Resumen Se realizó un estudio unicéntrico retrospectivo para evaluar la eficacia y seguridad de trombolisis dirigida por catéter (TDC) en pacientes con tromboembolismo pulmonar agudo (TEP) de 2014 a 2020. Se analizó la efectividad (mejoría de presión pulmonar), y seguridad (sangrado intracraneal y grave definido por compromiso hemodinámico). Se incluyeron 43 pacientes, de 67(56-79) años, 5 (12%) con shock, 41 (95%) con dilatación del ventrículo derecho y TEP bilateral. La decis ión de TDC fue: tratamiento inicial (53%), escalada de anticoagulación (42%) y rescate de trombolisis sistémica (5%). Se utilizó TDC facilitada por ultrasonido en 40 casos (93%), utilizándose 30 (25-35) mg de activador tisular del plasminógeno recombinante (rtPA) durante 20 h. Se administró un bolo de rtPA en 38 (89%) casos, que fue 5 mg (95%) o 1 mg (5%). Se utilizó un solo catéter por paciente. En 4 (9%) se decidió recolocación (mismo pulmón) para continuar infusión en otro sector. Se observó una disminución significativa de la presión media pulmonar (pre 35 [29-41] mmHg vs. post 24 [20-34] mmHg, p<0.001). No se observó ningún caso de hemorragia intracraneal, y un caso (2%) de sangrado grave. Se observó hematoma del sitio de punción en 5 (12%) (incluyendo el sangrado grave), y requirió transfusiones en 3 (7%). La mortalidad intrahospitalaria fue 12%, siendo un solo c aso (2%) atribuido al TEP. El tratamiento con TDC fue efectivo asociándose a una reducción significativa de la presión pulmonar, sin observarse ningún sangrado intracraneal y con un sangrado grave. Nuestros resultados se asemejan a lo publicado en otros estudios.


Abstract We performed a single center retrospective study in patients with pulmonary embolism (PE) undergoing catheter directed thrombolysis (CDT) from 2014 to 2020. Efficacy was defined by mean pulmonary pressure drop, and safety was assessed by intracranial and severe bleeding (defined by GUSTO). Forty-three patients were included, aged 64 (56-79) years old, 5 (12%) with shock, most with right ventricle dilation (95%) and bilateral PE (95%) or unilateral (5%) in patients with only one functional lung. CDT was used as first treatment (53%), upscale after anticoagulation alone (42%), or after failed systemic thrombolytics (5%). Median recombinant tissue plasminogen activator (rtPA) dose was 30 (25-35) mg over 20 (20-20) hours, and rtPA bolus was used after catheter placement in 38 cases (89%), consisting of 5 mg (95%) or 1 mg (5%). Only one lung was treated for technical reasons, and 4 (9%) were repositioned in the same lung for continuation of infusion. A significant reduction in mean pulmonary pressure was observed (pre 35 [29-41] mmHg vs. post 24 [20-34] mmHg, p<0.001) with no intracranial bleeding. One patient (2%) experienced severe bleeding, while 5 (12%) presented access site bleeding, and 3 (7%) required blood transfusions. In-hospital mortality was 12% but only one case (2%) due to PE. Our results are similar to previously reported studies.

Abstract Background: Low-dose alteplase (LrtPA) has been shown not to be inferior to the standard-dose (SrtPA) with respect to death/disability. Objective: We aim to evaluate the percentage of patients treated with LrtPA at our center after the ENCHANTED trial and the factors associated with the use of this dosage. Methods: Prospective study in consecutive patients with an acute stroke admitted between June 2016 and November 2018. Results: 160 patients were treated with intravenous thrombolysis, 50% female; mean age 65.4±18.5 years. Of these, 48 patients (30%) received LrtPA. In univariate analysis, LrtPA was associated with patient's age (p=0.000), previous modified Rankin scale scores (mRS) (p<0.000), hypertension (p=0.076), diabetes mellitus (p=0.021), hypercholesterolemia (p=0.19), smoking (p=0.06), atrial fibrillation (p=0.10), history of coronary artery disease (p=0.06), previous treatment with antiplatelet agents (p<0.000), admission International Normalized Ratio-INR (p=0.18), platelet count (p=0.045), leukoaraiosis on neuroimaging (p<0.003), contraindications for thrombolytic treatment (p=0.000) and endovascular treatment (p=0.027). Previous relevant bleedings were determinants for treatment with LrtPA. Final diagnosis on discharge of stroke mimic was significant (p=0.02) for treatment with SrtPA. In multivariate analysis, mRS (OR: 2.21; 95%CI 1.37‒14.19), previous antiplatelet therapy (OR: 11.41; 95%CI 3.98‒32.70), contraindications for thrombolysis (OR: 56.10; 95%CI 8.81‒357.80), leukoaraiosis (OR: 4.41; 95%CI 1.37‒14.10) and diagnosis of SM (OR: 0.22; 95%CI 0.10‒0.40) remained independently associated. Conclusions: Following the ENCHANTED trial, LrtPA was restricted to 30% of our patients. The criteria that clinicians apply are based mostly on clinical variables that may increase the risk of brain or systemic hemorrhage or exclude the patient from treatment with lytic drugs.


RESUMEN Introducción: Dosis reducidas de trombolitico (LrtPA) podrían no ser inferiores en muerte/discapacidad. Objetivo: Evaluar el porcentaje de pacientes tratados con LrtPA en nuestro centro después del ensayo ENCHANTED, y los factores asociados con el uso de esta dosis. Métodos: Estudio prospectivo de pacientes consecutivos con infarto cerebral ingresados ​entre junio de 2016 y noviembre de 2018. Resultados: 160 pacientes fueron tratados con trombólisis intravenosa, 50% mujeres; edad media 65,4±18,5 años. 48 casos (30%) recibieron LrtPA. En el análisis univariado, LrtPA se asoció con la edad del paciente (p=0,000), escala de Rankin modificadas (mRS) (p<0,000), hipertensión arterial (p=0,076), diabetes mellitus (p=0,021), hipercolesterolemia (p=0,19), tabaquismo (p=0,06), fibrilación auricular (p=0,10), antecedentes de enfermedad coronaria (p=0,06), tratamiento previo con antiplaquetarios (p<0,000), International Normalized Ratio-INR (p=0,18), recuento de plaquetario (p=0,045), leucoaraiosis en neuroimagen (p<0,003), contraindicaciones para el tratamiento trombolítico (p=0,000) y tratamiento endovascular (p=0,027). Las hemorragias previas relevantes fueron determinantes para el tratamiento con LrtPA. El diagnóstico al alta de imitador de accidente cerebrovascular fue significativo (p=0,02) para el tratamiento con dosis estándar. El análisis multivariado demostró que mRS (OR: 2,21; IC95% 1,37‒14,19), tratamiento antiplaquetario previo (OR: 11,41; IC95% 3,98‒32,7), contraindicaciones para trombólisis (OR: 56,1; IC95% 8,81‒357,8), leucoaraiosis (OR: 4,41; IC95% 1,37‒14,1) y un diagnóstico de imitador de accidente cerebrovascular (OR: 0,22; IC95% 0,1‒0,40) fueron asociados con la dosis recibida. Conclusiones: LrtPA está restringido al 30% de nuestros pacientes. Los criterios para tomar esta decisión se basan en variables que podrían aumentar el riesgo de hemorragia cerebral/sistémica o excluir al paciente del tratamiento con fármacos líticos.

The ability of the general population to recognize the initial symptoms of acute stroke is important for the prevention of neurological damage. The objective of this study was to investigate the knowledge held by university students in health fields and what they perceived was adequate conduct after recognizing that someone is having an acute stroke. The students took a semi-structured, two-part questionnaire, with the first part referring to social and demographic data and the second containing a description of stroke's typical clinical presentation and open-ended questions about it. Of the participants, 86.24% were able to recognize stroke. When comparing the initial and final years, the students in their final years had approximately a 10% increase in stroke recognition. Regarding the perceived correct conduct, it was observed that 84.60% of the students in the initial years would have the right conduct compared to 89.32% of the students in the final years. Even though a high percentage of students demonstrated some knowledge about stroke it is important that we aspire to further educate them about the subject, enabling them to eventually contribute to the general population's education and to promote health.


O reconhecimento dos sintomas iniciais pela população leiga é importante na prevenção de sequelas decorrentes do AVC. O objetivo desse trabalho foi verificar o conhecimento e conduta dos universitários em casos de AVC. Foi aplicado um questionário semi-estruturado aos estudantes, sendo este referente a dados sócios demográficos e a um caso clinico básico, onde o paciente apresentava sintomas clássicos de um AVC com perguntas discursivas onde se averiguava sobre o reconhecimento da doença. Dos participantes, 86,24% souberam reconhecer um AVC e comparando-se por séries mostrou que nas finais há um incremento em torno de 10%. Observou-se que 84,60% dos alunos das series inicias, e 89,32% das finais teriam uma conduta correta. Apesar da maioria apresentar conhecimento do AVC, bem como a conduta a ser utilizada, a importância de que este número seja maior, uma vez que, poderão contribuir para a educação da população e a promoção da saúde.

Objective: To compare salivary levels of PAI-2 in patients with moderate generalized chronic periodontitis before and after treatment and healthy subjects. Material and Methods: The present case-control study evaluated patients with generalized moderate chronic periodontitis (the case group) and subjects with healthy gingiva (the control group). The healthy subjects were evaluated once and the cases were evaluated twice (before and after treatment) by collecting their salivary samples. ELISA technique was used to determine PAI-2 salivary levels. Data were analyzed with the use of SPSS 17. The level of significance was set at 5%. Results: The mean salivary levels of PAI-2 in the case and control groups were 45.63 ± 8.63 and 22.01 ± 9.77 ng, respectively (p<0.0001). In addition, PAI-2 salivary levels in the case group subjects after treatment was 27.43 ± 5.79 ng, which was lower than that before treatment (45.63 ± 8.63 ng) (p<0.0001). The mean salivary level of PAI-2 in subjects with periodontitis after treatment (27.43 ± 5.79) was not significantly different from that in healthy subjects (22.01 ± 9.77) (p>0.05). Conclusion: The salivary levels of PAI-2 in patient with moderate generalized chronic periodontitis were higher than these in healthy subjects. However, the salivary levels of PAI-2 decreased in the case group subjects after treatment, with no significant difference from the healthy subjects.

Seminal plasma contains serine proteases and serine protease inhibitor, which are involved in mammalian fertilization, and the inhibitors can be applied to prevent cold-induced sperm capacitation. The effects of different concentrations of two serine protease inhibitors were analyzed, Plasminogen activator inhibitor 1 - PAI-1 (70ƞg, 140ƞg and 210 ƞg) and Antipain (10µg, 50µg and 100µg) as supplementation to bovine semen cryopreservation extender. The effects of the inhibitors on the sperm parameters (sperm kinetics - CASA, acrosome integrity, plasma membrane integrity, mitochondrial membrane potential, sperm defects and acrosome reaction rate) were evaluated in the post-thaw semen. Cryopreservation of sperm with Antipain decreased post-thaw kinetic parameters of MP, VSL, LIN, SRT and the percentage of hyper-activated sperm while PAI-1 (210 ƞg) decreased VSL and LIN. Antipain and PAI-1 had no effect on the integrity parameters of the plasma membrane, mitochondrial membrane potential and sperm defects. Sperm cryopreserved in the presence of Antipain and PAI-1 (70 and 140 ƞg) preserved acrosome integrity, as they were able to complete the in vitro acrosome reaction. In conclusion, the serine protease inhibitors, Antipain and PAI-1 (70 and 140ƞg) are able to preserve the acrosome integrity of cryopreserved bovine sperm.(AU)


A criopreservação é parcialmente prejudicial à fertilidade do sêmen de bovinos e induz mudanças semelhantes à capacitação em espermatozoides. O plasma seminal contém serina-proteases e inibidores de serina-proteases que estão envolvidos na fertilização de mamíferos, e os inibidores podem ser aplicados para evitar uma capacitação espermática induzida pelo frio. Analisaram-se os efeitos de diferentes concentrações de dois inibidores de serina-proteases, inibidor do ativador do plasminogênio 1 - PAI-1 (70ƞg, 140ƞg e 210ƞg) e antipaína (10µg, 50µg e 100µg) na suplementação ao diluidor de criopreservação de sêmen bovino. Trinta e seis ejaculados de quatro bovinos Curraleiro Pé-Duro foram usados para criopreservação. Os efeitos dos inibidores sobre os parâmetros dos espermatozoides (cinética espermática - CASA, integridade acrossomal, integridade da membrana plasmática, potencial de membrana mitocondrial, defeitos espermáticos e taxa de reação acrossomal) foram avaliados no sêmen pós-descongelamento. A criopreservação de espermatozoides com antipaína diminuiu os parâmetros cinéticos pós-descongelamento de MP, VSL, LIN, SRT e a porcentagem de espermatozoides hiperativados, PAI-1 (210ƞg) diminuiu VSL e LIN. Antipaína e PAI-1 não tiveram efeitos nos parâmetros de integridade da membrana plasmática, no potencial de membrana mitocondrial e nos defeitos espermáticos. Espermatozoides criopreservados na presença de antipaína e PAI-1 (70 e 140ƞg) preservaram a integridade acrossomal, assim como foram capazes de completar a reação acrossômica in vitro. Em conclusão, os inibidores de serina-proteases, antipaína e PAI-1 (70 e 140ƞg) são capazes de preservar a integridade acrossomal de espermatozoides criopreservados de bovinos.(AU)

Background: Intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) reduces disability in patients with ischemic stroke. However, its implementation in Chilean public general hospitals has been slow and faces some difficulties. Aim: To analyze the results of an intravenous thrombolysis protocol implementation in a public general hospital. Material and Methods: During a lapse of 28 months a standardized protocol for intravenous thrombolysis implemented in the emergency room of a public hospital, was prospectively evaluated. Fifty four patients with ischemic stroke were treated and assessed three months later as outpatients. Results: At three months of follow-up, 66.4% of patients subjected to thrombolysis had a favorable evolution, defined as having 0 to 1 points in the modified Rankin scale. Intracerebral hemorrhage rate was 11.1%, including 5.5% of symptomatic intracerebral hemorrhage. Four percent of patients had systemic bleeding complications after thrombolysis. The mortality rate was 14.8%. Conclusions: The success rates, mortality, and complications rate were comparable to the results obtained in international studies, despite of the absence of a stroke unit to manage stroke and its complications.

Con el objetivo de describir los resultados del uso de fibrinolíticos (alteplase endovenoso o rtPA) en pacientes con ECV isquemico, se realizó un estudio descriptivo, observacional y prospectivo en 23 pacientes que fueron diagnosticados con ECV isquemico, además se describieron los principales factores de riesgo y los porcentajes de recuperación usando las escalas de Rankin y Barthel modificados. Se encontró que el factor de riesgo más frecuente fue la hipertensión arterial. El tiempo total desde que se presentaron los síntomas hasta la administración del tratamiento tuvo una media de 168,78 minutos, la escala de NISSH aplicada a los pacientes tuvo un promedio de 2,1 puntos a las 24 horas, 1,26 puntos a los 7 días y 0,5 puntos a los 3 meses, 20 de los pacientes trombolisados obtuvieron un puntaje de 0-2 en la escala de Rankin de y de >95 en la escala de Barthel. Finalmente concluimos que el uso de fibrinoliticos en pacientes con ECV isquemico produce mejoría clínica y menor discapacidad evaluados por las escalas de Rankin y Barthel modificados. (AU)


In order to describe the results of the use of fibrinolytic (alteplase intravenous or rtPA) in patients with ischemic CVD, a descriptive, observational and prospective study was performed in 23 patients who were diagnosed with ischemic CVD, also were described the main risk factors and recovery rates using the modified Rankin scale Barthel. It was found that the most common risk factor was hypertension. The total time since the symptoms appeared to treatment administration had an average of 168.78 minutes, NISSH scale applied to the patients had an average of 2.1 points at 24 hours, to 1.26 points 7 days and 0.5 points at 3 months, 20 patients trombolisados average score was 0-2 at the Rankin scale and> 95 on the Barthel scale. Finally we conclude that the use of fibrinolytic in patients with ischemic CVD produces clinical improvement and less disability assessed by the modified Rankin scale Barthel. (AU)

La taquicardia por reentrada nodal es la causa más común de taquicardia supraventricular paroxística; en aquellos pacientes en quienes el manejo farmacológico no es efectivo o deseado la ablación por radiofrecuencia es un excelente método terapéutico dada su alta tasa de curación. Aunque en términos generales dichos procedimientos son rápidos y seguros, se han descrito varias complicaciones entre las que sobresale el accidente cerebrovascular isquémico. Se presenta el caso de una paciente de 41 años con episodios de taquicardia por reentrada nodal a repetición, que fue llevada a ablación por radiofrecuencia. En el post-operatorio inmediato se evidenció déficit neurológico focal con isquemia en el territorio de la arteria cerebral media derecha, tras lo cual se realizó angiografía con intento de angioplastia y abxicimab y posteriormente infusión local de activador de plasminógeno tisular (rtPA) con adecuado resultado clínico y angiográfico.


Atrioventricular nodal reentry tachycardia is the most common type of paroxismal supraventricular tachycardia. In those patients in whom drug therapy is not effective or not desired, radio frequency ablation is an excellent therapeutic method. Although overall these procedures are fast and safe, several complications among which ischemic stroke stands out, have been reported. We present the case of a 41 year old female patient with repetitive episodes of tachycardia due to nodal reentry who was treated with radiofrequency ablation. Immediately after the procedure she presented focal neurologic deficit consistent with ischemic stroke in the right medial cerebral artery territory. Angiography with angioplastia and abxicimab was performed and then tissue plasminogen activator (rtPA) was locally infused, with appropriate clinical and angiographic outcome.

No Brasil, segundo estimativas do Ministério da Saúde, cerca de 25% dos casos de câncer de mama são diagnosticados como tumores localmente avançados. Com o intuito de melhorar a possibilidade de ressecção da mama e diminuir na taxa de recaída local, bem como aumentar a sobrevida global, realiza-se neste grupo de mulheres quimioterapia pré-operatória... O termo ativadores de plasminogênio restringe-se a duas enzimas, o ativador de plasminogênio tipo uroquinase ou uPA (“urokinase-type plasminogen activator”), seu receptor uPAR(“urokinase-type plasminogen activator receptor”) e o ativador de plasminogênio tipo tecidual ou tPA (“tissue-type plasminogen activator”). Entre os inibidores das serino-proteinases (SERPINs) destacam-se o PAI-1 e PAI-2 no processo de inibição ou inativação dos ativadores de plasminogênio. Neste estudo o objetivo foi determinar a expressão e a relação prognóstica dos marcadores moleculares de invasão: uPA, uPAR e PAI-1 no câncer de mama feminino localmente avançado, pelo método imunohistoquímico em material parafinado através da técnica de Tissue-Array. Foram analisadas 128 pacientes. Os marcadores de ativação do plasminogênio foram assim distribuídos: 113 amostras foram classificadas como positivas para uPA (88,3% das pacientes), 71 amostras foram consideradas como positivas para uPAR (55,5%) e 54 amostras positivas para PAI-1 (42,2%)... Apesar de não encontrarmos correlação estatisticamente significativa entre o perfil de expressão dos ativadores de plasminogênio e a evolução clínica das pacientes com câncer de mama localmente avançado, nosso estudo conseguiu determinar uma proposta de regime pré-operatório de quimioterapia com impacto na sobrevida livre de doença (paclitaxel e doxorubicina), bem como do impacto positivo do tratamento quimioterápico e hormonioterápico adjuvantes nesta população.


Locally advanced breast cancer corresponds to about 25% of all breast cancers in Brazil according to the Brazilian Ministério da Saúde. In order to improve local control and overall survival these patients are submitted to preoperative chemotherapy. Tumor invasion plays a major role in breast cancer and extracellular matrix degradation is a common characteristic of this process. Extracellular matrix is composed of many elements like collagen, glycoproteins (laminina, fibronectina and entactin), proteoglycans and glycosaminoglycans. Those proteins are degraded by specific enzymes called proteases that include plasminogen activators. These enzymes transform plasminogen into plasmin, that degrades the extracellular matrix. The name plasminogen activator is restricted to two enzymes, the urokinasetype plasminogen activator or uPA and the tissue-type activator or tPA. There are two inhibitors, the plasminogen activator inhibitor 1(PAI-1) and the plasminogen activator inhibitor 2 (PAI-2). The objective of this study was to evaluate the relationship between uPA, it`s receptor (uPAR) and PAI-1 in locally advanced breast cancer using immunohistochemistry and TissueArray. We analyzed 128 female breast cancer patients. The results were as follows: 113 samples were positive for uPA (88,3%), 71 samples were positive for uPAR (55,5%) and 54 samples were positive for PAI-1 (42,2%). We found no statistical relationship between the expression of plasminogen activators and disease free survival or overall survival in this population. Multiple analyses demonstrated a statistically advantage for disease free survival among patients who did not receive adjuvant chemotherapy (p=0.002), tumor negative for estrogen receptor (p=0,001) and the presence of node invasion (p=0.023). Despite the absence of statistically significance between the expression of plasminogen activator and overall survival we confirmed a significant impact on disease free survival among patients receiving paclitaxel and doxorubicin as preoperative chemotherapy as well a positive impact of adjuvant hormonal and chemotherapy treatment.

El dermatán sulfato (DS) es un glicosaminoglicano endógeno, ampliamente conocido por su acción anticoagulante mediante su interacción con el cofactor II de la heparina para potenciar la inhibición de trombina. En los últimos años se ha sugerido que además el DS aumentaría la actividad fibrinolítica, aunque el mecanismo aún no ha sido completamente dilucidado. En este trabajo se presenta una revisión detallada de los resultados propios y una discusión de la bibliografía disponible respecto de la evaluación del efecto del DS sobre el sistema fibrinolítico. En estudios de activación fibrinolítica, por métodos amidolíticos y coagulométricos, el DS mostró tener efecto pro-fibrinolítico mediante potenciación de la activación de plasminógeno por t-PA y uPA, efecto independiente de su conocida acción anticoagulante. En estudios de caracterización de fibrina, las redes obtenidas en presencia de DS presentaron fibras más largas y delgadas que las redes control, mayor grado de compactación y de lisabilidad; efecto pro-fibrinolítico asociado a su acción anticoagulante. Los resultados presentados contribuyen al esclarecimiento del mecanismo de acción del DS sobre el sistema plasminógeno-plasmina y permiten plantear hipótesis sobre el rol fisiológico de este glicosaminoglicano.


Dermatan sulfate (DS) is well-known for its anticoagulant activity by binding to heparin cofactor II in order to enhance the antithrombin action. It has also been suggested that DS has a profibrinolytic effect, although the exact molecular mechanism is unknown. This review exposes the results obtained and discusses on the available literature on DS effect on the fibrynolytic system. DS exhibited a stimulating effect on the activation of plasminogen by plasminogen activators (t-PA and u-PA), by in vitro amidolytic and coagulometric methods, showing a pro-fibrinolytic effect independent of its known anticoagulant action. Studies of fibrin networks obtained in the presence of DS showed longer and thinner fibers than controls, increased degree of compaction and lisability. Thus, DS displayed a pro-fibrinolytic effect associated to its anticoagulant action. These results contribute to clarify the mechanism of DS action on the plasminogen-plasmin system and to the understanding of the physiologic role of this glycosaminoglycan.

Antecedentes: La trombosis venosa profunda (TVP), es una patología altamente prevalente, de difícil diagnóstico y con graves complicaciones. En la actualidad, los objetivos del tratamiento de esta enfermedad se enfocan en conseguir su control y evitar las complicaciones tanto de la patología como de su tratamiento. Objetivo: Analizar los resultados obtenidos con la nueva fracción proteica con actividad fibrinolítica, en un modelo de trombosis de la vena femoral en conejos. Lugar de trabajo: Laboratorio de Cirugía Experimental, Facultad de Medicina, Universidad Católica de Córdoba. Diseño: Registro prospectivo de datos. Población: 15 conejos New Zeland. Material y Métodos: Para la formación del trombo, se aisló la vena femoral y se generó la trombosis colocando sobre la vena un trozo de papel de filtro embebido con cloruro férrico. Se evalúo y registró seriadamente el flujo venoso, tanto en la pretrombosis como a los 10, 20 y 30 minutos, mediante Eco-Doppler. Comprobada la trombosis se inició la trombolisis. Resultados: En el grupo A, luego de la aplicación de APP, el análisis por eco-Doppler determinó la reperfusión en 11/11 animales (100%). En tanto, en el grupo B, que se aplicó placebo, la reperfusión fue negativa (0/4 conejos) (p=0,001). Conclusión: Posterior a la inducción de la trombosis con cloruro férrico en vena femoral, la infusión de APP (lote A) mostró reperfusión en 11/11, mientras que el resultado en la infusión de placebo (lote B) fue 0/4. Registro con Doppler vascular de la trombosis y fibrinolisis resultó ser método sensible y específico.

La fibrosis hepática involucra múltiples eventos celulares y moleculares que inducen un excesivo depósito de proteínas de matriz extracelular que distorsionan la arquitectura del parénquima hepático, cuya etapa final es conocida como cirrosis. El daño proviene de una variedad de causas como abuso de drogas y enfermedades virales, autoinmunes, metabólicas y colestásicas. La degradación de estas proteínas de matriz ocurre predominantemente como una consecuencia de la acción de metalopro teinasas (MMPs) que degradan sustratos colágenos y no colágenos. La degradación de la matriz en el hígado se lleva a cabo principalmente por la acción de cuatro de estas enzimas: MMP-1, MMP-2, MMP-3 y MMP-9. En el sistema fibrinolítico, las MMPs pueden ser activadas a través de un corte proteolítico por acción del activador de plasminógeno tipo urocinasa y un segundo mecanismo de activación es realizado por las mismas MMPs. La regulación para restringir la actividad puede ser a diferentes niveles; en el sistema fibrinolítico el principal regulador es el PAI- 1, molécula que bloquea la conversión de plasminógeno a plasmina y la MMP no puede ser activada. Un segundo nivel de inhibición es posible a través del TIMP, que inhibe la actividad proteolítica aun cuando las MMPs hayan sido activadas vía plasmina. Durante condiciones patológicas la sobreexpresión de estos inhibidores es dirigida por el factor de crecimiento transformante β, el cual en un padecimiento fibrótico actúa como el más importante factor adverso.


Liver fibrosis and cirrhosis involve multiple cellular and molecular events that lead to deposition of an excess of extracellular matrix proteins and increase the distortion of normal liver architecture. Etiologies include chronic viral hepatitis, alcohol abuse and drug toxicity. Degradation of these matrix proteins occurs predominantly as a result of a family of enzymes called metalloproteinases (MMPs) that specifically degrade collagenous and non collagenous substrates. Matrix degradation in the liver is due to the action of at least four of these enzymes: MMP-1, MMP-2, MMP 3 and MMP 9. In the fibrinolytic system, MMPs can be activated through proteolytic cleavage by the action of urokinase plasminogen activator; a second mechanism includes the same metalloproteinases. This activity is regulated at many levels in the fibrinolytic system. The main regulator is the PAI- 1. This molecule blocks the conversion of plasminogen into plasmin, and the MMP cannot be activated. At a second level, the inhibition is possible by binding to inhibitors called TIMP that can inhibit the proteolytic activity even when the MMPs had been previously activated by plasmin. During abnormal conditions, overexpression of these inhibitors is directed by the transforming growth factor-β that in a fibrotic disease acts as an extremely important adverse factor.

Streptokinase, a 47-kDa protein isolated and secreted by most group A, C and G ß-hemolytic streptococci, interacts with and activates human protein plasminogen to form an active complex capable of converting other plasminogen molecules to plasmin. Our objective was to compare five streptokinase formulations commercially available in Brazil in terms of their activity in the in vitro tests of euglobulin clot formation and of the hydrolysis of the plasmin-specific substrate S-2251Õ. Euglobulin lysis time was determined using a 96-well microtiter plate. Initially, human thrombin (10 IU/ml) and streptokinase were placed in individual wells, clot formation was initiated by the addition of plasma euglobulin, and turbidity was measured at 340 nm every 30 s. In the second assay, plasminogen activation was measured using the plasmin-specific substrate S-2251Õ. StreptaseÕ was used as the reference formulation because it presented the strongest fibrinolytic activity in the euglobulin lysis test. The UnitinaseÕ and SolustrepÕ formulations were the weakest, showing about 50 percent activity compared to the reference formulation. All streptokinases tested activated plasminogen but significant differences were observed. In terms of total S-2251Õ activity per vial, StreptaseÕ (75.7 ± 5.0 units) and StreptonaseÕ (94.7 ± 4.6 units) had the highest activity, while UnitinaseÕ (31.0 ± 2.4 units) and StrekÕ (32.9 ± 3.3 units) had the weakest activity. SolustrepÕ (53.3 ± 2.7 units) presented intermediate activity. The variations among the different formulations for both euglobulin lysis test and chromogenic substrate hydrolysis correlated with the SDS-PAGE densitometric results for the amount of 47-kDa protein. These data show that the commercially available clinical streptokinase formulations vary significantly in their in vitro activity. Whether these differences have clinical implications needs to be investigated.

Avaliação imediata e tardia da terapia fibrinolítica da trombose venosa profunda iliacofemoral por infusão seletiva de rt-PA em baixas doses. Os pacientes foram submetidos à infusão transcateter de rt-PA (1mg/hora) nos segmentos venosos trombóticos, por um tempo médio de 41 horas. Houve fibrinólise efetiva em 12 dos 15 casos, com correlação entre o percentual de fibrinólise e a magnitude da melhora clínica aguda / Early and late evaluation of fibrinolytic therapy of iliofemoral deep venous thrombosis by selective infusion of low-dose rt-PA. Patients were submitted to transcatheter infusion of rt-PA (1mg/kg) in the thrombotic venous segments, with a mean lysis time of 41 hours. Effective lysis was reached in 12 of 15 cases. Statistic correlation between lysis grade and acute clinical improvement was observed...

El carcinoma basocelular es una neoplasia cutánea de crecimiento lento. Rara vez produce metástasis, siendo un modelo ideal para analizar los factores que condicionan la misma. Se han identificado numerosos factores de riesgo: radiación ultravioleta, la cual lesiona directamente al ADN induciendo una inmunodepresión selectiva, algunas cepas del virus papiloma humano y trastornos autosómicos recesivos, donde se identificaron mutaciones del gen supresor del crecimiento tumoral. Actualmente, numerosas líneas de investigación están abocadas al estudio de la baja incidencia de metástasis de éste tumor. En éste carcinoma no existe una disminución en las caderinas epiteliales, las cuales intervienen en la adherencia celular. Asimismo, existe disminución de los receptores de laminina y fibronectina. En la matriz extracelular no se ha observado un aumento de enzimas proteolíticas que la degraden. La inmunovigilancia tumoral está muy activada por medio de un aumento de linfocitos T citolíticos y la producción de interferón gamma. Concluimos que no sólo los queratinocitos neoplásicos intervienen en la metástasis, sino que la matriz extracelular juega un rol preponderante en la escasa incidencia de diseminación de éste tumor, por lo que un estudio exhaustivo de la misma permitiría ser aplicado a otras neoplasias y otorgar una mayor sobrevida

The interaction of plasminogen, tissue plasminogen activator (t-PA) and urokinase with a clinical strain of Helicobacter pylori was studied. Plasminogen bound to the surface of H. pylori cells in a concentration-dependent manner and could be activated to the enzymatic form, plasmin, by t-PA. Affinity chromatography assays revealed a plasminogen-binding protein of 58.9 kDa in water extracts of surface proteins. Surface-associated plasmin activity, detected with the chromogenic substrate CBS 00.65, was observed only when plasminogen and an exogenous activator were added to the cell suspension. The two physiologic plasminogen activators, t-PA and urokinase, were also shown to bind to and remain active on the surface of bacterial cells. epsilon-Aminocaproic acid caused partial inhibition of t-PA binding, suggesting that the kringle 2 structure of this activator is involved in the interaction with surface receptors. The activation of plasminogen by t-PA, but not urokinase, strongly depended on the presence of cells and a 25-fold enhancer effect on the initial velocity of activation by t-PA compared to urokinase was established. Furthermore, a relationship between cell concentration and the initial velocity of activation was demonstrated. These findings support the concept that plasminogen activation by t-PA on the bacterial surface is a surface-dependent reaction which offers catalytic advantages

Objetivos: Analisar os resultados, anatômicos e funcionais, do tratamento das obstruções venosas retinianas pelo fator tecidual ativador de plasminogênio recombinante (rt-PA) intravítreo. Pacientes e Métodos: Cinco pacientes com obstrução da veia central da retina (OVCR) e um paciente com obstrução venosa hemi-retiniana foram submetidos a uma injeção intravítrea de 75mg de rt-PA e reavaliados através de exame oftalmológico completo, retinografia e retinografia flourescente. Resultados: Observou-se melhora anatômica em dois dos seis pacientes estudados, dentre os quais o paciente que apresentava obstrução venosa hemi-retiniana que evoluiu com rápida e acentuada resolução do quadro obstrutivo. Nenhum dos pacientes apresentou melhora funcional. Um dos pacientes apresentou hemorragia vítrea de pequena magnitude e rápida resolução, um mês após a terapêutica, e outro paciente apresentou hemorragia vítrea quanze meses após, tendo esta ocorrido após cirurgia cardíaca.

Urokinase plasminogen activator receptor (uPAR) has been identified some 15 years ago and the anticipation was that is presence on the cell surface will provide a focus for anchoring uPA and possibly protect the enzyme from native inhibitors. The studies of the last decade have shown that uPA localized to the surface of cells by uPAR is indeed an important factor in the process of cancer cell invasion and metastasis. We developed a chick embryo model in which we showed that uPAR is crucial in invasion of stroma and in intravasation (breaching of the blood vessels walls). More recently and unexpectedly, uPAR-a protein anchored in the outer leaflet of the plasma membrane, has been shown to initiate signal transduction events and affect cell migration. We have shown that uPAR co-associates with fibronectin binding integrin, alpha5beta1, activates them and that this interaction leads to a greatly increased level of active ERK. When the association between uPAR and integrin or integrin and fibronectin are interrupted either by reduction of surface uPAR expression, or by other means, human carcinoma cells enter a state of protracted dormancy. We show that very high levels of active ERK are required to keep cancer cells proliferating in vivo.

OBJETIVO: Avaliar se o uso do fator ativador do plasminogênio (rt-PA) nas primeiras 6h do acidente vascular cerebral isquêmico agudo melhora a capacidade funcional. MÉTODOS: Foram estudados 6 pacientes, sendo 4 mulheres, com média de idade 63ñ18 anos, portadores de déficit neurológico há menos de 6h e sem recuperaçäo espontânea, sendo a causa na metade dos casos embólica e nos demais trombótica. Foi realizada tomografia computadorizada do crânio (CT) seguida da angiografia cerebral seletiva (3 casos) ou Doppler transcraniano (3 casos), para demostraçäo da oclusäo arterial e desua recanalizaçäo após o fibrinolítico. A infusäo do rt-PA foi endovenosa periférica em 5 casos, na dose de 0,9/kg, ou intra-arterial em 1 caso, na dose de 0,5 mg/kg, ambas durante 60 min. Após 24h do uso rt-PA foi realizado CT crânio para controle de complicaçäo hemorrágica e novo exame neurológico. RESULTADOS: Todos apresentam oclusäo arterial, sendo o território da artéria cerebral média em 5 caos e da cerebra posterior em um. A recanalizaçäo foi obtida em 4 casos com manutençäo da sua perviabilidade na 24a hora. A recuperaçäo neurológica foi completa em um, parcial em 3 e inalterada em 2, os quais näo obtiveram recanalizaçäo arterial. O controle tomográfico de 24h demostrou transformaçäo hemorrágica em somente um paciente, porém dentro da área isquêmica e sem piora clínica. Näo ocorreram complicaçöes fatais pelo uso do rt-PA e um paciente faleceu por pneumonia. CONCLUSÄO: A recanalizaçäo arterial com o rt-PA foi obitida em 4 de 6 pacientes, com melhora funcional. O rt-PA deve ser utilizado em pacientes selecionados com intervalo de até 3h do envento, segundo estudos randomizados.


Purpose - To evaluate thrombolytic therapy with rt-PA for acute stroke within 6h of symptom onset, and assessment of neurologic outcome. Methods - We studied 6 patients, four women, mean age 63±18 years, with severe neurologic deficit within 6h of stroke onset, and with no spontaneous improvement. The stroke was embolic in 3, and thrombotic in the others. All patients were submitted to a head CT scan followed by either a cerebral angiography in 3 patients, or a transcranial Doppler, in the other 3 for assessment of arterial obstruction, and patency after thrombolytic therapy. We used 0.9mg/kg of rt-PA, IV, over 60min in 5 patients, and 0.5mg/kg by intra-arterial infusion, over 60min, in one. At the begining a bolus of 10% of the total dose was dilevered during 1 to 2min. Head scan was repeated 24h and 7 days after treatment to detect ischemic areas and hemorrhagic complications. Results - Middle cerebral artery occlusion was observed in 5 patients and posterior cerebral artery occlusion in one. The obstruction was cleared in 4 patients with persistence of the patency after 24h. A complete neurologic recovery was found in one patient, and a partial recovery in three. In two patients there was failure of arterial recanalization with no neurologic recovery. Only one patient had hemorrhagic transformation of ischemic tissue,without neurologic worsening. Death occured in one patient due to pulmonary infection.Conclusion - Arterial patency with thrombolytictherapy was effective in 4 of our 6 patients. All 4 patientsalso disclosed a certain degree of neurologic improvement, rt-PA can be successfuly used in selected patients upto 3h of the event onset, as shown in randomized studies
