RESUMO

ABSTRACT A 12-year-old boy with Donnai-Barrow syndrome diagnosed intra-uterus presented esotropia, high myopia, nystagmus, and optic disk staphyloma in an ophthalmologic examination. The patient had associated Fanconi syndrome and sensorineural hearing loss as well as facial manifestations as hypertelorism, downward slanting of palpebral fissures and low ear implantation. Magnetic resonance imaging revealed agenesis of the corpus callosum. To our knowledge, this is the first reported case associated with esotropia, nystagmus, and optic disk staphyloma.


RESUMO Paciente do sexo masculino, 12 anos, com diagnóstico intrauterino de síndrome de Donnai-Barrow, apresentava ao exame oftalmológico esotropia, alta miopia, nistagmo e estafiloma de disco óptico. Associado ao quadro, apresentava síndrome de Falconi e perda auditiva neurossensorial, além de alterações faciais, como hipertelorismo, inclinação inferior das fissuras palpebrais e implantação baixa das orelhas. Ressonância magnética revelou agenesia de corpo caloso. Ao nosso conhecimento, este é o primeiro caso relatado associando esotropia, nistagmo e estafiloma de disco óptico.

Introducción: en la actualidad, se reconocen cuadros vestibulares periféricos y centrales que pueden ser diagnosticados mediante la videonistagmografía (VNG). Los avances en la tecnología han provocado en los profesionales una constante actualización en el uso e interpretación de las distintas pruebas que conlleven, en su lectura cruzada, un diagnóstico acertado y a tratamientos de rehabilitación exitosos. El objetivo fue describir las interpretaciones de los resultados de las pruebas oculomotoras, posicionales y calóricas de la VNG para lograr un diagnóstico detallado de las disfunciones vestibulares. Materiales y métodos: revisión documental obtenida de 40 fuentes reportadas en la literatura científica entre 2010 a 2020 tomadas de bases de datos, tesis de grado y libros. Discusión: dentro de la revisión se encontraron tres categorías (pruebas oculomotoras, posicionales y calóricas) y siete subcategorías (nistagmo espontáneo, nistagmo evocado por la mirada, rastreo, sacadas, optocinético, Dix-Hallpike y roll test). Conclusión: los diversos elementos encontrados en la presente revisión son relevantes ya que precisan no solo el tipo de vértigo, sino también su localización topográfica, lo que favorece el proceso de evaluacióndiagnóstico en la población en general.


Introduction: At present, peripheral and central vestibular frames are recognized that can be diagnosed by videonystagmography (VNG). Advances in technology have caused professionals to constantly update the use and interpretation of the different tests that lead, in their cross-reading, to an accurate diagnosis and successful rehabilitation treatments. The objective was to describe the interpretations of the results of the oculomotor, positional and caloric tests of the VNG, for a detailed diagnosis of the vestibular dysfunctions. Materials and method: Documentary review obtained from 40 sources reported in the scientific literature between 2010 and 2020, taken from databases, thesis and books. Discussion: Within the review, three categories were found (oculomotor, positional and caloric tests) and seven subcategories (spontaneous nystagmus, gaze-evoked nystagmus, tracking, saccades, optokinetic, Dix-Hallpike and roll test). Conclusion: The various elements found in this review are relevant in that they specify not only the type of vertigo but also its topographic location, favoring the evaluation-diagnosis process in the general population.

Abstract Background: Spinocerebellar ataxia type 3 (SCA3) is the most common autosomal dominant spinocerebellar ataxia worldwide. Almost all patients with SCA3 exhibit nystagmus and/or saccades impairment. Objective: To investigate the presence of nystagmus as an early neurological manifestation, before ataxia, in some patients with SCA3 in the first six months of the disease. Methods: We evaluated a series of 155 patients with clinically and molecularly proven SCA3 between 2013 and 2020. Data regarding sex, age, age at onset, disease duration, CAG repeat expansion length, first symptom, presence of ataxia, scores on SARA and ICARS scales, and presence and characteristics of nystagmus were collected. Results: We identified seven patients with symptomatic SCA3 who presented with isolated nystagmus. In these seven individuals the age at onset ranged from 24 to 57 years, and disease duration from four to six months. Conclusions: Our study showed that nystagmus may be the first neurological sign in SCA3. This clinical observation reinforces the idea that the neurodegenerative process in SCA3 patients may start in vestibular system connections or in flocculonodular lobe. This study adds relevant information about pre-symptomatic features in SCA3 that may work as basis for a better understanding of brain degeneration and for future therapeutic clinical trials.


RESUMO Antecedentes: A ataxia espinocerebelar tipo 3 (SCA3) é a ataxia espinocerebelar de herança autossômica dominante mais comum em todo o mundo. Quase todos os pacientes com SCA3 têm nistagmo e/ou comprometimento das sácades. Objetivo: Investigar a presença de nistagmo como manifestação neurológica precoce, antes do surgimento da ataxia, em alguns pacientes com SCA3 nos primeiros seis meses de doença. Métodos: Foram avaliados 155 pacientes com diagnóstico clínico e molecular de SCA3, entre 2013 e 2020, em relação a sexo, idade, idade de início, duração da doença, expansão da repetição CAG, primeiro sintoma, presença de ataxia, pontuações nas escalas SARA e ICARS, e presença e caracterização de nistagmo. Resultados: Identificamos sete pacientes com SCA3 que apresentavam nistagmo isolado. A idade de início da doença nesses pacientes variou de 24 a 57 anos e a duração da doença variou de quatro a seis meses. Conclusões: O nosso estudo mostrou que o nistagmo pode ser o primeiro sinal neurológico na SCA3. Essa observação clínica reforça a ideia de que o processo neurodegenerativo nos pacientes com SCA3 pode se iniciar nas conexões do sistema vestibular ou no lobo floculonodular. Este estudo adiciona informações relevantes sobre características pré-sintomáticas na SCA3 e que podem servir de base para melhor entendimento da degeneração cerebral e para futuras terapias.

ABSTRACT Microperimetry biofeedback training is a vision rehabilitation method that involves the training of attention and oculomotor control, and the rehabilitation of poorly located and non-functional preferred retinal loci. It can significantly improve distance and near visual acuity in age-related macular degeneration. Previous studies have shown that biofeedback training using electrical nystagmography can reduce nystagmus amplitude and increase foveation time. However, these improvements have not been sustained following training sessions. We hereby report a pediatric case of idiopathic nystagmus in an 11-year old patient treated with microperimetric biofeedback to improve visual acuity and fixation stability. The training had a beneficial impact, positively affecting fixation stability as well as distance and near reading vision. Subjectively, improvement in quality of life was also reported. Conversely to previous studies, the positive effects in this case were maintained for as long as twelve months following therapy. To the best of our knowledge, this is the first case with long-term benefits to be reported in the literature.(AU)


RESUMO O treinamento de biofeedback por microperimetria é um método de reabilitação da visão que envolve treinamento de atenção, controle oculomotor e reabilitação do locus preferencial de fixação da retina. Esse treinamento pode melhorar significativamente a acuidade visual para longe e perto na degeneração macular relacionada à idade. Estudos anteriores mostraram que o treinamento de biofeedback usando a nistagmografia elétrica pode reduzir a amplitude do nistagmo e aumentar o período de foveação. Entretanto, os resultados não se mantiveram após o término das sessões. Aqui é relatado um caso de tratamento com biofeedback por microperimetria para melhorar a acuidade visual e a estabilidade de fixação em uma criança de 11 anos de idade. O treinamento teve impacto benéfico e afetou positivamente a estabilidade da fixação e a visão para longe, para perto e de leitura. Subjetivamente, foi relatada melhoria da qualidade de vida. Em contraste com estudos anteriores, os efeitos positivos foram mantidos até 12 meses após a terapia. Até onde sabemos, este é o primeiro caso na literatura que relata benefícios de longo prazo.(AU)

RESUMEN El downbeat nystagmus o nistagmo vertical hacia abajo es el nistagmo de fijación adquirido más frecuente, que en la mayoría de los casos es causado por patología a nivel del sistema nervioso central que genera disrupción del control inhibitorio del flóculo y paraflóculo cerebeloso sobre los núcleos vestibulares. Entre sus causas se encuentran enfermedades neurodegenerativas y vasculares de cerebelo o tronco cerebral, tumores y traumas, pero cerca del 40% de los casos son idiopáticos y hasta la mitad de los pacientes presentan estudio imagenológico negativo. En este artículo presentamos dos casos que consultaron en el Servicio de Otorrinolaringología del Hospital Clínico de la Universidad de Chile.


ABSTRACT Downbeat nystagmus is the most frequent acquired fixation nystagmus and it is generally caused by central pathology disrupting the inhibitory control of the cerebellar flocculus and paraflocculus over the vestibular nuclei. Among its causes are neurodegenerative and vascular diseases of cerebellum or brainstem, tumors and trauma, but nearly 40% of the cases are idiopathic and up to half the patients have negative imaging study. In this article we present two cases that were evaluated in the Otolaryngology Department of the Clinical Hospital of the University of Chile.

ABSTRACT Changes in postural balance and visual complaints are frequent consequences of stroke. We aimed to investigate the symptoms and the vestibular and oculomotor functions of patients with dizziness post ischemic and hemorrhagic stroke and compare the results among them. Methods: Fifty patients with dizziness after stroke were evaluated through a clinical anamnesis and computerized vector electronystagmography: calibration of ocular movements, spontaneous nystagmus, semi-spontaneous nystagmus, pendular tracking, optokinetic nystagmus, rotary chair testing, and the caloric test. Results: All patients complained of dizziness, especially imbalance. Ischemic stroke in the carotid territory was the prevalent type. Visual complaints were reported by 56% of the sample and were related to abnormalities in oculomotor and caloric tests. Conclusion: The occurrence of visual symptoms was related to some abnormalities in the vector electronystagmography tests, being more frequent in cases of stroke in the vertebrobasilar system, and with oscillopsia and reduced visual acuity as symptoms.


RESUMO Alterações no equilíbrio postural são consequências frequentes no acidente vascular cerebral (AVC). O objetivo deste estudo foi investigar os sintomas e as funções vestibular e oculomotora de sujeitos com tontura após AVC isquêmico e hemorrágico, comparando seus resultados. Métodos: Foram avaliados 50 sujeitos com tontura após AVC, por meio de anamnese clínica e vectoeletronistagmografia computadorizada (VENG): calibração dos movimentos oculares; nistagmo espontâneo e semi-espontâneo; rastreio pendular; nistagmo optocinético; prova rotatória pendular decrescente e prova calórica com estímulo a ar. Resultados: Todos relataram tontura, principalmente do tipo desequilíbrio. O AVC isquêmico e no sistema carotídeo foi o mais frequente. Sintomas visuais pós-AVC foram referidos por 56% da amostra, os quais tiveram relação com alterações nas provas oculomotoras e calórica da VENG. Conclusão: A ocorrência de sintomas visuais relacionou-se a alterações em alguns testes. Essas alterações foram mais frequentes nos casos de AVC da circulação posterior, naqueles com oscilopsia e diminuição da acuidade visual.

ABSTRACT Objective To describe and compare the vestibular findings most evident among the hereditary ataxias, as well as correlate their clinical features with the nervous structures affected in this disease. Methods Seventy-five patients were evaluated and underwent a case history, otorhinolaryngological and vestibular assessments. Results Clinically, the patients commonly had symptoms of gait disturbances (67.1%), dizziness (47.3%), dysarthria (46%) and dysphagia (36.8%). In vestibular testing, alterations were predominantly evident in caloric testing (79%), testing for saccadic dysmetria (51%) and rotational chair testing (47%). The presence of alterations occurred in 87% of these patients. A majority of the alterations were from central vestibular dysfunction (69.3%). Conclusion This underscores the importance of the contribution of topodiagnostic labyrinthine evaluations for neurodegenerative diseases as, in most cases, the initial symptoms are otoneurological; and these evaluations should also be included in the selection of procedures to be performed in clinical and therapeutic monitoring.


RESUMO Objetivo Descrever e comparar os achados vestibulares mais evidentes entre a ataxia hereditária, bem como correlacionar seus aspectos clínicos com o estudo das estruturas nervosas afetadas nesta doença. Métodos 75 pacientes foram avaliados e submetidos aos seguintes procedimentos: anamnese, avaliação otorrinolaringológica e vestibular. Resultados Clinicamente, os pacientes apresentaram sintomas de distúrbios da marcha (67,1%), tonturas (47,3%), disartria (46%) e disfagia (36,8%). No teste vestibular, as alterações foram predominantemente evidentes no teste calórico (79%), dismetria sacádicas (51%) e no teste rotatório (47%). A presença de alterações ocorreu em 87% dos pacientes. A maioria das alterações observadas foram da disfunção vestibular central (69,3%). Conclusão O estudo ressalta a importância da contribuição da avaliação labiríntica no topodiagnóstico para doenças neurodegenerativas, uma vez que, na maioria dos casos, os sintomas iniciais são otoneurológicos, e essas avaliações também devem ser incluídas na seleção de procedimentos a serem realizados no monitoramento clínico e terapêutico.

RESUMO Estudo retrospectivo, transversal e descritivo, aprovado pelo Comitê de Ética em Pesquisa (CEP), sob o número CAAE 16728013.0.0000.5346. Trata-se de uma série de casos que tem por objetivo investigar a presença de sintomas otoneurológicos e o equilíbrio postural de seis pacientes com tontura após traumatismo cranioencefálico (TCE). Foram submetidos a uma anamnese clínica e a avaliações: teste de organização sensorial por meio da posturografia dinâmica foam laser e provas oculomotoras e vestibulares da vectoeletronistagmografia computadorizada (VENG). Os dados foram analisados a partir dos valores de referência para cada avaliação. Na posturografia, observou-se que as maiores diferenças entre o valor obtido e o valor de referência foram nas posições VI, V e III, respectivamente. A análise sensorial indicou alteração, principalmente, nos sistemas de preferência visual e vestibular. Todos os pacientes avaliados apresentaram ao menos uma alteração nas provas da VENG. Cinco pacientes tiveram alteração na prova calórica e nenhum apresentou alteração na prova rotatória pendular decrescente, que avalia a compensação vestibular. Além das queixas vestibulares, quatro pacientes estavam em tratamento com psicotrópicos para depressão. Tais resultados ratificam a presença de alterações vestíbulo-oculares no pós-TCE, os quais devem receber atenção especial devido ao comprometimento central associado.


ABSTRACT It is a retrospective, cross-sectional, descriptive, approved by Ethics Research Committee (ERC), under number CAAE 16728013.0.0000.5346. This is a series of cases that aims to investigate the presence of otoneurological symptoms and the postural balance of six patients with dizziness after Traumatic Brain Injury (TBI). Participants were submitted to a clinical anamnesis and the evaluations: Sensory Organization Test through Foam Laser Dynamic Posturography and oculomotor and vestibular tests of Computerized Vectoelectronystamography (VENG). The data were analyzed from the reference values for each evaluation. In posturography, it was observed that the greatest differences between the value obtained and the reference value were in positions VI, V and III, respectively. Sensory analysis indicated alteration mainly in the visual and vestibular preference systems. All the evaluated patients presented at least one alteration in the VENG tests. Five patients had alterations in the caloric test, and none presented alterations to the rotatory chair test (RCT), which evaluates the vestibular compensation. Considering vestibular complaints, four patients were on psychotropic treatment for depression. These results demonstrate the presence of vestibulo-ocular alterations in post-TBI, which should receive special attention due to associated central impairment.

RESUMEN Alteraciones autonómicas agudas por infartos cerebrales se han descrito con frecuencia comprometiendo la región insular, dentro de las anormalidades se han documentado alteraciones de severidad variable del ritmo cardíaco, incluyendo la bradicardia extrema sintomática; en la literatura solo hay un reporte de caso de bradi-cardia extrema asociado a un infarto talámico paramediano bilateral, a continuación, describimos un segundo caso de un infarto talámico con la misma presentación.


SUMMARY Acute autonomic disturbances due to cerebral in farcts has been describe frequently with association of the insular cortex, within the describe abnormalities had been document a variety of cardiac disturbances including extreme symptomatic bradycardia; In the literature just exist a case report of extreme bradycardia associate with a bilateral paramedian thalamic infarct, in the following article, we describe a second case of this infarct with the same presentation.

Abstract Introduction Interdisciplinary research has contributed greatly to an improved understanding of the vestibular system. To date, however, very little research has focused on the vestibular system's somatosensory afferents. To ensure the diagnostic quality of vestibular somatosensory afferent data, especially the extra cranial afferents, stimulation of the vestibular balance system has to be precluded. Objective Sophisticated movements require intra- and extra cranial vestibular receptors. The study's objective is to evaluate an investigation concept for cervico-vestibular afferents with respect to clinical feasibility. Methods A dedicated chair was constructed, permitting three-dimensional trunk excursions, during which the volunteer's head remains fixed. Whether or not a cervicotonic provocation nystagmus (c-PN) can be induced with static trunk excursion is to be evaluated and if this can be influenced by cervical monophasic transcutaneous electrical nerve stimulation (c-TENS) with a randomized test group. 3D-video-oculography (VOG) was used to record any change in cervico-ocular examination parameters. The occurring nystagmuses were evaluated visually due to the small caliber of nystagmus amplitudes in healthy volunteers. Results The results demonstrate: no influence of placebo-controlled c-TENS on the spontaneous nystagmus; a significant increase of the vertical nystagmus on the 3Dtrunk- excursion chair in static trunk flexion with cervical provocation in all young healthy volunteers (n = 49); and a significant difference between vertical and horizontal nystagmuses during static trunk excursion after placebo-controlled c-TENS, except for the horizontal nystagmus during trunk torsion. Conclusion We hope this cervicotonic investigation concept on the 3D trunk-excursion chair will contribute to new diagnostic and therapeutic perspectives on cervical pathologies in vestibular head-to-trunk alignment.

RESUMO Objetivo: Determinar o perfil do paciente albino no departamento de visão subnormal do Instituto Benjamin Constant. Ressaltar o tempo de acompanhamento, a frequência do seguimento, e a melhora visual com a adaptação de recursos ópticos e/ou eletrônicos. Métodos: Estudo retrospectivo com dados de 77 pacientes albinos com idade entre 1 a 53 anos de idade atendidos no Instituto Benjamin Constant, entre 2003 e 2014. Resultados: O recurso óptico mais adaptado foi o telescópio de Galilleu 2.8x. Todos os pacientes referiram ganho de visão com os equipamentos. A maioria dos pacientes apresentaram acuidade visual com recursos ópticos entre 20/25 e 20/160. Conclusão: Os recursos ópticos auxiliaram na melhora da função visual e na qualidade de vida dos pacientes com albinismo ocular.


ABSTRACT Objective: Determine the profile of albinism in Low Vision Department of Benjamin Constant Institute. Highlight the follow-up, the frequency of follow-up, and the visual improvement with the adaptation of optical and / or electronic resources . Methods: A retrospective study with data from 77 patients with ocular albinism aged 1- 53 years old attended at Benjamin Constant Institute between 2003 and 2014. Results: The most suitable optical feature is the telescope Galilleu 2.8x . All patients reported gain vision with the resources . Most patients experienced visual acuity with optical devices between 20/25 - 20/160 . Conclusion: Optical resources assisted in the improvement of visual function and quality of life of patients with ocular albinism.

RESUMO

El síndrome de nistagmo vertical hacia abajo (NVA) es una forma común de nistagmo de fijación adquirido que se presenta con nistagmo persistente con fase rápida en dirección descendente, mareo, oscilopsia y alteraciones de la marcha. Se considera un trastorno vestíbulo-cerebelar debido a un defecto en las células de Purkinje en el flóculo del cerebelo. Las causas reportadas con mayor frecuencia son los trastornos degenerativos cerebelares e isquemia cerebelar, sin embargo, en un gran porcentaje de los pacientes la etiología permanece incierta (forma idiopática). El NVA se puede dar en un contexto más amplio de neuropatía somatosensorial y ataxia cerebelar en el síndrome CANVAS. Las medidas terapéuticas incluyen evitar la posición supina y prona al descansar, rehabilitación vestibular y tratamiento farmacológico con aminopiridinas, entre otros. En este artículo presentamos dos casos de NVA así como la revisión de la literatura.


Downbeat nystagmus syndrome (DBN) is a frequent form of acquired fixation nystagmus, it presents with persisting nistagmus with fast phases directed downward, dizziness, oscillopsia and gait disturbances. It is considered a vestibulocerebellar disorder due to a bilateral defect of the Purkinje cells in the cerebellar flocculus. Most reported causes are degenerative disorders of the cerebellum and cerebellar ischemia, nevertheless the etiology remains unknown in a large percentage of patients (idiopathic form). DBN may present in a broader context of somatosensory neuropathy and cerebellar ataxia as in CANVAS syndrome. Therapeutic measures includes avoiding the supine and prone position when resting, vestibular rehabilitation, and pharmacologic treatment with aminopyridines, among others. In this article we present two cases of DBN and review of literature.

Introducción: las oscilaciones oculares involuntarias en la infancia pueden comprometer la agudeza visual del niño; de ahí la importancia de manejarlas adecuadamente. Objetivo: describir el tipo de nistagmos y el tratamiento indicado en cada paciente con síndrome de nistagmos infantil. Métodos: se realizó un estudio observacional, descriptivo, de corte transversal, de una serie de 60 pacientes con diagnóstico síndrome de nistagmos infantil. Se analizaron la edad, los tipos de nistagmos, los defectos refractivos asociados y los tratamientos indicados en cada caso. Se procesaron con el programa informático para análisis estadístico SPSS para Window, versión 2.1, y se utilizó la media y la mediana como medidas de tendencia central, y la desviación estándar y el rango intercuartílico como medidas de dispersión. Resultados: la edad promedio de los pacientes estudiados fue de 5,8 años. Predominaron los pacientes del sexo masculino (56,7 por ciento vs. 43,3 por ciento). El nistagmos sensorial fue el encontrado con mayor frecuencia (80,0 por ciento) y la mediana fue de 4 años 6 meses. La causa más frecuente de este tipo de nistagmos fue la hipoplasia papilar (20,8 por ciento), en uno y otro sexos. El astigmatismo hipermetrópico fue el defecto refractivo hallado con mayor frecuencia en estos pacientes. El tratamiento farmacológico fue el más utilizado (65,0 por ciento), en particular con dorzolamida 2 por ciento colirio (94,9 por ciento). El tratamiento quirúrgico se empleó en el 20,0 por ciento de los pacientes y la técnica más empleada fue la recesión de los 4 rectos horizontales. Conclusiones: los nistagmos sensoriales son los más frecuentes y se manejan fundamentalmente con tratamiento farmacológico(AU)


Introduction: the involuntary eye movements in childhood may compromise the child´s visual acuity; hence it is important to properly manage them. Objectives: to describe the type of nystagmus and the prescribed treatment for each patient suffering the infantile nystagmus syndrome. Methods: observational, descriptive and cross-sectional study conducted in 60 patients with diagnosis of infantile nystagmus syndrome. Age, types of nystagmus, associated refractive defects and prescribed treatments in each case were analyzed and processed with statistical analysis software SPSS for Windows, version 2.1. Mean and median; and standard deviation and interquartile range were used as central tendency and as dispersion measures, respectively. Results: the average age of the studied patients was 5,8 years. Men predominated (56,7 percent vs. 43,3 percent). The sensorial nystagmus was the most frequent (80 percent) and the median was 4 years and 6 months. The most common cause in this nystagmus type was papillary hypoplasia (20,8 percent) in both sexes. The hypermetropic astigmatism was the most found refractive defect in these patients. The drug treatment was the most used (65 percent), particularly 2 percent dorzolamide eyedrop (94,9 percent). The surgical treatment was used in 20 percent of the patients and the most used technique was recession of the 4 horizontal rectus muscle. Conclusions: sensorial nystagmus is the most frequent and they are fundamentally managed with drug treatment(AU)

Benign paroxysmal positional vertigo (BPPV), the most frequent cause of vertigo is associated with high morbidity in the elderly population. The most common form is linked to debris in the posterior semicircular canal. However, there has been an increasing number of reported BPPV cases involving the horizontal canals. The purpose of this article is to highlight the clinical features, diagnosis, and treatment in 37 patients with horizontal canal BPPV; twenty-six with geotropic nystagmus, and eleven with the apogeotropic form. Treatment consisted of the Gufoni manoeuver in eighteen patients (48.6%), the barbecue 360° maneuver in twelve patients (32.4%), both manoeuvers in four patients (10.8%), both manoeuvers plus head shaking in one patient (2.7%), and the Gufoni maneuver plus head shaking in two patients. Cupulolithiasis patients were asked to sleep in a forced prolonged position. We obtained a complete resolution of vertigo and nystagmus in 30 patients (81.0%) on the initial visit.


Vertigem posicional paroxística benigna (VPPB) é a causa mais frequente de vertigem e promove alta morbidade na população idosa. A forma mais comum está relacionada com otoconias no canal semicircular posterior. Entretanto, nos últimos anos identifica-se cada vez mais casos de VPPB dos canais horizontais. Os principais objetivos deste artigo são destacar as características clínicas, diagnóstico e tratamentos aplicados em 37 pacientes com VPPB do canal horizontal; vinte e seis com nistagmo geotrópico, e onze com nistagmo apogeotrópico. O tratamento consistiu na manobra de Gufoni em dezoito pacientes (48,6%) manobra do churrasco 360° em doze pacientes (32,4%) ambas as manobras em quatro pacientes (10,8%) ambas as manobras mais a manobra de sacudir a cabeça (MSC) em um paciente (2,7%), e manobra de Gufoni mais MSC em dois pacientes (2,7%). Pacientes com cupulolitíase dormiram uma noite na posição forçada prolongada. Em 30 pacientes (81,0%) o sucesso terapêutico ocorreu na primeira consulta.

Purpose: Evaluate the relationship between the postural alignment of the head and possible interference in the view of children. Methods: We evaluated 11 children between 2 and 7 years of age of both sexes, with the visually impaired, who had nystagmus and head lock position. The test Lea Grating Acuity Test® was used to collect measurements of visual acuity. This applied on two occasions: with and without postural alignment of the head. For reliability of the postural alignment of the head, the slopes were measured by Fisiologic® software. Results: The children had a poorer performance after physiological postural alignment. This poor performance is possible due to loss of position lock nystagmus to gain postural alignment, said to be ideal. Postural compensations were observed, and sharply increased eyestrain. Conclusion: The pursuit of traditional postural alignment affect the visual response of children with visual impairments. .


Objetivo: Avaliar a relação entre o alinhamento postural da cabeça e a possível interferência na visão de crianças. Métodos: Foram avaliadas 11 crianças, entre 2 e 7 anos de idade de ambos os sexos, com o diagnóstico de deficiência visual, que apresentavam nistagmo e posição de bloqueio de cabeça. O teste Lea Grating Acuity Test® foi utilizado para coletar medidas de acuidade visual. Este aplicado em dois momentos: sem e com o alinhamento postural da cabeça. Para confiabilidade do alinhamento postural da cabeça, as inclinações foram medidas pelo software Fisiologic®. Resultados: As crianças apresentaram pior desempenho após o alinhamento postural fisiológico. Este pior desempenho é possível devido à perda da posição de bloqueio do nistagmo para ganho do alinhamento postural, dito como ideal. Foram observadas compensações posturais e maior esforço visual. Conclusão: A busca do alinhamento postural tradicional prejudica a resposta visual de criança com deficiência visual. .

Pacientes após Acidente Vascular Cerebral (AVC) podem apresentar sintomas otoneurológicos. OBJETIVO: Avaliar a função vestibular de pacientes com antecedente pessoal de AVC no território carotídeo. Desenho científico: estudo de coorte histórica com corte transversal. MÉTODO: Quarenta pacientes foram submetidos à anamnese, exame otorrinolaringológico, Dizziness Handicap Inventory e vectoeletronistagmografia. RESULTADOS: Anormalidades discretas dos movimentos sacádicos foram encontradas em 20 pacientes (50,0%); nove referiram desequilíbrio e um tontura. O ganho do rastreio pendular foi anormal em 17 casos (42,5%); seis referiram desequilíbrio e um tontura. Preponderância direcional anormal do nistagmoperrotatório ocorreu em dois casos (5,0%), que referiram desequilíbrio. A prova calórica identificou três casos (7,5%) com predomínio labiríntico anormal e dois (5,0%) com preponderância direcional anormal do nistagmo; os cinco casos relataram desequilíbrio. Dos 11 pacientes que não referiram manifestações de alteração do equilíbrio corporal, 10 apresentaram alterações nos movimentos sacádicos e no rastreio pendular e um apresentou exame vestibular normal. CONCLUSÃO: Pacientes com antecedente pessoal de AVC no território carotídeo podem apresentar tontura ou desequilíbrio corporal e sinais de comprometimento da motilidade ocular e da função vestibular.


Stroke patients may present otoneurological symptoms. OBJECTIVE: To assess the vestibular function of subjects with a history of carotid territory stroke. METHOD: This historical cohort cross sectional study enrolled 40 patients; subjects answered the Dizziness Handicap Inventory, were interviewed and submitted to ENT examination and vectorelectronystagmography. RESULTS: Mild saccadic movement anomalies were seen in 20 patients (50.0%); nine complained of imbalance and dizziness. Abnormal smooth pursuit gain was seen in 17 cases (42.5%); six subjects reported imbalance and one complained of dizziness. Abnormal directional preponderance during rotational nystagmus was seen in two cases (5.0%), who also reported imbalance. Three patients (7.5%) and two subjects (5.0%) were found to have abnormal labyrinthine predominance and abnormal nystagmus directional preponderance respectively; all five individuals reported imbalance. Ten of the 11 patients without complaints of disordered balance had altered saccadic and smooth pursuit eye movements, while one had unaltered vestibular function. CONCLUSION: Patients with a history of carotid territory stroke may suffer from dizziness or imbalance and present signs of compromised eye motility and vestibular function.

Recientes estudios clínicos han revelado la utilidad del test del nistagmus inducido por la hiperventilación aguda, en la investigación de patologías vestibulares retrolaberínticas, neurales y de origen central, con la aparición de nistagmus paréticos o bien excitatorios. El propósito de este estudio ha sido: primero, conocer la sensibilidad de este examen según el análisis de los estudios de diferentes autores; segundo, entender la fisiopatología de este nistagmus inducido por la hiperventilación y tercero, efectuar una interpretación y análisis de este examen que permita obtener conclusiones sólidas del mejor nivel de medicina basada en evidencias. Para efectuar este estudio y análisis se procedió a una investigación exhaustiva de las publicaciones sobre el tema en los últimos 25 años, recurriendo para ello como fuentes de información a las principales bases de datos biomédicos y a la revisión en texto completo de estas publicaciones. Llamó la atención la escasez de trabajos clínicos publicados sobre el tema que no superan los 40, y también la escasez de trabajos sobre la fisiopatología del nistagmus inducido por la hiperventilación aguda. La metódica del test es muy simple por lo que su utilización resulta muy atractiva, con un escaso número de falsos positivos. La mayor parte de los estudios publicados, se focaliza en su utilización en el schwanoma vestibularyen la neuronitis vestibular. Respecto al schwanoma vestibular de un total de 107 pacientes estudiados el test fue positivo en el 80 por ciento de los casos, con aparición de nistagmus ipsilesional o contralesional. En la neuronitis vestibular la positividad del test alcanzó a 60 por ciento de un total de 272 pacientes estudiados y que estaba en relación directa con el tiempo de evolución de la enfermedad. En la esclerosis múltiple, en pacientes con síntomas vestibulares, la positividad del test fue de 75 por ciento, en cambio en aquellos pacientes sin síntomas vestibulares...


Recent clinical studies have revealed the usefulness of the hyperventilation-induced nystagmus in retrolabyrinthine and central vestibular diseases with the appearance either excitatory (ipsilateral) or paretic (contralateral) nystagmus. The objectives of this study have been: 1) to get to know the sensitivity of the exam, according to the best literature available; 2) to get to know the physiopathology of this hyperventilation-induced nystagmus, and 3) to carry out an analysis and interpretation of this exam in order to reach solid conclusions of the highest level in evidence based medicine. An exhaustive research of literature published on this topic in the last 25 years was carried out. The main source of information being the most important medical data-based publications and also a thorough revision of these texts. The scarce number of publications on this topic (not more than 40) is what strikes the attention most, as well as the very few studies of the physiopathology of the hyperventilation-induced nystagmus. The method of the test is a simple one; this fact makes its application an attractive one, with a low number of false positive. Most of the studies published are focused on its application in the vestibular schwannoma and in the vestibular neuritis. As to what the Vestibular schwannoma is concerned, ofa group of 107 patients studied, the test was positive in 80 percent of the cases, with the appearance of ipsilesional or contralesional nystagmus. In vestibular neuritis the test was positive in 68 percent of a number of 272 patients; this amount was in direct correlation with the time of evolution of the disease. In multiple sclerosis, in patients with vestibular symptoms, the test gave positive in 75 percent, whereas in those patients without vestibular symptoms it was positive only in 7 percent. In patients with cerebellar diseases, particularly degenerative ones this test was positive in 78 percent of the cases...

La anestesia regional combinada es utilizada frecuentemente como herramienta para el tratamiento del dolor postoperatorio. Los efectos secundarios de los opioides utilizados por esta vía son similares a los que se presentan luego de la administración sistémica. La aparición de vértigo con nistagmo vertical es un efecto adverso muy pocas veces descripto con el uso de morfina por vía intratecal, epidural o endovenosa. Comunicamos el caso de un paciente que presentó esta complicación en el postoperatorio de una nefrectomía parcial, luego de la administración de morfina intratecal, con resolución completa mediante el uso de naloxona endovenosa.


Combined regional anesthesia is frequently used as a tool for management of postoperative pain. The profile of side effects of the opioids used via this route is similar to those occurring after systemic administration. The onset of vertigo with vertical nystagmus is an adverse effect rarely described after the use of intrathecal, epidural or intravenous morphine. We report the case of a patient who presented this complication in the postoperative period of a partial nephrectomy, after the administration of intrathecal morphine, with complete resolution by intravenous naloxone.
