A review of 60 patients with primary small bowel tumors seen at the University Hospital, Jamaica, during the 15 year period 1971-1985, revealed that adenocarcinoma was the commonest tumour (27 percent), followed by smooth muscle tumour (23 percent), and carcinoids (11 percent). There were 32 malignant and 28 benign tumours. The mean age at presentation was 56 years, with a range of 4 to 85 years. The most common clinical presentation was intestinal obstruction, followed by pain, weight loss, abdominal mass and intussusception. In the majority of patients the diagnosis was not made preoperatively, and 80 percent with adenocarcinoma had lymph node metastases. Increased awareness of the diagnosis in symptomatic patients may result in improved survival. (AU)

Duodenal carcinoma is a rare disease associated with a poor prognosis. Six patients with primary carcinoma of the duodenum are described. There were five men and one woman. The mean age was 73 years and the mean duration of symptoms was 10 months. Vomiting, abdominal distension and pain were the most frequent presenting symptoms. Fifty per cent had jaundice and hepatomegaly. The tumour was infra-ampullary in three patients, supra-ampullary in two and peri-ampullary in one. The mean survival was 6.5 months. Increased awareness and earlier detection are needed for improved survival (AU)

Only two cases of leimyosarcoma of the duodenum have been documented by the Jamaica Cancer Registry to date. One patient presented with obstructive jaundice which is an unusual presentation of this tumour. He died on the ninth post-operative day due to necrotising entero-colitis. The other patient presented with rectal bleeding, anaemia and an abdominal mass. The lesion was treated with pancreatico-duodenectomy and he is alive, but with recurrence up to the date of reporting. Neither of the two cases was diagnosed pre-operatively. Surgeons should be aware of this lesion while operating for upper-intestinal bleeding or on patients with obstructive jaundice (AU)

A case of post eclamptic anuria complicated by duodenal obstruction due to haemorrhage into a duodenal cyst is described. The difficulties encountered in the management of the case are discussed. A review of the literature on duodenal cysts is given.(AU)
