Resumen Introducción: Los inhibidores del punto de control inmunológico (IPCi) son utilizados en los últimos años en el tratamiento de neoplasias malignas avanzadas, con ellos se ha logrado un aumento significativo de la supervivencia; sin embargo, su uso se ha asociado a incremento del riesgo de enfermedades autoinmunes. Objetivo: Describir la incidencia y las características clínicas de los pacientes tratados con IPCi que desarrollaron tiroidopatía. Métodos: Se revisaron retrospectivamente los expedientes de todos los pacientes que recibieron IPCi en los últimos tres años y se identificaron aquellos que desarrollaron anomalías tiroideas. Resultados: La prevalencia de tiroiditis fue de 7 %, con una incidencia de 21.4 % pacientes/mes. La mediana del tiempo para el desarrollo de tiroiditis fue de 63 días. La mayoría de los pacientes presentó síntomas leves o moderados y no requirió hospitalización, si bien todos menos uno desarrollaron hipotiroidismo permanente y requirieron terapia de reemplazo hormonal con levotiroxina. Conclusiones: La disfunción tiroidea secundaria a inmunoterapia es una entidad común en nuestra población. El cuadro clínico suele ser leve y no requiere suspender el tratamiento; sin embargo, debido a la alta incidencia de este evento adverso, los médicos no oncólogos deben estar familiarizados con su diagnóstico y tratamiento, para brindar un manejo multidisciplinario.


Abstract Introduction: Immune checkpoint inhibitors (ICI) are a group of drugs that have been used in recent years for the treatment of advanced malignancies such as melanoma, non-small cell lung cancer and other tumors, significantly increasing survival. However, the use of ICI has been associated with an increased risk of autoimmune diseases, with endocrine organs, specifically the thyroid, being highly susceptible to this phenomenon. Objective: To describe the incidence and clinical characteristics of patients treated with ICI who develop thyroid disease. Methods: The medical records of all patients who received ICI treatment within the last three years were retrospectively reviewed, with those who developed thyroid abnormalities being identified. Results: The prevalence of thyroiditis was 7 %, with an incidence of 21.4 % of patients-month. Median time for the development of thyroiditis was 63 days. Most patients had mild or moderate symptoms and did not require hospitalization, although all but one developed permanent hypothyroidism and required hormone replacement therapy with levothyroxine. Conclusions: Thyroid dysfunction secondary to immunotherapy is a common entity in our population. Clinical presentation is usually mild and does not require treatment discontinuation; however, due to the high incidence of these adverse events, non-oncology specialists must be familiar with the diagnosis and treatment of these alterations in order to provide multidisciplinary management.

ABSTRACT Objectives The differentiation between the various etiologies of thyrotoxicosis, including those with hyperthyroidism (especially Graves' disease [GD], the most common cause of hyperthyroidism) and without hyperthyroidism (like thyroiditis), is an important step in planning specific therapy. Technetium-99m (99mTc) pertechnetate thyroid scanning is the gold standard in differentiating GD from thyroiditis. However, this technique has limited availability, is contraindicated in pregnancy and lactation, and is not helpful in cases with history of recent exposure to excess iodine. The aim of this study was to identify the diagnostic value of the peak systolic velocity of the inferior thyroid artery (PSV-ITA) assessed by color-flow Doppler ultrasound (CFDU) and compare the sensitivity and specificity of this method versus 99mTc pertechnetate thyroid uptake. Subjects and methods We prospectively analyzed 65 patients (46 with GD and 19 with thyroiditis). All patients were evaluated with clinical history and physical examination and underwent 99mTc pertechnetate scanning and measurement of TRAb levels and PSV-ITA values by CFDU. The diagnosis was based on findings from signs and symptoms, physical examination, and 99mTc pertechnetate uptake. Results Patients with GD had significantly higher mean PSV-ITA values than those with thyroiditis. At a mean PSV-ITA cutoff value of 30 cm/sec, PSV-ITA discriminated GD from thyroiditis with a sensitivity of 91% and specificity of 89%. Conclusion Measurement of PSV-ITA by CFDU is a good diagnostic approach to discriminate between GD and thyroiditis, with sensitivity and specificity values comparable to those of 99mTc pertechnetate thyroid uptake.

RESUMEN El lupus eritematoso sistémico se puede presentar con un amplio espectro de síntomas que en algunas ocasiones pueden enmascarar complicaciones graves asociadas a la misma enfermedad. Dentro de estas la pancreatitis es una causa poco común, y sin embargo de alta mortalidad, especialmente en pacientes con un tratamiento no oportuno. Reportamos el caso de una paciente que cursa con lupus eritematoso sistémico con compromiso renal y de sistema nervioso central, de reciente aparición, que se asocia a la aparición de pancreatitis y tiroiditis, presentando evolución satisfactoria con esquema terapéutico de ciclofosfamida y prednisolona.


ABSTRACT Systemic lupus erythematosus can present with a broad spectrum of symptoms that on some occasions may mask serious complications associated with the same disease. Within these, pancreatitis is an uncommon but high-mortality cause, especially in patients with non-oportune treatment. We report the case of a patient with systemic lupus eryt-hematosus with recent renal and central nervous system involvement that is associated with the onset of pancreatitis and thyroiditis. A satisfactory outcome was obtained with a cyclophosphamide and prednisolone therapeutic regimen.

INTRODUCTION: Riedel´s thyroiditis is a thyroid inflammation with invasive fibrosis. CASE REPORT: Female 84 years old, studied by dysphagia to liquids and dysphonia, with cervical mass poorly demarcated and immobile. Computed Tomography (CT) (CT) showed tumor that diverted the trachea and esophagus compressed. Fine Needle Aspiration (FNA) obtained cells with atypia. Thyroidectomy was performed, appreciating a stony tumor which included the right recurrent laryngeal nerve. Given the high suspicion of thyroid malignancy tracheostomy it was performed. The pathology report as fibrosing Riedel thyroiditis. DISCUSSION: This is the less common type of chronic thyroiditis. Etiology is unknown. The normal thyroid parenchyma is replaced by fibrous connective tissue. Clinically usually a hard mass, fast growing, why is confused with thyroid carcinoma and Hashimoto's thyroiditis. It´ss more common in women and is usually bilateral. The CT or MRI (Magnetic Resonance Imaging) help assess the extent. Puncturing the mass could give us the diagnosis but is often difficult. Treatment to relieve symptoms or rule out malignancy is surgery. The prognosis is usually good. CONCLUSIONS: Riedel´s thyroiditis is an entity with histopathologic diagnosis essential

La parálisis periódica tirotóxica hipocalémica es una patología de muy baja frecuencia en la Argentina; no obstante, es muy importante tenerla en cuenta como urgencia, ya que puede ser fatal si el diagnóstico y el tratamiento correctos no son oportunos. El objetivo de esta investigación bibliográfica es destacar el aumento de la incidencia de una enfermedad potencialmente fatal y, a la vez, fácilmente reversible con el tratamiento adecuado y oportuno, en las Salas de Guardia de nuestro país, a fin de que se la tenga en cuenta en las sospechas diagnósticas de emergencia y el aporte que puede hacer el bioquímico desde un Servicio de Urgencia, para ayudar al diagnóstico en forma rápida y certera.(AU)


Hypokalemic thyrotoxic periodic paralysis is a disease of very low frequency in Argentina; however, it is very important to take it into account as an emergency, because it can be fatal without a timely diagnosis and treatment. The objective of this research is to highlight its increased incidence in the emergency rooms of our country (it is a potentially fatal disease, but at the same time it is easily reversible with a proper and timely treatment), in order to include it in the emergency diagnostic suspicion and the biochemist contribution in an emergency service to reach a quick and accurate diagnosis.(AU)

El término tiroiditis comprende un grupo de enfermedades de la glándula tiroides caracterizado por la presencia de inflamación, abarcando entidades autoinmunes y no-autoinmunes. Pueden manifestarse como enfermedades agudas con dolor tiroideo severo (tiroiditis subaguda y tiroiditis infecciosas), y condiciones en las cuales la inflamación no es clínicamente evidente, cursando sin dolor y presentando disfunción tiroidea y/o bocio (tiroiditis inducida por fármacos y tiroiditis de Riedel). El objetivo de esta revisión es aportar un enfoque actualizado sobre las tiroiditis no-autoinmunes cubriendo sus aspectos clínicos, diagnósticos y terapéuticos.


The term thyroiditis comprises a group of thyroid diseases characterized by the presence of inflammation, including autoimmune and non-autoimmune entities. It may manifest as an acute illness with severe thyroid pain (subacute thyroiditis and infectious thyroiditis), and conditions in which the inflammation is not clinically evident evolving without pain and presenting primarily thyroid dysfunction and/or goiter (drug-induced thyroiditis and Riedel thyroiditis). The aim of this review is to provide an updated approach on non-autoimmune thyroiditis and its clinical, diagnostic and therapeutic aspects.

Objetivo: Revisão da literatura sobre a tireoidite de Hashimoto no universo das doenças autoimunes em crianças e adolescentes. Fontes: MEDLINE, utilizando os termos tireoidite, doença de Hashimoto, genética da autoimunidade tireoidiana. Resumo: Doenças tireoidianas autoimunes são doenças endócrinas frequentes da criança e do adolescente. Genes como antígeno humano leucocitário (HLA), antígeno-4 associado ao linfócito T citotóxico (CTLA-4), a proteína tirosina-fosfatase 22 (PTPN22) e os genes específicos da glândula tireoide, como o receptor do TSH (TSHR) e tireoglobulina (Tg) afetam a resposta imunológica da tireoide. Contexto: Pacientes com tireoidite autoimune crônica com anticorpos de peroxidase tireoideana (Anti-TPO) podem progredir para hipotireoidismo subclínico e clínico. Estudos anteriores mostraram uma redução no anti-TPO em pacientes com hipotireoidismo que receberam tratamento de reposição de LT4 e suplementação de selênio. Objetivos: Avaliar se a selenometionina (SeMet) reduz o anti-TPO em pacientes anti-TPO positivos eutireoideos tratados com LT4 e melhora a ecogenecidade da tireoide em comparação com grupo de pacientes tratados com placebo. Desenho: Estudo duplo-cego aleatório controlado com placebo. Pacientes e métodos: Um total de 55 pacientes consecutivos com TCA (50 mulheres, 5 homens, idade mediana de 48 anos variando de 20 a 58), 4 tabagistas, 4 anos desde o diagnóstico da doença (mediana), com TSH normal ou levemente elevado, T4L dentro dos níveis normais, anti-TPO maior que 100 U/ml foram aleatoriamente selecionados para receber uma dose diária de 200 ?g de selenometionina (grupo SeMet; n = 28) ou placebo (grupo P; n = 27) por 3 meses. Anticorpos da tireoperoxidase e tireoglobulina (TPO-Ab; TG-Ab), TSH, FT4, T3, T4, proteína C-reativa (PCR), citocinas (IL6, IL10, TNF), glicose sanguínea em jejum, calcitonina, selênio plasmático, concentração de iodo na urina, atividade da peroxidase de glutationa 1 em eritrócitos (GPx1) foram dosados no basal, após 3 e 6 meses de acompanhamento. Polimorfismo do GPx1-Pro198Leu foi avaliado no basal. Foi efetuado ultrassom de tireoide (US) em cada ponto de acompanhamento. Ecogenecidade do tecido foi caracterizada por análise histográfica computadorizada em escala de cinza e expressa como índice de ecogenecidade (EI). A relação entre concentração de Selênio e atividade GPx1 nos eritrócitos foi analisada para cada genótipo. O IE e associações de variáveis clínicas também foram avaliadas. Context: Autoimmune chronic thyroiditis (ACT) euthyroid subjects with positive thyroid peroxidase antibodies (TPO-Ab) may progress to subclinical and overt autoimmune hypothyroidism. Previous studies have shown a decrease in TPO-Ab in hypothyroid patients receiving LT4 replacement therapy plus selenium supplementation. Objectives: To evaluate in euthyroid TPOAb-positive LT4-treated subjects whether selenometionine (SeMet) decreases TPO-Ab and improves thyroid ecogenicity in comparison with placebo. Design: Randomized, placebo-controlled, double-blind study. Patients and methods: A total of 55 consecutive patients with ACT (50 F, 5 M, age median: 48, range: 20-58 years), 4 smokers, duration of disease (median) 4 years, normal or slightly elevated TSH and FT4 within the normal range, TPO-Ab >= 100 U/mL were randomized to receive 200 ?g selenometionine daily ((group SeMet; n = 28) or placebo (group P; n = 27) for 3 months. Thyroperoxidase and thyroglobulin autoantibodies (TPO-Ab; TG-Ab), TSH, FT4, T3, T4, C-reactive protein (CRP), cytokines (IL6, IL10, TNF), fasting blood glucose, calcitonin, plasma selenium, urine iodine concentrations, activity of glutathione peroxidase 1 in erythrocytes (GPx1) were estimated at baseline, after 3 and 6 months of follow-up; GPx1-Pro198Leu polymorphism was assessed at baseline. Thyroid ultrasound (US) was performed at each follow-up point. Tissue ecogenicity was characterized by computerized grey-scale histographic analysis and expressed as ecogenicity index (EI). The relationship between Se concentration and GPx1 erytrocyte activity was analysed for each genotype group. EI and clinical variables associations were also evaluated. Results: There were no differences in baseline characteristics between the SeMet group and the P group. Se presenta el primer paciente con una tiroiditis de Riedel que se le aplica un tratamiento alternativo con triamcinolona creado por el profesor Juan Gualberto Vivó Núñez. Es el caso de una mujer de 48 años de edad, que acude al cuerpo de guardia por aumento de volumen de la región anterior del cuello y referir ligera disfagia. Tiene antecedente inicialmente de hipertiroidismo, a los 9 años comenzó con hipotiroidismo y hace aproximadamente un año se le diagnosticó una tiroiditis de Hashimoto, con seguimiento por endocrinología. La ecografía del tiroides muestra un aumento marcado de la glándula tiroidea y las imágenes de la biopsia por aspiración con aguja fina (BAAF) son compatibles con una tiroiditis de Hashimoto. Con estos resultados se decide intervenirla quirúrgicamente eliminando así los signos compresivos. La biopsia postoperatoria describe una tiroiditis de Riedel. Por el gran tamaño de los lóbulos tiroideos, se le infiltra triamcinolona 1/2 cc en cada lóbulo, una vez al mes durante cuatro meses; logrando reducir aproximadamente el 50 por ciento de los lóbulos tiroideos. Por lo que se puede apreciar los resultados obtenidos con el uso de la triamcinolona(AU)


The first case of Riedel's thyroiditis treated with triamcinolone, a method created by Professor Juan Gualberto Vivo Nunez, was presented. The patient was a 48 years-old woman who went to the emergency service because she presented increased volume of the anterior region of the neck and slight dysphagia. She had a history of hyperthyroidism, at 9 years of age began hypothyroidism and one year before going to emergency room, she was diagnosed with Hashimoto thyroiditis and followed-up in the endocrinology service. Thyroid echography showed marked increase of thyroid gland and the images from fine needle aspiration biopsy were compatible with the existence of Hashimoto thyroiditis. The results backed up the decision of operating her to eliminate the compressive signs. Postoperative biopsy described Riedel's thyroiditis. Since the thyroid lobes were big, it was decided to apply triamcinolone at a dose of ½ cc to each lobe once a month for 4 months. This treatment managed to reduce roughly 50 percent of thyroid lobes. This showed the good results obtained with the use of triamcinolone(AU)

OBJETIVO: Caracterizar uma população de pacientes com diabetes melito tipo 1 (DMT1) relativamente à presença de outras entidades autoimunes que permitam estabelecer o diagnóstico de síndrome poliglandular autoimune (SPGA). SUJEITOS E MÉTODOS: Incluímos 151 pacientes com DMT1. Analisamos os seguintes parâmetros clínicos: gênero, idade atual, duração da doença, antecedentes pessoais de patologia autoimune e antecedentes familiares de diabetes melito. Submetemos cada doente a um estudo laboratorial com o objetivo de detectar a presença de marcadores imunológicos para a tireoidite, insuficiência adrenocortical, gastrite e doença celíaca, e eventual disfunção associada. RESULTADOS: Coorte com 51,7% homens, idade média atual de 33,4 ± 13 anos e duração da doença de 14,4 ± 9,6 anos. Antecedentes pessoais de autoimunidade presentes em 2% da amostra e história familiar de diabetes melito em 31,1%. A frequência de marcadores imunológicos foi de 24% para a tireoidite, 9,4% para a insuficiência adrenocortical, 17,2% para a gastrite e 2% para a doença celíaca. Foi diagnosticada SPGA em 25,2% dos pacientes. O risco de SPGA e tireoidite autoimune foi superior em mulheres. A duração da doença correlacionou-se diretamente com a presença de autoanticorpos gástricos e inversamente com a positividade dos anticorpos anti-ilhota, antiglutamato descarboxilase e antitirosina fosfatase. Constatou-se a existência de uma associação entre os marcadores imunológicos da tireoidite e gastrite, bem como entre a doença celíaca e insuficiência adrenocortical. CONCLUSÃO: Atendendo à frequência e ao prognóstico inerente à SPGA, a necessidade de realizar rastreio em pacientes com DMT1 é enfatizada. O diagnóstico atempado de outras doenças autoimunes permitirá individualizar o tratamento e seguimento do doente.


OBJECTIVE: To characterize a cohort of patients with type 1 diabetes mellitus (T1DM) on the presence of other autoimmune disorders that could establish the diagnosis of autoimmune polyglandular syndrome (APS). SUBJECTS AND METHODS: We included 151 patients with T1DM. The following clinical parameters were analyzed: gender, current age, disease duration, previous history of autoimmune disorders, and familial history for diabetes mellitus. Each patient was analyzed to detect autoimmune markers of thyroiditis, adrenocortical insufficiency, gastritis, and celiac disease, as well as possible associated dysfunctions. RESULTS: A cohort with 51.7% males, average current age of 33.4 ± 13 years and disease duration of 14.4 ± 9.6 years was analyzed. Previous history of autoimmunity was found in 2%, and familial history for diabetes mellitus in 31.1% of the cohort. Frequency of autoimmune markers was 24% for thyroiditis, 9.4% for adrenocortical insufficiency, 17.2% for gastritis, and 2% for celiac disease. APS was diagnosed on 25.2% of the patients. APS and autoimmune thyroiditis risk was higher in females. Disease duration correlated directly with gastric autoantibodies, and inversely with positive islet cell, glutamic acid decarboxylase, and tyrosine phosphatase antibodies. We noticed a correlation between autoimmune markers for thyroiditis and gastritis, as well as between celiac disease and adrenocortical insufficiency. CONCLUSION: Considering APS prevalence and prognosis, the need for APS screening in patients with T1DM is emphasized. Early diagnosis of other autoimmune disorders will enable us to adjust each patient treatment and follow-up.

Thyroidal 99mTc uptake in the acute thyrotoxic phase of subacute thyroiditis (SAT) is always inhibited. However, a patient with SAT had signs in the right-side thyroid gland with transient thyrotoxicosis and slightly high 99mTc uptake levels in the right lobe, low 99mTc uptake in the left lobe, and normal overall uptake. Histological examination showed cellular destruction and granulomatous inflammatory changes in the right lobe, with marked interstitial fibrosis in the left lobe. The patient was thyrotrophin-receptor antibody (TRAb) positive. After a short course of prednisolone, SAT-like symptoms and signs improved. TRAb-positivity resolved spontaneously after 22 months, and TSH levels were slightly low for 22 months. Levels then kept normal in the following four years. In conclusion, high 99mTc uptake by the right lobe was due to the combined effects of TRAb and left thyroid gland fibrosis.


A absorção tiroidiana de 99mTc no estado tirotóxico agudo da tireoidite subaguda (SAT) é sempre inibida. Entretanto, um paciente com SAT apresentou sinais na tiroide direita, com tirotoxicose transitória e níveis levemente elevados de 99mTc no lobo direito, baixa absorção de 99mTc no lobo esquerdo e absorção geral normal. O exame histológico mostrou destruição celular e alterações inflamatórias granulomatosas no lobo direito, com fibrose intersticial marcada no lobo esquerdo. O paciente foi positivo para anticorpos antirreceptores da tireotropina (TRAb). Após um curto tratamento com prednisolona, os sintomas e sinais da SAT melhoraram. A positividade para TRAb foi resolvida espontaneamente em 22 meses. Os níveis de TSH permaneceram levemente baixos por 22 meses e, depois, se mantiveram normais nos quatro anos seguintes. Concluiu-se que a alta absorção de 99mTc pelo lobo direito foi devida à combinação entre TRAb e fibrose da tiroide esquerda.

INTRODUÇÃO: O hipertireoidismo é caracterizado pelo aumento da síntese e liberação dos hormônios tireoidianos pela glândula tireoide. A tireotoxicose refere-se à síndrome clínica decorrente do excesso de hormônios tireoidianos circulantes, secundário ao hipertireoidismo ou não. Este artigo descreve diretrizes baseadas em evidências clínicas para o manejo da tireotoxicose. OBJETIVO: O presente consenso, elaborado por especialistas brasileiros e patrocinado pelo Departamento de Tireoide da Sociedade Brasileira de Endocrinologia e Metabologia, visa abordar o manejo, diagnóstico e tratamento dos pacientes com tireotoxicose, de acordo com as evidências mais recentes da literatura e adequadas para a realidade clínica do país. MATERIAIS E MÉTODOS: Após estruturação das questões clínicas, foi realizada busca das evidências disponíveis na literatura, inicialmente na base de dados do MedLine-PubMed e posteriormente nas bases Embase e SciELO - Lilacs. A força das evidências, avaliada pelo sistema de classificação de Oxford, foi estabelecida a partir do desenho de estudo utilizado, considerando-se a melhor evidência disponível para cada questão. RESULTADOS: Foram definidas 13 questões sobre a abordagem clínica inicial visando ao diagnóstico e ao tratamento que resultaram em 53 recomendações, incluindo investigação etiológica, tratamento com drogas antitireoidianas, iodo radioativo e cirurgia. Foram abordados ainda o hipertireoidismo em crianças, adolescentes ou pacientes grávidas e o manejo do hipertireoidismo em pacientes com oftalmopatia de Graves e com outras causas diversas de tireotoxicose. CONCLUSÕES: O diagnóstico clínico do hipertireoidismo, geralmente, não oferece dificuldade e a confirmação diagnóstica deverá ser feita com as dosagens das concentrações séricas de TSH e hormônios tireoidianos. O tratamento pode ser realizado com drogas antitireoidianas, administração de radioiodoterapia ou cirurgia de acordo com a etiologia da tireotoxicose, as características clínicas, disponibilidade local de métodos e preferências do médico-assistente e paciente.


INTRODUCTION: Hyperthyroidism is characterized by increased synthesis and release of thyroid hormones by the thyroid gland. Thyrotoxicosis refers to the clinical syndrome resulting from excessive circulating thyroid hormones, secondary to hyperthyroidism or due to other causes. This article describes evidence-based guidelines for the clinical management of thyrotoxicosis. OBJECTIVE: This consensus, developed by Brazilian experts and sponsored by the Department of Thyroid Brazilian Society of Endocrinology and Metabolism, aims to address the management, diagnosis and treatment of patients with thyrotoxicosis, according to the most recent evidence from the literature and appropriate for the clinical reality of Brazil. MATERIALS AND METHODS: After structuring clinical questions, search for evidence was made available in the literature, initially in the database MedLine, PubMed and Embase databases and subsequently in SciELO - Lilacs. The strength of evidence was evaluated by Oxford classification system was established from the study design used, considering the best available evidence for each question. RESULTS: We have defined 13 questions about the initial clinical approach for the diagnosis and treatment that resulted in 53 recommendations, including the etiology, treatment with antithyroid drugs, radioactive iodine and surgery. We also addressed hyperthyroidism in children, teenagers or pregnant patients, and management of hyperthyroidism in patients with Graves' ophthalmopathy and various other causes of thyrotoxicosis. CONCLUSIONS: The clinical diagnosis of hyperthyroidism usually offers no difficulty and should be made with measurements of serum TSH and thyroid hormones. The treatment can be performed with antithyroid drugs, surgery or administration of radioactive iodine according to the etiology of thyrotoxicosis, local availability of methods and preferences of the attending physician and patient.

A tireotoxicose é um estado hipermetabólico decorrente dos efeitos de níveis teciduais elevados de hormônios tireoideanos (HTs). O hipertireoidismo é a causa mais comum, e decorre do excesso de hormônios produzidos pela tireoide. Outras causas de tireotoxicose ocorrem por produção extratireoideana ou administração exógena de hormônios tireoideanos. A principal causa de tireotoxicose, o bócio difuso tóxico (BDT) de causa autoimune, responsável por até 80% dos casos, é aqui abordado, assim como as demais causas. O tratamento é realizado com drogas antitireoideanas, iodo radioativo ou cirurgia e exige acompanhamento de longo prazo, tanto pela possibilidade de recidivas como do desenvolvimento de hipotireoidismo.


Thyrotoxicosis is a hypermetabolic state due to the effects of high tissue levels of thyroid hormones (TH). Hyperthyroidism is the most common cause and results from excess hormones produced by the thyroid. Other causes of thyrotoxicosis occur extra thyroid production or exogenous administration of thyroid hormones. The main cause of thyrotoxicosis, the diffuse toxic goiter (DTG) of autoimmune cause, accounting for up to 80% of cases, is discussed here, as well as other causes. The treatment is performed with antithyroid drugs, radioactive iodine or surgery and requires long-term follow-up, due to both the possibility of recurrence and the development of hypothyroidism.

Se describe el caso de una mujer de 57 años de edad, con diagnóstico de síndrome de Wolf Parkinson White, en tratamiento con amiodarona 200 mg/día; que acudió por presentar palpitaciones, fatiga y aumento de sueño. Al examen físico no se encontró bocio, pero los reflejos osteotendinosos estaban prolongados. Los exámenes auxiliares mostraron TSH elevado, T4 libre disminuido y anticuerpos antiperoxidasa tiroidea negativos. Se suspendió la amiodarona y se inició levotiroxina. Después de dos meses; los niveles de TSH y T4 libre eran normales y la paciente no presentaba molestias. La amiodarona está asociada a diversos efectos adversos que pueden limitar su uso. Entre estos efectos adversos, se describe el hipotiroidismo inducido, que se caracteriza por TSH elevado, T4 libre disminuido y síntomas inespecíficos como fatiga, intolerancia al frío y piel seca. El tratamiento de elección es la levotiroxina.


We report a case of a Wolf Parkinson White syndrome in a 57-yearl-old woman receiving amiodarone (200 mg/ day) who presented with tachycardia, fatigue and somnolence. The physical examination did not reveal goiter, but the osteotendinous reflexes were brisk. Laboratory examinations revealed high TSH level, low T4 free level and negative antiperoxidase antibodies. Amiodarone was stopped and levotiroxine was started. Two months after, TSH and T4 free levels were normal and the patient was asymptomatic. Amiodarone is associated with several side effects that can preclude its indication; one of these side effects is hypothiroidism, characterized by high TSH levels, low T4 free levels and unspecific symptoms such as fatigue, cool intolerance and dry skin. The treatment of choice is levotiroxine.

Differentiated thyroid cancer (DTC) in the common endocrine malignancy. Its frequency increases with age and its have a female predominance. There is evidence that papillary thyroid cancer is increasing in recent years. It has been proposed that this increase is independent from the greater availability of diagnostic techniques, because is observed an increase in both small tumors like those of grater size and more advanced stage. The causes of this increase are unclear and several hypotheses have been raised, including within these possible factors the presence of chronic thyroiditis (CT). Among the reasons to make this association is the relationship described between inflammatory processes and the genesis of cancer as well as some descriptive studies that reports a high incidence of CT in patients with DTC. But the results of these studies are controversial and there are other reports in rejecting this association.

OBJECTIVE: To characterize thyroid disturbances induced by interferon-alpha and ribavirin therapy in patients with chronic hepatitis C. INTRODUCTION: Interferon-alpha is used to treat chronic hepatitis C infections. This compound commonly induces both autoimmune and non-autoimmune thyroiditis. METHODS: We prospectively selected 26 patients with chronic hepatitis C infections. Clinical examinations, hormonal evaluations, and color-flow Doppler ultrasonography of the thyroid were performed before and during antiviral therapy. RESULTS: Of the patients in our study, 54 percent had no thyroid disorders associated with the interferon-alpha therapy but showed reduced levels of total T3 along with a decrease in serum alanine aminotransferase. Total T4 levels were also reduced at 3 and 12 months, but free T4 and thyroid stimulating hormone (TSH) levels remained stable. A total of 19 percent of the subjects had autoimmune interferon-induced thyroiditis, which is characterized by an emerge of antithyroid antibodies or overt hypothyroidism. Additionally, 16 percent had non-autoimmune thyroiditis, which presents as destructive thyroiditis or subclinical hypothyroidism, and 11 percent remained in a state of euthyroidism despite the prior existence of antithyroidal antibodies. Thyrotoxicosis with destructive thyroiditis was diagnosed within three months of therapy, and ultrasonography of these patients revealed thyroid shrinkage and discordant change in the vascular patterns. DISCUSSION: Decreases in the total T3 and total T4 levels may be related to improvements in the hepatocellular lesions or inflammatory changes similar to those associated with nonthyroidal illnesses. The immune mechanisms and direct effects of interferon-alpha can be associated with thyroiditis. CONCLUSION: Interferon-alpha and ribavirin induce autoimmune and non-autoimmune thyroiditis and hormonal changes (such as decreased total T3 and total T4 levels), which occur despite stable free T4 and TSH levels. A thyroid hormonal evaluation, including the analysis of the free T4, TSH, and antithyroid antibody levels, should be mandatory before therapy, and an early re-evaluation within three months of treatment is necessary as an appropriate follow-up.
