RESUMEN
Latenció als infants amb sibilacions és una part important del treball assistencial dels pediatres datenció primària, urgències i especialistes en allèrgia i pneumologia. La resposta a la infecció vírica és individual per a cada infant, a causa de la diversitat genètica i ambiental, i depèn de la funció pulmonar i la hiperresposta bronquial preexistents i del tipus dexpressió cellular Th1/Th2. En aquest article es revisa el concepte dhiperresposta bronquial i la relació que té amb la inflamació asmàtica. També es planteja la controvèrsia entre les justificacions teòriques i les situacions reals del tractament de la bronquiolitis, les sibilacions recurrents i lasma. Sexposa la nova classificació de gravetat de lasma crònica adoptada per les societats espanyoles dallèrgia i pneumologia pediàtriques. I, finalment, es presenta el monitoratge de la resposta al tractament mitjançant lestudi de marcadors com lòxid nítric en laire exhalat, i sen revisa la relació amb els distints fenotips dasma greu observats després de lestudi de la funció pulmonar, la hiperresposta i la inflamació bronquial (AU)
ABSTRACT
The care for children with wheezing is a very important component of the work of general pediatricians, emergency physicians, and allergy and pulmonology specialists. Responses to viral infections are individualized as a result of genetic and environmental diversity; and they are also dependent on the pre-existing pulmonary function and airway hyper-reactivity status, as well as on the type of Th1/Th2 cellular expression. In this article, the concept of airway hyper-reactivity, and its relation with asthmatic inflammation are reviewed. Also, the controversies between theoretical and empirical rationales for the management of bronchiolitis, recurrent wheezing, and asthma, are considered. The new classification of severity of chronic asthma adopted by the Spanish societies of pediatric allergy and pulmonology is also discussed. Finally, we discuss the monitoring of response by means of the study of markers, such as exhaled nitric oxide, and in particular its relationship with the different phenotypes of severe asthma, airway hyper-reactivity, and bronchial inflammation (AU)
