RESUMO

Adamantinoma (AD) is a very rare, low-grade, malignant bone tumour that accounts for approximately 0.4% of all primary bone tumours. In 1913, Fischer named the lesion "adamantinoma" because it resembles AD of the jaw. In Contrast to those found in the jaw, adamantinomas of the long bones metastasize in 20% of the cases, usually to the lungs and nearby lymph nodes. The tumour primarily occurs in patients aged between 20-40 years and 85-90% of cases, the tibia is affected [15]. Diagnosis of adamantinoma is clinically complicated except for typical cases because it is quite similar to other benign bone lesions, such as fibrous dysplasia (FD) and osteofibro dysplasia (OFD) of long bones with respect to radiological and pathological findings. The typical location of adamantinoma is the intracortical areal of the diaphysis. Plain film have shown a well circumscribed, cortical, multilobulated osteolytic lesion with intralesional opacities, septation and peripheral sclerosis. These findings imply the lesions' nature of slow growth [6,7]. Immunohistochemical and ultrastructural evidence have shown that the neoplastic cells in AD derives from epithelial lineage. Recent reports have described another clinical entity-differentiated or OFD-like AD that appears between OFD and AD along a spectrum of disease. Features resembling other lesions such as OFD or FD may lead to misdiagnosis and result in inadequate treatment of this tumour[9].

RESUMO

Three cases of fibrosarcoma of the vulva are reported. In two, surgical resection resulted in remission over four years. In the third case, presenting with metastasis of the lung and liver, the patient died without surgery been carried out. The low mitotic index found in the first two cases seems to lead to a favourable prognosis, whereas the inferior vulvar localization found in the last case seems to be more dangerous because its diffusion into the paravaginal space

RESUMO

Fifty-two cases of choriocarcinoma were recorded in the Jamaica Cancer Registry for the parishes of Kingston and St.Andrew, giving an incidence of 1:7,384 live births. This is intermediate between figures reported for Western metropolitan populations and for Far Eastern countries. Clinicopathological studies on 26 cases treated at the University Hospital in Jamaica revealed that 65 percent followed either normal pregnancy or abortion. Choriocarcinoma following a normal pregnancy or occurring without a previous history of pregnancy carried a poorer prognosis than those with a preceding hydatidiform mole. Many patients had primary neurological manifestations. The overall prognosis was poor due to late presentation with disseminated metastases. Of the 26 patients, 15 died of the disease within two years and only six appeared for a six-year follow-up (AU)

RESUMO

Primary adenocarcinoma of the right Fallopian tube is reported in a 53-year-old Negro woman. The diagnosis was established by histopathological examination of the surgically resected specimen, there being no suspicion of the tumour pre-operatively. Metastates occurred to the iliac lymph nodes. The patient was treated by hyterectomy and bilateral salpingo-oophorectomy followed by high voltage irradiation of the pelvis. This is the first example of primary tubal carcinoma encountered at the University Hospital of the West Indies. The relevant literature is reviewed and discussed (AU)

RESUMO

A case of diffuse, malignant, pleural mesothelioma is reported in a 60-year-old Negro man. The clinical picture was most unusual; when the patient presented initially with chest pain, the radiographic appearances in the chest were normal, but extensive liver calcification was demonstrated. The latter was subsequently shown to be caused by metastases from the mesothelioma which took 2 more years to produce clinical and radiographic changes in the chest. There was microscopic evidence that the extensive liver calcification was dystrophic in nature and had resulted from marked degenerative changes and necrosis in the metastatic malignant pleural mesothelioma. Radiographs of the skeleton revealed focal, translucent lesions with sclerotic margins in the right seventh rib and the right iliac. Microscopic examination showed these bony lesions to be metastases. No previous report of secondary deposits in bone producing such radiographic appearances could be found in the literature (AU)

RESUMO

The alveolar soft-part sarcoma is a rare tumour that has recently been identified. It resembles chemodectomas and arises from primitive nerve cells. It differs from chemodectomas in its location and in its more malignant behaviour. The tumour was highly vascular and similar in appearance to chemodectomas. The author's patient complained of symptoms after an injury that was complicated by the development of myositis ossificans. The possible relationship between trauma and the development of the tumour is discussed. (AU)

RESUMO

Cancer of the breast should be treated by radical mastectomy only when there is reasonable expectation of cure. It is now possible to assess prognosis more logically if clinically operable patients are regarded in terms of the "triple biopsy", criteria for which operation are discussed. It is suggested that evidence of microscopic metastases to the apex of the axilla or to the internal mammary chain denotes inoperability and that in such instances radiotherapy has more to offer than radical surgery. The clinical experience with breast cancer as encountered at the University College Hospital of the West Indies for the past six years is reviewed and compared with that reported from other centres in America and Europe. Areas are indicated where local improvements are necessary. A strong plea is made for the urgent provision of better facilities and trained personnel for modern radiotherapy - surely Jamaica's most important medical need at this time (AU)
