RESUMO

This study reports the first 13 cases of biopsy-proven low rectal carcinoma treated by transanal electrocoagulation using a locally manufactured instrument. At the University Hospital of the West Indies, over a 16-year period, 9 patients were treated for cure, six of whom have no clinical evidence of recurrence for one to 12 years. Four cases who were offered this procedure for palliation defaulted after a single treatment. Transanal electrocoagulation provides an additional option for treating patients with small, mobile low rectal cancers, following careful screening for evidence of nodal or distant spread. The procedure can also be used in the local control of disease in patients with distant spread or in whom a major operation is not feasible (AU)

RESUMO

This study reports on the surgical experience for prostatic pathology in the urological department of the Centre Hospitalier Universitaire (CHU) of Guadeloupe for the period August 1, 1992 to June 30, 1993. There were 202 surgical interventions comprising 95 (48 percent) benign prostatic hypertrophies, 104 (52 percent) adenocarcinomas and 3 (1.5 percent) doubtful. The diagnostic efficiency of Prostatic Specific Antigen (PSA) was studied in terms of specificity and sensitivity to discriminate between benign and malignant diseases and the usefulness of scintigraphy. From the 104 adenocarcinomas, only 7 radical prostatectomies were performed. Other patients were either too old for surgery or presented bone metastasis. These results give evidence of a dramatically high percentage of adenocarcinomas in Guadeloupe (AU)

RESUMO

Paraffinized tissue from Barbadian women with histologically proven gential carcinoma was subjected to a consensus polymerase chain reaction method. Nineteen patients had cervical and one, vaginal carcinoma. The histological types were 17 squamous cell carcinoma, 2 adenocarcinoma and 1 adenosquamous carcinoma. HPVDNA was detected in 18/20 (90 percent). HPVDNA type 16 in 13 (65 percent), type 33 and type 45 in 1 (5 percent) each and 3 (15 percent) could not be typed. HPVDNA, type 16, was detected in one (50 percent) of the two cases of adenocarcinoma and 12/17 (71 percent) cases of squamous cell carcinoma. DNAHPV, type 33, and type 45 were each detected in 1/17 (6 percent) cases of squamous cell carcinoma. No HPVDNA, type 18, was detected (AU)

A case of the rare adenosquamous carcinoma of the stomach is presented. This unusual type of stomach tumor is the first such case to be reported in Jamaica and in the Commonwealth Caribbean. We provide evidence in support of the metaplastic derivation of the squamous component from an existing adenocarcinoma. (AU)

RESUMO

A review of 60 patients with primary small bowel tumors seen at the University Hospital, Jamaica, during the 15 year period 1971-1985, revealed that adenocarcinoma was the commonest tumour (27 percent), followed by smooth muscle tumour (23 percent), and carcinoids (11 percent). There were 32 malignant and 28 benign tumours. The mean age at presentation was 56 years, with a range of 4 to 85 years. The most common clinical presentation was intestinal obstruction, followed by pain, weight loss, abdominal mass and intussusception. In the majority of patients the diagnosis was not made preoperatively, and 80 percent with adenocarcinoma had lymph node metastases. Increased awareness of the diagnosis in symptomatic patients may result in improved survival. (AU)

RESUMO

Between the years 1958 and 1973, 119 patients with primary invasive cancer of the vulva were treated at the University Hospital of the West Indies, Jamaica. Seventy-seven (64.7 percent) were premenopausal women. Because the pattern of the disease differed from that in late middle and older aged Jamaican women, the results of the disease are presented. The analysis stresses that vulvar carcinoma before menopause in our community is rare in the absence of various sexually transmitted granulomatous diseases of the vulva (AU)

RESUMO

One hundred and thirty-four lymph nodes from forty-seven patients with renal adenocarcinoma and the same number of regional nodes from sixty-one patients with gastric ulcer were evaluated histologically by using the standardized reporting system introduced by Cottier, Turk & Sobin (1973). Special attention was focused on the morphologic features which correlate with the immunological responses of the antibody response were found to be active more often in the ulcer series than in the cancer series. On the other hand, the constituents of the cell-mediated immune system were depressed more often in the nodes draining renal carcinomas. The significance of the different immune responses evoked by these two diseases is discussed and the conclusion is drawn that there is some degree of derangement of both humorlal and cell-mediated immune systems in the regional lymph nodes of renal adenocarcinoma. The applicability of the reporting system used in assessing the morphology of human lymph nodes is to be emphasized.(AU)

RESUMO

The aetiology of carcinoma of the cervix uteri has been investigated in a retrospective interview study of patients with histologically diagnosed lesions, and suitably selected controls. Both groups were comprised of predominantly Negroes of low socio-economic status. There was significant association with early age at first coitus, early and multiple pregnancies, and promiscuity. The factors of male circumcision and sexual hygiene could not be evaluated because of unreliable information. Environmental influences seem to be largely responsible for the high incidence of the condition in Jamaica. It is hoped that, with the detection of further cases, it would be possible to compare the epidemiology in various ethnic groups and in different social classes (AU)

RESUMO

The incidence of lung cancer in the approximately one- half million population living in the parish of Kingston and St. Andrew of Jamaica- a tropical developing country- has been investigated for the years 1958-1970 inclusive. When compared- on an age standardized basis- with figures published fr om other areas, Jamaica's figures appear to be substantially above those from Africa, but much lower than those from Britain and the United States (table 12). The incidence appears to be rising both in males and females (fig.1, table 12). Clinical and pathologic data on 105 cases of lung cancer were obtained through the postmortem and surgical pathology services of the University Hospital of the West Indies, a 500-bed general hospital. The three main histologic types were equally represented at postmortem, but the squamous carcinoma type was more frequently seen in the surgical pathology material. The patients had an inadequate follow-up and were small in number, the data, therefore, did not allow an evaluation of the use of the W.H.O. histologic classification as to its prognostic significance. The duration of symptoms from onset to death or operation was gene rally less than 6 months (AU)

RESUMO

Primary adenocarcinoma of the right Fallopian tube is reported in a 53-year-old Negro woman. The diagnosis was established by histopathological examination of the surgically resected specimen, there being no suspicion of the tumour pre-operatively. Metastates occurred to the iliac lymph nodes. The patient was treated by hyterectomy and bilateral salpingo-oophorectomy followed by high voltage irradiation of the pelvis. This is the first example of primary tubal carcinoma encountered at the University Hospital of the West Indies. The relevant literature is reviewed and discussed (AU)

RESUMO

The clinical and pathological findings in 35 cases of endometrial carcinoma observed over a 13-year period (1953-1966) at the University Hospital of the West Indies are described. The study has confirmed the impression that carcinoma of the uterine body is a relatively uncommon form of malignancy in Jamaica, accounting for only 2 per cent of female malignancies and 6 per cent of uterine cancers. The clinical and pathological data are compared with those of other reported series, and found to be essentially similar. Malignant change in polyps was observed in 14 per cent of cases. Extended total hysterectomy and bilateral salpingo-oophorectomy produced the best results. Local recurrences were significantly reduced when surgery was combined with post-operative radiation. All six cases with adenoacanthoma showed myometrial invasion, and the squamous component (metaplastic and neoplastic) was associated with glands of varying degrees of differentiation (AU)
