RESUMO

Demineralized freeze-dried bone allograft (DFDBA) contains bone morphogenetic proteins (BMPs), hence is osteoinductive. Autologous platelet concentrates exhibit a higher quantity of growth factors. Both these biomaterials aid in bone regeneration when placed in three-wall intrabony defects. However, their efficacy when used alone and in conjugation is not clear. Aim: To assess clinical and radiographic efficacy of injectable platelet-rich fibrin (i-PRF) with microsurgical access flap in the treatment of three-wall intrabony defects in chronic periodontitis patients. Methods: Thirty sites with three-wall intrabony defects were randomly assigned to control and test group by computer generated method. The test group obtained i-PRF mixed with DFDBA while the control group received only DFDBA. Clinical parameters such as site-specific Plaque index (PI), Radiographic intrabony defect depth (IBDD), modified- Sulcular bleeding index (mSBI), Clinical attachment level (CAL), and Probing pocket depth (PPD) were measured at baseline, three and six months. Results: Intragroup comparison within the control group and test group exhibited statistically highly significant variation of mean PI, mSBI, PPD, CAL, and IBDD score from baseline to 3 months and from 3-6 months (p<0.001). However, intergroup comparison demonstrated no statistically significant variation of mean IBDD at all 3 intervals (p>0.05). Conclusion: i-PRF combined with DFDBA enhanced the radiographic and clinical parameters as opposed to DFDBA alone. The role of i-PRF is promising in its capacity for easy obtainability and increased potential to aid in regeneration

AND "sticky bone" OR "alveolar bone grafting" AND "sticky bone" OR "guided bone regeneration" AND "sticky bone" AND "alveolar ridge augmentation" OR "Alveolar ridge augmentation" AND "sticky bone".

Objective: to review the literature on the different types of autologous platelet derivatives and the clinical performance of using sticky bone for horizontal bone ridge augmentation. Materials and methods: In order to conduct this review, it was conducted searches in the PubMed, Google Scholar, and Web of Science databases using the following descriptors: "platelet-rich fibrin" AND "sticky bone" OR "alveolar bone grafting" AND "sticky bone" OR "guided bone regeneration" AND "sticky bone" AND "alveolar ridge augmentation" OR "Alveolar ridge augmentation" AND "sticky bone". It included articles published in English that addressed concepts related to platelet aggregates and guided bone regeneration for horizontal bone augmentation using platelet-rich fibrin associated with bone grafts (sticky bone). Results: After evaluating the studies found, were selected 11 articles on the use of sticky bone for horizontal ridge augmentation. To compose this work, 14 review studies and articles associated with the topic were also selected. Due to its ease of application and availability, many authors have explored the surgical applications of sticky bone, and the results indicate that horizontal ridge augmentation using this technique can be predictably performed. Conclusion: while there are promising studies on the use of sticky bone, the literature lacks evidence regarding its clinical success. Therefore, to fully understand the regenerative potential of this technique, further randomized studies are needed, involving different graft materials and standardized protocols for obtaining sticky bone.(AU)

(AU)

Soft tissue graft substitutes have been widely used to overcome the limitations of autogenous grafts in the treatment of periodontal and peri-implant soft tissue defects. However, the clinical performance of these biomaterials is still inferior. The biofunctionalization of collagen matrices using injectable platelet-rich fibrin (i-PRF) has been proposed as a strategy to enhance the bioactivity and, therefore, the clinical efficacy of these biomaterials. Thus, the aim of this study was to evaluate the effectiveness of using biofunctionalized volume-stable collagen matrix (VCMX) with i-PRF in the treatment of single gingival recessions (GRs) from clinical, esthetic, and patient-centered parameters. For this purpose, 66 patients with single RT GRs were selected and randomly allocated to one of the following groups: CAF group (n=22), coronally advanced flap (CAF); CAF+VCMX group (n=22), CAF combined with VCMX; and CAF+ VCMX +iPRF group (n=22), CAF combined with biofunctionalized VCMX with i-PRF. After 6 months, all three groups exhibited significant root coverage rates [CAF: 69.1% (2.02 ± 1.06 mm); CAF+FG: 67.44% (1.7 ± 0.81 mm); and CAF+FG+iPRF: 64.92% (1.64 ± 0.80 mm), with no difference between the groups (p=0.33). The groups that received the biomaterials showed a greater gain in keratinized tissue thickness (KTT) (CAF: 0.12 ± 0.2 mm; CAF+FG: 0.43 ± 0.24 mm; CAF+FG+i-PRF: 0.48 ± 0.25 mm; p=0.000). No significant differences were observed in terms of keratinized tissue height in any of the groups and assessed time points (p>0.05). All groups showed a significant reduction in dentin hypersensitivity and improvements in esthetic conditions (p>0.05). No differences were also observed in terms of post-operative pain and morbidity (p>0.05). Within the limitations of this study, it is concluded that all three approaches provided similar and satisfactory results after 6 months of follow-up. The addition of VCMX, whether biofunctionalized or not with i-PRF, provided additional benefits in terms of keratinized tissue thickness gain. (AU)

Tooth extraction is one of the most frequent procedures in oral and maxillofacial surgery and is related to physiological changes in the alveolar process. In this sense, among the main transoperative complications is hemorrhage, which usually occurs through the dental alveolus, due to damage to the blood vessels where the extraction was performed. An alternative to obtain hemostasis is the use of Fibrin Rich in Platelets (PRF), which is a white platelet concentrate obtained from the patient's blood that undergoes a centrifugation step. The membrane obtained after this process is rich in leukocytes, platelets and growth factors that promote modulation of cells involved in the healing process, favoring a better and faster repair of surgical lesions. This fibrin matrix has several uses for dentistry, showing good results, in addition to being low cost and easy to obtain. The aim of this article is to describe the technique for obtaining Platelet-Rich Fibrin (PRF) used as a hemostatic after tooth socket extraction, thus allowing its reproducibility and use.

Objective: To compare four different procedures for processing as well as obtaining PRF, and analyzing their formation through laboratory techniques. The purpose of the study is to validate a method that produces higher quality PRF for oral surgery use in different branches of dentistry. Material and method: The experiment consisted of collecting blood from 12 volunteers, and processing each patient's sample in 4 different ways. In the following two-stage process analysis, the first, quantitative, step was to analyze the samples' platelet-poor plasma (PPP),with a Neubauer Hemocytometer to count blood components. In the second, qualitative step, the sample's PRF were analyzed under microscopy using histological slides. Result: The statistical analysis of the leukocyte, red blood cell and platelet count did not show any significant distinction when comparing different processes. Histological analysis of the PRF showed fibrin network with platelet aggregation, several leukocytes and presence of red blood cells, with double centrifuged samples presenting more white blood cells than the others. Conclusion: Among the analyzed procedures, the sample centrifuged once at 3000 RPM (1900 G) for 10 min showed the best quality PRF.

Objective: This study evaluated the effect of leucocyte and platelet-rich fibrin (L-PRF) techniques used in alveolar ridge preservation on dimensional hard tissue changes of the alveolar ridge assessed using cone-beam computed tomography (CBCT) scans. Material and Methods: Elective alveolar ridge preservation surgeries were performed on seven systemically healthy patients who underwent single posterior tooth extraction. Patients were randomly treated with PRF mixed with a commercial bone xenograft (n = 3) or PRF plug (n = 4). CBCT scans were taken immediately after tooth extraction and then four months later, just before implant surgery. Dimensional alterations in socket height (SH), socket area (SA) and socket volume (SV) were evaluated on CBCT scans by percentage differences before and after treatments. Results: Sockets treated with PRF plus bone xenograft had a tendency to maintain alveolar bone dimensions over time (SH= 11.22% to 82.74%, SA= 1.84% to 48.91% and SV= 9.11% to 203.62%), while sockets treated only with PRF plug technique presented only a modest increase in height (SH= 1.47% to 11.11%) but greater alveolar ridge resorption confirmed by a decrease in socket area and volume dimensions (SA= 0.21% to -24.09% SV= 8.53% to -54.12%). Conclusion: Within the limitations of this study, the maintenance of alveolar socket dimensions was observed when PRF was associated with a xenograft. However, the loss of alveolar ridge dimensions was not entirely prevented by PRF treatment alone.(AU)

Introduction: The use of platelet concentrates for the treatment of complex wounds and tissue regenera-tion is being widely used worldwide. During the last time, the second generation of platelet concentrates, particularly L-PRF, has made it possible to effectively treat patients with this pathology. Due to its low cost and versatility, it has been possible to apply this technique in various clinical situations with good results. The objective of this work is to present our experience using L-PRF for the healing of complex wounds (HCC) as an alternative to the use of grafts of different degrees of complexity. Materials and Method: A prospective analysis was carried out with a series of cases who underwent surgical treatment of complex wounds using L-PRF at Santiago Oriente - Luis Tisné Brousse Hospital, between the months of January 2017 and December 2018. Through clinical examination and previously defined inclusion, success, and failure parameters, a total of 11 patients with complex wounds were evaluated who underwent local treatment with an L-PRF graft. Results: The etiology of the wounds was varied. 8 (72%) of the cases achieved 100% epithelialization and 3 (28%) failed. Predisposing factors for the failure of the technique were identified, and it was also possible to establish a predictive relationship of success where a high probability of epithelialization is related when the granulation of the wound occurs during the first 10 days on the L-PRF graft. Conclusion: The treatment of complex wounds using L-PRF is a feasible alternative, with low cost and requirements (compared to the use of grafts, flaps and dermal substitutes) and safe in the resolution of complex wounds, allowing to reduce morbidity, the costs associated with treatment and hospital stay.

Aims: To describe the clinical and laboratory characteristics of type 2 diabetic patients (DM2) treated with Platelet Rich Fibrin (FRP) at Hospital Clínico Herminda Martín between 2014 and 2016. Materials and Method: Analytical study that includes information from a secondary database. The clinical records of all patients treated with FRP at the Biomaterials Polyclinic were reviewed to search the HbA1c and glycemia values, as well as other clinical and laboratory variables that could be related to the evolution of diabetic foot wounds. Three comparison groups were established according to the healing time in rapid (time 75th percentile). Results: From a universe of 147 patients with DM2, results of 85 (58%) are reported. The average age in the included patients was 61 years. They had an average glycaemia of 243 g/dL, and HbA1c of 9.4%. They presented a platelet count within normal range (average 279 platelets/mm3). Statistical differences between the fast and slow healing groups were found in the wound assessment scale, specifically in the assessment of the total score, extension, depth, and pain. There were no differences in the HbA1c or glycemia level in the fast or slow healing groups, nor in other variables such as age, creatinine, and platelet count. Conclusion: No association was found between healing speed and HbA1c level in individuals treated with FRP.

Introduction: The oroantral communication is a communication between the oral cavity and the maxillary sinus, which has different etiologies, being the most common the extraction of upper posterior teeth, due to the proximity of their root apexes to the floor of the maxillary sinus. Diagnosis is obtained from a combination of anamnesis, physical examination and imaging exams, which may be through radiographs or tomography. There are several approaches to treatment, including the use of Leukocyte- and Platelet-RichFibrin membranes for local obstruction of communication. The aim of this work is to report and discuss the use of a Leukocyte- and Platelet-Rich Fibrin membrane for the management of oroantral communication associated with the reconstruction of the buccal bone plate with Stick Bone. Case report: A 61-year-old woman reported discomfort in the region of right maxillary first molar and a history of extraction of the same for about 8 months. The intraoral evaluation revealed the presence of a hyperemic area in the alveolus of the extraction region, and the tomographic examination revealed an oroantral communication associated with loss of local vestibular bone plate. Final considerations: Therefore, the use of these membranes are suitable for obstructing these communications, being a propper plug due to its adhesive properties in the perforation area, not being associated to any immunological reaction, easy and fast preparation, highly biocompatible, low cost, prevention of the depth of the vestibular sulcus and do not present any risk of infection...

Introduction: Temporomandibular disorders are a group of dysfunctions which affect the temporomandibular joint and/or associated musculoskeletal tissues. Objective: Determine the effectiveness of platelet concentrates in the treatment of temporomandibular disorders. Methods: A bibliographic search was conducted from January 2014 to April 2019 in the biomedical databases PubMed, Embase, SciELO, Scopus, Science Direct, System for Information on Gray Literature in Europe, Latin American and Caribbean Health Sciences Literature, Google Scholar, and Cochrane Central Register of Clinical Trials. The following selection criteria were defined for the studies: randomized clinical trials published in the last five years and reporting on the effectiveness (pain reduction and maximum opening increase) of platelet concentrates in the treatment of temporomandibular disorders. Bias risk analysis was based on the Cochrane manual of systematic reviews of interventions. Results: Nine papers were retrieved, of which 100 percent reported no differences in pain reduction or maximum opening increase resulting from the use of platelet concentrates in the treatment of temporomandibular disorders. Conclusions: The literature review conducted suggests that there is slight evidence of the potential benefits of intra-articular injections of platelet concentrates in patients with temporomandibular disorders. However, a standardized protocol should be established for the preparation and application of these concentrates(AU)
