RESUMO

This framework proposes a stagewise model for evaluating the effectiveness and sustainability of behaviourally and culturally informed interventions in complex settings, with detailed guidance and accompanying tools. It presents the theoretical background, addresses the challenges of assessing causality during times of change and of influencing factors, and provides a method for measuring the unintended positive and negative effects of interventions on well-being, trust and social cohesion.

RESUMO

Background: Socializing the health system is a new approach to participatory governance that aims to promote community health and enhance sustainable health equity in the community. This approach is a part of health system reform in the Islamic Republic of Iran. Aims: This study aimed to develop a conceptual framework for socializing the health system in the Islamic Republic of Iran as a theoretical basis for this reform. Methods: Data were gathered through a review of the literature, in-depth interviews, focus group discussions, and panel discussions. Participants were experts in the field of health systems within the health ministry and medical universities. Content analysis was used to analyse the data and identify core values, principles, and approaches for socializing the health system. Results: The core values identified included human dignity, equity, ethics, and trust. The principles were participation, empowerment, human rights, and responsiveness. The approaches identified were good governance, a health-oriented focus, capacity-building, a process-oriented method, and improvement in the social determinants of health. A strong political will was considered essential for socializing the health system. Conclusions: The conceptual framework outlines requirements for socializing the Iranian health system. As this is a new approach in health system governance, however, it needs to be refined and redefined through implementation and evaluation.

RESUMO

We aimed to determine the distribution of ophthalmic care providers and its correlation with health and socioeconomic status and health system indicators. Data were gathered from the Iran Medical Council and the Iranian Societies of Ophthalmology and Optometry. Concurrent indicators were collected from the Statistical Center of Iran and national studies. A population-adjusted number of combined ophthalmologists and optometrists was used as the main dependent variable. Optometrist/ophthalmologist ratio was 0.9. We had 1 ophthalmologist and 1 optometrist for every 40 000 and 45 000 individuals, respectively. We observed a direct correlation between the number of ophthalmologists, optometrists and life expectancy at the provincial level. Gross provincial income and expenditure and provincial literacy were correlated as well. Provincial unemployment had a negative correlation. Provincial hospital statistics and population density were also significantly correlated. The Islamic Republic of Iran has met the World Health Organization's desired per capita number of ophthalmologists and optometrists, but there is wide variation in their density


Nous avons cherché à déterminer la distribution des dispensateurs de soins ophtalmologiques et sa corrélation avec la situation sanitaire et socio-économique ainsi qu'avec les indicateurs des systèmes de santé. Des données ont été rassemblées par le Conseil médical iranien et les Sociétés d'Ophtalmologie et d'Optométrie iraniennes. Des indicateurs concurrents ont été collectés auprès du Centre statistique d'Iran et à partir d'études nationales. Un nombre d'ophtalmologues et d'optométristes ajusté en fonction de la population a été utilisé comme principale variable dépendante. Le ratio optométriste/ophtalmologue était de 0,9. Il y avait un ophtalmologue et un optométriste pour 40 000 et 45 000 individus respectivement. Nous avons observé une corrélation directe entre le nombre d'ophtalmologues, d'optométristes et l'espérance de vie à l'échelle des provinces. Le revenu et les dépenses bruts en province ainsi que l'alphabétisation à ce niveau étaient également liés. Le chômage à l'échelle des provinces avait une corrélation négative. Les statistiques des hôpitaux de province et la densité de la population entretenaient une corrélation significative. La République islamique d'Iran a atteint le nombre d'ophtalmologues et d'optométristes par habitant fixé par l'Organisation mondiale de la Santé, mais il existe une importante variabilité dans leur densité

RESUMO

Настоящий документ представляет собой попытку картографического отображения социальных последствий употребления алкоголя и является одним из результатов научно-исследовательского проекта, осуществляемого международной группой исследователей. В документе рассматриваются, в частности, имеющиеся фактические данные, касающиеся влияния алкоголя на основополагающие социальные отношения – такие, как семья. Рассматривается также влияние потребления алкоголя в более широком социальном контексте с заострением внимания на взаимоотношения между употреблением алкоголя и насилием. Приводится резюме исследований, которые были проведены для выявления влияния алкоголя на экономику.

Dieses Papier stellt einen Versuch dar, die sozialen Folgen des Alkoholkonsums aufzuzeigen. Es ist das ,,Destillat“ eines von einer internationalen Forschergruppe durchgeführten Forschungsprojekts. Ein eingehenderer Bericht über diese Arbeit wurde im Namen der Weltgesundheitsorganisation, Regionalbüro für Europa veröffentlicht (KLINGEMANN, H. & GMEL, G., ED. Mapping the social consequences of alcohol consumption. Dordrecht, Kluwer Academic Publishers, 2001). Das Projekt wurde vom Regionalbüro für Europa in Auftrag gegeben, um mehr Erkenntnisse über die sozialen Folgen des Alkoholkonsums zu erhalten, als Hintergrund für die Europäische ministerielle Konferenz der WHO ,,Jugend und Alkohol“ (Stockholm, 19.–21. Februar2001). In dem Papier werden insbesondere die vorliegenden Fakten bezüglich des Einflusses des Alkoholkonsums auf die primären gesellschaftlichen Beziehungen, beispielsweise die Familie, untersucht. Die Auswirkungen in einem breiteren sozialen Kontext werden ebenfalls untersucht, wobei der Akzent auf den Zusammenhang zwischen Alkoholkonsum und Gewalttätigkeit gerichtet wird. Außerdem wird eine zusammenfassende Übersicht über die zur Abschätzung der volkswirtschaftlichen Auswirkungen des Alkoholsdurchgeführten Forschungsprojekte gegeben.

RESUMO

L’objet de ce document est de décrire les conséquences sociales de la consommation d’alcool. Il fait la synthèse des résultats d’une étude menée par un groupe international de chercheurs. Celle-ci a fait l’objet d’un rapportplus détaillé, publié pour le compte du Bureau régional de l’Europe de l’Organisation mondiale de la santé (KLINGEMANN, H. & GMEL, G., ED. Mapping the social consequences of alcohol consumption. Dordrecht, Kluwer Academic Publishers, 2001). C’est le Bureau régional de l’Europe qui a demandé la réalisation de cette étude, afin d’obtenir des données plus complètes sur les conséquences sociales de la consommation d’alcool, envue de la Conférence ministérielle européenne de l’OMS sur les jeunes et l’alcool (Stockholm, 19–21 février 2001). Ce document passe notamment en revue les données concernant les effets de la consommation d’alcool sur les relations sociales, en particulier avec la famille. Il examine également ses incidences plus larges en étudiant, entre autres, le lien qui existe entre la consommation d’alcool et la violence. Il se termine par un résumé des travaux de recherche visant à estimer les effets économiques de la consommation d’alcool.
