Abstract This study investigated the synergy testing of penicillin, cephalosporin, amphenicols, and aminoglycoside in the camel milk (n=768 samples), subsequently used for isolation of MDR S. aureus targeting mecA gene. Antibiotic susceptibility of S. aureus showed >90% isolates were sensitive to ciprofloxacin and trimethoprim and resistant against oxacillin, ampicillin, and cefoxitin. Further, 50-85% of the S. aureus were sensitive to gentamicin, oxytetracycline, and chloramphenicol and resistant against cefotaxime, vancomycin, and cefixime. Minimum inhibitory concentration (MIC) of cefotaxime, (C) and ampicillin (A) in combination with gentamicin (G) was reduced by 99.34% and 70.46%, respectively, while with chloramphenicol (Ch), reduction was 57.49% and 60%, respectively. In addition, the Fractional Inhibitory Concentration Index (FICI) of G+A, Ch+C and Ch+G combinations showed synergy against 80%, 60%, and 30% of MDR S. aureus, respectively. Similarly, C+A and Ch+G displayed indifferent interaction against 70 % and 30% of isolates, respectively, while the later showed additive interaction against 10% of MDR S. aureus. Altogether, our results described effective combination of gentamicin and chloramphenicol with ampicillin and cefotaxime to combat MDR S. aureus

Plasmid-mediated polymyxin resistance was first described in 2015, in China, in Escherichia coli carrying the mcr-1 (Mobile Colistin Resistance-1) gene. Since then, it has become a major public health challenge worldwide, representing a major threat to human and animal health. In addition, there are still few reports on the prevalence of mcr-1 in Enterobacteriaceae isolated from humans, animals and food. Therefore, the purpose of the study was to investigate the occurrence of the mcr-1 gene in bacterial isolates with phenotypic resistance to polymyxin B obtained from clinical specimens of companion animals. Phenotypic resistance to polymyxin B were determined by broth microdilution and the susceptibility profile to other antimicrobials (amikacin, amoxicillin/clavulanate, ampicillin, ampicillin/sulbactam, aztreonam, cefazolin, cefepime, cefotaxime, cefoxitin, ceftazidime, ceftriaxone, chloramphenicol, ciprofloxacin, doxycycline, ertapenem, gentamicin, imipenem, marbofloxacin, meropenem, phosphomycin, piperacillin/tazobactam, tetracycline, ticarcillin/clavulanate, tobramycin and trimethoprim/sulfamethoxazole) by disc-diffusion agar method. The extraction of bacterial DNA was performed via heat shock followed by spectrophotometric evaluation. To verify the presence of mcr-1, the Polymerase Chain Reaction was employed using specific primers, followed by agarose gel electrophoresis. The positive isolates had the corresponding amplicons sequenced. In this study, there were identified the first isolates of Escherichia coli, Klebsiella spp. and Enterobacter spp. carrying the mcr-1 gene derived from specimens of companion animals in Brazil. Our results suggest the dissemination of resistance to polymyxins in the community and the environment, highlighting the need for surveillance and optimized treatment guidelines.(AU)


A resistência à polimixina mediada por plasmídeo teve sua primeira descrição em 2015, na China, em Escherichia coli portadora do gene mcr-1 (Mobile Colistin Resistance-1) e a partir de então tornou-se um grande desafio para a saúde pública em todo o mundo, constituindo uma grande ameaça à saúde humana e animal. Além disso, ainda existem poucos relatos sobre a prevalência de mcr-1 em Enterobacteriaceae isoladas de humanos, animais e alimentos. Sendo assim, o objetivo do estudo foi investigar a ocorrência do gene mcr-1 em isolados bacterianos com resistência fenotípica à polimixina B, oriundos de materiais clínicos de animais de companhia. A resistência fenotípica à polimixina B foi determinada por microdiluição em caldo e o perfil de sensibilidade aos demais antimicrobianos (amicacina, amoxicilina/clavulanato, ampicilina, ampicilina/sulbactam, aztreonam, cefazolina, cefepime, cefotaxima, cefoxitina, ceftazidima, ceftriaxona, cloranfenicol, ciprofloxacina, doxiciclina, ertapenem, gentamicina, imipinem, marbofloxacino, meropenem, fosfomicina, piperacilina/tazobactam, tetraciclina, ticarcilina/clavulanato, tobramicina sulfametoxazol/trimetoprim) foram determinados pelo método disco difusão. A extração do DNA bacteriano foi realizada via choque térmico, seguido de avaliação espectrofotométrica. Para a verificação da presença do mcr-1 foi utilizada a Reação em Cadeia da Polimerase com emprego de iniciadores específicos, seguida de eletroforese em gel de agarose. Os isolados positivos tiveram os correspondentes amplicons sequenciados. Nesse estudo foram identificados os primeiros isolados de Escherichia coli, Klebsiella spp. e Enterobacter spp. portadores do gene mcr-1 derivados de espécimes de animais de companhia no Brasil. Este estudo sugere a disseminação da resistência às polimixinas na comunidade e no meio ambiente, destacando a necessidade de vigilância e diretrizes otimizadas de tratamento.(AU)

Enteropathogenic Escherichia coli (EPEC) and Shigatoxigenic E. coli (STEC) strains are among the major pathotypes found in poultry and their products, which are capable of causing human enteric infections. Colistin has been claimed the drug of choice against diseases caused by multidrug-resistant Gram-negative bacteria (MDRGN) in humans. The mcr-1 gene was the first plasmidial gene that has been described to be responsible for colistin resistance and has also been detected in birds and poultry products. Our study aimed to detect the mcr-1 gene in enteropathogenic strains of E. coli in order to evaluate the resistance to colistin in broilers. The material was obtained from 240 cloacal samples and 60 broiler carcasses. The strains were isolated by the conventional bacteriological method and by the virulence genes, which characterize the enteropathogenic strains and resistance, and the samples were detected by polymerase chain reaction (PCR). Of the 213 isolated strains of E. coli, 57 (26.76%) were characterized as atypical EPEC and 35 (16.43%) as STEC. The mcr-1 gene was found in 3.5% (2/57) of the EPEC strains and 5.7% (2/35) of the STEC strains. In this study, it was possible to confirm that the mcr-1 resistance gene is already circulating in the broiler flocks studied and may be associated with the pathogenic strains.(AU)


Escherichia coli Enteropatogênica (EPEC) e Shigatoxigênica (STEC) estão entres os principais patotipos encontrados em aves e produtos avícolas que são capazes de causar doença entérica no homem. A colistina tem sido preconizada como droga de escolha para o tratamento de doenças causadas por bactérias Gram-negativas multirresistentes em humanos. O gene mcr-1 foi o primeiro gene plasmidial a ser descrito como responsável pela resistência a colistina e tem sido descrito em aves e produtos avícolas. Este estudo tem como objetivo a detecção do gene mcr-1 em estirpes de E. coli enteropatogênicas a fim de avaliar a resistência a colistina em frangos de corte. O material foi obtido a partir de 240 amostras cloacais e 60 carcaças de frango de corte. As estirpes foram isoladas pelo método bacteriológico convencional e os genes de virulência, que caracterizam as estirpes enteropatogênicas, e resistência foram detectados pela reação em cadeia pela polimerase (PCR). Das 213 estirpes de E. coli isoladas, 57 (26,76%) foram caracterizadas como EPEC atípica e 35 (16,43%) como STEC. O gene mcr-1 foi encontrado em 3,5% (2/57) das estirpes EPEC e 5,7% (2/35) das estirpes STEC. Neste estudo foi possível confirmar que o gene de resistência mcr-1 já está em circulação nos lotes de frango de corte estudados e pode estar associado às estirpes patogênicas.(AU)

BACKGROUND Healthcare-associated infections caused by bacteria such as Pseudomonas aeruginosa are a major public health problem worldwide. Gene regulatory networks (GRN) computationally represent interactions among regulatory genes and their targets. They are an important approach to help understand bacterial behaviour and to provide novel ways of overcoming scientific challenges, including the identification of potential therapeutic targets and the development of new drugs. OBJECTIVES The goal of this study was to reconstruct the multidrug-resistant (MDR) P. aeruginosa GRN and to analyse its topological properties. METHODS The methodology used in this study was based on gene orthology inference using the reciprocal best hit method. We used the genome of P. aeruginosa CCBH4851 as the basis of the reconstruction process. This MDR strain is representative of the sequence type 277, which was involved in an endemic outbreak in Brazil. FINDINGS We obtained a network with a larger number of regulatory genes, target genes and interactions as compared to the previously reported network. Topological analysis results are in accordance with the complex network representation of biological processes. MAIN CONCLUSIONS The properties of the network were consistent with the biological features of P. aeruginosa. To the best of our knowledge, the P. aeruginosa GRN presented here is the most complete version available to date.

ABSTRACT Objective: Several mechanisms account for carbapenem resistance in Pseudomonas (P) aeruginosa which is an emerging problem at a tertiary care hospital (TCH) in Jamaica. The observed pattern of carbapenem resistance that results from efflux mechanisms is unique because it is specific to meropenem (MEM). An investigation of efflux as a mechanism of carbapenem resistance was needed as the information obtained could inform therapeutic and infection control strategies. Methods: At the Microbiology Laboratory of a TCH in Jamaica, from May 2009 to March 2011, of 105 multidrug-resistant Gram-negative bacilli isolated from clinical specimens submitted for routine identification and susceptibility testing, all the MEM-resistant P aeruginosa isolates (a total of 10) were selected. They were tested for efflux using the efflux inhibitor phenylalanine-arginine-β-naphthylamide (PAβN) in a method described by Giske et al in 2008. Results: This study detected evidence of MEM efflux in 80% of MEM-resistant P aeruginosa implicated in nosocomial infections at this TCH in Jamaica, using the PAβN inhibition assay. Meropenem-efflux-positive isolates belonged to two unrelated chromosomal lineages. Conclusion: These results underscored the need for improved surveillance and control to prevent this mechanism from emerging in further P aeruginosa strains.


RESUMEN Objetivo: Varios mecanismos explican la resistencia al carbapenem en Pseudomonas (P) Aeruginosa - un problema que recientemente se está presentando en un hospital de atención terciaria (HAT) en Jamaica. El patrón observado de resistencia al carbapenem que resulta de los mecanismos de eflujo es único, porque es específico del meropenem (MEM). Fue necesario realizar una investigación del eflujo como mecanismo de resistencia al carbapenem, ya que la información obtenida podría usarse para las estrategias de terapia y de control de la infección. Métodos: En el Laboratorio de Microbiología de un HAT en Jamaica, de mayo de 2009 a marzo de 2011, de 105 bacilos gram-negativos polifármacorresistentes aislados de especímenes clínicos presentados para identificación de rutina y pruebas de susceptibilidad, se seleccionaron todos los aislados de P aeruginosa (un total de 10) resistentes a MEM. Estos aislados fueron sometidos a prueba de detección de eflujo, usando el inhibidor de eflujo de fenilalanina-arginina-β-naftilamida (PAβN) en un método descrito por Giske et al en 2008. Resultados: Este estudio detectó evidencia de eflujo de MEM en el 80% de P aeruginosa resistente a MEM implicado en infecciones nosocomiales en este HAT de Jamaica, usando el ensayo de inhibición PAβN. Los aislados positivos al eflujo de meropenem pertenecían a dos linajes cromosómicos no relacionados. Conclusión: Estos resultados subrayaron la necesidad de mejorar la vigilancia y el control para prevenir que este mecanismo vuelva a producirse en nuevas cepas de P aeruginosa.

La resistencia a las polimixinas mediada por plásmidos (gen mcr-1) representa una amenaza para la salud pública, puesto que colistina es utilizada en la práctica médica como una de las últimas alternativas para el tratamiento de gérmenes multiresistentes. Este estudio describe la circulaciónde cepas de Enterobacterias que portan este gen de resistencia, aisladas de pacientes hospitalizados, así como también de la comunidad. Los hallazgos de la Red de Vigilancia de la Resistencia a los Antimicrobianos-Paraguay fueron de casi el 5 % (4,7) en cepas remitidas con criterio de sospecha, siendo las especies involucradas Escherichiacoli, Klebsiella pneumoniae y Salmonella Schwarzengrund. Además, por métodos moleculares se confirmaron en todas ellas la portación de otros genes de resistencia (KPC, CTX-M, Qnr B, Qnr S, aac (6`)-Ib-cr) asociados al mcr-1. Resistance to polymyxins mediated by plasmids (mcr-1 gene) represents a threat to public health, since colistin is used in medical practice, as one of the last alternatives, for the treatment of multi-resistant germs. This study describes the circulation of strains of Enterobacteria that carry this resistance gene, isolated from hospitalized patients, as well as from the community. The findings of the Red de Vigilancia de la Resistencia a los Antimicrobianos­Paraguay were almost 5% (4.7) in strains submitted with suspicion criteria; the species involved being Escherichia coli, Klebsiella pneumoniae and Salmonella Schwarzengrund. In addition, molecular methods confirmed in all of them the carrying of other resistance genes (KPC, CTX-M, Qnr B, Qnr S, aac (6`)-Ib-cr) associated with mcr-1. In this study, we screened differentially expressed genes in a multidrug-resistant isolate strain of Clostridium perfringens by RNA sequencing. We also separated and identified differentially expressed proteins (DEPs) in the isolate strain by two-dimensional electrophoresis (2-DE) and mass spectrometry (MS). The RNA sequencing results showed that, compared with the control strain, 1128 genes were differentially expressed in the isolate strain, and these included 227 up-regulated genes and 901 down-regulated genes. Bioinformatics analysis identified the following genes and gene categories that are potentially involved in multidrug resistance (MDR) in the isolate strain: drug transport, drug response, hydrolase activity, transmembrane transporter, transferase activity, amidase transmembrane transporter, efflux transmembrane transporter, bacterial chemotaxis, ABC transporter, and others. The results of the 2-DE showed that 70 proteins were differentially expressed in the isolate strain, 45 of which were up-regulated and 25 down-regulated. Twenty-seven DEPs were identified by MS and these included the following protein categories: ribosome, antimicrobial peptide resistance, and ABC transporter, all of which may be involved in MDR in the isolate strain of C. perfringens. The results provide reference data for further investigations on the drug resistant molecular mechanisms of C. perfringens.

The multidrug resistant and the emergence of methicillin-resistant staphylococci isolated from animals, food, and humans are public health concern. These microorganisms produce different toxins related to food poisoning in humans. This study aimed to characterize Staphylococcus spp. isolated from two organic milk farms in Brazil. A total of 259 milk samples were collected, from which 58 (22.4%) Staphylococcus spp. were isolated. The highest sensibility to ceftiofur and sulfamethoxazole/trimethoprim was observed in 96.6% of Staphylococcus spp., and whereas 89% were resistant to penicillin G. The mecA gene was detected in 13.8% of the isolates. SEA and SEC were the most common enterotoxins detected. PFGE revealed genetic heterogeneity from S. intermedius and S. warneri analyzed, while S. aureus presented similar profiles among isolates from the two studied herds. To the best of our knowledge, the current study describes for the first time presence of enterotoxins, mecA gene, and genetic diversity of staphylococci isolated from organic dairy farms in Brazil.(AU)


A emergência de estafilococos multirresistentes e resistentes à meticilina, isolados de animais, alimentos e humanos é uma preocupação em saúde pública. Esses micro-organismos produzem diferentes toxinas relacionadas à intoxicação alimentar em humanos. Este estudo caracterizou Staphylococcus spp. isolados em duas fazendas orgânicas no Brasil. Foram coletadas 259 amostras de leite em duas propriedades leiteiras orgânicas, nas quais 58 (22,4%) estirpes de Staphylococcus spp. foram isoladas. A maior sensibilidade dos isolados foi observada para ceftiofur e sulfametoxazol/trimetoprim em 96,6%. Em contraste, acima de 89% de resistência dos estafilicocos foi encontrada para penicilina G. O gene mecA foi identificado em 13,8% dos isolados. SEA e SEC foram as enterotoxinas mais comumente detectadas. PFGE revelou heterogeneidade genética entre S. intermedius e S. warneri, enquanto S. aureus demonstraram perfis semelhantes entre isolados dos dois rebanhos estudados. Relata-se pela primeira vez no Brasil a detecção de enterotoxinas, o gene mecA e diversidade genética em estafilococos isolados de vacas em produção orgânica.(AU)

ABSTRACT Background Due to the high prevalence of co-infection by hepatitis C virus (HCV) and human immunodeficiency virus (HIV) and the severity of these infections, the understanding of the biological mechanisms involved in these processes, including viral behavior and host genetic profile, is of great importance for patient treatment and for public health policies.Some single nucleotide polymorphisms (SNPs) in the human genome, such as the SNP rs1045642 (C3435T) in the MDR1 gene, have been reported to be associated to the sustained virological response (SVR) to HCV treatment in HCV-HIV co-infected patients. Objective The present study analyzes the MDR1 gene C3435T polymorphism in HCV-HIV co-infected patients. Methods A total of 99 HCV-HIV patients were included in the study. The DNA was extracted from blood samples, and the SNP rs1045642 was assessed by Real Time PCR (qPCR). Risk factors for acquiring the virus and the SVR after HCV treatment with pegylated interferon-alpha and ribavirin were also analyzed. Results Among the patients, 54 (54.5%) were male and 45 (45.5%) were female. The average age was 46.1±9.8 years. The SVR after HCV treatment was 40%. The frequencies of MDR1 genotypes CC, CT and TT were 28.3%, 47.5% and 24.2%, respectively. Allele frequencies were 52% for the C allele and 48% for the T allele. No association was found for SNP rs1045642 (C3435T) regarding response to treatment (P=0.308). Conclusion - In this study, the C3435T polymorphism in the MDR1 gene appears not to be associated with SVR in HCV-HIV co-infected individuals.


RESUMO Contexto Em virtude da elevada prevalência da coinfecção pelos vírus da hepatite C (HCV) e da imunodeficiência humana (HIV) e às inúmeras complicações que esses vírus acarretam, é fundamental o maior entendimento do comportamento biológico dos mesmos. O polimorfismo de nucleotídeo único rs1045642 C3435T do gene de resistência a múltiplas drogas MDR1, no qual ocorre modificação do códon ATC para ATT, parece estar relacionado à resposta virológica sustentada ao tratamento do HCV em coinfectados HCV-HIV. Objetivo Mapear o polimorfismo C3435T do gene MDR1 em pacientes coinfectados HCV-HIV e correlacionar com dados clínicos e laboratoriais. Métodos Foram analisados 99 pacientes coinfectados HCV-HIV. A identificação molecular do polimorfismo de nucleotídeo único rs1045642 do gene MDR1 foi realizada pela técnica de PCR em tempo real (qPCR) alelo-específico com primers e sondas específicos para a identificação desse polimorfismo. Fatores de risco para a aquisição do HCV e a resposta virológica sustentada ao tratamento do HCV com interferon-alfa peguilado e ribavirina foram analisados. Resultados Dentre os pacientes avaliados, 54 (54,5%) eram do gênero masculino e 45 (45,5%) do gênero feminino. A média de idade foi de 46,1 anos (±9,8). As frequências dos genótipos CC, CT e TT foram 28,3%, 47,5% e 24,2% respectivamente, e as frequências alélicas foram 52% para alelo C e 48% para alelo T. Não houve associação entre o gene MDR1 e a resposta virológica sustentada (P=0,308). Conclusão Neste estudo, o polimorfismo C3435T no gene MDR1 não apresentou associação com a resposta virológica sustentada ao tratamento em indivíduos coinfectados HCV-HIV.

OBJECTIVES:Asthma is a chronic inflammatory lung disease characterized by bronchial hyperresponsiveness and airflow obstruction. Genetic and oxidative stress factors, in addition to pulmonary and systemic inflammatory processes, play a pivotal role in the pathogenesis of asthma. The products of the multidrug resistance-1 gene protect lung tissue from oxidative stress. Here, we aimed to evaluate the association between the multidrug resistance-1 gene C>T polymorphism and asthma with regard to oxidative stress-related parameters of asthmatic patients.METHODS:Forty-five patients with asthma and 27 healthy age-matched controls were included in this study. Blood samples were collected in tubes with ethylenediaminetetraacetic acid. DNA was extracted from the blood samples. The multidrug resistance-1 gene polymorphism was detected by polymerase chain reaction and a subsequent enzyme digestion technique. The serum levels of total oxidant status and total antioxidant status were determined by the colorimetric measurement method.RESULTS:The heterozygous polymorphic genotype was the most frequent in both groups. A significant difference in the multidrug resistance-1 genotype frequencies between groups indicated an association of asthma with the TT genotype. A significant difference between groups was found for wild type homozygous participants and carriers of polymorphic allele participants. The frequency of the T allele was significantly higher in asthmatic patients. The increase in the oxidative stress index parameter was significant in the asthma group compared with the control group.CONCLUSIONS:The multidrug resistance-1 gene C/T polymorphism may be an underlying genetic risk factor for the development of asthma via oxidant-antioxidant imbalance, leading to increased oxidative stress.

El aumento de tuberculosis y la multidrogo resistencia de cepas de micobacterias es un problema de los sistemas de salud, en 2009, en el Hospital Roosevelt Gordillo y cols, determinaron la TB-MDR en pacientes con tuberculosis diagnosticada microbiológicamente, la tasa de resistencia fue de 4.3%. Objetivo: Determinar los patrones de resistencia y perfiles genéticos de cepas con monoresistencia y cepas TB-MDR del Complejo M. tuberculosis. Métodos: Se utilizaron dos métodos para evaluar las cepas de M. tuberculosis, un método fenotípico, MGIT, y un método Genotípico, Genotype HAIN LifeScience para determinar el perfil genético de las cepas. Resultados: Se evaluaron 846 cepas de micobacterias de los años 2008 al 2013, encontrándose un 2.2% de TB-MDR. Las cepas evaluadas genotípicamente fueron 761, a las cuales se determinó los genes de resistencia, encontrándose monoresistencia a Isoniacida en 58 cepas, 7.6%, monoresistencia a Rifampicina en 18 cepas, 2.4% y 15 cepas MDR, 2.0%. Las mutaciones más frecuentes en monoresistencia fueron inhA MUT1 y katG MUT1 y la combinación de ambos genes 3.2%, 3.0% y 1.3%, para cepas TB-MDR la combinación rpoB Mutación silenciosa + katG MUT1 + inhA MUT1. Se encontró que en pacientes con cepas MDR el 3.1% son HIV+ y el 1.5% son HIV-...(AU)


Introduction: The increase of tuberculosis and multidrug resistance in mycobacteria strains is a problem for health systems, in 2009, in Hospital Roosevelt, Gordillo and cols, determined the TB-MDR in patients diagnosed with tuberculosis microbiologically, the resistance rate was 4.3%. Objective: To determine the resistance patterns and genetic profiles of monoresistant strains and MDR-TB strains of M. tuberculosis complex. Methods: Two methods for evaluating M. tuberculosis strains were used, a phenotypic method, MGIT, and a genotypic method, Genotype HAIN LifeScience to determine the genetic profile of the strains. Results: 846 strains of mycobacteria of the years 2008 to 2013 were evaluated, finding 2.2% of MDR-TB. The strains genotypically evaluated were 761, of wich, resistance genes were determined, finding isoniazid monoresistance in 58 strains, 7.6%, Rifampicin monoresistance in 18 strains, 2.4% and 15 MDR strains, 2.0%. The most frequent mutations for monoresistant strains were inhA MUT1 and katG MUT1 and the combination of both genes 3.2%, 3.0% and 1.3%, respectively, and the most frequent mutations for TB-MDR strains was the combination rpoB silent mutation + katG MUT1 + inhA MUT1. There was found that in patients with MDR strains 3.1% are HIV+ and 1.5% are HIV-. Conclusions: The percentage of TB-MDR strains was 2.3%, and the most common genes were rpoB silent mutation, inhA MUT1 y katG MUT1. There was found a higher percentage of monoresistance in isoniazid than rifampicin, being the HIV+ patient population the one that presented higher percentages in both monoresistance to RIF and INH and TB-MDR strains.

Nas infecções causadas pelo gênero Mycobacterium identificar a espécie é importante para distinguir entre as cepas e agrupá-las por critérios de interesse para microbiologistas e clínicos, influenciando inclusive no esquema de tratamento a ser aplicado. Segundo a World Health Organization, em 2014 a tuberculose atingiu o mesmo patamar que a infecção por HIV (Human Immunodeficiency Virus) em relação à mortalidade. Por outro lado, as micobacterioses por micobactérias não tuberculosas (MNT) vêm aumentando, mas como não é de notificação compulsória, pouco se sabe da diversidade de espécies que acometem os pacientes no estado do Piauí. Assim, o presente estudo teve como objetivo, estimar a frequência de infecção por Complexo Mycobacterium tuberculosis e MNT, correlacionar os métodos microbiológios e moleculares utilizados na sua identificação e descrever o perfil de sensibilidade a tuberculostáticos nas amostras de M. tuberculosis, isoladas de pacientes referenciados ao Laboratório Central de Saúde Pública do Piauí (LACEN-PI) no período de janeiro 2014 a março de 2015. Para tal, foi realizado estudo descritivo transversal em 142 espécimes clínicos de pacientes, correspondendo a 20,1 por cento dos casos suspeitos enviados ao LACEN-PI. Na maioria dos casos a espécie M. tuberculosis foi identificada (69,7 por cento; 99/142) e em 9,9 por cento (14/142) foram isoladas MNT. O restante dos casos (20,4 por cento 29/142) não foi possível fazer o diagnóstico definitivo. As espécies de micobactérias não tuberculosas identificadas foram M. abscessus (3,5 por cento ); M. avium (1,4 por cento), M. intracelullare (1,4 por cento), M. asiaticum (0,7 por cento ), M.szulgai (0,7 por cento) e M. kansasii (0,7 por cento), e maioria dos casos era do sexo masculino (63,4 por cento ; 9/14) e apenas 1/64 (1,6 por cento) era HIV positivo...

On the other hand,mycobacteriosis by nontuberculous mycobacteria (NTM) has increased, however as it is not of mandatorynotification and little is known about the diversity of species that affect patients in the state of Piaui. The presentstudy aimed to estimate the frequency of Mycobacterium tuberculosis and NTM infections, correlate themicrobiological and molecular methods used to identify them and describe the sensibility profile of the TB drugsin M. tuberculosis samples isolated from patients referred to t the Piaui State Public Health Central Laboratory(LACEN-PI), from January 2014 to March 2015. To this end, a cross-sectional descriptive study was performedon 142 clinical specimens of patients, corresponding to 20.1% of suspected cases submitted to LACEN-PI. Inmost cases the species M. tuberculosis was identified (69.7%, 99/142) and 9.9% (14/142) were isolated NTM. Inthe remaining cases (20.4%; 29/142) it was not possible to make a conclusive diagnosis. Objective To verify the effect of bathing on the body temperature of preterm infants (PTI). Method Systematic review conducted in the following bibliographic electronic sources: Biblioteca Virtual em Saúde/Lilacs (BVS), Cumulated Index of Nursing and Allied Health Literature (CINAHL), Cochrane Library, Google Scholar, PubMed, SCOPUS and Web of Science, using a combination of search terms, keywords and free terms. The review question was adjusted to the PICO acronym (Patient/population, Intervention, Control/comparative intervention, Outcome). The selected publications were evaluated according to levels of evidence and grades of recommendation for efficacy/effectiveness studies, as established by the Joanna Briggs Institute. Results Eight hundred and twenty four (824) publications were identified and four studies met the inclusion criteria, of which three analyzed the effect of sponge baths and the effect of immersion baths. Conclusion Sponge baths showed a statistically significant drop in body temperature, while in immersion baths the body temperature remained stable, although they studied late preterm infants. .


Objetivo Determinar el efecto del baño en la temperatura corporal del recién nacido prematuro. Método Revisión sistemática realizada en las fuentes bibliográficas electrónicas BVS, CINAHL, Cochrane Library, Google Scholar, PubMed, Scopus y Web of Science. Las búsquedas fueron realizadas mediante combinación de descriptores, palabras clave y términos libres y se ajustó la cuestión de la revisión a la estrategia PICO. Las publicaciones seleccionadas se evaluaron de acuerdo con los niveles de evidencia y grados de recomendación para los estudios de eficacia/efetividad establecidos por el Instituto Joanna Briggs. Resultados Se identificaron 824 publicaciones y cuatro atendieron a los criterios de inclusión, de los cuales, tres analizaron el efecto del baño de esponja y uno el efecto del baño de inmersión. Conclusión El baño de esponja mostró una disminución estadísticamente significativa en la temperatura corporal, en cuanto que el baño de inmersión, la temperatura corporal se mantuvo estable, aunque el estudio haya sido realizado con recién nacidos prematuros tardíos.

A pergunta da revisão foi ajustada ao acrônimo PICO (Paciente/população, Intervenção, Intervenção controle/comparativa, Desfecho analisado). As publicações selecionadas foram avaliadas de acordo com os níveis de evidência e graus de recomendação para estudos de eficácia/efetividade estabelecidos pelo Instituto Joanna Briggs. Resultados Foram identificadas 824 publicações e quatro estudos atenderam aos critérios de inclusão, dos quais três analisaram o efeito do banho de esponja e um o efeito do banho de imersão. Conclusão O banho de esponja mostrou queda da temperatura corporal estatisticamente significante, enquanto no banho de imersão a temperatura corporal permaneceu estável, embora tenham sido estudados RNPT tardios. .

Overexpression of cytokine-induced apoptosis inhibitor 1 (CIAPIN1) contributes to multidrug resistance (MDR) in breast cancer. This study aimed to evaluate the potential of CIAPIN1 gene silencing by RNA interference (RNAi) as a treatment for drug-resistant breast cancer and to investigate the effect of CIAPIN1 on the drug resistance of breast cancer in vivo. We used lentivirus-vector-based RNAi to knock down CIAPIN1 in nude mice bearing MDR breast cancer tumors and found that lentivirus-vector-mediated silencing of CIAPIN1 could efficiently and significantly inhibit tumor growth when combined with chemotherapy in vivo. Furthermore, Western blot analysis showed that both CIAPIN1 and P-glycoprotein expression were efficiently downregulated, and P53 was upregulated, after RNAi. Therefore, we concluded that lentivirus-vector-mediated RNAi targeting of CIAPIN1 is a potential approach to reverse MDR of breast cancer. In addition, CIAPIN1 may participate in MDR of breast cancer by regulating P-glycoprotein and P53 expression.

O surgimento de bactérias Gram-negativas resistentes à drogas tem sido progressivo ealarmante. Entre os patógenos de particular importância está Pseudomonas aeruginosamultirresistentes à drogas (MDR). Um clone epidêmico de P. aeruginosa MDR produtor dametalo-beta-lactamase SPM-1, chamado clone SP (ST277), tem sido encontrado em diferentesestados brasileiros. O gene blaSPM-1 foi descrito em uma estrutura genética chamada ISCR4,responsável pela sua mobilização e expressão. Além do gene blaSPM-1, tem sido descrito, nesteclone, a presença de um integron de classe I carreando genes de resistência (In163) e umelemento genético ISCR14 carreando uma metilase de RNAr 16S (rmtD), que confereresistência a altas concentrações de aminoglicosídeos. Este tipo de associação resulta em umperfil de resistência extremamente preocupante. Assim, este estudo teve como objetivoinvestigar a contexto genético dos genes blaSPM-1 e rmtD, além de caracterizar mutações emgenes cromossomais associados a multirresistência a antimicrobianos em amostras de P.aeruginosa pertencentes ao clone ST277, através de Sourthen blot e sequenciamento de novageração. A partir de 50 amostras de P. aeruginosa MDR isoladas entre 2007 e 2010, foramselecionadas 13 amostras pertencentes ao ST277 (através de MLST) que apresentarampositividade para blaSPM-1 e/ou rmtD e In163 (através de PCR), sendo 12 do clone SP (A) euma de um clone diferente, M, através de PFGE. Através de restrição do DNA com asenzimas de restrição SpeI e XbaI, e posterior hibridação com sondas dos genes alvos (blaSPM-1,rmtD e In163) foi possível observar que os genes alvos encontravam-se em fragmentosdistintos e que a amostra do clone M apresentou um perfil de hibridação diferentes das demaisamostras demonstrando a ocorrência de vários rearranjos na mesma, indicando uma maiordistância evolutiva...

OBJECTIVES: Conflicting data from studies on the potential role of multidrug resistance 1 gene polymorphisms in inflammatory bowel disease may result from the analysis of genetically and geographically distinct populations. Here, we investigated whether multidrug resistance 1 gene polymorphisms are associated with inflammatory bowel diseases in patients from Rio de Janeiro. METHODS: We analyzed 123 Crohn's disease patients and 83 ulcerative colitis patients to determine the presence of the multidrug resistance 1 gene polymorphisms C1236T, G2677T and C3435T. In particular, the genotype frequencies of Crohn's disease and ulcerative colitis patients were analyzed. Genotype-phenotype associations with major clinical characteristics were established, and estimated risks were calculated for the mutations. RESULTS: No significant difference was observed in the genotype frequencies of the multidrug resistance 1 G2677T/A and C3435T polymorphisms between Crohn's disease and ulcerative colitis patients. In contrast, the C1236T polymorphism was significantly more common in Crohn's disease than in ulcerative colitis (p = 0.047). A significant association was also found between the multidrug resistance 1 C3435T polymorphism and the stricturing form of Crohn's disease (OR: 4.13; p = 0.009), whereas no association was found with penetrating behavior (OR: 0.33; p = 0.094). In Crohn's disease, a positive association was also found between the C3435T polymorphism and corticosteroid resistance/refractoriness (OR: 4.14; p = 0.010). However, no significant association was found between multidrug resistance 1 gene polymorphisms and UC subphenotypic categories. CONCLUSION: The multidrug resistance 1 gene polymorphism C3435T is associated with the stricturing phenotype and an inappropriate response to therapy in Crohn's disease. Este estudo visou a caracterização molecular de mecanismos de virulência e resistência aos antimicrobianos em isolados de K. pneumoniae MDR provenientes de um hospital universitário em Recife-PE. Seis isolados de K. pneumoniae produtores de carbapenemase foram obtidos de pacientes hospitalizados na UTI de um hospital universitário do Recife. Os isolados apresentaram resistência a todos os antimicrobianos beta-lactâmicos e quinolonas testados, mas sensibilidade a amicacina, polimixina B e tigeciclina, por meio de microdiluição em caldo. A tipagem molecular por PFGE revelou que os isolados são intimamente relacionados, apresentando três subclones distintos. Dois STs foram detectados, o ST340 e o ST11, ambos pertencentes ao CC258. Os genes blaKPC-2 e blaSHV-11 foram detectados em todos os isolados, seguido do gene blaCTX-M-15 em quatro dos seis isolados e por fim os genes blaCTX-M-2, qnrB19, aac(6')-31 em dois dos seis isolados. Os genes blaKPC-2 e blaCTX-M-15 estavam presentes em um mesmo plasmídeo de aproximadamente 133 Kb pertencente ao IncI-gama em quatro isolados. Nos demais isolados os genes blaKPC-2 e blaCTX-M-2 eram carreados também por um plasmídeo de, aproximadamente, 133 Kb, entretanto, não foi possível tipar o mesmo com as metodologia utilizada. O gene qnrB19 foi detectado sendo carreado por um plasmídeo de 15 Kb pertencente ao IncY. Todos os isolados apresentaram integron de classe 1, associado com a resistência aos aminoglicosídeos. Mutações na região QRDR de GyrA (Ser83Ile) e ParC (Ser80Ile) foram detectadas em todos os isolados analisados, sendo esse o principal mecanismo de resistência as quinolonas detectadas ao longo do estudo. Adicionalmente, a permeabilidade de membrana externa foi analisada, verificando-se a ausência da Ompk35 em todos os isolados e da OmpK36 em uma das amostras analisadas. A investigação dos genes de virulência revelou a presença de antígenos capsulares do tipo K2 entre os isolados. Genes codificadores das fímbrias do tipo I e III foram detectados, assim como genes envolvidos na síntese de LPS e operon da urease. A presenca de micro-organismos multirresistentes e virulentos em unidades hospitalares reforça a necessidade de medidas para a rápida contenção de possíveis infecções hospitalares causadas por esses patógenos.

Multidrug resistance (MDR) poses a serious impediment to the success of chemotherapy for laryngeal cancer. To identify microRNAs and mRNAs associated with MDR of human laryngeal cancer Hep-2 cells, we developed a multidrug-resistant human laryngeal cancer subline, designated Hep-2/v, by exposing Hep-2 cells to stepwise increasing concentrations of vincristine (0.02-0.96'µM). Microarray assays were performed to compare the microRNA and mRNA expression profiles of Hep-2 and Hep-2/v cells. Compared to Hep-2 cells, Hep-2/v cells were more resistant to chemotherapy drugs (∼45-fold more resistant to vincristine, 5.1-fold more resistant to cisplatin, and 5.6-fold more resistant to 5-fluorouracil) and had a longer doubling time (42.33±1.76 vs 28.75±1.12'h, P<0.05), higher percentage of cells in G0/G1 phase (80.98±0.52 vs 69.14±0.89, P<0.05), increased efflux of rhodamine 123 (95.97±0.56 vs 12.40±0.44%, P<0.01), and up-regulated MDR1 expression. A total of 7 microRNAs and 605 mRNAs were differentially expressed between the two cell types. Of the differentially expressed mRNAs identified, regulator of G-protein signaling 10, high-temperature requirement protein A1, and nuclear protein 1 were found to be the putative targets of the differentially expressed microRNAs identified. These findings may open a new avenue for clarifying the mechanisms responsible for MDR in laryngeal cancer.

O regime de tratamento com múltiplas drogas é correspondente a uma interação de drogas que pode causar efeitos adversos e falha no tratamento. Essa interação pode gerar modificações funcionais dos transportadores de membrana e por consequência a biodisponibilidade das drogas durante o tratamento. Dentre os transportadores de drogas transmembranares está a glicoproteína-P (P-gp), uma proteína de 170KD, produto do gene MDR1, caracterizada como uma “ATP Binding cassete” (ABC). Seu papel está muito bem definido nas células neoplásicas multirresistentes a drogas, assim como sua relação com as drogas para o tratamento da infecção pelo HIV. Entretanto, pouco tem sido estudado sobre esta bomba de efluxo na tuberculose multirresistente (TBMR). Neste estudo analisamos por citometria de fluxo sua expressão e atividade de efluxo nos monócitos, principal célula relacionada com a tuberculose e também em linfócitos e granulócitos do sangue periférico por meio da citometria de fluxo. A taxa de efluxo foi medida através do uso da Rodamina 123 (Rho123) e a expressão da P-gp através do anticorpo monoclonal anti-CD243 (clone UIC2). A utilização direta do sangue total para a determinação da atividade de efluxo por citometria de fluxo caracterizou a implantação de uma nova ferramenta de análise para a pesquisaA análise contemplou 52 por cento do total de pacientes em tratamento de TBMR no ambulatório do Laboratório de Pesquisa em Micobacterioses do Instituto de Pesquisa Clínica Evandro Chagas (IPEC). Para as análises foram levadas em consideração a idade, cor da pele, o tempo de tratamento e a quantidade de drogas administradas. O estudo revelou que há correlação entre a expressão da P-gp nos monócitos e a idade dos pacientes (P<0,01). Diferenças entre pacientes brancos e não brancos também foram observadas. Em linfócitos a expressão de P-gp quando aumentada foi diretamente proporcional à atividade de efluxo observada nos monócitos (P< 0,05)...


When two or more drugs are administrated, such interactions can cause additive, synergisticor antagonistic effects. The drug transporters may undergo treatments effects and therefore change the drugs bioavailability during the treatment and can cause treatment failure. Amongthe known drug transporters there is transmembrane P-glycoprotein (P -gp). It is a 170 KDa transmembrane protein which is a product of multidrug resistance MDR1 gene. This protein isa well-characterized ABC transporter (ATP Binding Cassette) and is responsible for efflux of the chemotherapeutic agents from resistant cancer cells. There are few studies about this protein in multidrug-resistant tuberculosis (MDR-TB). In this study, P-gp expression and activity were detect by flow cytometry in monocytes, the main cell related with theM.tuberculosis infection, as well as in lymphocytes and granulocytes from whole peripheral blood. P-gp mediated efflux was determined indirectly by measuring the retention of afluorescent P-gp substrate, rhodamine 123 (Rho123). P-gp expression was assessed using anti-P-gp monoclonal antibody UIC2 (CD243). The efflux activity with whole blood usingflow cytometry technique as a new tool to pharmacogenetic research was developed in this study. We analyzed 52 percent of outpatient in MDR-TB treatment from Laboratory ResearchMycobacteriosis Institute Evandro Chagas Clinical Research (IPEC). We considered variables as age, race, treatment time, amount of drug administered at the time of blood collection and their correlation with P-gp expression and activity. INTRODUCTION: The prevalence of cephalosporins and carbapenem-resistant Klebsiella pneumoniae strains is rising in Brazil, with potential serious consequences in terms of patients' outcomes and general care. METHODS: This study characterized 24 clinical isolates of K. pneumoniae from two hospitals in Recife, Brazil, through the antimicrobial susceptibility profile, analyses of β-lactamase genes (blaTEM, blaSHV,blaCTX-MblaKPC, blaVIM, blaIMP, and blaSPM), plasmidial profile and ERIC-PCR (Enterobacterial repetitive intergenic consensus-polymerase chain reaction). RESULTS: ERIC-PCR and plasmidial analysis grouped the isolates in 17 and 19 patterns, respectively. Six isolates from one hospital presented the same pattern by ERIC-PCR, indicating clonal dissemination. All isolates presented blaSHV, 62.5% presented blaCTX-M-2, 29% blaTEM, and 41.7% blaKPC. Metallo-β-lactamase genes blaand blawere not detected. Eleven isolates were identified carrying at least 3 β-lactamase studied genes, and 2 isolates carried blaSHVblaTEM, blaCTX-M-2 and blaKPC simultaneously. CONCLUSIONS: The accumulation of resistance genes in some strains, observed in this study, imposes limitations in the therapeutic options available for the treatment of infections caused by K. pneumoniae in Recife, Brazil. These results should alert the Brazilian medical authorities to establish rigorous methods for more efficiently control the dissemination of antimicrobial resistance genes in the hospital environment.


INTRODUÇÃO: A prevalência de cepas de Klebsiella pneumoniae resistentes a cefalosporinas e carbapenêmicos está aumentando no Brasil, com sérias consequências em termos de desfechos dos pacientes e cuidados gerais. MÉTODOS: Este estudo caracterizou 24 isolados clínicos de K. pneumoniae provenientes de dois hospitais de Recife, Brasil, através do perfil de susceptibilidade a antimicrobianos, análise de genes de β-lactamase (blaTEM,blaSHV,blaCTX-MblaKPC,blaVIM, blaIMP,and blaSPM), perfil plasmidial e ERIC-PCR (Enterobacterial repetitive intergenic consensus-polymerase chain reaction). RESULTADOS: A análise da ERIC-PCR e do perfil plasmidial agrupou os isolados em 17 e 19 perfis, respectivamente. Seis isolados de um hospital apresentaram o mesmo padrão de ERIC-PCR, indicando disseminação clonal. Todos os isolados apresentaram blaSHV, 62,5% apresentaram blaCTX-M-2, 29% blaTEM e 41,7% blaKPC. Genes de metalo-β-lactamase blaVIM, blaIMP e blaSPM não foram detectados. Onze isolados foram identificados carreando, pelo menos, três dos genes de β-lactamase estudados, dentre estes, dois isolados continham blaSHV,blaTEM, blaCTX-M-2 e blaKPC simultaneamente. CONCLUSÕES: O acúmulo de genes de resistência em algumas cepas, observado nesse estudo, impõem limitações nas opções terapêuticas disponíveis para o tratamento de infecções causadas por K. pneumoniae em Recife, Brasil. Estes resultados devem alertar as autoridades médicas brasileiras para estabelecer rigorosos métodos para controlar eficientemente a disseminação de genes de resistência a antimicrobianos no ambiente hospitalar.
