RESUMO

The multidrug nature of anticancer treatment and different treatment protocols used in the studies are likely to be a major limitation in establishing real risk factors determining the occurrence of dental abnormalities. The authors aimed to establish a relationship between the duration and the dose of chemotherapy and the number of tooth adverse effects in the group receiving the same treatment. Of the 40 anticancer therapy recipients who attended the outpatient dental clinic, 7 leukemia survivors receiving the treatment according to the ALL IC-BFM 2002 protocol were selected. The study group consisted of four females and three males aged 92 to 207 months at the time of dental examination and 29 to 91 months at leukemia diagnosis. As a result of the clinical and radiological examination, dental abnormalities such as agenesis, tooth size reduction, root abnormalities, and taurodontia were identified, and the medical records of all survivors were reviewed in terms of drugs administered, their doses, and treatment schedules. No correlation was observed between the treatment duration of an intensive therapy, the entire therapy, and the number of tooth abnormalities. No relationship was also found between the number of dental abnormalities and the cumulative dose of vincristine, L-asparaginase, methotrexate, cyclophosphamide, cytarabine, and 6-mercaptopurine. The age at the onset of antineoplastic therapy is likely to be the strongest risk factor for toxic injury during tooth development.

RESUMO

BACKGROUND: Given the susceptibility of developing tissues to drugs, even small doses of anticancer drugs may affect odontogenesis. Although any toxic effect is transient, the treatment regimens are based on repeated drug administration. OBJECTIVES: The study aimed to establish the impact of antineoplastic therapy on the occurrence of longterm adverse dental effects in a dose-dependent manner in young survivors treated for cancer before 10 years of age. MATERIAL AND METHODS: In total, 37 cancer survivors treated with antineoplastic therapy before 10 years of age underwent a dental examination with a thorough analysis of panoramic radiographs. A total of 236 teeth with 243 different developmental abnormalities were revealed in 28 survivors. Agenesis, tooth size reduction, taurodontia, and enamel and root abnormalities were diagnosed. All survivors received multi-agent chemotherapy, with the most frequently used drugs being vincristine (VCR), doxorubicin (DXR), cyclophosphamide (CP), ifosfamide (IF), etoposide (VP-16), carboplatin (CBDCA), cisplatin (CDDP), and actinomycin-D (ActD). A detailed analysis of medical records was also performed to assess the relationship between the treatment duration as well as the cumulative drug dose administered and the occurrence of particular disturbances. RESULTS: When analyzing the treatment duration and the drug doses in the affected and non-affected participants, there were no statistically significant differences between the survivors with different disturbances within most of the specific drug groups. In some groups, the mean cumulative treatment dose was significantly higher in the non-affected patients. According to Spearman's rho, no significant relationships were observed. CONCLUSIONS: In the present study, no significant differences in terms of treatment duration or drug doses were observed between the patients with particular abnormalities. The developmental stage of tooth formation during chemotherapy is likely the most important factor influencing dental changes. For future research with respect to different treatment protocols, an analysis of a more homogenous group of survivors is warranted.

RESUMO

Panoramic radiographs are complementary exams to evaluate oral alterations in an early manner, these changes can be dental developmental anomalies, and post-eruption dental disorder. Aim: This study evaluated the findings in panoramic radiographs and correlated the variables of gender and dental location. Methods: A retrospective study was through the observation of 1.111 panoramic radiographs from the Radiology Department in Brazil. It was included patients from 5 to 79 years of age of both gender, and it classified the anomalies in shape, size, and number and post-eruption dental changes in and correlated with gender and location. Patients with syndromes were excluded from the sample. Results: The majority of the sample was composed of fameles 752 (67.7%), as to the frequency of dental developmental anomalies related lesions 684 cases (61.6%) and post-eruption dental disorder 567 (51.8%), in the radiographs. The most prevalent change was endodontic treatment (32.6%), followed by root dilaceration (25.9%), and included tooth (19.5%). The most prevailing alteration when correlated with the gender variables was the cyst root (p<0.01) in females, and orthodontic treatment (p=0.02) in males and the variable location in the mandible was root dilaceration, giroversion, impacted tooth, taurodontia, microdontia, and endodontic treatment (p<0.01). Conclusion: Our findings provide evidence that dental developmental anomalies e post-eruption dental disorder are frequent alterations in the population with particular characteristics of distribution by sex and location

Introduction: The 2q37.3 deletion syndrome is a disorder caused by the microdeletion of a subband on chromo-some 2, whose epidemiology is approximately 150 cases worldwide, and the prevalence of 1:10000. Its charac-teristics are developmental delay, facial dysmorphia, musculoskeletal abnormalities, and increased risk of congenital diseases. The purpose is to present, for the first time in the literature, the oral and dental clinical findings associated with the syndrome. Case report: The main physical characteristics of the syndrome were identified during the physical examination: thin upper lip, prominent columella, and epicanthic folds. In the intraoral examination, dental characteristics were reported, which may be associated with the syndrome: accumulation of dental calculus, hypomineralization and hypoplasia, crossbite, open bite, agenesis, taurodontism, and dentigerous cyst. The dental treatment consisted of the application of behavior management techniques, oral hygiene and diet instruction, prophylaxis, supragingival scraping, topical fluoride application, and extraction of the lower second molar. This is the first report of oral characteristics of the 2q37.3 deletion syndrome, giving importance to the characteristics observed in this patient. Such findings will be useful in the characterization of the syndrome, which is extremely rare, and may contribute to the clinical diagnosis of these patients, in addition to subsidizing the dental treatment of other individu-als. Conclusion: We present oral clinical findings such as hypomineralization and hypoplasia, crossed and an open bite, agenesis, taurodontism, and dentigerous cyst present in the patient with the syndrome and we suggest that the involved gene or its deletion may be responsible for such findings.

RESUMO

The present case report describes an endodontic treatment of a three-rooted maxillary first molar with taurodontia and a palatal root with two separate canals. Maxillary first molar with two palatal canals is rare since extra canals are found most in the mesiobuccal root as the second mesiobuccal canal. Clinicians should have a thorough knowledge of the root canal system and internal anatomy and be alert about the possible existence of any variation in the canal morphology because it determines the successful outcome of an endodontic treatment.

RESUMO

ABSTRACT Objective To verify the prevalence of dental anomalies in terms of number, position and shape, and the presence of bone alterations, through analysis of panoramic radiographs. Methods This study evaluated dental anomalies and bone alterations in children aged 6 to 12 years-old, detected on panoramic radiographies recorded in the radiographic database of the Federal University of Espírito Santo between 2012 and 2016. Clinical and demographic information were registered. Dental records were retrieved to obtain the medical history involving syndromes or other diseases that may interfere with the anomalies and alterations. Chi-square and exact Fischer's statistical tests were performed. Results There were alterations in 83 out of 301 panoramic radiographs analyzed. There were 169 anomalies or alterations (mean of 2,04±1,38 anomalies/patient), involving 201 teeth. The most prevalent anomaly was tooth impaction (26.63%), which affected the canines more frequently, followed by hypodontia (24.26%), more often found in the second premolar, and both affected mostly females. Hyperdontia and radiolucent lesions showed the same prevalence (18.93%), the most affected teeth were those of the anterosuperior region and the first mandibular molars, respectively. The least prevalent anomalies were dilaceration (7.69%), taurodontism (2.37%) and radiopaque lesions (1.18%). Bone alterations were significantly more found in mandible and in the posterior region, which confirms the prevalence in the lower molars. Conclusion Although there was no statistically significant difference regarding sex, there was a high prevalence of anomalies and alterations in our study population, which indicates the importance of its diagnosis by means of a panoramic x-ray examination.

Introduction: The changes that occur in the process of odontogenesis are called dental anomalies and are classified according to form as twinning, fusion, concrescence, laceration, claw cusp, dens in dente, taurodontia, micro and macrodontia, the number: teeth or agenesis and supernumerary roots and the position as transmigration or ectopic. Objective: To determine the prevalence of dental anomalies radiographs records of patients seen at the clinic of a private education institution. Material and Methods: Analysis of 300 panoramic radiographs in light box illuminated by two fluorescent lamps of 20 watts each, coated with black mask, with a central opening where the measuring 15x30cm panoramic film was set, held in dark, with the aid of magnifying (2X increase) and interpreted jointly by two calibrated observers. RESUMO

The case of an 18-year-old woman with generalized short root anomaly (SRA) is presented, and the clinical management and challenges associated with this rare condition are discussed. The patient was referred for restoration of the edentulous maxillary left anterior region. Due to the SRA, the patient had previously received limited orthodontic treatment for the ectopic maxillary canines. The maxillary left canine failed during orthodontic traction, the left lateral incisor migrated in its region and had poor prognosis due to severe mobility. Therefore, it was extracted and the region was restored with an implant-supported cantilever metal-ceramic fixed partial denture. Radiographic examination revealed generalized SRA along with other dental anomalies. From the patient's family history, as well as from the physical, clinical, radiographic, biochemical, and histologic evaluations, we were unable to identify the etiology behind this unique combination of dental anomalies. Esthetics and function were successfully restored and a strict recall system was implemented in order to monitor and maintain the short-rooted teeth. The need to prevent tooth and periodontal tissue deterioration in patients with generalized short roots is emphasized.

RESUMO

AIM: This was to elucidate dental and skeletal findings in individuals with unilateral and bilateral maxillary dental transpositions. MATERIAL AND METHODS: The sample comprised of radiographic materials from 63 individuals with maxillary dental transpositions from the Departments of Odontology at the Universities of Copenhagen and Aarhus and by the Danish municipal orthodontic service. The cases were divided into three groups: unilateral transposition of the canine and first premolar (Type 1U), bilateral transposition of canine and first premolar (Type 1B), and unilateral transposition of canine and lateral incisor (Type 2). The dentitions were analysed regarding agenesis and dental morphological anomalies on panoramic radiographs, and craniofacial aspects were cephalometrically analysed on profile images The results were statistically evaluated. RESULTS: All groups demonstrated increased occurrences of agenesis (Type 1U and Type 1B: 31 agenesis in 15 patients; and Type 2 three agenesis in three patients). Taurodontic root morphology was most dominant in Type 1U. Peg-shaped lateral incisors showed an increased occurrence, though not in Type 1U. Skeletally, Type 1B and Type 1U demonstrated maxillary retrognathia (more pronounced in Type 1B). Type 2 showed a significant posterior inclination of the maxilla. CONCLUSION: Transpositions of maxillary canines involve dental and skeletal deviations. Dental deviations were predominantly taurodontic root morphology and agenesis. Regarding skeletal deviations, bilateral transpositions of the canines and the first premolars are associated with skeletal changes. Unilateral transpositions are possibly a localised deviation with minor or no skeletal involvements. The results indicate a possible difference in the aetiologies of unilateral and bilateral transpositions.

RESUMO

Taurodontism is the consequence of a developmental disorder in which the invagination of Hertwig's epithelial root sheath doesn't occur at a proper level. As a result, the pulp body and the chamber of a multi-root tooth, usually permanent molar teeth, are enlarged by the apical displacement of the pulp floor. Despite its clinically normal appearance, the morphological variation of this tooth can be diagnosed by a routine radiographic exam that shows enlarged apico-occlusal pulp chamber and short roots. Due to these anatomical variations, endodontic treatment of a taurodontic element is a clinical challenge given the complexity of localization and instrumentation of the root canal system. According to the degree of displacement of the pulp floor, taurodontism can be classified as: hypotaurodontism, mesotaurodontism and hypertaurodontism. This study objective is to report a clinical case of a patient who was submitted to endodontic treatment of the second inferior molar affected by hypertaurodontism.

.

This case series depicts dental anomalies that may develop in children who have undergone bone marrow transplantation (BMT) for acute lymphoblastic leukemia (ALL). The most common finding in these patients was root stunting; other abnormalities included microdontia, hypodontia, taurodontia, caries, enamel pearls, and pulpal calcification. Recognition of these adverse effects of BMT on odontogenesis, as demonstrated on panoramic radiograph images, will allow healthcare providers to explain to parents and patients the possible dental outcomes associated with BMT and to optimize their dental health regimen.

The hypothyroidism is an endocrine disorder characterized by the systemic character of the thyroid gland dysfunction. The T3 and T4 thyroid hormones are necessary to fulfill with the normal functions of the physical body. Thus, the alterations in the T3 and T4 secretion may be associated with changes in the stomatognathic system. Among the changes are included the condylar hypoplasia, the mandibular or maxillary atresia, the underdeveloped mandible, the hypoplasia of the enamel and the dentine, the taurodontism and the delay of dentition as well as the radicular development. Thus, it is essential that stomatologist know on the local or systemic manifestations associated with this endocrine disorder. The aim of present paper is to approach the systemic and oral changes in patients presenting with hypothyroidism, as well as the better management and care applied during the dental treatment. The data collection was thorough the selection of articles published on the oral manifestations in patients presenting with hypothyroidism in large databases like Bireme, Scielo and Medline. The hypothyroidism is a common system disease that occasionally have oral manifestations known by the stomatologists since they are patients needing special care during the stomatologic treatments. Also, must to be considered the drug interaction in these patients, since the drugs regularly prescribed in stomatology may to provoke adverse reactions. It is necessary that surgeon knows on the appropriate procedures in the treatment of hypothyroidism patients(AU)

RESUMO

The hypothesis of this study is that children with unexpected early apical resorption of the primary teeth are also predisposed to resorption in the permanent dentition. Accordingly, the aim was to perform a longitudinal study focussing on the permanent teeth in children with unexpected early apical resorption in the primary dentition. Panoramic radiographs of 12 children (7 boys and 5 girls) aged 6 years 4 months to 8 years 9 months with unexpected early apical resorption of primary teeth were identified from a dental archive of 588 patients. After written request, follow-up radiographs were obtained (2-15 year interval between early and follow-up radiographs). The radiographs were examined in order to verify the abnormal resorption pattern of the primary teeth and dental deviations in the permanent teeth, known to predispose for root resorption (i.e. invaginations, narrow crowns, abrupt root deflections, slender roots, short roots, taurodontia, agenesis, deviant pattern of eruption). Primary dentition: Two phenotypically different resorption groups were identified: group I, eight patients (resorption of the roots only), and group II, four patients (resorption of root and crown). Permanent dentition: In all 12 children, dental deviations in the permanent dentition were observed. Additionally, idiopathic external apical resorption of the permanent teeth was seen in three children, two of whom had received orthodontic treatment.

Generalized diminished root formation is a rare condition leading to early loss of teeth. This report describes an unusual case of generalized short roots associated with microdontia, taurodontism of posterior teeth, and multiple dens invaginatus along with short stature in a 20-year-old man, who had lost several teeth because of spontaneous exfoliation.

Several studies concerning dental development abnormalities have not shown accordant results. The data about prevalence, sex distribution, dental arch, side and involved tooth have been different, depending on the author, due to the specific population researched. In this report, 533 panoramic radiographs of both gender were studied, in order to establish the occurrence of the following abnormalities: a) macrodontia – prevalence: 0,75% and the most involved tooth the upper third molar; b) microdontia – prevalence: 2,81% and the most involved tooth the right upper lateral incisor; c) taurodontism – prevalence: 1,5% and the most involved tooth the lower second molar; d) fused roots – prevalence: 9,9% and the most involved tooth the upper second molar; e) anodontia – prevalence: 6,57% and the most involved tooth the lower third molar; f) supernumerary tooth – prevalence: 1,5% and the most involved area the upper molars; g) impacted tooth – prevalence: 31,52% and the most involved tooth the lower third molar; h) transposition – prevalence: 0,38%. The aim was to delineate the population concerning the researched abnormalities. The analysis of the results showed that the studied population has its own features.

RESUMO

PURPOSE: The purpose of this study was to assess, by panoramic radiographs, the prevalence of morphological dental changes in children with cancer who were submitted for chemotherapy alone or concomitant radiotherapy of the head and neck. METHODS: All patients admitted between March, 1996 and February, 2004 were analyzed and 137 were included in this retrospective, nonrandomized, institutional study. The rates of microdontia, taurodontia, anodontia, macrodontia, blunt root, and tapered root were assessed. RESULTS: The patients were distributed into 2 groups: (1) those with lymphoproliferative neoplasias (61%); and (2) those with solid tumors (39%). Their mean age when treatment began was 5 years and 6 months. Dental abnormalities were found in 39 (29%) patients, while 98 (72%) patients did not present any abnormality. The abnormalities found were: (1) microdontia (7%; N= 10); (2) anodontia (6%; N=8); (3) taurodontia (14%; N=19); (4) macrodontia (5%; N=7); (5) blunted root (2%; N=2); and (6) tapered root (4%; N=5). Of these patients: 22% (N=30) presented 1 abnormality; 4% (N=6) presented 2 abnormalities; and 2% (N=3) presented 3 abnormalities. CONCLUSION: Taurodontia was the most frequent abnormality found in children and adolescents who underwent antineoplastic treatment, and its rate was significantly higher than those found for the healthy Brazilian population. This study's results show that it is necessary for the odontologist to systematically research the dental changes that occur among this special group of patients.
