Background/methods: The impact of clinical pharmacist on undiagnosed pregnancy hyperglycemia (PHG) in mid- and late- pregnancy as a major preventable cause of maternal and neonatal (M/N) complications is investigated. This longitudinal randomized controlled study of changes in plasma levels of predictive/prognostic/diagnostic biomarkers of oxytocin, thrombospondin, MCP1, IL6, MIF, insulin and LAR and undesirable M/N pregnancy outcomes in women with/out PHG (pregnancy normoglycemia; PNG) following the implementation of clinical pharmacist interventions were investigated. Results: A total of 68 PHG (36 intervention vs. 32 non-intervention) vs. 21 PNG participants were enrolled at 20–28 weeks and followed up till delivery. BMI of intervention PHG (unlike non-intervention) was greater (p=0.036) compared to PNG’s. LAR and insulin, oxytocin, thrombospondin1, adiponectin and MCP1 plasma levels and their differences between 2nd and 3rd pregnancy trimesters lacked discrepancies in participants. Both PHG groups in mid pregnancy had substantially greater HbA1c %, FPG and IL6 levels vs. PNG, while PHG non-intervention’ leptin was greater than PNG’s. In late pregnancy, greater SBP, IL6 and MIF levels between either PHG groups vs. PNG’s were observed. Unlike PHG non-intervention and PNG; IL6 level in PHG intervention group decreased (-2.54±6.61; vs. non-intervention PHG’s 4.26±5.28; p<0.001 and vs. PNG’s 2.30±4.27; p=0.023). None of the assessed M/N outcomes was found of differential significance between any of the three study groups. Conclusions: Proinflammatory IL6 as a robust and generalizable cardiometabolic risk-based and related pharmacotherapy biomarker in mid and late hyperglycemic pregnancy with likely implications of novel therapeutic targets was delineated by clinical pharmacist interventions.(AU)

RESUMO

La preeclampsia representa una complicación específica de hipertensión del embarazo, que aparece de novo después de la 20 semana de gestación, acompañada de proteinuria y/o disfunción orgánica materna o útero-placentaria. A pesar de una etiopatogenia incierta, la alteración en el remodelado vascular de la arteria espiral e isquemia placentaria es la hipótesis más generalizada. El hallazgo de niveles elevados de copeptina, en mujeres con preeclampsia respecto a gestantes normales, ha puesto en valor la implicación de la arginina-vasopresina en la etiopatogenia de esta complicación. En este trabajo se considera su utilidad como marcador de preeclampsia a través de la revisión de los principales estudios efectuados con esta molécula.(AU)


Preeclampsia represents a specific complication of pregnancy hypertension, which appears de novo after the 20th week of gestation, accompanied by proteinuria and/or maternal or utero-placental organ dysfunction. Despite an uncertain etiopathogenesis, impaired vascular remodeling of the spiral artery and placental ischemia is the most widespread hypothesis. The finding of elevated levels of copeptin in women with preeclampsia compared to normal pregnant women has valued the involvement of arginine vasopressin in the etiopathogenesis of this complication. In this paper, its usefulness as a marker of preeclampsia is considered through the review of the main studies carried out with this molecule.(AU)

Introducción: El objetivo de este trabajo consiste en describir y analizar los resultados perinatales de una serie de casos de anemia-policitemia (TAPS) seguidos y tratados en un centro de referencia en Medicina Fetal, en términos del tipo TAPS y la conducta prenatal aplicada. Métodos: Análisis estadístico descriptivo e inferencial con el programa IBM SPSS Statistics. Resultados: Se diagnosticaron 13 casos de TAPS en un total de 510 gestaciones monocoriales. La mayoría de TAPS espontáneos se diagnosticaron en estadios precoces (5 casos), mientras que, en TAPS posláser, 3/6 casos se diagnosticaron en estadios avanzados. Cuatro fetos (4/26; 15,4%) presentaron alteraciones en la neurosonografía, 3 (3/14; 21,4%) en TAPS espontáneos y uno (1/12, 8,3%) en TAPS posláser (p = 0,308). Se detectó hipertrofia miocárdica en 7 fetos, 5 (71,4%) de los cuales correspondían a TAPS en estadios avanzados. Cuatro eran TAPS espontáneos (4/14; 28,6%) y 3 TAPS posláser (3/12; 25%) (p = 0,797). Ocho casos (8/13; 62,5%) precisaron algún procedimiento de terapia fetal. Se produjeron 3 muertes fetales anteparto y una pareja decidió interrupción legal del embarazo (19,23%). Las 5 pérdidas se produjeron en estadio IV (p=0,008).La supervivencia global fue del 80,8% (21/26 recién nacidos vivos). No hubo diferencias significativas en función del tipo TAPS (p = 0,159) y la conducta prenatal adoptada (p = 0,746). Conclusiones: El TAPS espontáneo es, por tanto, una entidad clínica con un impacto en los resultados perinatales similar al posláser. Parece que la conducta expectante y el láser consiguen mayor edad gestacional al nacimiento.(AU)


Introduction: The aim of this paper is to describe and analyze the perinatal outcomes of a series of TAPS cases followed and treated in a Fetal Medicine referral center, in terms of the type of TAPS and the prenatal behavior applied. Methods: Descriptive and inferential statistical analysis with IBM SPSS Statistics software. Results: Thirteen cases of TAPS were diagnosed in a total of 510 monocorial gestations. Most of the spontaneous TAPS were diagnosed at early stages (5 cases), whereas, in post-laser TAPS, 3/6 cases were diagnosed at advanced stages. Four fetuses (4/26; 15.4%) presented alterations in neurosonography, 3 (3/14; 21.4%) in spontaneous TAPS and one (1/12, 8.3%) in post-laser TAPS (P=0.308). Myocardial hypertrophy was detected in 7 fetuses, 5 (71.4%) of which corresponded to advanced stage TAPS. Four were spontaneous TAPS (4/14; 28.6%) and 3 were post-laser TAPS (3/12; 25%) (P=0.797). Eight cases (8/13; 62.5%) required some fetal therapy procedure. There were 3 antepartum fetal deaths and one couple decided to legally terminate the pregnancy (19.23%). All 5 losses were stage IV (P=0.008). Overall survival was 80.8% (21/26 live newborns). There were no significant differences according to the type of TAPS (P= 0.159) and the prenatal behavior adopted (P=0.746).Conclusions: Spontaneous TAPS is therefore a clinical entity with an impact on perinatal outcomes similar to post-laser. It seems that expectant management and laser achieve higher gestational age at birth.(AU)

Objetivos: Evaluar si existe una mayor tasa de resultados obstétricos adversos, incontinencia urinaria posparto y problemas sexuales entre las mujeres que dan a luz después de los 50 años.Material y métodos: Estudio observacional ambispectivo de un solo centro. Se registraron la tasa de parto por cesárea, la diabetes gestacional, la preeclampsia, la restricción del crecimiento intrauterino (RCIU), la prematuridad, la incontinencia urinaria (Cuestionario de incontinencia en formato corto [ICIQ-SF]) y la disfunción sexual (índice de función sexual femenina [FSFI-6]). Resultados: Veinticinco (0,06%) de 38.510 nacimientos ocurrieron en mujeres mayores de 50 años durante el período de estudio en nuestro centro. Hubo 16 (64%) partos por cesárea. Siete (28%) mujeres padecieron diabetes gestacional. Se diagnosticó preeclampsia en 3 (12%) mujeres. Hubo 5 (20%) casos de RCIU. Hubo 5 (20%) partos prematuros. Las diferencias en la tasa de parto por cesárea, diabetes gestacional y RCIU entre el grupo de estudio y la población total fueron estadísticamente significativas. Los resultados de los cuestionarios ICIQ-SF y FSFI-6 se obtuvieron de 17 mujeres. Se encontró algún grado de incontinencia urinaria en 7 (41,1%) y disfunción sexual en 9 (52,9%) mujeres. Conclusiones: Los embarazos en mujeres mayores de 50 años parecen estar asociados con una mayor tasa de diabetes gestacional, RCIU y preeclampsia. Hay una alta prevalencia de incontinencia urinaria y problemas sexuales entre estas mujeres.(AU)


Objectives: To assess whether there is a higher rate of adverse obstetric outcomes, postpartum urinary incontinence, and sexual problems among women who give birth over 50. Material and methods: A single-center ambispective observational study. Rate of cesarean birth, gestational diabetes, preeclampsia, intrauterine growth restriction (IUGR), prematurity, urinary incontinence (Incontinence Questionnaire Short Form [ICIQ-SF]), and sexual dysfunction (Female Sexual Function Index [FSFI-6]) were recorded. Results: Twenty-five (0.06%) of 38,510 births occurred in women over 50 during the study period. There were 16 (64%) cesarean births. Seven (28%) women had gestational diabetes. Preeclampsia was diagnosed in 3 (12%) women. There were 5 (20%) cases of IUGR. There were 5 (20%) preterm births. The differences in the rate of cesarean birth, gestational diabetes, and IUGR between the study group and the total population were statistically significant. The results of the ICIQ-SF and FSFI-6 questionnaires were obtained from 17 women. Some degree of urinary incontinence was found in 7 (41.1%) and sexual dysfunction in 9 (52.9%) women. Conclusions: Pregnancies in women over 50 may be associated with a higher rate of gestational diabetes, IUGR, and preeclampsia. There is a high prevalence of urinary incontinence and sexual problems among these women.(AU)

Introducción: El embarazo representa un estado procoagulante que aumenta el riesgo de fenómenos trombóticos en la mujer. Al igual que en la población no gestante, deberemos diagnosticar y tratar lo más precozmente posible dichos eventos para evitar la migración del trombo a otras localizaciones. La anticoagulación constituye la medida terapéutica de primera línea. Sin embargo, ciertas situaciones de aumento de sangrado como el trabajo de parto pueden suponer una contraindicación para iniciar dicha anticoagulación. En el siguiente artículo se desarrolla un caso clínico en el que el diagnóstico de trombosis venosa profunda tuvo lugar al inicio del trabajo de parto. Principales síntomas y/o hallazgos clínicos: Aumento de diámetro de miembro inferior izquierdo, con enrojecimiento del mismo y aumento de temperatura local. La paciente describía dolor a nivel de dicha extremidad desde hacía 15 días. No presentaba disnea y la saturación de oxígeno era del 100%. Diagnósticos principales: Trombosis venosa profunda, trabajo de parto. Intervenciones terapéuticas: La contraindicación de anticoagulación y la necesidad de un tratamiento inmediato plantearon el filtro de vena cava inferior como la opción más razonable. Resultados: Tras la colocación del filtro, el parto cursó sin incidencias; no migró el émbolo y el filtro se retiró a los 29 días del parto sin complicaciones. Conclusión: Pese a nuestra escasa experiencia clínica en el manejo de estos casos, podemos afirmar que el filtro de vena cava inferior representa una opción razonable y segura en los casos de trombosis de miembros inferiores en mujeres en trabajo de parto.(AU)


Introduction: Pregnancy represents a procoagulant state that increases women's risk of thrombotic phenomena. As in the non-pregnant population, we must diagnose and treat these events as early as possible to avoid the migration of the thrombus to other locations. Anticoagulation is the first-line therapeutic measure. However, certain situations of increased bleeding such as labor may be a contraindication to initiate such anticoagulation. The following article develops a clinical case in which the deep vein thrombosis diagnosis occurred at the onset of labor. Main symptoms and/or clinical findings: Increase in diameter of the left lower limb, with redness of the same and increase in local temperature. The patient described pain at the level of this limb for 15 days. He had no dyspnea and oxygen saturation was 100%. Main diagnoses: Deep vein thrombosis, labor. Therapeutic interventions: The contraindication of anticoagulation and the need for immediate treatment raised the inferior vena cava filter as the most reasonable option. Results: After the placement of the filter, the delivery proceeded without incident. The plunger did not migrate and the filter was removed 29 days after delivery without complications. Conclusion: Despite our limited clinical experience in the management of these cases, we can affirm that the inferior vena cava filter represents a reasonable and safe option in cases of thrombosis of the lower limbs in women in labor.(AU)

Introduction: Pregnancy is a period of physical and physiological changes that progresses without complications when free from factors that can compromise maternal and fetal health. Diabetes mellitus, high blood pressure, obesity and inadequate weight gain are the most common causes of gestational risk, and these can be strongly associated with the nutritional status, eating habits and behavior of the pregnant woman. Objective: To describe and associate eating behavior with the nutritional status of high-risk pregnant women admitted to a reference center in the Northeast. Materials and methods: This is a cross-sectional study carried out with patients admitted to high-risk wards at the Instituto de Medicina Integral Prof. Fernando Figueira – IMIP (Recife-PE), anthropometric data and data related to eating behavior were collected (TEMS instrument). Results: Around 56,4% of pregnant women had a prepregnancy BMI classified as overweight or obese, with only 16% classified as underweight. The evaluation of the evolution of weight gain demonstrated that 66.4% had insufficient weight gain, while 12,8% had high gestational weight gain. Discussion: When analyzing eating behavior through the TEMS instrument, the relationship of giving a good image when eating had a significant value (p < 0,05) as well as considerations of choosing healthy foods and related to obtaining energy are directly associated with the current BMI, therefore, note- It is clear that emotional, social, physiological and economic factors influence the choice of food. Conclusion: The high prevalence of overweight and obesity found in this population demonstrates the influence of eating behavior on nutritional status. Reinforcing the importance of preventive measures to identify risk factors and obtain adequate nutritional monitoring during pregnancy with the aim of reducing damage to health and promoting autonomy to make more appropriate and healthy food choices.(AU)


Introdução: A gestação é um período de mudanças físi-cas e fisiológicas que evolui sem complicações quando isentade fatores que podem acarretar no comprometimento dasaúde materna e fetal. O diabetes mellitus, hipertensão arte-rial, obesidade, ganho de peso inadequado são as mais fre-quentes causadoras do risco gestacional, podendo estas serfortemente associadas com o estado nutricional, hábito ecomportamento alimentar da gestante. Objetivo: Descrever e associar o comportamento alimen-tar com o estado nutricional de gestantes de alto risco inter-nadas em um centro de referência no Nordeste. Materiais e métodos: Trata-se de um estudo do tipotransversal realizado com pacientes internadas nas enferma-rias de alto risco do Instituto de Medicina Integral Prof.Fernando Figueira – IMIP (Recife- PE), foram coletados dadosantropométricos, dados relacionados ao comportamento ali-mentar (instrumento TEMS). Resultados: Cerca de 56,4% das gestantes tinham IMCpré-gestacional classificados em sobrepeso ou obesidade sendo apenas 16% classificadas como baixo peso. A avalia-ção da evolução do ganho ponderal demonstrou que 66,4%apresentaram um ganho de peso insuficiente, enquanto12,8% tiveram um ganho de peso gestacional elevado. Discussão: Ao analisar o comportamento alimentar atravésdo instrumento TEMS, a relação de passar uma boa imagemao se alimentar teve valor significativo (p < 0,05) bem comoconsiderações de escolher alimentos saudáveis e relacionadosa obter energia estão diretamente associados com o IMCatual, assim sendo, nota-se que fatores emocionais, sociais, fi-siológicos, econômicos influenciam na escolha dos alimentos. Conclusão: A elevada prevalência do excesso de peso eobesidade encontrada nessa população demonstra a influência do comportamento alimentar no estado nutricional.Reforçando a importância de medidas preventivas para identificar fatores de risco e obter um acompanhamento nutricio-nal adequado durante a gestação com...(AU)

El embarazo es uno de los momentos más importantes y difíciles por los que transcurre una mujer a lo largo de su vida. Supone un periodo de grandes necesidades de macro y micronutrientes para satisfacer las demandas del feto en desarrollo y evitar carencias, para así obtener el mejor resultado posible. Hoy en día, la mayoría de mujeres embarazadas o planeando estarlo conocen la importancia de obtener la cantidad requerida de ciertos tipos de nutrientes (proteínas, grasas, folato, etc.), así como evitar ciertos compuestos (alcohol, tabaco, fármacos, etc.) para evitar posibles complicaciones durante el embarazo. En los últimos años, con la mayor evidencia científica disponible, se ha ido demostrando como algunos de estos nutrientes podrían tener un papel más relevante del que se creía en el resultado óptimo del embarazo, siendo uno de estos nutrientes la colina. La suplementación con colina durante el embarazo ha demostrado ser un tratamiento no farmacológico capaz de mejorar cualidades tanto físicas (crecimiento) como mentales (memoria) del nuevo individuo. La colina se conoce como un nutriente esencial desde 1998 y varios estudios han demostrado su efectividad en modelos de roedores. La existencia de recientes publicaciones que versan sobre su aplicación en humanos hace necesaria la realización de una revisión sistemática. En esta revisión sistemática de la evidencia científica disponible desde el año 2012 hasta la actualidad que versa sobre la aplicación de un mayor consumo de colina mediante suplementación como tratamiento para mejorar los resultados del embarazo, su objetivo principal es determinar los efectos que puede tener en la cognición de los niños una intervención nutricional mediante suplementación de colina en madres embarazadas (AU)


Pregnancy is one of the most important and difficult moments that a woman goes through throughout her life. It is a period of great need for macro and micronutrients to meet the demands of the developing fetus and avoid deficiencies, in order to obtain the best possible result. Nowadays, most women who are pregnant or planning to become pregnant know the importance of getting the required amount of certain types of nutrients (proteins, fats, folate, etc.), as well as avoiding certain compounds (alcohol, tobacco, drugs, etc.) to avoid possible complications during pregnancy. In recent years, with the greatest scientific evidence available, it has been shown how some of these nutrients could have a more relevant role than previously believed in the optimal outcome of pregnancy. One of these nutrients being choline. Choline supplementation during pregnancy has been shown to be a non-pharmacological treatment capable of improving both physical (growth) and mental (memory) qualities of the new individual. Choline has been known as an essential nutrient since 1998 and several studies have shown its effectiveness in rodent models. The existence of recent publications that deal with its application in humans makes it necessary to carry out a systematic review. In this systematic review of the scientific evidence available from 2012 to the present that deals with the application of a higher intake of choline through supplementation as a treatment to improve pregnancy outcomes, its main objetive is to determine the effects that a nutritional intervention through choline supplementation in pregnant mothers can have on children's cognition (AU)

Objetivo: Evaluar la asociación entre edad, paridad, nacionalidad, estudios, empleo y apoyo de la pareja con la depresión y ansiedad durante el embarazo. Determinar el impacto de la evolución de la pandemia COVID-19 en los niveles de ansiedad y depresión de las participantes. Método: Estudio descriptivo correlacional transversal realizado en el Osakidetza/Servicio Vasco de Salud, Atención primaria, Bizkaia (España). Participaron 295 gestantes entre ocho y 41 semanas. Se midió la asociación entre edad, paridad, nacionalidad, nivel de estudios, empleo, apoyo de la pareja y tasas de incidencia de COVID-19 durante el periodo del estudio y las puntuaciones de depresión (medido con la Escala de Depresión Posnatal de Edimburgo) y de ansiedad (medido con la subescala de ansiedad estado-rasgo del State-Anxiety Inventory [STAI-S]) durante el embarazo. Se construyó un modelo de regresión logística y se estimaron las odds ratios (OR) y sus intervalos de confianza de 95% (IC 95%). Resultados: La puntuación media en ansiedad fue de 18,75 puntos (DE = 8,69) y en depresión 6,45 puntos (DE = 4,32). Las mujeres que esperaban su segundo hijo o más obtuvieron puntuaciones más altas en depresión (OR 2,51 [IC 95%: 1,26-5,01]) y ansiedad (OR 1,98 [IC 95%: 1,01-3,89]). Haber cursado estudios universitarios se asocia con puntuaciones más bajas en depresión (OR 0,28 [IC 95%: 0,11-0,77]) y ansiedad (OR 0,2 [IC 95%:0,08-0,54]). Una buena calidad en la relación de pareja se asocia con menores niveles de ansiedad (OR 0,45 [IC 95%: 0,24-0,81). La puntuación media en ansiedad y depresión se relacionan con la incidencia de casos COVID-19; la puntuación en ansiedad fue significativamente más alta en los periodos de mayor incidencia. Conclusiones: Mayor atención emocional a las gestantes con bajo nivel de estudios, multíparas y con un débil apoyo de su pareja, permitiría reducir la ansiedad y depresión en el embarazo. Objective: To assess the association between age, parity, nationality, educational level, employment and partner support with depression and anxiety during pregnancy. To determine the impact of the evolution of the COVID-19 pandemic on the anxiety and depression levels of the participants. Method: Cross-sectional descriptive correlational study carried out in the Osakidetza/Basque Health Service, Primary Care, Bizkaia (Spain). A total of 295 pregnant women between 8 and 41 weeks participated. The association between age, parity, nationality, educational level, employment, partner support and COVID-19 incidence rates during the study period and depression (measured with the Edinburgh Postnatal Depression Scale) and anxiety (measured with the State-Anxiety Inventory/STAI-S subscale) scores during pregnancy was measured. A logistic regression model was constructed and odds ratios and their 95% confidence intervals were estimated. Results: The mean score was 18.75 points for anxiety (SD 8.69) and 6.45 points for depression (SD 4.32). Women expecting their second or later child had higher scores for depression (OR 2.51 [95%IC: 1.26-5.01]) and anxiety (OR 1.98 [95%IC: 1.01-3.89]). Having completed university studies was associated with lower scores in depression (OR 0.28 [95%IC: 0.11-0.77]) and in anxiety (OR 0,2 [95%IC: 0.08-0.54]). A good relationship with the partner was associated with lower levels of anxiety (OR 0.45 [95%IC: 0.24-0.81]). Conclusions: Greater emotional care for pregnant women with low educational level, multiparous and with weak support from their partner, would reduce anxiety and depression in pregnancy. Los embarazos en adolescentes se describen como embarazos en mujeres que no han alcanzado la mayoría de edad jurídica y están en situación de dependencia de la familia de origen. El embarazo suele implicar un riesgo en la trayectoria vital de las madres y padres adolescentes y un serio y prevalente problema médico-social. Por todo ello, el embarazo de la adolescente se convierte en una consulta «sagrada». Y el cine se convierte en una oportunidad para hablar con arte, ciencia y conciencia de un tema de trascendental importancia, así como una oportunidad para mejorar la relación profesional-paciente, para mejorar la humanización y para abrir el debate bioético. En este artículo os invitamos a vivir las emociones y reflexiones de la maternidad y el embarazo en alrededor de 40 películas, historias entre la ficción y la realidad. Antecedentes y objetivos La psoriasis afecta a un gran porcentaje de mujeres en edad fértil. Nuestro objetivo fue conocer las inquietudes de las mujeres con psoriasis en relación con la planificación familiar. Material y métodos Estudio observacional, descriptivo, transversal y multicéntrico realizado entre marzo del 2020 y octubre del 2021. Se recabaron datos sociodemográficos e inquietudes relacionadas con la planificación familiar de mujeres entre 18-45 años con psoriasis en placas y candidatas a recibir tratamiento sistémico. Resultados Se reclutaron 153 pacientes de 11 centros españoles (edad media: 35,4 ± 8 años, duración media de la enfermedad: 16,7 años); 38,4% de los casos tenían una enfermedad moderada/grave para los médicos, aunque la percepción de la actividad era significativamente superior para las pacientes. En una de cada tres mujeres, la enfermedad limitaba o retrasaba el deseo gestacional. Existía preocupación de que la enfermedad empeorara al tener que retirar o cambiar un fármaco o que los tratamientos perjudicaran al bebé. Alrededor de la mitad de las pacientes no había recibido información sobre planificación familiar en la consulta, especialmente aquellas mujeres sin embarazos previos. Las mujeres con tratamiento biológico (58,7%) tenían mejor situación clínica, mejor calidad de vida y menos alteraciones en la esfera sexual que las pacientes sin tratamiento biológico. Conclusiones Las pacientes con psoriasis tienen numerosas preocupaciones relacionadas con la planificación familiar. En algunos casos, estos miedos podrían llevar a retrasar y/o limitar el deseo gestacional. Sería necesario incrementar la información que se da a las pacientes y mejorar la formación de los dermatólogos en este tema (AU)

Introducción: El vínculo madre-feto juega un papel importante en la atención del embarazo, impactando los resultados del nacimiento. El monitoreo del movimiento fetal es una competencia fundamental para que las mujeres embarazadas lo hagan de manera independiente. Objetivo: Producir monitoreo audiovisual del movimiento fetal independiente y probar su efectividad en el apego materno-fetal y los resultados del parto. Métodos: La etapa I, desarrollo de Monitoreo de Bienestar Fetal Audiovisual, con estudio de literatura, etapas, desarrollo de escenarios, creación de audiovisuales, prueba de validez de expertos. La etapa 2 probó la efectividad de los medios audiovisuales sobre el apego materno-fetal con el instrumento Inventario de Apego Prenatal y los resultados del nacimiento a partir del peso del bebé al nacer. Diseño de un verdadero enfoque experimental de grupo de control pretest-postest. Muestras de mujeres embarazadas con antecedentes de atención prenatal en el Centro de Salud Kasihan II, Bantul, Yogyakarta, Indonesia con los siguientes criterios: embarazo único, normal, edad gestacional de 28 a 36 semanas. Los encuestados de 60 sujetos se dividieron en grupos experimentales y de control. Los datos normales se probaron mediante la prueba t pareada, la prueba t independiente y MANOVA. Resultados: Puntaje de prueba de validez de experto en contenido 81% muy válido, puntaje de validez de experto en medios 80.33%, válido. La aplicación audiovisual mostró los resultados de la prueba t pareada, tanto en el grupo experimental como en el control hubo diferencias en el pretest y postest, P<0.05. La prueba t de muestra independiente P < 0,05 y los resultados MANOVA simultáneos mostraron una puntuación de apego materno-fetal y un resultado del nacimiento P < 0,05 (AU)


Introduction: The mother-fetus bond plays an important role in pregnancy care, impacting birth outcomes. Monitoring fetal movement is a fundamental competence for pregnant women to do independently. Objective: to produce audiovisual monitoring of independent fetal movement and prove its effectiveness on maternal-fetal attachment and birth outcomes. Methods: Phase I, developing Audiovisual Fetal Well-being Monitoring, with literature study steps, developing scenarios, creating audiovisuals, testing the validity of experts. Stage 2 tested the effectiveness of audiovisual media on maternal-fetal attachment with the Prenatal Attachment Inventory instrument and birth outcomes from infant birth weight. Design of true experimental pretest-posttest control group approach. Samples of pregnant women with a history of antenatal care at the Kasihan II Health Center, Bantul, Yogyakarta, Indonesia with the following criteria: single pregnancy, normal, gestational age 28-36 weeks. Respondents of 60 subjects were divided into experimental and control groups. Normal data were tested by paired t test, independent t-test and MANOVA. Results: Content expert validity test score 81% very valid, media expert validity score 80.33%, valid. The audiovisual application showed the results of the paired t-test, both in the experimental and control groups there were differences in pretest and posttest, P <0.05. Independent sample t-test P < 0.05 and simultaneous MANOVA results showed maternal-fetal attachment score and birth outcome P < 0.05. Conclusion: Independent monitoring of fetal well-being using audiovisual media simultaneously affects the increase in maternal-fetal attachment scores and birth outcomes so it is recommended that second trimester pregnant women be taught techniques for counting fetal movements and practicing them routinely (AU)

Objetivo: Este estudio tiene como objetivo determinar el efecto de las galletas de tomate en los niveles de hierro de las mujeres embarazadas en ciertas áreas de Indonesia. Métodos: Este estudio utilizó un diseño de grupo de control cuasi-experimental antes y después de la prueba. Se tomó una muestra de 102 personas con igual número por grupo mediante muestreo aleatorio simple. Se dieron cinco piezas de galletas de tomate (@20 gramos) todos los días/durante siete días. Los niveles de hierro se midieron antes y después de la prueba. Se utilizaron hojas de observación, procedimientos operativos estándar y niveles de hemoglobina basados en pruebas de laboratorio. Análisis de datos usando la prueba T pareada y la prueba T independiente. Resultados: La mayoría de los encuestados tenían entre 20 y 35 años (90,4 %), tercer trimestre (58,1 %) y multigrávida (83,9 %). El grupo de control no mostró diferencias significativas entre los niveles de hemoglobina antes y después de la prueba (p = 0,317). Hubo un aumento en el nivel de Hb de las mujeres embarazadas después de dar galletas de tomate hasta 1,77 g/dL con p=0,011. Hubo una diferencia significativa en los niveles de hemoglobina entre los grupos de control y de intervención (p=0,025). Conclusiones: La provisión de galletas de tomate aumenta efectivamente los niveles de Hb de las mujeres embarazadas. Esta intervención puede ser una alternativa para la prevención y tratamiento de la anemia en mujeres embarazadas. Las mujeres embarazadas pueden consumir galletas de tomate junto con el consumo de tabletas de hierro (AU)


Primary Goal: This study aims to determine the effect of tomato cookies on the iron levels of pregnant women in certain areas in Indonesia. Methods: This study used a quasi-experimental pretest and posttest control group design. A sample of 102 people with the same number per group was taken using simple random sampling. Tomato biscuits were given five pieces (@20 grams) every day/for seven days. Iron levels were measured during the pretest and posttest. Observation sheets, standard operating procedures, and haemoglobin levels based on laboratory tests were used. In comparison, data Analysis using Paired T-Test and Independent T-Test. Results: The majority of respondents were aged 20-35 years (90.4%), third trimester (58.1%), and multigravida (83.9%). The control group showed no significant difference between pretest and posttest haemoglobin levels (p=0.317). There was an increase in the Hb level of pregnant women after giving tomato Cookies as much as 1.77 g/dL with p=0.011. There was a significant difference in haemoglobin levels between the control and intervention groups (p=0.025). Conclusions: The provision of tomato cookies effectively increases the Hb levels of pregnant women. This intervention can be an alternative for preventing and treating anaemia in pregnant women. Pregnant women can consume tomato cookies together with the consumption of iron tablets (AU)

Introducción: Existe la necesidad de proporcionar estrategias de analgesia que alienten y promuevan la participación de la mujer en la toma de decisiones en el momento del parto y las técnicas de relajación podrían ser un método analgésico no far-macológico complementario y/o alternativo a la anestesia epidural ampliamente utilizado. en la estándar atención del trabajo de parto. Objetivo: El objetivode este estudio es analizar los efectos obstétricos de las técnicas de relajación en el manejo del dolor durante el parto. Método: Se realiza una revisión sistemática con lectura crítica de los estudios incluidos. La búsqueda de estudios se realizó en las principales bases de datos MEDLINE, Cochrane Library, Cuiden, LILACS y SciELO. Se incluyen estudios publicados en inglés o español entre 2015 y febrero de 2021. Se incluyen una vez estudios, seis de los cuales son revisados sistemáticamente y cinco son ensayos clínicos aleatorios. Las intervenciones analizadas fueron técnicas de relajación como hipnosis, inyección intradérmica de agua estéril, inmersión en agua tibia, masaje, acu-puntura, musicoterapia, aromaterapia, apoyo continuo y prácticas mente-cuerpo como respiración relajante, yoga y meditación, entre otras. Conclusión: La principal conclusión de este estudio es que las técnicas de relajación pueden disminuir el nivel de dolor durante el trabajo, aunque la evidencia científica actual es limitada y la calidad metodológica varía de baja a moderada. Se necesitan más ensayos controlados aleatorios para apoyar esta investigación (AU)


Introduction:There is a need to provide analgesia strategies that encourage and promote women's participation in decision-making at the time of delivery and relaxation techniques could be a complementary and/or alternative non-pharmacological analgesic method to the widely used epidural anaesthesia in standard labour care. Objective: Theobjective of this study is to analyze the obstetric effects of relaxation techniques on pain management during labour. Method: A systematic review is performed with critical reading of included studies. The search for studies was carried out in the main databases MEDLINE, Cochrane Library, Cuiden, LILACS and SciELO. Studies published in English or Spanish between 2015 and February 2021 were included. Eleven studies were included, six of which are systematic reviews and five are randomised clinical trials. The interventions analysed were relaxation techniques such as hypnosis, intradermal injection of sterile water, warm water immersion, massage, acupuncture, music therapy, aromatherapy, continuous support and mind-body practices like relaxing breathing, yoga and meditation, among others. Conclusion: The main conclusion of this study is that relaxation techniques may decrease the level of pain during labour, although the current scientific evidence is limited and the methodological quality varies from low to moderate. More randomised controlled trials are needed to support this research (AU)

Objetivo: Analizar el impacto del confinamiento por la pandemia de la COVID-19 en la vivencia del embarazo y la maternidad de mujeres colombianas. Métodos: Cualitativo, basado en la teoría fundamentada. Se analizaron 17 entrevistas semiestructuradas realizadas a seis mujeres embarazadas y a 11 madres primerizas con bebés de hasta 11 meses de edad. El estudio fue realizado en varias ciudades de Colombia entre abril y junio de 2021. Resultados: Se identificaron cuatro categorías centrales que describen el impacto del confinamiento sobre la vivencia del embarazo y la maternidad: el apoyo social, especialmente limitado en el contacto con la familia y la pérdida de rituales de celebración de la maternidad; las emociones, reflejadas en la experiencia de agotamiento, ansiedad, soledad, frustración, y en el miedo al parto, al contagio y a las posibles secuelas del confinamiento en el desarrollo de los bebés; los cambios producto de la maternidad, evidenciados en roles y decisiones laborales; y los aspectos positivos de la pandemia, como los permisos de maternidad ampliados, la conciliación familiar y la mayor presencia e implicación de las parejas. Conclusión: La pandemia por COVID-19 afectó a la experiencia habitual del embarazo y a la maternidad de las mujeres colombianas que fueron entrevistadas. Este estudio permitió conocer sus experiencias emocionales, los principales retos a los que se enfrentaron y las estrategias que les permitieron adaptarse a esta coyuntura. (AU)


Objective: To analyse the impact of confinement due to the COVID-19 pandemic on Colombian women's experience of pregnancy and motherhood. Methods: Qualitative, based on grounded theory. Seventeen semi-structured interviews with six pregnant women and 11 new mothers with babies up to 11 months old were analysed. The study was conducted in several cities in Colombia between April and June 2021. Results: Four central categories that describe the impact of confinement on the experience of pregnancy and motherhood were identified: social support, especially limited contact with family and the loss of rituals for celebrating motherhood; emotions, reflected in the experiences of exhaustion, anxiety, loneliness, frustration, fear of childbirth, contagion, and the possible consequences of confinement on the development of babies; changes resulting from motherhood, evidenced in roles and work decisions; and the positive aspects of the pandemic, such as extended maternity leave, family balancing, and the greater presence and involvement of partners. Conclusion: The COVID-19 pandemic disrupted Colombian women's normal pregnancy and motherhood experiences. RESUMO

Objective: To determine the effectiveness of mindfulness interventions on anxiety through a systematic review. Method: Systematic review by searching articles through the PubMed, ProQuest, Science Direct, Wiley Library, Sage Journal, and Cochrane Library databases with publication years January 2012 to January 2022. Results: Eleven articles met the inclusion criteria covering several countries, including Canada 1 article, Egypt 1 article, Taiwan 1 article, Amsterdam 2 articles, Iran 1 article, Austria 1 article, San Francisco 1 article, Germany 1 article, Sweden 1 article, China 1 article, and Spain 1 article. Conclusions: Management of anxiety about childbirth is important for pregnant women. Mindfulness interventions are effective for reducing anxiety about labor and increasing comfort during labor. Mindfulness intervention mechanisms have the potential to reduce anxiety by increasing skills to regulate emotions.(AU)


Objetivo: Determinar la efectividad de las intervenciones de mindfulness en la ansiedad, el dolor de parto y el resultado neonatal a través de una revisión sistemática. Método: Revisión sistemática mediante búsqueda de artículos en las bases de datos PubMed, ProQuest, Science Direct, Wiley Library, Sage Journal y Cochrane Library entre enero de 2012 y enero de 2022. Resultados: Once artículos cumplieron los criterios de inclusión, que corresponden a diversos países, incluyendo Canadá 1 artículo, Egipto 1 artículo, Taiwán 1 artículo, Ámsterdam 2 artículos, Irán 1 artículo, Austria 1 artículo, San Francisco 1 artículo, Alemania 1 artículo, Suecia 1 artículo, China 1 artículo y España 1 artículo. Conclusiones: El manejo de la ansiedad debida al parto es importante para las gestantes. Las intervenciones de mindfulness son efectivas para reducir la ansiedad ante el trabajo de parto y aumentar la comodidad durante este. Los mecanismos de intervención de mindfulness tienen el potencial de reducir la ansiedad al aumentar las habilidades para regular las emociones.(AU)
