Interações e associações medicamentosas no tratamento da hipertensão – Bloqueadores alfa-adrenérgicos e vasodilatadores diretos / Pharmacological interactions and associations in hypertension treatment –Alpha-blockers and direct vasodilators

RESUMO

Com o envelhecimento da população, a ocorrência cada vez mais frequente de doenças crônicas e a coexistênciade outras entidades clínicas, torna-se fundamental, na prática médica, o conhecimento acerca das interações medicamentosas no manejo da hipertensão arterial. No conceito moderno do tratamento anti-hipertensivo, o uso de múltiplos fármacos é mandatório, pois permite obter efeito sinérgico e/ou aditivo dos diversos medicamentos, aumentando as chances de sucesso no alcance das metas pressóricas propostas pelas principais sociedades médicas. Entretanto, essa estratégia aumenta também o potencial de interações medicamentosas deletérias no tratamento dessa doença. A evolução das pesquisas médicas e a descoberta de novos agentes anti-hipertensivos mais eficazes e com menor potencial de efeitos colaterais deixaram os vasodilatadores diretos e os bloqueadores alfa-adrenérgicos reservados para situações especiais, tornando escassos os ensaios clínicos com esses fármacos, o que dificulta o estudo das interações medicamentosas a eles relacionadas. Os bloqueadores alfa-adrenérgicos possuem efeito anti-hipertensivo em monoterapia e há sinergismo em sua interação com outras classes de anti-hipertensivos. Entretanto, no estudo ALLHAT, a monoterapia com doxazosina resultou em excesso de eventos cardiovasculares neste grupo, levando à restrição da recomendação desses fármacos no tratamento da hipertensão arterial pelas principais sociedades médicas mundiais. ABSTRACT

As the population ages, the frequency of chronic diseases constantly increases and clinical conditions are often present together, the knowledge about drug interaction in the management of hypertension is very important for medical practice. In the modern concept of antihypertensive treatment, the use of multiple drugs is recommended because it offers a synergistic and/or aggregate effect of several drugs, increasing the chances for success in reaching the goals proposed by the most important medical societies. However, this strategy also augments the potential for harmful drug interaction in the treatment of this disease.The evolution of medical research, as well as the discovery of more effective new antihypertensive agents with fewer potential adverse effects have turned direct vasodilators and alpha-adrenergic blockers into drugs recommended for special situations. Therefore, there are few clinical trials with these drugs and even fewer studies about drug interaction related to them. The alpha-adrenergic blockers have hypotensive effect in monotherapy and there is synergism with other classes of antihypertensive drugs. Nevertheless, in the ALLHAT study, monotherapy with doxazosin resulted in an excess of cardiovascular events among patients treated with this dug. This has led several medical societies in the world to restrict recommendation of these drugs for the treatment of hypertension. A strategy commonly used to minimize vasodilators adverse events and also to potentiate their hypotensive effect is the association with diuretics and beta-blockers.
