Objectives: The therapeutic effect of acupuncture treatment on chronic kidney disease (CKD) was assessed, and the characteristics of acupoint selection in different CKD types were summarised. Methods: A total of 100 patients with CKD were enrolled in this retrospective cohort study, of whom 50 received acupuncture treatment for 12 weeks (acupuncture group) and 50 received routine treatment for 12 weeks (control group). Routine treatment included appropriate rest, low-salt and low-protein diet and correction of the disturbance in water, electrolyte and acid-base balance. Hypertensive patients received antihypertensive treatment, and patients with hyperuricaemia received uric acid lowering treatment. Acupuncture acupoints were selected in accordance with traditional Chinese medicine (TCM) theory. Baseline characteristics, therapeutic effects and adverse events were assessed, and kidney function indexes, urine albumin creatine ratios (UACRs) and estimated glomerular filtration rates (eGFRs) were compared at different time points (before acupuncture treatment, T1; Immediately after acupuncture treatment, T2; After 3 months of follow-up, T3). Results: The baseline data were slightly different between the groups (p > 0.05), specifically gender, age, duration time, TCM syndrome, CKD stage, systolic blood pressure (SBP), diastolic blood pressure (DBP), and cholesterol, triglyceride, low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C). The acupuncture group presented better therapeutic effects than the control group (82.00% vs. 60.00%, p = 0.015). At T2 or T3, the value of UACR in both groups obviously decreased (p < 0.05), whereas the eGFRs markedly increased (p < 0.05) (AU)

Objective: To explore the intervention value of needle acupuncture and scalp acupuncture in improving cognitive impairment and life in stroke patients; Methods: A total of 62 stroke patients who were healed in our hospital from August 2019 to October 2021 were retrospectively selected as the research objects, and were divided into a combined healing cluster (Combined healing cluster, CTG, n=31, The patients received conventional healing combined with acupuncture and acupuncture) and the general healing cluster (GTG, n=31). The healing effects of the two clusters were contrast, and the National Institutes of Health Stroke Scale (NIHSS), neurological deficit score before and after healing Table (NDS) and Barthel Index (BI) score changes, the follow-up outcomes of the two clusters of patients were calculated and contrast between the two clusters; Results: (1) The total effective rate of patients in CTG cluster was 96.77%, and the total effective rate of patients in GTG cluster was 80.65%, and the variation in effective rate between the two clusters was notable (P<0.05). The NIHSS and NDS marks of the CTG cluster were notably bottom than those of the GTG cluster, and the variation between the clusters was notable (P<0.05). (3) On the 7th, 15th and 30th days of healing, the BI marks of the CTG cluster were notably upper than those in the GTG cluster, and the variation between the clusters was notable (P<0.05). (4) There were a total of 3 recurrences in the CTG cluster after 6 months of follow-up, with a recurrence rate of 10.00%, and a total of 9 recurrences in the GTG cluster. The recurrence rate of patients in the CTG cluster was notably bottom than that in the GTG cluster (P<0.05) (AU)

Introducción y objetivo La parálisis de Bell (PB) es la mononeuropatía del nervio facial que provoca alteraciones de la movilidad voluntaria homolateral, impotencia oral e imposibilidad de cerrar el párpado. Actualmente, el abordaje es multidisciplinar, combinando farmacología y diversas modalidades de fisioterapia. El objetivo del presente trabajo es llevar a cabo una revisión sistemática de la literatura sobre tratamientos de fisioterapia en PB. Material y método Se efectuó una búsqueda PEDro, Pubmed, Cochrane y Embase de los ensayos clínicos aleatorizados publicados desde 2013 a 2020 que versaran sobre tratamientos de fisioterapia en PB. Se realizó un cribado siguiendo los criterios de inclusión planteados y se efectuó un análisis cualitativo de los estudios siguiendo los criterios CONSORT. Resultados Se analizaron 11 publicaciones que aplicaban terapia láser, terapia manual y ejercicios activos, acupuntura, técnica thick-needle, electroestimulación y la combinación de estas. Existía mucha heterogeneidad, tanto en las muestras de población utilizadas y criterios de inclusión planteados, como en las variables y escalas seleccionadas. En todos, aparecían sesgos de diferente origen. Los resultados respaldan el uso de terapia láser de alta intensidad, acupuntura, la acupuntura con láser, la electroestimulación y la técnica thick-needle en el tratamiento de la PB. La terapia manual combinada con ejercicios activos solo redujo el tiempo de recuperación en afectaciones más severas; mientras que los ejercicios de Kabat muestran eficacia en la disminución de sincinesias. Conclusiones La literatura analizada respalda el uso de terapia láser, acupuntura, acupuntura con láser, electroestimulación y ejercicios de Kabat como tratamiento de PB, lo que puede resultar útil para tomar decisiones clínicas y desarrollar nuevas líneas de investigación. Pero la calidad de los estudios requeriría reproducir parcial o totalmente las intervenciones ajustando sus diseños (AU)


Introduction and objective Bell's palsy (PB) is a peripheral mononeuropathy of the facial nerve that leads to loss of ipsilateral voluntary movement, oral impairment and inability to close the eyelid. The approach is currently multidisciplinary combining pharmacological treatment and physical rehabilitation. The main objective of this study is a systematic review of the literature on physical rehabilitation in patients with PB. Material and methods A search of randomized controlled trials on physical rehabilitation after PB published from 2013 to 2020 was performed using the undertaken following the abovementioned inclusion criteria and a qualitative analysis was carried out following the CONSORT criteria. Results Eleven publications were analysed. There was much heterogeneity both in the population samples used and the proposed inclusion criteria, as well as in the variables and scales selected. All of these studies had different biases. Laser therapy, manual therapy and active exercises, acupuncture, thick-needle therapy and electrostimulation were used as physiotherapy modalities. The results support the use of high intensity laser therapy, acupuncture, acupuncture with laser, electrostimulation and thick-needle therapy to treat PB. Manual therapy combined with active exercises only reduced recovery time in patients with severe damage; whereas Kabat exercises reduced synkinesis. Conclusions The literature analysed supports the use of laser therapy, acupuncture, acupuncture with laser, electrostimulation and Kabat exercises as a PB treatment, which can be useful for clinical decision-making and developing new lines of research. Because of the quality of the studies, it would be necessary to reproduce the interventions partially or totally adjusting their designs (AU)

Las enfermedades reumáticas de afectación axial que cursan con dolor cervical y lumbar son muy prevalentes y conllevan un importante consumo de recursos sanitarios. La acupuntura es un procedimiento médico avalado por la Organización Mundial de la Salud con un amplio abanico de indicaciones. Nuestro objetivo fue revisar la evidencia sobre la eficacia de la acupuntura en estas enfermedades. Se realizó búsqueda sistemática: MEDLINE, CENTRAL y EMBASE (mayo de 2017): 535 citas, incluimos 4 revisiones globales (45 revisiones sistemáticas) y 3 revisiones sistemáticas (70 ensayos clínicos).La acupuntura mejora el dolor cervical y la funcionalidad de forma inmediata y a corto plazo. También es eficaz en lumbalgia aguda y crónica como primera línea terapéutica, comparada con placebo, y como coadyuvante de otros tratamientos convencionales. Es una intervención segura. Los resultados no son concluyentes en radiculopatías (cervicales y lumbares), estenosis canal lumbar y espondilitis anquilosante. Faltan estudios bien diseñados con evaluación a medio y largo plazo


Rheumatic diseases involving the spine, mainly cervical and lumbar spine, are highly prevalent, and consume considerable health resources. Acupuncture is a medical procedure endorsed by the WHO, with a wide spectrum of medical applications. The aim of this study was to review the evidence on acupuncture efficacy in these diseases. A literature search was performed in MEDLINE, CENTRAL and EMBASE (May 2017): 535 citations. We included four global reviews(45 systematic reviews) and three Systematic Reviews(70 clinical trials). Acupuncture improved cervical pain and functionality both immediately and in the short term. It also proved effective in the treatment of acute and chronic low back pain as the first therapeutic line, compared with placebo, and as an adjunct to other conventional treatments. Acupuncture is a safe intervention. However, acupuncture efficacy in radiculopathies (cervical, lumbar), lumbar spinal stenosis and ankylosing spondylitis is still inconclusive. Well-designed studies with medium and long-term evaluation are required

OBJETIVO: Determinar la eficacia de la acupuntura para la reducción de la espasticidad en el paciente que ha sufrido un ictus. DISEÑO: Revisión sistemática. Fuente de datos: Búsqueda sistemática en las principales bases de datos de los ensayos clínicos, publicados en español e inglés desde enero de 2000 a enero de 2013, en los que los participantes cursaran con espasticidad a consecuencia de un ictus. Selección de los estudios: Se seleccionaron 9 registros de los 110 localizados. Los criterios de inclusión fueron: ensayos clínicos en pacientes con una edad igual o superior a 18 años, con espasticidad post-ictus y que al menos uno de los grupos experimentales fuese tratado con acupuntura. EXTRACCIÓN DE DATOS: Las principales variables analizadas fueron la resistencia pasiva al estiramiento del miembro afecto y el grado de dependencia personal. Fueron valoradas mediante la escala modificada de Ashworth y el índice de Barthel, respectivamente. MÉTODO: Realizamos una búsqueda en las bases de datos PUBMED, COCHRANE Library, PEDro, Dialnet, CSIC, CINAHL. Se empleó la combinación de los términos «acupuntura», «espasticidad» e «ictus». RESULTADOS: La resistencia pasiva al estiramiento, el grado de dependencia personal y la función motora del miembro afecto mostraron mejoras estadísticamente significativas en al menos un estudio de los incluidos. Se observó una mejora de la resistencia pasiva al estiramiento en codo, tobillo, rodilla y muñeca, hubo un incremento del rango articular, excepto en codo, antebrazo y dedo pulgar, y una mejora de la dependencia de los pacientes. CONCLUSIÓN: Aunque se muestran mejoras respecto a la reducción de la espasticidad, la eficacia de la técnica no ha podido demostrarse para esta enfermedad. Son necesarios estudios que calculen el tamaño de los efectos reportados, aplicando procedimientos homogéneos en el diseño así como en la duración, frecuencia e instrumentos de medida utilizados


OBJECTIVE: To determine the effectiveness of acupuncture for reducing spasticity in post-stroke patients. DESIGN: Literature review. Data source: The literature search was performed using scientific databases from January 2000 to January 2013. SELECTION OF STUDIES: Out of the 110 studies that were found, nine random and controlled trials were included. Inclusion criteria were based on clinical trials in which participants were over 18 years old, who were suffering with post-stroke spasticity, and one of the experimental groups was treated with acupuncture. Data extraction: The variables were the passive resistance to stretching of the affected limb, and the degree of personal dependence. The variables were assessed by the Modified Ashworth Scale and Barthel Index. METHODS: The search was performed in the PUBMED, COCHRANE Library, PEDro, Dialnet, CSIC, CINAHL, databases. Search terms included the combination of keywords 'acupuncture'; 'muscle spasticity'; 'stroke'. RESULTS: Passive resistance to stretching, the degree of personal dependence, and motor function of the affected limb showed statistically significant improvements in at least one study included; however, these improvements are not clinically relevant changes. Passive resistance improved in the elbow, ankle, knee, and wrist. An increased joint range was observed, except for the elbow, forearm, and thumb. Improved of the patient dependency was also observed. CONCLUSIONS: Although improvements relative to the reduction of spasticity are shown, the results have failed to demonstrate the effectiveness of the technique for this ailment. It would take a greater number of studies to calculate the size of the reported effects with homogeneous procedures in the design as well as in the duration, frequency, and measurement tools

La medicina integrativa es un intento ambicioso y, a la vez, generoso de suplir las deficiencias del sistema de salud operativo en nuestras sociedades occidentales, lastradas por la limitación de tiempo disponible, sobre todo en consultas externas. Por otro lado, supone no finiquitar las posibilidades de cierta utilidad terapéutica contrastada a través de los siglos (China, India, etc.), amén de algunos recursos que no alcanzan el nivel de fiabilidad científica deseable, pero que suponen un cierto apoyo terapéutico en determinados casos (homeopatía, acupuntura, etc.), y que siguen requiriendo un abordaje científico. Finalmente, el recurso de productos botánicos (fitoterapia) constituye un amplio mundo de posibilidades en el que se debe avanzar por parte de las universidades —y se está haciendo— al ir adquiriendo dichas sustancias un marchamo farmacológico asentado en el método científico y en los criterios de la medicina basada en la evidencia. Con ello se evitaría la sinrazón que supone el enfrentamiento cotidiano entre la medicina convencional científica, a la que nos sometemos la inmensa mayoría de los pacientes, y las otras «orientaciones» diagnóstico-terapéuticas (medicina naturista, medicina alternativa, medicina complementaria, medicina centrada en el paciente y otras) (AU)


Integrative medicine is an ambitious and noble-minded attempt to address the shortcomings of the current public health systems in our Western societies, which is restricted by the limited time available, especially in outpatient clinics. Integrative medicine also does not limit the possibilities of useful therapies that have been tested over the centuries (from China, India, etc.) or of certain resources that do not achieve the level of desired scientific credibility but that present certain therapeutic support in specific cases (homeopathy, acupuncture, etc.) but still require a scientific approach. Finally, the resource of botanical products (phytotherapy) constitutes a wide range of possibilities that universities can (and do) make progress on by providing drug brands for these products through the use of the scientific method and evidence-based medical criteria. This approach will help avoid the irrationality of the daily struggle between conventional scientific medicine (which we apply to the immense majority of patients) and the other diagnostic-therapeutic «guidelines» (natural medicine, alternative medicine, complementary medicine, patient-focused medicine and others) (AU)

Introducción. La bronconeumonía es una complicación frecuente en los primeros días después de una enfermedad cerebrovascular y se asocia con una mayor tasa de mortalidad. Se produce en pacientes con el nivel de conciencia o el reflejo tusígeno alterado, y podría prevenirse con un programa temprano de rehabilitación de la disfagia. Objetivo. Revisar la literatura científica en relación con el tratamiento y rehabilitación de pacientes con disfagia tras sufrir un ictus, entre 2002 y 2012. Desarrollo. Las bases de datos PubMed, Cochrane, PEDro, CINAHL y ENFISPO ofrecieron 15 artículos que cumplieron los criterios de inclusión y los objetivos planteados, con información sobre 3.212 pacientes. Se describen distintos protocolos y técnicas para la reeducación de la disfagia, como estrategias compensatorias, terapia de regulación orofacial, musicoterapia, estimulación sensorial, entrenamiento muscular labial, de la lengua, faringe, laringe y aparato respiratorio, maniobra de Mendelsohn, estimulación eléctrica neuromu cular, estimulación magnética transcraneal repetitiva y acupuntura. Conclusiones. Los estudios incluidos en esta investigación afirman que el tratamiento de la disfagia tras ictus puede mejorar la función deglutoria (coordinación, velocidad, volumen), la calidad de vida y las relaciones sociales de las personas. Existe una labor pendiente para establecer o definir qué tipo de terapias, técnicas, ejercicios o maniobras son los más eficaces en la disfagia, y para elaborar protocolos de tratamiento o rehabilitación consensuados dentro de las unidades que abordan integralmente el ictus (AU)


Introduction. Bronchopneumonia is a frequent complication in the first days after a cerebrovascular disease and is linked with a higher rate of mortality. It occurs in patients with an altered level of consciousness or tussigenic reflex, and could be prevented with an early dysphagia rehabilitation programme. Aims. To review the scientific literature on the treatment and rehabilitation of patients with dysphagia after suffering a stroke, published between 2002 and 2012. Development. A search conducted in the PubMed, Cochrane, PEDro, CINAHL and ENFISPO databases yielded 15 papers that fulfilled eligibility criteria and the initial aims of the study, providing information about 3,212 patients. The different protocols and techniques for re-education in dysphagia are described and include compensatory strategies, orofacial regulation therapy, music therapy, sensory stimulation, lip muscle, tongue, pharynx, larynx and respiratory tract training, Mendelsohn manoeuvre, neuromuscular electrical stimulation, repetiti transcranial magnetic stimulation and acupuncture. Conclusions. The studies examined in this research claim that the treatment of dysphagia following a stroke can improve the function of deglutition (coordination, speed, volume), quality of life and people’s social relationships. Further work needs to be carried out to establish or define what kind of therapies, techniques, exercises or manoeuvres are the most effective in dysphagia. Generally agreed treatment or rehabilitation protocols also need to be drawn up within units that address stroke in an integrated manner(AU)

Trata-se de uma revisão integrativa que busca sintetizar o conhecimento produzido acerca da utilização da acupuntura como instrumento de trabalho do enfermeiro entre2007 e 2011, nas bases: Bdenf, Cinahl, Redalyc, Lilacs e Scielo. Das nove referências analisadas foram extraídas três categorias temáticas: Experiências de cuidados de enfermagem com autilização da Acupuntura; Aspectos ético-legais da utilização da acupuntura como instrumento de trabalho do enfermeiro; A inserçãoda acupuntura na matriz curricular da Enfermagem. Destaca-se a necessidade de pesquisas na relação prática do enfermeiro e acupuntura,bem como ampliação na capacitação, desde agraduação até a pós graduação, bem como aprópria ampliação no Sistema Único de Saúde (AU)


Se trata de una revisión integradora de la literatura que busca sintetizar el conocimiento sobre el uso de la acupuntura como una herramienta para el trabajo de la enfermera, entre 2007 y 2011, en bases: Bdenf, Cinahl, Lilacs y Scielo, Redalyc. De las nueve referencias analizadas fueron dibujadas tres categorías temáticas: Experiencias de cuidados con el uso de la acupuntura; Aspectos éticos-legales del uso de la acupuntura como herramienta para el trabajo de las enfermeras y La inserción de la acupuntura en el plan de estudios de enfermería. Se destaca la necesidad de más investigaciones acerca de la práctica de enfermería asociada a la acupuntura, así como la expansión en formación, desde la graduación, así como el aumento de su oferta en el sistema unificado de salud (AU)


It is an integrative review that aims to synthesize the knowledge about use of acupuncture as a tool for nurse’s work between 2007 and 2011, in the bases: Bdenf, Cinahl, Lilacs, Scielo and Redalyc. Among the nine references analyzed were drawn three thematic categories: Experiences of nursing care with the use of acupuncture; Ethical-legal aspects of the use of acupuncture as a tool for the work of nurses and The insertion of acupuncture in the nursing curriculum. Were declined the needs about research in the practice of nurses and acupuncture, as well as expansion in training, since graduation to post-graduate classes, as well the increase of its offer in the Unified Health System (AU)

Objetivos. Valorar la efectividad global de la acupuntura como terapia complementaria en la reducción del dolor crónico en las cefaleas resistentes a terapias habituales en grupos apareados de pacientes en condiciones de práctica clínica real. Conocer en qué proporción disminuye el consumo de analgésicos en estos pacientes tras un ciclo de acupuntura. Determinar las reacciones adversas debidas a la acupuntura. Material y métodos. Estudio cuasi-experimental en 56 pacientes ambulatorios con cefalea refractaria a tratamiento farmacológico convencional. Medición con una escala múltiple de valoración del dolor antes de iniciar el ciclo de tratamiento con acupuntura, después de realizar diagnóstico según la medicina tradicional china, y al terminar. Resultados. Se ha obtenido un nivel de significación p<0,001 para el valor total de la escala de dolor y para cada una de sus variables (intensidad, frecuencia, consumo de analgésicos, discapacidad y sueño) antes y después del tratamiento con acupuntura. Tras la aplicación de un ciclo de acupuntura, el total de la escala de dolor disminuyó en un 50,8%; la intensidad, en un 48,3%; la frecuencia, en un 45%; el consumo de analgésicos, en un 48,5%; la discapacidad, en un 47%; el sueño mejoró en un 66,8%. No se observaron efectos adversos de la acupuntura. El 56% de las cefaleas fueron migrañas. El 14% de los sujetos presentaba intolerancia a fármacos. Conclusiones. La acupuntura es un procedimiento que consume escasos recursos y es altamente efectiva en el tratamiento complementario de la cefalea crónica refractaria a tratamiento. La reducción en la ingesta de fármacos en pacientes con dolor se traduce en un incremento de la calidad de vida, disminución de efectos secundarios, reducción del coste para el sistema sanitario y aumento de la satisfacción del usuario en pacientes con cefalea crónica refractaria (AU)


Objectives. We performed a quasi-experimental study in 56 outpatients with headache refractory to conventional pharmacological treatment. Pain was measured using a multiple-item pain scale before the acupuncture cycle was started, after the traditional Chinese medicine diagnosis was made, and when the treatment cycle was complete. Results. Significant differences (p<001) before and after acupuncture treatment were obtained for the overall pain score and for each of its variables (intensity, frequency, analgesic consumption, disability, and sleep). After the acupuncture cycle, the total pain score decreased by 50.8%, intensity by 48.3%, frequency by 45%, analgesic consumption by 48.5%, disability by 47%, and sleep improved by 66.8%. No adverse effects of acupuncture were observed. Fifty-six percent were migraine. Fourteen percent of the patients showed drug intolerance. Conclusions. Acupuncture consumes few resources and is highly effective in the complementary treatment of chronic refractory headache. The reduction in analgesic consumption in patients with pain increases quality of life and patient satisfaction, and reduces adverse effects and healthcare costs in patients with chronic refractory headache (AU)

El síndrome de la cirugía fallida de espalda es una complicación frecuente posquirúrgica que cursa con lumbalgia crónica con gran repercusión clínica y económica. Las posibilidades de tratamiento abarcan una gran variedad de técnicas y fármacos, siendo pese a ello una de las principales causas de dolor lumbar de más difícil tratamiento. Cada vez hay más pruebas sólidas de que la acupuntura puede ser un complemento útil al tratamiento convencional para el tratamiento de la lumbalgia. Se presenta el caso de un paciente de 37 años intervenido hace 2 años de artrodesis L4-L5,con reintervención posterior hace 1 año de retirada del material de osteosíntesis persistiendo con síndrome poslaminectomía no controlado con cicatriz disestésica pese a tratamiento con opiáceos débiles y anticonvulsivantes que se trató adecuadamente con sesiones de acupuntura y moxibustión. Como pruebas complementarias tenía una resonancia magnética en la que se aprecian además de cambios postquirúrgicos en la región lumbar posterior, deshidratación discal en L5-S1 y cambios degenerativos artrósicos de las pequeñas articulaciones interapofisarias a nivel de los últimos niveles lumbares, un estudio electroneurofisiológico en el que se observa una alteración en los potenciales evocados somestésicos realizados en el nervio fémorocutáneo lateral izquierdo, compatible con una neuropatía axonal de dicho nervio. El tratamiento se realizó en 10 sesiones durante 5 semanas, en cada una de las sesiones se hizo un tratamiento bifásico de 40min, tratando en primer lugar la zona cicatricial y a continuación la lumbalgia, en ambos se realizó moxibustión con puro de Artemisa con técnica de picoteo. El dolor fue controlándose pasando de una escala EAV inicial de 8 a una escala EAV al final del tratamiento de 0. Tras pasar revisión a los 6 meses continua con una escala EAV de 0 (AU)


Failed back surgery syndrome is a common post-surgical complication which leads to chronic low back pain with great clinical and financial repercussions. The treatment possibilities include a great variety of techniques and drugs, despite this being one of the main causes of a very difficult to treat lumbar pain. There is increasing solid evidence that acupuncture may be a useful complement to conventional treatment for treating low back pain. A case is presented of a 37 year-old patient surgically intervened 2 years ago for L4-L5 arthrodesis, with subsequent intervention 1 year ago to remove the osteosynthesis material. There was a persistent uncontrolled post-laminectomy syndrome with a dysesthetic scar despite treatment with weak opiates and anticonvulsants, which was treated adequately with sessions of acupuncture and moxibustion. A magnetic resonance imaging scan was performed as a complementary test, in which post-surgical changes could also be seen in the posterior lumbar region, disc dehydration in L5-S1, and degenerative arthrosis of the small interapophyseal lumbar joints. An electro-neurophysiological study showed a change in the somesthetic evoked potentials performed in the left lateral femoral-cutaneous nerve, compatible with an axonal neuropathy of that nerve. Treatment was given in 10 sessions over 5 weeks, in with a biphasic treatment of 40min being performed in each of them. The scar area was treated first and then the low back pain, moxibustion was performed on both with Artemis smoke and a pecking technique. The pain was controlled, going from a initial VAS score of 8 to 0 at the end of treatment. At the follow-up review at 6 months it was still 0 on the VAS scale (AU)

La acupuntura es una terapéutica de la medicina tradicional china utilizada desde hace más de 3.000 años. Recientemente ha ido ganando popularidad entre pacientes y médicos en el mundo occidental. Investigaciones en ciencias básicas aclaran los mecanismos fisiológicos de la acupuntura. Múltiples investigaciones muestran que la acupuntura desencadena una respuesta neuroendocrina que activa cascadas de opioides endógenos. Numerosos estudios clínicos controlados y aleatorizados, y revisiones sistemáticas han intentado evaluar la eficacia clínica de la acupuntura. La evidencia derivada de estos trabajos avala la acupuntura como un tratamiento eficaz para el dolor dental, las náuseas y vómitos de los posoperatorios y de la quimioterapia. Para otras condiciones, los resultados todavía no son concluyentes dado que la investigación clínica en acupuntura plantea retos metodológicos que están en proceso de ser superados. La acupuntura es una terapéutica segura en manos entrenadas. La comunidad médica se está abriendo a la utilización de la acupuntura como una terapéutica no farmacológica complementaria a la práctica médica habitual. Son necesarios más estudios que nos permitan ir esclareciendo las dudas y que sirvan de puente entre este antiguo arte de curar y los sistemas modernos de salud (AU)


Acupuncture is an important therapy from traditional Chinese medicine that has been used for more than 3,000years. Recently, the popularity of this technique has been increasing among patients and physicians in Western countries. Basic-science research has clarified the physiological mechanisms of acupuncture. It has been shown in many investigations that acupuncture precipitates a neuroendocrine response that activates cascades of endogenous opiates. Numerous randomized, controlled trials and systematic reviews have evaluated the clinical efficacy of acupuncture. The evidence obtained from these works have demonstrated that acupuncture is an effective treatment for dental pain, postoperative nausea and vomiting and chemotherapy-related nausea and vomiting. Evidence is non-conclusive for other conditions since clinical research in acupuncture poses methodological challenges that must still be overcome. Acupuncture is relatively safe when performed by experts. Health care providers are changing their view on the use of acupunture as a non-drug treatment that is complementary to the conventional medical approach. More studies are needed in order to help us clarify the doubts and to serve as a link between this ancient healing art and modern medicine (AU)

Introducción. En el tratamiento del dolor miofascial (DMF) se han recomendado diversos tipos de infiltración de puntos gatillo (PG). El propósito de este trabajo es comparar la eficacia y los efectos evolutivos de dos modalidades: punción seca (PS) e infiltración de anestésico local (IAL). Material y método. Se presenta un estudio observacional de casos y controles, en el que se incluyeron de forma consecutiva 24 pacientes con DMF, 15 tratados con PS y 9 con IAL. Se estudiaron las siguientes variables: dolor, mediante la escala analógica visual (EAV), en reposo y en esfuerzo, y el umbral doloroso mediante el algómetro de presión. Se valoró también la EAV durante la técnica y a los 20 minutos. Resultados. Se estudiaron 22 mujeres y dos varones con una edad media de 48 años. Todos los pacientes tratados mejoraron sus parámetros de dolor en reposo y al esfuerzo (p < 0,01). No hubo diferencias significativas entre ambas técnicas en cuanto a la mejora de la EAV. La EAV en reposo mejoró un porcentaje medio del 35 %, y un 33 % el de esfuerzo. La mejora del umbral de dolor fue mayor en los pacientes tratados con PS (p = 0,04). No hubo diferencia en las molestias sentidas durante la terapia, ni en el efecto inmediato del tratamiento. Discusión. Tanto la PS como la IAL son efectivas en la inactivación del PG, con efecto analgésico semejante. Sin embargo hay una cierta tendencia de la PS a mejorar más el umbral doloroso, hecho que sugiere mayor capacidad de inactivación del PG. Se concluye así que ambas técnicas pueden utilizarse para el alivio del dolor en aquellos pacientes diagnosticados de DMF


Introduction. Several kinds of trigger point injection have been recommended for myofascial pain syndrome. The aim of this study is to compare the efficacy and evolutive effects of two types of myofascial treatment: dry needling and local anaesthetic injection (LAI). Patients and methods. We included 24 myofascial pain patients, 15 treated by dry needling and 9 treated by anaesthetic local injection. The following variables were studied: resting and active pain by Visual Analogical Scale (VAS); pain threshold was measured by pressure algometer; discomfort during technique and immediate effect, at 20 minutes (VAS). Results. Twenty two females and 2 males with a mean age of 48 years, were treated. Patients improved resting and active pain level (p < 0.01). Pain threshold improved more in dry needling group (p = 0.04). Percentage transformed VAS improved 35 % resting and 33 % in activities. No differences were observed between groups in discomfort during therapy or in immediate effect of the treatment. Discussion. Dry needling and anaesthetic injection were effective for trigger point release. We observed more improvement on pain threshold in dry needling group, which suggests that technique is better to release trigger point. We concluded both techniques were useful to alleviate pain in myofascial syndrome patients

La aplicación de ventosas o cupping es una práctica medicinal muy extendida en los países orientales. La presencia de áreas circulares de eritema, equimosis o ampollas de contenido hemático distribuidas de manera simétrica en hombros, espalda, tórax o área lumbar deben sugerir la utilización de esta técnica. Los seguidores de la medicina tradicional china están aumentando en el mundo occidental, por lo que debemos conocer estas prácticas para prevenir conflictos sociales y/o legales que pueden provenir de diagnósticos erróneos de malos tratos. Presentamos el caso de un varón de 65 años con múltiples lesiones circulares, eritematosas y ampollosas, de distribución simétrica, producidas tras la aplicación de ventosas en el contexto de una polimialgia reumática


The application of suction cups or cupping is a medicinal practice that is very widespread in Asian countries. The presence of circular areas of erythema, ecchymosis or blood blisters symmetrically distributed on the shoulders, back, thorax or lumbar area should suggest the use of this technique. The number of followers of traditional Chinese medicine is increasing in the Western world, so we should be familiar with these practices in order to prevent social and/or legal conflicts that may arise from mistaken diagnoses of abuse. We present the case of a 65-year-old male with multiple circular, erythematous, bullous lesions, symmetrically distributed, which occurred after the application of suction cups in the context of polymyalgia rheumatica
