RESUMO

Objective: The purpose of this study was to investigate motion sensitivity in persons with pituitary adenomas in comparison to those without. Design and Methodology: A total of 9 patients were obtained to take part in the study alongside control patients of matching gender and ages. The following exam procedure was carried out at the UWI Optometry Clinic: Brief case history; Measurement of intraocular pressures using the non-contact tonometer (NCT); Measurement of refractive errors, using the Topcon Auto-Refractor; Measurement of visual acuities, using a Snellen Chart; Colour Vision testing using Ishihara Colour Plate test and Farnsworth d15 hue discrimination colour vision test; Visual Field Test using the Humphrey's Visual Field Machine; Motion sensitivity measurement using QuickRDK program and MCT program. Results: The Levene test showed that the variance for the angular motion threshold for both patient groups was not different (p = 0.163). The t-test showed no significant difference in angular motion threshold between the two groups. The Levene test showed that the variance for the coherence threshold for both patient groups was not different (p = 0.258). The t-test showed no significant difference in coherence threshold between the two groups (P=0.764) Conclusion: Motion sensitivity is not affected in persons with pituitary adenomas according to the data analysed. However, the majority of patients had microadenomas which rarely cause visual symptoms. So, it is better concluded that motion sensitivity is not affected in persons with microadenomas.

RESUMO

OBJECTIVE: To determine whether the transsphenoidal approach for pituitary tumours is advantageous over the traditional transcranial approach. DESIGN andMETHODS: Retrospective analysis of consecutive patients who underwent transsphenoidal (TS) and transcranial (TC) procedures for pituitary tumor during January 1998 to September 1999. Age, gender, presenting symptoms, preoperative findings, endocrine and imaging studies, operative procedure, complications,outcome, hospital stay, cost and patient satisfaction were recorded and analyzed. RESULTS: There was atotal of 15 cases (8 TS and 7 TC). The male to female ratio was 1:2, with an age range 10 to 71 years.All presented with visual impairment, 10 with headaches and 5 with hyperprolactinaemia. Average operative time for TS procedure was 2 hr 55 min and for TC, 2 hr 12 min. Average hospital stay for TS cases was 5.3 days whereas TC patients stayed for 5.7 days. Average cost of hospitalization (excluding fees for the surgeons and anaesthetists) for TS patients was significantly lower than for the TC patients (p<0.001). Patient satisfaction mean index for TS cases was 4.125 while it was 3.71 for TC cases. Three TS patients had excellent satisfaction. There were no mortalities. CONCLUSION: The TS approach has certain advantages compared to the TC approach in the management of pituitary tumors. TS approach is recommended in selected cases in our setting in Trinadad and Tobago. (Au)

RESUMO

Pituitary region tumours are common in Jamaica. The clinical and radiological presentation as well as the results in 20 patients treated by trans-sphenoidal surgery are presented. This overwhelmingly female group had an average age of 34, and presented mainly with amenorrhoea, headaches and visual failure, a pattern all too common in the developing world. While plain x-rays were informative, CT scanning provided the definitive diagnosis of a mass, which extended above the sella turcica in all cases. There were 6 endocrinologically active tumours: 2 patients with acromegaly and 4 with prolactin producing tumours. There was significant improvement in postoperative visual acuity and fields. Both acromegalics had normal postoperative growth hormone levels while half of the prolactin producing tumours had elevated postoperative levels. There were no postoperative deaths; no patient had worsening visual function. 1 patient developed transient and 1 permanent diabetes insipidus. The results indicate that this operation is effective and safe. (AU)

RESUMO

A statistical survey of the incidence, age and sex distribution, and preferential sites of occurrence of tumours of the central nervous system observed at the University Hospital of the West Indies during a 15-year period is presented. Four hundred and seventy-six histologically confirmed neoplasms were recorded. Intracranial tumours were 7 times as common as those of the spinal cord. Of 416 intracranial neoplasms, there were 40 percent neuropithelial tumours, 21 percent meningiomas, 15 percent pituitary adenomas, 9 percent metastatic lesions, 6 percent congenital tumours, 5 percent vasoformative tumours, 3 percent nerve sheath tumours and 1 percent unclassified neoplasms. Men and women were equally affected. Meningiomas and pituitary adenomas were more frequent than in other reported series. Metastases were relatively more frequent in the spinal than in the intracranial group. Of the primary spinal tumours, those of nerve sheath origin were nearly three times as common as the meningiomas (AU)

RESUMO

Three hundred and fifty pituitary glands were studied in an selected autopsy series. The ages of the cases ranged from newborn to 104 years. None had clinical manifestations of pituitary dysfunction. Thirty-five glands (10 percent) showed 40 microadenomas which were distributed equally among males and females, occuring most frequently in the 8th decade. The clinical and autopsy records and X-ray films of the patients with microadenomas were reviewed. Of these, 50 percent were hypertensive, 20 percent were known diabetics and 14 percent were mildly to moderately obese. Two of the eight available X-ray films showed minor abnormalities of the sella turcica. The tumours were less than 5 mm in diameter and half of them were located away from the periphery of the gland.
