RESUMO

O lúpus eritematoso é uma doença autoimune complexa que afeta diversos órgãos. A pele é o segundo local mais acometido e as manifestações cutâneas são divididas em específicas e não específicas. As primeiras possuem em comum o achado histopatológico de dermatite de interface com degeneração vacuolar da camada basal e infiltrado linfocitário na junção dermoepidérmica e são subdivididas em agudas, subagudas e crônicas. As manifestações não específicas compõem um grupo heterogêneo de condições que frequentemente está associado ao lúpus eritematoso sistêmico. O conhecimento e a interpretação adequada de tais manifestações é importante, pois, além da relevância diagnóstica, as lesões cutâneas nos dão informações prognósticas. Portanto, é imprescindível que o clínico conheça as principais manifestações cutâneas do lúpus eritematoso para a melhor condução desses pacientes.

Abstract Background: Clinical and histological features may overlap between lichen planopilaris-associated and discoid lupus erythematosus-associated scarring alopecia. Objectives: The aim of this study was to demonstrate the cutaneous infiltration of plasmacytoid dendritic cells and to compare their distribution pattern in discoid lupus erythematosus and lichen planopilaris. Methods: Twenty-four cases of discoid lupus erythematosus and 30 cases of lichen planopilaris were examined for immunostaining of the CD123 marker. The percentage and distribution pattern of plasmacytoid dendritic cells and the presence of the plasmacytoid dendritic cells clusters were evaluted in the samples. Results: The number of plasmacytoid dendritic cells was higher in the discoid lupus erythematosus specimens. Aggregations of 10 cells or more (large cluster) were observed in half of the discoid lupus erythematosus specimens and only 2 lichen planopilaris, with 50% sensitivity and 93% specificity for differentiating discoid lupus erythematosus from lichen planopilaris. Study limitations: Incidence and prevalence of discoid lupus erythematosus-associated scarring alopecia in the scalp are low, so the samples size of our study was small. Conclusions: We suggest that a plasmacytoid dendritic cells cluster of 10 cells or more is highly specific for distinguishing discoid lupus erythematosus from lichen planopilaris. It also appears that CD123 immunolabeling is valuable in both active and late stages of the disease.

Abstract: Lupus erythemathosus is a chronic, relapsing disease with acute, subacute, and chronic lesions. Effluvium telogen occurs in the setting of systemic activity of the disease, and cicatricial alopecia results from discoid lesionsin on the scalp. Other types of alopecia, like alopecia areata, may rarely be found in lupus. Frontal fibrosing alopecia is characterized by frontotemporal hairline recession and eybrow loss. Histophatologically, it cannot be differentiated from lichen planopilaris.It is controversial whether frontal fibrosing alopecia is a subtype of lichen planopilaris.. A pacient with chronic lichenoid lupus erythematosus is described with clinical, histophatological and dermoscopic features of frontal fibrosing alopecia.We have not been able to find in the literature cases of frontal fibrosing alopecia as a clinical manifestation of lupus.

Introducción: El lupus eritematoso infantil incluye las distintas variedades de lupus eritematoso del adulto y además una forma exclusiva de la edad pediátrica: el lupus neonatal. Aunque el lupus eritematoso sistémico, subagudo y discoide pediátricos comparten las características esenciales de las formas correspondientes en los adultos, presentan algunas peculiaridades. Casos: Describimos cinco casos clínicos de lupus infantil, de distintos tipos, dos casos del subtipo discoide, dos subagudos y un ampollar. Discusión: Entre las diferencias más importantes cabe destacar que en el lupus sistémico infantil es más frecuente encontrar nefropatía, fiebre y linfadenopatías al inicio de la enfermedad que en adultos. Durante el desarrollo de la enfermedad es más común el exantema malar. El lupus subagudo es excepcional en la infancia.


Introduction: Systemic lupus erythematosus in children includes adult different varieties plus and exclusive pediatric presentation, neonatal lupus. Case: We describe five cases of SLE in children, two cases of discoid subtype, two subacute and one blistering. Discusion: Although pediatric systemic lupus erythematosus discoid and subacute share the essential features of the corresponding forms in adults, they also have some peculiarities. Among the most frequent are nephropathy, fever and lymphadenopathy at the onset of the disease, whereas, during the development of the disease malar rash is quite common.

Abstract A 40-year-old female patient with a 5-year history of systemic lupus erythematosus was referred to our policlinic with complaints of erythema, atrophy, and telangiectasia on the upper eyelids for 8 months. No associated mucocutaneous lesion was present. Biopsy taken by our ophthalmology department revealed discoid lupus erythematosus. Topical tacrolimus was augmented to the systemic therapeutic regimen of the patient, which consisted of continuous antimalarial treatment and intermittent corticosteroid drugs. We observed no remission in spite of the 6-month supervised therapy. Periorbital discoid lupus erythematosus is very unusual and should be considered in the differential diagnosis of erythematous lesions of the periorbital area..

RESUMO

Abstract Frontal fibrosing alopecia is a distinctive form of scarring alopecia considered to be a clinical variant of lichen planopilaris. It predominantly occurs in postmenopausal women and has a slowly progressive course. It was first described by Kossard in 1994. Since then the number of reported cases has increased significantly. Coexistence of frontal fibrosing alopecia and autoimmune disorders - such as discoid erythematosus lupus and Sjögren's syndrome - may suggest a common pathogenic background among the diseases.

Abstract Objective To determine whether there is an association between the profile of cognitive dysfunction and academic outcomes in patients with juvenile systemic lupus erythematosus (JSLE). Methods Patients aged ≤18 years at the onset of the disease and education level at or above the fifth grade of elementary school were selected. Cognitive evaluation was performed according to the American College of Rheumatology (ACR) recommendations. Symptoms of anxiety and depression were assessed by Beck scales; disease activity was assessed by Systemic Lupus Erythematosus Disease Activity Index (SLEDAI); and cumulative damage was assessed by Systemic Lupus International Collaborating Clinics (SLICC). The presence of autoantibodies and medication use were also assessed. A significance level of 5% (p < 0.05) was adopted. Results 41 patients with a mean age of 14.5 ± 2.84 years were included. Cognitive dysfunction was noted in 17 (41.46%) patients. There was a significant worsening in mathematical performance in patients with cognitive dysfunction (p = 0.039). Anxiety symptoms were observed in 8 patients (19.51%) and were associated with visual perception (p = 0.037) and symptoms of depression were observed in 1 patient (2.43%). Conclusion Patients with JSLE concomitantly with cognitive dysfunction showed worse academic performance in mathematics compared to patients without cognitive impairment.


Resumo Objetivo Determinar se há associação entre o perfil de disfunção cognitiva e os resultados acadêmicos em pacientes com lúpus eritematoso sistêmico juvenil (LESj). Métodos Foram selecionados pacientes com idade de início da doença ≤ 18 anos e com escolaridade mínima do quinto ano do Ensino Fundamental seguidos em um hospital universitário. A avaliação cognitiva foi feita de acordo com as recomendações do Colégio Americano de Reumatologia (ACR). Os sintomas de ansiedade e depressão foram avaliados pelas escalas Beck, a atividade da doença foi avaliada pelo Systemic Lupus Erythematosus Disease Activity Index (Sledai) e o dano cumulativo pelo Systemic Lupus International Collaborating Clinics (Slicc). Também foram avaliados a presença de autoanticorpos e o uso de medicação. Adotou-se nível de significância de 5% (p < 0,05). Resultados Foram incluídos 41 pacientes com média de 14,5 ± 2,84 anos. Disfunção cognitiva foi observada em 17 (41,46%). Observou-se pioria significativa no desempenho de matemática em pacientes com disfunção cognitiva (p = 0,039). Sintomas de ansiedade foram observados em oito pacientes (19,51%) e estavam associados à percepção visual (p = 0,037) e sintomas de depressão foram observados em um paciente (2,43%). Conclusão Pacientes com LESj com disfunção cognitiva apresentam pior desempenho acadêmico em matemática em relação a pacientes sem disfunção cognitiva.

Abstract

We present the rare case of a 47-year-old patient, suffering from cheilitis granulomatosa and lupus erythematosus discoid: this association is really exceptional because only once reported in English literature. In addition, the treatment of cheilitis granulomatosa is a challenge for the dermatologist: the gold standard, represented by steroids, is in fact designed as a short-time option. Our report confi rms the good efficacy of methotrexate as a steroid-sparing agent.

Introduction The Young Doctor Project (YDP) uses Telehealth and Interactive Teleducation instruments to promote the integration of different areas of health and to build knowledge. This methodology can also foster public awareness on various issues related to health. In this context, the objective of this study was to emphasize cleft lip and palate (CLP), which is one of the most common birth defects in Brazil. Objective The study aimed to apply a model of education regarding CLP, based on the dynamics of the YDP, and to evaluate the participants' knowledge acquired after participating in the YDP. Methods The participants were 41 students, 13 to 15 years of age and at the eight- and ninth-grade levels in a private elementary school in Bauru (Brazil). To analyze the performance of the participants, a questionnaire was administered before and after the completion of the training program. The training program was structured in three steps using: (1) interactive teleducation classes, (2) a cybertutor, and (3) practical activities. Results There was a statistically significant difference between the pre- and postparticipation questionnaire results. The improved performance of participants is evidenced by the increase in the rate of correct answers on all issues. Conclusion The YDP on CLP was applied in the school setting following the three steps recommended by the project, and, after the implementation of the training program, there was a significant increase in participants' knowledge of CLP. The YDP on CLP proved an effective tool in promoting health education. .

El lupus eritematoso es una dermatosis inflamatoria, crónica, aguda o subaguda que evoluciona por brotes. Puede afectar a todos los órganos pero más frecuentemente a la piel. Presentamos a dos pacientes con lesión en forma de placa, localizada en zona interna y media del borde libre palpebral inferior, de bordes elevados, depresión central con fondo costroso y pérdida de las pestañas a ese nivel. Se decide -por la localización, extensión de la lesión, los antecedentes personales y la aplicación de tratamientos previos sin mejoría del cuadro clínico- realizar biopsia incisional para un mejor estudio. Los resultados anatomopatológicos revelaron el diagnóstico de lupus discoide palpebral. La evolución posterior al tratamiento médico fue satisfactoria, sin recidiva de la lesión durante el primer año de seguimiento.


Erythematosus lupus is an inflammatory dermatosis of chronic, acute or subacute nature that occurs in outbreaks. It can affect all the body organs, but mainly the skin. Here are two patients with plaque-form lesions located in the internal middle area of the free inferior palpebral border, thick contours, central depression with scabby bottom and loss of eyelash. Taking into account the location, the extension of the lesion, personal histories and application of previous treatment without improved clinical picture, it was decided to perform incisional biopsy for better examination. The anatomopathological results showed the diagnosis of palpebral discoid lupus. The recovery of the patient was satisfactory after the medical treatment, with no relapse during the first follow-up year.

BACKGROUND: It has been observed that patients with systemic lupus erythematosus and discoid lesions have a milder systemic disease. OBJECTIVE: To compare the clinical, demographic and autoantibody profile of systemic lupus erythematosus patients with and without discoid lesions. METHODS: We carried out a retrospective study involving 288 systemic lupus erythematosus patients who met at least four classification criteria of the American College of Rheumatology for systemic lupus erythematosus, comparing the clinical, serological and demographic factors between patients with and without discoid manifestations. RESULTS: Of the 288 patients, 13.8% had discoid lesions. Univariate analysis found no differences in the prevalence of malar rash, photosensitivity, arthritis, serositis, leukopenia, lymphopenia and hemolytic anemia or anemia of the central nervous system (p = ns). Renal lesions were more common in those without discoid lesions (p =0.016), and hemolysis (p<0.0001) was more common in those with discoid lesions. Regarding the profile of autoantibodies, only the anti-RNP antibody was more common in those with discoid events (p =0.04). In a logistic regression study, only the renal lesions and anti-RNP maintained their associations with discoid manifestations. CONCLUSION: Patients with lesions of systemic lupus erythematosus and discoid lesions have lower prevalence of renal involvement and a greater presence of anti RNP. .


FUNDAMENTOS: Existe a observação de que pacientes com lúpus eritematoso sistêmico e lesões discoides têm uma doença sistêmica mais branda. OBJETIVO: Comparar o perfil clínico, demográfico e de autoanticorpos de pacientes com lúpus eritematoso sistêmico com e sem lesões discoides. MÉTODOS: Estudo retrospectivo de 288 pacientes com lúpus eritematoso sistêmico que satisfizeram pelo menos 4 critérios classificatórios do Colégio Americano de Reumatologia para o diagnóstico de lúpus eritematoso sistêmico, comparando-se os achados clínicos, sorológicos e demográficos entre pacientes com e sem manifestações discoides. RESULTADOS: Dos 288 pacientes, 13,8% tinham lesões discoides. Na análise univariada não se encontraram diferenças quanto à prevalência de eritema malar, fotossensibilidade, artrite, serosite, leucopenia, linfopenia e anemia hemolítica ou de sistema nervoso central (p=ns). Lesões renais foram mais comuns naqueles sem lesão discoide (p =0,016), e a hemólise (p<0.0001) foi mais comum nos com lesão discoide. No que se refere ao perfil de autoanticorpos apenas o anticorpo anti-RNP foi mais comum naqueles com manifestações discoides (p=0,04). Em estudo por regressão logística, só as lesões renais e o anticorpo anti-RNP mantiveram suas associações com manifestações discoides. CONCLUSÃO: Pacientes de lúpus eritematoso sistêmico com lesões discoides têm menor prevalência de envolvimento renal e uma maior presença do anticorpo anti RNP. .

El lupus eritematoso discoide crónico hipertrófico (LEDCH) es una variedad poco frecuente del lupus eritematoso discoidecrónico (LEDC) y se manifiesta con lesiones hiperqueratósicas que comprometen áreas fotoexpuestas. Generalmente el cursoclínico está marcado por la cronicidad y resistencia al tratamiento. Presentamos una paciente con lesiones de LEDCH querespondió satisfactoriamente al tratamiento con fotoprotección, corticoides tópicos y sistémicos y antimaláricos. Las lesiones desaparecieron, dejando secuelas pigmentarias residuales.


Hypertrophic chronic discoid lupus erythematosus (LEDCH) is a rare variety of chronic discoid lupus erythematosus (LEDC)characterized by hyperkeratotic lesions that compromise photoexposed areas. Clinical course is marked by chronicity andtreatment resistance. A female patient with LEDCH successfully treated with photoprotection, topical and systemic corticosteroidsand antimalarials is presented. Lesions disappeared leaving residual pigmentary sequelae.

O lúpus eritematoso discóide é uma desordem autoimune que geralmente afeta áreas da pele expostas ao sol. A apresentação de lesões palpebrais na ausência de outras anormalidades cutâneas é incomum, sendo o envolvimento da pálpebra inferior prevalente em apenas 6% dos pacientes com lúpus eritematoso cutâneo crônico. Relatamos o caso de uma paciente do sexo feminino de 40 anos, com hiperemia, madarose e ulceração na pálpebra inferior do olho esquerdo refratária ao tratamento para blefarite. Inicialmente, outra lesão semelhante havia sido descrita na pálpebra superior do olho direito. No entanto, as biópsias incisionais mostraram-se inconclusivas. Diante de uma lesão migratória palpebral de características semelhantes à primeira, a hipótese clínica de lúpus eritematoso discóide foi aventada e o diagnóstico confirmado por meio de revisão histopatológica. Uma forte suspeita clínica e o reconhecimento precoce podem evitar erros diagnósticos, complicações clínicas e tratamentos inapropriados, como descrito neste caso de lesão palpebral como manifestação primária do lúpus eritematoso discóide.


The discoid lupus erythematosus is an autoimmune disorder which generally affects the sun-exposed skin. Presentation of lesions on the eyelids in the absence of any other cutaneous abnormality is uncommon and the lower-eyelid involvement is seen in 6% of patients with cronic cutaneous lupus erythematosus. We have reported the case of a 40 year-old, woman who presented hyperemia, madarosis and ulceration on the lower eyelid of the left eye. She was treated for blepharitis without resolution. Before that, another similar lesion had been described on the upper eyelid of the right eye. Nevertheless, the incisional biopsies of that eyelid were inconclusive. Faced with a migratory lesion similar to the first one, the clinical hypothesis of discoid lupus erythematosus was suggested and diagnosis was confirmed by histopathological review. A high index of suspicion and early recognition may prevent misdiagnosis, clinical complications and inappropriate treatment, as described in the case of eyelid lesion as a primary manifestation of discoid lupus erythematosus.

While most of those infected with hepatitis C virus (HCV) are asymptomatic or only develop liver manifestations, a significant percentage evolves with autoimmune and lymphoproliferative disorders, resulting in a clinical condition called HCV syndrome. This work involving case studies of six patients with hepatitis C and varied skin manifestation aimed to report skin lesions occurring with HCV infection and its treatment. Skin manifestations in hepatitis C have been based on epidemiological studies. This justifies the need for studies that correlate HCV infection and its treatment with skin manifestations.


A maioria dos infectados pelo vírus da hepatite C (VHC) é assintomática ou apresenta somente manifestações hepáticas, porém uma significativa porcentagem evolui com desordens autoimunes e linfoproliferativas resultando na síndrome da hepatite C. Este trabalho envolve seis estudos de casos de pacientes com hepatite C e várias manifestações cutâneas, visando relacionar lesões dermatológicas ao VHC e ao seu tratamento. Manifestações dermatológicas na hepatite C têm sido baseadas em estudos epidemiológicos. Isto justifica a necessidade de estudos que correlacionem infecção pelo VHC e seu tratamento com as manifestações dermatológicas.

El desprendimiento seroso de retina produce disminución de la agudeza visual debido al paso de fluido procedente de la coroides hacia el espacio subretiniano. Esta enfermedad tiene varias causas, entre las que se encuentran las idiopáticas, congénitas, posquirúrgicas, secundarias e uveítis infecciosas, autoinmune, vascular, hematológicas y neoplásicas.Objetivo: describir el caso inusual de un paciente joven con desprendimiento seroso de retina asociado a lupus fijo discoide y nefropatía por inmunoglobulina A con respuesta satisfactoria al uso de esteroides.Caso clínico: se presenta un paciente masculino de 18 años de edad que acudió a consulta de retina del centro oftalmológico del Hospital Universitario Manuel Ascunce Domenech, por disminución súbita de la agudeza visual en ambos ojos con diagnóstico inicial de coroidopatia serosa central, que posteriormente evolucionó hacia desprendimientos serosos bilaterales con afectación de todo el polo posterior. Por sus antecedentes se interconsultó con el especialista en dermatología y nefrología, donde se decidió iniciar tratamiento con esteroides a altas dosis, vitaminoterapia endovenosa, así como antiinflamatorios y midriáticos ciclopléjicos tópicos con respuesta excelente a la terapia


Serous retinal detachment causes visual acuity decrease due to the passage of fluid from the choroid into the subretinal space. This disease has several causes including idiopathic, congenital, postoperative, and secondary to infectious uveitis, autoimmune, vascular, hematological and neoplastic ones.Objective: to describe an unusual case of a young patient with serous retinal detachment associated with discoid fixed lupus and nephropathy by immunoglobulin A with satisfactory response to steroids use.Clinical case: a male patient of 18 years old was presented in the retina office of the Ophthalmological Center at the University Hospital Manuel Ascunce Domenech with sudden decrease of visual acuity in both eyes with an initial diagnosis of serous central choroidopathy, which later progressed into bilateral serous detachments with entire posterior pole involvement. By his antecedents, a specialty consultation in dermatology and nephrology was carried out, deciding to initiate treatment with high-dose steroids, intravenous vitamin therapy, and anti-inflammatory and topical cycloplegic mydriatic with excellent response to therapy

Lúpus eritematoso cutâneo crônico é doença autoimune, com apresentações polimorfas que podem, eventualmente, mimetizar outras condições clínicas, causando dificuldade diagnóstica. Quadro acneiforme, com comedões e lesões atróficas pontuadas (pitting scars) pode constituir apresentação atípica do lúpus eritematoso cutâneo discóide.


Chronic cutaneous lupus erythematosus is a polymorphous autoimmune disease which may mimic some other clinical conditions, causing diagnostic difficulties. Acneiform lesions, including comedones and pitting scars are occasionally atypical presentations of cutaneous discoid lupus erythematosus.

O lúpus eritematoso crônico discoide linear é manifestação rara da doença lúpica cutânea, em que lesões eritêmato-atrófico-discrômicas dispõem-se nas linhas de Blaschko. Descrevemos o caso de um menino de 15 anos, com dois anos de história de lesões discoides eritêmato-atróficas, hipo e hiperpigmentadas, dispostas nas linhas de Blaschko do membro superior direito. O exame histopatológico revelou atrofia da epiderme, hiperqueratose, rolhas córneas, espessamento da zona da membrana basal, infiltrado inflamatório crônico perianexial e perivascular superficial e profundo, depósito de mucina na derme, confirmando o diagnóstico. Há, no total, 14 casos descritos dessa variante que se inicia frequentemente na infância e que não apresenta predomínio quanto ao sexo. As lesões ocorrem preferencialmente na face. Não há relatos de associação com doença sistêmica.


Linear chronic discoid lupus erythematosus is a rare manifestation of cutaneous lupus in which erythematous, atrophic, dyschromic lesions are located along the lines of Blaschko. This report describes the case of a 15-year old boy with a 2-year history of discoid, erythematous, hyper and hypopigmented lesions with central atrophy, situated along the lines of Blaschko on his right arm. Histopathology showed epidermal atrophy, hyperkeratosis, follicular plugging, thickening of the basement membrane zone, and superficial and deep chronic perivascular and periadnexal inflammatory infiltrate, with dermal mucin deposit, thus confirming diagnosis. A total of 14 cases have been described of this variant, the onset of which is often in childhood. There is no difference in incidence between genders. Lesions most commonly develop on the face. There have been no reports of any association with systemic disease.

RESUMO

O lúpus eritematoso cutâneo crônico, caracterizado, sobretudo, pelo lúpus discoide, é uma entidade clínica incomum, porém de elevada prevalência em mulheres em idade fértil. A sua etiologia é desconhecida, mas fatores genéticos, autoimunes, hormonais e ambientais compõem o processo fisiopatológico da doença. Os meios diagnósticos utilizados para que se possa iniciar o tratamento específico, composto de protetores solares, corticosteroides tópicos e, se preciso, medicações sistêmicas, principalmente os antimaláricos, são o exame clínico, a imunofluorescência direta e o estudo histopatológico. Este artigo descreve de maneira sucinta os principais aspectos epidemiológicos, clínicos, diagnósticos e terapêuticos do lúpus eritematoso cutâneo crônico, conforme revisão de literatura.

Introducción. El lupus eritematoso (LE) es una enfermedad inflamatoria multisistémica de etiología desconocida que comprende una variedad de formas clínicas. Material y método. Se efectuó un estudio retrospectivo y descriptivo de los pacientes que consultaron en el Servicio de Dermatología del Policlínico Bancario durante el período comprendido entre enero de 1994 y enero de 2009. Se incluyeron aquellos pacientes mayores de 18 años con diagnóstico histopatológico de lupus eritematoso. Objetivos. Identificar las distintas variantes clínicas de lupus eritematoso, establecer su distribución por sexo y edad, y comparar los resultados con las estadísticas nacionales e internacionales. Resultados. Se evaluaron 46 pacientes (33 mujeres [71,73%]).Edad promedio al diagnóstico: 42 años; rango: 19-70 años. Presentaron lesiones específicas de LE 42 pacientes (91,30%), 28 correspondieron a la variante de lupus eritematoso cutáneo crónico (67%), 6 a lupus eritematoso cutáneo subagudo (14%) y 8 a lupus eritematoso cutáneo agudo (19%). Las lesiones inespecíficas se presentaron en 21 pacientes (45%) y las halladas con mayor frecuencia fueron: fotosensibilidad (38,09%) alopecia difusa (33,33%) y alteraciones vasculares (28,57%). Conclusión. Nuestros hallazgos son similares a los señalados en la bibliografía consultada, a excepción de la edad de presentación, que fue superior.
