RESUMO

The clinical, radiological, electrocardiographic and haemodynamic features of the 45 patients undergoing closure of atrial septal defects at the University Hospital of the West Indies from 1968 to 1978 were reviewed and found to be similar to those reported from other centres. Surgical treatment was effected with acceptable operative mortality and morbidity and good long-term functional results (AU)

RESUMO

This study reviews the clinical profile and surgical experience obtained with respect to the 52 patients who underwent surgical correction of an atrial septal defect at University Hospital of the West Indies (UHWI) between 1968-1978. The data was obtained by analysis of the patients case records. The female: male ratio was 1.6:1 and mean age 21 years (range 3 - 55 years). Pre-operative clinical data was available on 40 patients. Dyspnoea 25 patients (63 percent) and palpitations 18 patients (45 percent) were the commonest symptoms, and pulmonary systolic murmurs 40 patients (100 percent) right ventricular lift 29 patients (73 percent) and fixed splitting of S2 26 patients (65 percent) were most often found on examination. Per cent left to right shunt (35 patients) was 33-90 percent, (mean 70 percent). At operation 41 patients (79 percent) had ostium secundum defects, either isolated (32 patients) or with associated anomalies (9 patients). Seven patients (13 percent) had ostium primum defects and 3 patients (6 percent) had sinus venosus defects. The operative mortality was 10 percent (5 patients) and post-operative complications included dysrhythmias 5 patients (10 percent), respiratory failure 3 patients (6 percent) and suspected air embolism 3 patients (6 percent). Long term follow-up data was available on 34 of the 47 survivors of operation. Two patients died late (4 percent late mortality) and 31 of the remaining 32 patients were functional Class 1 when last seen. Follow-up duration 3 months - 11« years (mean 3« years). Conclusion: The above results suggest that atrial septal defects in Jamaica present clinically in the usual manner, and can be corrected surgically with acceptable mortality and good long term results (AU)

RESUMO

The clinical and pathological features of 9 cases of congenital mitral atresia are described. Six cases were associated with aortic atresia or hypoplasia and one with transposition of the great vessels. The usual clinical presentation was sudden onset of dyspnoea and cyanosis in the neonatal period followed by rapid deterioration, a picture producing diagnostic difficulties that are often increased because of coexisting cardiac anomalies. A correct anatomical diagnosis can be made at necrops by careful inspection of the heart and great vessel, aided in some cases by microscopical examination.(AU)
