RESUMO

Fundamento Las Islas Canarias reciben migrantes de origen subsahariano que llegan a la costa tras largas travesías marinas en condiciones adversas. El «pie de patera» es una entidad previamente descrita consistente en un cuadro clínico desarrollado a partir de heridas en los pies que se infectan por estar en contacto con aguas fecales estancadas en el fondo de estas embarcaciones. Describimos un nuevo cuadro clínico, hasta ahora no publicado, consistente en un edema masivo de las extremidades que asocia necrosis de la piel y del tejido celular subcutáneo, respeta el músculo y su etiología no es de origen infeccioso. Material y métodos Diseñamos un estudio descriptivo observacional entre los meses de septiembre del año 2020 y enero del año 2022, ambos incluidos, en el ámbito del hospital de tercer nivel situado en Gran Canaria. Se incluyeron un total de 86 pacientes en una base de datos donde se analizaron 39 variables cualitativas y cuantitativas. Resultados Un total de 16 pacientes desarrollaron el cuadro consistente en una celulitis necrosante estéril. Su fisiopatogenia difiere de la del pie de patera, ya que en todos los casos los cultivos resultan estériles. Conclusiones Como mecanismo responsable, postulamos una teoría inflamatoria derivada del efecto osmótico de la ingesta de agua de mar y/o la reposición agresiva de fluidos a su llegada a los centros hospitalarios donde ingresan con deshidrataciones hipernatrémicas graves. El tratamiento quirúrgico precoz consiste en evacuar el edema mediante incisiones tipo escarotomías, el cual alivia la sintomatología y previene la progresión del cuadro (AU)


Background In the past few years, the Canary Islands received immigrants from sub-Saharan countries that arrive to the coast after long boat trips in extreme adverse conditions. The named «trench foot» is a previously described infectious entity developed after feet wounds that get infected by being in contact with water, urine and excrements in these small and crowded boats. We describe a new clinical entity, not published yet, that consists in massive edema in the extremities associating necrosis of the skin and subcutaneous tissue that characteristically respects the muscle and with a non-infectious etiology. Material and methods A database including 86 patients arrived by boat («patera») from sub-Saharan countries from September 2020 to January 2022 was made and 39 qualitative and quantitative items were analyzed. The Research Unit performed an observational prospective statistical analysis. Results A total of 16 patients developed the entity described as necrotizing cellulitis. Its physiopathology completely differs from the one described in the trench foot, since all the cultures resulted sterile. Conclusions We postulate an inflammatory theory due to the osmotic effect from ingesting sea water and/or the aggressive fluid reposition when they arrive to the emergency room with severe dehydration and hypernatremia. Early surgical evacuation of the edema with escharotomies incisions alleviate the symptoms and prevents progression of the disorder (AU)

En 1966, 137 toneladas de mineral de uranio español llegaron al puerto de Nueva Orleans procedentes del puerto de Cádiz. En este artículo quiero utilizar este viaje que conectó agentes políticos, industriales y empresariales con capacidades y experiencias técnicas, para explorar el uranio como un objeto híbrido —físico, técnico y diplomático—. El uranio conectó los intereses de las autoridades franquistas con los de los científicos y empresarios de la industria eléctrica. Sirvió al régimen de Franco para formar investigadores en nuevos métodos y prácticas experimentales y también para popularizar las políticas e ideologías atómicas de Estados Unidos. Al tiempo que alimentó los reactores nucleares, el uranio generó nuevos espacios disciplinares, modificó paisajes y diseñó nuevas cartografías industriales y administrativas. Además de ser una pieza clave en la política exterior del régimen franquista —le sirvió al régimen español para alinearse con el pensamiento occidental imperante sobre los usos civiles de la energía nuclear—, internamente sirvió para abandonar los discursos autárquicos y reforzar el poder de los tecnócratas, que utilizaron la energía atómica para transformar la producción eléctrica española. Este viaje cambió la materialidad del mineral de uranio, su física y su química, y también sus significados. (AU)


In 1966, 137 tons of Spanish uranium ore arrived at the port of New Orleans from the port of Cádiz. In this paper, I want to use this trip, which involved political, industrial, and business agents as well as technical capabilities, to explore the uranium as a hybrid object - physical, technical and diplomatic. This material connected the interests of the Franco authorities, scientists, and businessmen from the electrical industry. It served the Franco regime in training researchers, in buying and importing technologies, methods and experimental practices, and in introducing ways to popularize atomic policies and ideologies from the United States. Uranium was a fuel that not only powered nuclear reactor but also fed new disciplinary spaces, modified landscapes, and sketched new industrial and administrative cartographies. Besides being a key piece of the Franco regime’s foreign policy —it was useful to the Spanish regime to align itself with prevailing Western thought on the civil uses of nuclear energy— it served internally to effectively abandon autarchic discourses and strengthen the power of the technocrats, who used atomic energy to transform Spanish electrical production. The materiality of uranium ore changed with travel and also its meanings. (AU)

Background/Objective: This study explored the association between active school travel (AST) and suicide attempts among adolescents in low- and middle-income countries (LMICs). Method: We used the data from the Global School-based Health Survey, including 127,097 adolescents aged 13-17 years from 34 LMICs. A self-reported survey was used to collect data on AST and suicide attempts as well as some variables. Multivariable logistic regression was performed to assess the association between AST and suicide attempts. A meta-analysis with random effects was undertaken to identify the difference in the association between AST and suicide attempts. Results: Across all the adolescents, the prevalence of AST was 37% and the prevalence of suicide attempts was 11.60%. Adolescents who engaged in AST were less likely to have suicide attempts irrespective of gender. The country-wise analysis indicated a large inconsistency in the association between AST and suicide attempt across the countries. Conclusions: AST would appear to be a protective factor for reducing suicide attempts among adolescents. However, the association between AST and suicide attempts varied greatly across the countries. Future studies should confirm the association between AST and suicide attempts. (AU)


Objetivo: Se exploró la asociación entre desplazamientos escolares activos (AST, por sus siglas en inglés) e intentos de suicidio entre adolescentes en países de ingresos bajos y medios. Método: Se utilizaron datos de la Global School-based Health Survey, que incluyó a 127.097 adolescentes de 13 a 17 años de 34 países de ingresos bajos y medios. Se utilizó una encuesta autoinformada para recopilar datos sobre AST e intentos de suicidio, así como otras variables. Se realizó una regresión logística multivariable para evaluar la asociación entre AST e intentos de suicidio. Se realizó un metanálisis con efectos aleatorios para identificar la diferencia en la asociación entre AST e intentos de suicidio. Resultados: La prevalencia de AST fue del 37% y la prevalencia de intentos de suicidio fue del 11,60%. Los adolescentes que participaron en AST tenían menos probabilidades de tener intentos de suicidio independientemente del sexo. El análisis por países indicó una gran inconsistencia en la asociación entre AST e intento de suicidio. Conclusiones: AST parece ser un factor protector para reducir los intentos de suicidio entre adolescentes. Sin embargo, la asociación entre AST e intentos de suicidio varió mucho entre países. Estudios futuros deberían confirmar la asociación entre AST e intentos de suicidio. (AU)

Una colección de cartas escritas por la enfermera británica con la intención de dar noticias a su familia de su viaje a Egipto; cartas que ofrecen datos históricos, culturales, antropológicos, opiniones personales a veces controvertidas y polémicas, y también comentarios sobre la crisis espiritual por la que pasaba en esos momentos de su vida. Una obra importante para entender las decisiones que tomó sobre su futuro en la década de los cincuenta del siglo XIX


In this article we have studied "Letters from Egypt 1849-1850" by Florence Nightingale (1820-1910). A collection of letters written by the British nurse with the intention of giving her family news about her trip to Egypt; letters that offer historical, cultural, anthropological data, personal opinions sometimes controversial and polemic, and also comments on the spiritual crisis that she was going through at those moments in her life. An important work to understand the decisions she took in the 1850s about her future

Este artigo centra-se no vasto e complexo processo científico e institucional ligado à recolha de artefactos e à formação de colecções científico-naturais, etnológicas e antropológicas, provenientes de vários espaços coloniais extra-europeus em Portugal durante a segunda metade do século XVIII. Faz ainda referência às instituições que albergaram estes materiais. Neste período em que a ciência era, por definição, útil e devia servir ao interesse público na prossecução do bem-estar, do progresso e da felicidade dos povos, os estudos produzidos tiveram como objectivo contribuir para um levantamento científico com pretensões enciclopedistas, destinado a produzir um conhecimento minucioso dos espaços imperiais através da identificação, recolecção e estudo das produções naturais e das "características físicas e morais" dos indígenas. A curiosidade científica que caracterizou os grupos sociais envolvidos neste processo permitiu a corporização dum conjunto de práticas que era pensado em Lisboa e implementado pelos vassalos da coroa em todo o império. Deste conjunto de práticas, quero destacar as viagens filosóficas, explorações científicas vocacionadas para a descrição física e económica dos territórios e para a inventariação dos recursos naturais, consideradas como instrumento de modernização política e administrativa do império numa altura em que a ciência e a técnica eram vistas como ferramentas imprescindíveis ao desenvolvimento do Estado Moderno


This article focuses on the vast and complex scientific and institutional process related to the collection of artefacts and the formation of scientific-natural, ethnological and anthropological collections from various extra-European colonial spaces in Portugal during the second half of the eighteenth century. It also addresses the institutions that housed these materials. In a period when science was, by definition, useful and should serve the public interest in the pursuit of welfare, progress and the happiness of peoples, scientific studies had the objective of contributing to a scientific survey with encyclopaedic pretensions of the imperial spaces through the identification, collection and study of the natural productions and the "physical and moral characteristics" of the natives. The scientific curiosity that characterized the social groups involved in this process allowed the embodiment of a set of practices that was thought in Lisbon and implemented by Portuguese subjects throughout the empire. From this set of practices, I would like to highlight the philosophical voyages, scientific explorations aimed at the physical and economic description of the territories and the inventory of natural resources, considered as an instrument of political and administrative modernization of the empire at a time when science and technology were considered as essential tools for the development of the Modern State

Introducción. El objetivo fue analizar los síndromes febriles en pacientes que regresan de regiones tropicales, atendidos en la Unidad de Enfermedades Infecciosas y Medicina Tropical (UEIMT) de nuestro centro. El conocimiento de estas enfermedades persigue realizar un diagnóstico precoz y, de esta forma, evitar la emergencia y diseminación de enfermedades importadas, así como establecer actividades preventivas adecuadas. Métodos. Se realizó un estudio observacional descriptivo transversal, seleccionando de la cohorte de viajeros atendidos en la UEIMT entre 1998 y 2013 aquéllos que consultaban al regreso con fiebre (n = 247). Se analizaron las características demográficas relativas al viaje, así como motivos de consulta y el diagnóstico final. Resultados. Se incluyeron 241 viajeros con fiebre al regreso. Los diagnósticos más comunes fueron: fiebre autolimitada sin foco (29,1%), malaria (25,1%), gastroenteritis invasiva (15,4%), otras infecciones bacterianas (7,4%) e infecciones parenquimatosas genitourinarias (4,5%). De todos los casos sólo un 8,5% requirió ingreso hospitalario, no registrándose ningún caso de mortalidad. Conclusiones. En la serie presentada existen datos comunes con otras publicadas (malaria como enfermedad fundamental, dificultades en el diagnóstico) y otros diferentes (menor número de consultas pre-viaje e ingresos, mayor incidencia de prostatitis y menor número de casos de dengue) (AU)


Background. The aim was to analyse those syndromes with fever in travellers who return from tropical regions evaluated at our Department of Infectology and Tropical Medicine. Clarifying these conditions intends to reach earlier diagnoses and, perhaps, to avoid the emergence and spread of imported diseases, as well as to establish appropriate preventive measures. Methods. An observational descriptive transversal study was performed, selecting those patients who returned from tropical regions with fever, amongst all travellers evaluated in our department between 1998 and 2013. We analysed demographic features, information related to the trip, as well as the presenting complaints and the diagnoses. Results. A total of 247 international travellers presenting with fever were analysed. The most common diagnoses were self-limiting fever (29.1%), malaria (25.1%), invasive gastroenteritis (15.4%), other bacterial infections (7.4%) and genitourinary parenchymal infections (4.5%). Of all the cases, only 8.5% required hospitalization, and no case of mortality was recorded. Conclusions. In the series presented, there are common data with other published data (malaria as a fundamental disease, difficulties in diagnosis) and different ones (less number of pre-trip consultations and admissions, higher incidence of prostatitis and fewer dengue cases) (AU)

OBJETIVOS: describir la población a estudio y comparar el motivo de viaje laboral con el motivo turístico y con la Visita de Amigos o Familiares (VFRs) para estimar si existe un perfil de riesgo diferente. METODOLOGÍA: estudio de las variables edad, género, duración, motivo, destino y recomendación de quimioprofilaxis en viajeros que acuden al Centro de Atención Pre-viaje Internacional de Asturias (España) entre 2008 y 2016. Un total de 9.527 (89,83%) viajeros cumplieron el criterio de inclusión (edad ≥18 años). RESULTADOS: en 2013 el motivo laboral constituyó el 43,85% de todas las causas de viaje internacional. La mayoría de los trabajadores (70,89%) fueron hombres (p = 0,00), más jóvenes que los turistas (38 vs 40 años; p = 0,00) o que los VFRs (38 vs 39 años; p = 0,03). Además, los trabajadores viajaban durante más tiempo que los turistas (91 vs 23 días; p = 0,00) o que los VFRs (91 vs 50 días; p = 0,00). África Subsahariana fue la región más visitada por el motivo profesional (35,89%). Se recomendó quimioprofilaxis en 39,19% trabajadores vs 38,72% turistas más VFRs (p = 0,68). Los desplazamientos laborales internacionales por regiones no mostraron diferencia respecto a la recomendación de quimioprofilaxis vs turistas y VFRs. CONCLUSIONES: en la población estudiada el motivo laboral de un viaje internacional tiene un perfil del viajero específico. Se deben desarrollar iniciativas de salud pública para prevenir riesgos concretos y promover el consejo sanitario individualizado


OBJECTIVES: describe the study population and compare the travelling work purpose with recreational and Visiting Friends or Relatives (VFRs) to assess whether there is a different risk health profile. METHODOLOGY: study of the variables age, gender, length of stay, purpose, destination and chemoprophylaxis recommended in travelers attending the Pre-travel Consultation Clinic of Asturias (Spain) between 2008 and 2016. A total of 9,527 (89.83 %) travelers met inclusion criteria (≥18 years old). RESULTS: in 2013 the working purpose was 43.85% of all international travel reasons. Most of the workers (70.89%) were men (p = 0.00), younger than tourists (38 vs 40 years; p = 0.00) or VFRs (38 vs 39 years; p = 0.03). In addition, workers had higher length of stay than tourists (91 vs 23 days; p = 0.00) or VFRs (91 vs 50 días; p = 0.00). Sub-Saharan Africa was the most common region of destination (35.89%). Chemoprophylaxis was recommended in 39.19% workers vs 38.72% tourists plus VFRs (p = 0.68). International workers by regions showed no difference from the recommendation of chemoprophylaxis vs tourists plus VFRs. CONCLUSIONS: in the studied population working purpose for an international travel has a specific profile of the traveler. Public health initiatives should be developed to prevent these travelers from specific risks and promote patient-centered counseling

BACKGROUND: The prevalence of allergic diseases is increasing. We evaluated temporal trends in the prevalence of asthma, rhinitis and eczema in adolescents (13-14 years) living in Taubaté, SP, Brazil (2005-2012) and assessed the relationship between these prevalences and the residential proximity to Presidente Dutra Highway (PDH, a heavily travelled highway). METHODS: This cross-sectional study of adolescents (N=1039) from public and private schools was evaluated using the standard questionnaire of the International Study of Asthma and Allergies in Childhood (ISAAC) plus a question about their place of residence in relation to PDH. The data obtained were compared to the 2005 data using a chi-square test or Fisher's exact test. An analysis by groups consisting of two phases (two-step cluster) was used to evaluate the effect of living near PDH. RESULTS: There was a lifetime increase in the prevalence of active asthma (15.3% vs. 20.4%, p = 0.005) and physician-diagnosed asthma (6.8% vs. 9.2%, p = 0.06) and a decrease in the symptoms of active rhinitis (36.6% vs. 18.5%) between 2005 and 2012. A high frequency of asthma and rhinitis (18.1% vs. 23.2%, respectively) was observed among adolescents living close or very close to PDH; furthermore, 85.6% of the adolescents without symptoms of asthma or rhinitis lived far from PDH. CONCLUSION: An increase in the prevalence of asthma and a decrease in the prevalence of rhinitis were observed during the studied period. Living near PDH was associated with higher rates of asthma, rhinitis, and eczema

BACKGROUND: Medical travel is a general term for all journeys made by a health professional, and are common practice for education and training as a significant globalised topic in modern medical education, and their relevance dates back far into history. METHODS: A historical-documental method is used in this study. It re-examines and discusses critically the J. P. Frank text De medicis peregrinationibus (medical travel) written in 1792. Findings. This paper enlightens on the meaning and usefulness of medical travels towards the end of the 18th century. A critical review of the text is carried out in order to compare medical travel and medical education towards the end of the 18th century with the same travel and education today. The authors discuss a number of relevant points that Frank makes regarding travelling physicians, the meaning and usefulness of medical travels, as well as about a proposed curriculum for travelling physicians that could still be followed today. CONCLUSIONS: As a general conclusion, Frank's paper could be considered a seminal work on medical travel for educational purposes. His observations are still relevant today, reflected in students travelling abroad as part of mobility programmes in medical education


ANTECEDENTES: Los viajes médicos, como término general para todo viaje realizado por un profesional de la salud, son una práctica habitual para su educación y capacitación, que conforma además un significativo tópico globalizado de la educación médica moderna, aunque su relevancia se remonte en la historia. MÉTODO: Esta investigación utiliza un método histórico-documental. Revisa y analiza críticamente el texto De medicis peregrinationibus (Viajes médicos) escrito por J. P. Frank en 1792. Hallazgos: Este artículo explica el significado y la utilidad para el aprendizaje de los viajes médicos hacia finales del siglo xviii. Se lleva a cabo una revisión crítica del texto, tratando de comparar viajes médicos e instrucción médica hacia finales del sigloxviii con los mismos viajes y educación hoy en día. Se discuten una serie de puntos relevantes que Frank realiza con respecto a los viajes médicos, su significado y la utilidad de estos, comentando una propuesta curricular para los médicos viajeros que bien podrían seguirse aún hoy. CONCLUSIONES: Como conclusión general, el texto de Frank podría considerarse una obra fundamental sobre los viajes médicos con fines educativos. Sus observaciones son aún relevantes hoy en día, tal como se refleja en los estudios sobre estudiantes que viajan al extranjero en la actualidad como parte de programas de movilidad en su formación médica

RESUMO

Se estudia la aportación de Alexander von Humboldt a la representación natural que se sitúa entre la visión científica y el sentimiento artístico. Humboldt es considerado como el creador de un ‘arte científico’: la representación artística al servicio de la ciencia. Esta representación iconográfica de la botánica, la zoología y, ante todo, de los restos materiales de las culturas prehispánicas así como su expresión artística, proporcionó una nueva imagen de América que hasta entonces estaba ampliamente dominada por la fantasía europea. En el trabajo se exponen sus criterios para elaborar lo que se ha llamado un arte científico y se profundiza sobre su teoría de la impresión total de la naturaleza. Finalmente, se reflexiona sobre las referencias artísticas que Humboldt pudo incorporar a su sistema de representación y las influencias de éstas en su programa teórico (AU)


This study addresses the contribution of Alexander von Humboldt to the natural representation that is situated between scientific vision and artistic sentiment. Humboldt is considered as the creator of a ‘scientific art’, i.e., artistic representation in the service of science. This iconographic representation of botanic and zoological specimens, and above all of the material remains of Pre-Hispanic cultures and their artistic expression provided a new image of America that had previously been heavily dominated by European fantasy. This paper describes his criteria for developing what he called scientific art and explores his theory of the total representation of nature. Finally, it reflects on the artistic references that Humboldt was able to incorporate into his representation system and their influence on his theoretical program (AU)
