Violence is a public health problem, and when it affects childhood, it can cause illness throughout the individual's life. Apart from being able to cause damage in the physical, mental and social spheres, it represents a violation of the rights of the affected children, and a high consumption of resources, both economic and social.A multitude of investigations have improved attention to this violence. However, these advances are not consistent with the practical management of victims, both in Primary and Hospital Care. There is a significant area of improvement for paediatric care.Through this article, different professionals from all established paediatric health care facilities develop general lines of knowledge and action regarding violence against children. An overview is taken of the legislation related to childhood, the different types of abuse that exist, their effects, management and prevention. It concludes with an epilogue, through which we aim to move sensibilities. In summary, this work aims to promote the training and awareness of all professionals specialized in children's health, so that they pursue the goal of achieving their patients’ greatest potential in life, and in this way, to help create a healthier society, with less disease, and more justice.(AU)

RESUMO

Background: Experiences of childhood psychological maltreatment have been found to be associated with various mental health outcomes, and this association persists into adulthood. Objective: This study investigated whether some types of psychological maltreatment are more harmful than others; whether the harms associated with different types of psychological maltreatment are generalized or specific to particular domains of psychopathology; and whether the associations vary by gender. Method: Participants (N = 544, 63.9 % mother as primary caregiver) were Chinese adults from various regions in China. Participants completed measures of childhood psychological maltreatment experiences perpetrated by their primary caregiver and the mental health outcomes of depression, anxiety, anger, physical aggression, and hostility. The data were analyzed in a hierarchical model in which depression and anxiety were defined as indicators of an internalizing factor, while anger, physical aggression, and hostility were defined as indicators of an externalizing factor. Internalizing and externalizing then defined a higher-order general psychopathology factor. The results suggested equivalent harms of psychological abuse and psychological neglect. Further, the associations between psychological maltreatment and mental health were not unique to specific symptom domains but showed broadband associations with general psychopathology. Results: These findings suggest that trans-diagnostic interventions may be the most effective approach for addressing the mental health impacts of psychological maltreatment. Conclusion: Childhood psychological maltreatment may pose a broadband risk for any and all forms of psychopathology.(AU)

RESUMO

Childhood trauma is a leading risk factor for adolescents developing major depressive disorder (MDD); however, the underlying neuroimaging mechanisms remain unclear. This study aimed to investigate the association among childhood trauma, MDD and brain dysfunctions by combining static and dynamic brain network models. We recruited 46 first-episode drug-naïve adolescent MDD patients with childhood trauma (MDD-CT), 53 MDD patients without childhood trauma (MDD-nCT), and 90 healthy controls (HCs) for resting-state functional magnetic resonance imaging (fMRI) scans; all participants were aged 13–18 years. Compared to the HCs and MDD-nCT groups, the MDD-CT group exhibited significantly higher global and local efficiency in static brain networks and significantly higher temporal correlation coefficients in dynamic brain network models at the whole-brain level, and altered the local efficiency of default mode network (DMN) and temporal correlation coefficients of DMN, salience (SAN), and attention (ATN) networks at the local perspective. Correlation analysis indicated that altered brain network features and clinical symptoms, childhood trauma, and particularly emotional neglect were highly correlated in adolescents with MDD. This study may provide new evidence for the dysconnectivity hypothesis regarding the associations between childhood trauma and MDD in adolescents from the perspectives of both static and dynamic brain topology.

Objective: To describe the extension of the adverse experiences, including victimization, lived in childhood and adulthood in a sample of people with severe mental illness residing in Barcelona, Spain. Method: Descriptive, quantitative and cross-sectional study. Adverse and victimization experiences were assessed using 26 items from the Adverse Childhood Experiences Questionnaire (ACE-IQ). Items were included obtain information regarding victimization during adulthood. Seventy-four people with severe mental illness were interviewed (median age: 42.03 years; standard deviation: 9.60). Results: All participants reported at least two victimization experiences throughout their lifetime. The most frequent experiences were conventional crimes (87.8%) and victimization by caregivers (86.5%). Before age 18, half of the participants reported having experienced physical violence by their caregivers (52.7%) and almost one out of three reported to have been victim of sexual abuse (32.4%). Women were almost 13 times more likely to experience sexual victimization than men throughout their lifetime (odds ratio: 12.75; 95% confidence interval: 4.19–38.71). Conclusions: Victimization experienced by people with severe mental illness is a widespread problem that has received little attention. The results of this study are consistent with those obtained in previous investigations and underscore the need to translate this knowledge into medical practice into medical practice, considering them in order to develop treatments with a comprehensive approach to mental health.(AU)

Traumatic experiences are destructive experiences that affect in-dividuals in their adulthood, damaging their sense of trust towards them-selves, the world and other people, leading them to despair. This study aims to test the effect of childhood traumatic experiences on attachment styles in adulthood and to find out what kind of traumatic experiences ex-plain the attachment styles to what extent. Relational survey method was used in the research and the data were analysed with structural equation modelling. The study group is 804 students aged 17-24.For data collec-tion, Childhood Trauma Questionnaire developed by Bernstein et al. (1997) andRelationship Scales Questionnaire developed by Griffin and Bartholomew (1994) were used. The study has concluded that traumatic experiences have a negative and significant effect on secure attachment and a positive effect on fearful and preoccupied attachment, but they are not a significant predictor of dismissing attachment. As a result of the sec-ond model of the relationship between traumatic experiences and secure and insecure attachment dimensions, traumatic experiences have been found to have a negative significant effect on secure attachment but a posi-tive significant effect on insecure attachment. With the third model, three types of traumatic experiences have been examined to explain the attach-ment styles. Physical, emotional neglect and abuse, and sexual abuse have predicted secure attachment in a negative significant way and insecure at-tachment style in a positive significant way. The study revealed the effect of childhood traumatic experiences on attachment styles.(AU)

Early caregiving experiences set the stage for children’s developmental trajectories. Children who experience earlyadversity are more likely to show difficulties regulating their behaviors, emotions, and physiology than children whodo not experience adversity. Parenting interventions designed to enhance parental sensitivity and responsivenesscan buffer children from the detrimental effects of early adverse experiences and ultimately enhance child outcomes. One such intervention is Attachment and Biobehavioral Catch-up (ABC), an evidence-based home visiting programdeveloped to enhance parental sensitivity and responsive care. In this paper, we review the intervention objectivesand effectiveness of ABC. We then describe efforts made to supervise the fidelity and dissemination of ABC,including its implementation in diverse cultural contexts and with Spanish-speaking families in the United States.Lastly, we discuss the potential of ABC as a novel intervention to be implemented within the child welfare systemin Spain and Latin America.(AU)

INTRODUCTION: children have feelings and experiences that they cannot or will not express and that may manifest through their drawings and imagination. The Asociación Española de Pediatría de Atención Primaria (Spanish Association of Primary Care Pediatrics, AEPap) wanted to explore how children have experienced the confinement. MATERIAL AND METHODS: we organized a contest through the Family and Health website. The study population consisted of children residing anywhere in Spain aged 3 to 16 years. The works submitted were drawings, micro stories or micro videos. RESULTS: we received 53 works from children in different autonomous communities and gave 5 awards. The elements featured most frequently were the coronavirus, figures of children indoors, a rainbow in the clouds and health care professionals. Some of the subjects expressed in the works were boredom, missing grandparents and friends, trust in health care professionals, a positive perception of the change in life in the family, the improvement in environmental pollution and a positive view that everything will turn out well. CONCLUSIONS: the results obtained in this study suggest a generalized optimism and allow us to conclude that drawings, stories and videos are a useful tool to analyze the perception of this population in risk situations. We also ought to highlight the interest of primary care paediatricians in how children have experienced the confinement imposed due to the coronavirus pandemic and raise awareness that children's drawings should be carefully studied

Attachment relationships and upbringing during childhood condition emotional development and incidences of mental disorders or lack thereof. Some adverse experiences in childhood are considered general health risk factors and mental health risk factors in particular. Said adversities are known to be reflected in the physiological system and to provoke epigenetic changes in the DNA. A retrospective study was conducted in a sample of 29 patients who had been diagnosed with severe mental disorders during adulthood. The majority of them were male, and their average age was 25.2 years old. Risk factors found at higher rates were: mother’s depression and/or stress during pregnancy, early and prolonged separations of care figures, and emotionally absent fathers. These adversities affect the processes of attachment and relationships within the family. We conclude that these kinds of studies are necessary, starting in childhood, to detect and prevent alterations in attachment relationships that will cause emotional and physiological changes that will condition the person’s mental health

RESUMO

BACKGROUND: The experience of coping with negative events in early ages (childhood and adolescence) has consistently been linked to some specific deviant behaviors, such as juvenile justice involvement or persistence in crime. In contrast, very few studies have focused on the link between Adverse Childhood Experiences and altruistic behavior. The objective of this study is to examine the possible influence of Adverse Childhood Experiences on the social behavior exhibited in emerging adulthood (specifically deviant and altruistic behavior). METHOD: The study population consisted of 490 young adults between the ages of 18 and 20, with a mean of 18.90 years (SD = .77). All voluntarily completed the following self-report questionnaires: the Adverse Childhood Experiences questionnaire, the Deviant Behavior Scale, and the Altruistic Scale. RESULTS: Linear regression models found that Adverse Childhood Experiences were strong, positive predictors of deviant behaviors. Moreover, specific Adverse Childhood Experiences (physical abuse for deviant behavior, and emotional neglect for altruistic behaviors) had notable, differential effects. CONCLUSIONS: The prevention or early detection of Adverse Childhood Experiences during childhood could contribute to reducing maladaptive patterns of behavior and to increasing altruistic patterns during emerging adulthood

Background: The Early Life Experiences Scale (ELES) evaluates the memory of threat and subordination perceived in childhood family relationships based on social rank theory. This scale was adapted to Spanish, its factor structure was tested by cross-validation, and its psychometric properties were explored. Method: 960 subjects participated (863 subjects from the general population and 97 patients). Results: Data confirm the factor structure of the original version by Authors (2003), obtaining a similar model with three factors: submission, threat, and devaluation, which explained 66.31% of the variance. The Spanish version of the ELES was comprised of the same number of items as in the original study and showed adequate indices of validity, internal consistency, retest and combined reliability. Conclusions: The Early Life Experiences Scale (ELES) can be a useful measure for evaluating early life experiences. Its application may be very relevant in studying the relationship between these experiences and psychopathological manifestations in constructing explanatory models

(AU)

Conceptually this paper is framed within the thinking of numerous psychoanalysts, from Freud, Klein, Winnicott, Milner and León and Rebeca Grinberg, among others, to the current conceptualizations of Roussillon on the role of the processes of symbolization in psychic constitution. A moving analytic experience with a four-year-old girl who was made homeless before her birth is illustrated. This clinical material provides a detailed analysis of how little Milagros gradually achieves the beginnings of the capacity for representation, especially of the unsymbolized traces underlying her severe eating disorder, perceived as a “message-bearing act” for painful experiences suffered during her early years of life. Vignettes from the sessions are presented, through which the analyst was able to make contact with transference demands of the patient’s very early psychic needs, but also with the intense countertransference experience elicited by the young girl from their very first encounter. Several of the themes explored are related to access to unsymbolized traumatic experiences, their site of inscription in the psycho-soma, the inability to access them as memories and how they are treated via the specific technique of child psychoanalysis.
