At the end of the consultation and the application of the scales, a nursing care plan was designed to contemplate the evidenced health deficits obtained, a plan that aimed to contribute to maintaining and recovering the functional capacity of the elderly person. The nurses completed the consultation, identified the Diagnoses and applied a Nursing care plan in the home visit, contributing significantly to the maintenance of the overall functionality of the elderly person, starting from the first of which the prevention, promotion and rehabilitation of the health are the preponderant factors for the autonomy and independence of the elderly person and consequently for a healthy aging
