A total of 206 community-acquired and 73 nosocomial infections in 50 malnourished Jamaican children were studied prospectively. Predominantly community-acquired infections in the 50 children, included gastroenteritis (68 percent), otitis media (60 percent), rhinopharyngitis (60 percent), oral candidiasis (46 percent), skin infections (40 percent), pneumonia (28 percent), bacteraemia (24 percent) and bacteriuria (18 percent). The most frequent nosocomial infections were rhinopharyngitis (34 percent), lower respiratory tract infections (24 percent) and septicaemia (18 percent). In those infections where an aetiological agent was identified, Giardia lamblia was the commonest enteric pathogen, Staphylococcus epidermidis, the most frequent blood culture isolate and Klebsiella sp. were recovered from the majority of urines. The lack of clinical signs and symptoms and atypical clinical presentation in some infected malnourished children were attributed to impairment of the acute inflammatory response. Diagnosis of infection in these children required a high index of suspicion and a comprehensive screening system. Nasal throat and axial swabs taken on admission revealed significant colonization with coliforms and pneumococcus; however, these swabs were not useful as indicators of potentially infective organisms. Four of the fifty children died and two of these deaths were attributed to infection. (AU)

RESUMO

Childhood gastroenteritis is a universal disease which continues to be a major contributor to infant mortality, especially in developing countries. Within recent years rotavirus and, most recently, Campylobacter jejuni have been identified as the most common pathogens in childhood gastroenteritis. We present a pilot study of 400 children and aged 3 months to 6 years with gastroenteritis who were studied between July 15, 1983 and July 14, 1984. Rotavirus (11 percent), was the most frequently identified pathogen followed by Shigella in 7 percent, enteropathogenic E. coli in 4 percent, Salmonella species in 3 percent and C. jejuni in 1 percent of children; 8 (2 percent) had multiple pathogens. Rotavirus and C. jejuni cases were analysed in detail. Of the patients with C. jejuni, 66 percent presented with bloody diarrhoea, and fever, while for Rotavirus 80 percent of cases had diarrhoea, 70 percent vomiting and 43 percent upper respiratory symptoms. C. jejuni strains were uniformly sensitive to Erythromycin, Tetracycline, Chloramphenicol, Gentamicin and Nitrofurantoin. All patients had an uneventful recovery after their illness (AU)

RESUMO

Since 1952, 112 childen with sickle cell anaemia (SCA) in Jamaica have had an aplastic crisis. Outbreaks occurred in 1956, 1960, 1965-67, 1971-73, and 1979-80. Most cases occurred in children under 10 years of age, and an aplastic crisis in a patient over the age of 15 years is rare. There were 38 cases in 1970-80 and stored serum specimens from 28 of these were available for virus studies. Evidence for infection with a parvovirus-like agent was found in 24 of these 28 cases. Viral antigen was detected in 2 patients, both of whom demonstated seroconversion. Serconversion during 1980 was detected in a further 7, increasing amounts of antibody during the convalescent period were found in 5, antibody was found in 2 of 4 patients from whom only an acute phase specimen was available and the remaining 10 were antibody positive in the only convalescent phase sample available for testing. Antibody was found in 4 of 94 controls with the SS genotype (in retrospect 2 of these may have had an aplastic crisis) and in 17 percent of 48 controls with a normal haemoglobin (AA) genotype. The results accord with the possibility that the parvovirus-like agent is the principal cause of aplastic crisis in SCA (Summary)

Sera from 39 species of bats collected in Trinidad between 1972 and 1974 were tested against some or all of 18 arboviruses in hemagglutination inhibition (HI) and/or suckling mouse neutralization (N) tests. A few sera were HI-positive with Mucambo, eastern equine encephalitis (EEE), Oriboca, Restan, Manzanilla, Guama, Bimiti, and Catu. No sera were HI-positive with Mayaro, Caraparu or Maguari. Many sera inhibited one or more of the group B hemagglutinins: Ilheus, St. Louis encephalitis (SLE), dengue 2, and yellow fever (YF), positives occurred in nearly every species of bat, being most frequent with Ilheus. In N tests, a few or single sera were found to protect against Ilheus, Nepuyo, Guama, Bimiti, and Cocal, while none protected against EEE, SLE, YF or Catu. Many sera positive in HI test with Ilheus, SLE or YF failed to neutralize the respective virus. Tacaribe neutralizing antibody was demonstrated in Artibeus jamaicensis and A.lituratus, the sources of past virus isolation, in the fruit bats Sturnira lilium and Vampyrops helleri, and in the vampire bat Desmodus rotundus. Sera from 19 other species gave either negative or inconclusive results. No convincing evidence of Tacaribe antibody was found in 29 human sera, 20 from bat collectors (AU)

RESUMO

An analysis of infections and infestations in Dominica indicates that these were major health problems. The common diseases were gastro-enteritis, typhoid fever, amoebiasis, and helminthiasis. Hospital bacteriological investigations showed a pattern similar to that seen elsewhere in the Commonwealth Caribbean. A striking feature was the obvious need for proper assessment of antibiotic sensitivity patterns. For example, staphylococci were sensitive to penicillin in 40 percent of cases, a situation which is far different from that which is at present observed in North America. This survey was carried out in collaboration with physicians practising in Dominica, and their years of experience of the local situation allowed the authors of this paper to introduce perspectives and priorities which are not usually possible in publications based entirely on figures reported by statistical departments. Recent improvements in clinical and laboratory services in Dominica have made accurate diagnosis possible in most cases except those in which virological studies were necessary (AU)
