RESUMO

This theoretical essay discusses long-term care policies for dependent older adults. It aims to analyze the content and strategies that guided the formulation of the so-called "dependence policies" in some European states, seeking guidance to formulate actions related to the same issue in the Brazilian case. The knowledge bases are official documents and scientific papers analyzing the institutionalized proposals. The study shows that all the countries investigated included dependence policies within their social security system framework. Some offer total protection, while others only partial protection to older adults and family caregivers. However, older adults and their caregivers never fail to receive the care they need. In Brazil, some local experiences meet comprehensive care requirements. Initiatives of Belo Horizonte and São Paulo are narrated, and while important, they are not policies. They are successful cases that can evolve to increase social awareness or simply disappear as non-institutionalized experiences. The issue addressed in this paper is very relevant, due to the inexorable fact of the accelerated growth of the long-lived population, which requires care from others the most.


Neste ensaio teórico discutem-se políticas sobre cuidados de longa duração para pessoas idosas dependentes. O objetivo é analisar o conteúdo e as estratégias que guiaram a formulação das chamadas "políticas de dependência" em alguns estados europeus, buscando orientações para a realização de ações relativas à mesma questão no caso brasileiro. As bases de conhecimento são documentos oficiais e artigos científicos que descrevem e analisam a institucionalização das propostas. O estudo mostra que os países europeus aqui analisados incluíram as políticas sobre a dependência nos marcos de seu sistema de seguridade social; alguns oferecem proteção total, outros, apenas parcial ao idoso e ao cuidador familiar. Em nenhum deles, a pessoa idosa dependente deixa de receber os cuidados de que precisa. No Brasil, há algumas experiências locais que atendem aos requisitos de atenção integral. São narradas uma de Belo Horizonte e outra de São Paulo. Embora sejam importantes, tais iniciativas não constituem uma política, são casos exitosos que podem evoluir para o aumento da consciência social ou se esvaírem como experiências não institucionalizadas. A questão tratada neste artigo é de grande relevância, pelo fato inexorável do crescimento acelerado da população longeva, a que mais depende do cuidado de terceiros.

RESUMO

This paper presents the different stages of the editorial process of Journal Ciência & Saúde Coletiva. Different processing work methods have been overcome over these 25 years, and others have been introduced. Each stage of this construction will be analyzed in-depth to discuss the "making" of an academic publication of such a large scale and complexity. The Journal Ciência & Saúde Coletiva delves into each issue a theme in the field, addressing its cross-sectionality and complexity. This thematization ranges from 10 to 35 papers. Thirty-five papers are published monthly (thematic and free subjects), mostly in Portuguese, English, and Spanish, fulfilling the most prestigious national and international open-access databases' deadline requirements. In this making, one of the significant issues is funding because the crucial development institutions' support is very scarce.

Cada etapa dessa construção será aprofundada com o objetivo de discutir "o fazer" cotidiano de uma publicação acadêmica de tamanha envergadura e complexidade. A Revista Ciência & Saúde Coletiva aprofunda em cada edição um tema da área, tratando-o em sua transversalidade e complexidade. This is a study on scientific work on the ideation, suicide attempt and self-neglect of elderly residents in LTCEs from 2002 to 2017. Documents were retrieved from the following sources: BVS/SP, SciELO, Scopus, PubMed and Web of Science, with the following descriptors: suicide attempt, suicidal ideation, self-neglect, elderly, long-term care establishment, and their correspondents in Portuguese, Spanish and French. Twenty-six papers on the subject were found. There is a consensus among the authors, whose texts are analyzed here, concerning the factors that lead the elderly to suicidal behavior: depression, illness and pain, complicated and traumatic mourning, anxiety and despair after recovery from depressive episode, poor living conditions, death of close relatives, friends, family conflicts, family history of self-inflicted events. The protection factors found are religiosity, optimistic lifestyle, satisfaction with life and investment in the autonomy and power of relationships and communication and monitored drug therapy for mental disorders such as depression.

Resumo Neste artigo estão resumidas e analisadas 16 histórias de vida de homens e mulheres idosos que residem em nove Instituições de Longa Permanência no Estado do Rio de Janeiro e que tentaram dar cabo à vida ou têm comportamento suicida. O estudo qualitativo foi precedido por uma pesquisa do perfil de 122 idosos residentes, por meio de um formulário do qual constam: dados socioeconômicos e demográficos: nome, data de nascimento, idade, sexo, estado civil, numero de filhos vivos e mortos, nacionalidade, raça, religião, grau de instrução, profissão/ocupação, renda e tempo em que reside na instituição; e cinco perguntas que direcionaram a seleção das pessoas com ideações e tentativas de suicídio, segundo um modelo criado pelo pesquisador italiano Scocco e seu grupo. Segundo as histórias narradas pelos oito homens e oito mulheres, observam-se relevantes diferenciações por gênero tanto nos aspectos sociodemográficos como motivacionais para o comportamento suicida. Os principais fatores comuns a homens e mulheres institucionalizados em situação de risco para a morte autoinfligida são: perda de laços afetivos e de pessoas referenciais, abuso de álcool e outras drogas, inadequação à vida institucional, doenças crônicas incapacitantes e dolorosas que repercutem em solidão, desesperança e falta de sentido para a vida.


Abstract This article summarizes and analyzes 16 life histories of elderly men and women residing in nine Long Stay Institutions in the State of Rio de Janeiro and who tried to end life or have suicidal behavior. The qualitative study was preceded by a survey of the profile of 122 elderly residents, using a form that includes: socioeconomic and demographic data: name, date of birth, age, sex, marital status, number of children living and dead, nationality, race, religion, degree of education, profession / occupation, income and time in which the institution resides; And five questions that guided the selection of people with ideations and suicide attempts, according to a model created by the Italian researcher Scocco and his group. According to the stories narrated by the eight men and eight women, there are relevant gender differences in both socio-demographic and motivational aspects of suicidal behavior. The main factors common to institutionalized men and women at risk for self-inflicted death are: loss of affective and referential ties, alcohol and other drug abuse, inadequate institutional life, disabling and painful chronic diseases that lead to loneliness, hopelessness and meaninglessness to life.

The article analyses the quality and consistency of a comprehensive interview guide, adapted to study attempted suicide and its ideation among the elderly, and imparts the method followed in applying this tool. The objective is to show how the use of a semi-structured interview and the organization and data analysis set-up were tested and perfected by a network of researchers from twelve universities or research centers in Brazil, Uruguay and Colombia. The method involved application and evaluation of the tool and joint production of an instruction manual on data collection, systematization and analysis. The methodology was followed in 67 interviews with elderly people of 60 or older and in 34 interviews with health professionals in thirteen Brazilian municipalities and in Montevideo and Bogotá, allowing the consistency of the tool and the applicability of the method to be checked, during the process and at the end. The enhanced guide and the instructions for reproducing it are presented herein. The results indicate the suitability and credibility of this methodological approach, tested and certified in interdisciplinary and interinstitutional terms.

Family conflicts and problems involve meanings that are constructed during the course of an existence, and become associated with other factors in manifestations of suicidal thoughts and suicide attempts in elderly people. These questions are analyzed in a qualitative study of interviews with elderly people in four different locations in Brazil. A total of 63 men and women took part, and the interviews were held in 2013 and 2014. The field data showed the following factors - in order of the importance that the interviewees gave to them: significant family losses; family and inter-generation conflicts; and explicit and veiled violence. The speech of the subjects showed, as elements that led them to try to end their lives: sadness; feelings of abandonment; isolation, incomprehension of their desires by their family members, and absence of manifestations of affection and/or respect. When telling their stories, they also gave clues about what they expect from their families: welcome, acceptance, comprehension and freedom to carry out their minor wishes; to end their lives in a dignified manner without suffering; to find help and protection for the progressive reduction of their capacities; to continue to participate in family decisions, and to prolong to the maximum their social achievements and prerogatives, such as property, authority and respect.

This study presents a qualitative analysis of how older people who had attempted suicide began to overcome the desire and the urge to take their own life. This article is based on a survey of 87 Brazilian men and women aged 60 and over, living in different regions of Brazil, who have demonstrated suicidal behaviour; twenty of whom gave important information about their coping strategies. The analysis in this article only refers to the aforementioned twenty participants. All the participants were heard through semi-structured interviews, which included questions about the process of overcoming suicidal ideation and suicide attempts. The central focus of the analysis - overcoming suicidal ideation and suicide attempts - was based on the following concepts: coping strategies, autonomy, and emotional balance. Irrespective of the gender of the respondents, five centers of meaning emerged from the discourses of the elderly, which highlighted the effectiveness of the following factors: religiosity and religious practices; social and family support; the support of health services; contact with pets; and the recovery of the autonomy to manage their own lives. This study can help to support the primary and secondary prevention of suicidal behavior in older people.

Este estudo apresenta uma análise qualitativa sobre como idosos que tentaram suicídio passaram a superar o desejo e o impulso de tirar a própria vida. Faz parte de uma pesquisa com 87 homens e mulheres brasileiros de 60 anos ou mais, residentes em várias regiões do país e com comportamento suicida, dos quais 20 deram pistas importantes sobre suas estratégias de enfrentamento (coping). A análise aqui realizada se refere apenas a esses últimos. Todos foram ouvidos por meio de entrevistas semiestruturadas, que incluíam em seu escopo perguntas sobre o processo de superação das ideações e tentativas. O foco central da análise – superação – se apoiou nos seguintes conceitos: estratégias de enfrentamento (coping), autonomia e equilíbrio emocional. E da fala dos idosos emergiram cinco núcleos de sentido que ressaltam a efetividade dos seguintes elementos, independentemente do sexo dos entrevistados: religiosidade e práticas religiosas; apoio social e familiar; suporte dos serviços de saúde; contato com animais de estimação; e retomada da autonomia para gerir a própria vida. Este estudo pode subsidiar a prevenção primária e secundária dos comportamentos suicidas em pessoas idosas.

O artigo analisa a qualidade e a consistência de um roteiro de entrevista compreensiva, adaptado para o estudo da tentativa de suicídio e sua ideação entre pessoas idosas e apresenta o método utilizado na aplicação desse instrumento. O objetivo é demonstrar como o uso da entrevista semiestruturada e sua forma de organização e análise de dados foram testados e aperfeiçoados por uma rede de pesquisadores de doze universidades ou centros de pesquisa do Brasil, Uruguai e Colômbia. O método envolveu a aplicação do instrumento em que se socializou um manual de instruções sobre a coleta, sistematização e análise de dados. A metodologia foi aplicada em 67 entrevistas com idosos na faixa de 60 anos ou mais e em 34 entrevistas com profissionais da saúde em treze municípios brasileiros, em Montevidéu e Bogotá, permitindo a verificação da consistência do instrumento usado e a aplicabilidade do método, durante o processo e ao final. O roteiro aperfeiçoado e as instruções para replicá-lo e analisá-lo são aqui apresentados. Os resultados apontam a adequação e a credibilidade dessa abordagem metodológica testada e qualificada de modo interdisciplinar e interinstitucional.

Conflitos e problemas familiares envolvem significados construídos no decorrer da existência e se associam a outros fatores nas manifestações de pensamentos e tentativas de suicídio em pessoas idosas. Essas questões são analisadas por meio de um estudo qualitativo em que os próprios idosos foram ouvidos, em 14 localidades diferentes do Brasil. Participaram 63 homens e mulheres e as entrevistas aconteceram entre 2013 e 2014. Os dados de campo segundo as relevâncias que os idosos apresentaram foram: perdas familiares significativas, conflitos familiares e intergeracionais e violências explícitas e veladas. A fala dos idosos demonstrou a tristeza, os sentimentos de abandono, o isolamento, a incompreensão de seus desejos pelos familiares e a ausência de manifestações de afeto e respeito, como elementos que os levaram a tentar dar cabo a sua vida. Ao contarem suas histórias, eles também deram pistas do que esperam de suas famílias: acolhimento, compreensão e liberdade para realizar seus pequenos desejos; terminar a vida de forma digna e sem sofrimento; encontrar ajuda e proteção para a progressiva diminuição de suas capacidades; continuar a participar das decisões da família e prolongar ao máximo as conquistas e as prerrogativas sociais, como propriedades, autoridade e respeito.

O artigo analisa diferentes faces da depressão associadas a suicídio em idosos, a partir de autópsias psicológicas. Utilizou-se um método retrospectivo para o estudo de mortes indeterminadas, a partir de entrevistas e laudos, que analisa circunstâncias psicossociais associadas ao suicídio. O artigo traz uma predominância de gênero, classe social e raça - mulheres, classe média e alta, brancos - e contextualiza de modo inédito a depressão numa população de alto risco de suicídio, pouco estudada no Brasil e no mundo. Buscou-se reconstituir o nível de saúde física e mental, as circunstâncias sociais e os traços biográficos marcantes para compreender a magnitude da depressão no suicídio. A depressão apareceu na quase totalidade dos casos como diagnóstico primário ou secundário, como sintoma associado a outras comorbidades ou como reação a estressores sociais, demandando diferentes condutas e procedimentos terapêuticos. A associação entre depressão e comorbidade múltipla agrava o prognóstico do caso e amplia cumulativamente o risco de suicídio. A depressão acompanhada de ideações suicidas é um fator de risco que justifica medidas preventivas e imediatas. Recomenda-se ampliar as formas de diagnosticar e tratar assertivamente a depressão em idosos.

This study analyses psychological autopsies and contextualizes problems and issues that led to elderly people taking their own lives in the city of Rio de Janeiro between 2004 and 2007. The study began with an analysis of 26 expert findings of elderly men and women who committed suicide in the central, northern and southern areas of Rio de Janeiro. The sample was contacted by letter and telephone and after that, by a one-on-one conversation. Eight psychological autopsies were conducted, in which identification data and family genograms were collected followed by an interview to profile the life style and the reasons for the self-inflicted violence. The interviewees were family members, friends and acquaintances of the victims. The suicides are associated with depression, serious physical and mental illness, as well as socio-cultural factors related to professional and socio-economic decline. The suicides occurred with and without family support, with and without medical care. The cumulative fragility of personal and social resources within the life cycle reveals that the risk of suicide among the elderly demands permanent care from the public health authorities.

Esse estudo analisa autópsias psicológicas e contextualiza problemas e fatores que levaram algumas pessoas idosas a acabar com a própria vida na cidade do Rio de Janeiro, entre 2004 e 2007. O estudo parte de 26 laudos periciais de homens e mulheres que cometeram suicídios no centro, na zona norte e na zona sul da cidade. As famílias desses idosos foram contatadas por carta e telefone e depois, pessoalmente. Foram oito autópsias psicológicas, por meio das quais foram coletados dados de identificação, genograma da família e uma entrevista que reconstituiu o modo de vida e as razões da violência autoinfligida. Os entrevistados foram familiares, amigos e conhecidos das vítimas. Os suicídios estão associados a depressão, a enfermidades físicas e mentais graves e a fatores socioculturais como decadência profissional e socioeconômica. Esses eventos ocorreram com e sem apoio familiar, com e sem acompanhamento médico. A fragilização cumulativa de recursos pessoais e sociais no ciclo vital revela que o risco do suicídio em idosos exige cuidados permanentes de saúde pública.

The scope of this paper is an analysis of suicide of elderly people and the impact on the dynamics of their families is presented. The method used is of the qualitative research type known as psychosocial autopsy and is based on interviews with the family members of 51 elderly people who committed suicide in 10 Brazilian cities. The study in these cities was defined by epidemiological research that revealed the relevance of this phenomenon. Many themes were analyzed in the investigation. However, this text focuses on how the families coped with the death of the elderly person, their impressions regarding the act, and the repercussions on family members and the social network. Pursuant to a comprehensive analysis of the testimonies, the following nuclei of significance were revealed: feelings of guilt for the act; social isolation and its manifestations on health; social stigma and prejudice; prospects of overcoming family suffering; anger and feelings of the improbability of the act; and care for the family members. The families manifested suffering, sadness, and perplexity at the death of the elderly person, which influences and has repercussions on their dynamics and at an individual level. Such consequences are different in each area researched depending on the experiences the family has had with acts of this nature.

The article analyzes the quality and consistency of a semi-structured interview script, adapted for the study on suicide among elderly people, and presents the psychological and psychosocial autopsy method, which is the result of application of this instrument. The objective is to demonstrate how the in-depth interview and subsequent data organization and analyses were tested and improved by a network of researchers from eight regions in Brazil. Evaluation of the method was conducted before and after the application of the instruments to collect, systematize and analyze the data. This methodology was applied in 51 cases of elderly people who committed suicide in ten Brazilian municipalities. The study did more than just collect data with scientific rigor, since it also verified the consistency of the instrument used and the applicability of the method. The improved script and the instructions of how to apply and analyze it are thus presented here. The results reveal the rigor and credibility of this methodological approach tested and qualified by a multidisciplinary and inter-institutional procedure.

Apresenta-se uma análise sobre o impacto do suicídio de idosos, na dinâmica de suas famílias. O artigo é fruto de um tipo de pesquisa qualitativa a que se denomina autópsia psicossocial e está baseado em entrevistas com familiares de 51 idosos suicidas de 10 cidades brasileiras. O estudo nessas localidades foi definido por levantamento epidemiológico que revelou a relevância do fenômeno. Foram vários os temas analisados na investigação. Mas este texto se debruça sobre como a família enfrentou a morte da pessoa idosa, suas impressões sobre o ato e as repercussões nos seus membros e na rede social. A partir da análise compreensiva dos depoimentos, foram construídos os seguintes núcleos de sentido: culpa pelo ato, isolamento social e suas manifestações na saúde, estigma e preconceito social, sofrimento familiar e perspectivas de superação, raiva e crença na improbabilidade do ato e atenção aos familiares. As famílias manifestaram sofrimento, tristeza e perplexidade pela morte do idoso, o que influi e tem repercussões na sua dinâmica e no âmbito individual. Tais consequências são diferenciadas nos locais pesquisados e dependendo das experiências da família com atos dessa natureza.

O artigo analisa a qualidade e a consistência de um roteiro de entrevista semiestruturada, adaptado para o estudo do suicídio de pessoas idosas e apresenta o método das autópsias psicossociais que resultou da aplicação desse instrumento. O objetivo é demonstrar como o uso da entrevista em profundidade e sua forma de organização e análise de dados foram testados e aperfeiçoados por uma rede de pesquisadores de vários centros de pesquisa do Brasil. O método envolveu a aplicação do instrumento em que se socializou um manual de instruções sobre a coleta, sistematização e análise de dados. A metodologia foi aplicada no estudo de 51 casos de idosos que faleceram por suicídio em dez municípios brasileiros, e permitiu a verificação da consistência do instrumento usado e a aplicabilidade do seu método, durante o processo e ao final, por meio de uma avaliação em rede. O roteiro aperfeiçoado e as instruções para replicá-lo e analisá-lo são aqui apresentados. Os resultados apontam o rigor e a credibilidade dessa abordagem metodológica testada e qualificada de um modo interdisciplinar e interinstitucional.
