Introdução: considerando a prevalência de pacientes em terapia renal substitutiva, torna-se extremamente revelevante a avaliação da capacidade de entendimento e aplicacação das orientações que esses indíviduos recebem nos serviços de saúde. Objetivo: estudar o impacto do nível de letramento em saúde de pacientes submetidos à hemodiálise, diálise peritoneal e transplante renal, associado aos aspectos cognitivos, adesão medicamentosa e qualidade de vida. Material e Método: estudo transversal, realizado com indivíduos acima de 18 anos, submetidos a algum tipo de terapia renal substitutiva, há três meses consecutivos. Foram aplicadas as versões brasileiras dos instrumentos: Short Assessment of Health Literacy for Portuguese-speaking Adults-18, Mini Exame do Estado Mental, 36-Item Short Form e a escala de Morisky, além de um questionário sobre dados clínicos e sociodemográficos. Resultados: foram avaliados 138 pacientes, sendo 50 (36,2) submetidos à hemodiálise, 23 (16,7%) a diálise peritoneal e 65 (47,1%) ao transplante renal. A média de idade foi 52,0+/-15,5 anos e 58,0% do sexo masculino. Os pacientes com letramento inadequado (51,4%) tinham renda igual ou inferior a um salário mínimo (P=0,002) e ensino fundamental completo ou inferior (P<0,001). Os indivíduos com letramento adequado apresentavam ensino médio incompleto ou maior escolaridade, renda igual ou superior a cinco salários mínimos e maior escore no domínio dos aspectos emocionais referente a qualidade de vida (P=0,052). Conclusões: nossos resultados sugerem que o nível de letramento está associado com a renda e escolaridade nos pacientes que realizam a substituição da função renal


Introducción: Considerando la prevalencia de pacientes en tratamiento renal sustitutivo, es extremamente relevante la valoración de la capacidad de comprensión y aplicación de las orientaciones que esos individuos reciben en los servicios de salud. Objetivo: estudiar el impacto del nivel de alfabetización en salud de pacientes sometidos a hemodiálisis, diálisis peritoneal y trasplante renal, asociado a los aspectos cognitivos, adhesión a la medicación y calidad de vida. Material y Método: Estudio transversal, realizado con individuos mayores de 18 años, sometidos a algún tipo de tratamiento renal sustitutivo, durante al menos tres meses consecutivos. Fueron aplicadas las versiones brasileñas de los instrumentos: Short Assessment of Health Literacy for Portuguese-speaking Adults-18, "Mini Exame do Estado Mental, 36-Item Short Form y la Escala de Morisky, así como un cuestionario sobre datos clínicos y sociodemográficos. Resultados: se valoraron 138 pacientes, 50 (36,2%) sometidos a hemodiálisis, 23 (16,7%) a diálisis peritoneal y 65 (47,1%) trasplantados renales. La media de edad fue de 52,0+/-15,5 años y 58,0% del sexo masculino. Los pacientes con alfabetización en salud inadecuada (51,4%) tenían salario igual o inferior a un salario mínimo (P=0,002) y educación básica completa o inferior (P=0,001). Los individuos con alfabetización adecuada presentaban educación secundaria incompleta o mayor escolaridad, salario igual o superior a cinco salarios mínimos y mayor puntuación en el dominio de aspectos emocionales referentes a calidad de vida (P=0,052). Conclusiones: Nuestros resultados sugieren que el nivel de literacia en salud está asociado con el salario y escolaridad en los pacientes que realizan tratamiento renal sustitutivo


Introduction: Considering the prevalence of patients on renal replacement therapy, it is extremely revealing to evaluate the ability to understand and apply the guidelines that these individuals receive in health services. Objective: To study the impact of the health literacy level of patients on hemodialysis, peritoneal dialysis and kidney transplantation, associated with cognitive aspects, therapeutic adherence and quality of life. Material and Method: Cross-sectional study conducted with individuals over 18 years old, undergoing some type of renal replacement therapy, for three consecutive months. The Brazilian versions of the instruments were applied: Short Assessment of Health Literacy for Portuguese-speaking Adults-18, Mini Mental State Examination, 36-Item Short Form and the Morisky scale, as well as a questionnaire on clinical and sociodemographic data. Results: 138 patients were evaluated, 50 (36.2%) on hemodialysis treatment, 23 (16.7%) peritoneal dialysis and 65 (47.1%) renal transplantation. The average age was 52.0 +/- 15.5 years and 58.0% male. Patients with inadequate literacy (51.4%) had an income equal to or less than the minimum salary (P=0.002) and completed elementary school or lower (P<0.001). Individuals with adequate literacy had incomplete high school or higher education, income equal to or higher than five minimum salaries and higher score in the 'emotional aspects related to quality of life' domain (P=0.052). Conclusions: Our results suggest that the level of literacy is associated with income and education in patients undergoing renal function replacement

BACKGROUND AND OBJECTIVES: Dilated cardiomyopathy is a state of progressive enlargement of cardiac chambers mainly left ventricle which leads to decreased cardiac output and ultimately cardiac failure. Although it has multifactorial etiology, it is quite common in patients with end stage renal disease who require renal transplant surgery for their cure. Both conditions go side by side and anesthetic management of such cases poses real challenge to anesthesiologist. Strict monitoring and control of cardiac physiology is of utmost importance besides meticulous fluid management, thus preserving renal blood flow on one hand and preventing cardiac failure on other hand. This is the basis of achieving good outcome of the renal transplant surgery. METHODS: This is a retrospective observational study done by analysing electronic database of 31 patients with dilated cardiomyopathy who underwent renal transplant surgery. Data was studied in terms of demographics, duration of renal disease, comorbidities mainly hypertension, cardiac echo graphic findings including ejection fraction, medications and post-operative outcome. RESULTS: Most common perioperative complication in this patient population was hypotension (51.61%) followed by pulmonary complications postoperative mechanical ventilation (12.9%) and pulmonary edema (6.45%). High incidence of hypotension may be a causative factor to increased rate of delayed graft functioning (12.9%) and acute tubular necrosis (2.23%) in these patients. CONCLUSION: Strict monitoring and control of hemodynamic parameters as well as meticulous fluid therapy is the cornerstone in improving outcome in patients with dilated cardiomyopathy undergoing renal transplant surgery.

OBJECTIVE: to evaluate the oral conditions and the main predisposing factors for dental treatment of patients on the waiting list for liver and simultaneous pancreas-kidney transplantation, in a single center. METHODS: we evaluated 100 patients in the waiting list, 50 candidates for liver transplantation and 50 for simultaneous kidney-pancreas transplantation, from August 2015 to February 2018. We correlated extra and intraoral examinations with pre-transplant demographic variables. RESULTS: the main oral alteration in the pancreas-kidney and liver transplant candidates were decayed, lost and filled teeth, present in 83% and 100% of the candidates, respectively (p=0.03). The need for dental treatment was equal in both groups: 71% and 70%. In liver transplant candidates, the predisposing factors for dental treatment were age, color and etiological diagnosis of liver cirrhosis. We did not identify predisposing factors for dental treatment in candidates for simultaneous pancreas-kidney transplant. CONCLUSION: candidates for liver and for simultaneous pancreas-kidney transplantation had poor oral hygiene, with cavities, residual roots, gingivitis and periodontitis, revealing that dental evaluation should be part of the transplantation waiting list.


OBJETIVO: avaliar as condições bucais e os principais fatores predisponentes para tratamento odontológico de pacientes em lista de espera para transplante simultâneo de pâncreas-rim e para transplante hepático, em um centro único. MÉTODOS: foram avaliados 100 pacientes na fila de espera, 50 candidatos a transplante hepático e 50 a transplante simultâneo de pâncreas-rim, no período de agosto de 2015 a fevereiro de 2018. Exames extra e intrabucais foram correlacionados com variáveis demográficas pré-transplante. RESULTADOS: a principal alteração bucal nos candidatos a transplante de pâncreas-rim e de transplante hepático foram dentes cariados, perdidos e obturados, presentes em 83% e 100% dos candidatos, respectivamente (P=0,03). A necessidade de tratamento odontológico foi igual nos dois grupos: 71% e 70%. Nos candidatos a transplante hepático, os fatores predisponentes para tratamento odontológico foram idade, cor e diagnóstico etiológico da cirrose hepática. Não identificamos fatores predisponentes para tratamento odontológico nos candidatos a transplante simultâneo pâncreas-rim. CONCLUSÃO: pacientes candidatos a transplante simultâneo de pâncreas-rim e transplante hepático apresentaram higiene bucal precária com presença de cárie, raízes residuais, gengivite e periodontite, revelando que a avaliação odontológica deve fazer parte do protocolo de atendimento dos pacientes em fila de espera para transplantes.

Objetivo: Avaliar as características clínicas e imunológica dos receptores de transplante renal. Métodos: estudo documental e retrospectivo, realizado em um Ambulatório do Hospital Geral de Fortaleza, Fortaleza, Ceará, Brasil, com pacientes internados no período de junho de 2012 a junho de 2014. A amostra foi composta por 300 pacientes submetidos ao transplante renal. As variáveis preditoras de interesse, foram subdivididas em: características prétransplante, características pós-transplante e características imunológicas. Utilizou-se testes de Pearson e Spearman para avaliar correlação entre variáveis. Resultados: Houve predomínio de pacientes do sexo masculino (65%), com faixa etária entre 44 e 56 anos (31,4%). Demonstrou-se relação estatisticamente significante entre o DSA e a disfunção do enxerto (p<0,04), Rejeição celular o Painel Reativo classe I (p< 0,05), o tempo de internação e a disfunção do enxerto (p<0,001) e entre o entre o HLA e o MISMATCH. Conclusão: Aponta-se a necessidade de um acompanhamento crítico e individualizado do paciente transplantado por parte dos profissionais para garantir o sucesso do transplante a longo prazo


Objective: The study's purpose has been to assess both clinical and immunological characteristics of renal transplant recipients. Methods: It is a documentary and retrospective study that was performed at the renal transplantation ambulatory from the Hospital Geral de Fortaleza (HGF), Fortaleza city, Ceará State, with patients hospitalized from June 2012 to June 2014. The sample consisted of 300 patients submitted to renal transplantation. The predictive variables of interest were subdivided in the following categories: pre-transplant characteristics, post-transplant characteristics and immunological characteristics. Pearson and Spearman tests were used to evaluate the correlation between variables. Results: There was a predominance of male patients (65%), with ages ranging from 44 to 56 years (31.4%). A statistically significant relationship was found between the Donor-Specific Antibody and Delayed Graft Function (p<0.04), Cellular Rejection and PanelReactive Antibody class I (p<0.05), duration of hospitalization and Delayed Graft Function (p<0.001) and also between the Human Leukocyte Antigen and MISMATCH. Conclusion: It is pointed out the need for a critical and individualized follow-up of the transplanted patient by the professionals to guarantee the long-term transplantation success


Objetivo: Evaluar las características clínicas e inmunológicas de lós receptores de trasplante renal. Métodos: estudio documental y retrospectivo realizado en una clínica del Hospital General de Fortaleza, Fortaleza, Ceará, Brasil, con pacientes ingresados desde junio de 2012 a junio de 2014. La muestra fue de 300 pacientes sometidos a trasplante de riñón. Las variables predictoras de interés, fueron subdivididas en: características pretrasplante, características post-transplante y características inmunológicas. Se utilizaron pruebas de Pearson y Spearman para evaluar la correlación entre variables. Resultados: Hubo un predominio de pacientes del sexo masculino (65%), con edades comprendidas entre 44 y 56 años (31,4%). Se demostró una relación estadísticamente significativa entre el DSA y la disfunción del injerto (p <0,04), el rechazo celular del panel reactivo clase I (p <0,05), el tiempo de internación y la disfunción del injerto (p <0,001) y entre el HLA y el MISMATCH. Conclusión: Se apunta la necesidad de un acompañamiento crítico e individualizado del paciente trasplantado por parte de los profesionales para garantizar el éxito del trasplante a largo plazo

INTRODUCTION: The number of incident and prevalent patients on dialysis has increased, as well as the number of candidates for renal transplantation in Brazil, without a proportional increase in the number of organ donors. The use of expanded kidneys, as to renal function, may be an alternative to increase the supply of organs. OBJECTIVE: to discuss the feasibility of using expanded kidneys for renal function, which are in severe acute renal injury. METHODS: All cases of renal transplantation of deceased donors performed at the Hospital das Clínicas de Botucatu of UNESP, from January 2010 to June 2018, totaling 732 cases were evaluated. Cases with final donor creatinine greater than 6 mg/dL were selected. RESULTS: four patients were selected, of whom all donors were in severe acute kidney injury (AKI). These donors presented rhabdomyolysis as a probable cause of severe AKI, were young, with no comorbidities and had decreased urinary volume in the last 24 hours. The clinical evolution of all the recipients was satisfactory, with a glomerular filtration rate after transplantation ranging from 48 to 98 mL/min/1.73 m2. CONCLUSION: this series of cases shows the possibility of using renal donors in severe AKI, provided the following are respected: donor age, rhabdomyolysis as the cause of AKI, and implantation-favorable biopsy findings. Additional studies with better designs, larger numbers of patients and longer follow-up times are needed.

INTRODUCTION: Renal fibrosis is the end point of a process that begins at transplant, with ischemia reperfusion and early inflammation, and progresses over time with immunological and non-immunological phenomena. Early identification of morphological markers and intervention could improve graft function and survival. OBJECTIVE: to evaluate the correlation between intensity and specificity of pre-transplant anti-HLA antibodies and kidney allograft pathology in order to identify early risk factors or markers of allograft dysfunction. METHODS: A retrospective cohort of kidney transplant recipients with pre-transplant anti-HLA antibodies who underwent graft biopsy within the first two years post-transplant was divided into two groups according to the specificity of anti-HLA antibodies: nonspecific (non-DSA, n = 29) and specific (DSA+, n = 16). Kidney graft pathology, renal function, and proteinuria were analyzed. RESULTS: general characteristics were similar in both groups, except for the higher dose of thymoglobulin in DSA+ group (p < 0.05). The non-DSA group had higher scores for glomerulosclerosis, interstitial inflammation (i) and interstitial fibrosis (ci) (p < 0.05) and higher incidence of cell-mediated acute rejection. No statistical difference in incidence of antibody-mediated rejection, renal function, and proteinuria was observed during follow up. DISCUSSION AND CONCLUSIONS: the difference in inflammation scores and interstitial fibrosis may be associated to the higher incidence of acute cell-mediated rejection and polyomavirus nephropathy in the Non-DSA group. We also should take into account the protective effect of higher doses of thymoglobulin, reducing ischemia reperfusion injury in the DSA+ group. The short follow-up might have been insufficient to detect long-term changes in allograft tissue, renal function, and proteinuria.

In 2004, a global spread of Chikungunya fever affected most tropical and subtropical regions of the world. In 2016, an outbreak occurred in Northeast Brazil with hundreds of cases documented. Solid organ transplant recipients have a modified immune response to infection and the clinical course is usually different from immunocompetent patients. The diagnosis can be challenging in this population. Most reports describe patients residing in endemic areas, although we must emphasize the importance of differential diagnosis in kidney transplanted travelers who visit endemic regions, such as Northeast Brazil. Here, we reported a case of a kidney transplant recipient that acquired Chikungunya fever after a trip to an endemic region at Northeast Brazil during the outbreak in 2016, with a good clinical evolution. We also present warning recommendations for travelers to endemic areas as additional measures to prevent disease outbreaks.

Resumo Objetivo Avaliar a cultura de segurança do paciente no perioperatório de transplante renal pela equipe multidisciplinar atuante nesse cenário. Métodos Estudo transversal com 33 profissionais de saúde, que responderam ao questionário "Safety Attitudes Questionnaire", traduzido para a língua portuguesa. A coleta de dados foi em setembro de 2016 em um hospital público no oeste catarinense. Para análise dos dados utilizou-se Teste t e análise de variância ANOVA f, um valor p ≤ 0,05 foi considerado significativo. Resultados O escore médio dos domínios avaliados variou 62,4 para Condições de Trabalho, a 84,2 para Satisfação no trabalho. A percepção positiva da cultura de segurança foi evidenciada neste estudo nos domínios Satisfação do trabalho e Percepção do estresse com médias de 83,8±15 e 78,6±15, respectivamente. A associação entre os domínios e as variáveis de caracterização da equipe multidisciplinar foi significativa entre o domínio Clima de trabalho em equipe e cargo (p=0,05). As demais variáveis não apresentaram significância estatística. Conclusão Quatro dos seis domínios avaliados obtiveram escores negativos, apontando a necessidade de estratégias voltadas à cultura de segurança efetiva aos pacientes no perioperatório de transplante.

La recolección de datos fue en septiembre de 2016 en un hospital público en el oeste del estado de Santa Catarina. Para analizar los datos, se utilizó el test-T y análisis de varianza ANOVA f, un valor p ≤ 0,05 fue considerado significativo. Resultados la puntuación promedio de los dominios evaluados varió 62,4 en Condiciones de trabajo, a 84,2 en Satisfacción en el trabajo. La percepción positiva de la cultura de seguridad fue observada en este estudio en los dominios Satisfacción en el trabajo y Reconocimiento de estrés, con promedio de 83,8±15 y 78,6±15, respectivamente. La asociación entre los dominios y las variables de caracterización del equipo multidisciplinario fue significativa entre el dominio Clima de trabajo en equipo y cargo (p=0,05). Las demás variables no presentaron significación estadística. Conclusión cuatro de los seis dominios evaluados obtuvieron puntuación negativa, lo que indica la necesidad de estrategias orientadas a la cultura de seguridad efectiva para los pacientes en perioperatorio de trasplante.


Abstract Objective To evaluate the patient safety culture in the perioperative renal transplant period within the multidisciplinary team working in this setting. Methods A cross-sectional study with 33 health professionals, who responded to the "Safety Attitudes Questionnaire", translated into Portuguese. The data collection occurred in September of 2016, in a public hospital, in western Santa Catarina state. The t-test and analysis of variance (ANOVA f) was used for data analysis, and the p-value ≤ 0.05 was considered significant. Results The mean score of the domains evaluated ranged from 62.4, for working conditions, to 84.2 for job satisfaction. The positive perception of the safety culture was evidenced in this study in the areas of job satisfaction and stress recognition, with means of 83.8 ± 15 and 78.6 ± 15, respectively. The association between the domains and the characterization variables of the multidisciplinary team was significant for teamwork climate and position (p=0.05). The other variables did not present statistical significance. Conclusion Four of the six domains evaluated showed negative scores, pointing to the need for strategies aimed toward an effective safety patient culture in the perioperative transplant period.

Objetivo: avaliar o sucesso do transplante renal diante das variáveis de doador e receptor, tipo de enxerto, idade, complicações, infecções e duração do tempo de isquemia. Método: trata-se de estudo quantitativo, do tipo coorte documental retrospectivo, avaliando-se 282 prontuários de pacientes atendidos pela Unidade de Terapia Renal, com idade maior que 18 anos e os resultados apresentaram-se em forma de tabelas e figura. Resultados: mostrou-se que dos transplantes realizados, 100 foram com rins de doador vivo e 182 com rins de doador falecido, sendo que evoluíram para óbito, respectivamente, 18 e 37 pacientes, e o tempo médio de isquemia fria foi de 25,9 horas para os órgãos de doadores falecidos. Conclusão: refere-se o número maior de pacientes que desenvolveram infecções e complicações aos que receberam enxerto advindo de doador falecido, mostrando-se como um fator influenciador no sucesso do transplante renal, evidenciando que, quanto maior for o tempo de isquemia fria, maior será a perda de tecido viável do enxerto, o que acarreta maior propensão a eventos infecciosos e falha de função renal, portanto, é de extrema importância que se implementem ações e novas condutas, favorecendo, assim, bases para o sucesso dos transplantes renais.(AU)


Objective: to evaluate the success of renal transplantation in relation to donor and recipient variables, graft type, age, complications, infections and duration of ischemia. Method: this is a quantitative study of the retrospective documentary cohort, evaluating 282 medical records of patients attended by the Renal Therapy Unit, aged over 18 years and the results were presented in the form of tables and figures. Results: it was shown that of the transplants performed, 100 were with live donor kidneys and 182 with deceased donor kidneys; 18 and 37 patients died, respectively, and the mean cold ischemia time was 25.9 hours for organs of deceased donors. Conclusion: the greater number of patients who developed infections and complications of grafts from a deceased donor is shown to be an influencing factor in the success of renal transplantation, showing that the longer the cold ischemia time, the greater it will be the loss of viable tissue from the graft, which causes a greater propensity for infectious events and failure of renal function, therefore, it is extremely important to implement actions and new behaviors, thus favoring bases for the success of renal transplants.(AU)


Objetivo: evaluar el éxito del trasplante renal ante las variables de donante y receptor, tipo de injerto, edad, complicaciones, infecciones y duración del tiempo de isquemia. Método: se trata de estudio cuantitativo, del tipo cohorte documental retrospectivo, evaluando 282 prontuarios de pacientes atendidos por la Unidad de Terapia Renal, con edad mayor que 18 años y los resultados se presentaron en forma de tablas y figura. Resultados: se mostró que de los trasplantes realizados, 100 fueron con riñones de donante vivo y 182 con riñones de donante fallecido, siendo que evolucionaron a muerte, respectivamente, 18 y 37 pacientes, y el tiempo promedio de isquemia fría fue de 25,9 horas para los órganos de donantes fallecidos. Conclusión: se refiere al número mayor de pacientes que desarrollaron infecciones y complicaciones a los que recibieron injerto proveniente de donante fallecido, mostrándose como un factor influyente en el éxito del trasplante renal, evidenciando que, cuanto mayor sea el tiempo de isquemia fría, mayor es la pérdida de tejido viable del injerto, lo que acarrea mayor propensión a eventos infecciosos y fallo de función renal, por lo tanto, es de extrema importancia que se implementen acciones y nuevas conductas, favoreciendo así bases para el éxito de los trasplantes renales.(AU)

RESUMO

Objetivo: descrever a epidemiologia do transplante renal no Brasil e os gastos públicos hospitalares para o Sistema Único de Saúde entre 2013 e 2017. Metodo: trata-se de estudo quantitativo, ecológico e descritivo realizado com dados do Sistema de Informações Hospitalares. Selecionou-se as variáveis: regiões brasileiras; autorização de internação hospitalar e os custos hospitalares. Resultados: registrou-se 19.823 transplantes renais no Brasil. Verificou-se que 79,94% foi por meio de doadores falecidos e 53,01% ocorreu na região sudeste. Salienta-se que o transplante gerou impacto financeiro superior a R$ 588,3 milhões de reais. Destaca-se, ainda, que 84,55% foram por doadores falecidos e o sudeste causou maior ônus (52,48%). Conclusão: reforça-se a necessidade de capacitação das equipes de captação de órgãos para abordagens diretas e eficientes no intuito de sensibilizar as famílias para o processo de doação. Adverte-se ainda, que a educação nas séries iniciais revela-se como uma importante ferramenta para o processo de reflexão crítica e precoce acerca do processo de doação que poderá no futuro, mudar o cenário de doações de órgãos no país.(AU)


Objective: to describe the epidemiology of renal transplantation in Brazil and the hospital public spending for the Unified Health System between 2013 and 2017. Method: this is a quantitative, ecological and descriptive study conducted with data from the Hospital Information System. The following variables were selected: Brazilian regions; authorization for hospital admission and hospital costs. Results: there were 19,823 kidney transplants in Brazil. 79.94% occurred with deceased donors and 53.01% occurred in the southeast region. Transplantation generated a financial impact exceeding R$ 588.3 million. Moreover, 84.55% were deceased donors and the southeast region caused greater burden (52.48%). Conclusion: there is need for empowering teams of organ capitation for direct and efficient approaches to sensitize families to the donation process. Education in the initial grades appears as an important tool for the process of critical and early reflection on the process of donation that may change the future scenario of organ donations in the country.(AU)

También se advierte que la educación en los primeros años se revela como una herramienta importante para el proceso de reflexión crítica y los primeros pasos en el proceso de donación que pueda, en el futuro, cambiar el panorama de la donación de órganos en el país.(AU)

OBJECTIVE: considering simultaneous pancreas-kidney transplantation cases, to evaluate the financial impact of postoperative complications on hospitalization cost. METHODS: a retrospective study of hospitalization data from patients consecutively submitted to simultaneous pancreas-kidney transplantation (SPKT), from January 2008 to December 2014, at Kidney Hospital/Oswaldo Ramos Foundation (Sao Paulo, Brazil). The main studied variables were reoperation, graft pancreatectomy, death, postoperative complications (surgical, infectious, clinical, and immunological ones), and hospitalization financial data for transplantation. RESULTS: the sample was composed of 179 transplanted patients. The characteristics of donors and recipients were similar in patients with and without complications. In data analysis, 58.7% of the patients presented some postoperative complication, 21.8% required reoperation, 12.3% demanded graft pancreatectomy, and 8.4% died. The need for reoperation or graft pancreatectomy increased hospitalization cost by 53.3% and 78.57%, respectively. The presence of postoperative complications significantly increased hospitalization cost. However, the presence of death, internal hernia, acute myocardial infarction, stroke, and pancreatic graft dysfunction did not present statistical significance in hospitalization cost (in average US$ 18,516.02). CONCLUSION: considering patients who underwent SPKT, postoperative complications, reoperation, and graft pancreatectomy, as well as surgical, infectious, clinical, and immunological complications, significantly increased the mean cost of hospitalization. However, death, internal hernia, acute myocardial infarction, stroke, and pancreatic graft dysfunction did not statistically interfere in hospitalization cost.


OBJETIVO: avaliar o impacto financeiro das complicações pós-operatórias no transplante simultâneo pâncreas-rim durante a internação hospitalar. MÉTODOS: estudo retrospectivo dos dados da internação hospitalar dos pacientes submetidos consecutivamente ao transplante simultâneo pâncreas-rim no período de janeiro de 2008 a dezembro de 2014 no Hospital do Rim/Fundação Oswaldo Ramos. As principais variáveis estudadas foram a reoperação, pancreatectomia do enxerto, óbito, complicações pós-operatórias (cirúrgicas, infecciosas, clínicas e imunológicas) e os dados financeiros da internação para o transplante. RESULTADOS: a amostra foi composta de 179 pacientes transplantados. As características dos doadores e receptores foram semelhantes nos pacientes com e sem complicações. Na análise dos dados, 58,7% dos pacientes apresentaram alguma complicação pós-operatória, 21,8% necessitaram de reoperação, 12,3%, de pancreatectomia do enxerto e 8,4% evoluíram para o óbito. A necessidade de reoperação ou pancreatectomia do enxerto aumentou o custo da internação em 53,3% e 78,57%, respectivamente. A presença de complicação pós-operatória aumentou significativamente o custo. Entretanto, a presença de óbito, hérnia interna, infarto agudo do miocárdio, acidente vascular cerebral e disfunção do enxerto pancreático não apresentaram significância estatística no custo, cuja média foi de US$ 18,516.02. CONCLUSÃO: complicações pós-operatórias, reoperação e pancreatectomia do enxerto aumentaram significativamente o custo médio da internação hospitalar do SPK, assim como as complicações cirúrgicas, infecciosas, clínicas e imunológicas. No entanto, o óbito durante a internação, a hérnia interna, o infarto agudo do miocárdio, o acidente vascular cerebral e a disfunção do enxerto pancreático não interferiram estatisticamente neste custo.

The occurrence of ascites after Renal Transplant (RT) is infrequent, and may be a consequence of surgical or medical complications. Case report: 61 year-old, male, history of arterial hypertension, tongue carcinoma and alcoholic habits 12-20g/day. He had chronic kidney disease secondary to autosomal dominant polycystic kidney disease, without hepatic polycystic disease. He underwent cadaver donor RT in September 2017. He had delayed graft function by surgically corrected renal artery stenosis. He was admitted in January 2018 for ascites de novo, with no response to diuretics. HE had visible abdominal collateral circulation. Graft dysfunction, adequate tacrolinemia, Innocent urinary sediment, mild anemia, without thrombocytopenia. Serum albumin 4.0g / dL. Normal hepatic biochemistry. Peritoneal fluid with transudate characteristics and serum albumin gradient > 1.1. Ultrasound showed hepatomegaly, permeable vascular axes, without splenomegaly. Mycophenolate mofetil was suspended, with reduced remaining immunosuppression. He maintained refractory ascites: excluded infectious, metabolic, autoimmune and neoplastic etiologies. No nephrotic proteinuria and no heart failure. MRI: micronodules compatible with bile cysts. Upper Digestive Tract Endoscopy did not show gastroesophageal varicose veins. Normal abdominal lymphoscintigraphy. He underwent exploratory laparoscopy with liver biopsy: incomplete septal cirrhosis of probable vascular etiology some dilated bile ducts. He maintained progressive RT dysfunction and restarted hemodialysis. The proposed direct measurement of portal pressure was delayed by ascites resolution. There was further recovery of the graft function. Discussion: Incomplete septal cirrhosis is an uncommon cause of non-cirrhotic portal hypertension. Its definition is not well known, morphological and pathophysiological. We have not found published cases of post-RT ascites secondary to this pathology, described as possibly associated with drugs, immune alterations, infections, hypercoagulability and genetic predisposition.

Introdução: O paciente com Doença Renal Crônica (DRC) apresenta alterações respiratórias que persistem após o transplante e o restabelecimento da função renal. Objetivos: Avaliar a relação entre o tempo de transplante renal e sua influência sobre a força dos músculos respiratórios. Métodos: Foram avaliados voluntários adultos atendidos no Ambulatório de Pós-Transplante Renal do Hospital das Clínicas da Universidade Federal de Pernambuco, quanto à força muscular respiratória (manovacuometria), à função pulmonar (espirometria) e o nível de atividade física (Questionário Internacional de Atividade Física ­ IPAQ). Resultados: Foram avaliados 15 voluntários no total. No grupo <154 meses de transplante renal, 57,14% dos voluntários apresentaram fraqueza muscular inspiratória (FMI), 57,14% e 71,42% apresentaram, respectivamente, redução dos valores previstos de capacidade vital forçada (CVF%) e do volume expiratório forçado no primeiro segundo (VEF1%), e 100% foram considerados ativos. No grupo ≥154 meses de transplante renal, 50% deles apresentaram FMI, 37,5% mostraram valores de CVF% e VEF1% reduzidos e 62,5% foram classificados inativos. Todos os voluntários do estudo apresentaram fraqueza muscular expiratória. Conclusão: O grupo com menor tempo de transplante apresentou maior comprometimento da força muscular respiratória, sendo observado um nível de atividade física significativamente menor no grupo com maior tempo de realização do transplante. Não houve diferença em relação à função pulmonar entre os grupos.


Introduction: The chronic renal patient has respiratory changes that persist after transplantation, even after reestablishment of renal function. Objectives: To evaluate the relationship between renal transplantation time and its influence on the strength of the respiratory muscles. Methods: Adult volunteers were evaluated at the Outpatient Renal Transplant Clinic of the Hospital das Clínicas of the Federal University of Pernambuco, regarding respiratory muscle strength by means of manovacuometry, pulmonary function by spirometry and the level of physical activity by the International Activity Questionnaire Physics ­ IPAQ. Results: A total of 15 volunteers were evaluated. In the group <154 months of renal transplantation, 57.14% of the volunteers had inspiratory muscle weakness (IMF), 57.14% presented reduced forced vital capacity (FVC) values, 71.42% had predicted expiratory volume forced in the first second reduced (FEV1%) and 100% were considered active. In the group ≥154 months of kidney transplantation, 50% had IMF, 37.5% showed FVC% and FEV1% values reduced, and 62.5% were classified as inactive. All study volunteers had expiratory muscle weakness. Conclusion: The group with shorter transplant time presented greater impairment of respiratory muscle strength, and a significantly lower level of physical activity was observed in the group with longer transplantation time. There was no difference in lung function between groups.

Objetivo: conhecer os motivos que levam os pacientes com doença renal crônica a recusarem o transplante renal. Método: trata-se de estudo qualitativo, descritivo e exploratório realizado com 22 pacientes submetidos à hemodiálise em um Centro de Tratamento de Doenças Renais. Utilizou-se uma entrevista semiestruturada transcrita e submetida à Análise de Conteúdo na modalidade Análise Temática. Resultados: percebe-se que após a análise de conteúdo para a organização dos dados, emergiram as categorias "Sentimento de culpa por desejar a morte de alguém e preocupação com a vida do doador vivo"; "Implicações financeiras e demora na realização do transplante" e "Adaptação ao tratamento hemodialítico e as incertezas quanto ao transplante". Conclusão: constatou-se que vários motivos influenciam a recusa para o transplante renal, como questões financeiras e burocráticas, o medo da cirurgia e do seu prognóstico influenciado pelo conhecimento de experiências negativas de pacientes transplantados, o medo justificado pela adaptação ao tratamento dialítico e a algumas comorbidades, bem como a culpa por desejar um órgão. Os profissionais de saúde têm papel fundamental no esclarecimento sobre o transplante para que esses pacientes tomem uma decisão consciente acerca do tratamento.(AU)


Objective: to learn the reasons that lead patients with chronic kidney disease to refuse kidney transplantation. Method: this is a qualitative, descriptive and exploratory study of 22 patients submitted to hemodialysis at a Center for the Treatment of Renal Diseases. A semi-structured interview was used, transcribed and submitted to Content Analysis in the Thematic Analysis modality. Results: it is noticed that after the content analysis for the organization of the data, the categories "Feeling guilty for wanting someone's death and concern for the life of the living donor" emerged; "Financial implications and delay in carrying out the transplant" and "Adaptation to hemodialysis treatment and uncertainties regarding transplantation". Conclusion: it was observed that several reasons influence the refusal for renal transplantation, such as financial and bureaucratic issues, fear of surgery and its prognosis influenced by the knowledge of negative experiences of transplanted patients, fear justified by adaptation to dialysis treatment and some comorbidities, as well as the guilt for wanting an organ. Health professionals play a key role in clarifying the transplant so that these patients make a conscious decision about the treatment.(AU)

Método: se trata de un estudio cualitativo, descriptivo y exploratorio realizado con 22 pacientes sometidos a la hemodiálisis en un Centro de Tratamiento de Enfermedades Renales. Se utilizó una entrevista semiestructurada transcrita y sometida al Análisis de Contenido en la modalidad Análisis Temático. Resultados: se percibe que después del análisis de contenido para la organización de los datos, emergieron las categorías "Sentimiento de culpa por desear la muerte de alguien y preocupación por la vida del donante vivo"; "Implicaciones financieras y demora en la realización del trasplante" y "Adaptación al tratamiento hemodialítico y las incertidumbres en cuanto al trasplante". Conclusión: se constató que varios motivos influencian la negativa para el trasplante renal, como cuestiones financieras y burocráticas, el miedo a la cirugía y su pronóstico influenciado por el conocimiento de experiencias negativas de pacientes trasplantados, el miedo justificado por la adaptación al tratamiento dialítico y a algunas comorbilidades, así como la culpa por desear un órgano. Los profesionales de la salud desempeñan un papel fundamental en la aclaración del trasplante para que estos pacientes tomen una decisión consciente sobre el tratamiento.(AU)

Objective:To know the scientific production about the way of life of the renal transplant user in their home environment and the educational actions of the nurse capable of promoting quality of life. Method: An exploratory and descriptive study, which follows the steps of the integrative review, which enables the researcher to problematize the subject by grouping and systematizing what has already been produced scientifically and incorporating them into the care practice. Outcome: The total of 58 studies included in the sample. After careful reading of the abstracts, 50 studies had no adherence to the subject matter and were excluded. Therefore, 08 articles were read in full and make up the sample. Conclusion: Knowing the way of life of the renal transplant user allows the focus of care, besides being individual, to be based on human relationships


Objetivo: Conocer la producción científica sobre el modo de vivir del usuario trasplantado renal en su ambiente domiciliar y las acciones educativas del enfermero capaces de promover la calidad de vida. Método: Estudio de carácter exploratorio y descriptivo, que sigue los pasos de la revisión integrativa, que posibilita al investigador problematizar la temática por medio de la agrupación y sistematización de lo que ya fue producido científicamente e incorporarlos a la práctica asistencial. Resultado: El total de 58 estudios incluidos en la muestra. Después de la lectura cuidadosa de los resúmenes 50 estudios no tenían adherencia con la temática en cuestión y fueron excluidos. Por lo tanto, 08 artículos fueron leídos en la integración y componen la muestra. Conclusión: Conocer el modo de vivir del usuario trasplantado renal permite que el foco del cuidado, además de individual, sea basado en relaciones humanas


Objetivo: Conhecer a produção científica sobre o modo de viver do usuário transplantado renal no seu ambiente domiciliar e as ações educativas do enfermeiro capazes de promover a qualidade de vida. Método: Estudo de caráter exploratório e descritivo, que segue os passos da revisão integrativa, que possibilita ao pesquisador problematizar a temática por meio do agrupamento e sistematização do que já foi produzido cientificamente e incorporá-los à prática assistencial. Resultado: O total de 58 estudos incluídos na amostra. Após a leitura criteriosa dos resumos, 50 estudos não possuíam aderência com a temática em questão e foram excluídos. Portanto, 8 artigos foram lidos na íntegra e compõem a amostra. Conclusão: Conhecer o modo de viver do usuário transplantado renal permite que o foco do cuidado, além de individual, seja baseado em relações humanas

INTRODUCTION: Delayed graft function (DGF) is a frequent complication after deceased donor kidney transplantation with an impact on the prognosis of the transplant. Despite this, long-term impact of DGF on graft function after deceased donor kidney transplantation has not been properly evaluated. OBJECTIVE: The main objective of this study was to evaluate risk factors for DGF and the impact of its occurrence and length on graft survival and function. METHODS: A retrospective cohort study was performed in 517 kidney transplant recipients who received a deceased donor organ between January 2008 and December 2013. RESULTS: The incidence of DGF was 69.3% and it was independently associated with donor's final serum creatinine and age, cold ischemia time, use of antibody induction therapy and recipient's diabetes mellitus. The occurrence of DGF was also associated with a higher incidence of Banff ≥ 1A grade acute rejection (P = 0.017), lower graft function up to six years after transplantation and lower death-censored graft survival at 1 and 5 years (P < 0.05). DGF period longer than 14 days was associated with higher incidence of death-censored graft loss (P = 0.038) and poorer graft function (P < 0.001). No differences were found in patient survival. CONCLUSIONS: The occurrence of DGF has a long-lasting detrimental impact on graft function and survival and this impact is even more pronounced when DGF lasts longer than two weeks.

INTRODUCTION: The prediction of post transplantation outcomes is clinically important and involves several problems. The current prediction models based on standard statistics are very complex, difficult to validate and do not provide accurate prediction. Machine learning, a statistical technique that allows the computer to make future predictions using previous experiences, is beginning to be used in order to solve these issues. In the field of kidney transplantation, computational forecasting use has been reported in prediction of chronic allograft rejection, delayed graft function, and graft survival. This paper describes machine learning principles and steps to make a prediction and performs a brief analysis of the most recent applications of its application in literature. DISCUSSION: There is compelling evidence that machine learning approaches based on donor and recipient data are better in providing improved prognosis of graft outcomes than traditional analysis. The immediate expectations that emerge from this new prediction modelling technique are that it will generate better clinical decisions based on dynamic and local practice data and optimize organ allocation as well as post transplantation care management. Despite the promising results, there is no substantial number of studies yet to determine feasibility of its application in a clinical setting. CONCLUSION: The way we deal with storage data in electronic health records will radically change in the coming years and machine learning will be part of clinical daily routine, whether to predict clinical outcomes or suggest diagnosis based on institutional experience.

INTRODUCTION: The development of novel non-invasive biomarkers of kidney graft dysfunction, especially in the course of the delayed graft function period would be an important step forward in the clinical practice of kidney transplantation. METHODS: We evaluated by RT-PCR the expression of miRNA-146 to -5p ribonucleic micro-acids (miRNAs) in the peripheral blood and renal tissue obtained from kidney transplant recipients who underwent a surveillance graft biopsy during the period of delayed graft function. RESULTS: In biopsy samples, the expression of miR-146a-5p was significantly increased in the group of patients with delayed graft function (DGF) (n = 33) versus stables patients (STA) (n = 13) and patients with acute rejection (AR) (n = 9) (p = 0.008). In peripheral blood samples, a non-significant increase of miR-146a-5p expression was found in the DGF group versus STA and AR groups (p = 0.083). No significant correlation was found between levels of expression in biopsy and plasma. ROC curve analysis revealed an AUC of 0.75 (95% CI: 0.62-0.88) for the renal tissue expression and 0.67 (95% CI 0.52-0.81) for the peripheral blood expression. CONCLUSION: We conclude that miR-146a-5p expression has a distinct pattern in the renal tissue and perhaps in the peripheral blood in the setting of DGF. Further refinements and strategies for studies should be developed in the field of non-invasive molecular diagnosis of kidney graft dysfunction.
