RESUMO

The Caribbean region is experiencing a demographic transition in terms of ageing of its population in both numbers and life expectancy. Thirty-five years of life have been added to the life span since 1950. This calls for adequate arrangement to meet the social, economic and health needs.(AU)

The world is experiencing an epidemiological transition; people are living longer. As a result, the population is ageing globally. There is a dilemma on how best to care for our elderly especially those 75 and over who it is thought will use up most of the medical services. This study involved 12 elderly from the nursing home/health care facility in August Town and 48 elderly living in their own home representing a total of 60 elderly person 75 years and over. A questionnaire and two observation guides were used to assess the care given in the areas of demographic condition, social, safety, physical, financial and emotional. Findings revealed that the elderly 75 years and older were very funtional especially those living in thier own home and did not need much care from their care-givers. When it was required, most of them were given care. In the nursing home, the elderly usually had children abroad who supported them. These elderly were usually more in firmed. The illnesses were the same throughout: osteoarthritis, hypertension and diabetes. One could not conclude where better care was received. Recommendations were made for further research into the subject using a wider population. Also to investigate what happens to the elderly living alone who may be mentally challenged. Policies should be adjusted so that a better financial and by extension quality of life will be attained by the elderly (AU)

The presence of chronic diseases is accompanied by the threat of complications that can be averted if the elderly can obtain the necessary health care. In 1996, the government of Jamaica launched the Jamaica Drug for the Elderly Programme (JADEP) to assist persons 60 years and over with any six chronic diseases, in the purchase of medication. The study aims to evaluate public attitude toward and participation in the programme. A cross-sectional study was conducted in randomly selected pharmacies in Kingston and St. Andrew among 94 elderly persons with certain chronic diseases and 9 providers of JADEP. All elderly persons who entered the selected pharmacy, fulfilled the criteria and who consented to be interviewed were included in the study as was the main pharmacist for that location. A focus group discussion with ten persons helped to eludiate information already obtained. The study revealed that while most elderly persons had neither a positive nor negative attitude towards the programme, the level of registration was not high (52 percent, n=94). Among JADEP participants many (67.4 percent or 29/43) were unable to obtain all their medication at one time due to cost constraints. Both providers and the elderly raised concerns about the limited range of drugs available which was the reason commonly cited for not being able to use the benefit card. The study showed that the extent to which the programme was able to assist the elderly was dependent on their awareness of how the programme functioned and the availability of their medication. The service is a necessary one and its failure would have public health implications as chronic illnesses that are not well managed will increase the health burden of the population and overall health care costs. It is recommended that the range and quantity of drugs that can be prescribed for the particular chronic conditions be re-examined.(Au)

RESUMO

This article acknowledges the increased life expectancy which Caribbean populations presently experience. It draws upon data collected throughout the region and identifies some of the main concerns which families, communities and governments need to address, in order to ensure that the elderly will not be severely disadvantaged as a consequence of their increased life expectancy. The article recommends closer cooperation between governmental agencies and non-government organizations (NGOs) in determining the needs of the elderly, as well as the provision of services for them (AU)

RESUMO

The volcano on Montserrat, after being dormant for over 400 years, has been active for the past two years, last erupting on 27 June, 1997. With the capital, Plymouth, in the unsafe zone, major dislocation of people, facilities and services has occurred. The Health Department is splintered over five sites across an eight mile span and the temporary 30 bed hospital, sited at a primary school, is separated from its Casualty and Out-patient Department and Operating Theatre by 0.25 mile. In order to maintain continuity of care for communities, efforts have been made to keep evacuated clients and their community health workers as close together as possible. The mass emigration has depleted the health services, creating severe stress for those remaining. Elderly relatives have frequently been left behind, necessitating the establishment of special geriatric care facilities to cater to their needs. Increased and continuous health surveillance and mass media education have been integral to the prevention of major disease outbreak - particularly with added challenges to food safety, and management of liquid and solid waste disposal. Cooperation from neighbouring states, particularly Antigua, Barbados and Guadeloupe, as well as from the United Kingdom, has been critical in the management of the continuing crisis.(Au)

This study investigated the need for care among the elderly living in a institution (nursing home or a home for the elderly) in Curacao. Answers were sought for the following questions: (1) what type of care do the elderly living in an institution need (2) is there a difference in needs for care between those living in a nursing home and those living in a home of the elderly? (3) is there an overlap between populations of the two? Interviews were conducted with the head nurses of both types of institutions on the need for care. The instruments used were an 8-item scale on Activities of Daily Living (ADL), a 5-item scale on Household Activities (HHA) and a 4-item scale on Memory and Orientation Disorders (MOD). Additional questions were asked about demographic characteristics, being beridden or incontinent and other physical problems. There were severe limitation for HHA (85 percent), and ADL (57 percent); 43 percent had severe MOD, 55 percent had severe problems with incontinence and 23 percent had one or more additional physical problems. With the exception of MOD, the nursing home population had more severe problems. However, almost half of the elderly in homes had a similarly extensive need for care. Despite differences in the manner in which each institute was financed and the amount of money they received as well as differences in admission criteria and educational level of the personnel, there was a great overlap between the nursing home population and those living in homes for the elderly. It is therefore necessary to (better) integrate these two forms of intramural elderly care.(AU)

RESUMO

This was a cross-sectional study of elderly persons living in the institutional, rural, and urban settings. It was conducted to determine prevalence of depression and to determine the factors which may act as protective mechanisms against depression. Elderly males and females were chosen haphazardly from senior citizens clubs in Kingston and St. Andrew. The Golden Age Home in Kingston was the institution used for the selection of elderly, using the accidental chunk method. The study revealed that the highest prevalence of depression was in the institutional setting followed by urban and then rural areas. Young elderly were at greater risk of developing depression as were those with no income. The elderly who were not depressed were more likely to have adequate levels of privacy and live with children with whom they had satisfying relationships. These findings disclosed that there is a significant level of depression among elderly, and certain members of the elderly population should be clearly monitored for signs of depression. As well, programmes need to developed to cope with the emotional and economic needs of the elderly. (AU)

RESUMO

The past two decades have witnessed the growth of the over-60,"elderly" population both in numbers and life expectancy. Similarly, there has been a shift in overall morbidity from acute to chronic causes and associated lifestyle factors. The increasing elderly population provides health care planners with new challenges at a time of decreasing resources. The public health approach, which equally stresses the responsibility of the individual, the family, the community and health care providers, is suggested as the starting point for addressing the issue. RESUMO

In the Commonwealth of Dominica, the health and disability status of 108 people who, fo various reasons, are confined to their homes -- the so-called "shut-ins" -- were studied. Eighty per cent of them were over 65 years of age and 10 percent were children. Most shut-ins were independent in self-care but dependent in general activities. Their overall health was poor. Twenty-one suffered from diseases of the musculo-skeletal system c.q. osteoarthritis. 13 from blindness of various origins, 13 from neurological diseases, 13 from CVA, psychosis or dementia and 13 from different other diseases. One-third suffered from more than one disease. A special programme would be required to optimize the home-care for the shut-ins and to encourage them to become more active (AU)

RESUMO

Developing countries are facing multiple problems in establishing and funding sociomedical services for the elderly. This study outlines the need for such services especially in the transitional stage of urbanization, changes in the structure and role of families, and the inadequate resources for establishing community-based elderly care programs. The study focuses on the analysis of human resource needs for training of the different categories of health workers, both educational degree and non-degree training for elderly care programs. It proposes an integrated approach for short-term training of physicians, nurses, and social workers. This concept was accepted and adopted by the Expert Group Meeting of the International Institute on Aging in Malta, 1989. This study also presents applications of this model in Romania, Barbados, and Kuwait. (AU)

Many elderly in both developing and develop countries are institutionalized - often irrespective of whether their ability to function requires it. Increased attention is now being given to prospects for decanting geriatric institutions and planning new forms of care. However, methodologic difficulties exist, it being hard to determine how much of the institutionalized elderly population could be effectively accommodated by alternate forms of care requiring certain levels of social, physical, and mental capacity. The procedure described in this article, based on work performed in Barbados, seeks to assess the eligibility of an existing institutionalized geriatric population for alternate type of care, thereby laying the groundwork for future planning (AU)

In this article the author examines the specialty of geriatrics, the lack of trained staff in this field and in general the 'problem of the aged' arising directly from the arithmetic of demography. The rate of occupancy of hospital beds by the elderly in Barbados and the attitude of medical and support staff to this group of people are also examined. RESUMO

Empirical data on conditions affecting elderly people in the Caribbean are very limited. To help deal with this lack of information in the specific case of Barbados, in 1982 a survey was conducted of 525 randomly selected people 65 years of age or older. This article summarises data derived from that survey in order to provide an overview of the social and economic circumstances affecting Barbados' elderly population. While it is true that these circumstances do not necessarily mirror those found elsewhere, they clearly have elements in common; and it seems likely that the survey approach applied in this instance could prove useful elsewhere in obtaining worthwhile information. RESUMO

This paper examines the implications of demographic and epidemiological aspects of aging for the care of the elderly, in the Commonwealth Caribbean. Increases in the elderly population stem from declining mortality, increasing life expectancy, declining fertility, and heavy out-migration of the younger population. The concept of caring for the elderly has three domains. The first concerns program development and actual service delivery. A second dimension includes social, economic, and psychological aspects of elderly care. The third domain deals with the process by which decisions are made about whether care will be provided institutionally, or within the family and community, or as a combination of these. Several program needs relating to the three domains are addressed. These include upgraded income maintenance, housing, nutrition, counseling, labor market participation, and wage protection. Even if these programs become reality, utilization may be adversely affected by the independence and privacy desire of the aging themselves (AU)

RESUMO

The Jamaican population aged 60 and over has grown both in absolute numbers and as a percentage of the total population in the last 60 years; future increases will even be more dramatic. This paper documents the growth of this age group since 1921 and provides projections up to 2025. Despite the various social and economic changes of recent years, the family remains the main source of support and caring for the elderly in Jamaica. Programs for the aged are included in the social security programs designed by the government to provide social and economic assistance to the population. Program activites include: food distribution, senior day care centers, and home help service providers. In addition, private and governmental pension and insurance plans provide both saving and protection for the elderly. HelpAge Jamaica, a private voluntary organization for matters concerning the elderly, was established in 1987. It is hoped that HelpAge and the various governmental organizations will work together to improve the care of the elderly in Jamaica (AU)
