RESUMO

The POEMS syndrome is an unusual disorder manifesting primary as peripheral sensorimotor neuropathy in association with a monoclonal gammopathy. The importance of its recognition is that significant clinical improvement may result from localization and treatment of the underlying plasma cell tumour. AU)

RESUMO

Since the development of a reliable method of assay for prolactin, it has become widely accepted that disorders of prolactin secretion constitute the most common disorder of hypothalamic/pituitary function. Ninety female patients with serm prolactin concentrations above 40ng/ml (normal 0 - 14 ng/ml) over a one-year period were reviewed. The mean age of presentation was 31 years (range 16 -54). Of the 90 patients, 60 percent were Negro, 36 percent East Indian and 4 percent mixed. The commonest presenting complaints were (86 percent), galactorrhoea (68 percent) and hirsutism (57 percent). Fourteen patients had radiological evidence of tumour; all these had prolactin levels greater than 100ng/ml (range 121 - 895ng/ml) and 60 percent had LH/FSH ratios of less than 1.00. Over the last three years, requests for prolactin assays have increased markedly. During the period June 1984 to July 1985, 1,030 assays were done of which 20 percent were above 40ng/ml. This trend reflects an increased awareness of the importance of this condition (AU)

RESUMO

Thyroid acropachy is one of the more odd manifestaions of an endocrine disorder and is the last and rarest part of a syndrome comprising hyperthyroidism, exophthalmos and pretibial myxeodema, usually arising in that order. It is estimated that it occurs in 1 percent of patients with graves' disease and Melkinson (1963) could find only 26 authentic cases in the literature. Two cases of hyperthroidism and localized myxoedema are presented. In case I the mild degree of localized myxoedema disappeared with medical treatment of the hyperthyroidism. In case II thyroid acropachy. The McKenzie mouse bioassay for the long-acting thyroid stimulator (LATS) performed 3 years post thyroidectomy showed a moderately high LATS. This is in keeping with the observation that patients who develop pretibial myxoedema frequently have very high LATS levels. In contrast there is disagreement about the possible association between LATS and the eye signs of thyrotoxicosis (AU)

RESUMO

Two cases of endocrine failure in two sisters were reported. In one, symptoms of acute depression and paranoia followed a severe episode of mumps with complete recovery after thyroid replacement therapy. In the other , symptoms of thyroid and adrenal failure, either primary or due to pituitary failure, were less dramatic but responded to approriate endocrine replacement therapy. The need for adequate facilities for the investigation of endocrine disease in Barbados was stressed, as it was estimated that 0.2 percent of mental or physical disease there is caused by thyroid disorders (AU)
