles were found in journals indexed by Google Scholar, and 6.6% in PubMed: 4.7% were published in 2000, 9.4% in 2005, 14.6% in 2010, and 11.9% in 2015 (Log Rank test 0.008), with a statistically significant increase between 2010 and 2015 compared to 2000 (HR 3.3; 95% CI 1.5–7; p 0.002 and HR 2.9; CI 1.4–6.3; p 0.005 respectively). The median SJR of the journals was 0.46 and 67.6% had SJR available. Conclusions: The publication rate was low, and only a few articles were published in the most prestigious journals within the speciality.(AU)


Objetivo: Analizar la tasa de publicación de los resúmenes presentados al Congreso Argentino de Reumatología (ACOR) en 2000, 2005, 2010 y 2015. Métodos: Todos los resúmenes enviados al ACOR fueron evaluados. Se determinó la tasa de publicación mediante una búsqueda en Google Scholar y PubMed. Se examinó la relevancia de las revistas científicas a través del indicador SCImago Journal (SJR). Resultados: Se evaluaron 727 resúmenes. Se encontró un 10,2% de artículos publicados en revistas indexadas por Google Scholar y un 6,6% en PubMed. El 4,7% fueron publicados en 2000, el 9,4% en 2005, el 14,6% en 2010 y el 11,9% en 2015 (Log Rank test: 0,008), con un aumento estadísticamente significativo entre 2010 y 2015 frente al 2000 (HR: 3,3; IC95%: 1,5-7; p=0,002 y HR: 2,9; IC95%: 1,4-6,3; p=0,005, respectivamente). La mediana del SJR de dichas revistas fue de 0,46, y el 67,6% tenían SJR disponible. Conclusiones: La tasa de publicación es baja, y solo unos pocos trabajos fueron publicados en las revistas más prestigiosas de la especialidad.(AU)

La publicación es el medio clave por el cual la ciencia se difunde. El propósito de las revistas científicas es publicar artículos novedosos y de calidad. Los editores de las revistas evalúan el contenido de los manuscritos sometiéndolos a un proceso denominado «revisión por pares» (peer review), considerado hoy en día el estándar de oro para garantizar la adecuada publicación de los artículos científicos. Un informe de revisión por pares crítico y bien elaborado es un tesoro, tanto para el autor como para el editor. En el presente manuscrito examinaremos los aspectos clave de la revisión por pares. Comenzaremos explicando en qué consiste exactamente este proceso y desde cuándo existe, para después aclarar por qué es tan importante. Luego argumentaremos por qué deberíamos querer ser evaluadores de artículos científicos. Repasaremos cuáles son las reglas fundamentales para llevar a cabo una buena revisión de un manuscrito y en qué aspectos de este nos deberíamos centrar. Posteriormente veremos qué formato debe tener un informe de revisión por pares y cómo redactar sus distintos apartados, así como las opciones de su dictamen final. Dedicaremos especial atención a comentar los aspectos éticos y los errores más frecuentes que se comenten en la evaluación de manuscritos. Finalmente, reconoceremos cuáles son las limitaciones fundamentales de la revisión por pares y terminaremos proponiendo algunas sugerencias para su mejora. Nuestro objetivo final no es otro que estimular a los investigadores –y autores– a dar un paso más y acometer el reto de ser revisores de manuscritos científicos.(AU)


Publication is the key means by which science spreads. The purpose of scientific journals is to publish novel and quality articles. The editors of the journals evaluate the content of the manuscripts by submitting them to a process called «peer review», considered today the gold standard to guarantee the adequate publication of scientific articles. A well-crafted and critical peer-review report is a treasure for both authors and editors. In the present manuscript we will examine the key aspects of the peer review process. We will begin by explaining what exactly this process consists of and since when it has existed, and then clarifying why it is so important. Then we will argue why we should want to be reviewers of scientific papers. We will then review what are the fundamental rules to carry out a good review of a manuscript and what aspects of it we should focus on. Later we will see what format a peer review report should have and how to write its different sections, as well as the options for its final resolution. We will pay special attention to commenting on the ethical aspects and the most frequent errors that are made in the evaluation of manuscripts. Finally, we will recognize what the fundamental limitations of peer review are, and we will end by proposing some suggestions for their improvement. Our ultimate goal is to stimulate researchers –and authors– to go one step further and undertake the challenge of being peer reviewers of scientific manuscripts.(AU)

Objetivo: Analizar el efecto del estilo de liderazgo sobre la gestión del conocimiento y la eficiencia hospitalaria aplicando para ello mapas cognitivos difusos (MCD). Método: Los MCD son modelos relacionales que se pueden utilizar para representar gráficamente la opinión y el conocimiento de un grupo de personas expertas e inferir las relaciones causa-efecto que hay entre distintos conceptos. La utilización de MCD como herramienta de simulación permite evaluar posibles escenarios basados en distintos estilos de liderazgo en los hospitales. Resultados: En la matriz aumentada resultante, los efectos estandarizados varían de 0,02 a 0,84, representando el valor más alto la relación entre explotación del conocimiento y eficiencia hospitalaria. Para las personas expertas, la creación de conocimiento en el hospital también influye en la eficiencia hospitalaria. Se han identificado efectos positivos, aunque con distinta intensidad, de la autoridad, la benevolencia y el carisma del líder en la creación y la explotación del conocimiento y en la eficiencia hospitalaria. El liderazgo transformacional se asocia a los valores más altos de gestión de conocimiento y eficiencia hospitalaria. Conclusiones: El estilo de liderazgo autoritario parece ofrecer niveles más bajos de creación y gestión de conocimiento, así como una menor eficiencia hospitalaria. Se asocian al estilo de liderazgo paternalista mejores valores tanto en la creación y la explotación de conocimiento como en la eficiencia hospitalaria, en comparación con el estilo de liderazgo autoritario. Por último, se atribuyen al liderazgo transformacional las mayores cotas en los aspectos relacionados con gestión del conocimiento y eficiencia hospitalaria.(AU)


Objective: To analyse the effect of leadership style on knowledge management in hospitals and hospital efficiency based on the opinion of experts in hospital management, applying fuzzy cognitive maps (FCM). Method: FCM are relational models that can be used to graphically represent expert opinion and knowledge to infer cause–effect relationships between different concepts. The use of FCM as a simulation tool allows the evaluation of possible scenarios based on different leadership styles in hospitals. Results: In the resulting augmented matrix, standardized effects range from 0.02 to 0.84, with the highest value representing the strongest relationship between knowledge exploitation and hospital efficiency. From the viewpoint of experts, knowledge creation within the hospital also influences hospital efficiency. Regarding variables reflecting leadership characteristics, positive effects have been identified, though with varying intensities, between authority, benevolence, and charisma, both in terms of knowledge creation and exploitation, as well as hospital efficiency. The transformational leadership style is associated with coefficients having higher values for knowledge management and hospital efficiency. Conclusions: Experts suggest that hospitals with authoritarian leadership styles would exhibit lower levels of knowledge creation and management, as well as lower hospital efficiency. On the other hand, they associate hospitals managed with a paternalistic leadership style with better values in both knowledge creation and exploitation, as well as hospital efficiency, compared to the authoritarian leadership style. Finally, they attribute the highest levels in aspects related to knowledge management and hospital efficiency to the transformational leadership style.(AU)

Objetivo Conocer el destino de los trabajos rechazados en Medicina Intensiva (MI) en el período del 2015 al 2017 con seguimiento hasta el 2019. Diseño Estudio retrospectivo observacional. Ámbito Publicaciones en revistas biomédicas. Participantes Manuscritos rechazados en la revista Medicina Intensiva. Intervenciones Ninguna Variables de interés Tiempo de publicación, factor de impacto (FI), citas generadas y variables asociadas con la publicación. Resultados De 344 originales y 263 cartas científicas, se rechazaron 420 (69,2%). Se publicaron después 205 (48,8%) y 66 de ellos generaron 180 citas. El FI de las revistas fue menor en 173 casos (84,4%). En 21, el número de citas válidas para FI fue mayor que el FI de MI. El origen del manuscrito odds ratio (OR) 2,11 (IC 95% 1,29 a 3,46), la mujer como autora OR 1,58 (IC 95% 1,03 a 2,44), que estuviera en lengua inglesa OR 2,38 (IC 95% 1,41 a 4,0) y que el artículo hubiera pasado a revisores OR 1,71 (IC 95% 1,10 a 2,66) se asociaron con mayor tasa de publicación en revistas indexadas en PubMed. Conclusiones Los artículos rechazados en MI tienen una tasa media de publicación en otras revistas, principalmente con menos FI y generando menor número de citas que el FI de MI. (AU)

Main variables of interest Time of publication, impact factor (IF), generated citations and variables associated to publication. Results The 69% (420) of analyzed articles (344 originals and 263 scientific letters) were rejected, and 205 (48.8%) were subsequently published, with 180 citations of 66 articles. Journal IF was lower in 173 (84.4%) articles. The number of FI-valid citations was higher than the FI of MI in 21 articles. Origin of manuscript OR 2,11 (IC 95% 1.29 – 3.46), female author OR 1.58 (IC 95% 1.03-2.44), english language OR 2,38 (IC 95% 1.41-4.0) and reviewed papers OR 1.71 (IC 95% 1.10-2.66) were associated to publication in PubMed database. Conclusions The rejected articles in MI have a mean publication rate in other journals. Most of these articles are published in journals with less IF and fewer citations than the IF of MI. (AU)

Scholarly publishing is in a crisis, with the many stakeholders complaining about different aspects of the system. Authors want fast publication times, high visibility and publications in high-impact journals. Readers want freely accessible, high-quality articles. Peer reviewers want recognition for the work they perform to ensure the quality of the published articles. However, authors, peer reviewers, and readers are three different roles played by the same group of individuals, the users of the scholarly publishing system-and this system could work based on a collaborative publishing principle where "nobody pays, and nobody gets paid"
