RESUMO

Fourteen adult ferrets of both sexes and ranging in weight from 800-1500 gm were used in this investigation. Six of the ferrets were anaesthesized and a midline abdominal incision made to expose the spleen. With the aid of a Hamilton syringe and needle, the spleen of each ferret was injected with 0.1ml of 5% WGA-HRP in 0.5 M sodium chloride. The eight remaining ferrets were used as controls. Two of these had injections of 0.1ml normal saline into the spleen. The second set of two ferrets was injected with 0.1 ml of 5% WGA-HRP in buffer after bilateral trunkal vagotomy. The third set of two ferrets received intraperitoneal injection of 0.1ml of 5% WGA-HRP while in the last set the tracer was injected into the hepatic portal vein. Following the injections, the ferrets were allowed to survive for 48-72 hours after which each ferret was perfused transcardially with normal saline followed by a fixation containing paraformaldehyde and glutaraldehyde in phosphate buffer, pH 7.4 at room temperature and finally with 10% buffered sucrose at 4°C. After perfusion, the brainstem was extracted from the skull and immersed in 10% buffered sucrose and kept in the fridge overnight. Tranverse frozen sections of the brainstem were then taken, processed for WGA-HRP neurohistochemistry and analysed under light and dark field illuminations. Examination of the section taken from the six experimental ferrets revealed presence of WGA-HRP in neurons of the dorsal motor neurons in any brainstem nucleus.

RESUMO

Sickle cell disease is a global health problem. It is an inherited blood disorder affecting the red blood cells. There are different types of sickle cell disease. Homozygous sickle cell (SS) disease, results when the infant inherits two abnormal sickle genes, one from each parent. In Jamaica SS disease affects approximately 1 in 300 births and some form of sickle cell disease, 1 in 150 births. Complications of sickle cell disease include acute splenic sequestration. The cause and triggers of acute splenic sequestration (ASS) are largely unknown. The spleen becomes acutely enlarged, trapping a proportion of the red cell mass and leading to acute anaemia, circulatory failure and sometimes death. Emergency blood tranfusion is extremely important in the management of acute splenic sequestration. Parental education may allow prevention of death from ASS by early transfusion. Parents need to be taught how to diagnose splenic enlargement, and to recognize the signs and symptoms of increasing anaemia characterized by pallor. It is important for mothers to examine splenic size on a regular basis and when the child looks ill. At the Jamaican Sickle Cell Unit, parents are instructed to examine the spleen at least once a day. The aim was to identify the parents' and guardians' knowledge of ASS, their attitudes towards the examination of the abdomen and practices in detecting an enlarging spleen. The objectives were to determine the percentage of parents and guardian who regularly examined the abdomen to determine spleen size; who had detected an enlarging spleen and the outcome; who were aware of the proper technique for examining the spleen; who were aware of what to do if an enlarging spleen was found; and identify the barriers that prevent regular examination for splenic enlargement. The study was carried out on the parents and guardians of children four years and younger, with homozygous sickle cell disease, attending the sickle cell unit. At the time of the study, the population of children four years and younger with SS disease was 23.7. A questionnaire was designed based on the objectives of the study, to solicit data from the partcipants. The researcher administered the questionnaire to 60 parents and guardians who attended the clinic over a two-month period- March and April 2000. The participants were also asked to demonstrated their competence in examining the spleen.(Au)

Spleen-intact and splenectomized Saimiri monkeys of Guyanan origin were examined for their potential suitability for Plasmodium falciparum protection studies. The animal could be readily infected with adapted strains of P. falciparium (Indochina 1/CDC and Uganda Palo Alto FUP strains), but spontaneously recovered without drug treatment and without development of severe clinical disease. In intact animals, peak parasitemia prior to recovery generally ranged from 0.1 percent to 10 percent, whereas in splenectomized animals the peak parasitemia was generally higher so that some animals were given drug treatment to assist in recovery from infection. In reinfection studies, previously infected spleen-intact monkeys demonstrated sterile immunity to the homologous parasite strain but not to a heterologous strain. However, in monkeys infected with the heterologous strain, the peak parasitemia was less than in the first infection and of shorter duration. Splenectomized animals did not demonstrate sterile immunity although the peak parasitemia achieved was less than in the previous infection of each of these monkeys. While the lack of major clinical disease indicated that these monkeys did not provide a good animal model for human malaria, the development of protective immunity was consistent with a useful role in evaluating candidate vaccine antigens (AU)

RESUMO

Spontaneous rupture of the spleen in pregnancy is rare. It presents with severe internal haemorrhage with a high perinatal and maternal mortality. Splenic preservation is now a well-known option in dealing with splenic damage, and is recommended where possible to avoid diminished immunological competence. This is the first reported case of splenic preservation following splenic damage in pregnancy.(AU)

RESUMO

A 3-week-old female infant with polysplenia syndrome and partial situs inversus with reversed rotation of the intestinal tract who presented with subacute bowel obstruction at the University Hospital of the West Indies is reported. The absence of associated cardiovascular anomalies is notable. A modified Ladd's procedure was successful in relieving the obstruction. The clinical and diagnostic features of polysplenia syndrome are discussed. When complex cardiovascular anomalies, which usually account for the high mortality rate, are absent, normal growth and development can be expected. (AU)

RESUMO

A prospective radiological and haemotological study of 182 patients with homozygous sickle cell anaemia has been undertaken to assess the prevalence and pattern of splenic opacification and relate this to the blood indices. Opacification was observed in 31 percent of patients. In 55 percent of these, the pattern was punctuate, whereas in 32 percent it was amorphous. A curvilinear appearance was seen in the remainder. In the amorphous group, a high percentage (72 percent), the spleen was severely contracted. The pattern of opacification and degree of contraction was related to age. The haemotological indices indicate a lower haemolytic rate in patients with splenic opacifications indicating a milder disease process with a greater persistence of the splenic capillary bed (AU)

Attention is drawn to the incidence of accessory spleens in Suriname. In a post-mortem study at the Mortuarium of the Academic Hospital Paramaribo, consecutive examinations were performed to locate nodules around and near the spleen and the pancreas-tail. Histologic sections from the accessory spleens and from the main spleens were routinely prepared and microscopically examined. The hematoxylin and the eosin methods were used for staining. Between 1 January 1976 and 31 August 1978, 518 effective post-mortem examinations were performed (neonatal age to elderly). Accessory spleens occurred in 57 cases (11.0 per cent). The racial distribution was: 1 of 8 Creoles, 1 of 12 East-Indians, 1 of 12 Javanese and 1 of 6 others. These cases seemed unrelated to other congenital anomalities. The histologic diagnosis in each case was of a normal accessory spleen, but accessory spleens often mimic changes in the main spleen. It is reccommended that an effort should be made to locate and remove any accessory spleen, especially when splenectomy is performed for hypersplenism and other hematologic disorders

Between 1972 and 1974, 3,278 bats of 28 species were processed for virus isolation by suckling mouse (SM) inoculation. Two strains each of two related viruses, not hitherto reported from Trinidad, were isolated from insectovorous bats. Rio Bravo (RB) virus was isolated from salivary glands and saliva of the house bat, Molossus ater. The other virus, isolated from salivary glands, saliva and spleen of the molustache bat, Pteronotus parnollii, is a hitherto undescribed agent herein named Tamana bat virus (TBV). This virus has arbovirus characteristics: sensitivity to ether, pathogenicity for SM, and ability to hemagglutinate goose erythrocytes, but no serological relationship with known arboviruses and other viruses could be detected. In inoculation experiments with TBV, fatal illness was produced only in infant mice and rats, salivary virus excretion was demonstrated in a monkey and in Phyllostomus hastatus bats, and virus was passed in bats by subcutaneous inoculation of infected saliva. Sera of humans and 39 species of bats were tested for hemagglutination inhibition (HI): 46 out of 169 human and 125 out of 887 bat sera reacted with RB antigen, and of the positive bat sera reacted with TBV antigen, positives occurring in 15 bat species comprising insectovorous, fruit-eating and vampire bats, with highest incidence in cave-roosting species. In SM neutralization tests, 18 out of 27 HI-positive human sera protected against RB, 1 out of 10 against TBV; bat sera protective against RB were found in 4 insectovorous species, and against TBV in 8 species including the vampire bat, Desmodus rotundus. No evidence of arthropod transmission of either virus was found (AU)

Polysplenism ( accessory spleen, supernumary spleen, accessory spleniculi) is said to be common, and is reported in one-fifth to one-third of all postmortems (Robbins, 1967)). They are small, but variable in size, and may be single or multiple. The usual locations are near the hilus of the spleen, in the gastrosplenic ligament, or near the tail of the pancraes. Eraklis et al. (1972) cite the as being found in 16 percent of about 1500 cases of splenectomy. If there is an indication for splenectomy, probably exept for trauma, accessory spleens should also be removed. This is because lesions affecting the main spleen usually affect the accessory spleen (Miale, 1972), and functionally they are similar. Thus, although it may take years, patients may have relapses, for example in hereditary spherocytosis, after previous splenectomy, because of stimulated growth of accessory spleens not removed at surgery. This rare situation, however, is more commonly due not to accessory spleens but to splenules due to trauma of the spleen at time of surgery with seeding of the abdominal cavity with splenic tissue-a condition called splenosis (Williama et al., 1972). The author wishes to draw attention to the incidence of accessory spleens in a postmortem study in Surinam mainly to remind the surgeons to look for and remove them when splenectomy is performed especially for hematologic disorders....AU

RESUMO

The electron microscope provides easy confirmation in Gaucher's disease. This may be done with conventional ultrathin sections or, more quickly, by negative staining of tissues minced in 1 percent ammonium acetate. Typical helical structures associated with glucocerebroside storage were easily demonstrated with tissues that had been fixed in formaldehyde solution and embedded in paraffin blocks. Similar success was obtained with splenic tissue that had been in a museum jar for 17 years.(AU)

Nine patients had a syndrome characterized by the presence of a sea-blue histiocyte and splenomegaly. Lipid analysis of liver from the most recent patient revealed accumulations of specific glycosphingolipids and phospholipids. Urinary mucopolysaccharide excretion was increased, and there was a suggestion of mucopolysaccharide storage in the hepatic parenchymal cells. Clinically, patients with the disease may have a relatively benign course, with mild purpura secondary to thrombocytopenia, or may have progressive hepatic cirrhosis, hepatic failure and death. The name "syndrome of the sea-blue histiocyte," the striking morphologic feature of the disease, is suggested for these cases (AU)

Two young Jamaican women of Chinese descent with idiopathic thrombocytopenic purpura were found to have lipid-laden histiocytes in their spleens; their condition has improved following splenectomy. The electron microscopic appearances of the spleens suggest that platelet destruction contributed to the accumulation of lipid (AU)

RESUMO

A case of diffuse, malignant, pleural mesothelioma is reported in a 60-year-old Negro man. The clinical picture was most unusual; when the patient presented initially with chest pain, the radiographic appearances in the chest were normal, but extensive liver calcification was demonstrated. The latter was subsequently shown to be caused by metastases from the mesothelioma which took 2 more years to produce clinical and radiographic changes in the chest. There was microscopic evidence that the extensive liver calcification was dystrophic in nature and had resulted from marked degenerative changes and necrosis in the metastatic malignant pleural mesothelioma. Radiographs of the skeleton revealed focal, translucent lesions with sclerotic margins in the right seventh rib and the right iliac. Microscopic examination showed these bony lesions to be metastases. No previous report of secondary deposits in bone producing such radiographic appearances could be found in the literature (AU)

The occurrence is described of a natural infection with Trypanosoma (Schizotrypanum) cruzi in the common opossum, Didelphis marsupialis, captured in El Cayo District, British Honduras. The identity of the parasite was established from histological sections if the heart muscle, which showed pseudocysts filled with leishmania forms. Seven species of wild animal were experimentally infected with T. cruzi. Rats of the species Heteromys desmarestianus, Ototylomys philotis, Tylomys nudicaudus and Oryzomys sp. were all highly susceptible to the parasite and developed a fatal illness. It ie concluded that these species are unlikely to act as natural reservoirs for T. cruzi, but that they may prove useful in further experimental studies on the parasite and for the passage of strains. A rat of the species Nyctomys sumichrasti showed a high resistance to the infection; only scanty intracellular forms of T. cruzi were found in heart-muscle smears, and chagastic pseudocysts were present in small numbers in histological sections of the heart muscle. Results similar to those obtained with the Nyctomys rat were observed in a coati (Nasua narica), which also showed a high resistance to the challenge . No intracellular forms of T. cruzi were found in the heart muscle, but they were present in small numbers in the liver and spleen; they were morphologically identical to those seen in the heart muscle smears from the Nyctomys rat. No infection developed in two young grey foxes (Urocyon cinereo-argenteus) when challenged with a highly virulent strain of T. cruzi. It is concluded that these animals are unlikely to act as natural reservoirs for the parasite in British Honduras. It is believed that, although experimental inoculation of wild animals with T. cruzi may be prejudiced by their possible previous contact with the parasite, the results may give some indication of the nature of possible reservoir-hosts. The pathological findings at autopsy in 7 patients over the age of 30 with sickle cell anaemia are described and discussed. The 3 youngest died of well recognized complications of sickle cell anaemia. The 4 older patients, all over 50, had been remarkably free of direct complications and died of disease which could have been unrelated to sickle cell anaemia. Cirrhosis and chronic nephritis were not seen. Pulmonary vascular disease was seen in 2 patients but its significance in relation to sickle cell anaemia has not yet been established in Jamaican patients.(AU)

RESUMO

The movements of iron-dextran, following a total dose infusion of up to 2550 mg. iron in six severely iron deficient patients, have been studied using a Fe tagged complex. The preparation is removed from the plasma in 8-10 days. Fifty per cent of the iron was incorporated into haemoglobin in 3-4 weeks and the iron stores replenished rapidly. Using organ scanning, the liver was confirmed as the site of primary plasma clearance, but accumulations of iron were detected in the spleen and bone marrow. Only minute quantities of radioactivity appeared in the urine. No untoward effects on iron metabolism have been detected, but reduction of the serum iron and total binding capacity to low levels 6 weeks post infusion is of interest. An attempt has been made to interpret the findings to elucidate the ferrokinetics of this method of treatment.(Summary)

Duplication of the entire stomach and first part of the duodenum in a ten week old male infant is reported. This is the fourth case of complete duplication of the stomach and the first in which the duodenum is also duplicated. The history on gastro-intestinal duplication is reviewed and the aetiology of the anomaly discussed. (AU)
