RESUMO
Giant arteriovenous fistulas (AVF) pose a challenging management problem with respect to open surgery versus endovascular interventional techniques. Endovascular techniques may not be ideal for larger fistulas and may be associated with potential risks of embolism, whilst open surgery poses the risk of uncontrollable hemorrhage. We describe a safe surgical technique for this challenging problem.

One case of atherosclerotic gluteal artery aneyrysm (GAA) is presented. The diagnosis was made pre-operatively, and treatment involved ligation and division of the internal iliac artery only. The clinical, radiological, and pathological findings in case of localized aneurysm of the left subclavian artery due to cystic medionecrosis are presented. The patient was a 22-year-old Negress with no stigmata of Marfan's syndrome. The aneurysm was treated successfully by resection and insertion of a Dacron graft. The pathogenesis of the aneurysm is briefly discussed. A search of the published material has failed to reveal any such case previously reported (AU)

The value of femoral arteriography performed as an emergency procedure is illustrated by two cases. The first case was an aneurysm of the lower femoral artery which had ruptured and produced acute ischaemic changes in the affected limb. The second case of a varicose aneurysm was due to a gun-shot wound in the thigh. The aneurysm presented clinically as an abscess. Surgical excisison and repair of the aneurysm was performed in each case (AU)

An unusual case of ruptured papillary muscle of the heart is desired. The study of serial sections revealed a small aneurysm in the ruptured papillary muscle. The cause of the aneurysm was not certain but it was probably syphilitic. (Summary))
