RESUMO

Synopsis.In febreary 1994, a clinical trial by the Pediatrics AIDS Clinical Trial Grups (PACTG 076)demostrated, for the first time, that it was possible to reduce the risk of vertical transmission of human immunodeficiency virus type 1 (HIV-1) by nearly 70 by means of a triple therapeutic regimen: 1) oral zidovudine (ZDV)beginning anytime between the 14th and 34th week of gestation until the end of pregnancy; 2)intravenous ZDV during pregnancy, and 3)administration of oral ZDV to neonates during the first 6 weeks of life. Later, epidemiologic studies performed in the United States of America and France showed that this regimen drastically reduce perinatal transmission in clinical practice. Since then, important strides have been made, not only in terms of treatment(new drugs and highly effective therapeutic regimens) and diagnosis (tests that can measure the viral load),but also in terms of an increeased underestanding of the pathogenesis of perinatal transmission of HIV-1. This report contains: a) a review of special considerations to be kept in mind when administering antiretroviral agents to pregnant women; b) a current overview of the results of clinical and epidemiologic trials dealing with the prevention of perinatal transmission of HIV-1; c) a look at the use of test for measuring HIV-1 RNA (viral load)during pregnancy; d) the most recent recommendations on the use of antiretroviral chemoprophylaxis and elective cesarean section for reducing perinatal viral transmission. These recommendation apply to the United State, and other countries may prefer to use different approaches

RESUMO

En algunos países, los pacientes con el síndrome de inmunodeficiencia adquirida (SIDA) están usando muchas plantas medicinales cuya actividad contra el virus de la inmunodeficiencia humana (VIH) no se ha comprobado científicamente. Hoy en día, los curanderos tradicionales están poniendo sus remedios en manos de investigadores científicos y unos cuantos estudios contienen información sobre la capacidad que tienen ciertas plantas, entre ellas Viola yedoensis, Arctium lappa, Epimedium grandiflorum, Glycyrrhiza uralensis y Catanospermum australe, de inhibir el VIH. La selección de aquellos productos naturales que serán sometidos a pruebas biológicas preliminares puede hacerse al azar o aplicando criterios etnomédicos o quimiotaxonómicos (esto es, buscando compuestos similares en distintas especies de una misma familia botánica), pero si se consideran los beneficios en función del costo, la manera más ventajosa de identificar plantas eficaces contra el VIH parece ser la de elegir e investigar productos vegetales de acuerdo con indicios ya señalados en la literatura. Ningún método in vitro es ideal para determinar si una planta posee actividad contra el VIH y se necesitan pruebas confirmatorias en múltiples sistemas para investigar exhaustivamente la posible utilidad de un compuesto. Con el fin de impulsar futuras investigaciones sobre la medicina tradicional y el SIDA, se identifican algunas instituciones donde podrían llevarse a cabo las distintas actividades de evaluación científica sobre la utilidad potencial de las plantas medicinales y sus extractos en el tratamiento del SIDA

RESUMO

Many plant products are being used by patients with acquired immunodeficiency syndrome (AIDS) in some countries without any sicentific proof that they possess anti-HIV (human immunodeficiency virus) activity. Traditional healers are now offering their remedies for scientific evaluation, and a few studies provide information on the inhibitory activity against HIV of plants such as Viola yedoensis, Arctium lappa, Epimedium grandiflorum, Glycyrrhiza uralensis and Castanospermum australe


Natural products can be selected for biological screening based on ethnomedical use, random collection or a chemotaxonomic approach (i.e., screening of species of the same botanical family for similar compounds), but the follow-up and selection of plants based on literature leads would seem to be the most cost-effective way of identifying plants with anti-HIV activity. No single in vitro screening methodology for anti-HIV activity is ideal and confirmatory assays in multiple systems are needed to examine completely the potential use of a compound


To promote further research in traditional medicine and AIDS, appropriate institutions will be identified where the different activities for the scientific evaluation of plants and their extracts for possible treatment of AIDS can be carried out(AU)

RESUMO

Las enfermedades infecciosas víricas constituyen un serio problema mundial; periódicamente se identifican "nuevas" enfermedades como resultado de estudios epidemiológicos más eficaces y de mejores técnicas para el diagnóstico de virus. Todavía más importante, a medida que aprendemos a combatir el cáncer y a llevar a cabo transplantes de órganos y tejidos, el paciente inmunosuprimido es más sensible a las infecciones víricas. En la actualidad existen muy pocos agentes antivíricos de aceptación generalizada aunque las investigaciones recientes son alentadoras. En este artículo se analiza la situación de los agentes aprobados así como la de aquellos que se consideran más prometedores (AU)
