RESUMO

The incidence of congenital syphilis reflects the incidence of total and maternal syphilis in the community and syphilitic seroprevalence rates among antenatal attenders. We reviewed notifications of syphilis in Jamaica over the last 20 years as well as 30 cases of congenital syphilis admitted to Bustamante Hospital Children and the University Hospital of the West Indies during 1986 and 1987. Reported syphilis cases were 2611 in 1972, fell to 1367 in 1979, rose to 3070 in 1989 and again declined to 2121 in 1992. Between 1979 and 1985 there were 10 or less reported cases of congenital syphilis annually. Increasing numbers of congenital syphilis were reported in the late 1980s with a high of 60 cases in 1990 which has since fallen to 50 cases in 1992. The antenatal clinic VDRL reactor rate increased from around 7 percent in 1980 to 17 percent in 1990. The increase in congenital syphilis appears to be related to the increase in syphilis throughout the Americas, the decline in the public health services due to economic constraints, the shortage of nurses and laboratory staff and a breakdown in antenatal care, specifically the prompt identification and treatment of pregnant women with syphilis. Measures are being put in place to address these deficiencies and prevent the occurrence of congenital syphilis in Jamaica (AU)

Congenital syphilis now occupies a high priority as a public health concern. In 1994, a case control study was conducted to elucidate factors associated with congenital syphilis. Cases (n=61) were obtained from the Bustamante Hospital for Children, Victoria Jubilee Hospital and the University Hospital of the West Indies. Data collection was done by hospital records review. Results indicate that mothers of cases were more likely to be unmarried (p<0.005), unemployed and unskilled (p<0.03). The time of mother's first antenatal visit (p<0.03) and, the type of health care facility where antenatal service was sought were significantly associated with congenital syphilis (p<0.0001). Cases were predominantly from the parish of Kingston & St. Andrew and were more likely to have been of low birth weight, less than 2.5 kilograms (p<0.0001). As expected, cases were significantly associated with mothers who were positive for syphilis (p<0.0001). Recommendations are to develop strategies for reduction in the vulnerability of the "at risk" population as well as promotion of early antenatal attendance of pregnant females thereby facilitating early treatment of maternal syphilis (AU)

RESUMO

Seroprevalence of toxoplasma gondii, rubella virus, cytomegalovirus, herpes simplex virus infections (TORCH) and syphilis were determined in order to assess the immune/susceptibility status in Jamaican pregnant women in 1986. The positive rates were 57 percent (T. gondii), 69 percent (rubella), 97percent (CMV), 91 percent (HSV), and 4.9 percent (syphilis), respectively. The rate of reactivity for rubella was over 50 percent in all parishes, the highest being 85 percent in St. Thomas. The seroprevalence of T. gondii was lowest in Trelawny (37.5 percent). There were no significant differences in seropositivity of CMV and HSV infections between women from various parishes. The importance of seroprevalence of the TORCH group of agents and syphilis on perinatal morbidity and mortality in Jamaican women is discussed, and appropriate recommendations for prevention and control of congenital infections in Jamaica are suggested. Serological tests for syphilis (STS) were performed on sera from 15,876 members of the general adult population, 3,039 pregnant women and 1,029 children, in order to assess the prevalence and value of reactive STS in Jamaicans; 2.7 percent of sera from the general adult population, 4.7 percent from pregnant women and 2.2 percent from children were reactive in the Venereal Disease Research Laboratory (VDRL) test. Reactive Flourescent Treponemal Absorption (FTA-ABS) tests occurred in significantly higher frequency in pregnant women (4 percent ) than in the general population (2.2 percent; p<0.001). The prevalence of reactive STS in children did not differ significantly from that of the general adult population. The predictive value of a reactive VDRL test in pregnant women (85 percent) was higher than that of the general population (79 percent). These results suggest that inadequate screening with the inexpensive VDRL test would enable the early detection and treatment of syphilis, and could prevent increasing incidence of congenital syphilis in Jamaica (AU)

RESUMO

Twenty-eight cases of congenital syphilis (CS) were diagnosed shortly after birth at the Mount Hope Women's Hospital, Trinidad, during the period January 1985 to December 1988. The average annual incidence of CS was 115 per 100,000 live births, and the absolute number of cases and annual incidence doubled in the latter 2 years of the study. RESUMO

The ultrastructural changes in the liver in a case of congenital syphillis were studied. Treponema organisms were easily demonstrated in 1-æm sections stained by toluidine blue, as well as in electron micrographs. Most organisms were extracellular, but some were identified within hepatocytes.(AU)

RESUMO

Twenty-three cases of congential syphilis (CS) were diagnosed shortly after birth at the Mount Hope Women's Hospital, Trinidad during the period Janaury 1985 to June 1988. Diagnostic criteria used were a positive VDRL test in the 19 liveborn infants (100 percent) and their mothers during pregnancy (20/30) or after delivery (3/23), and signs suggestive of CS, including osteochondritis and/or periostitis (58 percent), mucocutaneous changes (52 percent), hepatomegaly (83 percent), splenomegaly (70 percent), intrauterine growth retardation (39 percent), hydrops (13 percent), thrombocytopenia (11/16), jaundice (11/19), CSF pleocytosis (4/8), and increased CSF protein (5/8). In the 4 stillbirths and 3 of the 5 neonatal deaths, post mortem findings confirmed the diagnosis of CS. All 23 mothers attended antenatal clinic at least once; 16(70 percent), were 25 years or younger and there was significant reproductive wastage. Only 12 (60 percent) of the 20 gravidae, with a positive VDRL test received treatment. Of 11 untreated gravidae, 3 did not have a VDRL test done, 3 delivered before test results became known, 1 went into labour before going for treatment, and 4(2 of whom were substance abusers) did not comply with instructions to obtain treatment. The absolute number and the rate of CS per 100,000 births doubled from 1986-1987 in this series. A steep increase in the incidence of infectious syphilis is preventable and measures should be introduced (a) to detect syphilis early in all pregnant women, (b) to minimize delay in receiving VDRL test results, (c) to provide treatment and follow-up at the antenatal clinic, and (d) improve data capture and documentation of all forms of syphilis on a national scale (AU)
