RESUMO

Una práctica muy habitual en la investigación médica, durante el proceso de análisis de los datos, es dicotomizar variables numéricas en dos grupos. Dicha práctica conlleva la pérdida de información muy útil que puede restar eficacia a la investigación. A través de varios ejemplos, se muestra cómo con la dicotomización de variables numéricas los estudios pierden potencia estadística. Esto puede ser un aspecto crítico que impida valorar, por ejemplo, si un procedimiento terapéutico es más efectivo o si un determinado factor es de riesgo. Por tanto, se recomienda no dicotomizar las variables continuas si no existe un motivo muy concreto para ello. (AU)


Abstract. A very common practice in medical research, during the process of data analysis, is to dichotomise numerical variables in two groups. This leads to the loss of very useful information that can undermine the effectiveness of the research. Several examples are used to show how the dichotomisation of numerical variables can lead to a loss of statistical power in studies. This can be a critical aspect in assessing, for example, whether a therapeutic procedure is more effective or whether a certain factor is a risk factor. Dichotomising continuous variables is therefore not recommended unless there is a very specific reason to do so. (AU)

Cuando decidimos hacer un estudio, una de las primeras cuestiones que se plantea es ¿qué número de individuos debo incluir en la muestra para que sea ‘representativa’ y el estudio sea ‘válido’? Como en otros ámbitos de la vida, hay muchas cuestiones para las que no hay una cantidad ‘adecuada’ y son válidas diferentes cantidades. Aquí ocurre lo mismo. La pregunta ‘¿cuántos euros costó esta bicicleta?’ tiene como respuesta un número concreto. Pero la pregunta ‘¿cuántos euros necesito para comprar una bicicleta?’ admite muchas cifras distintas como respuesta, dependiendo del tamaño y otras características de la bicicleta. Los libros de estadística contienen fórmulas que relacionan el tamaño de la muestra con ciertos parámetros y la mayoría de los médicos cree que una de ellas les dará el tamaño ‘adecuado’ para su investigación, y que usándolas queda ‘justificado el tamaño de la muestra’ ante posibles revisores. En este documento se hace una reflexión sobre el verdadero peso que tienen dichas fórmulas y cuál debe ser el uso adecuado que el investigador haga de ellas. Es necesario mostrar errores y simulaciones que no benefician a nadie y perjudican a muchos restando tiempo y energía.(AU)


When we decide to conduct a study, one of the first questions that arises is what number of individuals should be included in the sample for it to be ‘representative’ and for the study to be ‘valid’? As in other areas of life, there are many matters for which there is no ‘right’ amount and different quantities are valid. The same applies here. When asked the question ‘How many euros did this bicycle cost?’, the answer is a definite number. But the question ‘How many euros do I need to buy a bicycle?’ can be answered in many different ways, depending on the size and other characteristics of the bicycle. Statistics textbooks contain formulas relating sample size to certain parameters and most doctors believe that one of these will give them the ‘right’ size for their research, and that by using them their choice of sample size will be justified in the eyes of potential reviewers. This document reflects on the true value of these formulas and how researchers should make proper use of them. It is necessary to show errors and simulations that benefit no one and hinder many by taking up large amounts of time and energy.(AU)

Hace 25 años, en 1998, ante la demanda de que la Sanidad Pública asumiera el coste del tratamiento alternativo del cáncer propuesto por el Profesor Luigi Di Bella -que incluía hormonas, vitaminas y, en ocasiones, quimioterapia- el Parlamento italiano aprobó realizar ensayos clínicos con pacientes oncológicos avanzados para conocer la eficacia de esta terapia. Aunque los estudios en fase II paralelos que se llevaron a cabo en diversos tumores demostraron la falta de actividad del esquema, algunos profesionales han seguido empleándolo desde entonces y han publicado unos resultados aparentemente prometedores en diversas revistas científicas. Este ejemplo real plantea tres escenarios éticos interesantes. El primero es el de la ética de los tratamientos alternativos propuestos por profesionales de la medicina o del ámbito académico que no consiguen distinguir entre hipótesis y eficacia real, algo que influye también en las ex pectativas que genera en pacientes y familiares que deben afrontar una patología potencialmente mortal con pocas o ninguna expectativa de curación con los tratamientos tradicionales. El segundo escenario es el del diseño de ensayos clínicos y la buena práctica para llevarlos a cabo, que fue también motivo de debate en relación con el método Di Bella. Y el último, la ética de las publicaciones científicas. Desde el año 2000, los seguidores de Di Bella han publicado 13 trabajos de calidad limitada con series de pacientes, la mayoría de ellos en una revista de pago por publicación en cuyo comité de editores se encuentra Giuseppe Di Bella, hijo del profesor Di Bella (AU)


Twenty-five years ago, in 1998, the Italian Parliament approved to implement clinical trials in patients with advanced cancer to know the efficacy of an alternative cancer treatment that associated hormones, vitamins and, occasionally, chemotherapy proposed by Professor Luigi Di Bella. It was the answer to people demanding Public Health assume the cost of this therapy. Although parallel phase II trials in various tumors demonstrated the lack of activity, some professionals have continued to use this method since then and have published apparently promising results a few various scientific journals. This real example raises three interesting ethical scenarios. The first one is the ethics of alternative treatments proposed by medical pro fessionals or from the academic field. In these cases, the difficulty in differentiating between hypothesis and real efficacy. This problem impacts on patients and relatives’ expectations who must face a potentially fatal disease with little or no hope of a cure with traditional treatments. The second scenario is the design and good practice in the development of clinical trials, which was also the subject of debate in relation to the Di Bella method. And the last one, the ethics of scientific publications. Di Bella’s followers published since 2000 12 papers with limited quality on series of patients treated with his method, the majority in a pay-per-publication journal of which Giuseppe Di Bella, son of Professor Di Bella, is included in the board of editors (AU)

La implementación de la Convención de Derechos de las Personas con Discapacidad obliga a los Estados Partes a realizar reformas legales que afectan a las normas que regulan la participación de las personas con discapacidad en la investigación biomédica clínica, en especial en los ensayos clínicos con medicamentos. En España, se ha tenido que reformar el Código Civil y la Ley General de Derechos de las Personas con Discapacidad para adecuar el ejercicio de la capacidad jurídica y para garantizar la accesibilidad cognitiva. Estas reformas podrían servir como modelo de las reformas que deben implementarse en otros países.(AU)

A Espanya, s'ha hagut de reformar el Codi Civil i la Llei General de Drets de les Persones amb Discapacitat per adequar l'exercici de la capacitat jurídica i garantir l'accessibilitat cognitiva. The implementation of the Convention on the Rights of Persons with Disabilities obliges the States Parties to carry out legal reforms that affect the norms that regulate the participation of persons with disabilities in clinical biomedical research, especially in clinical trials with drugs. In Spain, the Civil Code and the General Act on the Rights of Persons with Disabilities have had to be reformed to adapt the exercise of legal capacity and to guarantee cognitive accessibility. These reforms could serve as a model for reforms to be implemented in other countries.(AU)

El proceso de investigación biomédica debe seguir unos criterios de calidad en su diseño y elaboración que garanticen que los resultados son creíbles y fiables. Una vez finalizado, llega el momento de escribir un artículo para su publicación. Este debe presentar con suficiente detalle, y de forma clara y transparente, toda la información del trabajo de investigación realizado. De esta forma, los lectores, tras una lectura crítica de lo publicado, podrán juzgar la validez y la relevancia del estudio, y si lo consideran, utilizar los hallazgos. Con el objetivo de mejorar la descripción del proceso de investigación para su publicación, se han desarrollado una serie de guías que, de forma sencilla y estructurada, orientan a los autores a la hora de elaborar un manuscrito. Se presentan en forma de lista, diagrama de flujo, o texto estructurado, y son una ayuda inestimable a la hora de escribir un artículo. Este artículo presenta las guías de elaboración de manuscritos de los diseños más habituales, con sus listas de verificación.(AU)


The biomedical research process must follow certain quality criteria in its design and development to ensure that the results are credible and reliable. Once completed, the time comes to write an article for publication. The article must present in sufficient detail, and in a clear and transparent manner, all the information on the research work that has been carried out. In this way, readers, after a critical reading of the published content, will be able to judge the validity and relevance of the study and, if they so wish, make use of the findings. In order to improve the description of the research process for publication, a series of guidelines have been developed which, in a simple and structured way, guide authors in the preparation of a manuscript. They are presented in the form of a list, flowchart, or structured text, and are an invaluable aid when writing an article. This article presents the reporting guidelines for the most common designs along with the corresponding checklists.(AU)

El conocimiento en metodología es imprescindible para poner en marcha nuestros proyectos de investigación y nuestros diseños epidemiológicos. Plantear adecuadamente un diseño nos permitirá comunicar debidamente los resultados de nuestra experiencia clínica. Existen diferentes tipos de diseño en función de múltiples factores, como la participación activa del investigador, la aleatorización de los pacientes o la formación del grupo control. Conocerlos es fundamental para saber cuál es el más apropiado para comenzar nuestra andadura como investigadores. En este trabajo mostramos las características y la terminología con las que nos referimos a los diferentes diseños epidemiológicos y expondremos algunos de estos diseños, que terminaremos de ver en próximos artículos.(AU)


Methodological knowledge is essential to implement our research projects and epidemiological designs. adequatedesign will allow us to adequately communicate the results of our clinical experience.there are different types of design depending on multiple factors such as the active participation of the investigator,the randomization of patients or the composition of the control group. it is essential to be familiar with them inorder to know which is the most appropriate to start our journey as researchers.in this paper we show the characteristics and terminology with which we refer to the different epidemiologicaldesigns and we will present some of these designs, which we will finish in future papers.(AU)

RESUMO

La realización de cualquier investigación con seres humanos o sus muestras biológicas requiere una valoración ética previa con el fin de evitar riesgos y problemas a todos los actores del proceso. La legislación al respecto es muy amplia, y hace hincapié en la preservación de los derechos de los pacientes en relación con los principios universales de autonomía, beneficencia y justicia. En este manuscrito se revisan las normativas sobre ensayos clínicos, estudios observacionales con medicamentos, proyectos de biobanco o cualquier otro tipo de estudio que se pueda realizar en el ámbito de la salud. Así mismo se analiza el papel de los comités de ética en investigación, la protección de datos y las bases de la integridad científica. (AU)


Any research conducted on human beings or human biological samples requires a prior ethical assessment to avoid risks and problems for all involved parties. The legislation in this regard is very broad and emphasizes the safeguarding of patient rights in relation to the universal principles of autonomy, beneficence and justice. The present article reviews the regulations applicable to clinical trials, observational studies with drugs, biobank projects or any other type of study that may be conducted in the health care field. It also addresses the role of research ethics committees, data protection and the foundations of scientific integrity. (AU)

Introducción: el aumento de la producción científica se ha acompañado de plataformas para autores donde pueden compartir el curriculum vitae y los artículos publicados. El objetivo de este artículo es hacer una revisión narrativa en relación con los perfiles digitales, el uso de redes sociales (RRSS) profesionales y gestores bibliográficos de utilidad para la investigación en profesionales de Atención Primaria. Material y métodos: se realizó una búsqueda en la literatura médica para identificar estudios que informaran sobre temas relacionados con perfil investigador, plataformas online para perfiles digitales y gestores de referencias bibliográficas. Resultados: se recomienda incluir en un perfil: formación, actividad laboral, membresía a instituciones o sociedades científicas, artículos publicados, índice H, citaciones, proyectos financiados, premios recibidos y revisión de artículos en revistas indexadas. Las plataformas más frecuentes son ResearchGate, ORCID, Scopus y Publons, esta última integrada ya en la Web of Science. Todas ellas permiten compartir filiación, artículos publicados y premios recibidos. Las citaciones recibidas solo están disponibles en Scopus y Publons. El índice H está presente en todas las plataformas menos en ORCID. Los proyectos financiados pueden encontrarse en todas las plataformas menos en Publons. ORCID y Publons recogen las revisiones por pares en artículos indexados. Otras plataformas son: Pubmed y Google Scholar que comparten algunas funcionalidades. Los gestores bibliográficos permiten perfiles personales (Zotero), pero ofrecen menos funciones que otras plataformas, aunque sí la creación de grupos de trabajo (Mendeley y Zotero). Conclusión: tener un perfil investigador permite visibilizar la producción científica en Atención Primaria y contactar con otros investigadores.(AU)


Introduction: the increase in scientific production has been accompanied by platforms for authors where they can share their curriculum vitae and published articles. The aim of this article is to perform a narrative review in regard to digital profiles, the use of professional social networks and useful bibliographical managers for research into primary care professionals. Material and Methods: the medical literature was searched to identify studies that reported topics related to researcher profile, online platforms for digital profiles and bibliographical reference managers. Results: it is recommended including in a profile: training, work activity, membership in scientific institutions or societies, published articles, H index, citations, funded projects, awards received and review of articles in indexed journals. The most common platforms are ResearchGate, ORCID, Scopus and Publons, the latter already integrated into the Web of Science. All of them allow you to share affiliation, published articles and awards received. The citations received are only available in Scopus and Publons. The H-index is present on all platforms except ORCID. Funded projects can be found on all platforms except Publons. ORCID and Publons collects peer reviews on indexed articles. Other platforms are: PubMed and Google Scholar that share some functionalities. Bibliographical managers enable personal profiles (Zotero) but offer fewer functions than other platforms, although they do create work groups (Mendeley and Zotero). Conclusion: having a research profile enables scientific production to be visible in primary care and contact with other researchers.(AU)

La evaluación de la actividad de los investigadores, de sus publicaciones y de las revistas donde se publican dichos trabajos, es un proceso continuo y necesario para poder valorar la calidad y el impacto de los mismos. Estas revistas están indexadas en diferentes bases de datos, siendo la Web Of Science una de las más importantes, junto con Scopus, debido a la cantidad de información que tiene recogida y a los rigurosos criterios que deben cumplir las revistas para ser incluidas en ella. Una nueva herramienta de esta base de datos es el Emerging Sources Citation Index, que contiene revistas que o bien están en proceso de evaluación, o bien son revistas que presentan una calidad científica acreditada en su disciplina, pero que nunca llegarán a integrarse en la Web of Science ya que su área de conocimiento es limitada y previsiblemente nunca llegaran a tener un factor de impacto suficientemente alto. El estar incluido en este recurso, como lo está ROL Revista de Enfermería, tiene un gran valor para las revistas y para los autores que publican sus trabajos en ellas ya que supone un mérito valorado en la mayoría de los procesos de evaluación de los investigadores, como por ejemplo en la Agencia Nacional de Evaluación de la Calidad y Acreditación (ANECA). The evaluation of the activity of researchers, their publications and the journals where these works are published, is a continuous and necessary process to assess their quality and impact. These journals are indexed in different databases, being Web Of Science one of the most important, together with Scopus, due to the amount of information it has collected and the rigorous criteria that journals must meet to be included in it. A new tool of this database is the Emerging Sources Citation Index, which contains journals that are either in the process of evaluation, or are journals that have a proven scientific quality in their discipline, but that will never be included in the Web of Science because their area of knowledge is limited and they will probably never have a sufficiently high impact factor. Being included in this resource, as ROL Revista de Enfermería is, has a great value for the journals and for the authors who publish their work in them, since it is a merit valued in most of the evaluation processes of researchers, such as the National Agency for Quality Assessment and Accreditation (ANECA). Background: Allergic rhinoconjunctivitis (ARC) is a common chronic inflammatory disease. Numerous studies on the treatment of ARC have been published. By contrast, there are few bibliometric studies on immunotherapy for ARC. The purpose of this article is to describe the current treatments for ARC and to identify the trends in immunotherapy for ARC. Methods: Publications were searched from the Web of Science (WOS) Core Collection on April 25, 2022. CiteSpace and Microsoft Excel software were used for further bibliometric analysis. Results: A total of 969 publications on immunotherapy for ARC in English were retrieved. The number of relevant publications has been continuously increasing over the past 20 years, with many of the publications coming from Germany and the United States of America. In terms of institutions, the ALK Company in Denmark, Imperial College London in United Kingdom, and Charite–Universitatsmedizin Berlin in Germany published the most articles on immunotherapy for ARC. Meanwhile, Allergy and Journal of Allergy and Clinical Immunology published the most number of studies, and Oliver Pfaar from Germany authored the most number of articles. “Subcutaneous immunotherapy,” “international consensus,” “allergen immunotherapy,” and “recommendation” were the most popular subjects. Thus, directions in research can be predicted as studies regarding mechanisms of ARC, clinical trials, and extracts have reported high-quality results. Conclusion: Over the past 20 years, the overall quality of research on immunotherapy for ARC has gradually improved, allowing the introduction of specific and targeted treatment. Currently, the main focus of ARC research is the novel routes of drug delivery and combined treatment with biological agents (AU)

RESUMO

Nedd4 family interacting protein 1 (Ndfip1) was first mentioned in an article in 2000. Since its discovery, related studies have shown that this protein is associated with apoptosis, neuroprotection, substance transport, ubiquitination, and immune regulation. It is noteworthy that the lack of Ndfip1 can lead to death in fetal mice. Researchers generally believe that the function of Ndfip1 is closely related to individual immune capacity and have published a large number of articles. However, a comprehensive classification of the immune regulatory function of Ndfip1 is still lacking. In this review, we will overview and discuss this new perspective, focusing on the role of Ndfip1 in the proliferation, differentiation, and cell activity of CD4+ T cells, CD8+ T cells, mast cells, and eosinophils. This review provides an updated summary of Ndfip1, which will unveil novel therapeutic targets. Finally, the conclusion is that Ndfip1 mainly plays a negative regulatory role in immune cells by maintaining the stability of the immune response and limiting its overexpression (AU)
