Experiencia de los pediatras con los dilemas éticos y la consulta ética en la práctica médica en Argentina / Experience of pediatricians with ethical dilemmas and ethical consultation in medical practice in Argentina

RESUMEN

El objetivo del presente estudio es explorar las experiencias de pediatras que pertenecen a la Sociedad Argentina de Pediatría (SAP) en relación a los dilemas éticos (DE) que se presentan en su actividad asistencial, la accesibilidad a un comité de ética (CE) y la utilidad que éste pudo brindar a los pediatras. Se desarrolló un cuestionario que se incorporó al portal de Internet de la SAP. Se incluyeron preguntas referidas a la actividad profesional de los encuestados, los dilemas éticos que se presentaron en su actividad asistencial, la accesibilidad a la consulta a un CE, y si la ayuda recibida fue útil. Para el análisis de los resultados se empleó estadística descriptiva. Para comparar entre pediatras que consultaron o no a un CE se usó test de Chi cuadrado o test de t y el valor significativo fue <0.05. Se usó el paquete estadístico STATA 9.1. Respondieron al cuestionario 450 pediatras de 21 provincias argentinas; promedio de edad 49.5 años y 22.8 años de actividad profesional; el 47.5% trabajaba en hospital y consultorio; 78% lo hacía en hospitales públicos; 75% admitió no tener suficientes conocimientos para resolver DE que se presentaron en su actividad asistencial; 48% tenía acceso a la consulta ética y 44% tenía CE en el lugar de trabajo pero el 60% nunca consultó a un CE. Menos del 10% de los pediatras nunca vivenció un DE en su tarea asistencial. Los DE más frecuentes fueron desacuerdos entre el paciente y su familia y el médico en relación con el tratamiento propuesto, cuestiones de limitación de soporte vital y disponibilidad de recursos. El 76% de los médicos que consultaron a un CE consideraron útil la ayuda recibida. Como conclusión del presente trabajo, se observó que los DE son muy frecuentes en la práctica pediátrica asistencial; aproximadamente la mitad de los encuestados tiene acceso a un CE y el 76% de los pediatras que consultaron a un CE admitieron que la ayuda recibida fue útil.(AU)

ABSTRACT

The aim of the present study is to explore experiences of pediatricians who are members of Sociedad Argentina de Pediatría (SAP) [Argentinian Society of Paediatrics] as regards ethical dilemmas (ED) which present to them in their work activities, the accessibility to a committee of ethics (CE) and the utility that this latter may have to pediatricians. We developed a questionnaire that was posted at SAP Internet portal. We included questions referring to the professional activity of the interrogated persons, the ethical dilemmas that appear in their work activity, the accessibility to consultation to a CE and question on whether the received help was useful. For the analysis of the results a descriptive statistic model was used. To compare pediatricians who consulted or not a CE we used T-test and the significant value was <0.05. We used the statistical program STATA 9.1. 450 pediatricians of 21 Argentine provinces answered the questionnaire; their average age was 49.5 years, and the average time of profesional activity was 22.8 years; 47.5% were employed at hospitals and had an office; 78% worked in public hospitals; 75% admitted not having enough knowledge to resolve ED that they had to face in their work activity; 48% had access to ethical consultation and 44% had a CE at the work place but 60% never consulted a CE. Less than 10 % from the pediatricians had never had to face an ED in their work activities. The more frequent ED were disagreements between the patient and family and the doctor as regards the proposed treatment, questions of limitation of vital support and availability of resources. 76% of the doctors who consulted a CE considered the received help to be useful. As a conclusion of the present work, we observed that ED are very frequent in pediatric help practice; about half of the interrogated pediatricians has access to a CE and 76 % of those who consulted a CE admitted that the received help was useful.(AU)
