RESUMO

Research from the European Observatory’s Economics of Health and Active Ageing series finds overwhelmingly that population ageing is not a major problem for the sustainability of health care systems or societies. So why is it so often treated as a threat? This brief draws on a book presenting and synthesizing the international evidence on this question. It first identifies three myths that are widely influential in debates about ageing. The myths are that ageing societies are fiscally unsustainable, that older people prefer better benefits for themselves at the expense of younger people, and that politicians give older people what they want – benefits for older people at the expense of younger generations. If these myths were true, there would indeed be a major crisis in the increasing number of ageing societies. The brief and research discussed in it find that none of these three myths is true. The brief then reviews evidence on the possibility of ‘win–win’ politics that produce good outcomes for people of all ages. In terms of policy design, this means focusing on life-course policies. Life-course approaches have extensive implications for policy because they suggest ways to make policies that invest for the future at every stage of people’s lives. They also have distinctive politics because they ask for political leaders, interests and advocates to form coalitions among different groups that mutually benefit from the same policies. The brief concludes with lessons on ways to develop political coalitions in support of life-course policies.

La Revue de Santé de la Méditerranée Orientale est une revue de santé officielle publiée par le Bureau régional de l’Organisation mondiale de la Santé pour la Méditerranée orientale. This brief reviews the evidence on health and social care needs and expenditures at the end of life. End of life costs are the main reason for high per person spending levels observed at older ages in many European countries. However, there is huge variation between individuals, with estimates suggesting that just 10% of people near the end of life are responsible for over 60% of total end of life health and social care expenditures. High-cost individuals tend to be frail people with multiple chronic illnesses who use considerable hospital and social care services.In most European countries, there will be large increases in annual deaths in the coming years as the ‘baby boom’ generation ages and improved survival ultimately leads to more deaths each year. This will inevitably lead to growth in end of life needs and, ultimately, spending levels. However, there are a number of strategies available to address these challenges. Some end of life needs can be reduced with more active public health measures and active rehabilitation, much of which can be very cost-effective. More active assessment of care needs can also significantly improve the experiences of patients and families, reduce unnecessary tests and interventions, will not shorten survival, and can help to limit the growth in costs.

В нем рассматриваются основные фактические данные о расходах на здравоохранение и долгосрочный уход, связанных со старением, с тем чтобы лучше понять ожидаемое увеличение затрат в связи с изменением возрастного состава населения. В то же время в данном кратком обзоре обсуждается, каким образом пожилые люди могут вносить и вносят значимый вклад как в экономическом, так и в социальном плане, особенно если они способны оставаться здоровыми и активными в пожилом возрасте, формируя так называемую «серебряную экономику». В заключение данного обзора рассматриваются отдельные меры политики, которые продемонстрировали своюдейственность в поддержании здоровья и активного долголетия пожилых людей, либо могут иным образом укреплять устойчивые системы ухода в более широком контексте старения населения.

Мы вводим новый методологический подход для моделирования воздействия старения населения на сбор доходов в бюджет от подоходного налога (включая налоги на прибыль и прирост капитала), налогов на товары и услуги, имущественных налогов и социальных отчислений. Далее рассматриваются варианты политики, направленные на устранение дефицита поступлений, который может возникнуть в результате старения населения. Подобные связанные с мерами политики выводы актуальны как для стран с большой долей стареющего населения, так и для стран, переживающих переходный период в возрастной структуре населения. Предлагаемый краткий обзор подготовлен совместно с Центром ВОЗ по развитию здравоохранения в г. Кобе, Япония.

This policy brief in the Observatory’s Economics of Healthy and Active Ageing series explores available information on the health and disability of older people in Europe and how it relates to increases in life expectancy. It considers the main theories on health and ageing, explores the latest evidence on health and disability measures, and considers policy options to support healthy and active ageing. The policy brief argues that the health of older people is best captured by measures of disability or functional impairment. Studies using such measures have found different trends in different countries, with vast differences in the health of older people across and within countries. One overarching finding is that later cohorts of older people have much better cognitive functioning than earlier cohorts. The policy brief concludes that health systems can be important contributors to increases in life expectancies, decreases in severe disability, and better coping and functioning with chronic disease.

It reviews the main evidence on the health and long-term care costs associated with ageing populations to better understand the expected cost pressures due to changing demographics. At the same time, the brief explores how older populations can and do contribute meaningfully both in economic and societal terms, particularly if they are able to remain healthy and active into later life. The brief concludes by reviewing selected policy areas that have been shown to either support the health and activity of older people or which otherwise reinforce sustainable care systems more broadly in the context of population ageing.

This is the second brief in the Economics of Healthy and Active Ageing series. While there have been many prior analyses of how population ageing affects health and long-term care expenditure trends, very little is known about how population ageing affects a country’s ability to generate revenues to pay for health. This brief looks at how health and long-term care systems are financed and reports on the potential effects of population ageing on countries’ ability to generate sufficient and stable revenues for health from common funding sources (i.e. income taxes, consumption taxes, property taxes and social contributions) in the future. The authors simulate the effects of population ageing on countries’ ability to generate revenues, a) for countries with younger populations undergoing rapid population ageing and b) for countries that already have large populations of older adults. The analyses find that for countries where the population is comprised of a large share of relatively younger people who are likely to be active in the labour market, population ageing can have a positive impact on revenue generation from all funding sources. This leads to the challenge in many country contexts of strengthening tax collection mechanisms to take advantage of this opportunity. However, in countries with a large (and growing) older population, relying in particular on social contributions generated from the labour market to raise revenues for health is expected to result in fewer revenues per person over the coming decades. The authors consider a number of policy options to address potential revenue shortcomings as a result of population ageing.

This policy brief critically examines the economic arguments for later retirement and longer working careers in response to population ageing. It supports the view that dominant policy approaches such as increasing the statutory retirement age are often overshadowed by a lack of attention to two critical questions: firstly, are older workers in sufficiently good health to work longer? And secondly, does increasing the statutory retirement age have consequences, negative or positive, for the health of older workers? The brief argues that a more comprehensive approach is necessary to ensure longer working careers. It argues that health systems are instrumental to achieving this goal, particularly by embracing policies that support older people’s health through the workplace, and incentivizing employers to create supportive environments for older people.

The third policy brief in the Economics of Healthy and Active Ageing series looks at the long-term implications of population ageing for health spending growth. With the number and share of older people in the population increasing in nearly every country, there is much concern that this will translate into significantly higher growth in health spending in the future given that older people, on average, have higher health care costs than younger counterparts. Yet, a closer look at the evidence suggests this is unlikely to transpire, irrespective of the current and future age structure of a country. In this brief, the authors use historical data on per person health spending by age group to develop a set of projections on health care spending growth through 2060 due to population ageing for a) countries that already have a large share of the population at older ages and b) countries with relatively younger populations that are undergoing rapid population ageing. Hypothetical scenarios are also explored in recognition that health spending by age patterns might vary in the future. These scenarios are intended to approximate 1) an increase in demand and a concomitant increase in the volume of health services provided to older people; 2) an increase in the intensity of services for older people; 3) the introduction of a new high- cost medicine aimed primarily at older people; and 4) a structural change in which spending on long-term care (including health and social care) is covered by public health budgets. These analyses find that population ageing is not, and will not become, a major driver of growth in health expenditures. Moreover, they suggest that in countries where age demographics are changing but the size of the older population is not yet large, the costs of improving coverage and access to services for older people are likely to be manageable and now is a good time to begin investing in the health system while the population is relatively young. However, while population ageing will not become a main driver of health expenditure growth, policy choices related to how health services are delivered, the prices paid (or negotiated) for services, medicines and technologies, and volumes of care will ultimately determine health spending by age trends. The authors conclude by considering policy options that can be implemented to target these factors, helping to limit future growth in health expenditures within a context of population ageing.
