RESUMO

Objetivo: avaliar do conhecimento dos alunos de odontologia sobre os protocolos de atendimento para as urgências endodônticas. Método: 182 alunos dos últimos anos do curso de Odontologia do Centro Universitário Doutor Leão Sampaio, responderam a um questionário contendo perguntas referente ao protocolo adotado em casos de urgência de origem endodôntica. Os dados foram analisados pelo teste de Qui-quadrado de Pearson (p<0,05). Resultados: foram observadas diferenças entre a conduta relata pelos alunos do quarto e quinto ano de graduação quanto a indicação da incisão para drenagem em abscesso periapical agudo submucoso (evoluído), indicação de antibióticos nos casos de flare-up e indicação de antibióticos na dor com edema póstratamento endodôntico. A prescrição de antibióticos foi excessiva para os casos de dor entre consultas (flareup) e dor no pós-operatória. Para as patologias da polpa, a maioria dos alunos indicou protocolos de urgência recomendados na literatura. Conclusão: os resultados indicam a necessidade de melhoria dos programas de treinamento dos alunos em urgências endodônticas, principalmente quanto aos protocolos farmacológicos. (AU)


Objective: to evaluate the knowledge of dentistry students about care protocols for endodontic emergencies. Method: 182 students from the last years of the Dentistry course at Doctor Leão Sampaio University Center answered a questionnaire containing questions regarding the protocol adopted in urgent cases of endodontic origin. Data were analyzed using Pearson's Chi-square test (p<0.05). Results: differences were observed between the conduct reported by fourth- and fifth-year undergraduate students regarding the indication of incision for drainage in submucosal acute periapical abscess (evolved), indication of antibiotics in cases of flare-up and indication of antibiotics in pain with edema after endodontic treatment. The prescription of antibiotics was excessive for cases of pain between appointments (flare-up) and postoperative pain. For pulp pathologies, most students indicated emergency protocols recommended in the literature. Conclusion: the results indicate the need to improve student training programs in endodontic emergencies, especially regarding pharmacological protocols. (AU)

É sabido que a cavidade oral é a porta de entrada para muitos microrganismos e que possui alguns gêneros de bactérias que quando se associam ou proliferam em excesso, com a falta de higienização correta, fatores ambientais e imunológicos podem causar danos em algumas partes dos dentes como no tecido pulpar e canais radiculares. Neste sentido, a endodontia tem sido capaz de tratar pessoas com problemas dentários em fases muito precoces como as medidas profiláticas até casos infeciosos que afetam a polpa dos dentes causando as lesões perirradiculares que são infecções causadas por microrganismos, Para tanto, é necessário que a odontologia ofereça transformações positivas por meio de técnicas que sejam mais previsíveis para os tratamentos. A literatura tem indicado o uso da laserterapia como técnica auxiliar para o tratamento endodôntico pelo seu potencial mínimo invasivo, preciso e eficaz em menor tempo de cura de patologias orais como as lesões perirradiculares. Este estudo pretendeu abordar a importância da laserterapia sobre as variáveis patológicas de pacientes com lesão perirradiculares. Para realiza-lo foi necessário buscar na literatura estudos que associem este tipo de tratamento e sua eficiência no tratamento desta patologia. Para realizar esta pesquisa foi necessário buscar dados na literatura que tratam sobre o tema para investigar o tempo indicado ao tratamento com a laserterapia bem como o estágio em que as lesões podem ser tratadas com auxílio deste método. Corroborando com o estudo apresentamos um caso clínico(AU)


It is known that the oral cavity is the gateway to many microorganisms and that it has some genera of bacteria that when they associate or proliferate in excess, with the lack of correct hygiene, environmental and immunological factors can cause damage in some parts of the teeth such as in pulp tissue and root canals. In this sense, endodontics has been able to treat people with dental problems at very early stages such as prophylactic measures to infectious cases that affect the pulp of teeth causing perirradicular lesions that are infections caused by microorganisms, therefore, it is necessary that dentistry offer positive transformations through techniques that are more predictable for treatments. The literature has indicated the use of laser therapy as an auxiliary technique for endodontic treatment for its minimal invasive, precise and effective potential in shorter time of cure of oral pathologies such as perirradicular lesions. This study aimed to address the importance of laser therapy on the pathological variables of patients with perirradicular lesions. To accomplish this, it was necessary to seek studies in the literature that associate this type of treatment and its efficiency in the treatment of this pathology. To carry out this research it was necessary to seek data in the literature that deal with the subject to investigate the time indicated for treatment with laser therapy as well as the stage at which lesions can be treated with the aid of this method. Objetivo: Determinar los hábitos de medicación sistémica de odontólogos especialistas y no especialistas en endodoncia ante diferentes patologías pulpares previos al tratamiento en- dodóntico en Argentina. Materiales y métodos: Se diseñó una encuesta para evaluar la prescripción de antibióticos, tipo de antibióticos, tiempo de prescripción, indicación de antinflamatorios no es- teroides y esteroides ante diferentes patologías pulpares. Se envió a 635 odontólogos especialistas y no especialistas en endodoncia a través de SurveyMonkey. Por medio de la prue- ba de Chi cuadrado se evaluaron las diferencias de medica- ción entre los grupos estudiados. Resultados: En pulpitis se medicó con antibióticos en el 3,48% de los casos y con antinflamatorios en un 62,60%. En necrosis pulpar sin fístula no se indicó ninguna medica- ción en un 64,47% de los casos, seguido de antibióticos en un 24,56%. En necrosis con fístula, el 52,38% no indicó nin- guna medicación, seguido de medicación con antibióticos en un 35,49%. En periodontitis apical aguda la principal medica- ción fue con antinflamatorios (52,79%), seguido de antibió- ticos (32,87%); y en el absceso alveolar agudo, un 57,10% indicó antibióticos seguido de antinflamatorios. El antibiótico de elección fue la penicilina en un 65,23% de los casos, y en caso de alergia a la misma, el antibiótico elegido fue azitromi- cina (30,12%). El tiempo de prescripción fue de 7 días. En la comparación entre especialistas y no especialistas hubo dife- rencias estadísticamente significativas para pulpitis y necrosis con fístula (p<0,01) y no las hubo entre necrosis sin fístula, periodontitis apical aguda y absceso alveolar agudo (p> 0,05). Conclusiones: La penicilina fue el antibiótico de elec- ción de la mayoría de los odontólogos argentinos encuestados junto al ibuprofeno como anti-inflamatorio. Existiría una so- bremedicación en patologías endodónticas que podría contri- buir a la resistencia microbiana a los antibióticos (AU)


Aim: Determine the systemic medication habits of den- tists specialists and non-specialists in endodontists in differ- ent pulp pathologies prior to root canal treatment in Argen- tina. Materials and methods: A survey was designed to evaluate the prescription of antibiotics, the type of antibiotics, prescription time, indication of non-steroidal anti-inflamma- tory drugs in different pulp pathologies. It was sent to 635 general dentists and endodontic specialists via SurveyMon- key. A Chi-square test was made to evaluate the differences in medication between the studied groups. Results: In pulpitis, antibiotics were prescribed in 3.48% of cases and anti-inflammatories in 62.60%. In pul- pal necrosis without fistula, no medication was indicated in 64.47% of cases, followed by antibiotics in 24.56%. In ne- crosis with fistula, 52.38% did not indicate any medication, followed by medication with antibiotics in 35.49%. In acute apical periodontitis the main medication was anti-inflamma-tories (52.79%), followed by antibiotics (32.87%); and for acute alveolar abscess, 57.10% indicated antibiotics, fol- lowed by anti-inflammatories. The antibiotic of choice was penicillin in 65.23% of the cases, and in case of allergy to it, the chosen antibiotic was azithromycin (30.12%). The prescription time was 7 days. In the comparison between specialists and non-specialists, there were significant dif- ferences for pulpitis and necrosis with fistula (p<0.01) and there were no significant differences between necrosis without fistula, acute apical periodontitis and acute alveo- lar abscess (p>0.05). Conclusions: Penicillin was the antibiotic of choice for the majority of the surveyed Argentine dentists, as well as ibuprofen as an anti-inflammatory drug. These could reflect an overmedication in endodontics pathologies that could con- tribute to microbial resistance to antibiotics (AU)

Objective: Evaluate the effectiveness of nanosilver fluoride in comparison to silver diamine fluoride in management of deep carious lesions. Material and Methods: This double-blinded randomized controlled clinical trial included thirty-six patients with deep carious mandibular molars. Patients were recruited form the outpatient clinic of Suez canal university, Egypt and randomly allocated into three groups, Nanosilver fluoride group, Silver diamine fluoride group and control group. For all groups, teeth were isolated by rubber dam. Access was done followed by partial caries removal. Silver diamine fluoride or Nanosilver fluoride were applied, and all teeth were restored with composite resin restorative. Patients were recalled after 3 and 6 months to follow-up the pulp vitality. R-statistical analysis software for Windows, version 4.1.1, was used to conduct the statistical analysis Results: At 3 month, for all groups, all teeth were successful. After 6 months, for silver diamine fluoride group, a single case failed. Regarding the nanosilver fluoride, all cases were successful. For the control group, two cases failed. No statistically significant difference was found between the tested groups. Conclusion: Application of 5% Nanosilver to fluoride varnish has similar clinical efficacy as silver diamine fluoride in arresting the dentin caries progression (AU)


Objetivo: Avaliar a eficácia de fluoreto de nanoprata em comparação ao fluoreto de diamina de prata no manejo de lesões profundas de cárie. Material e Métodos: Este estudo clínico controlado randomizado duplo-cego incluiu trinta e seis pacientes com lesões de cárie profunda em molares inferiores. Os pacientes foram recrutados no ambulatório da Universidade do Canal de Suez, no Egito, e alocados aleatoriamente em três grupos de tratamento: fluoreto de nanoprata, fluoreto de diamina de prata e grupo controle. Para todos os grupos, os dentes foram submetidos ao isolamento absoluto. O acesso à lesão foi feito seguido pela remoção parcial de tecido cariado. Tanto o fluoreto de diamina quanto fluoreto de nanoprata foram aplicados, e todos os dentes foram restaurados com resina composta. Os pacientes foram avaliados após 3 e 6 meses para o acompanhamento da vitalidade pulpar. O software estatístico R-statistical para Windows, versão 4.1.1, foi usado para as análises. Resultados: Em 3 meses, para todos os grupos, todos os dentes apresentaram sucesso no tratamento. Após 6 meses, para o grupo que utilizou o fluoreto de diamina de prata um único caso falhou. Em relação ao fluoreto de nanoprata, todos os casos apresentaram sucesso. Para o grupo controle, dois casos falharam. Não houve diferença estatística entre os grupos testados. Conclusão: A aplicação de verniz de fluoreto de nanoprata a 5% tem eficácia clínica semelhante ao fluoreto de diamina de prata na interrupção da progressão de cárie no tecido dentinário (AU)

Objetivo: La reabsorción dentinaria interna es un pro- ceso causado por la actividad odontoclástica asociada princi- palmente a la inflamación pulpar crónica y/o traumatismos, y se caracteriza por la pérdida progresiva de tejido dentinario y la posible invasión al cemento. El presente informe describe el diagnóstico y tratamiento de un molar inferior que presentó un cuadro sintomático de reabsorción dentinaria interna. Caso clínico: Un paciente de 38 años fue derivado a la consulta por presentar una zona de reabsorción interna en un segundo molar inferior. Durante el examen clínico y ra- diográfico se tomó una radiografía preoperatoria periapical con radiovisiógrafo en la que se observó la presencia de un área compatible con el diagnóstico de reabsorción dentinaria interna, el que fue posteriormente confirmado por medio de una tomografía computada de haz cónico. La imagen de la lesión se presentó como una zona radiolúcida deformante de bordes nítidos, localizada a nivel de la cámara pulpar. El tra- tamiento consistió en la extirpación de la pulpa coronaria y de la instrumentación, desinfección y obturación de los conduc- tos radiculares y la cavidad de acceso. En el control clínico y radiográfico realizado luego de 3 años se observó que el paciente estaba asintomático y las estructuras perirradiculares se encontraban dentro de los límites normales. La observación histológica del material removido de la cámara pulpar reveló la presencia de un tejido granulomatoso con numerosos vasos sanguíneos y escasos focos micro hemorrágicos. Hasta el momento, el tratamiento endodóntico es el pro- cedimiento indicado para el tratamiento de la reabsorción dentinaria interna. Se destaca la importancia de la tomografía computada de haz cónico para el diagnóstico y tratamiento temprano de las reabsorciones dentinarias internas a efectos de contar con un pronóstico favorable (AU)


Aim: Internal dentine resorption is aprocess caused by odontoclastic activity, mainly associated with chronic pulpal inflammation and/or trauma, and it'scharacterized by a pro- gressive loss of dentine tissue and the possible invasion of the cementum. This report describes the diagnosis and treatment of a lower molar that presented a symptomatic case of inter- nal dentine resorption. Clinical case: A 38-years old patient was referred to the office because of presenting an area of internal resorption in a lower second molar. During clinical and radiographic exam- ination, a periapical preoperative radiograph with radiovisio- graph was taken, in which the presence of an area compatible with the diagnosis of internal dentine resorption was observed, which was later confirmed by a cone-beam computed tomog- raphy.The image of the lesion was presented as a deforming radiolucent area with sharp edges, located at pulp chamber level. Treatment consisted of the removal of the coronary pulp and the instrumentation, disinfection and filling of the root ca- nals and the access cavity. In the clinical and radiographic control carried out 3 years after procedure, it was observed that the patient was asymptomatic and the periradicular struc- tures were within normal limits. The histological observation of the removed material from the pulp chamber revealed the presence of a granulomatous tissue with numerous blood ves- sels and scarce micro hemorrhagic focus. Until now, the endodontic treatment is the indicated pro- cedure to treat internal dentine resorption. It is necessary to highlight the importance of the cone-beam computerized to- mography for the early diagnosis and treatment of internal dentine resorptions in order to have a favorable outlook (AU)

RESUMO

La persistencia de lesiones perirradiculares luego del tra- tamiento endodóntico es un problema que requiere del clínico un conocimiento cabal de la histofisiología y de la histopato- logía del sistema de conductos radiculares del tejido pulpar y de los tejidos perirradiculares (periodonto y hueso); además de considerar siempre la posible existencia de enfermedades sistémicas que también pueden actuar como factores de in- fluencia. La presencia de bacterias remanentes a posteriori del tratamiento es considerada como una de las causas principales y más frecuentes para la perpetuación de las lesiones perirra- diculares. Sin embargo, existen otros factores causales, como la existencia de conductos laterales o accesorios infectados y no tratados, la reabsorción dentinaria interna, intercomunica- ciones, cul-de-sacs o istmos; que representan áreas de difícil acceso durante la instrumentación e irrigación. Cuando la cau- sa original se localiza en la zona perirradicular, como en los casos de actinomicosis, reacciones a cuerpo extraño, cristales de colesterol (CRCo) y granulomas o quistes con alto conte- nido de CRCo, la indicación más adecuada es el retratamiento y la cirugía periapical como complemento (AU)


The persistence of periradicular lesions after endodontic treatment is a problem that requires the doctor to have a thor- ough knowledge of the histophysiology and histopathology of the root canal system, the pulp tissue and periradicular tis- sues (periodontium and bone); as well as always considering the possible existence of systemic alterations that can also be influencing factors. Persisting bacteria within the root canal system after treatment is one of the major and most frequent causes for the perpetuation of periradicular lesions. Howev- er, there are other possible causal factors such as the exist- ence of untreated lateral or accessory canals, internal dentin resorption, intercommunications, cul-de-sacs or isthmuses; areas that represent a difficulty in access during instrumen- tation and irrigation. If the original cause is located in the periradicular area, in cases like actinomycosis, foreign-body reactions, cholesterol crystals (CRCo) and granulomas or cysts with high content of CRCo, retreatment coupled with periapical surgery is the best approach to treatment (AU)

El diagnóstico molecular, mediante la aplicación de técnicas molecu- lares, ha permitido estudiar microorganismos presentes en el inicio y progresión de la caries dental, enfermedad periodontal, y en los fracasos endodónticos. Las técnicas moleculares permiten la detección y cuan- tificación del material genético del ácido desoxirribonucleico (DNA), ácido ribonucleico (RNA) o proteínas, lo que posibilita el estudio del genoma completo o secuencias de DNA específicas. Estas técnicas surgen como una necesidad de detectar microorganismos de difícil o lento crecimiento en cultivos; la técnica más utilizada es la reacción en cadena de la polimerasa (PCR) que permite la amplificación de peque- ños segmentos de material genético al utilizar cebadores, por lo que es un método económico, preciso, sensible y rápido para la detección de microorganismos. El presente artículo de revisión bibliográfica servirá para informar sobre los avances de las técnicas moleculares utilizadas para el diagnóstico en la práctica odontológica (AU)


Molecular diagnosis, through the application of molecular techniques, has made it possible to study microorganisms present in the onset and progression of dental caries, periodontal disease, and endodontic failures. Molecular techniques allow the detection and quantification of the genetic material of deoxyribonucleic acid (DNA), ribonucleic acid (RNA) or proteins, allowing the study of the complete genome or specific DNA sequences, they arise as a need to detect difficult or slow growing microorganisms in cultures. The most widely used technique is the polymerase chain reaction (PCR) that allows the amplification of small segments of genetic material using primers, it is an economical, precise, sensitive and fast method for the detection of microorganisms. This bibliographic review article will serve to report on the advances in molecular techniques used for diagnosis in dental practice (AU)

La OMS cataloga al cáncer como uno de los principales problemas en el ámbito mundial, los pacientes sometidos a terapia oncológica son más vulnerables a desarrollar complicaciones en los tejidos de la cavidad bucal entre las que tenemos: mucositis, infecciones, osteorradionecrosis. En el manejo endodóntico hay que tomar en consideración que los trata- mientos como yodoterapia, radioterapia y quimioterapia pueden generar efectos sobre el complejo dentinopulpar. El objetivo de este artículo es determinar el estatus del tejido pulpar postratamiento oncológico mediante la revisión sistemática en bases de datos de gran relevancia científica, como PubMed, Scielo, Medigraphic, Science direct. Se concluye que el sistema estomatognático es un receptor importante de estos efectos y secuelas en pacientes con terapia oncológica, el tejido pulpar no está libre de estas secuelas ya que genera daño celular, como la hipovascularidad, hipocelularidad e hipoxia la cual incrementa el riesgo de necrosis de la región (AU)


The WHO lists cancer as one of the main problems worldwide, patients undergoing oncological therapy are more vulnerable to developing complications in the tissues of the oral cavity among which we have: mucositis, infections, osteoradionecrosis. In endodontic management, it should be taken into consideration that treatments such as iodine therapy, radiotherapy, and chemotherapy can generate effects on the dentin-pulp complex. This article aims to determine the status of the pulp tissue after oncologic treatment. Through a systematic review in databases of great scientific relevance, such as PubMed, Scielo, Medigrafhic, Science direct. It is concluded that the stomatognathic system is an important receptor of these effects and sequelae in patients with oncological therapy, the pulp tissue is not free of these sequelae as it generates cellular damage because of the hypo vascularity, hypocellularity, and hypoxia which increases the risk of necrosis of the region (AU)

Introduction: The purpose of this study was to evaluate the effectiveness and compare the accuracy of pulp tests in the diagnosis of teeth pulpal health. Material and Methods: Traumatized (n=71) and non-traumatized (n=71) teeth from 42 patients were evaluated. Each tooth underwent cold, heat, electric and oximetry tests, followed by radiographic examination and calculation of the sensitivity, specificity, PPV, NPV and accuracy. Results: Clinical and radiographic examination showed no alteration for the 71 teeth from the intact contralateral group. From the traumatized group, 29 teeth presented complete endodontic treatment, 17 presented periapical alterations that required endodontic treatment and 25 teeth did not present conclusive radiographic alteration. The cold test showed a significantly higher proportion of correct results, while the electric test showed a significantly lower proportion. The data showed higher accuracy for the cold, followed by oximeter and heat tests, while the electric test presented the lowest accuracy. Cold and oximeter tests proved superior over the electric and heat tests, while the electric test showed better parameters when diagnosing diseased pulp. Conclusion: Combining two pulp tests seems reasonable for improving the pulp diagnoses using both oximeter and cold or oximeter and heat tests to detect healthy pulp; or cold and electric tests to define diseased pulp.


Introducción: El propósito de este estudio fue evaluar la efectividad y comparar la precisión de las pruebas pulpares en el diagnóstico de la salud pulpar de los dientes. Material y Métodos: Se evaluaron dientes traumatizados (n=71) y no traumatizados (n=71) de 42 pacientes. Cada diente se sometió a pruebas de frío, calor, eléctricas y de oximetría, seguidas de examen radiográfico y cálculo de la sensibilidad, especificidad, VPP, VPN y precisión. Resultados: El examen clínico y radiográfico no mostró alteración en los 71 dientes del grupo contralateral intacto. Del grupo traumatizado, 29 dientes presentaron tratamiento endodóntico completo, 17 presentaron alteraciones peri-apicales que requirieron tratamiento endodóntico y 25 dientes no presentaron alteración radiográfica concluyente. La prueba en frío mostró una proporción significativamente mayor de resultados correctos, mientras que la prueba eléctrica mostró una proporción significativamente menor. Los datos mostraron mayor precisión para la prueba de frío, seguida de las pruebas de oxímetro y calor, mientras que la prueba eléctrica presentó la menor precisión. Las pruebas de frío y oxímetro demostraron ser superiores a las pruebas eléctricas y de calor, mientras que la prueba eléctrica mostró mejores parámetros al momento de diagnosticar pulpa enferma. Conclusión: La combinación de dos pruebas pulpares parece razonable para mejorar los diagnósticos pulpares utilizando tanto el oxímetro como las pruebas de frío u oxímetro y calor para detectar una pulpa sana; o pruebas de frío y eléctricas para definir pulpa enferma.

La celulitis facial odontogénica es comúnmente observada en las salas de emergencia hospitalarias en los pacientes pediátricos debido a la caries dental. Objetivo: Validar el instrumento de prevalencia de celulitis facial y su relación con la caries dental en pacientes de 5 a 9 años de edad en el Hospital General IESS Milagro. Materiales y métodos: Enfoque mixto cuantitativa y cualitativa, tipo de investigación de campo, corte transversal, descriptivo, no experimental, se realizó un plan piloto de 15 pacientes pediátricos, un instrumento cuantitativo (Ficha de recolección de datos) contó con 9 expertos para un juicio de expertos y en el instrumento cualitativo (Entrevista) contó con 6 expertos para la validación del instrumento en total son de 15 profesionales en salud, calificando Validez, Pertinencia y Coherencia. Resultados: El instrumento cuantitativo tiene una puntuación de 80,85 de confiablidad y el instrumento cualitativo tiene una puntuación de 87,11 de confiablidad. En el instrumento cuantitativo se determinó la prevalencia de celulitis facial de niños de 5 a 9 años de edad, género masculino (80%), rango de 7 a 8 años (53,3%), cita por primera vez (60%), antecedente referencial caries dental (93,3%), diagnóstico CIES10 fue el L032 celulitis de la cara (93,3%), región afectada en la zona submandibular (73,3%), prevalencia de caries dental, molares deciduos (93,3%), no registran antecedentes de buena salud (73,3%), tratamientos odontológicos de prevención al año ninguna (80%), causa frecuente en la aparición de celulitis facial, la necrosis pulpar K041. Conclusión: La validación del instrumento tuvo un grado de confiabilidad muy buena y su cifra de alfa de Cronbach alto, la misma que da seguridad y confianza, gracias a la calificación de los jueces que revisaron cada detalle(AU)


Odontogenic facial cellulite is commonly observed in hospital emergency rooms in pediatric patients due to the dental caries. Objective: to validate the facial cellulite prevalence instrument and its relationship with dental caries in patients from 5 to 9 years of age in the IESS Miracle General Hospital. Materials and methods: mixed approach quantitative and qualitative, type of field research, cross section, descriptive, non-experimental, a pilot plan of 15 patients was carried out ediatric patients, a quantitative instrument (data collection form) counted with 9 experts for an expert judgment and in the qualitative instrument (Interview) had 6 experts for the validation of the instrument in total they are of 15 health professionals, qualifying Validity, Relevance and Coherence. Results: the quantitative instrument has a score of 80.85 of reliability and the qualitative instrument has a score of 87.11 out of reliability. In the quantitative instrument, the prevalence of facial cellulite in children 5 to 9 years of age, male gender (80%), range 7 to 8 years old (53.3%), appointment for the first time (60%), referential history dental caries (93.3%), CIES10 diagnosis was L032 facial cellulite (93.3%), affected region in the submandibular area (73.3%), prevalence of dental caries, deciduous molars (93.3%), do not record a history of good health (73.3%), preventive dental treatments per year none (80%), frequent cause in the appearance of facial cellulitis, pulpal necrosis K041. Conclusion: the validation of the instrument had a very high degree of reliability. good and its high Cronbach's alpha figure, the same one that gives security and confidence, thanks to the rating of the judges who reviewed every detail(AU)

Es importante realizar una detenida evaluación clínica y radiográfica para esta-blecer un correcto diagnóstico y plan de tratamiento. Por este motivo, el objetivo de esta guía es presentar un protocolo consensuado entre las Cátedras de Pe-riodoncia y Endodoncia de la Facultad de Odontología de la Universidad de Buenos Aires (FOUBA), para su tratamiento (AU)

La microendodoncia involucra la visualización a través de un microscopio operatorio de todas las fases del tratamiento de conductos y procedimientos de cirugía apical y correctiva por parte del endodoncista. Existe sobrada evidencia acerca de las mejoras que puede aportar la magnificación al tratamiento; la literatura demuestra que la capacidad del operador mejora si su visión del campo gana claridad y precisión, ambos recursos pueden ser proporcionados por el microscopio operatorio, aunado a que posibilita diagnósticos más certeros junto con mejoras en el pronóstico, lo que permite evitar posibles complicaciones. La calidad de los tratamientos endodóncicos involucra infinidad de factores, cada uno relevante en sí mismo pero, en determinados casos, el microscopio puede significar la diferencia entre un tratamiento exitoso o un fracaso clínico. En la actualidad, se ha convertido en un tema de lo más relevante, por lo que el objetivo del presente trabajo es revisar la literatura con el fin de ayudar al entendimiento basado en evidencia científica de los criterios que determinan la relevancia del uso del microscopio en el ámbito endodóncico (AU)


Microendodontics involves the visualization through an operating microscope of all phases of root canal treatment and apical and corrective surgery procedures by the endodontist. There is plenty of evidence about the improvements that magnification can provide, the literature shows that the operator's ability improves if his vision of the field gains clarity and precision, both resources can be provided by the operating microscope, added to the fact that it enables more accurate diagnoses together with improvements in the prognosis allowing to avoid possible complications. The quality of endodontic treatments involves countless factors, each relevant in itself, but in certain cases the microscope can mean the difference between a successful treatment or a clinical failure. At present, it has become a very relevant topic, so the objective of this work is to review the literature in order to help understand the criteria that determine the relevance of the use of the microscope in the endodontic field based on scientific evidence (AU)

Introduction Pulp and periodontal tissues may communicate and, in pathological situations, combined endodontic-periodontal lesions may be established. Objective The aim of this study was to evaluate the prevalence of endo-perio lesions in non-molar and molar teeth referred for endodontic treatment. Material and method The sample consisted of 104 teeth evaluated in 79 consecutive patients in a cross-sectional design. Visible plaque, probing depth, and bleeding on probing were evaluated. Endodontic evaluation included the presence or absence of caries, fistulas, pain, and pulp sensitivity. The presence/absence of periapical lesion, bone loss in the furcation region, and proximal area were evaluated. Result The results showed that pain was the main reason for seeking dental care in 63.3% of patients. The molar teeth demonstrated higher presence of probing depth (PD) ≥ 7 mm (38.3%) and higher PD mean (6.17 mm) than non-molar teeth (P<0.05). It was verified that 65.4% of the teeth were diagnosed with a primary endodontic lesion and that the periodontal component was present in 34.6% of the teeth, either in a primary (10.6%), secondary (11.5%), or combined form (12.5%). True combined endodontic-periodontal lesion occurred significantly in molar teeth compared to non-molar teeth (p<0.05). Conclusion The primary endodontic lesion was found in a greater proportion in teeth referred for endodontic treatment; however, approximately 1/3 of the sample had periodontal involvement, which demonstrates the importance of the periodontal examination together with the general clinical examination.


Introdução Os tecidos pulpar e periodontal podem se comunicar e, em situações patológicas, podem-se estabelecer lesões endodônticas-periodontais combinadas. Objetivo O objetivo deste estudo foi avaliar a prevalência de lesões endo-perio em dentes não molares e molares encaminhados para tratamento endodôntico. Material e método A amostra consistiu de 104 dentes avaliados em 79 pacientes consecutivos em um desenho transversal. Placa visível, profundidade de sondagem e sangramento à sondagem foram avaliados. A avaliação endodôntica incluiu a presença ou ausência de cáries, fístulas, dor e sensibilidade pulpar. Foram avaliados a presença / ausência de lesão periapical, perda óssea em região de furca e área proximal. Resultado Os resultados mostraram que a dor foi o principal motivo de procura de atendimento odontológico em 63,3% dos pacientes. Os dentes molares demonstraram maior presença de profundidade de sondagem (PS) ≥ 7 mm (38,3%) e maior média de PS (6,17 mm) do que os dentes não molares (P <0,05). Verificou-se que 65,4% dos dentes tinham diagnóstico de lesão endodôntica primária e que o componente periodontal estava presente em 34,6% dos dentes, seja na forma primária (10,6%), secundária (11,5%) ou combinada (12,5%). Lesão endodôntica-periodontal combinada verdadeira ocorreu significativamente em dentes molares em comparação com os dentes não molares (p <0,05). Conclusão A lesão endodôntica primária foi encontrada em maior proporção nos dentes encaminhados para tratamento endodôntico; entretanto, aproximadamente 1/3 da amostra apresentava acometimento periodontal, o que demonstra a importância do exame periodontal em conjunto com o exame clínico geral.

Objetivos: analizar y describir los requerimientos de atención quirúrgica y no quirúrgica de urgencia en la población infantil y juvenil en un servicio de urgencias de Argentina durante el aislamiento social preventivo y obligatorio (ASPO). Materiales y métodos: se realizó un estudio observacional y descriptivo de análisis retrospectivo durante el período inicial de 93 días de ASPO, comprendido entre 20 de marzo de 2020 hasta el 20 de junio de 2020. Se registraron los pacientes menores de 17 años que acudieron al servicio de urgencias del Hospital Odontológico de la Universidad de Buenos Aires y se caracterizó aquellos que requerían distintos tipos de tratamientos. Resultados: se atendieron 4654 pacientes durante el periodo estudiado, de los cuales 116 eran menores de 17 años. Se realizaron 142 prestaciones. A 63 piezas dentarias se les realizó la exodoncia. A otras 63, algún tipo de tratamiento endodóntico. 11 piezas requirieron de restauraciones directas, y en 5 se realizaron otros tipos de tratamientos. Conclusión: fue importante la presencia de recurso humano calificado en el área de la cirugía bucal y pediátrica en los servicios de guardia, considerando el incremento sustancial de necesidad de tratamiento suscitado durante el ASPO (AU)


Objective: to analyse and describe the requirements for emergency surgical and non-surgical care in the paediatric population in an emergency department in Argentina during Preventive and Mandatory Social Isolation. Materials and Methods: an observational, descriptive, retrospective study was conducted during the initial period of 93 days of Preventive and Mandatory Social Isolation, from March 20, 2020 to June 20, 2020. Patients under 17 years of age who attended the emergency department of the Dental Hospital of the University of Buenos Aires were registered and those requiring different types of treatment were characterised. Results: 4654 patients were seen during the period studied, 116 of whom were under 17 years of age. A total of 142 services were performed. Sixty-three teeth were exodontically extracted. A further 63 underwent some form of endodontic treatment. 11 teeth required direct restorations and 5 required other types of treatment. Conclusion: the presence of qualified human resources in oral and paediatric surgery in the on-call services was important, considering the substantial increase in the need for treatment during the ASPO (AU)

Las fuerzas ejercidas durante el tratamiento ortodóncico son consideradas un tipo particular de traumatismo dental. El movimiento ortodóncico de una pieza dentaria puede producir inflamación o necrosis pulpar y reabsorción radicular. Estos efectos colaterales indeseables son difíciles de tratar, especialmente cuando las fuerzas aplicadas son excesivas y no controladas. Diferentes estudios han evaluado el impacto de las fuerzas excesivas durante el movimiento dentario. Sin embargo, los resultados son confusos y contradictorios. La predisposición genética y la variabilidad biológica individual de los pacientes son factores importantes que deben ser tenidos en cuenta. Por eso es necesario contar con una historia clínica completa, utilizar imágenes 3D y realizar diferentes pruebas clínicoradiográficas con el fin de obtener información precisa acerca del diagnóstico, la indicación de tratamiento y el posible pronóstico a distancia. El objetivo de este artículo es realizar una revisión de la patología pulpar y la reabsorción radicular en relación con el tratamiento ortodóncico (AU)


Orthodontic forces are recognised as a particular type of dental trauma. During orthodontic tooth movement, the occurrence of pulp inflammation or necrosis and subsequent root resorption are undesirable side effects that are difficult to treat, especially when uncontrolled excessive forces are applied. Several studies have evaluated the impact of excessive forces during teeth movement. However their results are confused and contradictory. Genetic disposition and individual biological variability are important factors that must be always considered. Therefore, a complete clinical history, the use of 3D images along with different clinical and radiographic diagnostic methods are necessary to provide accurate diagnosis and prognosis of the treatment. The objective of this article is to review the possibility of pulp pathology and root resorption related to orthodontic treatment (AU)

RESUMEN La mayoría de los pacientes que acuden a consulta de urgencias de Estomatología refieren una sintomatología dolorosa causada por las enfermedades pulpares. Se realizó un estudio observacional-analítico; la población estuvo conformada por los 252 pacientes que acudieron al Servicio Estomatológico del Policlínico Chiqui Gómez Lubián, Santa Clara, y que fueron diagnosticados con una afección pulpar reversible transitoria, en el período enero-diciembre 2016. La muestra (167 pacientes) fue elegida de forma intencional. El principal objetivo fue describir los agentes causales, relacionar la evolución clínica y las variables clínico-epidemiológicas, e identificar los factores asociados a la evolución clínica en los procesos pulpares reversibles. El rango de edad más afectado fue de 22 a 52 años y predominó el sexo masculino. El agente causal que afectó a la mayor cantidad de pacientes fue el bacteriano; se relacionaron todas las variables con el resultado de la evolución clínica.


ABSTRACT Most of the patients who come to the Dental Emergency Consultation report painful symptomatology caused by pulp diseases. An observational analytical study was performed; the population consisted of 252 patients who were seen at the Dental Service of "Chiqui Gómez Lubián" Polyclinic, Santa Clara, and were diagnosed with a transient reversible pulp condition from January to December 2016. The sample (167 patients) was chosen intentionally. The main objective was to describe the causal agents, relate clinical evolution and clinical-epidemiological variables, and identify factors associated with clinical evolution in reversible pulp processes. The most affected age range was 22 - 52 years old and male gender predominated. The causal agent that affected the greatest number of patients was the bacterial one; all variables were related to the result of clinical evolution.

Introducción: el empleo de software educativo es una alternativa motivadora en los diferentes escenarios docentes. Objetivo: mostrar la validación del software educativo sobre diagnóstico y tratamiento de enfermedades pulpares y periapicales para su generalización en estudiantes de Estomatología. Material y Métodos: se realizó un estudio de innovación tecnológica o producto terminado digital en la etapa comprendida desde marzo de 2018 a marzo de 2019. Se utilizó la herramienta Mediator v9.0. Se utilizaron métodos teóricos: histórico-lógico, analítico-sintético y el sistémico-estructural; y empíricos: la revisión documental y una encuesta a estudiantes y expertos en función de evaluadores externos del producto. Para evaluar la factibilidad del software se utilizó el método de Criterio de Experto y el Criterio de usuario. Las principales variables evaluadas fueron el diseño de las secciones del software, su funcionalidad, cientificidad del contenido, relación de las imágenes con el objetivo del software, así como su uso en la docencia. Resultados: el producto obtenido consta de un texto básico con hipervínculos para acceder a los demás componentes, así como galerías de imágenes y ejercicios de autoevaluación. Según Criterio de Experto la calidad técnica del producto y la calidad del contenido quedaron evaluadas en la categoría Muy Adecuado. Según Criterio de Usuario, la funcionalidad, la motivación individual y los aspectos de diseño fueron evaluados de Bien. Conclusiones: se desarrolló un software educativo que cuenta con recursos variados para el aprendizaje sobre el tema abordado(AU)


Introduction: The use of the educational software is a motivating alternative in different teaching scenarios. Objective: To validate the educational software related to the diagnosis and treatment of pulp and periapical diseases for its generalization in dental students. Material and Methods: A study linked with technological innovation or digital finished product was conducted during the period between March 2018 and March 2019. The tool used was Mediator v9.0. Theoretical methods such as historical-logical, analytical-synthetic, inductive-deductive, and systemic-structural were used; empirical methods, which included the documentary review and a survey, were applied to students and experts as external evaluators of the product. The methods of Expert Judgment and Customer Judgment were used to evaluate the feasibility of the software. The main variables evaluated were the design of software sections, its functionality, scientific content, relationship between the images and the objective of the software and its use in the teaching-learning process. Results: The product obtained is a basic text with hyperlinks to access the rest of the components, image galleries and self-evaluation exercises. According to Expert Judgment, the technical quality of the software and the quality of its content were evaluated as good. According to Customer Judgment, the functionality, self-motivation and design aspects were evaluated as optimal. Conclusions: An educational software with various resources for learning about the topic approached was developed(AU)

Abstract Objective: The present systematic review and meta-analysis aimed at evaluating the efficacy of laser's treatment of exposed pulps to stimulate healing. Material and Methods: MEDLINE, PubMed, Cochrane Library, Embase, ISI, Google Scholar have been searched as the electronic databases for performing systematic literature between 2005 to 2019. Searches were performed with MESH terms. The quality of the studies included was assessed using the Critical Appraisal Skills Program checklist. For Data extraction, two reviewers blind and independently extracted data from the abstract and full text of the studies included. Moreover, the fixed-effect model's odds ratio for a 95% confidence interval (CI) was calculated. Random effects were used to deal with potential heterogeneity, and I2 showed heterogeneity. The meta-analysis and forest plots have been evaluated using the Comprehensive Meta-Analysis Stata. Results: Five studies (3 RCTs and 2 animal studies) were included in this study. The Odds Ratio was 1.90 (95% CI 1.39-2.42; p=0.00); there was a statistically significant difference between the laser and the control groups (p=0.00). Conclusion: The success rate of laser treatment for exposed pulps is higher than mineral trioxide aggregate, resin, calcium hydroxide and resin-modified glass-ionomer cement. The exact mechanism of this effect has not yet been determined; further research on lasers' impact on exposed pulps treatment is required.

O tratamento endodôntico de dentes permanentes jovens com infecções pulpares/periapicais antes de completar a rizogênese ainda é um desafio para a Endodontia e a Odontopediatria. Relatos científicos têm mostrado que a curcumina (CUR), um fitoquímico polifenólico, apresenta diversas propriedades terapêuticas, entre as quais, amplo espectro de ação antimicrobiana e a capacidade de induzir a proliferação e migração celular. Além disso, devido à sua capacidade excitatória na presença de luz, a CUR também tem sido utilizada como fotossensibilizante em terapia fotodinâmica associada ao LED (light emitting diode), promovendo aumento dos seus efeitos biológicos. Uma forma de aumentar seu potencial terapêutico e reduzir algumas limitações do uso da CUR é a síntese de análogos a partir de pequenas modificações químicas na estrutura original, entretanto, mantendo sua capacidade fotossensibilizante. O objetivo desse estudo foi avaliar a ação antimicrobiana e antibiofilme de análogos de curcumina sob a influência ou não do LED sobre microrganismos de interesse endodôntico e sua influência sobre a viabilidade, proliferação e migração de fibroblastos da linhagem L-929. Uma série de compostos análogos de CUR (PCR-4 H, PCR-3 OH, PCR-4 OH, PCR-3 OCH3, PCR-4 OCH3, PCR-3 acetil, PCR-4 acetil) foram sintetizados pela metodologia de Pabon. A atividade antimicrobiana da CUR e seus análogos foi determinada pelo ensaio de Concentração Inibitória Mínima (CIM) e Concentração Bactericida Mínima (CBM) sobre Streptococcus mutans, Lactobacillus casei, Actinomyces israelii, Enterococcus faecalis e Fusobacterium nucleatum, sob a ação ou não do LED InGaN (nitreto de gálio e índio, com potência de saída de 100 mW/cm², ponta do LED com área de 0,78 cm², 60 s). A curcumina e seu análogo com melhor efeito antimicrobiano (PCR-3 OH) foi avaliado sobre o biofilme inicial (72h) e maduro (1 semana) dessas espécies em microplacas e sobre biofilmes multiespécies formados em túbulos dentinários por contagem das UFC/mL e por microscopia confocal, respectivamente, sob ação ou não do LED. Também foram avaliados quanto à citotoxicidade e a capacidade de induzir proliferação e migração em fibroblastos, por meio de ensaios de metiltetrazólio, azul de tripan e azul de Coomassie, respectivamente. Os dados foram avaliados estatisticamente (p<0,05). Dos 7 análogos de curcumina sintetizados, PCR-3 OH foi o único composto que apresentou atividade bactericida quando testado sobre as bactérias de interesse endodôntico selecionadas. Seu efeito foi potencializado na presença do LED, variando entre as espécies bacterianas. A curcumina teve efeito bactericida para as espécies S. mutans, A. israelii, L. casei e F. nucleatum, e em algumas delas, foi independente do LED. Ambos os compostos reduziram o crescimento dos biofilmes iniciais ou maduros, independente do LED. Entretanto, quando irradiados, o efeito dos compostos variou de acordo com a espécie bacteriana, sendo que A. israelii e S. mutans foram os mais afetados. Ambos os compostos reduziram significativamente os biofilmes multiespécies quando comparados ao controle sem tratamento, sendo que melhor efeito foi observado para PCR-3 OH. A curcumina foi considerada citocompatível a partir de 0,039µg/mL e PCR-3 OH a partir de 0,019 µg/mL. Houve redução significativa na viabilidade celular quando os compostos foram irradiados com LED nas concentrações 0,039 e 0,019 µg/mL. O LED, dentro dos parâmetros testados, reduziu significativamente a viabilidade, a proliferação e a migração celular, independente do composto ou tempo de exposição. Conclui-se que PCR-3 OH apresentou atividade bactericida e sobre biofilmes simples e multiespécies de bactérias de interesse endodôntico superior à CUR, principalmente sob ação do LED. Entretanto, sua citocompatiblidade foi inferior à da CUR. A presença do LED afetou a viabilidade, proliferação e migração dos fibroblastos, mostrando que os parâmetros utilizados para fins antimicrobianos não foram adequados para aplicação em células eucarióticas(AU)


Endodontic treatment of young permanent teeth with pulp / periapical infections before completing rhizogenesis is still a challenge for Endodontics and Pediatric Dentistry. Scientific reports have shown that curcumin (CUR), a polyphenolic phytochemical, has several therapeutic properties, including a broad spectrum of antimicrobial action and the ability to induce cell proliferation and migration. In addition, due to its excitatory capacity in the presence of light, CUR has also been used as a photosensitizer in photodynamic therapy associated with LED (light emitting diode), promoting an increase in its biological effects. One way to increase its therapeutic potential and reduce some limitations of the use of CUR is the synthesis of analogues from small chemical modifications in the original structure, however, maintaining its photosensitizing capacity. The aim of this study was to evaluate the antimicrobial and antibiofilm action of curcumin analogues under the influence or not of LED on microorganisms of endodontic interest and their influence on the viability, proliferation and migration of L-929 fibroblasts. A series of CUR analog compounds (PCR-4 H, PCR-3 OH, PCR-4 OH, PCR-3 OCH3, PCR-4 OCH3, PCR-3 acetyl, PCR-4 acetyl) were synthesized by Pabon's methodology. The antimicrobial activity of CUR and its analogs was determined by the Minimum Concentration Inhibitory (CIM) and Minimum Bactericidal Concentration (CBM) assay on Streptococcus mutans, Lactobacillus casei, Actinomyces israelii, Enterococcus faecalis and Fusobacterium nucleatum, with or without the InGaN LED (gallium and indium nitride, with output power of 100 mW / cm², LED tip with an area of 0.78 cm², 60 sec). Curcumin and its analog with the best antimicrobial effect (PCR-3 OH) were evaluated on the initial (72h) and mature (1 week) biofilm of these species in microplates and on multispecies biofilms formed in dentinal tubules by counting CFU / mL and by confocal microscopy, respectively, under the action or not of the LED. They were also evaluated for cytotoxicity and the ability to induce proliferation and migration in fibroblasts, using methyltetrazolium, trypan blue and Coomassie blue assays, respectively. The data were evaluated statistically (p <0.05). Of the 7 curcumin analogues synthesized, PCR-3 OH was the only compound that showed bactericidal activity when tested on selected bacteria of endodontic interest. Its effect was enhanced in the presence of LED, varying between bacterial species. Curcumin had a bactericidal effect for the species S. mutans, A. israelii, L. casei and F. nucleatum, and in some of them, it was independent of the LED. Both compounds reduced the growth of the initial or mature biofilms, regardless of the LED. However, when irradiated, the effect of the compounds varied according to the bacterial species, with A. israelii and S. mutans being the most affected. Both compounds significantly reduced multispecies biofilms when compared to the untreated control, with the best effect being observed for PCR-3 OH. Curcumin was considered cytocompatible from 0.039 µg / mL and PCR-3 OH from 0.019 µg / mL. There was a significant reduction in cell viability when the compounds were irradiated with LED at concentrations of 0.039 and 0.019 µg / mL. The LED, within the parameters tested, significantly reduced cell viability, proliferation and migration, regardless of the compound or time of exposure. It is concluded that PCR-3 OH showed bactericidal activity and on simple and multispecies biofilms of bacteria of endodontic interest superior to CUR, mainly under the action of LED. However, its cytocompatibility was lower than that of the CUR. The presence of the LED affected the viability, proliferation and migration of fibroblasts, showing that the parameters used for antimicrobial purposes were not suitable for application in eukariotic cells(AU)

ntrodução: a calcificação pulpar é um dos fatores que tornam o tratamento endodôntico desafiador e capaz de comprometer o acesso dos instrumentos e soluções irrigadoras por toda extensão do canal radicular, impossibilitando sua adequada desinfecção. A Endodontia Guiada traz mais previsibilidade e segurança ao tratamento endodôntico nessa situação complexa. Métodos: uma vez constatada calcificação severa com necessidade de intervenção endodôntica, o paciente é encaminhado ao centro radiológico para o planejamento da Endodontia Guiada. Um modelo 3D da arcada a ser tratada é obtido por meio de um scanner de bancada e, posteriormente, transferido para um software de planejamento virtual de implante. A TCFC é adicionada a esse software e ambas são sobrepostas, com base em estruturas visíveis radiograficamente. O software Simplant é programado para projetar uma broca física, utilizada para o acesso endodôntico guiado, sobreposta virtualmente à calcificação do canal radicular. De posse da guia impressa, essa é posicionada na arcada do paciente e o procedimento clínico, executado. Conclusão: a técnica de Endodontia Guiada é rápida, previsível e clinicamente viável. Além disso, pode ser executada por profissionais menos experientes, não necessitando da utilização de microscópio operatório (AU).


Introduction: Pulp calcification is one of the factors that make endodontic treatment challenging and capable of compromising access of instruments and irrigant solutions to the entire extension of the root canal, making it impossible to disinfect it adequately. Guided endodontics makes the endodontic treatment more predictable and safer in this complex situation. Materials and Methods: Once severe calcification requiring endodontic intervention has been found, the patient is referred to the radiology center for the planning of guided endodontics. A 3D model of the arch to be treated is obtained by means of a bench scanner, afterwards transferred to a virtual implant planning software program. The CBCT is added to this software and both are superimposed on the basis of radiographically visible structures. The Simplant software is programmed to project a physical bur used for guided endodontic access, virtually superimposed on the root canal calcification. Once the printed guide has been obtained, it is positioned in the patient's arch and the clinical procedure is performed. Conclusion: The guided endodontic technique is easy, predictable and clinically feasible to perform. Moreover, it may be performed by less experienced professionals, and does not require the use of an operating microscope (AU).
